Chordomas of the skull base: MR features.   总被引:9,自引:0,他引:9  
PURPOSE: To characterize the MR features of skull base chordomas with regard to signal intensity, size, position, extension, and Gd-DTPA enhancement. PATIENTS AND METHODS: The MR imaging studies of 28 patients with surgically proven chordomas of the skull base were retrospectively reviewed. Twenty-two of these patients received intravenous administration of Gd-DTPA. RESULTS: On short TR/short TE images, chordomas generally had low to intermediate signal. On long TR/long TE images, chordomas generally had very high signal that was heterogeneous in 79%. After Gd-DTPA administration, all chordomas demonstrated some degree of contrast enhancement. In most cases, enhancement was demonstrated throughout most of each tumor in a heterogeneous pattern. Chordomas were associated with MR findings of displacement and encasement of vessels, and frequent extension into adjacent structures such as the cavernous sinus, sella, nasopharynx, and hypoglossal canal. CONCLUSION: The MR characterization of the position and extent of these neoplasms played an important role in determining the optimal surgical approaches for gross total tumor resection.  相似文献

Painful sickle cell crisis: bone marrow patterns observed with MR imaging   总被引:5,自引:0,他引:5  
Eleven patients with homozygous sickle cell anemia (SCA) undergoing painful crisis were studied with magnetic resonance (MR) imaging. The signal intensity of bone marrow was diffusely decreased in the axial and peripheral skeleton on short repetition time (TR)/echo time (TE) images and long TR/TE images, which suggested hematopoietic marrow hyperplasia and was confirmed by isotope marrow scans in five patients. Focal areas of further decrease in signal intensity were seen on short TR/TE images in 12 of the 14 (86%) painful joints and three of the five (60%) painless joints. In the painful joints, these focal areas converted to high signal intensity on long TR/TE images, presumably due to edema, which suggested acute marrow infarction. In the painless joints, these low-intensity focal areas remained as low signal on long TR/TE images, which suggested absence of edema and thus areas of old infarction or fibrosis. These results indicate that MR imaging may enable differentiation between acute and chronic marrow infarcts in patients with SCA and serve as a useful guide in monitoring and directing therapy.  相似文献

The authors examined the magnetic resonance (MR) appearance of inverted papillomas to determine if this histologically benign lesion could be distinguished from malignancies of the sinonasal cavity. MR images in 10 patients with histologically proved inverted papilloma were retrospectively reviewed. The signal intensity of inverted papillomas on short repetition time (TR) images was iso- to slightly hypertintense to muscle in all 10 patients. Inverted papillomas had intermediate signal intensity on the long TR/echo time (TE) images. The tumors were iso- or slightly hypointense to fat on long TR/short TE images. In the seven patients who received gadopentetate dimeglumine, all inverted papillomas showed solid inhomogeneous enhancement. A review of eight sinonasal malignancies showed no distinctive signal intensity or enhancement characteristics to help differentiate inverted papillomas from various malignant tumors. The authors conclude that there is no signature MR appearance for the benign inverted papilloma. The main utility of MR imaging is in defining the extent of the lesion.  相似文献

Forty-one patients with migraine headaches were referred for MR imaging of the brain. Intermediate and T2-weighted images were obtained to determine the frequency of areas of high intensity within the white matter. The average age of these patients was 29.8 years; only five were over 40 years old. Each patient was evaluated in the axial plane with long TR/short and long TE images. Twenty-three patients also had T1-weighted short TR/short TE MR scans; enhancement with gadopentetate dimeglumine was used in three patients. Intracranial abnormalities were seen in only six patients: foci or white matter high intensity on intermediate and T2-weighted images in five and a venous angioma in one. Prior studies have indicated that parenchymal brain abnormalities may be found in up to 46% of patients with migraines. The current study demonstrated parenchymal brain lesions in only 12%. This study suggests that the frequency of foci of high intensity seen on long TR sequences in the migraine patient is much lower than previously reported, especially in patients under 40 years old (5.5% in our series).  相似文献

