BACKGROUND: The aims of this study were to characterize the features of the biliary strictures that occur after duct-to-duct biliary reconstruction during right-lobe living-donor liver transplantation (LDLT) and to evaluate the feasibility of correcting such stricture endoscopically by inserting an "inside stent," that is, a short internal stent, above the sphincter of Oddi. METHODS: Biliary stricture occurred in 26 (35.6%) of 73 consecutive patients who underwent right-lobe LDLT with duct-to-duct biliary reconstruction from July 1999 through October 2001 and survived for more than 3 months. Of the 26 patients who had biliary stricture, 22 were referred for endoscopic retrograde cholangiography (ERC) and 4 for percutaneous cholangiography. RESULTS: ERC disclosed biliary stricture in 19 (86.4%) of the 22 patients who underwent the procedure. One patient had an unbranched stricture, 16 had a fork-shaped stricture, 1 had a trident-shaped stricture, and 1 had a stricture with more than three branches. Fourteen (73.7%) of the patients with strictures were treated endoscopically by inserting inside stents ranging from 7 F to 12 F in size, three underwent a Roux-en-Y hepaticojejunostomy to repair their stricture, and two were closely observed as outpatients. Of the 14 patients who were treated with the inside-stent, only 1 had acute cholangitis immediately after the procedure and underwent a Roux-en-Y hepaticojejunostomy. The other 13 patients who were treated with the inside stent have not required surgical repair for as long as an average of 586 days. CONCLUSION: Endoscopic placement of an inside stent is useful for treating biliary strictures in patients who have undergone right-lobe LDLT with duct-to-duct reconstruction.  相似文献

Objectives

We sought to examine biliary complications in adult right-lobe living donor liver transplantation (LDLT) with duct-to-duct anastomosis (RL-LDLT-DD), evaluating the efficacy of endoscopic retrograde cholangiography (ERC) in the diagnosis and management of biliary complications following LDLT.

Methods

Ninety adult RL-LDLT-DD were performed from June 2004 to August 2007, including 21 (23.3%) cases of biliary complications.

Results

The endoscopic retrograde cholangiopancreatiography (ERCP) findings were stricture only (n = 8), stricture plus leakage (n = 9), and leakage only (n = 4). In the overall 13 cases of leakage, nine patients recovered after treatment by stent or endoscopic nasobiliary drainage. The time to resolution was 3.0 ± 1.3 months with 2.2 ± 1.3 endoscopic examinations. All bile duct complications were treated by ERC first. Among 17 cases with stricture, seven cases were successfully treated by endoscopy and three cases by percutaneous transhepatic cholangiography plus stent (PTCS). In the other seven cases, the treatment was still ongoing in five cases and two subjects died during treatment. The mean time to stricture resolution 7.2 ± 3.3 months with 3.9 ± 1.4 endoscopic examinations. The results of 21 cases were 5/21 mortalities (23.8%), successful ERC treatment in 9/21; (42.9%), successful PTCS treatment in 3/21 (14.3%), and ongoing ERC treatment in 5/21, (23.8%), including one case with successful ERC treatment who died of lung infection postoperatively. During follow-up (13.1 ± 9.9 months), there was no recurrence in the stricture or leak.

Conclusions

When compared with the literature, RL-LDLT-DD without biliary drainage does not increase the incidence of biliary complications. From our study, ERC and PTC play a complementary roles in the treatment of bile duct complications.  相似文献

目的:探讨内镜下逆行胰胆管造影术(ERCP)在治疗肝移植术后胆道并发症方面的临床疗效.方法:回顾性分析2002年8月-2012年12月采用ERCP治疗8例肝移植术后胆道并发症患者的临床资料,其中胆道狭窄5例(吻合口狭窄4例,肝内型胆道狭窄1例),胆瘘1例,胆石和胆泥形成2例.8例患者共行ERCP治疗21次,对胆道狭窄患者行括约肌切开、胆管扩张、鼻胆管引流和内支架置放术等治疗;对胆瘘患者行鼻胆管引流及塑料内支架置放术等治疗;对结石患者行括约肌切开、鼻胆管冲洗引流术及取石网篮取石等治疗.结果:ERCP手术成功率为100% (21/21);4例吻合口狭窄、1例胆瘘和2例结石患者均治愈,1例肝内型胆道狭窄治疗未成功,建议再次肝移植;术后胆道感染的发生率为14.3%(3/21),胰腺炎发生率为19.0% (4/21),经对症治疗后均痊愈.结论:ERCP是治疗肝移植术后胆道并发症微创、安全和有效的方法.  相似文献

