目的旨在探讨ASSR与ABR在极重度感音神经性耳聋幼儿及成人感音神经性耳聋患者测试中的临床应用价值.方法对36例(72耳)小于3岁极重度感音神经性耳聋幼儿分别行ASSR和ABR测试;对32例(64耳)成人感音神经性耳聋患者分别行ASSR和电测听测试.结果①极重度感音神经性耳聋幼儿ABR均未引出V波,而ASSR在0.5 kHz、1 kHz、2 kHz、4 kHz的引出率分别为66.67%、86.11%、88.89%、94.44%,ASSR在0.5 kHz、1 kHz、2 kHz、4 kHz的阈值均数、标准差分别为82.56±9.26 dB HL、90.31±6.94 dB HL、88.12±7.93 dB HL、88.62±8.12 dB HL.②对成人感音神经性耳聋患者0.5 kHz、1 kHz、2 kHz、4 kHz ASSR测试阈值与电测听语频听阈(dB HL)进行配对两两比较的t检验,各组P值均大于0.05,无显著性差异.结论ASSR有助于极重度感音神经性耳聋幼儿残余听力的客观评估,尤以高频听阈为佳;ASSR与电测听在感音神经性耳聋诊断上有良好的一致性.  相似文献

目的探讨听性稳态反应(auditory steady-state responses,ASSR)和短声诱发听性脑干反应(ABR)在感音神经性聋人群客观听阈评估中的作用及其在耳聋鉴定中的价值。方法对感音神经性聋组(35耳)及正常组(22耳)分别进行纯音听阈、ABR及ASSR测试,并记录0.5、1、2及4kHz ASSR反应阈(dBHL)、纯音听阈(dBHL)及ABR反应阈(dBnHL)。结果正常组在0.5、1、2、4kHz的ASSR反应阈与纯音听阈相比差异无统计学意义(P>0.05),ABR反应阈与各频率纯音听阈差异有显著统计学意义(P<0.05);耳聋组各频率ASSR反应阈与纯音听阈差异无统计学意义(P>0.05),ABR反应阈与0.5kHz纯音听阈差异有统计学意义(P<0.05),与1、2、4kHz纯音听阈差异无统计学意义(P>0.05)。结论ASSR与ABR联合测试是临床工作中用于客观听阈评估的有效方法。  相似文献

目的旨在观察正常年轻人多频稳态诱发电位(ASSR)测试的正常值特点。方法对60例第一军医大学学员行ASSR、ABR和电测听测试,比较男.女及左、右耳测试值的差异。结果ASSR0.5kHz、1kHz、2kHz、4kHz测试值均数与纯音听闻均数各频率的差值分别为13.86dBHL、14.63dBHL、13.06dBHL.15.28dBHL;ASSR0.5kHz、1kHz、2kHz、4KHz测试值均数与ABRV波反应阈的差值分别为6.14dBHL、5.37dBHL、6.94dBHL、4.72dBHL;男、女学员ASSR测试值比较无显著性差异(P〉0.05)。结论ASSR正常值不受性别.耳别的影响;用ASSR测试值来推断预估纯音听阈.ABRV波反应阈要综合考虑各频率的差值。  相似文献

目的探讨听力测试组合(ABR+ASSR+声场环境中的行为测听)在感音神经性聋患儿残余听力评估中的应用价值。方法48名(96耳)感音神经性聋患儿中能配合纯音测听的患儿19人(38耳)设为PTA组,进行纯音测听及ASSR检测;不能配合纯音测听的患儿29人(58耳)设为BA组,进行声场环境中的行为测听(behavior audiometry,BA)、ABR及ASSR检测。结果①PTA组0.5、1、2、4kHz各频率ASSR反应阈与纯音听阈显著相关(P〈0.01),各频率相关系数分别为0.75、0.76、0.76、0.83,建立本实验室的ASSR—PTA直线回归方程;②BA组23耳ABR无反应但仍可引出ASSR,而ASSR无反应耳ABR均未能引出;BA组29例患儿中ASSR检测反应较好耳(29耳)在0.5~4kHz四个频率上,ASSR可测得83个反应阈值,行为测听可测得89个反应阈值,综合ASSR和行为测听可以得到96个反应阈值。结论与单项听力测试方法相比,听力测试组合(行为测听+ABR+ASSR)能为更小年龄、听力损失更重的患儿进行残余听力的评估,同时能对双侧耳间听力差异、各频率的听力损失程度进行评估,为听力损失病变的定位判断提供参考。  相似文献

