OBJECTIVE. We evaluate the uniformity and reproducibility of thermal lesion ablation and quantify the volume of tissue destruction and hemorrhage induced with two different commercially available radiofrequency ablation devices. MATERIALS AND METHODS. A four-array anchor expandable needle electrode and a triple-cluster cooled-tip needle electrode were used to induce lesions in three explanted calf livers and in vivo in eight swine livers. The sizes of the radiofrequency-induced lesions were macroscopically evaluated by measuring two perpendicular dimensions immediately after the experiment. Bleeding was evaluated by weighing gauze swabs used to dry the hemorrhage caused by electrode insertions. RESULTS. In explanted liver, the mean diameter of the radiofrequency-induced lesion was 5.3 +/- 0.7 cm for the cooled-tip needle and 3.7 +/- 0.4 cm for the expandable needle (p = 0.042), which correspond to approximate volumes of 65.35 +/- 26.22 cm(3) and 26.67 +/- 9.59 cm(3), respectively (p < 0.002). In vivo, the mean diameter was 3.7 +/- 0.4 cm for the cooled-tip needle and 3 +/- 0.4 cm for the expandable needle (p < 0.0001), which correspond to approximate volumes of 24.18 +/- 7.56 cm(3) and 11.16 +/- 3.65 cm(3), respectively (p < 0.0001). Blood loss attained a median value of 3.5 g for the cooled-tip needle and 2.6 g for the expandable needle; this difference was not statistically significant (p = 0.06). CONCLUSION. The cooled-tip needle induced significantly larger lesions than the expandable needle, but the lesions produced by the expandable needle are more reproducible, uniform, and spheric. The larger size of the lesions produced by the cooled-tip needle may be attributed to the higher maximum power used by the generator and the higher energy deposition, which is due to the cooling of the needle electrode.  相似文献

The aim of this study was to assess the effects of simultaneous instillation of NaCl solutions during radio-frequency ablation (RFA) on the dimension of the ablated lesion in ex vivo bovine lung tissue and in vivo rabbit lung tissue. The RFA was induced in ex vivo bovine lung tissue which was inflated with room air and in vivo rabbit lung tissue by a 500-kHz RF generator and a 17-G cooled-tip electrode. In in vivo experiments, RFA was performed using CT guidance. The RF energy was applied for 5 min with or without instillation of 0.9 or 36% NaCl solutions. The changes in tissue impedance, current, power output, and temperature of the electrode tip were automatically measured. The maximum diameter of all thermal lesions was measured perpendicular to the electrode axis by two observers. In an ex vivo study, the mean lesion diameters using 36 and 0.9% NaCl solutions were larger than those of the control group: 51±8, 34±6, and 5±2 mm (p<0. 05). In in vivo rabbit lung tissue, the mean lesion diameter with NaCl solution (15.3±3.1 mm) was larger than that of the lesion without NaCl solution (8.5±1.4 mm; p<0.05). With instillation of NaCl solutions, a marked decrease of tissue impedance (>100 Ω) and corresponding increase of current flow occurred in both ex vivo and in vivo studies. This experimental study demonstrates that RF ablation with simultaneous NaCl solution infusion of the lung is more effective in achieving coagulation necrosis than conventional RFA procedure.  相似文献