Recurrent thyroid carcinoma: characteristics on MR images   总被引:1,自引:0,他引:1  
Magnetic resonance (MR) imaging was used in 32 patients, including eight with benign disease, after partial or total thyroidectomy to determine sensitivity and specificity of MR imaging in the detection of tumor recurrence, to compare signal intensities of scar versus recurrent tumor qualitatively and quantitatively, and to define the extent of recurrent tumor. Findings from surgery (n = 23), needle biopsy (n = 1), or clinical follow-up (n = 8) were used for verification. Of 24 patients with primary thyroid carcinoma, 15 had recurrence and nine had a normal postsurgical thyroid bed. Diagnosis from MR images was correct in 20 cases, but false positive in three and false negative in one. Local recurrence was characterized by low to medium intensity on short repetition time (TR)/short echo time (TE) images and medium to high intensity on long TR/long TE images. Scar in the normal postsurgical thyroid bed showed low intensity on both short and long TR/TE images. Local recurrence of thyroid carcinoma and lymph node metastasis produced positive contrast compared with muscle on short TR/short TE (31 + 19%) and long TR/long TE (85 + 30%) images; fibrosis produced negative contrast, particularly on long TR/long TE (-56, -80%) images. These results indicate the capability of MR imaging in the evaluation of recurrence of thyroid tumors and in the differentiation of abnormal tissue due to tumor recurrence from postoperative fibrosis by means of signal contrast relative to a reference tissue.  相似文献

Ten patients with intracerebral metastases from malignant melanoma were evaluated with magnetic resonance (MR) imaging performed at 1.5 T using spin-echo techniques. On the basis of histopathologic findings in three of 10 cases and CT appearances in all 10 cases, three patterns were identified on analysis of MR signal intensities in both short repetition time/echo time (TR/TE) and long TR/TE spin-echo scans. In comparison to normal cortex, nonhemorrhagic melanotic melanoma appeared markedly hyperintense on short TR/TE images and isointense, mildly hypointense on long TR/TE images. Nonhemorrhagic, amelanotic melanoma appeared isointense or mildly hypointense on short TR/TE and isointense or mildly hyperintense on long TR/TE images. Hemorrhagic melanoma varied in appearance, depending on the stage of hemorrhage. Melanotic, nonhemorrhagic melanoma can be distinguished from early and late subacute hemorrhage by its signal intensity on long TR/TE images. Spin-echo MR appears to be the method of choice for diagnosing melanotic metastases.  相似文献

Pigmented villonodular synovitis: a report of MR imaging in two cases   总被引:3,自引:0,他引:3  
Although magnetic resonance (MR) imaging has been used to evaluate many musculoskeletal lesions, the MR appearance of pigmented villonodular synovitis (PVNS) has not been described in detail. The authors describe two cases of PVNS in the knee imaged with both computed tomography and MR. In both cases parts of each lesion had very low signal intensity on both short repetition time (TR)/echo time (TE) sequences and long TR/TE sequences. Other portions of both lesions had intermediate signal intensity (equal to or higher than that of muscle but lower than that of fat) on short TR/TE sequences and increasing signal intensity on longer TR/TE images. In one case, the lesion also had a cystic component that showed MR changes typical of complex fluid. The authors propose that the MR signal characteristics demonstrated in these cases may be explained by the unique tissue components of the lesion, particularly hemosiderin and fat.  相似文献

Recent experience has shown that parathyroid adenomas vary in their MR signal intensity, which raises the question of whether the signal intensity is related to different histologic characteristics. In order to address this question, 10 patients who had MR imaging studies (four at 0.35 T, six at 1.5 T) showing large- to medium-sized parathyroid adenomas and who subsequently underwent surgery with histologic proof of the lesion were evaluated. The MR appearance was compared with histologic characteristics. The adenomas were classified into three groups according to the MR appearance: group I, low signal intensity on short TR/TE images, high signal intensity on long TR/TE images (n = 5); group II, low signal intensity on short and long TR/TE images (n = 3); group III, high signal intensity on short and long TR/TE images (n = 2). Histologic analysis revealed that the major features of each group were different. High cellularity without degeneration or fibrosis was observed for all five adenomas from group I. In group II, all three adenomas showed cellular degenerative changes, old hemorrhage with hemosiderin-loaded macrophages, and/or fibrosis. In group III, both adenomas showed evidence of acute hemorrhage without significant degenerative or fibrotic changes. These data suggest that the signal intensity of parathyroid adenomas on T1- and T2-weighted images corresponds at least in part to differences in histologic composition.  相似文献

The magnetic resonance (MR) examinations of 18 patients with dilated bile ducts were reviewed retrospectively to determine the capability of MR to demonstrate biliary dilatation, assess MR appearance of the dilated biliary tract using spin-echo techniques, and define the optimal MR imaging parameters (repetition time [TR] and echo time [TE]) for its demonstration. On images with short TR (0.5 sec) and TE (28 msec), the dilated intrahepatic and intrapancreatic bile ducts usually had lower signal intensity compared with the surrounding liver or pancreas; on images with long TR (2.0 sec) and TE (56 msec), they had higher signal intensity. Because of the observed variation in percentage of contrast between dilated bile ducts and surrounding liver and pancreas, two imaging sequences are recommended to obtain reliable demonstration of dilated intrahepatic and intrapancreatic bile ducts. The dilated common bile duct at the level of the hepatic hilus is best seen with a short TR and TE.  相似文献