目的对100例极重度感音神经性听力损失婴幼儿进行短纯音听性脑干反应(tone burst auditory brainstemresponse,tb-ABR)和CE-Chirp声诱发的听性稳态反应(auditory steady-state response,CE-Chirp ASSR)测试,对各频率反应阈的结果进行总结分析,比较两种测试方法的异同点。方法对100例极重度感音神经性听力损失婴幼儿进行短纯音ABR与CE-Chirp ASSR各频率反应阈值的测试,分别记录0.5k、1.0 k、2.0 k和4.0k Hz的反应阈值,所得数据采用SPSS 11.0统计软件进行相关性检验和χ2检验。结果短纯音ABR与CE-Chirp ASSR各频率能引出不同程度的阈值,V波引出阈值百分比主要集中在91~100d BHL,500~4000Hz波V阈值引出率之间的相关系数分别为:0.852、0.911、0.870、0.910,所有P<0.001。CE-Chirp ASSR与短纯音ABR各频率反应阈的χ2检验,P>0.05。。结论短声ABR提示极重度感音性听力损失婴幼儿,其短纯音ABR与CE-Chirp ASSR各频率均有不同程度的残余听力,短纯音ABR与CE-Chirp ASSR各频率反应阈引出率有很好的相关性,耳别间也无显著性差异。  相似文献

目的:分析感音神经性聋儿童听性稳态反应(auditory steady-state response,ASSR)与行为听阈的相关性。方法63例(126耳)重度或极重度感音神经性听力损失儿童,年龄0.5~6岁,按年龄分为 A 组(0.5~1.0岁,21例),B 组(1.1~3.0岁,21例),C 组(3.1~6.0岁,21例);分别进行 ASSR 及小儿行为听阈测试,A 组采用行为观察测听(BOA),B 组采用视觉强化测听(VRA),C 组采用游戏测听(PA),采用 SPSS17.0统计软件分析二种检查结果的相关性。结果①ASSR 反应阈与行为听阈在0.5、1、2和4 kHz 处的相关系数分别为0.75、0.76、0.76、0.83,呈极显著相关(P <0.01)。②小儿行为听阈值通常比 ASSR 反应阈值低,随着频率的增高,两者间差距缩小。③A 组(相关系数0.70)各频率 ABR 反应阈与行为听阈相关性均较 B(相关系数0.78)、C(相关系数0.83)组低。结论0.5~6.0岁感音神经性聋儿童 ASSR 与行为听阈值均有良好的相关性,ASSR 可预测感音神经性聋儿言语频率的客观听阈,为低龄儿童及难以检测行为听力的患儿验配助听器提供参考。  相似文献

目的:测定成人感音神经性聋患者听觉稳态诱发反应(ASSR)和纯音听阈值,为临床诊断提供客观依据。方法:选成人感音神经性聋患者33例(65耳),行ASSR阈值纯音听测试。结果:ASSR、纯音听阈值对比有统计学意义,听阈有显著相关性,0.5、1.0、2.0、4.0kHz频区间相关系数分别为:0.769、0.772、0.743、0.756。ASSR阈值较行为阈值高,ASSR阈值与纯音听阈间差值多在3.0~10.0dB HL之间。在0.5、1.0、2.0、4.0kHz频区间分剐为:4.77、8.83、6.49、3.67dB HL,随频率增高,两者的差异有缩小趋势。结论:成人感音神经性聋患者ASSR阈值与纯音听阈间对比有统计学意义,通过测试ASSR阈值可推断出被检查者纯音听阈的阈值。  相似文献

