目的:采用相对定量分析法评估99mTc-ECD脑血流灌注显像对小儿原发性癫痫定性和病灶定位的诊断价值。方法:21例CT检查正常的小儿原发性癫痫病人进行99mTc-ECD脑显像和相对定量分析,并同步进行EEG检查。结果:21例病人SPECT显像18例异常,3例正常,阳性率857%,假阴性10%,18例脑显像异常患儿中,16例局部脑血流(rCBF)减少,99mTc-ECD摄取率726±89%,2例在额叶有限局性rCBF增加,99mTc-ECD摄取率分别为1495%和1309%。EEG检查21例病人20例异常,1例正常,阳性率952%,能定位诊断的7例,定位率为35%。结论:99mTc-ECD脑显像相对定量分析法对小儿原发性癫痫病灶的定位有临床意义,优于EEG。  相似文献

目的:评估彩色室壁动力分析-多巴酚丁胺负荷超声心动图(Color Kinesis-Dobutamine Suress Echocardiography,CK-DSE)对诊断冠心病(coronary heart disease,CHD)的准确性。方法:以冠状动脉造影(coronary angiography,CAG)为标准,采用CK-DSE检测了50例可疑CHD患者。结果:CK-DSE诊断CHD的敏感性、特异性、准确性、阳性预测值和阴性预测值分虽为92.6%、78.3%、86%、83.3%和94.2%。结论:CK-DSE诊断CHD安全、准确、客观。  相似文献

本文报告国产99mTc—MIBI心肌灌注断层显像80例与冠状动脉造影(CA)对照分析。结果表明:99mTc—MIBI心肌灌注断层显像对冠心病诊断的灵敏度96.7%、特异性为75%、准确性为91.3%、阳性预测率92.1%。对急性或陈旧性心肌梗塞、心绞痛及X综合症的心肌灌注显像阳性率分别是100%、93.9%和83.3%。心肌灌注显像所示异常部位与CA检查冠状动脉供血区基本相符,有较可靠的定位准确性,心肌灌注显像放射性分布与冠状动脉狭窄程度亦有一定相关性。  相似文献

^99mTc—tetrofosmin SPECT显像评价脑肿瘤的临床研究   总被引:1,自引:0,他引:1  
目的:探讨99mTc-tetrofosmin(TF) SPECT显像对脑肿瘤诊断的临床价值。方法:在28例CT或MR示脑占位性病变患者中,术前1周内进行脑99mTc-TF SPECT显像,其中17例还进行了2~3小时的延迟显像,观察病变部位99mTc-TF的摄取变化情况,并进行定性和半定量分析,即在横断面或冠状面图像上寻找肿瘤区摄取放射性最高的断面,画取肿瘤部位和相应对侧部位同样大小的感兴趣区(ROI),计算肿瘤摄取比值(TUR)。结果: 25例99mTc-TF SPECT断层显像病灶处TF摄取不同程度的增高,3例未见TF摄取。半定量分析结果显示:17例早期和延迟显像肿瘤病灶的TF、TUR分别为2.34 ±0.41和2.06±0.29、未见明显变化(P>0.05)。Ⅲ-Ⅳ级胶质瘤的TF、TUR(2.44±0.14)与Ⅰ~Ⅱ级胶质瘤的TF、TUR(1.46±0.36),两者有显著差异(P<0.01),诊断灵敏度为89.3%。结论:99mTc-TF SPECT显像在脑肿瘤诊断上有一定的临床价值,特别是在区分脑胶质瘤的恶性程度上。  相似文献

对68例老年人核心素心肌断层显像进行了分析,其中正常人27例,非冠状动脉化性心脏病10例,冠心病心绞痛19例,陈旧性心肌梗塞12例,除27例正常人外,均作了冠状动脉造影,结果显示,SPECT诊断老年冠心病的敏感性为96.77%,特异性为81.08%,SPECT阴性者患CHD的可能性很小,SPECT阳性者需排除非冠心病的可能。  相似文献

