目的 探讨股骨侧严重骨缺损(Paprosky ⅢA型)翻修中应用髓内打压植骨结合广泛多孔涂层长柄假体的临床疗效.方法 2006年3月~2010年9月,对38例(38髋)Paprosky ⅢA型股骨骨缺损患者行翻修重建,其中男24例,女14例,平均年龄62岁.翻修原因:骨溶解、无菌性松动29例,全髋关节置换术(THA)术后感染二期翻修6例,假体周围骨折3例(Vancouver B3型).股骨侧干骺端骨缺损采用同种异体颗粒骨髓腔内打压植骨进行修复,股骨柄采用全涂层长柄假体(7~10英寸).术后定期随访,髋关节功能评价采用Harris评分,影像学采用X线片及CT观察:假体柄有无松动下沉、股骨近端应力遮挡情况、植入的异体颗粒骨与宿主骨整合情况.结果 38例均获得随访,平均随访53.4个月(23~62个月),Harris评分由术前平均42分(32~47分),提高至末次随访时平均86分(69~95分).无患者发生脱位、假体周围骨折.1例术后感染,行再次二期翻修;1例假体柄在术后6个月内下沉4.24 cm,再次翻修时选择更粗的假体柄,末次随访时假体柄稳定;其余所有患者假体柄均牢固固定.3例出现轻-中度应力遮挡.15例近端皮质骨密度及厚度有增加,厚度平均增加约1.8 mm(0.7~3.5 mm),植入骨与宿主骨逐渐整合并增加了骨缺损区的骨质储备.结论严重骨缺损(Paprosky ⅢA型)的股骨翻修中,采用髓内颗粒骨打压植骨可以很好的修复股骨中上段骨缺损,重建股骨干骺端.依靠广泛多孔涂层长柄假体在远端的牢固压配固定,结合股骨干骺端髓内紧密打压植骨,使假体柄在股骨中上段及远端均能获得较好的初始稳定性,近期临床和影像学结果满意,远期疗效有待观察.  相似文献

[目的]探讨结合应用解剖型骨水泥股骨假体和冷冻干燥颗粒骨打压植骨(IBG)进行股骨翻修的可行性及临床效果。[方法]作者在2001年1月~2005年12月期间,在髋关节翻修中,有49例在股骨翻修中应用解剖型骨水泥柄结合冷冻干燥颗粒骨打压植骨技术,其中有36例(73%)存在严重的骨缺损(Paprosky分型ⅢB或Ⅳ)。平均随访时间为35.3个月(26~52个月)。通过Harris评分和X线片对结果进行评价。[结果]Harris评分从术前的平均44.6分提高到最后评估时的平均88.3分,Harris评分优良率为89.8%。X线片未显示有明显的股骨假体下沉。1例出现术后感染,1例出现术后脱位,感染率及脱位率均为2%,3例出现了术中股骨骨折及股骨柄穿孔,其发生率为6.1%,但这些与假体及植骨材料选择无关。[结论]使用解剖型骨水泥股骨假体结合异体冷冻干燥颗粒骨打压植骨技术,对有严重骨缺损的股骨进行翻修是可行的,并且中短期的临床效果满意,长期效果还有待观察。  相似文献

目的 研究辐照深冻异体骨用于髋臼翻修中严重骨缺损的打压植骨重建的临床效果.方法 2006年2月至2009年1月髋关节翻修术中共有20例21髋应用打压植骨技术结合金属网重建严重髋臼侧骨缺损,18例19髋获得随访.患者翻修时平均年龄为64.4岁(43~81岁).采用Paprosky分型,本组患者均有髋臼骨缺损,其中PaproskyⅡB型4髋,PaproskyⅡC型8髋,PaproskyⅢA型5髋,PaproskyⅢB型2髋.手术用金属网修复髋臼节段性缺损,采用辐照深冻同种异体颗粒骨打压植骨结合骨水泥臼杯完成髋臼翻修.患者术后定期进行影像学和临床评估,观察手术前后髋关节Harris评分变化,假体移位松动及异体骨骨整合情况,及术中、术后并发症等.术前Harris评分平均42.5分(31~56分).术前疼痛评分平均14.4分(10~20分).结果 患者平均随访时间22.4个月(12~48个月).术后Harris评分提高到88.6分(82~96分).术后疼痛评分提高至平均42.3分(40~44分).并发症:术后感染1例,经清创愈合.股神经损伤1例,术后1年随访时已恢复正常.1例患者术后出现股骨近端外侧皮质吸收,大粗隆骨折.有1例Paprosky ⅢB型患者出现金属网及髋臼杯明显移位等影像学松动表现,其余18髋的髋臼假体稳定,未发现臼杯在垂直和水平方向大于1 mm的移位,也未发现髋臼假体外展角的改变;植骨层和宿主骨床可见连续骨小梁通过、移植骨与周围骨床骨密度趋向一致等骨整合表现.结论 金属网与打压植骨及骨水泥髋臼技术联合应用能有效地完成髋臼的生物学翻修.辐照深冻异体骨用于打压植骨能与周围骨床很好的整合.  相似文献