目的 探讨听力损失儿童残余听力的评估方法 .方法 应用听性稳态反应(auditory steady-state response,ASSR)与听性脑干反应(auditory brainstem response audiometry,ABR)对395例(790耳)感音神经性聋患儿在睡眠状态下进行各频率ASSR反应阈及ABR的反应阈测试,比较其结果 .结果 395例(790耳)患儿的ASSR高频反应阈与ABR反应阈比较,差异无统计学意义(P>0.05).结论 对听力损失婴幼儿者进行ASSR与ABR测试,有助于其听力损失程度的评估及干预婴幼儿耳聋诊断,为选配助听器和植入人工耳蜗提供依据.  相似文献

目的探讨成人感音神经性聋的听觉稳态反应(auditory steady-state responses,ASSR)反应阈与纯音听阈的关系。方法选择中国医科大学附属一院耳鼻咽喉科门诊感音神经性聋的成人患者,分别进行纯音听力测试、ASSR检查,比较ASSR在0.5、1、2、4 kHz频率处的反应阈与纯音听阈的相关性及按听力损失程度比较两者的差值。结果 ASSR反应阈与纯音听阈在各频率处的相关系数分别为0.840、0.905、0.886、0.924;随着感音神经性听力损失的加重二者的差值明显缩小。随着频率的增加,两者的差值明显缩小。结论成人感音神经性聋ASSR反应阈与纯音听阈有显著相关性,随着听力损失的加重,ASSR反应阈愈接近纯音听阈,ASSR作为成人感音神经性聋听力定量诊断的客观方法有很大的临床应用价值。  相似文献

目的用多频稳态反应(auditory steady-state response,ASSR)与听性脑干反应(auditory brainstemre-sponse audiometry,ABR)测试聋儿残余听力,观察两者反应阈之间的相关性,对测试结果的准确性做出初步评价。方法对300例(600耳)感音神经性聋儿和30例(60耳)听力正常儿童(对照组)在睡眠状态下进行ASSR与ABR测试,得到不同阈值分布范围,并进行两者反应阈的相关性分析。结果30例(60耳)正常儿童ASSR各频率结果与ABR测试结果进行配对t检验,两者差异无统计学意义(P>0.05)。600例聋儿ASSR高频与ABR测试结果:Pearson统计高频相关系数为0.992,两者有显著相关性。结论ASSR和ABR两者高频反应阈之间有相关性,ASSR频率特性好,对聋儿听力检测及助听器验配具有临床价值。  相似文献

The main issue regarding pediatric audiology diagnosis is determining procedures to configure reliable results which can be used to predict frequency-specific hearing thresholds.AimTo investigate the correlation between auditory steady-state response (ASSR) with other tests in children with sensorineural hearing loss.MethodsProspective cross-sectional contemporary cohort study. Twenty-three children (ages 1 to 7; mean, 3 years old) were submitted to ASSR, behavioral audiometry, click audiometry brain stem response (ABR), tone burst ABR, and predicting hearing level from the acoustic reflex.Resultsthe correlation between behavioral thresholds and ASSR was (0.70- 0.93), for the ABR tone burst it was (0.73 -0.93), for the ABR click it was (0.83-0.89) only at 2k and 4 kHz. The match between the ASSR and the hearing threshold prediction rule was considered moderate.Conclusionthere was a significant correlation between the ASSR and audiometry, as well as between ABR click (2k and 4 kHz) and for the ABR tone burst. The acoustic reflex can be used to add information to diagnosis in children.  相似文献

