While longitudinal magnetic resonance imaging (MRI) studies have demonstrated progressive gray matter reduction of the superior temporal gyrus (STG) during the early phases of schizophrenia, it remains largely unknown whether other temporal lobe structures also exhibit similar progressive changes and whether these changes, if present, are specific to schizophrenia among the spectrum disorders. In this longitudinal MRI study, the gray matter volumes of the fusiform, middle temporal, and inferior temporal gyri were measured at baseline and follow-up scans (mean inter-scan interval = 2.7 years) in 18 patients with first-episode schizophrenia, 13 patients with schizotypal disorder, and 20 healthy controls. Both schizophrenia and schizotypal patients had a smaller fusiform gyrus than controls bilaterally at both time points, whereas no group difference was found in the middle and inferior temporal gyri. In the longitudinal comparison, the schizophrenia patients showed significant fusiform gyrus reduction (left, − 2.6%/year; right, − 2.3%/year) compared with schizotypal patients (left: − 0.4%/year; right: − 0.2%/year) and controls (left: 0.1%/year; right: 0.0%/year). However, the middle and inferior temporal gyri did not exhibit significant progressive gray matter change in all diagnostic groups. In the schizophrenia patients, a higher cumulative dose of antipsychotics during follow-up was significantly correlated with less severe gray matter reduction in the left fusiform gyrus. The annual gray matter loss of the fusiform gyrus did not correlate with that of the STG previously reported in the same subjects. Our findings suggest regional specificity of the progressive gray matter reduction in the temporal lobe structures, which might be specific to overt schizophrenia within the schizophrenia spectrum.  相似文献

OBJECTIVE: To evaluate patients with early HIV-cognitive motor complex (HIV-CMC) for possible regional cerebral blood flow (rCBF) abnormalities on perfusion MRI (pMRI). BACKGROUND: Nuclear medicine techniques have demonstrated global and focal cerebral perfusion abnormalities in patients with HIV dementia. Ultrafast pMRI enables the measurement of rCBF throughout the brain without the need to apply radioactive tracers or ionizing radiation. METHODS: pMRI was used to measure the rCBF in 19 patients with early stages of HIV-CMC and 15 healthy seronegative control subjects. The rCBF maps were registered to high-resolution anatomic MRI scans and transformed into Talairach space. Statistical analysis of the rCBF maps was performed with SPM96. RESULTS: Compared with the control subjects, the patients with HIV had statistically significantly decreased rCBF bilaterally in the inferior lateral frontal cortices (right: -15%, p < 0.002; left: -12%, p < 0.005) and in the inferior medial parietal brain region (-15%, p < 0.0009). In contrast, rCBF was increased bilaterally in the posterior inferior parietal white matter (right: +19%, p < 0.0001; left: + 17%, p < 0.001). Furthermore, rCBF abnormalities correlated significantly with clinical disease severity as measured by CD4 count, plasma viral load, Karnofsky score, and HIV dementia scale. DISCUSSION: Our results are consistent with previous findings from PET and SPECT studies. Furthermore, pMRI can detect rCBF abnormalities that correlate with disease severity in HIV-CMC. Because pMRI is more cost-effective, faster, and safer than nuclear medicine techniques for monitoring rCBF changes, pMRI may be more feasible for monitoring the effects of therapy for HIV-CMC.  相似文献

The aim of this study was to determine if volumes of frontotemporal regions associated with language were related to thought disorder in 42 children, aged 5–16 years, with cryptogenic epilepsy, all of whom had complex partial seizures (CPS). The children with CPS and 41 age- and gender-matched healthy children underwent brain MRI scans at 1.5 T. Tissue was segmented, and total brain, frontal lobe, and temporal lobe volumes were computed. Thought disorder measures, IQ, and seizure information were collected for each patient. The subjects with CPS had more thought disorder, smaller total gray matter and orbital frontal gray matter volumes, as well as larger temporal lobe white matter volumes than the control group. In the CPS group, thought disorder was significantly related to smaller orbital frontal and inferior frontal gray matter volumes, increased Heschl’s gyrus gray matter volumes, and smaller superior temporal gyrus white matter volumes. However, significantly larger orbital frontal gyrus, superior temporal gyrus, and temporal lobe gray matter volumes and decreased Heschl’s gyrus white matter volumes were associated with thought disorder in the control group. These findings suggest that thought disorder might represent a developmental disability involving frontotemporal regions associated with language in pediatric CPS.  相似文献

