Purpose

Groove pancreatitis is a rare focal form of chronic pancreatitis that occurs in the pancreaticoduodenal groove between the major and minor papillae, duodenum and pancreatic head. Radiologic appearance and clinical presentation can result in suspicion of malignancy rendering pancreaticoduodenectomy inevitable. This study reports dual phase CT findings in a series of 12 patients with pathology proven groove pancreatitis.

Materials and methods

Retrospective review of preoperative CT findings in 12 patients with histologically proven groove pancreatitis after pancreaticoduodenectomy. Size, location, attenuation, presence of mass or cystic components in the pancreas, groove and duodenum, calcifications, duodenal stenosis and ductal changes were recorded. Clinical data, laboratory values, endoscopic ultrasonographic and histopathological findings were collected.

Results

Soft tissue thickening in the groove was seen in all patients. Pancreatic head, groove and duodenum were all involved in 75% patients. A discrete lesion in the pancreatic head was seen in half of the patients, most of which appeared hypodense on both arterial and venous phases. Cystic changes in pancreatic head were seen in 75% patients. Duodenal involvement was seen in 92% patients including wall thickening and cyst formation. The main pancreatic duct was dilated in 7 patients, with an abrupt cut off in 3 and a smooth tapering stricture in 4. Five patients had evidence of chronic pancreatitis with parenchymal calcifications.

Conclusion

Presence of mass or soft tissue thickening in the groove with cystic duodenal thickening is highly suggestive of groove pancreatitis. Recognizing common radiological features may help in diagnosis and reduce suspicion of malignancy.  相似文献

Groove pancreatitis is a distinct form of chronic pancreatitis characterized by inflammation and fibrous tissue formation, affecting the groove area between the head of the pancreas, the duodenum and the common bile duct. It is manifested on imaging by a sheet-like mass in the groove area near the minor papilla. Thickening of the duodenal wall and cystic transformation in the duodenal wall also represent common imaging features. Pathogenesis is still unclear, and clinical presentation is not specific. Endoscopic ultrasonography (EUS), computed tomography (CT) and magnetic resonance imaging (MRI) demonstrate imaging findings consistent with the disease in typical cases, but specific diagnosis is challenging in a number of patients where biopsy is required. The disease may mimic pancreatic, common bile duct or duodenal wall cancer that requires prompt and excessive surgical intervention, as opposed to groove pancreatitis where initial conservative treatment is suggested. The clinical, histopathological and radiological features on cross-sectional imaging of this entity are discussed in this review, and differential diagnostic clues are given.  相似文献

Differential diagnosis of periampullary carcinomas at MR imaging.   总被引:19,自引:0,他引:19  
Periampullary carcinomas arise within 2 cm of the major duodenal papilla and comprise carcinomas of the ampulla, distal common bile duct, pancreas, and duodenum. Their clinical features and anatomic locations are similar, as are the therapeutic approaches; however, their long-term outcomes vary. Magnetic resonance (MR) images of 89 pathologically proved periampullary carcinomas (29 ampullary carcinomas, 27 distal common bile duct carcinomas, 21 pancreatic carcinomas, six duodenal carcinomas, and six unclassified carcinomas) were reviewed. Ampullary carcinoma manifests as a small mass, periductal thickening, or bulging of the duodenal papilla. Pancreatic carcinoma is characterized by a discrete parenchymal mass, which enhances poorly on dynamic gadolinium-enhanced images. Sometimes, two proximal and two distal pancreatic and biliary ducts appear as four separate ducts (the four-segment sign). Dilatation of side branches of the pancreatic ducts is frequently seen in pancreatic carcinoma but not in other periampullary carcinomas. Distal bile duct carcinoma manifests as luminal obliteration and wall thickening or as an intraductal polypoid mass. A dilated proximal bile duct, a nondilated distal bile duct, and a dilated or nondilated pancreatic duct may form the three-segment sign. MR cholangiopancreatography and sectional MR imaging are useful in determining the origins of periampullary carcinomas.  相似文献

