A case of Takayasu's disease in a 22-year-old woman who complained of severe fainting attacks is presented. Bilateral obstruction of the cervical arteries was confirmed by digital subtraction angiography. Preoperative technetium-99m hexamethylpropylene amine oxime brain SPET in the sitting position showed bilateral hypoactivity in the temporoparietal areas. Subtraction brain SPET showed slightly increased activity in the lying position. The patient has had no fainting attacks since bypass surgery. Postoperative 99mTc-HMPAO brain SPET in the sitting position showed normal activity except in the right temporoparietal area. This area was filled in the lying position. 99mTc-HMPAO brain SPET is the only technique that can visualize the cerebral blood flow in any position, this capability deriving on the fact that the distribution of 99mTc-HMPAO in the brain is fixed in the first 2–3 min following injection. The use of both sitting and lying 99mTc-HMPAO brain SPET is very useful for detecting an abnormality (i.e. an inhomogeneous response due to the fall in perfusion pressure) that could not be seen if the cerebral blood flow were to be assessed only in the lying position.

Regional cerebral blood flow was investigated in 14 patients with major depression diagnosed according to the DSM-III-R criteria (six patients with single and eight patients with recurrent episodes) and in ten healthy volunteers. The mean ages of the patients and the controls were 33.5 +/- 2.7 and 31.6 +/- 2.6 years, respectively. The severity of the depression was assessed using the 17-item Hamilton Depression Scale (mean: 23.2 +/- 1.5). None of the patients was under medication. After administration of 500 MBq technetium-99m hexamethylpropylene amine oxime, a single photon emission tomography study was performed and then transaxial, sagittal and coronal slices were obtained. For the semiquantitative analysis of the data, the ratios of the mean counts/pixel to the whole slice were calculated for 24 regions on three consecutive transaxial slices in the orbitomeatal plane. Additionally, left/right and frontal/occipital ratios were calculated. Both sides of the temporal region had a significantly decreased cerebral blood flow (CBF) when compared to the controls. The left/right ratio of the prefrontal region was also significantly lower in the patients than in the controls. The Hamilton score had a negative correlation with blood flow in the anterofrontal and left prefrontal regions. According to our results, regional CBF seems to be decreased in the left prefrontal and in both temporal regions in major depression. The severity of depression is correlated with the reduction in CBF in the regions of the anterofrontal and left prefrontal cortex. 相似文献

Regional hypoperfusion is a very frequent complication of subarachnoid haemorrhage (SAH), being related to vasospasm in the majority of cases. Twenty-six patients who were admitted for SAH underwent follow-up with technetium-99m hexamethylpropylene amine oxime single photon emission tomography (SPECT) 3 and 8 days after surgery. Fifteen patients of these had one more examination 15 days after surgery. The degree of hypoperfusion was quantified using an index of asymmetry which allow the comparison of two symmetrical regions of interest (ROIs) on the transaxial slice which presented the greatest perfusion defect. Comparison of CT data, transcranial Doppler data and clinical signs with the perfusion as quantified by99mTc-HMPAO SPECT indicates that a difference in counts of less than 10% between the two symmetrical ROIs is of no diagnostic value. Follow-up of the brain perfusion clearly shows that the most pronounced hypoperfusion was observed just after surgery, with progressive normalization at 8 and 15 days after surgery.99mTc-HMPAO SPECT performed 8 days after surgery allows prediction of the clinical outcome. For these reasons,99mTc-HMPAO SPECT, which is the only method for follow-up of cerebral perfusion in routine clinical practice, should be the first examination to be performed after surgery in patients with SAH. 相似文献

Objective Neurological complications have been reported in patients with sickle-cell disease (SCD) using positron emission tomography
(PET), magnetic resonance imaging (MRI), and computed tomography (CT), but not with single photon emission computed tomography
(SPECT). The objective of this study was to investigate brain perfusion in the patients with SCD using SPECT after technetium-99m
hexamethylpropylene amine oxime (99mTc-HMPAO), was administered and compare the findings with those of demography, physical examination, MRI and hematological
profile.
Methods The study involved 21 patients (12 males, 9 females, age at study 8–45 years) who were known to be having SCD for a duration
of at least 5 years. The patients were not in acute crisis and had normal neurological assessments with no known history of
stroke or transient ischemic episode or previous abnormal CT or MRI brain scan, and were right-handed. The brain SPECT was
performed after intravenous injection of 740 MBq (20 mCi) 99mTc-HMPAO in adults or an appropriate dose in pediatric patients. The scans were visually interpreted by two nuclear medicine
physicians and a decision was reached by consensus. An MRI done 3 months later was interpreted by a radiologist. The demographic
data and hematological profile were obtained from the medical records of the patients.
Results Of the 21 patients, 7 (age 11–22 years) had brain perfusion deficit mostly in the frontal lobe either alone or in combination
with temporal and/or parietal lobe. The MRI was abnormal in 2 patients. The brain perfusion deficit was not associated with
the demographic data of the patients or hematological profiles.
Conclusions The findings show that SPECT was useful in detecting brain perfusion deficit in SCD patients, and such an early detection
may be clinically useful in the subsequent follow-up of such patients, since it is known that cerebral perfusion deficit can
lead to silent infarct and/or overt stroke, and affect cognitive skills. 相似文献

