Abstract –  Root fractures of the posterior teeth are rare and occur as a result of severe trauma. This study describes the horizontal root fracture of a maxillary first premolar. The fractured roots were discovered during a routine radiographic examination. The tooth was asymptomatic and responded positively to electric pulp testing. The patient reported accidental trauma, which occurred 14 years before. Our case is an example of spontaneously healed fractured roots. The interesting findings were that the healing was observed even in the presence of two roots, including preservation of the vitality of the pulp.  相似文献

Abstract  – Radicular fractures in permanent teeth are uncommon injuries among dental traumas, comprising 0.5–7% of the cases. Fracture occurs most often in the middle-third of the root and rarely at the apical-third. The present paper reports a clinical case of a horizontal radicular fracture located between the middle- and apical-third of a upper left-central incisor followed-up for over 3 years. The tooth was extracted owing to periodontal reasons. Histomorphologically, it showed pulp-vitality preservation and root healing by hard-tissue deposition.  相似文献

Abstract  – We report a clinical case of horizontal root fracture in the mandibular first premolar induced by a traffic accident. The fractured tooth spontaneously healed and diagnosed radiographically after 2 years. Fractured fragments were separated by a narrow radiolucent line and the edges were rounded, and healing of the tooth is likely to be related to interproximal connective tissue healing. Our case is an example of spontaneously healed fracture roots. An interesting finding is that the healing was observed even in the presence of two root canals, the vitality of the pulp was preserved and displacement of fragments were prevented.  相似文献

Abstract A case is presented in which an untreated root fracture in a maxillary central incisor of an adult male was found radiographically 10 years after injury. The tooth was removed and the area of the root corresponding to the fracture line was examined by SEM while the remaining tooth was processed for decalcified sections. Calcification of the coronal pulp associated with extensive tissue atrophy and normal pulp tissue apically was observed. Two barriers composed of dent in al and cemental hard tissue were present apically in the coronal fragment and in the fracture line.  相似文献

Abstract This case report describes successful long-term conservative management of a cervical root fracture and a complicated crown fracture of the maxillary central incisors in a 12-year-old patient. A mineral trioxide aggregate partial pulpotomy was performed on the maxillary right central incisor, while the maxillary left central incisor was splinted to the neighbouring lateral incisor using an acid-etch technique. Both teeth remained asymptomatic throughout the 3.5 years of a review period, with the cervical root fracture having mostly healed with the formation of a calcified tissue between the fragments. Two different treatment methods were used for two different injuries that resulted in pulp preservation in both cases. This in turn has provided for normal root development to occur while also allowing for preservation of bone.  相似文献

Horizontal root fractures (HRF) usually affect anterior teeth of male patients as a result of trauma. The consequences can be complex because of combined damage to different tissues, but it has been reported that root fractures undergo healing in majority cases. Diagnosis of HRF must be based on clinical findings, sensibility tests, and radiographic examination. The cone‐beam computed tomography (CBCT) has already been proven to be superior among other radiographic modalities for diagnostic imaging of root fractures. However, CBCT in these cases needs to be used in a careful manner, and only when the radiation exposure should be justified by the potential diagnostic benefits and improvement of the treatment results. This case report describes a case of spontaneously healed horizontal root fracture with displacement of the fragments and discusses the usefulness of CBCT in the follow‐up of root fractures cases.  相似文献

Abstract –  Intra-alveolar root fractures are relatively uncommon, and sometimes may be healed without treatment. Diagnosis of intra-alveolar root fracture is frequently made by exclusion of clinical signs of other traumatic injury, and by radiographic examination with films taken at different tube angulations. This case report presents a horizontal root fractured tooth that was healed satisfactory without treatment and moved orthodontically after prolonged time from trauma.  相似文献

Abstract A case is presented in which a horizontal root fracture of the mesiobuccal root of a maxillary first molar was diagnosed as the source of a patient's chronic pain. Horizontal root fractures arc rare in posterior teeth. The source of the fracture was determined to be trauma from access or curettage during two Caldwell-Luc sinus procedures. A detailed history and endodontic retreatment led to the diagnosis. A mesiobuccal root resection resolved the chronic pain.  相似文献

Abstract –  This clinical case study describes a multidisciplinary modified technique for the treatment of an oblique root fracture. A-38-year old woman with a history of trauma and a broken tooth was referred to our clinic. There was an oblique crown fracture extending the coronal third of the root just underneath the cingulum of the crown. The patient was willing to keep her tooth in function by any means. Two weeks after root canal therapy the flap was raised. The coronal two third of the root canal was enlarged. An impression of the enlarged root canal and tooth surface with surrounding bone was taken. The flap was sutured and the impression was sent to the laboratory for the post-casting preparation. After 7 days, the flap was opened again and casting was cemented. The flap was sutured. Ten days after the second surgery, three anterior teeth were prepared and a final impression was taken for the preparation of porcelain crowns. Restoration was finished 15 days after the second operation. The patient is still under maintenance therapy and the 12 month results are presented in this report. Extraction may not be the only alternative for the root fractures. Even for fractures under the alveolar margin, alternative multidisciplinary approaches can be used to restore and allow the tooth to survive.  相似文献