OBJECTIVE: To determine the clinical usefulness of the dichotic multiple-frequency (MF) auditory steady-state response (ASSR) technique for estimating normal hearing compared to a 0.5-kHz tone burst and broadband click auditory brainstem response (ABR) protocol in a sample of adults. MATERIAL AND METHODS: A comparative experimental research design was selected in order to compare estimations of normal hearing obtained with the dichotic ASSR technique at 0.5, 1, 2 and 4 kHz with a 0.5-kHz tone burst and broadband click ABR protocol. The recording times required for each procedure were also compared. Normal-hearing subjects (n = 28) were selected according to immittance values within normal limits and pure-tone behavioural thresholds of < 25 dB HL across frequencies. RESULTS: The dichotic MF ASSR estimated normal hearing to be, on average, 30-34 dB HL across the range 0.5-4 kHz. The mean estimate of normal hearing for 0.5 kHz using tone burst ABRs was 30 dB nHL and the mean click ABR threshold was 16 dB nHL, i.e. 14-18 dB better than the ASSR thresholds. The dichotic MFASSR technique recorded 8 thresholds (4 in each ear) in a mean time of 23 min. The ABR protocol recorded 4 thresholds (2 in each ear) in a mean time of 25 min. CONCLUSION: Both the dichotic MF ASSR and ABR protocols provided a time-efficient estimation of normal hearing. There was no significant difference between the tone burst ABR and MF ASSR techniques in terms of estimation of normal hearing at 0.5 kHz. The dichotic MF ASSR technique proved more time-efficient by determining more thresholds in a shorter time compared to the ABR protocol.  相似文献

OBJECTIVE: To determine the effectiveness of auditory steady-state response (ASSR) as a measure of hearing sensitivity in young children suspect for significant hearing loss. DESIGN: Within-subject comparisons of click auditory brainstem response (ABR) thresholds and ASSR thresholds. SUBJECTS: The study population comprised 42 children suspect for hearing loss and subsequently referred for hearing assessment using electrophysiologic techniques. MAIN OUTCOME MEASURES: Electrophysiologic threshold responses for click ABR and ASSR stimuli (0.5, 1, 2, and 4 kHz) for right and left ears. RESULTS: Based on ABR and ASSR thresholds, 50% of the subjects demonstrated significant hearing loss in the severe to profound range. In some subjects, ASSRs were present at higher stimulus levels when click ABRs were absent. Significant correlations (P<.05) were found between high-frequency ASSR and click ABR thresholds for this study sample. For some subjects, ASSR findings suggested differences between ears that were not observable from the no-response click ABR results. CONCLUSIONS: Auditory steady-state response testing may provide additional information for children who demonstrate hearing levels in the severe to profound range. This information may be helpful when selecting the ear for cochlear implantation for a young hearing-impaired child. Multiple objective methods, such as ABR and ASSR testing, may be needed to determine accurate hearing sensitivity for young children being considered for sensory devices, and in particular, cochlear implants.  相似文献

The inclusion of the auditory steady-state response (ASSR) into test-batteries for objective audiometry has allowed for clinical comparisons with the most widely used procedure, the auditory brainstem response (ABR). The current study describes ASSR and ABR thresholds for a group of infants and young children with various types and degrees of hearing loss. A sample of 48 subjects (23 female) with a mean age of 2.8 ± 1.9 years SD were assessed with a comprehensive test-battery and classified according to type and degree of hearing loss. Thresholds were determined with a broadband click-evoked ABR and single frequency ASSR evoked with continuous tones (0.25–4 kHz) amplitude modulated (67–95 Hz). Mean difference scores (±SD) between the ABR and high frequency ASSR thresholds were 9.8 (±11), 3.6 (±12) and 10.5 (±12) dB at 1, 2 and 4 kHz, respectively. An ASSR mean threshold for 2–4 and 1–4 kHz compared to the ABR threshold revealed an average difference of 7 (±9) and 7.9 (±8) dB, respectively. The overall correlation between the ABR and ASSR thresholds was highest for the mean ASSR thresholds of 2–4 and 1–4 kHz (r = 0.92 for both conditions). Correlations between the ABR and individual ASSR frequencies were slightly less (0.82–0.86). The average of the 2–4 kHz ASSR thresholds correlated best with the click-evoked ABR for all categories of hearing loss except for the sensorineural hearing loss category for which the 1–4 kHz ASSR average was better correlated to ABR thresholds. Findings demonstrate the reliability of verifying high frequency ASSR thresholds with a click-evoked ABR as an important cross-check in infants for whom behavioural audiometry may not be possible.  相似文献