目的探讨采用大块异体骨和混合颗粒骨移植对人工全髋关节置换术后股骨严重骨缺损翻修重建的疗效。方法 1999年7月至2006年6月,采用大块异体骨和混合颗粒骨移植翻修股骨严重骨缺损为主的重建人工全髋关节稳定11例11髋。男7例,女4例,翻修时平均年龄58岁(48~81岁)。11例中10例为骨水泥固定型全髋关节假体,1例为非骨水泥固定假体。累及股骨严重骨吸收MalloryⅢC型11例,臼骨吸收5例中PaproskyⅠ或ⅡC型。辐照冻存的大块异体骨覆盖严重股骨缺损节段和混合颗粒骨填充股骨腔内缺损11例。混合颗粒异体骨打压植骨翻修骨水泥杯2例,非骨水泥杯3例。11例患者进行有规则的临床和影像学随访,行Harris评价,对假体和异体骨的移位、松动和并发症进行观察。结果 平均随访8年11个月(6~11年)。术后Harris评价:其中有9例患者由术前41~49分提高到81~89分,有2例提高到90~95分。术后疼痛(VAS)评分:平均提高至13.1分(11~20分),比翻修前41.7分(39~44分)明显改善。并发症:术后3个月发现股骨柄旋转移位,有1例出现前倾角缩小3°,到最后随访时再无移位;1例出现术后切口溢液,21d后自愈。所有11例患者的大块异体骨和混合颗粒骨在X线中均显示整合血管化良好,骨小梁通过宿主骨。结论 大块异体骨和混合颗粒骨移植是翻修严重股骨缺损的有效方法,并能充分整合成周围的宿主骨。  相似文献

目的 探讨股骨前外侧皮质骨开窗技术在髋关节翻修术中取出股骨远端稳定固定骨水泥的价值.方法 2005年5月至2009年6月,共14例(14髋)因各种原因致髋关节置换术后失败患者接受全髋关节翻修手术.其中男性10例,女性4例,年龄54~75岁,平均66岁.翻修原因为股骨头置换术后髋臼磨损5例、全髋关节置换术后假体周围骨溶解并松动6例、骨水泥柄股骨近端骨溶解柄断裂1例、髋臼骨溶解假体松动翻修同时行股骨柄翻修1例,感染后二期翻修时远端骨水泥取出困难1例.14例(14髋)股骨柄均为骨水泥同定.术中按术前计划开窗部位、开窗范围于股骨皮质骨开一长方形骨窗.通过骨窗直视下彻底清除髓腔内稳定固定的骨水泥,修整股骨髓腔.植入翻修柄后将皮质骨开窗骨瓣原位回植,双股钢丝捆绑固定.术后定期随访拍摄x线片.观察皮质骨开窗骨瓣与周围骨愈合情况、骨瓣有无移位、股骨柄有无下沉以及有无捆绑钢丝断裂等.结果 10例患者术后获得随访,平均随访时间24.6个月.股骨皮质骨开窗长度2.5~6.0 cm,平均3.4 cm,宽度0.8~1.4 cm,平均1.2 cm.股骨开窗远端以远部分发生纵形劈裂骨折1例.无术中皮质骨穿孔及股骨干骨折.向远段扩大开窗1例,扩大长度1.5 cm.开窗部位皮质骨骨瓣原位回植选择2~3道双股钢丝固定,平均2.3道.随访期间2例发牛假体柄下沉(平均2.5 mm),无皮质骨瓣移位以及捆绑钢丝断裂,术后3~5个月皮质骨瓣已于周围骨纤维愈合.随访期间无一例因各种原因致再次翻修.结论 股骨皮质骨开窗技术在髋关节翻修术中有助于直视下彻底取出股骨髓腔远端稳定固定的骨水泥,同时不会造成股骨骨丢失、不影响翻修柄植入后的稳定固定.  相似文献