OBJECTIVE: The purpose of this study was to compare ferumoxides-enhanced MR imaging with combined helical CT during arterial portography and CT hepatic arteriography for preoperative detection of hepatocellular carcinomas. SUBJECTS AND METHODS: Twenty patients with 30 hepatocellular carcinomas underwent ferumoxides-enhanced MR imaging and combined helical CT during arterial portography and CT hepatic arteriography. The diagnosis was established by pathologic examination after surgical resection in 18 patients and by biopsy in two. The MR protocol included fast spin-echo with two echo times, T2(*)-weighted fast multiplanar gradient-recalled acquisition in the steady state, proton density-weighted fast multiplanar spoiled gradient-recalled echo, and T1-weighted fast multiplanar spoiled gradient-recalled echo images. The MR images of all sequences and the paired CT during arterial portography and CT hepatic arteriography images were independently evaluated by three radiologists on a segment-by-segment basis. Diagnostic accuracy was assessed with receiver operating characteristic analysis. RESULTS: The accuracies (A(z) values) of ferumoxides-enhanced MR imaging and combined CT during arterial portography and CT hepatic arteriography for all observers were 0.964 and 0.948, respectively. The mean sensitivities of MR imaging and CT were 93% and 91%, respectively. The differences were not statistically significant. The mean specificity of MR imaging (99%) was significantly higher than that of combined CT during arterial portography and CT hepatic arteriography (94%). CONCLUSION: Ferumoxides-enhanced MR imaging can be used successfully in place of combined CT during arterial portography and CT hepatic arteriography for the preoperative evaluation of patients with hepatocellular carcinomas.  相似文献

OBJECTIVE: The purpose of this study was to assess perfusion patterns seen on slow-infusion MR arteriography using the hepatic arterial infusion system compared with those seen on CT arteriography. SUBJECTS AND METHODS: In 37 patients with liver metastases who had implantable port systems for hepatic arterial infusion chemotherapy, slow-infusion MR arteriography using an infusion rate of 10 mL/hr through an implantable port and CT arteriography using an injection rate of 0.7 mL/sec were performed. In 15 of 37 patients, we evaluated enhancement patterns of tumors of the liver and visceral organs using slow-infusion MR arteriography. In all 37 patients, we compared slow-infusion MR arteriography with CT arteriography concerning intra- and extrahepatic perfusion patterns. RESULTS: On slow-infusion MR arteriography performed 10-20 min after initiation of infusion, tumors of the liver revealed significant enhancement with only a slight effect of systemic enhancement. In seven (19%) of 37 patients, intrahepatic distributions on slow-infusion MR arteriography differed from those on CT arteriography. In eight patients, the patterns of extrahepatic perfusion into the duodenum and the pancreas head differed on slow-infusion MR arteriography from those seen on CT arteriography. In addition, strong artifact caused by platinum coils in the gastroduodenal artery interfered with the evaluation of perfusion in the area around the coils on CT arteriography, whereas no imaging artifact was seen on slow-infusion MR arteriography. CONCLUSION: We believe that slow-infusion MR arteriography reflects the actual distribution of infused drugs more accurately than CT arteriography. When clinical complications occur during treatment, slow-infusion MR arteriography should be used to assess perfusion abnormalities.  相似文献