Cerebral barotrauma, or the neurological manifestation of the "bends", is a relatively common disease of divers and aviators. To date, however, no-one has succeeded in demonstrating a cerebral or spinal cord lesion in vivo following a decompression incident, despite the presence of definitive clinical signs and symptoms of central nervous system involvement. This paper describes the use of 99Tcm-labelled hexamethylpropyleneamine oxime (99Tcm-HMPAO) with single photon emission tomography in a study of three individuals involved in driving accidents. All three suffered cerebral barotrauma during decompression and all exhibited clinical signs and symptoms of dysbarism to a varying degree. Imaging was performed at time intervals ranging from 2 h to several days following the incidents. The results showed well defined cerebral ischaemic lesions in all three subjects. We conclude that 99Tcm-HMPAO imaging provides a significant advance in locating and demonstrating cerebral lesions following barotrauma and will contribute greatly to our understanding of the pathophysiological processes involved. 相似文献

目的分析颅内动脉重度狭窄或闭塞后脑组织血流灌注情况,并评估预后。方法收集海军总医院神经内科2014年3月—2015年11月收治的25例单侧或双侧颅内动脉重度狭窄或闭塞患者,男性14例、女性11例,年龄22~71岁。所有患者经数字减影脑血管造影(digital subtraction angiography,DSA)或CT血管造影确诊,99mTc 双半胱乙酯单光子发射型计算机断层扫描完成脑血流灌注显像,分析患者一般临床特点及影像学资料。结果经DSA发现颅脑已建立侧支循环代偿供血系统13例。单侧皮质区血流灌注局限性减低22例,其中伴单侧基底节区血流轻度减低10例、伴单侧小脑灌注减低3例、伴单侧丘脑灌注减低9例;未见异常3例。入院时美国国立卫生研究院卒中量表评分平均1.2分,经治疗后美国国立卫生研究院卒中量表评分平均0.5分。结论采用单光子发射型计算机断层扫描脑血流灌注显像可有效评估颅内血管重度狭窄或闭塞后的局部脑动脉血流灌注,安全有效,尤其有助于进一步评估DSA检查未见局部代偿血管形成患者的脑组织血流灌注。 相似文献

The diagnostic potential of technetium-99m hexamethylpropylene amine oxime (HMPAO) following systemic administration of the cerebral vasodilator acetazolamide (acetazolamide test) was evaluated by regional cerebral blood flow (rCBF) single-photon emission tomography (SPET) in patients with Alzheimer's disease (AD) or vascular dementia (VD). An initial, high-resolution SPET study was performed with 99mTc-HMPAO, and after 2 days the patients were re-evaluated with 99mTc-HMPAO following systemic administration of acetazolamide. Reconstructed SPET slices were evaluated visually and semiquantitatively by a semi-automatic rCBF map method. When 99mTc-HMPAO alone was used, bilateral hypoperfusion was found in the temporal and/or parietal regions in 33% (6/18) of the VD patients and in 70% (23/33) of the AD patients. The corresponding data obtained by quantitative evaluation were 41% (7/17) and 71% (15/21), respectively. The vascular reserve capacity, as determined with the acetazolamide test, was preserved visually in 22% (4/18) and quantitatively in 29% (5/17) of the VD patients, but in 73% (24/33) and 76% (16/21) of the AD patients. The differences in the perfusion patterns between the VD and AD patients were statistically significant (P<0.01, Fischer's exact test). Of the VD patients with hypoperfusion (bilateral temporal and/or parietal), 4/6 (67%, visual evaluation) and 4/7 (57%, quantitative evaluation) had a decreased vascular reserve capacity as determined with the acetazolamide test. In the AD group of patients the corresponding results were 3/23 (13%) and 4/15 (27%). It is concluded that the acetazolamide test is promising in rCBF SPET to differentiate VD from AD. 相似文献