Purpose To evaluate the usefulness of a guiding needle for radiofrequency (RF) ablative treatment of liver tumors. Methods Forty-two patients, 38–78 years old (57 ± 17), with 42 liver tumors (18 HCC, 24 colon cancer metastases) underwent RF ablation using a 14-gauge guiding needle with an external insulated sheath in which any 18-gauge or smaller needle can be placed, including a specially designed 3.5 cm LeVeen RF electrode. One guiding needle was used in 20 tumors to provide biopsy and RF treatment in a single puncture. Three to five guiding needles were loaded in 22 tumors measuring 35 to 64 mm in their largest diameter before starting RF treatment requiring multiple overlapping RF applications. Results In the 20 RF treatments combined with biopsy, the biopsy was always contributive. Because of pre-positioning of the sheath, postbiopsy modifications (bleeding or air artifacts) did not hinder subsequent RF treatment. The 22 large tumors received 5 to 12 RF applications (mean = 6.8) through the three to five preloaded guiding needles. The RF ablation zones measured 46 to 94 mm (mean = 55) in their largest dimension, with ablative margins in all cases. After 8 to 32 months (mean = 20), 14 of the 22 tumors are considered completely destroyed on computed tomography follow-up and one tumor seeding has been found. Conclusion The Leveen CoAccess needle allows precise tumor targeting when treating large tumors requiring multiple RF applications. A 53-year-old-man underwent US-guided percutaneous thermal ablation with a cooled-tip needle of three liver metastases from gastric cancer. Six days later, the patient was re-admitted for melena, scleral jaundice, and anemia. Abdominal US disclosed echogenic material in the gallbladder lumen (hemobilia) and a focal lesion with mixed echotexture in segment III (hepatic hematoma). On day 5 portal cavernomatosis was diagnosed at US and confirmed by color Doppler and a helical CT exam. The case described emphasizes that radio-frequency interstitial hyperthermia may cause not only traumatic injury of the liver parenchyma but also thermally mediated damage of vascular structures. The objective of this study was to analyze long-term results of radiofrequency thermal ablation (RFA) for colorectal metastases (MTS), in order to evaluate predictors for adverse events, technique effectiveness, and survival. One hundred ninety-nine nonresectable MTS (0.5–8 cm; mean, 2.9 cm) in 122 patients underwent a total of 166 RFA sessions, percutaneously or during surgery. The technique was “simple” or “combined” with vascular occlusion. The mean follow-up time was 24.2 months. Complications, technique effectiveness, and survival rates were statistically analyzed. Adverse events occurred in 8.1% of lesions (major complication rate: 1.1%), 7.1% with simple and 16.7% with combined technique (p = 0.15). Early complete response was obtained in 151 lesions (81.2%), but 49 lesions (26.3%) recurred locally after a mean of 10.4 months. Sustained complete ablation was achieved in 66.7% of lesions ≤3 cm versus 33.3% of lesions >3 cm (< 0.0001). Survival rates at 1, 3, and 5 years were 91%, 54%, and 33%, respectively, from the diagnosis of MTS and 79%, 38%, and 22%, respectively, from RFA. Mean survival time from RFA was 31.5 months, 36.2 in patients with main MTS ≤3 cm and 23.2 in those with at least one lesion >3 cm (p = 0.006). We conclude that “simple” RFA is safe and successful for MTS ≤3 cm, contributing to prolong survival when patients can be completely treated. Presented at CIRSE 2006.  相似文献