目的回顾性分析非骨水泥全涂层长柄假体在全髋关节翻修术中应用的临床疗效。方法对2006年1月至2011年1月四川大学华西医院收治的采用全涂层长柄假体全髋关节翻修术的53例患者(53髋)进行随访、分析。其中男26例,女27例;年龄49—78岁,平均62岁。股骨假体采用强生(Depuy)公司全涂层长度为165mm的直柄AML假体10例、全涂层165mm的Solution假体19例和203mm带弧度分左右侧的Solution假体24例。比较髋关节术前术后的Harris评分,x线片评价假体柄有无松动下沉及应力遮挡情况。结果53例患者均获随访,随访时间1.8~6.8年,平均4.7年。术前平均Harris评分为37分,最后随访时增加至86分。所有患者均在术后3个月后完全负重,原有髋关节疼痛缓解。在最后随访时,有45例股骨柄假体获得骨长人固定。并发症:术后脱位1例,手法复位后未再发生脱位,术后感染1例,假体柄明显下沉1例,后两例患者行再次翻修术,末次随访假体柄稳定。随访X线片显示多数患者骨质改建,密度增加,未发现假体周围有连续亮带及假体柄下沉的患者。结论复杂的股骨侧翻修手术,应尽可能选择非骨水泥长柄假体。特别是股骨干骺端骨质条件差,中段有骨缺损,近端不能获得牢固固定的病例。术中结合同种异体颗粒骨打压植骨处理股骨中上段中重度骨缺损,中期临床疗效良好,远期效果有待随访。  相似文献

目的 回顾性研究采用大转子延长截骨(extended troehanteric osteotomy,ETO)行全髋关节翻修术后股骨柄的位置变化,评价ETO在股骨假体稳定件髋关节翻修术中的作用.方法 1998年1月至2007年6月,采用ETO对股骨柄或骨水泥壳固定稳定性全髋关节33例33髋进行翻修.翻修术后采用Harris评分和MOMAC评分评估髋关节功能,摄动态X线片观察截骨块愈合、假体位置改变及股骨柄与股骨髓腔匹配等情况.结果 25例随访12~103个月,平均63个月.Harris评分由术前平均38.4分,提高到末次随访时88.7分;WOMAC评分由术前平均56.2分,降至末次随访时42.8分.大转子截骨块均在术后4~10个月骨性愈合.3例发生股骨柄下沉.平均3.4mm.股骨柄假体出现外翻、内翻各1例.无术中或术后骨折、钢丝断裂、感染、假体周围骨溶解以及异位骨化发生.术后关节脱位1例.结论 对假体固定稳定性股骨柄进行翻修,采用ETO有利于假体的安全取出,术后截骨块愈合率高,延长截骨不影响假体稳定性.股骨柄下沉、位置改变、截骨块骨折等并发症发生率低.  相似文献

目的分析全涂层远端固定长柄假体治疗髋关节置换术后假体周围骨折的临床效果。方法回顾性分析35例使用全涂层远端固定长柄假体治疗的髋关节置换术后股骨假体周围骨折病例,根据Vancouver分型,B2型12例,B3型20例,C型3例,通过Harris评分对患髋进行功能评价,X线片观察骨折愈合、股骨柄下沉情况。结果3例行单纯长柄假体翻修,10例行长柄假体翻修+钢丝环扎固定,22例行长柄假体翻修+同种异体颗粒骨压配植骨+同种异体骨板捆绑固定治疗,平均随访4.5年,所有骨折均顺利愈合,平均愈合时间为15.3周,Harris评分平均86.5分,2例股骨柄分别下沉3mm和4mm,均在术后3个月达到稳定,32例假体获得骨长入固定,3例获得稳定的纤维固定,未发现骨溶解、感染以及再发骨折。结论髋关节置换术后假体周围骨折选择全涂层远端固定长柄假体翻修,根据骨缺损严重程度合理选择同种异体颗粒骨打压植骨+同种异体骨板捆绑固定可以获得良好的临床效果。  相似文献