【摘要】 目的 运用磁共振成像(magnetic resonance imaging,MRI)基于白质地图的扩散张量成像(diffusion tensor imaging,DTI)技术研究皮质下缺血性血管病(subcortical ischemic vascular disease,SIVD)患者联络纤维的超微结构改变,并分析DTI参数与认知功能的关系。 方法 连续入选40例SIVD患者,分为血管性认知障碍组18例,无认知障碍组22例,另设立20例正常老年人为对照组。对三组进行MRI检查并对其白质改变进行评分,对联络纤维进行基于白质地图分析(atlase based analysis,ABA)DTI成像,比较组间部分各向异性指数(fractional anisotrophy,FA)、平均扩散率(mean diffusivity,MD)差异,并对SIVD患者DTI参数与蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)得分进行相关性分析。 结果 无认知障碍组较正常组在右侧矢状层(下纵束/下额枕束)、右侧扣带(海马部)及左侧上额枕束的FA值减低;认知障碍组较正常组存在广泛的联络纤维FA值降低,差异均具有显著性。无认知障碍组与正常组比较,仅左侧扣带海马部及左侧上纵束MD值无显著差异,余联络纤维的MD值均显著增高;认知障碍组较正常组存在广泛的联络纤维MD值增高,差异均具有显著性。与无认知功能障碍组比较,认知功能障碍组在双侧矢状层(下纵束/下额枕束)、双侧扣带扣带回部、海马部、双侧上纵束、双侧上额枕束及双侧钩束的FA值减低,MD值增高,差异具有显著性。SIVD患者双侧矢状层(下纵束/下额枕束)、扣带扣带回部、上纵束、上额枕束及钩束和左侧扣带海马部的FA值与MoCA评分呈正相关,MD值与MoCA评分呈负相关。 结论 SIVD患者联络纤维的FA值减低,MD值增高,且DTI参数与其认知功能密切相关。  相似文献

Several studies suggest that cognitive tasks attenuate activation of the limbic system by emotional stimuli. We investigated the possibility that worry would similarly inhibit the limbic system by examining its effects on regional cerebral blood flow (rCBF). Ten nonanxious volunteers underwent four scans within one session, using positron emission tomography (PET) with H215O as tracer. The first two scans recorded emotionally neutral thinking induced after listening to tapes describing neutral statements. Preceding the third and fourth scans, subjects listened to the self-recorded tape describing their individual worries, were instructed to continue to worry, and were scanned 5 min later. Subjects rated themselves as more anxious during the worry scans but showed no significant heart interbeat or skin conductance changes. During worry, rCBF increases were found bilaterally in the medial fronto-orbital gyri and the right thalamus; rCBF decreases were found bilaterally in the hippocampi and amygdalae, in the right insula, the left and right inferior, middle and superior temporal gyri and the occipito-temporal gyri, the right inferior occipital gyrus and the left supramarginal gyrus. Activity of the left orbito-frontal gyrus was negatively correlated with activity of the amygdalae. The results support the hypothesis that worry-induced prefrontal activity suppresses affect-related subcortical regions.  相似文献

