Granuloma anulare disseminatum – erfolgreiche Therapie mit Fumarsäureester

A 64-year old female patient was treated for a therapy-resistant generalized granuloma annulare with fumaric acid esters (Fumaderm initial). One week following begin of therapy with an initial dose of 30 mg per day, the papules had regressed significantly. A 6-week therapy with a final dose of 90 mg Fumaderm initial per day led to a nearly complete healing of the illness.

Erfolgreiche Therapie eines ILVEN bei einem 7jährigen Mädchen mit Calcipotriol

A 7-year-old otherwise healthy girl presented with a 2-year history of an ILVEN (inflammatory linear verrucous epidermal nevus) located on the inner part of her right upper arm. The diagnosis was histologically confirmed. Different conservative therapeutic strategies with corticosteroids, antibiotics and antimycotics produced little or no improvement. Because of encouraging reports describing the successful use of 0.005% calcipotriol ointment in patients with ILVEN, we treated our patient with this regimen. After 4 weeks we could recognize a impressive improvement and after 8 weeks the ILVEN had nearly completely disappeared. 25 weeks after withdrawal of calcipotriol, no relapse had occurred. The dramatic response to calcipotriol suggests some pathological links between ILVEN and psoriasis.     

Purpura Schönlein-Henoch – Erfolgreiche Therapie mit Dapson

A 26-year old patient who suffered from purpura for four years evolved into the complete clinical picture of Henoch-Sch?nlein-purpura in the past two years. A causative agent was never found. Multiple therapeutic approaches including systemic steroids, azathioprine as well as pentoxyfylline failed to control the arthralgias, intestinal symptoms and skin lesions. Therefore dapsone therapy was initiated at a dosage of 100 mg daily after checking the glucose-6-phosphate-dehydrogenase and with regular control of the blood methemoglobin level. Within two weeks, the patient's symptoms completely cleared, he has now been in remission for the past six months. As noted elsewhere, dapsone is an effective therapy for severe cases of Henoch-Sch?nlein- purpura.     

Vergleichende ph-Messungen an der Hautoberfläche mit einer Chinhydron- und einer Glaselektrodenkette

Zusammenfassung Auf Grund von etwa 2000 Einzelmessungen mit einer Glas- und einer Chinhydronelektrodenkette wird bewiesen, daß die mit beiden Meßsystemen gefundenenpH-Werte vollständig übereinstimmen.Die Messungen wurden an hautgesunden und -kranken Patienten an der Unterarm-Beugeseite, an der Unterarm-Streckseite, in der Handinnenfläche und am Handrücken durchgeführt. Die Differenzen innerhalb der einzelnen Untersuchungsreihen betrugen im HöchstfallpH 0,04, lagen also größenordnungsmäßig weit im Bereiche der Meßfehlergrenzen.Auf die Notwendigkeit einer genügend großen Anzahl von Einzelmessungen in einer bestimmten Hautregion zur Feststellung von Durchschnittswerten bei der Haut-pH-Messung wird hingewiesen, die infolge der Mosaikstruktur des Säuremantels vorhandene Streubreite der Einzelwerte durch entsprechende Untersuchungen mit einer Glaselektrode in zwei Serien zu je 6 Einzelmessungen aufgezeigt.Die Glaselektrode steht in der Meßgenauigkeit gleichberechtigt neben der Chinhydronelektrode. Auf gewisse technische Vorbedingungen, die zur exakten Messung für beide Elektrodensysteme erforderlich sind, wird hingewiesen. Kugelige Glaselektroden eignen sich nicht zur Haut-pH-Messung.Die Tatsache, daß diepH-Messung der Hautoberfläche mit zwei völlig verschieden arbeitenden Meßelektroden gleichepH-Werte ergab, wird als ein Beweis für die Zuverlässigkeit der bisherigenpH-Messungen mit Chinhydronelektrodenketten betrachtet.Assistent der Dermatologischen Klinik und Poliklinik der Universität München.

Fumars?ureestertherapie bei einer jungen Patientin mit ausgepr?gter Cheilitis granulomatosa

Granulomatous cheilitis is a rare granulomatous inflammation of the lips of unknown origin; mainly young adults are affected. So far, there is no generally effectual treatment available for this disfiguring dermatosis. We show the efficacy of a treatment with fumaric acid esters reporting the case of a 14-year-old girl with granulomatous cheilitis resistant to previous therapy. Our successful therapy consisted of fumaric acid esters according to the therapeutic schedule for psoriasis and showed a good tolerance subjectively and objectively.     