OBJECTIVE: Impaired cerebrovascular reactivity (CVR) to vasodilating agents is a predictor of the onset and prognosis of ischemic stroke. It is realized that the CVR improves or worsens when measured periodically during the clinical course in medically treated patients with occlusive cerebrovascular disease. In these patients, we investigated the possible relationship between the interval change in CVR and that in systemic blood pressure (BP). METHODS: Forty-two patients (14 females and 28 males, mean age +/- SD: 65.3 +/- 8.8 years) with severe stenosis or occlusion of the common carotid, internal carotid, or middle cerebral arteries repeatedly underwent single photon emission computed tomography (SPECT) studies using 123I-iodoamphetamine to measure cerebral blood flow (CBF) distribution and CVR at a more-than-6-month interval (mean +/- SD: 18.5 +/- 8.8 months). The CVR was separately estimated in cerebral hemispheres ipsilateral and contralateral to the most severe vascular lesion as the % increase in CBF after acetazolamide loading to CBF at rest. Systemic BP was measured four times at enrollment and the follow-up SPECT studies during resting and acetazolamide loading. Average BP at each SPECT study was an average of BP measurements during resting and acetazolamide loading. Interval changes in CVR were correlated with those in average systolic BP, average diastolic BP, and average mean arterial BP. RESULTS: The interval changes in CVR were significantly correlated with those in average diastolic BP in the ipsilateral hemisphere (y = 0.71x + 1.43, r2 = 0.11, p < 0.05) and in the contralateral hemisphere (y = 0.88x - 0.46, r2 = 0.16, p < 0.05) but not with those in average systolic BP or average mean arterial BP. CONCLUSIONS: In medically treated patients with steno-occlusive carotid artery or middle cerebral artery lesions, the interval change in CVR to acetazolamide by means of 123I-IMP SPECT was influenced by the diastolic BP at the SPECT studies. Monitoring diastolic BP is important to evaluate interval change in CVR. 相似文献

The quantitative assessment of coronary flow reserve (CFR) may be useful for the functional evaluation of coronary artery
disease (CAD), allowing judgment of its severity, tracking of disease progression, and evaluation of the anti-ischemic efficacy
of therapeutic strategies. Invasive techniques, such as intracoronary Doppler ultrasound and the pressure-derived method,
which directly assess CFR velocity and fractional flow reserve, have been used for the evaluation of the physiologic significance
of coronary lesions. Considerable progress has been made in the improvement of technologies directed toward the noninvasive
quantification of myocardial blood flow and CFR. Positron emission tomography has emerged as an accurate technique to quantify
CFR. The absolute measurements obtained with this noninvasive approach have been widely validated. Nevertheless, it has not
been applied to routine studies because of its high cost and complexity. On the other hand, technetium 99m-labeled tracers
have been largely used for the evaluation of myocardial perfusion with single photon emission computed tomography (SPECT)
imaging in patients with suspected or known CAD. Recently, attempts to estimate CFR with SPECT tracers have been made to obtain,
with noninvasive methods, data for quantitative functional assessment of CAD. This review analyzes the relative merit and
limitations of CFR measurements by cardiac SPECT imaging with Tc-99m-labeled tracers and describes the potential clinical

In this study, 99mTc-hexamethyl propyleneamine oxime (HMPAO) SPECT was used to evaluate the regional cerebral blood flow (rCBF) of the brain in patients with primary antiphospholipid antibody syndrome (PAPS). METHODS: Twenty-two women who were PAPS patients, aged 28-60 y, with mild neuropsychiatric manifestations and normal brain MRI findings were enrolled in this study. Brain HMPAO SPECT was performed to detect brain abnormalities. Meanwhile, serum anticardiolipin antibodies (ACA) and lupus anticoagulant (LA) were measured. RESULTS: HMPAO SPECT revealed hypoperfusion lesions in 16 of 22 (73%) PAPS patients. Cerebral cortex and cerebellum were the most and the least commonly involved areas, respectively. Eighteen of 22 (82%) and 14 of 22 (64%) patients had positive ACA and positive LA, respectively. ACA and LA results were related to HMPAO SPECT findings. CONCLUSION: HMPAO SPECT is a sensitive tool for detecting brain abnormalities in PAPS patients with only mild neuropsychiatric manifestations and normal brain MRI findings. 相似文献

The use of cerebral activating procedures within the scan protocol enables investigation of the mechanisms involved in specific brain functions in health and disease. Activation studies involve the comparison of at least two data sets describing brain activity generated in conditions that differ for the specific function in question. When designing an activation study, decisions regarding methodology include: the nature of the activation regime, the tracer-ligand utilized, the SPET instrument and the manner of subsequent data analysis. These issues are discussed in this review, both theoretically and with reference to published studies. Means of activating particular cerebral structures and functions are reviewed, as are the limitations of the techniques with respect to temporal and spatial resolution and the potentially confounding nature of preconceived ideas regarding the mechanisms of brain function.