Reduced regional cerebral blood flow in non-psychotic violent offenders   总被引:5,自引:0,他引:5  
The present study was designed to replicate previously reported findings of abnormal frontal and/or temporal cerebral blood flow in violent offenders and to control for the influence of major mental disorder (MMD), substance abuse, and current medication. HMPAO-SPECT-CBF and MRI scans from pretrial forensic psychiatric investigations of 21 subjects convicted of impulsive violent crimes were retrospectively re-evaluated. In 16/21 subjects, visual assessment of SPECT scans showed some hypoperfusion in the temporal and/or frontal lobes. MRI showed no corresponding structural damage. Quantified regional cerebral blood flow (rCBF) in defined regions of interest was compared between index cases and 11 healthy control subjects. Index subjects had significant reductions in the right angular gyrus and the right medial temporal gyrus, bilaterally in the hippocampus, and in the left white frontal matter, but they had significantly increased rCBF in the parietal association cortex bilaterally. The aberrations were as frequent and severe among the subjects without MMD, substance abuse, and current medication (n=7) as in the entire group of index subjects.  相似文献

Background: Several patterns of grey and white matter changes have been separately described in young adults with first-episode psychosis. Concomitant investigation of grey and white matter densities in patients with first-episode psychosis without other psychiatric comorbidities that include all relevant imaging markers could provide clues to the neurodevelopmental hypothesis in schizophrenia. Methods: We recruited patients with first-episode psychosis diagnosed according to the DSM-IV-TR and matched controls. All participants underwent magnetic resonance imaging (MRI). Voxel-based morphometry (VBM) analysis and mean diffusivity voxel-based analysis (VBA) were used for grey matter data. Fractional anisotropy and axial, radial and mean diffusivity were analyzed using tract-based spatial statistics (TBSS) for white matter data. Results: We included 15 patients and 16 controls. The mean diffusivity VBA showed significantly greater mean diffusivity in the first-episode psychosis than in the control group in the lingual gyrus bilaterally, the occipital fusiform gyrus bilaterally, the right lateral occipital gyrus and the right inferior temporal gyrus. Moreover, the TBSS analysis revealed a lower fractional anisotropy in the first-episode psychosis than in the control group in the genu of the corpus callosum, minor forceps, corticospinal tract, right superior longitudinal fasciculus, left middle cerebellar peduncle, left inferior longitudinal fasciculus and the posterior part of the fronto-occipital fasciculus. This analysis also revealed greater radial diffusivity in the first-episode psychosis than in the control group in the right corticospinal tract, right superior longitudinal fasciculus and left middle cerebellar peduncle. Limitations: The modest sample size and the absence of women in our series could limit the impact of our results. Conclusion: Our results highlight the structural vulnerability of grey matter in posterior areas of the brain among young adult male patients with first-episode psychosis. Moreover, the concomitant greater radial diffusivity within several regions already revealed by the fractional anisotropy analysis supports the idea of a late myelination in patients with first-episode psychosis.  相似文献

Alterations in parietal and temporal white matter microstructure derived from diffusion tensor imaging occur in preclinical and clinical Alzheimer's disease. Amyloid beta (Aβ) deposition and such white matter alterations are two pathological hallmarks of Alzheimer's disease. However, the relationship between these pathologies is not yet understood, partly since conventional diffusion MRI methods cannot distinguish between cellular and extracellular processes. Thus, we studied Aβ‐associated longitudinal diffusion MRI changes in Aβ‐positive (N = 21) and Aβ‐negative (N = 51) cognitively normal elderly obtained from the Alzheimer's Disease Neuroimaging Initiative dataset using linear mixed models. Aβ‐positivity was based on Alzheimer's Disease Neuroimaging Initiative amyloid‐PET recommendations using a standardized uptake value ratio cut‐off of 1.11. We used free‐water imaging to distinguish cellular and extracellular changes. We found that Aβ‐positive subjects had increased baseline right uncinate fasciculus free‐water fraction (FW), associated with worse baseline Alzheimer's disease assessment scale scores. Furthermore, Aβ‐positive subjects showed faster decrease in fractional anisotropy (FW‐corrected) in the right uncinate fasciculus and faster age‐dependent right inferior longitudinal fasciculus FW increases over time. Right inferior longitudinal fasciculus FW increases were associated with greater memory decline. Importantly, these results remained significant after controlling for gray and white matter volume and hippocampal volume. This is the first study to illustrate the influence of Aβ burden on early longitudinal (in addition to baseline) white matter changes in cognitively normal elderly individuals at‐risk of Alzheimer's disease, thus underscoring the importance of longitudinal studies in assessing microstructural alterations in individuals at risk of Alzheimer's disease prior to symptoms onset.  相似文献

