Closing remarks

Switch therapy, the switch from a parenteral to an oral antimicrobial agent, has been used successfully in the treatment of many serious infections. Several studies have found that significant cost savings can be achieved by switch therapy. Moreover, it has the further advantages of shortening hospital stay and reducing nosocomial bacteremia. With the exception ofStaphylococcus aureus and penicillin-resistantStreptococcus pneumoniae, cefixime has similarin vitro activity to cefotaxime and ceftriaxone. It has a prolonged half-life, allowing for once-a-day dosing, and has excellent tissue penetration (132%). These characteristics support the use of cefixime for switch therapy when a susceptible pathogen has been identified.Sequenztherapie mit Umsetzen von einem parenteralen auf ein orales Antibiotikum wurde bei vielen schweren Infektionen mit Erfolg eingesetzt. In verschiedenen Studien wurde festgestellt, daß mit Sequenztherapie eine erhebliche Kostenersparnis möglich ist. Darüber hinaus hat die Sequenztherapie den Vorteil, die stationäre Behandlung zu verkürzen und nosokomiale Bakteriämien zu vermindern. Cefixim hat eine ähnlichein vitro-Aktivität wie Cefotaxim und Ceftriaxon, mit Ausnahme vonStaphylococcus aureus und penicillinresistentenStreptococcus pneumoniae-Stämmen. Aufgrund seiner langen Halbwertzeit eignet sich Cefixim für die einmal tägliche Applikation. Seine Gewebepenetration ist ausgezeichnet (132%). Aufgrund dieser Eigenschaften ist die Anwendung von Cefixim in der Sequenztherapie bei Infektionen durch einen gesicherten empfindlichen Erreger gerechtfertigt.