目的 探讨颗粒骨打压植骨结合非骨水泥臼在髋臼翻修中的临床应用.方法 应用颗粒骨打压植骨结合非骨水泥臼技术重建髋臼骨缺损10例,术中使用异体颗粒骨直径4~6 mm,髋臼假体均为生物型髋臼,上2或3枚螺钉固定,内衬均为高交联聚乙烯.结果 切口均一期愈合,无感染、脱位等并发症发生.随访9~28个月,平均19.5个月,X线片上髋臼骨床、颗粒骨及假体界面未见明显透亮带存在,原骨缺损区修复良好.Harris评分术前平均38.1分,术后平均85.5分.结论 颗粒骨打压植骨结合非骨水泥臼固定方法可有效修复髋关节翻修中髋臼AAOSⅡ、Ⅲ型骨缺损,恢复骨量,并能提供良好的早期稳定性.  相似文献

背景:颗粒骨打压植骨是修复髋臼骨缺损的重要方法,对大面积髋臼骨缺损采用打压植骨结合非骨水泥臼杯重建仍然存在争议。目的:探讨同种异体颗粒骨打压植骨结合多孔非骨水泥臼杯髋臼重建术修复AAOSⅢ型髋臼大面积骨缺损的方法及近期疗效。方法:随访2005年6月至2010年4月收治的28例(28髋)AA0SⅢ型髋臼大面积骨缺损患者。男18例,女10例,年龄34-74岁,平均53.8岁。髋关节置换术后感染醐翻修9例,骨溶解及髋臼假体松动翻修19例。本组AAOSⅢ型髋臼骨缺损面积大,术中见髋臼骨缺损超过髋臼关节面的50%。全部采用同种异体颗粒骨打压植骨结合多孔非骨水泥臼杯髋臼重建术。术后定期随访,观察臼杯有无松动、植入骨与宿主骨整合情况,髋关节功能评分采用Harris评分。结果:28例均获得随访,术后随访时间为13-70个月,平均43个月。Harris评分由术前32-48分,平均41分,提高至末次随访时86-95分,平均90分。28例髋臼假体均固定牢固无松动征象,植入骨逐渐与宿主骨整合,无感染及坐骨神经损伤。结论:在获得臼杯初始稳定固定的情况下,大面积颗粒骨打压植骨(臼杯与宿主骨接触面积〈50%)结合多孔非骨水泥臼杯髋臼重建术修复AAOSⅢ型髋臼大面积骨缺损近期疗效较好,远期疗效尚待进一步随访。  相似文献

BACKGROUND: Cancellous impaction allografting with cement for revision of the femoral component has conventionally been performed with a polished, tapered implant, which was designed to allow subsidence of the component. However, subsidence has been associated with pain in the thigh, dislocation of the hip, and revision of the component. This prospective study tested the hypothesis that good clinical results can be achieved--without subsidence of the component--with use of impaction allografting and a precoated, collared, straight stem for difficult femoral revisions. METHODS: Twenty-nine patients had revision of the femoral component with use of impaction allografting with cement and a Harris Precoat stem. Impaction allografting was performed when loss of metaphyseal and diaphyseal bone precluded revision with more straightforward techniques or when reconstitution of bone was considered a specific goal of the reconstruction (as was sometimes the case with revision of the component in younger patients). The patients were followed prospectively and were evaluated with use of the Harris hip score and serial radiographs. The patients were followed for a minimum of four years (mean, sixty-three months), except for four who died. RESULTS: Four patients died before the minimum four-year follow-up period had elapsed; all four had the prosthesis in place at the time of death. The Harris hip scores improved from a preoperative mean of 54 points (poor) (range, 21 to 91 points) to a mean of 87 points (good) (range, 41 to 100 points) at the time of the most recent follow-up. Kaplan-Meier survivorship analysis, with aseptic loosening as the end point, was 92 percent (95 percent confidence interval, 82 to 100 percent) at six years; one additional hip failed because of a hematogenous infection at seventy-three months postoperatively, for an overall failure rate of 12 percent (three of twenty-five patients) at the time of the most recent follow-up. Two hips needed a repeat revision; one was revised because of subsidence of the stem with recurrent osteolysis and the other, in a patient who had hemodialysis, because of late sepsis. A third femoral component subsided and failed but was not revised. Radiographic evidence of bone-stock reconstitution was observed in six (29 percent) of the twenty-one patients for whom radiographs were available. As in other series of patients managed with impaction allografting, the complication rate was high; excluding the revisions, three reoperations were performed, and six patients had either intraoperative femoral fracture or perforation necessitating cerclage wiring or cortical strut allografting and cerclage wiring at the time of the procedure. There were six nonunions in eighteen patients who had been operated on with a transtrochanteric approach. CONCLUSIONS: Difficult revisions of the femoral component with use of impaction allografting and a precoated stem provided satisfactory clinical and radiographic results at the time of intermediate-term follow-up. However, the high rate of complications in our series led us to refine our indications for the procedure.  相似文献