目的:观察和分析运动(EX)、巯甲丙脯酸(CAP)及巯甲丙脯酸运动(CAP-EX)介入对轻中度原发性高血压(EH)患者肾功能的影响。方法:10例正常志愿者和15例轻中度原发性高血压患者,在1个月内完成基础双核素肾显像(BS-RS)、运动双核素肾显像(EX-RS)、巯甲丙脯酸双核素肾显像(CAP-RS)及巯甲丙脯酸运动双核素肾显像(CAP-EX-RS)4次检查。图像均按ADAC公司提供的计算机软件进行处理。结果:EX和CAP-EX介入,GFR,ERPF均明显降低(P<001);而CAP介入未见明显变化。EH组与对照组相应方法间GFR,ERPF不存在显著差异(P>005)。15例EH患者中,基础肾显像及巯甲丙脯酸肾显像结果均正常,而40%(6/15)的患者出现异常运动肾图,30%(5/15)出现异常巯甲丙脯酸运动肾图,二者同时异常的有5例,其中1例于CAP-EX-RS介入时,异常更加明显,均表现为对称性的131I-OIH和99mTc-DTPA排泄延迟,131I-OIH肾图和99mTc-DTPA肾图曲线宽大,排泄段抬高。上述异常变化在131I-OIH和99mTc-DTPA肾图曲线中,以99mTc-DTPA肾图  相似文献

Purpose: The assessment of myocardial viability has become an important aspect of the diagnostic and prognostic work‐up of patients with coronary artery disease. Technetium‐99m labelled sestamibi (99mTc‐MIBI) myocardial perfusion imaging may underestimate the viability of ischaemic myocardium. Technetium‐99m labelled 4,9‐diaza‐3,3,10,10‐tetramethyldodecan‐2,11‐dione dioxime (99mTc‐HL91) is a hypoxia‐avid agent which can identify acutely ischaemic viable myocardium in a canine model using a standard gamma camera. The aim of this study was to evaluate uptake character of ischaemic viable myocardium and diagnostic performance of single‐photon emission computed tomography (SPECT) imaging by 99mTc‐HL91 and 99mTc‐MIBI in detecting ischaemic viable myocardium in coronary heart disease. Methods: A total of 41 patients with coronary artery disease were recruited from March 2008 to May 2009. For detecting ischaemic viable myocardium, SPECT imaging by 99mTc‐HL91 and 99mTc‐MIBI were performed in all patients before coronary revascularization. Six patients with single ischaemic myocardial segment received a 2‐day SEPCT/CT imaging protocol and the uptake of 99mTc‐HL91 in ischaemic myocardium was quantitatively analysed. The remaining 35 patients received a 1‐day 99mTc‐HL91 and 99mTc‐MIBI SPECT imaging protocol. Resting 99mTc‐MIBI myocardial perfusion imaging in 3–18 months after revascularization was used as the standard methodology to evaluate the myocardial viability. Results: In 41 patients, 66 ischaemic myocardial segments were proven to be viable and 12 to be necrotic by resting 99mTc‐MIBI myocardial perfusion imaging after coronary revascularization. Furthermore, 60 viable segments with negative uptake of 99mTc‐MIBI showed positive uptake of 99mTc‐HL91. The remaining six viable segments and 12 necrotic segments showed both negative uptake of 99mTc‐HL91 and 99mTc‐MIBI. The sensitivity, specificity, accuracy, Younden Index, positive predictive value and negative predictive value for evaluating ischaemic viable myocardium were 90·9%, 100%, 92·3%, 90·9%, 100% and 66·7%, respectively. Ischaemic viable myocardium had the negative 99mTc‐MIBI uptake and positive 99mTc‐HL91 uptake, which demonstrated a mismatched uptake character. Quantitative analysis indicated the uptake of 99mTc‐HL91 in viable myocardium was increasing in the first 1–3 h and remained stable at the 3–4 h after injection. Conclusion: Functional SPECT imaging with 99mTc‐HL91 and 99mTc‐MIBI can be used to detect the seriously ischaemic but viable myocardium with a mismatched uptake character. The uptake of 99mTc‐HL91 in the viable myocardium reached a stable level at 3–4 h after injection.  相似文献

Kawasaki disease (KD) is an acute vasculitis syndrome of unknown etiology that mainly affects the coronary arteries. The purpose of this study was to assess the agreement between dipyridamole stress technetium-99m tetrofosmin (Tc-TF) myocardial perfusion single photon emission computed tomography (SPECT) and coronary angiography in these patients. Twenty-nine children with KD were included in this study. All of the 29 children also received dipyridamole stress Tc-TF myocardial perfusion SPECT within 1 month of their coronary angiographic studies. The results showed that (1) 89.7% of children had negative coronary angiographic findings without significant coronary stenoses, and 10.3% of children had positive coronary angiographic findings with significant coronary stenosis; (2) 44.8% of children had negative Tc-TF myocardial perfusion SPECT findings without abnormal myocardial perfusion, and 55.2% of children had positive Tc-TF myocardial perfusion SPECT findings with abnormal myocardial perfusion; (3) 44.8% of children had both normal coronary angiographic and Tc-TF myocardial perfusion SPECT findings, and 10.3% of children had both abnormal coronary angiographic and Tc-TF myocardial perfusion SPECT findings; and (4) There was no significant agreement between coronary angiographic and Tc-TF myocardial perfusion SPECT findings. We concluded that poor agreement exists between coronary angiographic and Tc-TF myocardial perfusion SPECT findings with coronary stenoses and abnormal myocardial perfusion in children with KD.  相似文献