The relationship between impairment of consciousness and quantitative cerebral blood flow (CBF) was investigated. The mean
CBF of the whole brain was measured by the Patlak-plot method using technetium-99m hexamethylpropylene amine oxime single-photon
emission tomography (99mTc-HMPAO SPET) in patients with the following diseases: cerebral infarction, intraparenchymal haemorrhage, subarachnoid haemorrhage,
brain tumour and cerebral contusion. The clinical symptoms were evaluated according to the severity of impaired consciousness,
aphasia and dementia. Four hundred and eighty-five CBF measurements were performed. Patients with alert consciousness showed
an age-related decline in mean CBF. Patients with aphasia showed a significant reduction in mean CBF compared with those without
aphasia. Impaired consciousness was proportional to reduction in mean CBF regardless of types of pathology, and the size of
lesion did not influence the mean CBF. Patients with dementia showed a significant reduction in mean CBF proportional to the
severity of dementia. The quantitative measurement of CBF using 99mTc-HMPAO SPET is reliable in clinical evaluations.

Brain single photon emission computed tomography (SPET) using 99mTcm-hexamethylpropyleneamine oxime (HMPAO) was performed in 37 patients satisfying the standard clinical criteria of Alzheimer's disease (AD), correlating results with patient's age of onset, duration of illness, severity of dementia, neuropsychological impairment, computed tomography (CT) and EEG findings. All patients had abnormal SPET: 23 were bilaterally symmetric, 10 were bilaterally asymmetric and four showed unilateral low regional cerebral perfusion. Low perfusion in bilateral parietal regions was the most consistent finding and was present alone or with other abnormal regions in 31 patients (84%). The only significant clinical correlation was found between asymmetric SPET changes and onset of disease before the age of 65 years. Fifty per cent of patients with presenile onset, but only 19% with senile onset had asymmetric low regional cerebral perfusion. Fifteen of the 36 patients had normal brain CT scans, and 9 of 31 patients receiving EEG examination had normal results. Although clinical evaluation is the most valuable diagnostic tool, brain SPET is a sensitive test for physiological investigation in AD. 相似文献

We report our initial experience with technetium-99m hexamethyl propylene amine oxime (99mTC-HMPAO) brain single photon emission tomography (SPET) in the investigation of 19 children presenting with febrile convulsions. Two patients with complex febrile convulsions showed focal SPET lesions contralateral to the neurological deficit. However, in 9 out of 17 patients with simple febrile convulsions, focally disturbed perfusion was shown. In 4 out of 6 patients with electroencephalogram (EEG) abnormalities on admittance, SPET revealed at least 2 focal lesions. The temporofrontal region was the one most commonly involved. The SPET findings presented here also suggest a temporal relationship with the febrile convulsions, with markedly fewer lesions if examined after 12 days. In our initial experience, perfusion SPET did not show any particular pattern helpful in the differential diagnosis of the child presenting with febrile convulsions. Physiopathologically, our findings may support the hypothesis that brain tissue is regionally more vulnerable to fever, in patients presenting with febrile convulsions. 相似文献

In the pre-operative assessment of patients being considered for temporal lobe surgery, accurate lateralization and localization of the epileptic focus is mandatory. Surface electroencephalography (EEG) can be misleading and confirmatory evidence, preferably non-invasive, of the site of the epileptic focus is valuable. Sixteen patients with partial epilepsy, six of whom were considered to be good surgical candidates, had technetium-99m hexamethylpropylene amine oxime (99Tcm-HMPAO) scanning. In those patients with poor epileptic control and a well localized EEG focus, there was a correlation between EEG and HMPAO scan abnormalities, whilst in all patients where no correlation existed, the HMPAO scan was normal. We conclude that 99Tcm-HMPAO single photon emission computed tomography may be a useful non-invasive, adjunctive investigation in the preoperative assessment of patients with partial epilepsy. 相似文献