OBJECTIVE: Magnetic resonance imaging (MRI) studies of schizophrenia reveal temporal lobe structural brain abnormalities in the superior temporal gyrus and the amygdala-hippocampal complex. However, the middle and inferior temporal gyri have received little investigation, especially in first-episode schizophrenia. METHOD: High-spatial-resolution MRI was used to measure gray matter volume in the inferior, middle, and superior temporal gyri in 20 patients with first-episode schizophrenia, 20 patients with first-episode affective psychosis, and 23 healthy comparison subjects. RESULTS: Gray matter volume in the middle temporal gyrus was smaller bilaterally in patients with first-episode schizophrenia than in comparison subjects and in patients with first-episode affective psychosis. Posterior gray matter volume in the inferior temporal gyrus was smaller bilaterally in both patient groups than in comparison subjects. Among the superior, middle, and inferior temporal gyri, the left posterior superior temporal gyrus gray matter in the schizophrenia group had the smallest volume, the greatest percentage difference, and the largest effect size in comparisons with healthy comparison subjects and with affective psychosis patients. CONCLUSIONS: Smaller gray matter volumes in the left and right middle temporal gyri and left posterior superior temporal gyrus were present in schizophrenia but not in affective psychosis at first hospitalization. In contrast, smaller bilateral posterior inferior temporal gyrus gray matter volume is present in both schizophrenia and affective psychosis at first hospitalization. These findings suggest that smaller gray matter volumes in the dorsal temporal lobe (superior and middle temporal gyri) may be specific to schizophrenia, whereas smaller posterior inferior temporal gyrus gray matter volumes may be related to pathology common to both schizophrenia and affective psychosis.  相似文献

目的探讨3.0T常规磁共振阴性的药物难治性颞叶癫痫(rTLE-N)与药物控制性颞叶癫痫(cTLE-N)脑灰质体积及脑白质结构网络拓扑属性差异。方法选取2017年3月至2019年8月在广西医科大学第一附属医院就诊的20例rTLE-N、15例cTLE-N完成头颅3DT1及DTI扫描,20例健康对照(HC)也完成此项检查。基于体素的形态学分析(VBM)方法比较3组脑灰质体积差异,基于图论的方法比较3组脑白质结构网络拓扑属性差异。结果①VBM结果:与HC组相比,rTLE-N组在双侧海马、丘脑、颞中回、内侧和旁扣带回,左侧直回、眶内额上回,右侧海马旁回灰质体积减少(P<0.001);cTLE-N组右侧颞中回、眶内额上回灰质体积减少(P<0.001)。与cTLE-N组相比,rTLE-N组在双侧岛叶、中央沟盖,左侧海马旁回、眶内额上回、梭状回、小脑6区,右侧丘脑、枕下回灰质体积减少(P<0.001);②脑白质结构网络拓扑属性结果:3组脑白质结构网络均表现出小世界属性。与HC组比较,rTLE-N组最短路径长度(Lp)增加、局部效率(Eloc)及全局效率(Eg)下降(P<0.05),cTLE-N组Lp增加、Eg下降(P<0.05);与cTLE-N组比较,rTLE-N组Lp增加、Eloc及Eg下降(P<0.05)。结论TLE-N是一种脑网络疾病,但rTLE-N与cTLE-N致痫网络不同,cTLE-N脑灰质萎缩较局限但已出现脑结构网络拓扑属性受损,而rTLE-N涉及多个脑区灰质萎缩且脑结构网络拓扑属性损害更严重。  相似文献