Forty-five patients with biliary atresia were accepted for orthotopic liver transplantation. Nine patients died awaiting transplantation, and 36 underwent transplantation. A portoenterostomy had been performed in 28 of these 36 patients, and its presence did not significantly affect the intraoperative blood loss (5.6 vs 4.1 blood volumes), the need for retransplantation (21% vs 12%), biliary complications (21% vs 12%), postoperative infections (36% vs 25%), or survival (82% vs 63%). These results indicate that early portoenterostomy is appropriate early therapy for biliary atresia; however, prompt referral to a liver transplant center for evaluation at the first sign of cholestasis is needed to attain optimal results for transplantation. Revisions of the portoenterostomy prior to transplantation did not improve the longevity of the procedure but did substantially increase complications and death after orthotopic liver transplantation.  相似文献

BACKGROUND: Biliary anastomotic strictures are a common complication of liver transplantation, occurring in up to 7% of patients at our center. Endoscopic therapy has started to replace surgical biliary reconstruction as the favored means of managing these patients in some centers, although the utility of this approach has never been tested in the setting of a standardized prospective study. METHODS: This was a standardized, prospective observational study in the liver transplantation unit, Queen Elizabeth Hospital, Birmingham, United Kingdom. Between June 2000 and August 2006, a total of 791 adults underwent liver transplantation at the Birmingham liver unit and 53 patients were diagnosed with biliary anastomotic strictures. All 53 patients chose to undergo endoscopic therapy and were managed according to the unit's standardized treatment protocol. Data and information from the patient records was collated prospectively, stored in a specific database, and analyzed by intention-to-treat. RESULTS: Endoscopic therapy was successful in 69% of patients referred with anastomotic strictures with a median stent free follow up of 18 months. Most patients required a median of 3 endoscopic procedures and two 24F balloon dilatations to adequately treat the stricture. The median continuous indwelling stent period was 11 months. Two patients were re-stented because of jaundice although only one patient had recurrence of the anastomotic stricture (3%). CONCLUSIONS: Endoscopic balloon dilatation and stenting is a safe and effective means of treating biliary anastomotic strictures complicating liver transplantation.  相似文献

BACKGROUND/PURPOSE: The aims of this study were to characterize the features of the biliary complications that occur after right-lobe living-donor liver transplantation (RL-LDLT) with duct-to-duct biliary anastomosis, and to evaluate the efficacy of treating biliary complications endoscopically. METHODS: The records of 273 consecutive patients who underwent RL-LDLT with duct-to-duct biliary anastomosis from July 1999 through July 2005 at Kyoto University Hospital were reviewed to determine the overall incidence of postoperative biliary complications and the outcome of endoscopic repair of those complications. RESULTS: Biliary complications occurred in 93 (34.1%) of the patients. These complications were: 80 biliary strictures (75 anastomotic and 5 nonanastomotic) and 16 biliary leakages (5 patients with biliary leakage also had a biliary stricture); most (72%) of the anastomotic strictures were complex (i.e., fork-shaped or trident-shaped). The strictures and leakages were repaired by the endoscopic placement of multiple inside stents above the sphincter of Oddi, and by nasobiliary drainage, respectively. The procedure was successful in repairing 51 (68.0%) of the anastomotic strictures and 8 (50.0%) of the biliary leakages. CONCLUSIONS: Endoscopic stenting of the bile ducts is efficacious in treating biliary complications related to RL-LDLT with duct-to-duct biliary anastomosis and the stenting should be attempted before surgical revision of strictures and leakages.  相似文献