A new approach for producing primarily T2- and proton-density-weighted MR images in less time than the conventional long TR, long TE imaging is to reduce the TR of a double spin-echo pulse sequence and to also reduce the RF excitation flip angle to minimize the resulting T1 sensitivity. In preliminary studies with a human volunteer and five patients with various diseases of the head and neck, conventional long TR, long TE and short TR, short TE images were compared with short TR, long TE images with reduced flip angles (45 degrees, 30 degrees), which required only 40% of the imaging time of the long TR images. The latter images showed a similar contrast pattern to the conventional T2-weighted image, and contrast-to-noise measurements indicated an increase in contrast between the lesion and nearby tissue when the flip angle was reduced. Furthermore, the maximum contrast/noise per unit imaging time on the short TR, long TE image was comparable to that on the long TR, long TE image. Optimization of the flip angle with short TR allows a substantial reduction in imaging time but with a reduction in multislice capability. This technique will be most useful in areas of complex anatomy where two or more orthogonal imaging planes are required, such as the head and neck.  相似文献

Radiographically negative avascular necrosis: detection with MR imaging   总被引:7,自引:0,他引:7  
To correlate the morphologic appearance on magnetic resonance (MR) images of radiographically negative avascular necrosis (AVN) of the femoral head with that on computed tomographic (CT) and radionuclide scans, the radiographic and clinical records of 24 patients were reviewed retrospectively. In 18 patients the MR signal intensity features were monitored by means of serial imaging. All MR studies included T1-weighted (short repetition time [TR], short echo delay time [TE] ) imaging and T2-weighted imaging (long TR, long TE). Thirty-one hips were determined with MR to be involved by AVN; 27 were staged on the basis of signal intensity characteristics within the low-intensity rim. Core decompression was performed on 18 hips. Afterward, progression of disease occurred in only one hip. Fourteen of the 16 asymptomatic patients (88%) had early-stage focal lesions. CT scans were obtained in 15 patients and radionuclide scans in 21. Ten hips at radionuclide imaging and five at CT appeared normal when MR results were distinctly abnormal. MR can depict early radiographically negative AVN in asymptomatic individuals. At this early stage, the lesions in this series appear to be nonprogressive after treatment.  相似文献

Magnetic resonance (MR) imaging was performed in 32 male patients, 20 with no abnormalities and 12 with clinically suspected undescended testes. The results were compared with ultrasonographic, computed tomographic, clinical, and surgical findings. The undescended testes were unilateral in eight patients (one had testicular duplication) and bilateral in four. Of 16 undescended testes, 15 were correctly identified on MR images. One intraabdominal testis was not seen. Testis-fat contrast at 0.35 T was optimal with a short repetition time (TR) and a short echo time (TE). At 1.5 T, good contrast was achieved with short TR/TE sequences, but the contrast was even more pronounced with even longer TR/TE parameters. In seven patients with unilateral undescended testes, the undescended and contralateral testes showed symmetrical tissue signal intensity on both T1- and T2-weighted images. In three, the undescended testis was of lower signal intensity, suggesting atrophy. MR imaging promises to become an important diagnostic tool in the detection of undescended testes.  相似文献