We report the initial results of an ongoing randomised, prospective study on migration of the Exeter and Elite Plus femoral stems after impaction allografting, as measured by radiostereometry. Clinically, the impaction technique gave good results for both stems. The mean subsidence in the first year was 1.30 mm and 0.20 mm for the Exeter and the Elite Plus stems, respectively. In the second year, the Exeter stem continued to subside further by a mean of 0.42 mm, while the Elite Plus stem did not do so. Subsidence of the Exeter stem correlated with deficiency of bone stock as graded on the Gustilo and Pasternak scale. This correlation was not found for the Elite Plus stem. None of the other parameters which were studied predisposed to subsidence. There was no significant association between the amount of subsidence and the radiological appearance of the graft for either stem. Our findings do not support the theory that radial compression, due to subsidence of the Exeter stem, is the essential stimulus for remodelling in impaction allografting.  相似文献

Background Substantial bone loss and bone defects increase the amount of allografting required in hip revision surgery. Thus, the use of a synthetic material to limit the amount of allograft tissue required for impaction grafting is desirable. We evaluated the potential of hydroxyapatite (HA) mixtures to provide initial mechanical stability to a polished tapered femoral stem.

Material and methods We determined the initial stability of a polished tapered femoral stem after reconstructing a cavitary femoral bone defect by impaction bone grafting with cement in Sawbones composite femurs. Three types of graft material were tested for their ability to improve initial rotational stability. The graft materials investigated were pure allograft, a mixture of 50% allograft and 50% hydroxyapatite (HA), and pure HA.

Results We found a statistically significant difference between the three groups as regards torsional micromotion and failure load.

Interpretation Our findings suggest that reconstruction of femoral bone defects with pure HA or a mixture of allograft and HA provides adequate initial stability for femoral revision arthroplasty using impaction grafting.  相似文献

We present a prospective study of impaction grafting using collared textured stems in femurs with massive defects (defined as requiring >150 cm(3) of cancellous allograft). Eighteen hips were followed for a minimum of 2 years. Average Harris hip scores increased from 50 preoperatively to 83 at most recent follow-up. Seventeen hips were functioning well at the time of most recent follow-up and exhibited no stem subsidence or aseptic loosening. One hip was associated with visible subsidence and required further revision. Single-photon emission computed tomography analyses performed at an average of 37 months indicated active graft reorganization in all cases studied. Impaction allografting with collared textured stems for massive defects demonstrates good intermediate-term clinical results and may be a useful alternative to allograft stem composites and megaprostheses.  相似文献

Periprosthetic femoral fractures are increasing in frequency and in complexity. They occur intra-operatively, or post-operatively, when they are frequently associated with loosening, with or without osteolysis. Periprosthetic femoral fractures usually occur at low energy levels, either after falls or spontaneously during activities of daily living. At present, the Vancouver classification system probably comes closest to the ideal, as it considers the fracture configuration, the stability of the implant and the quality of the bone stock. When the stem is stable, open reduction and internal fixation is suggested. Several authors have used strut grafts for the treatment of periprosthetic femoral fractures, with good results. If the stem is loose, or revision is indicated for other reasons, it is generally advisable to by-pass the most distal fracture line with a longer stem prosthesis by at least two femoral diameters. Augmentation of this intra-medullary fixation with an external cortical strut to improve rotational stability and/or internally with impaction allografting to compensate for bone defects is also advisable. Vigilant post-operative clinical and radiological assessment following total hip replacement should identify those with recurrent dislocation, loosening, subsidence and osteolysis. These patients are at greatest risk of developing femoral periprosthetic fractures.  相似文献