目的 探讨十二指肠恶性肿瘤的CT诊断与鉴别诊断。方法 回顾性分析 2 0例手术病理证实的十二指肠恶性肿瘤的CT平扫和增强资料。结果 原发十二指肠恶性肿瘤 8例 ,其中十二指肠腺癌 3例 ,恶性淋巴瘤 2例 ,恶性间质瘤 3例 ;十二指肠继发性恶性肿瘤 12例 ,其中壶腹癌 3例 ,胰头癌 8例 ,胰腺无功能性神经内分泌癌 1例。十二指肠腺癌CT表现为肠腔局限性不规则环状狭窄伴软组织肿块 ,增强扫描肿块有中度强化 ;恶性淋巴瘤的特点是长范围的肠壁增厚或肠腔外肿块 ,增强扫描肿块有轻度强化 ;恶性间质瘤的特点是巨大软组织肿块伴明显、不均匀强化 ,肠梗阻不明显。壶腹癌表现为十二指肠降部内侧壁的局限性肿块 ,增强有轻中度强化 ,伴肝内外胆管扩张 ;胰头癌累及十二指肠表现为十二指肠内侧壁凹凸不平 ,邻近的胰头部肿块 ,增强扫描不强化 ,伴肝内外胆管扩张 ;胰腺无功能性神经内分泌癌侵及十二指肠表现为胰头部巨大肿块伴明显强化 ,肝内外胆管不扩张 ,肿块与十二指肠分界不清。结论 CT检查对十二指肠恶性肿瘤有重要的诊断和鉴别诊断价值。  相似文献

Autoimmune pancreatitis: imaging features   总被引:24,自引:0,他引:24  
PURPOSE: To retrospectively determine imaging findings in patients with autoimmune pancreatitis. MATERIALS AND METHODS: Twenty-nine patients (25 male and four female; mean age, 56 years; range, 15-82 years) with histopathologic diagnosis of autoimmune pancreatitis were examined. Data were reviewed by two radiologists in consensus. Imaging findings for review included those from helical computed tomography (CT), 25 patients; magnetic resonance (MR) imaging with MR cholangiopancreatography (MRCP), four patients; endoscopic ultrasonography (US), 21 patients; endoscopic retrograde cholangiopancreatography (ERCP), 19 patients; and percutaneous transhepatic cholangiography, one patient. Images were analyzed for appearances of pancreas, biliary and pancreatic ducts, and other findings, such as peripancreatic inflammation, encasement of vessels, mass effect, pancreatic calcification, peripancreatic nodes, and peripancreatic fluid collection. Follow-up images were available in nine patients. Serologic markers such as serum immunoglobulin G (IgG) and antinuclear antibody levels were available in 12 patients. RESULTS: CT showed diffuse (n = 14) and focal (n = 7) enlargement of pancreas. Seven patients had minimal peripancreatic stranding, with lack of vascular encasement, calcification, or peripancreatic fluid collection. Nine patients had enlarged peripancreatic lymph nodes. MR imaging showed focal (n = 2) and diffuse (n = 2) enlargement with rimlike enhancement in one. MRCP revealed pancreatic duct strictures in two and sclerosing cholangitis-like appearance in one. Endoscopic US showed diffuse enlargement of pancreas with altered echotexture in 13 patients and focal mass in the head in six. ERCP showed stricture of distal common bile duct in 12 patients, irregular narrowing of intrahepatic ducts in six, diffuse irregular narrowing of pancreatic duct in nine, and focal stricture of proximal pancreatic duct in six. Serologic markers showed increased IgG and antinuclear antibody levels in seven of 12 patients. At follow-up, CT abnormalities and common bile duct strictures resolved after steroid therapy in three patients. CONCLUSION: Features that suggest autoimmune pancreatitis include focal or diffuse pancreatic enlargement, with minimal peripancreatic inflammation and absence of vascular encasement or calcification at CT and endoscopic US, and diffuse irregular narrowing of main pancreatic duct, with associated multiple biliary strictures at ERCP.  相似文献