Schizophrenia is characterised by the presence of a heterogeneous range of symptoms. Although there is a consensus regarding ventricular enlargement and regional grey matter deficits, the brain structural correlates of specific symptoms, such as auditory hallucinations, are not clearly defined. We used an automated voxel-wise analysis of dual-echo spin-echo MRI data from 28 patients with schizophrenia characterised by persistent hallucinations and 32 healthy controls. Patients demonstrated grey matter (GM) volume decrements in the insula bilaterally, and in the right superior temporal and fusiform gyri, and left inferior temporal gyrus. With the exception of the insula, these GM volume losses were correlated with severity of auditory hallucinations. GM excesses were observed in the right caudate nucleus and middle temporal gyrus. White matter deficits were observed adjacent to the left superior temporal gyrus, in the right internal capsule and inferior longitudinal fasciculus. These findings support the proposition that there are structural changes in the neural circuits underlying broader processing of affect-laden information in patients with schizophrenia prone to experiencing auditory hallucinations. Such deficits may obscure important cues for recognition of internal speech, contributing to failures of self-monitoring.  相似文献

Previous studies have demonstrated lower-diffusion anisotropy within white matter in late-onset depression measured by diffusion tensor imaging, which provides information about brain white matter integrity. We have examined whether white matter is abnormal in first-episode remitted geriatric depression by using diffusion tensor imaging. Sixteen remitted geriatric depression patients and 14 well matched healthy controls underwent diffusion tensor-imaging scans of magnetic resonance imaging, which were analyzed by a rigorous voxel-based approach. We found that fractional anisotropy in white matter was lower in patients than in controls at the right superior frontal gyrus, left inferior frontal gyrus, left middle temporal gyrus, right inferior parietal lobule, right middle occipital gyrus, left lingual gyrus, right putamen and right caudate. These results suggested that the white matter integrity of the whole brain was disrupted in first-episode remitted geriatric depression, and that these abnormalities were perhaps involved in the psychopathology and pathophysiology of cognitive impairment in remitted geriatric depression.  相似文献

目的研究无先兆偏头痛患者大脑结构和功能异常的MRI表现。方法选取30例无先兆偏头痛患者作为观察组,同期25例健康志愿者作为对照组。Signa Excite 3.0 T磁共振扫描仪,采用平面回波成像序列采集对象静息态功能MRI图像,采用单次激发回波平面成像序列采集脑白质DTI图像。采用肯德尔系数评估静息态局部一致性,采用基于纤维束追踪空间统计分析技术(TBSS)分析脑白质纤维的分数各项异性(FA)、平行弥散系数(RD)、轴向弥散系数(AD)、以及平均弥散系数(MD)。结果与对照组比较,观察组患者多个脑区存在局部一致性降低,包括左侧前后扣带回、左侧颞下回、左侧梭状回、右侧中央前后回、右侧额中回、右侧舌回局部一致性减弱(FEW校正,P0.05)。观察组患者双侧大脑脚、双侧内囊后肢、右侧内囊前肢、右侧上纵束、右侧胼胝体体部和压部、右侧上放射冠和后放射冠、左侧扣带的FA值显著降低(FEW校正,P0.05);双侧上放射冠和后放射冠、双侧扣带、双侧丘脑后辐射、双侧上纵束、右侧大脑脚、右侧内囊前后肢以及右侧胼胝体体部和压部的RD值显著升高(FEW校正,P0.05);胼胝体体部和压部,右侧上放射冠和后放射冠,以及右侧上纵束的MD值显著升高(FEW校正,P0.05);两组各脑区的AD值无显著性差异(FEW校正,P0.05)。结论无先兆偏头痛患者大脑静息态局部一致性和脑白质纤维束存在改变,且明显区别于健康志愿者。  相似文献