BACKGROUND: A reverse redistribution pattern during myocardial perfusion imaging is most widely described using thallium (Tl-201), when stress images exhibit greater perfusion than rest. Technetium (Tc-99 m) radiopharmaceuticals may also yield a reverse perfusion (RP) pattern, but its significance is uncertain. This study tested the hypothesis that RP correlates with the presence and location of flow limiting coronary stenosis(es). METHOD: We reviewed 842 consecutive Tc-99 m tetrofosmin SPECT stress studies performed at a cardiothoracic centre over a 15 month period. 69 (8.2%) demonstrated RP. Thirty-three patients (age 32-79 mean 56, 17 female) had undergone cardiac catheterisation within 12 months of the scan. Correlation was sought between the presence and location of angiographic stenoses and RP pattern. RESULTS: 10/33 (30.3%) had significant (>60%) coronary stenosis(es); 5 single-vessel, 2 two-vessel and 3 three-vessel disease (3VD). Stenosis location correlated poorly with the RP territory (LAD/Anterior 5/17, RCA/Inferior 1/10, Cx/lateral 0/4 (p = 0.57)). Of the 6 patients with a lesion in the RP territory, 3 had 3VD; 2 of these had a simultaneous reversible defect. All 5 patients with previous myocardial infarction had a simultaneous fixed defect. However only 3/12 with co-existent reversible defects had significant disease. CONCLUSION: The reverse perfusion pattern is a poor predictor of flow limiting coronary disease, and does not correlate with stenosis location in those with significant lesions. Such patients should not undergo invasive investigation purely on the basis of this result.  相似文献

Trimetazidine (TMZ) increases the mithocondrial oxidative metabolism and improves Tc-99m sestamibi uptake in myocardial single photon emission tomography (SPECT). The aim of this study was to evaluate whether the acute administration of TMZ improved myocardial perfusion and modified left ventricular ejection fraction (LVEF) in ischaemic left ventricular impairment. METHODS: Thirty-one patients (23 males, age 66 years) with prior myocardial infarction (>6 months) and echocardiographic LVEF < or = 45% underwent coronary angiography, rest basal myocardial SPECT (after 3-day placebo administration) and rest TMZ myocardial SPECT [after 3-day TMZ administration (60mg/die)]. The left ventricle was analysed in 16 segments. The summed placebo score (SPS) and the summed TMZ score (STS) were calculated with a 5-point scale (from 0 = normal uptake to 4 = absent uptake) by two blinded operators. The GATED Tc-99m SPECT was always provided. RESULTS: After TMZ administration GATED LVEF improved from 26.5+/-9.7% to 29.1+/-11.3% (p = 0.04) and left ventricular end-systolic volume (LVESV) was reduced from 90.2+/-40.7 to 85.6+/-39.2 ml/mq (p = 0.006). Similarly the addition of TMZ to myocardial SPECT significantly reduced the STS compared to SPS (21.5+/-11 vs. 26.6+/-10.5 p = 0.0001). Eleven patients (35.5%) had an echocardiographic LVEF < or = 30%; in these patients who had severe ventricular dysfunction, GATED LVEF and LVESV did not change after TMZ (20.2+/-5.7% vs. 21+/-6.9% p =0.6; 116.7+/-35.3 ml vs. 112.6+/-32.3 ml p = 0.08, respectively). CONCLUSION: In comparison with placebo, the addition of TMZ to myocardial Tc-99m tetrofosmin SPECT improved myocardial perfusion and LVEF, reducing LVESV. These effects were lost in patients with more severe ventricular dysfunction.  相似文献