目的 探讨MRI联合序列检查在壶腹周围癌(VPC)鉴别诊断中的价值.方法 回顾性分析53例经手术病理证实的VPC患者的MRI资料(壶腹癌17例、胰头癌15例、十二指肠乳头癌10例、胆总管下段癌11例),所有患者均行常规上腹部MRI平扫、磁共振胰胆管成像(MRCP)及多期动态增强扫描,重点观察分析肿瘤的形态学特征、动态强化特点、胰胆管形态学改变并测量相关数据.结果 4组VPC在大小、中心位置及信号均匀性方面总体存在差异性(P<0.05),其动态强化特点也存在差别;VPC最常出现“双管征”(26/53),胰头癌易出现“四管征”(8/15),胆总管下段癌易出现“三管征”(8/11);“鸟嘴征”多见于壶腹癌(10/17),“鼠尾征”多见于胰头癌(7/15),“截断征”多见于胆总管下段癌(10/11);胰头癌组主胰管直径最大、胆总管下段癌组最小且其差异有统计学意义(P<0.05);胰头癌组和胆总管下段癌组的胆总管梗阻末端及主胰管梗阻末端到十二指肠主乳头的距离、胰胆管汇合角度均大于壶腹癌组及十二指肠乳头癌组.结论 MRI联合序列检查能较好显示不同组织学来源VPC的影像学特征,综合分析这些特征有助于胰头壶腹区肿瘤组织学类型的鉴别.  相似文献

目的:探讨沟槽区胰腺炎的MSCT征象,提高对该病的影像学诊断水平.方法:回顾性分析53例GP患者的临床及CT影像学资料,所有病例经手术病理或ERCP活检病理证实.观察沟槽区肿块或囊状病变的强化特点和分型,以及邻近结构的影像学改变.结果:53例的主要CT表现:①均可见沟槽区软组织增厚改变,CT平扫呈等或混杂稍低密度,动脉期呈轻度强化或无强化,实质期呈渐进性不均匀延迟强化;②57%(30/53)伴有副胰管开口邻近十二指肠降部肠壁或胰头实质的囊性变(长径10~48 mm);③十二指肠降部肠壁增厚45例(85%),其中40例为弥漫性水肿,5例为局部性水肿;④胰管扩张占19%(10/53),胆总管扩张占15%(8/53);⑤其它:沟槽区积液占89%(47/53),淋巴结炎(短径8~17 mm)占23%(12/53).结论:沟槽区胰腺炎的MSCT征象具有一定特征性,双期增强扫描对本病的诊断具有重要价值.  相似文献

The objective of this study was to demonstrate the appearance of ampullary carcinoma using current MR techniques, including fat suppression, gadolinium enhancement, and MR cholangiography. Nine patients with ampullary carcinoma were examined by MRI at 1.5 T. MR examinations included T1-weighted spoiled gradient echo, T1-weighted fat-suppressed, and immediate postgadolinium spoiled gradient echo images for all patients and MR cholangiography for three patients. The imaging features of ampullary carcinomas, including tumor size and morphology, signal intensity, and enhancement characteristics, were determined. Ampullary carcinomas shown on MR images ranged in size from 1.5 to 5.5 cm. Tumors were low in signal intensity on precontrast T1-weighted spoiled gradient echo and T1-weighted fat-suppressed images relative to normal pancreatic tissue and enhanced less than normal pancreas on immediate postgadolinium spoiled gradient echo images. Tumor conspicuity was greatest on immediate postgadolinium spoiled gradient echo images. MR cholangiography demonstrated high grade obstruction of the common bile duct and mild dilatation of the pancreatic duct at the level of the ampulla with abrupt termination of the ducts in two untreated patients and moderate dilatation of the common bile duct in one patient who had a biliary stent. Ampullary carcinomas can be demonstrated on MR images as small masses arising at the ampulla. Tumors are well defined on immediate postgadolinium spoiled gradient echo images.  相似文献

