军队医院临床医师绩效考评系统的设计与实践

随着信息化建设的飞速发展.军队医院进入数字化阶段后,医院管理更趋于科学化、标准化、规范化.军队实施"军卫一号"工程以来,针对每位临床军医的医疗数质量指标、科研、经济、为兵服务、各项军事训练、军事行动、军人素质等等方面从未开发过统计查询系统软件,对医生的总体工作绩效考评更是一个盲区.本院开发的军队医院临床医师绩效考评系统解决了上述问题,可以方便快捷的查询临床医师的相关工作数质量情况、生成报表,并建立了科学的考评l临床医师指标体系,改变了以往时临床医生的粗放型、经验式、印象型的考评方法 .不仅提高了医师的各项工作业绩,而且也提高了管理者的工作效率,使医院的科学规范化管理、信息化建设又上一台阶.

目的 总结医生工站、护士工作站系统的上线经验和应用情况分析,并促进其不断完善,使其更好的为临床工作服务.方法 在医疗体制改革的今天,医院信息管理系统正从"面向医院事务管理"向"临床医疗信息一体化"发展.医生工站、护士工作站是医院信息系统的关键,应用的好坏直接影响医院的管理.结果 通过对医生工作站、护士工作站系统的实施与应用,充分体会到了其科学性、先进性、准确性、实用性、高效性,对缩短医护人员的工作流程、减少文字工作、减少差错、提高工作效率、提高医疗质量有着重要的作用.结论 通过不断使用、升级系统功能使医院信息管理系统日趋完善,逐步实现了真正意义上的医院信息管理.

为了能及时了解医院临床外科医师手术工作量及质量情况，编制临床外科医师手术质量控制计算机管理系统，该系统将医师的职称、手术名称分类、手术切口类别、切口愈合等级及切品愈合等级率等几个指标综合起来评价临床外科医师医疗质量标准之一。

直着计算机网络技术的发展，微机已成为医院日常医疗业务管理工作的重要组成部分和必不可少的工具，医疗工作的正常运转越来越依赖于局域网。我院计算机网络系统由HIS系统（医院信息系统）、LIS系统（检验信息系统）、电子图书系统（CHKD《中国医院知识仓库》）构成，医院工作的特点决定医院局域网必须每天24小时不间断地实时处理信息，任何一个故障都可能影响临床一线正常工作。

基于《医院处方点评管理规范(试行)》上的处方点评分析

目的 探讨一种行之有效的管理程序使某二甲医院的处方点评工作真正落在实处,并起到规范、监督、审核医师处方的作用,使得医师提高临床用药的规范性、合理性.方法 本文主要根据卫生部2010年2月发布的<医院处方点评管理规范>,建立了该二甲医院的处方点评工作系统.并由处方点评得到的结果对该院的处方进行相应的干预.结果 与结论实施结果提高了该院处方点评管理水平和处方合理率,较好地促进了临床合理用药;同时提出处方点评中需解决的几个问题.

目的：满足医院各级管理者对所属人员个体工作指标量化管理的需求。方法：基于医院信息系统实时采集的数据资源,着重研究“医师主要工作效率指标和效益指标”的量化和分类。结果：通过应用程序,实现了门诊、临床和辅诊各级医师工作效率指标和经济效益指标的查询和报表。结论：人员个体指标量化方法的研究和应用是深化信息资源利用、加强环节质量控制和促进科学管理的有效方法。

HRP(Health Resource Plan)即医院资源管理平台,该平台是在提高医院服务质量的基础上实现以"结构化电子病历、质量管理、成本管理和医患关系管理"为特点的新一代医院管理系统,"住院流程标准化系统"就是HRP的一部分,文章介绍了该系统的主要模块和成效。     

医学影像设备管理系统的设计与开发

目的：设计和开发出一套功能全面、操作简便而又实用的放射科设备管理信息系统。方法：本论文从软件工程的角度出发进行分析和设计,采用J2EE、SPRING、HIBERNATE等先进流行的Web框架技术,对一款医学影像设备管理维护系统进行初步设计和开发。结果：根据对现有医院影像设备管理的需求分析和实地调研,完成了一款医学影像设备管理维护系统的具体开发,系统主要功能模块包括系统管理、采购管理、设备信息管理、故障信息管理、设备维护管理、设备说明书管理等等。结论：该医学影像设备管理系统总体来说,功能全面、操作简便而又实用,能够满足医院影像设备管理的大部分需求,极大的提高了医学影像设备的管理效率。     

Hospital information system--project of implementation of SAP information system at Sveti Duh General Hospital

Nowadays, as medical and hospital institutions have been facing a growing need of a more efficient provision of healthcare services to patients, with simultaneous complete monitoring of the successfulness of business activities, integrated information systems appear as the logical choice for the support to hospital business processes. The integrated business information system implemented at Sveti Duh General Hospital is a comprehensive system that supports all hospital, clinical and administrative processes, while providing the basis for decision making regarding the patients and hospital management. The system also enables transfer of all data with specific medical business segments such as laboratory device management. The project for the implementation of the information system was realized in accordance with the requests from the Ministry of Health, applying the proven methodology for the execution of such complex projects. The project team consisted of a number of consultants from b4b Co. from Zagreb, as well as Hospital employees. The new information system is completely ready for going live; however, the necessary decisions have to be made first. The application of the system gives the medical staff more time for their professional work with patients, and through longterm collection and analysis of data on symptoms, illnesses and medical treatments, the information system becomes an important tool for the improvement of health and quality of healthcare system in general.     

