OBJECTIVE: This study measured total energy expenditure (TEE) in symptomatic outpatient women with bulimia nervosa and normal controls. The study aimed to test the conceptual model of bulimia nervosa as an illness characterized by a physiological state of starvation, despite normal weight. METHOD: Total fat and fat-free mass were measured using hydrodensitometry and total energy expenditure was assessed via the doubly-labeled water method, in nine normal weight outpatient females with DSM-III-R bulimia nervosa and ten healthy female controls. RESULTS: Patients and controls were similar in age, body mass index, weight, lean body mass, and levels of exercise and general activity. Patients had an average baseline binge frequency of 14.7 episodes per week and purge frequency of 16.8 times per week, and had been ill for an average of 11.9 years. Group mean TEE did not differ between patients and controls (patients 2380 +/- 482 kcal/day, controls 2368 +/- 515 kcal day). Observed TEE in the bulimic subjects did not differ significantly from TEE predicted on the basis of data from the controls. DISCUSSION: This finding of normal TEE in symptomatic outpatients with bulimia nervosa is consistent with a previous study that found no difference in TEE in a sample of symptomatic inpatients with bulimia nervosa. These data suggest that the energy conserving metabolic adaptations characteristic of semi-starvation do not occur in patients with bulimia nervosa. 相似文献

Data from a year-long study conducted by the US Department of Agriculture's Beltsville Human Nutrition Research Center were used to identify sources of variation in daily energy intakes. A specific hypothesis was that an inverse relationship exists between past body weight and future energy intake whereas a direct relationship exists between past energy intake and future weight. Daily energy intakes of 29 male (n = 13) and female (n = 16) adult subjects were related through linear-regression analysis to sex (+ for males), age (-), height (+), weight (- for past and present measurements, + for future measurement), data collection method (- for duplicate plate), day of week (+ for Friday and Saturday), and month (+ for May, July, August, December, and January). These relationships were statistically significant (p less than 0.05). The results confirmed the hypothesis and were in general agreement with previously published studies that used larger samples but fewer days of data. 相似文献

A diet history method for estimating energy and N intakes was validated against 24 h urinary N excretion and energy expenditure measured by the doubly-labelled water (DLW) method. Forty-eight women aged 50-65 years were studied over 1 year. Weighed diet records from 4 d and two 24 h urine collections, for measurement of urinary N excretion, were obtained in each of four seasons. At the end of the year, a diet history was obtained, BMR was measured by whole-body calorimetry, and, in sixteen women, total energy expenditure (EE) was measured by DLW. Energy intake (EI) and N intake (NI) were calculated using food tables. Using weighed records and diet history respectively mean NI were 11.21 (SD 2.09) g and 11.47 (SD 2.40) g (NS) and EI were 8.08 (SD 1.54) MJ and 8.20 (SD 1.86) MJ (NS). Mean urine N:NI and EI:BMR values indicated bias to under-reporting by weighed record and diet history techniques in some individuals, but there was no significant difference between these measures at the group level. The Pearson correlation coefficient (r) for urine N v. NI was 0.81 for the weighed record and 0.38 for the diet history. The correlation of EE v. EI was r 0.48 for weighed record and r 0.11 for diet history. In this study the diet history gave the same estimate of mean intake, but the weighed record appeared to perform better in ranking individuals. 相似文献

OBJECTIVE: The purpose of this study was to determine whether energy expenditure estimated from physical activity and energy intake were equivalent to total daily energy expenditure in an elderly rural population. METHODS: Twenty-seven elderly male (n = 14) and female (n = 13) subjects (mean age, 74 y) were recruited from a rural Pennsylvania population. Over a 2-wk period, total daily energy expenditure was measured by doubly labeled water (TEE) and estimated from 7-d physical activity recall factors multiplied by weight (PA(WT)), estimated basal metabolic rate (PA(BMR)) and resting energy expenditure from indirect calorimetry (PA(REE)), and energy intake from 3-d self-reported diet records (EI). Analysis of variance was used to determine significant within-subject differences in physical activity, energy intake, and energy expenditure. RESULTS: PA(REE) (men: 13.69 +/- 3.23 MJ, women: 9.51 +/- 2.40 MJ) and PA(BMR) (men: 13.69 +/- 2.99 MJ, women: 10.15 +/- 2.21 MJ) were not significantly different from TEE (men: 12.43 +/- 1.63 MJ, women: 9.44 +/- 0.90 MJ). EI (men: 8.66 +/- 2.34 MJ, women: 7.12 +/- 0.93 MJ) was significantly less than TEE, and PA(WT) (men: 17.03 +/- 4.07 MJ, women: 12.86 +/- 3.41 MJ) was significantly greater than TEE. CONCLUSIONS: Whereas 7-d physical activity recall determined with an age- and gender-specific estimate of resting metabolic rate or measured using indirect calorimetry accurately estimated TEE for this group of rural elderly, self-reported diet records consistently underestimated and physical activity recall determined with weight alone consistently overestimated energy expenditure measured by doubly labeled water. 相似文献