Advanced imaging of melorheostosis with emphasis on MRI   总被引:3,自引:0,他引:3  
Objective: To describe the CT and MR imaging appearance of both osseous and extraosseous manifestations of melorheostosis. Design and patients: We retrospectively reviewed the CT (n=7) and/or MR imaging findings (n=12) of 17 patients with characteristic radiographic findings of melorheostosis (undulating cortical hyperostosis with marked uptake on radionuclide bone scintigraphy). Results: CT and MR imaging revealed cortical hyperostosis as high attenuation and low signal intensity on all MR pulse sequences, respectively. Encroachment on the marrow space was seen in all cases resulting from endosteal involvement. Thirteen patients demonstrated 14 soft tissue masses with infiltrative margins in 80% of cases and seven showed extensive mineralization on CT or MR imaging (low intensity on all pulse sequences). Seven soft tissue masses were predominantly nonmineralized with intermediate signal intensity on T1-weighted and intermediate to high signal on T2-weighted MR images corresponding to vascularized fibrous tissue with variable collagen content pathologically. Enhancement after intravenous gadolinium was seen in all patients imaged with soft tissue masses (n=2). Two patients demonstrated muscle atrophy resulting from nerve involvement. Conclusions: The osseous abnormalities in melorheostosis are identical on advanced imaging and radiographs. Mineralized or nonmineralized soft tissue masses should be recognized as another manifestation of this disease as opposed to a more ominous finding, making biopsy unwarrranted. Groove pancreatitis is an uncommon type of chronic pancreatitis that affects the space between the head of the pancreas, the second portion of the duodenum, and the common bile duct. The main trigger is chronic alcohol abuse, which eventually leads to leakage of pancreatic juices into the pancreaticoduodenal groove, causing inflammation and fibrosis. The main differential diagnosis is with pancreatic adenocarcinoma, which is more common than groove pancreatitis.Different imaging techniques make it possible to identify various findings (e.g., duodenal thickening or duodenal and paraduodenal cysts, which are characteristic of groove pancreatitis) that sometimes enable differentiation between groove pancreatitis and other entities, although there are no specific findings for each of them. Sometimes biopsy or surgery is required to establish the definitive diagnosis.The treatment of groove pancreatitis is usually conservative, but in cases in which the symptoms do not improve, interventional procedures (biliary drainage) or surgery (Whipple technique) can be done.  相似文献

Magnetic resonance (MR) images and computed tomographic (CT) scans of nine patients with histologically proved cholangiocarcinoma were compared retrospectively to assess the potential of MR imaging in the detection and staging of the disease. Cholangiocarcinomas were demonstrated as soft-tissue masses by both techniques in seven of the nine patients. In three patients, the masses were more apparent with MR because of a greater degree of contrast between the tumor and the surrounding tissues. In all four patients with the scirrhous subtype of cholangiocarcinoma, the soft-tissue masses showed decreased signal intensity on the second spin-echo image (echo time = 56 msec). Displacement or encasement of the adjacent vessels was well demonstrated by MR. Distal extension of the tumor (hepatic metastases, regional lymphadenopathy) appeared on both MR images and CT scans but was more apparent with MR. Both MR and CT demonstrated intrahepatic bile duct dilatation, but CT demonstrated it more readily. MR appears to be an effective modality for the detection and staging of cholangiocarcinoma.  相似文献

目的分析研究眶壁转移瘤的CT和MRI特点,提高诊断准确率.方法回顾性分析23例经手术病理证实的眶壁转移瘤的CT和MRI表现,成人组16例,儿童组7例.所有病例均行CT平扫与MR平扫和增强扫描.结果16例成年患者CT表现均为溶骨性骨质破坏和眼眶不规则肿块,9例骨质破坏区邻近的颅内可见扁平不规则肿块;16例MRI表现为骨质破坏区邻近的眼眶和颅内均可见略长T1、略长T2信号的不规则软组织影,增强后呈明显强化.7例儿童患者CT表现均为溶骨性骨质破坏和眼眶不规则肿块,其中2例可见垂直针状高密度影,3例骨质破坏区邻近的颅内可见扁平不规则软组织肿块;MRI显示7例骨质破坏区邻近的眼眶内和6例颅内可见略长T1、略长T2信号不规则的软组织影,其中1例为双侧,增强后呈明显强化;7例双侧眶骨、4例斜坡和双侧岩尖及1例双侧颞骨鳞部骨髓腔呈略长T1、略长T2信号影,增强后呈明显强化.结论眶壁转移瘤的CT和MRI表现有一些特点,有助于诊断和鉴别诊断.  相似文献