目的探讨肝移植术后并发胆管结石受者行内镜逆行胰胆管造影术(ERCP)治疗的安全性和有效性。 方法回顾性分析南京军区福州总医院肝胆外科2005年1月至2015年12月肝移植术后并发胆管结石受者的临床资料。24例受者胆管结石确诊主要依据T管造影、MRCP或ERCP。确诊胆管结石受者均采用ERCP下行球囊扩张联合胆道塑料支架置入治疗,术中采用地西泮镇静,同时密切监测生命体征。观察肝移植术后并发胆管结石受者结石类型、狭窄情况、ERCP治疗情况及其治疗前后肝功能指标变化。采用Wilcoxon符号秩和检验比较ERCP治疗前后受者血清总胆红素(TBil)、碱性磷酸酶(ALP)、谷氨酰转肽酶(GGT)、ALT和AST水平变化。P<0.05为差异有统计学意义。 结果24例受者中胆总管结石20例(包括单纯胆总管结石11例、胆总管结石合并胆管狭窄9例),肝内、外胆管结石4例。肝移植至并发胆管结石平均间隔时间(604±215)d。19例发生在术后12~66个月,余5例发生在术后3个月内。11例胆总管结石受者采用柱状球囊扩张+取石篮取石+胆总管置入内支架引流治疗,治疗有效。9例胆总管结石合并胆管狭窄受者采用柱状球囊扩张+取石篮取石+胆总管置入内支架+鼻胆管引流治疗,其中8例治疗有效;1例因重度胆管狭窄,反复内镜取石不能取尽,继发感染再次行肝移植。4例肝内、外胆管结石受者均采用柱状球囊扩张+取石篮取石+左、右肝管置入内支架+鼻胆管引流治疗,治疗有效。受者内镜治疗后血清TBil、ALP和GGT分别为31、179和247 mmol/L,均低于内镜治疗前水平(43、273和385 mmol/L),差异均有统计学意义(z=0.042、0.001、0.004,P均<0.05)。截至2017年12月,24例受者随访时间为1~2年,4例因原发性肝癌复发分别于肝移植术后9、5、34、25个月死亡,1例因上消化道出血于肝移植术后34个月死亡,1例因重度胆管狭窄行二次肝移植并于2014年4月因肝脓肿继发感染性休克死亡,1例因感染性休克于肝移植术后33个月死亡,其余17例随访期间未见结石再发。 结论内镜下行球囊扩张联合塑料支架置入治疗原位肝移植术后并发胆管结石安全、有效,可作为目前原位肝移植术后并发胆管结石的首选治疗方案。  相似文献

目的 研究肝移植后胆道并发症的诊断和处理及分析相关因素。方法 回顾性研究Pittsburgh移植中心96例肝移植病人。结果 94例(97次移植)存活2d以上的病人,92例为端端+T管的胆道吻合,随访时间为5.8个月(0.3-10.2个月)。分析发现92例病人中8例有胆道并发症(8.51%);T管拔除时胆漏2例,术后早期胆漏2例,狭窄2例。75%胆道并发症有诱因,诱因:肝动脉狭窄2例,其中1例合并严重排斥反应;肝动脉血栓3例;供一受体胆管直径不匹配1例。冷缺血时间无显著性差异。5例有肝动脉血栓和(或)狭窄>50%行再移植,另3例无肝动脉血栓和(或)狭窄<50%经皮穿刺和内窥镜+支架或行气囊扩张,所有病人均获得良好疗效。结论 肝移植术后胆道并发症发生率为8.51%(胆-胆端端吻合+T管),胆道狭窄晚于胆漏,肝动脉检塞和(或狭窄是最重要的相关因素;无肝动脉栓塞和(或)狭窄,则无需手术治疗,若有肝动脉栓塞和(或)狭窄>50%,应尽早做再次肝移植。  相似文献

目的:探讨肝移植术后胆漏的诊断及治疗方法。方法:回顾分析我院6例肝移植并发胆漏病人(男4例,女2例)的诊断及治疗。胆管重建方式为胆总管端端吻合术。1例为肝脏移植术后第3天T管脱落.1例术后3个月拔除T管后出现胆漏,4例为吻合121漏,其中2例合并胆管狭窄。5例非手术治疗,1例手术治疗。结果:6例胆漏中5例治愈,1例并发感染死亡。结论:术后及时诊断并阻止胆漏.感染恶性循环是治疗胆漏的关键。胆漏多可经非手术治愈。经非手术治疗失败者,应予积极手术治疗。  相似文献

目的探讨原位肝移植术后非吻合口胆管狭窄(NABS)的预防和治疗措施。方法对2004年1月至2006年12月中山大学附属第一医院收治的516例同种原位肝移植病人的临床资料进行回顾性分析。总结肝移植术后发生NABS的情况。结果共发生NABS18例(3.5%),其中肝门部胆管狭窄9例,肝内胆管多发狭窄6例,肝内外胆管多发狭窄3例。18例病人采用给予反复的介入、内镜治疗、外科重建胆道及再次肝移植治疗。该组近期临床治愈率为55.6%(10/18),与NABS相关的再次肝移植率为38.9%(7/18),与NABS相关病死率为22.2%(4/18)。结论肝移植术后发生NABS临床处理棘手,应注重预防。NABS的治疗主要包括介入治疗和手术治疗,其中胆管介入治疗在NABS的临床处理中仍占重要地位,对于介入治疗、外科手术重建胆道等措施均无法控制其进行性发展的重度NABS病人,应把握好时机行再次肝移植。  相似文献