To test the validity of survey techniques for measuring diet and activity patterns of Pima Indians, sequential 24-hour recalls, a food frequency questionnaire (FFQ), and an activity questionnaire were compared to free-living energy expenditure. Total energy expenditure (TEE) measured by doubly labeled water was 13.27 +/- 2.95 MJ/d for the 12 males (mean +/- SD: 35 +/- 14 yr; 97 +/- 35 kg; 32 +/- 9% body fat) and 11.67 +/- 1.85 MJ/d for the 9 females (31 +/- 13 yr; 106 +/- 32 kg; 49 +/- 6% body fat). Energy intake assessed by 24-hour recall was 13.59 +/- 7.81 MJ/d for men and 9.29 +/- 2.77 MJ/d for women, compared to 12.84 + 2.85 and 9.40 + 2.61 MJ/d for men and women, respectively, by FFQ. Both dietary methods indicated significant underreporting by women when compared to TEE. Energy intake assessed by FFQ was significantly correlated with TEE (r=0.48, p=0.03). This was true with 24-hour recall energy intake only when data from two extremely large alcohol consumers were eliminated (r=0.64, p=0.03, N=19). Although a low level of activity was apparent, the activity questionnaire produced significant correlations with measurements of energy expenditure and therefore represents an important tool for examining the relationship between physical activity and diseases. 相似文献

BACKGROUND: Energy requirements and nutrient intakes are commonly estimated from self-reported dietary records, but such estimation has proven to be unreliable. When energy intakes determined from dietary records are compared with energy expenditures measured with the use of doubly labeled water, the former consistently underestimate energy requirements and have a high degree of variability. OBJECTIVE: The objective of this study was to reduce the bias and variability of self-reported dietary records through the use of stepwise multiple regression analysis to develop models that relate energy expenditure measured with the use of doubly labeled water to energy intake from dietary records, sex, and fat-free mass (or weight and height). DESIGN: Data from 54 healthy adult men and women were used to develop these models. RESULTS: Fat-free mass, energy intake, and sex accounted for 86% of the variability in energy expenditure, whereas energy intake, sex, height, and weight accounted for 83%. When the model relating fat-free mass, energy intake, and sex to energy expenditure was tested on published data, it reduced the bias and variability of self-reported dietary records from -17 +/- 27% to 3 +/- 16%. When the model relating energy intake, sex, weight, and height to energy expenditure was tested on published data, it reduced the bias and variability of self-reported dietary records from -19 +/- 25% to -0.3 +/- 19%. CONCLUSION: Results from this study indicate that a simple relation can be used to correct self-reported dietary records to estimated energy requirements. 相似文献

Objective
This study attempts to describe trends in energy intake and weight change over 12 years according to age at first participation in the study.Design
Prospective cohort study.Setting
The National Institute for Longevity Sciences - Longitudinal Study of Aging (NILS-LSA), a community-based study.Participants
Participants included 922 men and 879 women who participated in the first study-wave (age 40–79 years) and also participated in at least one study-wave from the second to seventh study-wave. Each study-wave was conducted biennially. For individuals, the entire follow-up period was 12 years.Measurements
Energy intake was calculated from 3-day dietary records with photographs. Weight and height were measured under a fasting state. To estimate linear changes in energy intake and weight over 12 years according to age at first study-wave, we used the mixed-effects model. Results
Mean (SD) follow-up time and number of study-wave visits were 9.5 (3.7) years and 5.4 (1.8) times, respectively. The fixed effect of the interaction of age and time in energy intake and weight was statistically or marginally statistically significant both in men (p<0.01) and in women (p<0.06). In men, when energy intake was estimated according to age, the rate of decrease in energy intake increased from -6.8 to -33.8 kcal/year for ages 40-79 years. In women, the rate of decrease in energy intake slightly increased in older age groups (-9.1 to -16.7 kcal/year for ages 40-79 years). Weight increased in males in their 40s (0.07 kg/year from age 40) and started to decline by age 53. In women, weight started to decline around age 47 (-0.04 kg/year).Conclusion
Twelve-year longitudinal data showed energy intake declined both in men and women in their 40s, and the rate of decrease increased in older males. Weight started to decline in men in their mid-50s and women in their late 40s. Further studies that focus on energy intake and weight reduction are needed to prevent weight loss or underweight in an increasingly aging society.11.