Fine needles with an end hole or multiple side holes have traditionally been used for percutaneous ethanol injection (PEI) of hepatomas. This study retrospectively evaluates the safety and efficacy of PEI of unresectable medium-to-large (3.5–9 cm) hepatomas using a multipronged needle and with conscious sedation. Twelve patients, eight men and four women (age 51–77 years; mean: 69) received PEI for hepatomas, mostly subcapsular or exophytic in location with average tumor size of 5.6 cm (range: 3.5–9.0 cm). Patients were consciously sedated and an 18G retractable multipronged needle (Quadrafuse needle; Rex Medical, Philadelphia, PA) was used for injection under real-time ultrasound guidance. By varying the length of the prongs and rotating the needle, the alcohol was widely distributed within the tumor. The progress of ablation was monitored by contrast-enhanced ultrasound, computed tomography (CT) or magnetic resonance imaging (MRI) after each weekly injection and within a month after the final (third) injection and 3 months thereafter. An average total of 63 mL (range: 20–154 ml) of alcohol was injected per patient in an average of 2.3 sessions. Contrast-enhanced CT, ultrasound, or MRI was used to determine the degree of necrosis. Complete necrosis was noted in eight patients (67%), near-complete necrosis (90–99%) in two (16.7%), and partial success (50–89%) in two (16.7%). Follow-up in the first 9 months showed local recurrence in two patients and new lesions in another. There was no mortality. One patient developed renal failure, liver failure, and localized perforation of the stomach. He responded to medical treatment and surgery was not required for the perforation. One patient had severe postprocedural abdominal pain and fever, and another had transient hyperbilirubinemia; both recovered with conservative treatment. PEI with a multipronged needle is a new, safe, and efficacious method in treating medium-to-large-sized hepatocellular carcinoma under conscious sedation. The purpose of this study was to assess the efficacy of a new perfused-cooled electrode that uses a single pump for creating a large ablation zone in explanted bovine liver. This was done by comparing with the radiofrequency (RF) ablation zones that were created with a monopolar cooled electrode to the RF ablation zones that were created by the new perfused-cooled electrode. We developed a new perfused-cooled electrode that uses a single pump by modifying a 17-gauge cooled electrode (Radionics) with a 2.5-cm outer metallic sheath (15-gauge) in order to allow use of the internal cooling water (5.85 % hypertonic saline) for the infused saline. Thirty ablation zones were created in explanted bovine livers (12-min ablation cycle; pulsed technique; 2000 mA, maximum) with three different regimens: group A, RF ablation with the 17-gauge cooled electrode; group B, RF ablation with the 15-gauge cooled electrode; group C, RF ablation with the perfused-cooled electrode. T2-weighted magnetic resonance (MR) imaging was obtained immediately after RF ablation for calculating volumes of the ablation zone. Following MR imaging, the ablation zones were excised and measured for transverse diameters and vertical diameters. The transverse diameter, vertical diameter, and the calculated volumes of the ablation zones on MR imaging were compared among the groups. Ablation zones created with the perfused-cooled electrode (group C) were significantly larger than those created with the 17-gauge cooled electrode (group A) and the 15-gauge cooled electrode (group B) according to the transverse diameter and vertical diameter on the gross specimens (p < 0.05): 3.6 ± 0.38 cm and 4.4 ± 0.20 cm in group A, 3.7 ± 0.08 cm and 4.6 ± 0.16 cm in group B, and 5.4 ± 0.65 cm and 6.0 ± 0.56 cm in group C, respectively. On the MR imaging, the calculated volumes of the ablation zones in group C were significantly larger than those in groups A and B (p < 0.05): 23.1 ± 8.7 cm3 in group A, 28.9 ± 5.7 cm3 in group B, and 80.0 ± 34 cm3 in group C, respectively. A new perfused-cooled electrode using a single pump could efficiently increase the size of the ablation zone in liver compared with a monopolar cooled electrode, and this was due to its simultaneous use of internal cooling and saline infusion.  相似文献

The present comparative study was conducted to validate a newly developed "expandable-wet" electrode for an increased lesion size of radiofrequency ablation (RFA) on excised beef liver. The expandable-wet electrode, which allows interstitial hypertonic saline infusion through retractable curved needles, was compared with "expanded-dry" and "unexpanded-wet" electrodes for RFA lesion size and other parameters. A total of 120 lesions were created under 50 W (groups A-C) and 90 W (groups A'-C') power control mode for 10 min at each ablation site with the following groups: group A and A' of expanded-dry electrode (needles deployed but saline uninfused); group B and B' of unexpanded-wet electrode (saline infused but needle undeployed); and group C and C' of expanded-wet electrode (needles deployed and saline infused). Together with lower impedance and higher power output, the lesion size in group C (5.3+/-0.4 cm) and C' (6.0+/-1.0 cm) were significantly larger (P<0.01) than that in group A (3.3+/-0.3 cm) and A' (2.0+/-0.2 cm), and group B (3.8+/-1.0 cm) and B' (2.6+/-0.4 cm). The RFA lesion size can be significantly enlarged when the expandable electrode is complemented with interstitial hypertonic saline infusion. This design may improve the efficacy of RF tumor ablation.  相似文献