To assess the role of 1.5-T magnetic resonance (MR) imaging in evaluation of the adnexa, 43 consecutive examinations that revealed 61 adnexal masses were retrospectively reviewed. T1- and T2-weighted images in coronal, axial, and/or sagittal planes were included. Available ultrasound (US) (n = 30) and/or computed tomographic (CT) (n = 9) scans were then correlated with the MR images. On T2-weighted images at least part of all adnexal masses was of higher signal intensity than surrounding muscle and adipose tissue, and therefore the adnexal masses were best seen with these sequences. T1-weighted imaging improved tissue characterization by revealing signal characteristics of fat in teratomas and characteristics of blood in endometriomas or hemorrhagic cysts, pelvic inflammatory disease, ovarian carcinomas, serous cystadenomas, and teratomas. MR imaging provided additional information or increased diagnostic confidence in 25 of 30 patients who underwent US or CT. MR imaging is a promising problem-solving modality after US in the study of adnexal abnormalities.  相似文献

The pancreas develops from ventral and dorsal buds, which undergo fusion. Failure to fuse results in pancreas divisum, which is defined by separate pancreatic ductal systems draining into the duodenum. Risk of developing pancreatitis is increased in pancreas divisum. MR cholangiopancreatography (MRCP) is the technique of choice for detecting it non-invasively. Annular pancreas is the result of incomplete rotation of the pancreatic bud around the duodenum with the persistence of parenchyma or a fibrous band encircling (stenosing) the duodenum. Acute pancreatitis is usually caused by bile duct stones or alcohol abuse. Contrast-enhanced multi-detector row CT is the method of choice to assess the extent of this disease. In acute pancreatitis, the role of MRCP is mainly limited to finding bile duct stones in patients with suspected biliary pancreatitis. Chronic pancreatitis results in relentless and irreversible loss of exocrine (and sometimes endocrine) function of the pancreas. MDCT even shows subtle calcifications. MRCP is the method of choice for non-invasive assessment of the duct. Inflammatory pseudotumor in chronic pancreatitis and groove pancreatitis are difficult to differentiate from pancreatic cancer. In these cases, multiple imaging methods such as MDCT, MRI and endosonography including biopsy may be used to make a diagnosis.  相似文献

Duodenal neoplasms: role of CT   总被引:5,自引:0,他引:5  
In a retrospective study of 14 cases of duodenal neoplasms evaluated by computed tomography (CT), there were four primary adenocarcinomas of the duodenum, one lymphoma, five metastatic carcinomas, two duodenal lipomas, one villous adenoma, and one leiomyoma. The CT findings were diagnostic in patients with duodenal lipomas. In 11 cases, a primary origin of the mass was clearly identifiable in the duodenum. Thickening of the bowel wall, tumor necrosis, ulceration, and intraluminal defects were common. The relationship of the masses to adjacent structures was clearly shown on CT scans. Extraluminal extent of the lesion was noted on CT scans in seven patients; however, at surgery only four of six were found to have extramural disease. The presence of adenopathy, liver metastases, and vascular and mesenteric invasion were also demonstrated on CT scans. Twelve patients underwent upper gastrointestinal tract series. A duodenal abnormality was seen in all 12 patients, but the extramural extent and distant metastatic involvement could not be seen on these examinations. CT scans allowed the accurate staging of eight of ten malignant lesions and thus helped in the management of duodenal tumors.  相似文献