目的 探讨原位肝移植胆系并发症的预防及治疗。方法 回顾性分析1999年2月至2002年12月完成的95例次原位肝移植。门静脉、下腔静脉转流下原位肝移植12例,背驮式肝移植78例,活体部分肝移植5例。胆道重建:胆管对端吻合91例,放置胆道外引流55例,36例未放置胆道引流。胆管空肠Roux-en-Y吻合4例。胆管吻合及胆肠吻合应用5旬或6-0 Vicryl缝线或PDS缝线间断或连续缝合。术后以血清学检查、超声、胆道造影等手段随访。结果 共发生胆系并发症7例,发生率7.3%。2例术后7、10d吻合口胆漏,致胆汁性腹膜炎,再次手术引流。1例术后1个月胆管吻合口狭窄,内窥镜下支架内支撑治愈。2例拔除T管时胆漏,1例保守治愈,1例腹腔镜下缝合窦道、腹腔引流治愈。1例术后5个月因胆管消融、胆管炎死亡。1例术后10d因肝动脉血栓形成继发胆管坏死后死亡。74例随访1-42个月(平均11.4个月),胆道造影及(或)超声检查未见肝内外胆管狭窄,无胆泥、胆石形成,血清学检查提示肝功能状态良好。结论供肝切取时保护胆管黏膜及肝外胆管动脉供给,实现精细胆管黏膜对黏膜无张力对端吻合是预防胆系并发症的关键。内窥镜下狭窄段胆管扩张、内支撑是治疗肝移植肝外胆管狭窄的有效手段。  相似文献

Six hundred sixty-six patients received 792 liver transplants between February 1, 1984 and September 30, 1991. Biliary reconstruction was by choledochocholedochostomy (CDCD) with T-tube (n = 509) or Roux-en-Y choledochojejunostomy (CDJ) (n = 283). Twenty-five patients (4%) developed biliary strictures. Anastomotic strictures were more common after CDJ (n = 10, 3.5%) than for CDCD (n = 3, 0.6%). Intrahepatic strictures developed in 12 patients. Six patients had occult hepatic artery thrombosis (HAT). The other six patients received grafts in which cold ischemia time exceeded 12 hours. Anastomotic strictures were successfully managed by percutaneous dilation (PD) in five patients (n = 10), operation in three (n = 6), with retransplantation required in two patients. Intrahepatic strictures were managed by PD in seven, retransplantation in one, and expectantly in four patients. Of 25 patients, 19 (76%) are alive with good graft function. In three of six deaths, the biliary stricture was a significant factor to the development of sepsis and allograft failure. The authors conclude that (1) anastomotic strictures are rare after LT; (2) the development of biliary strictures may signify occult HAT; (3) PD is effective for most strictures; and (4) extended cold graft ischemia (less than 12 hours) may be injurious to the biliary epithelium, resulting in intrahepatic stricture formation.  相似文献

目的探讨经内镜逆行胆胰管造影术(ERCP)在诊断和治疗肝移植术后胆道并发症中的应用价值。方法对2001年4月至2004年12月经ERCP诊治的33例肝移植术后出现胆管并发症病人的临床资料进行回顾性分析。结果33例肝移植术后出现胆道并发症的病人共行ERCP46例次,明确诊断31例(93.9%),其中胆漏4例,胆管狭窄18例,胆管狭窄伴胆泥淤积或结石6例,单纯胆泥淤积或结石2例,Oddi括约肌功能失调1例。针对不同的诊断结果,采取了不同的治疗方式,治疗成功27例(81.8%),发生并发症5例次(10.9%)。结论内镜处理肝移植术后胆道并发症是一种安全、有效的方法,应该作为首选方法在临床上推广应用。  相似文献