The aim of our study was to compare the diagnostic efficacy of power Doppler imaging and conventional color Doppler sonography for differentiating between hepatocellular adenoma (HCA) and focal nodular hyperplasia (FNH) of the liver. Thirty-one focal liver lesions (in 29 patients) with histologic proof of HCA (n = 9) or FNH (n = 22) were studied with power and color Doppler sonography according to a standardized examination protocol. The size of the lesions ranged between 1.5 and 14.5 cm (HCA, 3.5–14.5 cm, mean ± SD 7.3 ± 3.3 cm; FNH, 1.5–9.1 cm, mean ± SD 5.1 ± 2.1 cm). Intratumoral vessels with a venous Doppler spectrum, associated with either pulsatile or continuous peripheral flow, were detected in HCA (eight of nine lesions by power Doppler imaging and six of nine by color Doppler imaging) but not in FNH. In contrast, color signals with an arterial Doppler spectrum, radiating from the center to the periphery of the lesion, were depicted in FNH (20 of 22 cases by power Doppler imaging and 15 of 22 by color Doppler sonography) but not in HCA. Differentiation of HCA and FNH was achieved in 28 of 31 cases (90 %) by power Doppler imaging and in 21 of 31 (68 %) by color Doppler sonography (p < 0.01). Power Doppler imaging is superior to conventional color Doppler sonography in the depiction of the intratumoral flow characteristics of HCA and FNH, and enables a more accurate differential diagnosis than color Doppler sonography. The purpose of this study was to evaluate the effectiveness of a newly developed perfused monopolar radiofrequency (RF) probe with an umbrella-shaped array. A perfused umbrella-shaped monopolar RF probe based on a LeVeen electrode (Boston Scientific Corp., Natick, MA, USA) with a 3-cm array diameter was developed. Five different configurations of this electrode were tested: (a) perfusion channel/endhole, (b) perfusion channel/endhole + sideholes, (c) 1 cm insulation removed at the tip, (d) 1 cm insulation removed at the tip + perfusion channel/endhole, and (e) 1 cm insulation removed at the tip + perfusion channel/endhole + sideholes. An unmodified LeVeen electrode served as a reference standard. RF ablations were performed in freshly excised bovine liver using a commercial monopolar RF system with a 200-W generator (RF 3000; Boston Scientific Corp.). Each electrode was tested 10 times applying the vendor’s recommended ablation protocol combined with the preinjection of 2 ml 0.9% saline. Volumes and shapes of the lesions were compared. Lesions generated with the original LeVeen electrode showed a mean volume of 12.74 ± 0.52 cm3. Removing parts of the insulation led to larger coagulation volumes (22.65 ± 2.12 cm3). Depending on the configuration, saline preinjection resulted in a further increase in coagulation volume (25.22 ± 3.37 to 31.28 ± 2.32 cm3). Besides lesion volume, the shape of the ablation zone was influenced by the configuration of the electrode used. We conclude that saline preinjection in combination with increasing the active tip length of the umbrella-shaped LeVeen RF probe allows the reliable ablation of larger volumes in comparison to the originally configured electrode.  相似文献

目的 探讨rACE联合CT引导热消融(射频/微波)治疗肝内胆管癌(ICC)的疗效及安全性.方法 回顾分析2009年9月至2013年7月用TACE序贯CT引导下RFA/MWA治疗病理诊断的ICC患者14例,18个病灶,病灶最大径(多个病灶取最大径之和)2.2~7.2 cm,平均(4.2±1.4) cm.TACE随访病灶的完全消融率、手术并发症、无瘤生存时间及总生存时间.结果 15个病灶实现了完全消融,肿瘤的完全消融率为83.3%(15/18),中度并发症的发生率为6.2%,无严重并发症及死亡患者.术后随访6-~44个月,平均(16.0±10.3)个月.至随访结束,死亡6例(42.9%,6/14).完全消融患者的中位无瘤生存时间为17个月,全组患者的中位生存时间为20个月,1、2、3年总体生存率分别为82.5%、41.3%及20.6%.结论 TACE联合CT引导热消融可作为ICC治疗的选择之一.  相似文献