Multiple Mantelome bei Birt-Hogg-Dubé-Syndrom Erfolgreiche Therapie mit dem CO2-Laser

Birt-Hogg-Dubé-syndrome, first described 20 years ago, is an autosomal dominant inherited disease, characterized by the occurrence of multiple cutaneous hamartomas with adnexal differentiation. An association with intestinal neoplasms has been mentioned repeatedly. Recently, the cutaneous lesions in this syndrome have been interpreted as different developmental stages of one single hamartoma with sebaceous differentiation, called mantleoma. Numerous dermatohistopathologic studies contribute to the certain diagnosis of Birt-Hogg-Dubé-syndrome. In contrast, little information is available on the therapy of the multiple skin lesions. A father and his daughter with Birt-Hogg-Dubé-syndrome were treated with the CO2 laser, producing satisfactory results.

Niedrig dosierte prophylaktische orale Therapie mit Isotretinoin über 18 Jahre bei einer Patientin mit Epidermodysplasia verruciformis und zahlreichen Plattenepithelkarzinomen

Epidermodysplasia verruciformis (also known as Lewandowsky–Lutz dysplasia) is an extremely rare autosomal recessive genodermatosis, in which the skin is unusually sensitive to human papilloma viruses (HPV). It is associated with a high risk of developing non-melanocytic skin tumors. Treatment with keratolytic retinoids is currently considered to be the most effective therapy. Retinoids have a broad spectrum of activity and inhibit the growth of squamous cell carcinoma and other malignant tumors. We report the case of an 81-year-old woman who had been receiving prophylactic treatment with oral isotretinoin at a daily dose of 1.0–0.33?mg/kg bodyweight for about 18 years because of her epidermodysplasia verruciformis (HPV types 9 & 57 detected). We observed a reduction of the incidence of squamous cell carcinomas of the skin and presume a causal relationship between the treatment with this retinoid and the reduction of squamous cell carcinomas of the skin in this patient.

Probiotika und Präbiotika zur Prävention und Therapie des atopischen Ekzems

The rapid increase of atopic diseases, particularly in western industrialized countries, demands comprehensive and cost-effective primary prevention. Existing findings regarding the use of probiotic lactic acid bacteria for the prevention of atopic eczema are promising. Therapeutic use appears to be more promising for infants with mild to moderate skin lesions and elevated immunoglobulin E levels than for older patients without any sensitization. Depending on the original bacterial count, prebiotics such as fructooligosaccharides and galactooligosaccharides cause an increase of bifidobacteria within the colon. The benefit of this increase in bifidobacteria in allergic diseases is unclear. In patients with atopic eczema a correlation was shown between the amount of bifidobacteria and the severity of atopic eczema.     

Disseminierte ischämische Nekrosen und Livedo racemosa bei einer Patientin mit Kalziphylaxie nach Langzeitdialyse

Calciphylaxis occurred in a 40-year old female patient with end-stage renal failure. The patient developed livedo racemosa ("livedo reticularis") with painful skin necrosis and ulcers involving multiple areas of the hip and legs after 22 years of hemodialysis. X-ray-examinations revealed calcinosis of peripheral arteries, especially of the pelvis, thigh and hands, while histological examinations showed a fibrosis and calcinosis of small subcutaneous arteries. A generalized cutaneous microangiopathy could be demonstrated by transcutaneous oxygen pressure measurements. Laboratory data showed a moderate secondary hyperparathyroidism with mild elevation of calcium-phosphate product. In addition to the hemodialysis an attempt was made to improve the microcirculation by vasoactive drugs. The clinical course was characterized by slow healing of the ulcers and occurrence of new areas of cutaneous necrosis. Calciphylaxis is a rare late complication in patients with advanced, often end-stage renal failure. It has characteristic histopathological features and is frequently, but not always, associated with a disturbed calcium and phosphorus metabolism and mildly elevated levels of parathyroid hormone. Calciphylaxis is classified as a special type of metastatic calcinosis.