We observed the characteristics of white matter fibers and gray matter in multiple sclerosis patients, to identify changes in diffusion tensor imaging fractional anisotropy values following white matter fiber injury. We analyzed the correlation between fractional anisotropy values and changes in whole-brain gray matter volume. The participants included 20 patients with relapsing-remitting multiple sclerosis and 20 healthy volunteers as controls. All subjects underwent head magnetic resonance imaging and diffusion tensor imaging. Our results revealed that fractional anisotropy values decreased and gray matter volumes were reduced in the genu and splenium of corpus callosum, left anterior thalamic radiation, hippocampus, uncinate fasciculus, right corticospinal tract, bilateral cingulate gyri, and inferior longitudinal fasciculus in multiple sclerosis patients. Gray matter volumes were significantly different between the two groups in the right frontal lobe(superior frontal, middle frontal, precentral, and orbital gyri), right parietal lobe(postcentral and inferior parietal gyri), right temporal lobe(caudate nucleus), right occipital lobe(middle occipital gyrus), right insula, right parahippocampal gyrus, and left cingulate gyrus. The voxel sizes of atrophic gray matter positively correlated with fractional anisotropy values in white matter association fibers in the patient group. These findings suggest that white matter fiber bundles are extensively injured in multiple sclerosis patients. The main areas of gray matter atrophy in multiple sclerosis are the frontal lobe, parietal lobe, caudate nucleus, parahippocampal gyrus, and cingulate gyrus. Gray matter atrophy is strongly associated with white matter injury in multiple sclerosis patients, particularly with injury to association fibers.  相似文献

We examined the associations of current alcohol consumption with brain morphometric measures in a healthy, community-dwelling cohort. Cranial T1-weighted 3D-structural MRI scans were obtain in 383 adults (men=211) aged 60-64 years, randomly selected form the larger PATH Through Life study. Voxel-based morphometric analyses were applied to detect regional gray matter and white matter volume changes related to reported weekly alcohol consumption (mean 7.04+/-8.15 drinks per week). Alcohol consumption in men had a linear association with greater gray matter in bilateral superior and medial frontal gyrus, bilateral middle occipital gyrus, right inferior parietal gyrus, bilateral precentral gyrus, left paracentral gyrus, left uncus and left inferior occipital gyrus, and with lesser white matter in bilateral superior temporal and left parahippocampal gyrus, after adjustment for age, education, total intracranial volume, smoking, hypertension, diabetes and hyperlipidemia. In women, there was no significant linear association between alcohol consumption and total or regional brain volumes. Our results showed a dose-related, sexually dimorphic impact of alcohol on brain tissue volumes independent of cerebrovascular risk factors. These findings are consistent with an inverse-U association between alcohol use and brain morphometry, while suggesting an increased vulnerability of white matter to alcohol-related brain damage.  相似文献

PURPOSE: Rasmussen encephalitis is a progressive inflammatory process with difficult-to-control focal or lateralized seizure activity, leading to hemispheric dysfunction and atrophy in advanced stages. Anatomic changes of atrophy may be subtle in earlier phases of the disease, and progressive changes on serial scans may be difficult to detect. We report a case of early-stage Rasmussen encephalitis with a relatively stable clinical course in whom we performed magnetic resonance imaging (MRI)-based volumetric analysis over an interval of 1 year, to assess for volumetric changes. METHODS: Volumetric analysis was performed on two successive MRI scans obtained at age 5 and 6 years, by using the CARDVIEWS program (J Cogn Neurosci, 1996). The images were segmented into gray- and white-matter structures according to signal intensity of their borders semiautomatically, with manual corrections. The cerebral cortex was further subdivided into smaller parcellation units according to anatomic landmarks identifiable on MRI. RESULTS: Stable left cerebral hemispheric atrophy and progressive atrophy in the left precentral gyrus, left inferior frontal gyrus, and left cerebellar atrophy were detected over the 1-year interval. CONCLUSIONS: Volumetric analysis enables early detection and quantification of anatomic changes, identification of focal involvement, and assists in determining the severity of disease and timing for surgical interventions such as hemispherectomy.  相似文献