Background: Assessing the viability in akinetic myocardium is vital for predicting functional recovery after therapeutic management in patients with chronic coronary artery disease (CAD) and depressed left ventricular (LV) function. The present study aimed to evaluate the efficacy of Tc99m MIBI SPECT enhanced with nitroglycerine infusion in detecting myocardial viability, as well as to asses the relationship between the myocardial viability and the subsequent treatment and outcome of patients. Methods and results: Sixty-seven consecutive patients with CAD and LV dysfunction (LV ejection fraction 36.6 ± 8.4%) underwent Tc99m MIBI imaging – at rest and during intravenous nitroglycerine infusion – for viability assessment. Fourteen patients were treated pharmacologically (Group I), and fifty-three (Group II) were submitted to coronary revascularization (PTCR or CABG). Fifteen major cardiac events were observed during 25 months of the follow-up. A significantly worse event-free survival was registered in the subjects of Group I than in Group II subjects. The prognostic predictors of cardiac events were: (1) the number of viable, non-revascularized segments in perfusion imaging (p < 0.001), (2) the severity of the disease assessed by coronary angiography (p < 0.05). Conclusions: Viability detection in nitroglycerine infusion enhanced Tc99m MIBI imaging offers significant prognostic value in patients with CAD after myocardial infarction. Patients with preserved viability showed better prognosis after revascularization than those treated pharmacologically.  相似文献

Small animal imaging of cardiovascular disease using single photon emission tomography (SPECT) can be used to provide quantitative measurements of myocardial infarct. The purpose of this study was to demonstrate the accuracy of pinhole SPECT imaging with [99mTc]sestamibi for estimation of infarct size in a rat model of coronary artery disease. Nine rats had their left anterior descending artery ligated to induce a region of myocardial infarct. These animals were injected with 37 MBq [99mTc]sestamibi, and, 1 h later, scanned on a pinhole SPECT system for 30 min. The defect size measured with SPECT, which was dependent on a threshold applied to the short axis circumferential profiles, was compared against the gold standard triphenyltetrazolium chloride (TTC) staining. The size of the perfusion deficit measured using [99mTc]sestamibi SPECT compared very favorably with the TTC staining result, for threshold values in the range 50–70%. The optimum threshold was approximately 70%, giving an excellent correlation (R 2=0.89, p<0.001). Estimation of infarct size by [99mTc] sestamibi SPECT yielded an excellent agreement with TTC staining. In conclusion, measurement of myocardial infarct with SPECT can be used to study the rat heart in vivo, and provides a quantitative measure of myocardial viability.  相似文献

目的 比较18F FDGSPECT/CT显像与99Tcm MDP骨显像在肺癌骨转移诊断中的临床价值。方法  19例经病理证实的肺癌患者同期进行18F FDGSPECT/CT显像和常规99Tcm MDP骨显像。结果  19例患者 8例有骨转移 ,共发现 47个转移灶。18F FDGSPECT/CT发现 8例骨转移 ,99Tcm MDP骨显像发现 7例骨转移 (P >0 .0 5 ) ;18F FDGSPECT/CT发现 45个转移灶 ,99Tcm MDP骨显像发现 2 6个转移灶 (P <0 .0 1)。18F FDGSPECT/CT探测骨转移的灵敏度 95 .74% ,特异度 89.47% ,准确度93 .94% ;99Tcm MDP探测骨转移灵敏度 5 5 .3 2 % (P <0 .0 1) ,特异度为 94.74% (P >0 .0 5 ) ,准确度 66.67% (P <0 .0 1)。结论 在探测肺癌骨转移方面 ,18F FDGSPECT/CT显像较常规99Tcm MDP骨显像具有更高的灵敏度和准确度。对99Tcm MDP骨显像阴性、单一病灶及可疑脊椎转移患者 ,应进一步行18F FDGSPECT/CT显像 ,可起到信息互补、明确诊断的作用。  相似文献

目的 探讨脑膜瘤99Tcm MIBI和99Tcm ECDSPECT脑显像的影像特征 ,并评价其应用价值。方法 对 10例正常对照者及 18例脑膜瘤患者进行99Tcm MIBI和99Tcm ECDSPECT脑显像 ;分析病灶的影像特征。结果 正常对照者99Tcm MIBI显像脑实质内均无核素浓聚 ;99Tcm ECD显像脑实质内放射性核素分布大致均匀 ,左右侧基本对称。 18例脑膜瘤99Tcm MIBI显像见肿瘤处均表现为均匀的圆形或卵圆形放射性核素浓聚 ,其阳性率为 10 0 % ;而99Tcm ECD显像 18例中有 17例示肿瘤处表现为形态规整、边缘整齐的圆形或卵圆形内凹缺损 ,其形态与99Tcm MIBI显像所示的浓聚区相吻合 ,另1例为假阴性结果 ,其阳性率为 94.4%。结论 99Tcm MIBI结合99Tcm ECDSPECT脑显像可用于脑膜瘤的辅助诊断  相似文献