Purpose  The aim of this study is to describe the procedure technique, clinical and imaging outcomes of patients treated with radiofrequency ablation for chondroblastoma. Materials and methods  Four patients (female/male, 3:1; mean age, 13 years; age range; 9–16 years) underwent the procedure. All had pre-operative magnetic resonance imaging (MRI) and symptomatic, biopsy-proven chondroblastomas (two proximal femur, two proximal tibia). The lesion size ranged from 1.5 to 2.5 cm in maximal dimension (mean size, 1.8 cm). Bone access was gained with a Bonopty biopsy needle system (mean number of radiofrequency needle placements, 5; mean ablation time, 31 min). Results  Clinical and MRI follow-up was available in all cases (mean, 12.25 months; range, 5–18 months). All patients reported resolution of symptoms at 2–6 weeks post ablation. At their most recent clinical follow-up, three patients remained completely asymptomatic with full return to normal activities and one patient had minor local discomfort (different pain pattern) that was not limiting activity. All four patients’ follow-up MRI studies demonstrated resolution of the oedema pattern around the lesion and temporal evolution of the internal signal characteristics with fatty replacement. Conclusion  Radiofrequency ablation for chondroblastoma provides an alternative to surgical curettage, and we have demonstrated both a clinical improvement in symptoms and the follow-up MRI appearances.  相似文献

Purpose To evaluate the feasibility and potential advantages of the radiofrequency ablation of liver tumors using new MRI-compatible semiflexible applicators in a closed-bore high-field MRI scanner. Methods We treated 8 patients with 12 malignant liver tumors of different origin (5 colorectal carcinoma, 2 cholangiocellular carcinoma, 1 breast cancer) under MRI guidance. Radiofrequency ablation (RFA) was performed using 5 cm Rita Starburst Semi-Flex applicators (Rita Medical Systems, Milwaukee, WI, USA) which are suitable for MR- and CT-guided interventions and a 150 W RF generator. All interventions were performed in a closed-bore 1.5 T high-field MRI scanner for MRI-guided RFA using fast T1-weighted gradient echo sequences and T2-weighted ultra-turbo spin echo sequences. Control and follow-up MRI examinations were performed on the next day, at 6 weeks, and every 3 months after RFA. Control MRI were performed as double-contrast MRI examinations (enhancement with iron oxide and gadopentetate dimeglumine). All interventions were performed with the patient under local anesthesia and analgo-sedation. Results The mean diameter of the treated hepatic tumors was 2.4 cm (±0.6 cm, range 1.0–3.2 cm). The mean diameter of induced necrosis was 3.1 cm (±0.4 cm). We achieved complete ablation in all patients. Follow-up examinations over a duration of 7 months (±1.3 months, range 4–9 month) showed a local control rate of 100% in this group of patients. All interventions were performed without major complications; only 2 subcapsular hematomas were documented. Conclusion RFA of liver tumors using semiflexible applicators in closed-bore 1.5 T scanner systems is feasible. These applicators might simplify the RFA of liver tumors under MRI control. The stiff distal part of the applicator facilitates its repositioning.  相似文献

The aim of this study was to determine apparent diffusion coefficients (ADCs) of focal liver lesions on the basis of a respiratory triggered diffusion-weighted single-shot echo-planar MR imaging sequence (DW-SS-EPI) and to evaluate whether ADC measurements can be used to characterize lesions. One hundred and two patients with focal liver lesions [11 hepatocellular carcinomas (HCC), 82 metastases, 4 focal nodular hyperplasias (FNH), 56 hemangiomas and 51 cysts; mean size, 16.6 mm; range 5–92 mm] were examined on a 1.5-T system using respiratory triggered DW-SS-EPI (b-values: 50, 300, 600 s/mm2). Results were correlated with histopathologic data and follow-up imaging. The ADCs of different lesion types were compared, and lesion discrimination using optimal thresholds for ADCs was evaluated. Mean ADCs (×10−3mm2/s) were 1.24 and 1.04 for normal and cirrhotic liver parenchyma and 1.05, 1.22, 1.40, 1.92 and 3.02 for HCCs, metastases, FNHs, hemangiomas and cysts, respectively. Mean ADCs differed significantly for all lesion types except for comparison of metastases with HCCs and FNHs. Overall, 88% of lesions were correctly classified as benign or malignant using a threshold value of 1.63 × 10−3mm2/s. Measurements of the ADCs of focal liver lesions on the basis of a respiratory triggered DW-SS-EPI sequence may constitute a useful supplementary method for lesion characterization.  相似文献