Imaging of pancreatic neoplasms: comparison of MR and CT   总被引:2,自引:0,他引:2  
Thirty-two patients with pathologically proved pancreatic carcinomas or cystadenomas were evaluated with MR images obtained with T1-weighted spin echo (short TR/short TE), inversion recovery, and T2-weighted spin-echo (long TR/long TE) pulse sequences. CT was used as the reference standard to determine the ability of MR to delineate normal and abnormal pancreatic anatomy and thereby to exclude or detect pancreatic malignancy. Short TR/short TE spin-echo sequences were significantly better (p less than .05) than inversion recovery or T2-weighted spin-echo sequences in resolution of both normal and abnormal anatomy. Resolution of pancreatic anatomy correlated (r = .9) with the image signal-to-noise ratio. In seven (22%) of 32 cases, MR visualized pancreatic tumors better than CT did because it showed a signal intensity difference between the tumor and normal pancreatic tissue. Overall, the slight superiority of MR over CT for tumor visualization tended to occur in larger tumors and was not statistically significant. On T1-weighted images, 63% (20 of 32) of pancreatic tumors studied had lower signal intensities than normal pancreatic tissue, whereas on T2-weighted sequences (TE = 60, 120, and 180 msec) only 41% (13 of 32) of tumors had increased signal intensities. Currently available MR imaging techniques offer no significant advantages over CT for evaluating the pancreas for neoplasia.  相似文献

The relationship between tumor mass and vascular involvement as seen on MR imaging was examined in 11 patients with masses in the parasellar region, and the findings were correlated with CT and angiography. In six cases, MR was superior to CT and angiography in depicting the relationship of the tumor to adjacent blood vessels. In these cases, MR demonstrated tumor surrounding the blood vessel without changing the diameter of its lumen. Angiography did not reveal encasement in these cases. In four cases, both MR and angiography showed signs of vascular encasement with narrowing of the vessel's lumen. In two cases, MR was equivocal while angiography revealed vascular encasement in one case and was negative for encasement in the other. CT was less sensitive than MR in defining vascular encasement since there is usually little contrast between an enhancing tumor and the major blood vessels. Coronal scanning appeared to be the best plane of imaging and correlated well with the anteroposterior angiogram. We propose that MR is the method of choice for evaluating arterial encasement by tumors and may obviate the need for angiography in those cases in which MR is positive for a basal lesion.  相似文献

The pancreas develops from ventral and the dorsal buds, which undergo fusion. Failure to fuse results in pancreas divisum, which is defined by separate pancreatic ductal systems draining into the duodenum. Risk of developing pancreatitis is increased in pancreas divisum because of insufficient drainage. MR cholangiopancreatography (MRCP) is the technique of choice for detecting pancreas divisum non-invasively. Annular pancreas is the result of incomplete rotation of the pancreatic bud around the duodenum with the persistence of parenchyma or a fibrous band encircling (and sometimes stenosing) the duodenum. Acute pancreatitis is usually caused by bile duct stones or alcohol abuse. The Atlanta classification differentiates between mild acute and severe acute pancreatitis associated with organ failure and/or local complications such as necrosis, abscess or pseudocyst. Contrast-enhanced multi-detector row CT is the method of choice to assess the extent of disease. Balthazar et al.’s CT severity index assesses the risk of mortality and morbidity. In acute pancreatitis, the role of MRCP is mainly limited to finding bile duct stones in patients with suspected biliary pancreatitis. Chronic pancreatitis results in relentless and irreversible loss of exocrine (and sometimes endocrine) function of the pancreas. MDCT even shows subtle calcifications. MRCP is the method of choice for non-invasive assessment of the duct. Inflammatory pseudotumor in chronic pancreatitis and groove pancreatitis are difficult to differentiate from pancreatic cancer. In these cases, multiple imaging methods such as MDCT, MRI and endosonography including biopsy may be used to make a diagnosis.  相似文献