目的探讨原位肝移植术后缺血型胆道病变(ITBL)的病因及预防、诊断和治疗的措施。方法回顾性分析1999年2月至2005年4月间291例次原位肝移植后发生ITBL患者的临床资料。结果291例次原位肝移植术后共发生ITBL 20例(6.9%)。术后发生ITBL的高危因素为:原发病为重型乙型肝炎、供受者ABO血型不符、供肝冷保存时间超过12h和术后肝动脉病变。其发生率分别为12.5%(9/71)、20.0%(2/10)、11.1%(9/81)和60%(3/5)。采用药物、经内镜逆行胰胆管造影(ERCP)介入、胆道外科手术及再次肝移植等方法治疗,有效率为80.0%(16/20)、治愈率为50.0%(10/20),与ITBL相关的病死率为10.0%(2/20),与ITBL相关的移植物功能丧失发生率为20.0%(4/20)。结论针对ITBL的高危因素进行相应处理是预防ITBL的有效措施。胆道造影和核磁共振胆胰管成像对诊断ITBL有很高的敏感性和特异性。根据不同的病因和病变程度采用适当的方法治疗ITBL,可获得良好的疗效。  相似文献

目的 探讨原位肝移植术后胆道并发症的发生原因及防治措施。方法 回顾性研究110例原位肝移植的临床资料。结果 110例病人术后共发生胆道并发症15例(13.6%)。其中单纯胆癌5例,迟发性弥漫性肝内胆管损伤3例,拔除T管后胆瘘2例,胆管空肠吻合口狭窄、肝内胆管结石、胆道狭窄合并胆泥形成、胆瘘后继发胆道狭窄、胆道狭窄合并肝内胆汁瘤形成各1例。除3例弥漫性肝内胆管损伤外,12例中2例死于严重胆道感染,其余10例经治疗痊愈。结论 原位肝移植术后胆道并发症病因复杂,治疗相对困难,因此首先应该注重预防,其次尽量做到早期诊断,针对不同的情况采取相应的治疗措施;重视ERCP和PTC等辅助性介入治疗方法。  相似文献

Abstract: Background: Biliary complications (BC) account for much of the morbidities seen after living donor liver transplantation (LDLT). Surgical reconstruction might be necessary after the failure of endoscopic or percutaneous procedures. Methods: Between November 2002 and December 2009, a total of 76 LDLTs were performed. Six patients were excluded from statistical analysis because of early graft or patient loss. Results: Of 70, 26 (37.1%) developed BC; 12 (46.2%) were successfully managed by non‐surgical procedures, three (11.5%) died from BC‐related sepsis, one (3.8%) died from BC‐unrelated causes, and 10 (38.5%) underwent surgical reconstruction. Of those 10, four patients had single duct reconstruction, five patients had double ducts reconstruction, and reconstruction was abandoned in one patient because of hepatic artery thrombosis. After a median follow‐up period of 4.5 yr (0.1–6), seven (70%) remained well with no recurrent biliary problems, and three (30%) had recurrent BCs that were managed either conservatively or by retransplantation. Patients who underwent surgical reconstruction had significantly fewer hospital admissions, less need for invasive procedures, and shorter cumulative hospital stay (p < 0.05). Conclusions: In our experience, BCs after LDLT were frequently resistant to non‐surgical procedures. Surgical reconstruction is associated with fewer hospital admissions and less need for invasive procedures leading to reduced resources utilization.  相似文献

Objective

To determine the effect of endoscopic sphincterotomy in the management of biliary hydatid disease.

Design

A case study between January 1992 and December 1994.

Setting

A university-affiliated hospital in Adana, Turkey.

Patients

Five patients with biliary hydatid disease, in which the cyst had ruptured into the biliary tree. The follow-up ranged from 3 to 12 months.

Intervention

Endoscopic sphincterotomy.

Main Outcome Measures

Morbidity, mortality and recurrence of the disease.

Results

All patients underwent successful endoscopic sphincterotomy, including removal of daughter cysts. During the follow-up period, ultrasonography and laboratory investigations showed complete cure in all patients. There were no complications due to endoscopic sphincterotomy.

Conclusion

Endoscopic sphincterotomy is the treatment of choice for the management of hydatid cysts that have ruptured into the biliary tract causing obstructive jaundice.  相似文献

Summary Endoscopic retrograde cholangiography (ERC) was performed in two patients with persistent bile fistulae after cholecystectomy. Contrast leakage was demonstrated in a leaking cystic duct stump and an aberrant intrahepatic duct. Endoscopic insertion of nasobiliary catheters led to rapid drainage and healing of the biliary fistulae.  相似文献