The aim of this study was to assess the efficacy of percutaneous cementoplasty (PC) with polymethylmethacrylate (PMMA) in painful extravertebral lytic bone metastases not responding to conventional therapy. Fifty patients (25 females), mean age 64.7 ± 11.2 years, underwent PC after giving informed consent. Procedures were performed under fluoroscopy (1/50) or combined fluoroscopy-CT (49/50) guidance in local anesthesia or under deep sedation in 7 patients with large metastases who underwent radiofrequency thermoablation (RFA) in the same session. Seventy lesions were treated (1-6 per patient; average, 1.4 ± 0.9), arranging in size from 1 to 10 cm (average, 3.6 ± 2.1 cm). Mean volume of PMMA per lesion was 5.9 ± 3.2 ml (range, 1.5–15.0 ml). Pain was prospectively evaluated on an 11-point visual analog scale (VAS) before and after the procedure (follow-up, 15 to 36 months). Mean VAS score dropped from 9.1 ± 1.2 (range: 6–10) to 2.1 ± 2.5 (range: 0–9). Mean VAS difference was 7.0 ± 2.3 (range, 1–10; p < 0.0001, Wilcoxon signed rank test). Forty-seven of the 50 patients (94%) suspended narcotic drugs, in 22 (44%) pain was controlled with a nonsteroidal anti-inflammatory drug, in 25 (50%) analgesic therapy was suspended, and 13 of 50 (26%) had complete pain regression. In 3 of the 50 patients (6%) pain was not improved. No statistical difference between osteoplasty and osteoplasty plus RFA was found (p = 0.8338, Mann–Whitney test). No complications arose during the procedure. Two patients with metastases in the femoral diaphysis reported a fracture 1 month after treatment. PC is effective to obtain pain regression in painful bone metastases not responding to conventional analgesic therapy; bone consolidation cannot be obtained in the diaphysis of long weight-bearing bones.  相似文献

We evaluated the safety and efficacy of image-guided radiofrequency ablation (RFA) using a triple-spiral-shaped electrode needle for unresectable primary or metastatic hepatic tumors. Thirty-four patients with 46 index tumors were treated. Ablation zone, morbidity, and complications were assessed. The lesions were completely ablated with an ablative margin of about 1 cm. Five patients (14.7%) with a lesion larger than 4.5 cm had local tumor progression after 1 month and were retreated. Hemothorax, as a major complication, occurred in 1 of 34 patients (3.0%) or 1 of 46 lesions ablated (2.2%). RFA using this new electrode needle can be effective in the treatment of large unresectable hepatic tumors.  相似文献