BACKGROUND: Insulin-like growth factor I (IGF-I) has been implicated in several chronic diseases, including cancer, heart disease, and osteoporosis. OBJECTIVE: Our aim was to assess whether intakes of total energy, alcohol, vitamins, minerals, and foods rich in protein and minerals (including red meat, fish and seafood, poultry, and milk) are associated with serum IGF-I concentrations in middle-aged and elderly men. DESIGN: We measured serum IGF-I concentrations in 226 free-living healthy men aged 42-76 y. The average of fourteen 24-h dietary telephone interviews performed over 1 y was used to estimate long-term dietary intake. RESULTS: We observed statistically significant positive associations between intakes of protein (P for trend = 0.001) and zinc (P for trend = 0.002) and serum IGF-I concentrations after adjusting for age. The difference in mean IGF-I concentrations for the highest compared with the lowest quintile of intake was approximately 17% (162 microg/L compared with 139 microg/L) for protein and approximately 16% (166 microg/L compared with 143 microg/L) for zinc. Consumption of red meat (P for trend = 0.05) and fish and seafood (P for trend = 0.07) was modestly positively associated with IGF-I concentrations. Other dietary factors were not associated with IGF-I concentrations. CONCLUSION: In this population of healthy well-nourished men, greater dietary intakes of protein, zinc, red meat, and fish and seafood were associated with higher IGF-I concentrations. 相似文献

Chronic inflammation that occurs with aging is one of the risk factors for cardiovascular disease. Regular exercise may prevent cardiovascular morbidity by decreasing chronic systematic inflammation. Additionally, excess inflammation can be reduced by the anti-inflammatory protein pentraxin 3 (PTX3). Thus, both habitual exercise and PTX3 have an anti-inflammatory effect. However, it is unclear whether regular exercise leads to increased plasma PTX3 concentration. In the present study, we investigated the effects of regular aerobic exercise on plasma PTX3 concentration in middle-aged and elderly women. Twenty-two postmenopausal women (60 ± 6 years) were randomly divided evenly into 2 groups (i.e., exercise intervention and control). Subjects in the exercise group completed 2 months of regular aerobic exercise training (walking and cycling, 30-45 min, 3-5 days·week?1). Before and after the intervention, we evaluated plasma PTX3 concentration, peak oxygen uptake, blood chemistry, and arterial distensibility (carotid arterial compliance and β-stiffness) in all participants. There were no significant differences in baseline parameters between the 2 groups. Plasma PTX3 concentration was significantly increased in the exercise group after the intervention (p < 0.05). High-density lipoprotein cholesterol, peak oxygen uptake, and arterial compliance were also significantly increased (p < 0.05), while β-stiffness was markedly decreased (p < 0.01) after the intervention. On the other hand, there was no change in the parameters tested in the control group. This study demonstrates that regular aerobic exercise increases plasma PTX3 concentration with improvement of high-density lipoprotein cholesterol, peak oxygen uptake, and arterial distensibility in postmenopausal women. 相似文献

Two hundred sixty-six free-living human volunteers, 21-64 y old, were trained by dietitians to record daily their food intake for at least 7 d. Subsequently, they were fed diets of conventional foods adjusted in amounts to maintain their body weight for greater than or equal to 45 d. Comparing their estimated energy intake with the intake determined to maintain weight yielded mean differences of 2365 and 1792 kJ (565 and 428 kcal) in men and women, respectively, representing an underreporting of 18%. Twenty-two individuals (8%) overestimated and 29 (11%) were accurate to within 419 kJ (100 kcal) of their maintenance requirement. The remaining 215 individuals (81%) reported their habitual intake at 2930 +/- 1586 kJ (700 +/- 379 kcal) below that subsequently determined as their maintenance requirement. These findings suggest caution in the interpretation of food-consumption data. 相似文献

Effect of a high-protein, very-low-calorie diet on resting metabolism, thyroid hormones, and energy expenditure of obese middle-aged women 总被引:1,自引:0,他引:1
A 4-6 mo study was conducted to examine effects of a very-low-calorie, high-protein diet and realimentation on energy expenditure, resting metabolic rate (RMR), and serum thyroid hormones of obese women aged 30-54 yr. Fifteen healthy women, greater than or equal to 126% ideal body weight, were placed on the diet (420 kcal/day) and lost an average of 1.1 kg/wk until a predetermined goal weight was attained. RMR, triiodothyronine (T3), and reverse T3 decreased significantly (p less than 0.05). Thyroxine remained unchanged. Upon gradual realimentation onto solid foods, all metabolic parameters increased significantly within 5 wk toward pre-diet baseline values, but RMR (kcal/h) and T3 values remained significantly below pre-study values. Estimates of mean energy expenditure, utilizing a technique based on energy intake and body composition changes, averaged 1719 kcal/day during the diet period. 相似文献