ISO 9001:2008质量管理体系在医院设备管理中的应用

目的用ISO9001:2008质量管理体系的方法来规范和提升医院设备管理工作。方法按照质量管理体系要求建立标准体系文件,包括程序文件、管理制度、记录表单和应急预案。结果本院自2011年建立ISO9001:2008质量管理体系以来,一直按照制度流程运作,每年持续改进。结论 ISO9001:2008质量管理体系可有效应用在医院设备管理中,对整个工作流程有很好的指导意义。     

Virtual polyclinic--consultant health service for rural areas and islands

The development of medical sciences and digital revolution in health care have made possible the creation of new models for hospital care and primary health care. Health care management usually does not follow all possible solutions of new computer technology. Croatian health system is based on large hospital centers in the mainland and small, poorly equipped remote medical units. The possibility of sharing medical information using telematic technologies brings all knowledge of the large, powerful hospital centers close to remote medical units. Working in a small, remote consultation office on the island Cres, the author A. M. became part of medical periphery far from the security of hospital on the mainland. After five years of the work on telemedicine projects for the islands, we have introduced a new medical practice concept of communication between remote medical units and clinical hospital centers in the mainland. The pilot project Virtual Polyclinic-Medical Consultation System for the Islands and Remote Areas is a good example of the possible new models in health care structure. Our experience indicates that organization of health care in remote areas depends not only on advanced technical solutions but also on team work and medical professionals.     

医院管理信息系统在医院中的应用

医院信息系统是利用计算机技术、网络技术、数据库技术对医院进行现代化、数据化管理,为各级领导、部门提供可靠的参考数据与决策支持。在医院管理、临床医疗、护理、财务、后勤物资、医保等多层次多部门之间发挥着重要的作用,并可促使医院的管理模式发生重大变革。医院信息的特点是信息量大,增长快速且复杂,同时各部门之间(包括院外,如医保)的数据需要实现共享及交换。     

Design and development of a mobile computer application to reengineer workflows in the hospital and the methodology to evaluate its effectiveness

This paper describes a new method of collecting additional data for the purpose of skin cancer research from the patients in the hospital using the system Mobile Computing in Medicine Graz (MoCoMed-Graz). This system departs from the traditional paper-based questionnaire data collection methods and implements a new composition of evaluation methods to demonstrate its effectiveness.The patients fill out a questionnaire on a Tablet-PC (or iPad Device) and the resulting medical data is integrated into the electronic patient record for display when the patient enters the doctor’s examination room. Since the data is now part of the electronic patient record, the doctor can discuss the data together with the patient making corrections or completions where necessary, thus enhancing data quality and patient empowerment. A further advantage is that all questionnaires are in the system at the end of the day - and manual entry is no longer necessary - consequently raising data completeness. The front end was developed using a User Centered Design Process for touch tablet computers and transfers the data in XML to the SAP based enterprise hospital information system. The system was evaluated at the Graz University Hospital - where about 30 outpatients consult the pigmented lesion clinic each day - following Bronfenbrenner’s three level perspective: The microlevel, the mesolevel and the macrolevel:

On the microlevel, the questions answered by 194 outpatients, evaluated with the System Usability Scale (SUS) resulted in a median of 97.5 (min: 50, max: 100) which showed that it is easy to use.

On the mesolevel, the time spent by medical doctors was measured before and after the implementation of the system; the medical task performance time of 20 doctors (age median 43 (min: 29; max: 50)) showed a reduction of 90%.

On the macrolevel, a cost model was developed to show how much money can be saved by the hospital management. This showed that, for an average of 30 patients per day, on a 250 day basis per year in this single clinic, the hospital management can save up to 40,000 EUR per annum, proving that mobile computers can successfully contribute to workflow optimization.

     

医院触摸屏查询系统的设计与开发

目的 设计和开发医院触摸屏查询系统。方法利用计算机网络、数据库技术和触摸屏硬件设备，采用PowerBuilder6．5编程实现。结果 医院触摸屏查询系统实现了多种功能，如药品、收费标准、特色专科、医院和医保政策介绍、门诊和住院费用查询等。在我院运行的一年多时间里，该系统极大地方便了病人，体现了“以病人为核心”的政策。结论 医院触摸屏查询系统方便实用，有利于完善医院信息系统(HIS)，实现医院管理的数字化、现代化。