The MR images and CT scans of 14 patients with surgically verified pyogenic cerebral abscesses were reviewed. The MR findings correlated well with those seen on CT and were believed to be sufficiently characteristic to allow early and accurate diagnosis with MR alone. These features include (1) peripheral edema producing mild hypointensity on short TR/short TE and marked hyperintensity on long TR/intermediate to long TE scans; (2) central necrosis with abscess fluid hypointense relative to white matter and hyperintense relative to CSF on short TR/short TE scans and hyperintense relative to gray matter on long TR/intermediate to long TE scans (the fluid had concentric zones of varying intensity in seven cases, a finding not previously identified in other lesions); (3) extraparenchymal spread (intraventricular or subarachnoid), which was detected more easily on MR than on CT and was manifested by increased intensity relative to normal CSF on both short TR/short TE and long TR/intermediate TE scans; and (4) visualization of the abscess capsule, which was iso- to mildly hyperintense relative to brain on short TR/short TE scans and iso- to hypointense relative to white matter on long TR/intermediate to long TE scans. On the long TR scans, the relative hypointensity of the rim allowed for visualization of the typical morphologic features of the capsule, which in turn aided in differentiation of abscesses from other lesions (as it does on CT). To investigate the cause of the capsular intensity, pathologic studies of the capsules were reviewed when available (10 cases). Fibrosis was identified in all mature abscess capsules, but the combination of the intensities seen on short TR/short TE and long TR/intermediate to long TE scans as well as the temporal changes in intensity were believed to be incompatible with fibrosis as a cause of the capsular changes. Intensity patterns were suggestive of hemorrhage, but neither acute nor chronic hemorrhage was identified on routine H and E stains, while iron stain revealed scant hemorrhage in only two of the eight patients in whom these stains were used. We believe the capsular intensity (in particular the hypointense rims on long TR scans) may reflect paramagnetic T1, and to a greater extent T2, shortening, possibly due to the presence of heterogeneously distributed free radicals that are products of the respiratory burst produced by actively phagocytosing macrophages in the capsule wall. Distinctive MR features of pyogenic abscesses should afford early and accurate diagnosis.  相似文献

Gd-DTPA-enhanced MR of suspected spinal multiple sclerosis   总被引:1,自引:0,他引:1  
A prospective study was undertaken to evaluate the potential of Gd-DTPA-enhanced MR to differentiate active from inactive demyelinating lesions of the cervical spinal cord. Five patients with elongated high-signal-intensity lesions in the cervical cord on long TR/TE spin-echo MR images and a clinical suspicion of demyelinating disease had MR before and after IV Gd-DTPA. Delayed contrast enhancement (after 45-60 min) of the lesions was seen on short TR/TE images in two patients with clinically active disease, but no enhancement could be detected in three patients with stable disease. The patients with active disease underwent repeated MR examinations until the enhancement disappeared. The decrease in Gd-DTPA enhancement paralleled a decrease in clinical signs and symptoms of cervical myelopathy. MR is useful in evaluating patients suspected of having demyelinating disease. The MR finding of asymptomatic lesions in the brain lends support to the diagnosis of multiple sclerosis. Other possible causes of myelopathy, such as spinal cord compression and intramedullary tumor, can be excluded with the use of MR.  相似文献

MR studies of the lumbar spine in 41 patients were analyzed at 203 disk interspaces to assess the appearance and frequency of bone marrow signal changes in the vertebral bodies adjacent to normal and degenerated disks. Degenerative changes were found at 58 interspaces; an abnormal bone marrow signal was identified in 29 (50%) of these. On spin-echo pulse sequences with short and long repetition times (TRs) and echo times (TEs), an area of relative increased signal intensity was seen in the vertebral body adjacent to the disk in 24 cases (17 were bandlike on both sides of the disk, four were focal on one side of the disk, and three were bandlike and focal on one or both sides of the disk). In one patient decreased signal was noted on both short and long TR/TE imaging. In the other four patients decreased signal was noted on short TR/TE pulse sequences and increased signal was evident on long TR/TE. These marrow changes were not present adjacent to normal disks. The relatively high signal intensity on both short and long TR/TE pulse sequences suggests that the increased signal resulted from the conversion of normal hemopoietic bone marrow to fatty marrow. We conclude that bandlike or focal areas of high signal intensity in the bone marrow adjacent to degenerated intervertebral disks occur commonly on MR images of the spine and must not be confused with signal changes from tumors or infections involving the disk space and adjacent vertebral end plates.  相似文献

The authors evaluated the use of magnetic resonance (MR) imaging in diagnosis of avascular necrosis (AVN) of carpal bones by examining 21 patients with wrist pain and two healthy volunteers. MR images were compared with conventional radiographs in every case and with bone scintigrams in 18 cases. MR imaging was slightly less sensitive than bone scintigraphy in depicting AVN, but in patients who were imaged with long repetition time (TR)/long echo time (TE) sequences in addition to short TR/short TE sequences, MR imaging was found to be more specific. While the authors believe that bone scintigraphy remains the screening test of choice for patients with wrist pain and normal plain radiographs, MR imaging promises to add significant diagnostic information in cases in which bone scans are abnormal.  相似文献