Cranial T1-weighted 3D-structural MRI scans were obtain in 383 adults (men = 211) aged 60–64 years, randomly selected form the larger PATH Through Life study. Voxel-based morphometric analyses were applied to detect regional gray matter and white matter volume changes related to reported weekly alcohol consumption (mean 7.04 ± 8.15 drinks per week). Alcohol consumption in men had a linear association with greater gray matter in bilateral superior and medial frontal gyrus, bilateral middle occipital gyrus, right inferior parietal gyrus, bilateral precentral gyrus, left paracentral gyrus, left uncus and left inferior occipital gyrus, and with lesser white matter in bilateral superior temporal and left parahippocampal gyrus, after adjustment for age, education, total intracranial volume, smoking, hypertension, diabetes and hyperlipidemia. In women, there was no significant linear association between alcohol consumption and total or regional brain volumes. Our results showed a dose-related, sexually dimorphic impact of alcohol on brain tissue volumes independent of cerebrovascular risk factors. These findings are consistent with an inverse-U association between alcohol use and brain morphometry, while suggesting an increased vulnerability of white matter to alcohol-related brain damage.  相似文献

Depression and apathy are among the most common neuropsychiatric disturbances in Parkinson's disease (PD), and among the most important factors associated with a poor quality of life. However, their neural bases remain unclear. The results of the magnetic resonance imaging (MRI) studies on depression in PD differ dramatically. Some of them proposed a role of morphologic changes in the mediodorsal thalamus. In contrast to previous voxel-based morphometry (VBM) data, our study did not confirm a decrease in gray matter (GM) density in any brain region of depressed PD patients. Instead, a more severe white matter (WM) loss in the right frontal lobe was found, including the anterior cingulate bundle and the inferior orbitofrontal (OF) region. We suggested that the negative correlation between the severity of depression and WM density in the right OF region reinforces the hypothesis of depression in PD as a "disconnection syndrome". Only one MRI study using VBM found that high apathy scores correlated with low GM density values in the right (posterior) cingulate gyrus and the bilateral inferior frontal gyrus, in line with the findings in Alzheimer's disease and elderly adults with major depression.  相似文献

Frontotemporal dementia is a degenerative brain condition characterized by focal atrophy affecting the frontal and temporal lobes predominantly. Changes in white matter with disease progression and their relationship to grey matter atrophy remain unknown in FTD. This study aimed to establish longitudinal white matter changes and compare these changes to regional grey matter atrophy in the main FTD subtypes. Diffusion and T1‐weighted images were collected from behavioral‐variant FTD (bvFTD: 12), progressive non‐fluent aphasia (PNFA: 10), semantic dementia (SD: 11), and 15 controls at baseline and 12 months apart. Changes in white matter integrity were established by fractional anisotropy, mean, axial and radial diffusivity measurements using tract‐based spatial statistics. Patterns of cortical grey matter atrophy were measured using voxel‐based morphometry. At baseline, bvFTD showed severe cross‐sectional changes in orbitofrontal and anterior temporal tracts, which progressed to involve posterior temporal and occipital white matter over the 12‐month. In PNFA, cross‐sectional changes occurred bilaterally in frontotemporal white matter (left > right), with longitudinal changes more prominent on the right. Initial white matter changes in SD were circumscribed to the left temporal lobe, with longitudinal changes extending to bilateral frontotemporal tracts. In contrast, progression of grey matter change over time was less pronounced in all FTD subtypes. Mean diffusivity was most sensitive in detecting baseline changes while fractional anisotropy and radial diffusivity revealed greatest changes over time, possibly reflecting different underlying pathological processes with disease progression. Our results indicate that investigations of white matter changes reveal important differences across FTD syndromes with disease progression.