Options in the management of the deficient femur during revision hip arthroplasty include cemented or cementless fixation. The results with cemented femoral revision have not been historically successful. While the use of extensively coated implants in revision total hip arthroplasty has been more uniformly excellent, issues, such as thigh pain, stress shielding, and lack of bone stock restoration, have been raised. Impaction grafting in revision hip arthroplasty is an attempt to reconstitute bone stock and avoid problems associated with excessively large or long uncemented stems. The original concept of impaction grafting was promoted by Slooff and applied to the femur by Gie and Ling. While originators' results were promising, issues, such as fracture and poor graft delivery, were noted. Modifications to the original technique were recently described by Howie, which used longer stems when necessary, as well as an improved graft delivery system. We report our results with 30 consecutive hips using this method. At follow-up, 3 patients were known to have died, leaving 27 for evaluation. Two of the 27 were failures: 1 recurrence of infection and 1 loose stem. The remaining 25 were clinical successes with bone stock restoration in all and no periprosthetic fractures. We believe that impaction grafting remains a viable option for the management of the severely deficient femur in whom cementless fixation methods are questionable.  相似文献

目的 评估大转子延长截骨在股骨假体固定稳定型全髋关节翻修术中应用的中期临床效果.方法 1998年1月至2005年6月对27例患者(27髋)采用大转子延长截骨对股骨柄和(或)骨水泥壳固定稳定的全髋关节翻修.临床随访评估包括Harris评分和WOMAC评分,术前Harris评分平均42.7分,WOMAC评分平均55.6分;影像学评估包括术后拍摄X线片,对比观察截骨块愈合时间、是否存在截骨延迟愈合或不愈合,截骨块是否发生移位以及假体是否下沉等.结果 共19例患者(19髋)获得随访,平均随访时间5.3年.无一例发生术中或术后骨折.术后Harris评分平均87.3分,WOMAC评分平均46.3分.所有患者大转子截骨块均于术后6个月内愈合.无股骨大转子截骨块向近端移位,3例发生股骨柄下沉,平均下沉3.4 mm,无钢丝断裂.结论 对于假体固定稳定型股骨柄翻修,采用股骨大转子延长截骨有利于手术操作和翻修假体的植入和固定,有利于截骨块的愈合,降低术中、术后并发症发生率,中期疗效显著.  相似文献

Impacted morcellised bone allograft and a Charnley stem was used to revise 59 loose femoral components in 57 consecutive patients. Femoral bone loss was rated as Endo-Klinik grade 2 in nine patients, grade 3 in 41, and grade 4 in nine. The immediate postoperative radiographs and those taken at the most recent follow-up were compared for radiolucencies, subsidence and incorporation of the graft. One patient was lost to follow-up and two were not available for radiological analysis. The mean clinical follow-up in 58 procedures was 56.7 months (24 to 144) and the mean radiological review of 56 reconstructions was 54.4 months (24 to 144). An intraoperative femoral fracture occurred in one patient (1.7%) and was successfully treated by strut grafting and cerclage wiring. Extrusion of cement through perforations or incomplete hoop fractures was detected in the postoperative radiographs of ten procedures (17%); none of these patients sustained a complete fracture. Three patients had dislocations (5%) and two (3.5%) developed painful subsidence of the stem which required a further revision. The latest follow-up radiographs in 56 reconstructions showed a well fixed stem and radiological healing of the graft in 52 (93%), and definite loosening in four (7%). Of these four, two were revised again and two were asymptomatic after a follow-up of 120 months each. The mean subsidence in the 52 successful revisions was 0.38 mm (0 to 4). Impaction allografting with a Charnley stem restored bone stock and provided adequate fixation of the stem in 93% of the hips. There was a low rate of rerevision (3.5%) and a low incidence of intraoperative and postoperative complications.  相似文献