The loss of anterior teeth can be psychologically and socially damaging to the patient. Despite a wide range of treatment options available, traumatised teeth may be inevitably lost on certain occasions. This paper describes the immediate replacement of a right central incisor using a fibre-composite resin with the natural tooth crown as a pontic. The abutment teeth can be conserved with minimal or no preparation, thus keeping the technique reversible, and can be completed at chair side thereby avoiding laboratory costs. It can be used as an interim measure or a definitive prosthesis.  相似文献

Abstract A case of a 9-year-old boy who presented with intruded maxillary central incisors, one of which had also sustained a middle third root fracture, is reported. The root fracture was diagnosed after orthodontic extrusion was started, which resulted in separation of the apical and coronal fragments. The hypermobile coronal fragment was stabilised by passing a K file through a palatal access cavity into the root canal of the apical fragment. Orthodontic alignment was completed and the fractured incisors were restored with the file left in situ. At a 10-year follow-up, the incisors were clinically healthy and continued to satisfy aesthetic and functional demands.  相似文献

Abstract –  Root fracture injuries affect 0.5–7% of permanent teeth. Although this type of injury is rarely seen in teeth with immature root formation, the prognosis is generally good depending on the site of the fracture. We report on two undiagnosed horizontal root fractures of two maxillary right central incisors in two different patients caused by dental trauma which occurred several years ago. These fractures were discovered during a routine full-mouth radiographic examination. While, in case one, the fracture was located in the middle-third of the root, it was in the cervical-third of the root in case two. Abstract  – Crown–root fractures are frequently difficult to treat and have a bad prognosis. To restore the biological space, intentional replantation can be chosen when osteotomy/osteoplasty and dental traction are not viable. Intentional replantation with tooth rotation consists of extraction, 180° rotation and replantation, thus restoring the biological space. After treatment of inflammatory resorption with calcium hydroxide, endodontic treatment was completed. The patient was followed up with clinical examination (mobility test, gingival sulcus depth) and radiographic analysis (to analyze root resorption: inflammatory and/or substitution, and the integrity of the alveolar cortex and the periodontal space) for 3 years and there were no signs of resorption.  相似文献

AIM: To illustrate significant tipping orthodontic movement of root fractured teeth. SUMMARY: As the frequency of root fractured maxillary teeth is related to increased overjet and reduced lip coverage, orthodontic treatment may increase lip coverage and reduce the risk of trauma or its severity. It may also be necessary to move previously traumatized teeth. Two root fractured teeth were tipped through a considerable angle (19 degrees ) to reduce a large overjet and followed up for 5 years. KEY LEARNING POINTS: -- Reduction of large overjets involving root fractured teeth may not affect pulp vitality. -- Root fragment separation prior to orthodontics did not appear to increase in this patient, but angulation of the fragments did not completely follow the major change to the coronal part of the tooth.  相似文献

Root fractures vary in severity, extent, and location, according to the physical and mechanical aspects of the accident. Root fractures are rare in primary teeth and they affect dentin, cementum, periodontal ligament and the pulp. This paper reports a case of a two-and-a-half-year-old baby who had a root fracture of the upper right primary central incisor (tooth 51) as a consequence of trauma that was managed with minimally invasive intervention. After clinical and radiographic examinations, the presence of tooth mobility, pain, bleeding and a horizontal apical root fracture was diagnosed on tooth 51. The treatment of choice was splinting for 120 days, which can deliver excellent results regardless of the patient's age. The tooth was preserved without needing endodontic intervention until its physiologic exfoliation and normal eruption of its permanent successor. As sequelae to the fractured tooth 51, there was some coronal color alteration and dystrophic calcification of the root canal. This tooth did not develop ankylosis or mobility during the follow-up period. After 57 months of follow-up, repositioning and splinting were appropriate ways to manage this horizontal root fracture in the apical third of this primary tooth. It allowed the tooth to be preserved in the arch with normal function.  相似文献

AIM: To demonstrate the need for proper restoration of root-filled teeth. SUMMARY: Tooth fracture can be a sequel to root-canal treatment. It is associated with the removal of excessive amounts of the tooth substance during the instrumentation phase, the use of unnecessary force during obturation, or inadequate planning of the design of the eventual coronal restoration to include appropriate cuspal protection. Rarely, a fracture may be present in a tooth before treatment commences, but remains undetected. A case describing incomplete vertical tooth fracture of a root-filled mandibular first molar is presented.  相似文献