The effect of energy supplementation was evaluated in six underweight adults under free-living conditions. Customary energy intake (EI) over 4 wk and estimated body composition were defined. Then for 12 wk subjects were fed their customary EI under controlled conditions to assure stable energy reserve. Finally, intake was increased by a mean of 720 kcal/d for 8 wk adjusted to increase body fat from 9 to 18%. Body weight (means +/- SD) increased from 55.2 +/- 3.4 to 57.0 +/- 4.2 kg. Body fat gain was highly variable ranging from 0.6 to 3.8 kg. Energy expenditure (EE) by the factorial and doubly labeled water methods did not change. Fat storage accounted for 66% of the supplemental energy; 237 kcal/d remained unaccounted for. EE by the factorial method, which uses indirect calorimetry or isotopic measurements, were highly correlated (r = 0.8; p less than 0.01). Under the conditions of this study EI does not affect EE. The labeled water method permits the evaluation of energy expenditure in free-living conditions. 相似文献

OBJECTIVE: To estimate the effect of a low energy diet on body fat mass measured by bioelectrical impedance (BIA) in overweight (BMI > or = 25 < 30) and obese women (BMI > or = 30). DESIGN: Randomised six weeks trial. METHODS: The studied group consisted of 86 women aged 20-67 attended the Outpatients Clinic of Metabolic Disorders. Dietary assessments were performed by 24-hour dietary recall. Body fat mass was measured by bioelectrical impedance method and by skinfolds thickness method. Percent body fat and fat free mass was estimated from equations based on BMI and skinfolds thickness. RESULTS: In overweight women after 6 weeks of dietary treatment mean intake of energy decreased from 10071 +/- 2678 kJ to 4560.6 +/- 1405.8 kJ, total fat intake from 88.7 +/- 33.9 g/d (33.4% of energy) to 38.8 +/- 19.9 g/d (31.1% of energy), protein intake from 89.5 +/- 36.5 g/d (14.8% of energy) to 50.7 +/- 16.9 g/d (19.0% of energy), carbohydrates intake from 312.7 +/- 106.6 g/d (51.8% of energy) to 134.5 +/- 53.7 g/d (50% of energy). In obese women mean intake of energy decreased from 10,376.3 +/- 2953.9 kJ to 4665.2 +/- 1380.7 kJ. The value of total energy, total fat and saturated fatty acids intake correlated with body weight, BMI and body fat. After dietary treatment the body weight decreased by 2.4 kg (3.3%) in overweight women and by 3.9 kg (4.1%) in obese women and percent of body fat decreased by 1.6% and 2.3%, respectively. Body fat mass determined by BIA method significantly correlated with skinfolds method by Siri, Schutte, Rathbun, Brozek, Keys-Brozek, and BMI method by Webster, Deurenberg. CONCLUSION: BIA method may be a helpful tool for the analysis of changes in total body composition occurring under obesity treatment. 相似文献

Parity is associated with mortality among middle-aged women, while substantially less is known about this relationship for
men and the elderly. Using the census-based Israel Longitudinal Mortality Study (ILMS) II (1995–2004) we sought to examine
the parity–mortality relationship among men and women, middle-aged and elderly. In our study cohort of 71,733 married men
and 62,822 married women ages 45–89 years at baseline, 19,437 deaths were reported. Mortality differentials by parity were
assessed using Cox proportional hazard regression models adjusted stepwise for age, origin, education and number of rooms.
Analyzes were carried out for middle-aged (45–64 years) and elderly (65–89 years) men and women separately. We observed a
non-linear relationship between parity and mortality for all individuals even after adjustment for demographic and socio-economic
variables. In fully adjusted models, for example, nulliparous middle-aged women experienced the highest mortality risks (hazard
ratios
[HR] = 1.57, 95% confidence intervals [CI] 1.24, 1.98) followed by those with one child (HR = 1.29, 95% CI 1.10, 1.51). These
results were attenuated somewhat for nulliparous older women (HR = 1.25, 95% CI 1.11, 1.41). The detrimental effects of low
and high parity on mortality among both men and women suggest a non pregnancy-related pathway that is likely mediated by biological
and psychosocial factors and other lifestyle characteristics that have long-term consequences into older ages. Further research
is warranted to examine the effects of parity by specific cause of death. 相似文献