Fast gradient-echo magnetic resonance (MR) imaging of 38 adrenal masses with proved diagnosis was performed during suspended respiration with various repetition times (TRs), echo times (TEs), and flip angles. Dynamic perfusion studies after gadolinium diethylenetriamine-pentaacetic acid (DTPA) administration were performed by repeated imaging at short time intervals. With more T2 weighting (TR = 60 msec, TE = 30 msec, and flip angle = 15 degrees), malignant tumors and pheochromocytomas had a significantly higher relative signal intensity than adenomas; overlap of signal intensity led to equivocal findings in nine cases. After administration of Gd-DTPA, adenomas showed only mild enhancement and quick washout; malignant tumors and pheochromocytomas showed strong enhancement and slower washout. Five of the nine cases that were equivocal in precontrast images could thus be correctly classified. In addition to this improved classification of adrenal masses, fast, dynamic contrast material-enhanced MR imaging resulted in a reduction in total examination time.  相似文献

To better correlate the appearance of avascular necrosis (AVN) of the femoral head on magnetic resonance (MR) images with the stage of disease, MR images of 56 proved AVN lesions were compared with staging from corresponding radiographs (n = 56), Tc-99m scans (n = 41), and grade of symptoms (n = 28). Fractures complicating AVN were seen in 28 (50%) of 56 radiographs (radiographic stages III-V). With long repetition (TR) and echo delay (TE) times, a characteristic "double line sign" consisting of high signal intensity inside a low-intensity peripheral rim was seen in 45 lesions (80%). The central region within the rim was isointense with marrow fat on both short and long TR and TE images in 20 (71%) of 28 lesions uncomplicated by fracture (stages I-II) but in only four (14%) of 28 stage III-V lesions (P less than .001). Symptoms were least severe in lesions isointense with fat and most severe in lesions with low-signal central regions at short and long TRs and TEs. The peripheral double line sign on long TR/TE images may add specificity to the diagnosis of AVN by MR imaging. A chronologic pattern of central MR signal features is presented which may allow staging of AVN by MR imaging.  相似文献

The magnetic resonance (MR) images from 17 patients with chondrosarcomas of the skull base were retrospectively reviewed to characterize the size, location, signal intensity, and extension of these tumors. Eleven patients with chondrosarcomas received intravenously administered gadopentetate dimeglumine. In 16 patients, computed tomographic (CT) scans were obtained to evaluate intratumorous mineralization and bone erosion. On short repetition time (TR)/echo time (TE) MR images, chondrosarcomas generally had low to intermediate signal intensity; on long TR/TE MR images, they generally had very high signal intensity. Signal heterogeneity on long TR/TE MR images was seen in 10 of 17 tumors (59%) and was caused by matrix mineralization, fibrocartilaginous elements, or both. Matrix mineralization was demonstrated with CT in seven of the 16 chondrosarcomas. Chondrosarcomas showed marked enhancement after administration of gadopentetate dimeglumine in either a heterogeneous (n = 8) or homogeneous (n = 3) pattern. The information about the size and extent of these neoplasms was important in the choice of surgical approaches for gross total resection of tumor.  相似文献

Combined gadolinium-enhanced and fat-saturation MR imaging of renal masses   总被引:4,自引:0,他引:4  
Combined gadopentetate dimeglumine enhancement and fat-saturation (FS) spin-echo (SE) magnetic resonance (MR) imaging for the detection and characterization of renal masses was evaluated in 43 patients with a total of 71 lesions (28 solid masses and 43 cysts). SE MR sequences compared were the following: short repetition time (TR)/echo time (TE), conventional SE, short TR/TE FS SE, long TR/TE conventional SE, gadolinium-enhanced short TR/TE conventional SE, and gadolinium-enhanced short TR/TE FS SE techniques. MR findings were compared with findings of contrast-enhanced computed tomography (CT) and with pathologic findings in all patients. The sensitivities for detection of renal masses with gadolinium-enhanced FS (71 of 71 lesions) and with gadolinium-enhanced short TR/TE conventional (65 of 71 lesions) SE sequences were significantly (P less than .01) greater than with any unenhanced (short TR/TE conventional [40 of 71 lesions], or long TR/TE [39 of 71 lesions]) SE sequence. Lesion characterization was also best with the gadolinium-enhanced FS SE sequence (65 of 71 lesions correctly classified). When combined pre- and postcontrast short TR/TE FS SE images were analyzed with both qualitative (visual) and quantitative (region-of-interest measurements) assessment, lesion characterization improved even further (70 of 71 lesions were correctly characterized). All lesions detected with CT were visualized with the gadolinium-enhanced FS SE MR sequence, which in addition depicted seven cysts and two small renal cell carcinomas. In summary, the use of gadopentetate dimeglumine, especially when combined with the FS technique, was superior to unenhanced MR imaging for detection and characterization of renal lesions.  相似文献
