Anästhesieverfahren in der Dermatologie

Zusammenfassung Die Verfahren der Lokalanästhesie sind integraler Bestandteil der operativen Dermatologie. Sie gewährleisten eine effiziente und sichere Analgesie in umschriebenen Haut- und Weichteilregionen und ermöglichen, einen sonst schmerzhaften diagnostischen oder therapeutischen Eingriff bei erhaltenem Bewusstsein zu tolerieren. Einzelne Methoden der Applikation sind "konkurrenzlos", wie die topische Applikation von EMLA^® oder die Kryoanästhesie, andere bieten alternative Optionen zur Allgemeinanästhesie. Die Tumeszenzlokalanästhesie wurde—jenseits der kosmetischen Liposuktion—zu einer effizienten Anästhesieform für größere Operationen bei Tumoren der Haut, plastische Rekonstruktionen und in der Phlebochirurgie weiterentwickelt. Die Wahl des Verfahrens im Einzelfall wird vom Alter, der Kooperationsfähigkeit und der Komorbidität des Patienten bestimmt. Für Infiltrationsanästhesien werden heute vorwiegend Lokalanästhetika vom Amidtyp eingesetzt. Fundierte Kenntnisse über die Anatomie der sensiblen Nerven sind Voraussetzung für erfolgreiche operationsfeldnahe periphere Blockaden. Wenn die Wirkungsweise der Lokalanästhetika, ihre toxischen Effekte und potenzielle Arzneimittelinteraktionen bei ihrem Metabolismus in der Praxis beachtet werden, dann ist das Risiko von Komplikationen relativ gering. Es sollte dennoch nicht unterschätzt, und adäquate Notfallmaßnahmen im Operationsteam sollten regelmäßig trainiert werden.

Miracles which can be observed in different religions are often closely connected to medicine. These miraculous cures, considered to be inexplicable by medical science, have been documented in Christian religion and in occidental culture for more than 2000 years. These miraculous healings are also reported in the field of dermatology. The miraculous bulletins documented by the Catholic Church take their beginning in the Old Testament and extend to the miraculous cures of Lourdes in present time. We provide an overview of miraculous healings of skin diseases, whereby accounts documented in the Bible, reports during the Middle Ages, and cases documented throughout the last century are discussed. In view of modern medicine, most of the miraculous healings do not meet modern demands of science, but remain important as milestones in medical and religious history.     

Hochdosis-Steroid-Pulstherapie Gibt es Indikationen in der Dermatologie?

In recent years, high-dose steroid pulse therapy has been considered an effective treatment regimen in a number of non-dermatological diseases. This review addresses in the first part the pharmacological basics and mechanisms of action of high-dose glucocorticosteroid therapy. In the second part, we discuss the clinical experience using this treatment for dermatological diseases including pemphigus vulgaris, pyoderma gangrenosum, alopecia areata, and dermatomyositis.

Suizidalität in der Dermatologie

Even in dermatology one can potentially encounter suicidal patients. A risk of suicide can be preexisting, appear as complication of skin disorders or be triggered by medications such as interferons. Patients at risk must be specifically asked about suicidal ideations and tendencies. Acute suicide risk requires immediate crisis intervention. In dermatology suicide risk has been described in severe acne conglobata (especially men) and metastatic melanoma. Patients with chronic or potentially fatal disease or severe pain may be suicidal. In addition patients with depression, alcohol dependency, substance abuse, schizophrenia or borderline personality disorder are at special risk. We review psychodermatological diseases with risk of suicide and point out treatment strategies. More attention should be focused on the early recognition of a possible risk of suicide in dermatology patients.

Zusammenfassung Bei Notfällen in der Dermatologie handelt es sich um ein weites Spektrum von Erkrankungen, die eine umgehende stationäre oder auch intensivmedizinische Versorgung erfordern. Eine schnelle Diagnosestellung und umgehende Einleitung einer adäquaten Therapie sind notwendig, da einige dieser dermatologischen Erkrankungen mit einer hohen Mortalität oder mit schwerwiegenden Dauerschäden assoziiert sein können. Klassische Beispiele für dermatologische Notfälle sind: die bakteriellen Infektionen Waterhouse-Friderichsen-Syndrom, das staphylokokken- und streptokokkenbedingte toxische Schocksyndrom, die nekrotisierende Fasziitis und das nekrotisierende Erysipel. Einen weiteren besonderen Notfall stellen die Giftschlangenbisse dar.

In the everyday practice of dermatology, we encounter light in many ways. The biological and physical interactions between light and skin make light a potent diagnostic and therapeutic tool when well directed. This contribution intends to illuminate the possible uses of light-emitting diodes and high-energy flashlamps in dermatology. Both sources of light have only recently been employed in dermatology and in addition to broadband ultraviolet (UV) light sources, high-pressure gas discharge lamps, and halogen lamps enrich the armamentarium of incoherent light sources.

Because of reduced resources for health care, provision of modern effective dermatologic care for patients with German public health insurances has become more and more difficult. This survey is designed to aid in the assessment of published health economic studies as well as encourage participation in health economic evaluations. These studies should analyze and optimize the allocation of the available resources. Four main approaches can be used to calculate the incidental expenses; they differ in their methods of evaluation and assessment of health services. They all offer methods to calculate the anticipated costs and consequences. The methods include cost minimization analysis (CMA), cost effectiveness analysis (CEA), cost utility analysis (CUA) or cost benefit analysis (CBA). Many subgroups also exist. Important criteria for the evaluation of health economic trials and hints for the publication of the results are provided on the basis of the recommendations for health economic evaluations.