The aim of this study was to evaluate capabilities of pulse inversion harmonic imaging (PIHI) with Levovist in detection of liver metastases compared with conventional ultrasound (US) and helical CT (HCT). One hundred sixty consecutive patients with known malignancies were evaluated by conventional US, PIHI 2 min (40 patients) or 4 min (120 patients) after Levovist injection and HCT. Conspicuity and number of the identified metastatic lesions were evaluated and assessed by statistical analysis (significance p<0.05). Mean diameter of the smallest identified metastases was computed for conventional US, PIHI and HCT. In cases where PIHI revealed more lesions suspicious for metastases than HCT, intraopertive US with surgical biopsy or 3–6-month US follow-up were performed to confirm diagnosis. Images were stored on magneto-optical disk and evaluated off-line by a dedicated software. Metastases conspicuity was significantly improved on PIHI if compared with conventional US (p<0.05). In 49 patients all the employed imaging techniques did not reveal any lesion, whereas in the remaining 111 patients, 28 patients revealed more than five metastatic lesions and 83 patients presented from one to five metastatic lesions. In comparison with conventional US, PIHI revealed more metastases in 39/83 (47%), the same number in 44/83 (53%) and a lower number in 0/83 (0%) patients. In comparison with HCT, PIHI revealed more metastases in 10/83 (12%), the same number in 61/83 (74%) and a lower number in 12/83 (14%) patients. Average number ± SD (standard deviation) of confirmed metastases for patients was 2.21±1.6 for conventional US, 3.1±2.44 for PIHI and 3.05±2.41 for HCT. The difference between PIHI and conventional US was statistically significant (p<0.0001), whereas the difference between PIHI and HCT was not significant (p=0.9). PURPOSE: The purpose of this study was to determine the efficacy and safety of radiofrequency (RF) ablation for the treatment of hepatocellular carcinoma (HCC) in the hepatic dome with CT-guided extrathoracic transhepatic approach. MATERIALS AND METHODS: Fifteen patients with 15 HCCs (size range: 0.8-4 cm, mean size: 1.8 cm) in the hepatic dome were treated by RF ablation using cooled-tip electrodes and with CT-guided extrathoracic transhepatic approach. Therapeutic response of the tumor to RF ablation and procedure-related complications including hepatic injury, hemoperitoneum, and thermal injury of diaphragm were evaluated. RESULTS: The average number of needle punctures to ensure the correct needle position in the targeted tumor was 3.7 (range: 1-6 punctures). The average ablation time was 14.7 min (range: 8-25 min). Complete necrosis without marginal recurrence after at least 13-month follow-up was attained in 13 tumors (86.7%). There were no major complications related to the procedures. Six patients had shoulder pain that lasted three days to two weeks after the procedures and their symptoms were resolved with conservative treatment. CONCLUSIONS: RF ablation using CT-guided extrathoracic transhepatic approach is an effective and safe technique for the treatment of HCC in the hepatic dome.  相似文献

PURPOSE: This study was undertaken to compare the thermal lesion volumes in normal pig lungs when radiofrequency (RF) ablation is performed with and without airway occlusion. MATERIALS AND METHODS: RF ablation was performed in six pigs. A straight 17-gauge internally cooled-tip electrode with a 2-cm exposed tip was inserted into the center of the lower lobe of the lung under biplane fluoroscopic guidance. In each animal, RF ablation was performed for 12 minutes with balloon occlusion of the main bronchus in one lung and without balloon occlusion in the contralateral lung. The tissue temperature around the electrode tip was measured immediately after RF application. The volumes of the thermal lesions were compared by histologic examination of the groups of lungs ablated with and without airway occlusion. RESULTS: Tissue temperature was significantly higher in the bronchial occlusion group than in the group with normal ventilation (51 degrees C +/- 7 vs. 44 degrees C +/- 2; P < .05). RF ablation with bronchial occlusion resulted in the creation of a significantly greater thermal lesion volume compared with RF ablation with normal ventilation (6,535 mm(3) +/- 1,114 vs 3,368 mm(3) +/- 676; P < .03). CONCLUSION: Prevention of ventilation in the normal swine lung via bronchial balloon occlusion during RF ablation increases the thermal ablation lesion volume, suggesting that active ventilation is a significant cause of in vivo heat loss.  相似文献

Although surgical resection remains the best option as potentially curative therapy for hepatocellular carcinoma, radiofrequency thermal ablation has begun to receive much attention as an effective minimally invasive technique for the local control of unresectable malignant hepatic tumors. Most recent radiofrequency devices equipped with a powerful generator and larger needle electrode permit larger thermal lesions, up to 5 cm in diameter, with a single ablation.