The accuracy of dichotic multiple frequency auditory steady state in predicting pure-tone thresholds at 0.5, 1, 2, and 4.0 kHz compared to an ABR protocol (click and tone burst at 0.5 kHz) were explored in a group of 25 hearing-impaired subjects across the degree and configuration spectrum. Mean steady state thresholds were within 14, 18, 15, and 14 dB of the pure tones at 0.5, 1, 2, and 4 kHz, compared to the tone-burst ABR at 0.5 kHz pure-tone difference of 24 dB, and a click-evoked pure-tone (2-4 kHz) difference of 9 dB. Recording time for the steady state protocol was 28 minutes (+/- 11) compared to 24 minutes (+/- 9) of the ABR protocol. Degree of loss had a significant effect on steady state; configuration of hearing loss had a limited effect. Mf ASSR predicted thresholds with relative accuracy although some configurations showed discrepancies for low-frequency estimates.  相似文献

目的 建立听力正常婴儿短音听性脑干反应(tone-pip ABR)和听觉稳态反应(auditorysteady state response,ASSR)反应阈的正常参考值,研究其听觉发育的生物学规律,并比较两种听力检测技术的频率特性.方法 选取0~6月龄听力正常婴儿80例(160耳),按月龄分为四组:新生儿组、42 d组、3月龄组和6月龄组,每组20例(40耳),男女例数均等,分别记录其短声ABR的潜伏期及在0.25、0.5、1、2、4、8 kHz频率范围内tone-pip ABR和ASSR的反应阈.结果 在70 dB正常听力级短声刺激下,短声ABRⅠ、Ⅲ、Ⅴ波潜伏期、Ⅰ~Ⅲ、Ⅲ~Ⅴ、Ⅰ~Ⅴ波间期随月龄增加逐渐缩短,波Ⅰ于42 d前、波Ⅲ于3个月前发育变化显著.tone-pip ABR波形与短声ABR相似,Ⅰ、Ⅲ、Ⅴ波潜伏期随频率增加逐渐缩短,波形分化逐渐清晰.不同频率、不同月龄tone-pip ABR和ASSR反应阈差异具有统计学意义(P值均<0.05).除0.25 kHz外,其余频率tone-pip ABR反应阈均低于ASSR.不同月龄tone-pip ABR和ASSR听力曲线形状相似.结论 0~6月正常婴儿tone-pip ABR的潜伏期和波间期随月龄增加逐渐缩短,而反应阈无明显变化.tone-pip ABR和ASSR均有稳定的频率特异性,tone-pip ABR反应阈低于ASSR,可能更接近主观纯音听阈.  相似文献

