Superior vena cava (SVC) syndrome was originally described as being secondary to an infection. Currently, it is almost exclusively secondary to malignancy. A case of SVC syndrome presenting with dyspnea, facial swelling, neck distension and cough developed over a period of 10 days is reported. The approach included imaging studies and tissue diagnosis. Computed tomography scan of the chest revealed a lobulated mass on the right upper chest invading the mediastinum, and cytology obtained from bronchoscopy revealed squamous cell carcinoma. The etiology, diagnosis and treatment modalities of the SVC syndrome are discussed.  相似文献

A 60-year-old man with polyarteritis nodosa under treatment presented with syncope. Echocardiography demonstrated hypertrophic obstructive cardiomyopathy; coronary arteriography revealed normal findings, and Holter monitor showed episodes of non-sustained ventricular tachycardia. This is the first report of hypertrophic obstructive cardiomyopathy developing in a patient with polyarteritis nodosa. BACKGROUND: Hospital emergency departments (EDs) serve an aging population with an increased burden on health resources. Few studies have examined the effects of comprehensive geriatric assessment interventions on ED use. This study aimed to systematically review the literature and compare the effects of these interventions on ED visits. METHODS: Relevant articles were identified through electronic databases and a search of reference lists and personal files. Inclusion criteria included: original research (written in English or French) on interventions conducted in noninstitutionalized populations 60 years old or older, not restricted to a particular medical condition, in which ED visits were a study outcome. Data were abstracted and checked by the first author and a research assistant using a standard protocol. RESULTS: Twenty-six relevant studies were identified, reported in 28 articles, with study samples obtained from EDs (9), hospitals (4), outpatient or primary care settings (10), home care (4), and community (1). The study designs included 17 randomized controlled trials, 3 trials with nonrandom allocation, 4 before-after studies, 1 quasi-experimental time-series study, and 1 cross-sectional study. Hospital-based interventions (mostly short-term assessment and/or liaison) had little overall effect on ED utilization, whereas many interventions in outpatient and/or primary care or home care settings (including geriatric assessment and management and case management) reduced ED utilization. Heterogeneity in study methods, measures of comorbidity, functional status, and ED utilization precluded meta-analysis of the results. CONCLUSION: Further research, using improved methodologies and standardized measures, is needed to address the effects of innovative geriatric interventions on ED visits.  相似文献

There is little agreement on the real prevalence and significance of varices of the lower limbs. The discrepancies between the data produced by the various studies are attributable to several factors, of which the most important has to be the absence of any definition of the forms and types of varices. In the Basle study there has been an attempt to classify into two groups, "disorder" and "disease", according to the severity of the clinical picture. These two groups are clearly distinct regarding complications and the number of risk factors. A study on the long-term development has also shown that at the end of 11 years the group "disease" more frequently presented socio-medical after-effects than the group "disorder". Pronounced varices therefore do constitute a real disease and merit appropriate attention.  相似文献

The aortic pulse contour displays characteristic changes associated with the progression of aortic stenosis (AS). A diminished and delayed aortic pulse contour is indicative of significant AS, but the aortic contour may also be affected by factors including aging, hypertension and increased peripheral arterial elastance. After the successful introduction of highly active antiretroviral agents the survival of patients infected with the human immunodeficiency virus (HIV) in developed countries has increased substantially. This has allowed the surfacing of several chronic diseases among which cardiovascular disease (CVD) is prominent. The pathogenesis of CVD in HIV is complex and involves a combination of traditional and HIV related factors. An accurate assessment of risk of CVD in these patients is still elusive and as a consequence the most appropriate preventive and therapeutic interventions remain controversial.  相似文献