OBJECTIVE: To investigate the clinical usefulness of the dichotic single-frequency auditory steady-state response (ASSR) for estimation of behavioral thresholds in children with severe to profound congenital sensorineural hearing loss. DESIGN: A comparative experimental research design was selected to compare behavioral and ASSR thresholds for the sample. Behavioral pure-tone audiometry served as the criterion standard. SETTING: Hearing Clinic, Department of Communication Pathology, University of Pretoria, Pretoria, South Africa. PATIENTS: A referred sample of 10 patients (20 ears), 5 girls and 5 boys aged 10 to 15 years (mean age, 13 years 4 months), with severe to profound sensorineural hearing impairment. MAIN OUTCOME MEASURES: The difference, and correlation, between 160 pure-tone behavioral and ASSR thresholds at 0.5, 1, 2, and 4 kHz. RESULTS: Mean differences between ASSR and behavioral thresholds were 6 dB for 0.5 kHz and 4 dB for 1, 2, and 4 kHz, with standard deviations varying between 8 and 12 dB. No significant differences (P<.05) were observed between ASSR and behavioral thresholds, except at 0.5 kHz, and Pearson correlation coefficients varied between 0.58 and 0.74 across the evaluated frequencies, with best correlation at 1 kHz and worst at 0.5 kHz. CONCLUSIONS: The ASSR thresholds provided reliable estimations of behavioral thresholds for children with severe to profound hearing loss and indicated an increased sensitivity for more profound hearing loss.  相似文献

目的探讨同步多频听觉稳态诱发反应(auditory steady-state responses to multiple simultaneous stimuli,MSS-ASSR)与听性脑干诱发电位(auditory brainstem response audiometry,ABR)的关系。方法受试者为不同程度听力损失的儿童70名(140耳),年龄范围为3月龄~6岁。测试状态为水合氯醛镇静睡眠。仪器采用美国INTELLEGENT HEARING的诱发电位仪,MSS-ASSR及ABR测试的软件分别为SMART ASSR和SMART EP,刺激声信号的载波频率为0.5kHz、1kHz、2kHz及4kHz。左耳上述各个载波频率的调制频率分别为77Hz、85Hz、93Hz、101Hz,右耳为79Hz、87Hz、95Hz、103Hz。测试时,双耳八个调幅调制声信号经ER-3A标准插入式耳机同时给出。ABR的刺激声为短声,极性为交替波,刺激速率为19.3次/分,耳机也是ER-3A插入式耳机。受试儿先进行ABR测试、然后进行MSS-ASSR的测试,分别以降10升5法得出二者的反应阈。将MSS-ASSR 0.5kHz、1kHz、2kHz和4kHz的反应阈与ABR反应阈比较,以SPSS 10.0软件进行相关性分析。结果除去58耳ABR无反应外,剩余82耳ABR反应阈与MSS-ASSR 0.5kHz、1kHz、2kHz、4kHz的反应阈相关性具有显著性意义(P=0.000)。Pearson相关系数分别为0.757,0.854,0.906,0.912。结论MSS-ASSR作为儿童听力定量诊断的客观方法有很大的临床应用价值。  相似文献

The auditory steady-state response (ASSR) has gained popularity as an alternative technique for objective audiometry but its use in less severe degrees of hearing loss has been questioned. The aim of this study was to investigate the usefulness of the ASSR in estimating moderate degrees of hearing loss. Seven subjects (12 ears) with moderate sensorineural hearing loss between 15 and 18 years of age were enrolled in the study. Forty-eight behavioural and ASSR thresholds were obtained across the frequencies of 0.5, 1, 2, and 4 kHz. ASSR thresholds were determined using a dichotic multiple frequency recording technique. Mean threshold differences varied between 2 and 8 dB (±7–10 dB SD) across frequencies. The highest difference and variability was recorded at 0.5 kHz. The frequencies 1–4 kHz also revealed significantly better correlations (0.74–0.88) compared to 0.5 kHz (0.31). Comparing correlation coefficients for behavioural thresholds less than 60 and 60 dB and higher revealed a significant difference. Eighty-six percent of ASSR thresholds corresponded within 5 dB of moderate to severe behavioural thresholds compared to only 29% for mild to moderate thresholds in this study. The results confirm that the ASSR can reliably estimate behavioural thresholds of 60 dB and higher, but due to increased variability, caution is recommended when estimating behavioural thresholds of less than 60 dB, especially at 0.5 kHz.  相似文献
