Treatment of comminuted distal radius fractures with pins and plaster

Based on this review, it is evident that the anatomic results obtained by the use of pins and plaster for the treatment of comminuted distal radial fractures is insufficient to yield a high percentage of satisfactory functional end results. In addition, the high complication and reoperation rate noted in our series makes us question whether the technique of pins and plaster should remain a treatment option for these difficult fractures. Perhaps a combination of internal fixation with bone grafting combined with external fixation will provide a solution to this fracture, particularly in young, active patients.     

外固定支架治疗桡骨远端粉碎性骨折

2003年3月～2009年10月,我科采用上肢多功能外固定器治疗桡骨远端粉碎性不稳定骨折38例,疗效满意. 1 材料与方法 1.1 病例资料本组38例,男17例,女21例,年龄20～65(37.3±10.2)岁.均为粉碎性骨折,其中Colles骨折10例,Smith骨折8例,Barton骨折20例(见图1A).1例为开放性伴肌腱、血管、神经损伤骨折,余为新鲜闭合骨折.有8例骨折门诊手法复位后石膏托固定,复查X线示骨折再移位而改用外固定器固定手术.     

有限手术结合单臂外固定支架治疗桡骨远端粉碎性骨折

有限内固定结合石膏外固定治疗桡骨远端粉碎性骨折

1996年8月—2000年12月，我院采用有限内固定结合石膏外固定治疗桡骨远端粉碎性骨折36例，经随访观察，效果满意，现总结报告如下。     

股骨远端粉碎性骨折的内固定治疗

目的探讨不同内固定器材治疗股骨远端粉碎性骨折的临床疗效。方法应用L形钢板、动力加压髁钢板、股骨远端解剖钢板和逆行股骨髁上交锁髓内钉等4种材料治疗股骨远端粉碎性骨折。结果32例随访12~34个月,无感染病例,1例发生断钉致骨折延迟愈合。结论4种内固定都能起到较好治疗效果,手术规范操作是关键。     

单侧多功能组合式外固定器治疗桡骨远端不稳定骨折39例

硬纸板夹板外固定治疗桡骨远端移位骨折

桡骨远端骨折较为常见,老年人尤为多见,女性明最多于男性,约占全身骨折的6．7％～11％^[1]。国内骨伤科界治疗桡骨远端骨折以闭合手法复位外固定为主^[2],外固定方法较多。硬纸板夹板外固定治疗桡骨远端移位骨折,易于取材,剪裁方便,经济实用,临床获得了满意疗效。1989年至2009年,采用硬纸板夹板外阎定法,治疗中老年桡骨远端移位骨折,病例资料完整的672例,进行总结分析如下。     

有限内固定结合外固定架治疗桡骨远端不稳定骨折

目的探讨涉及关节面的桡骨远端关节内粉碎性骨折的治疗方法。方法自2003年3月-2005年11月对16例桡骨远端关节内粉碎性骨折采用外固定架结合有限内固定,必要时行有限切开复位内固定治疗。结果疗效按Dienst功能评估标准:优11例,良3例,可2例,优良率87.5%。结论涉及关节面的桡骨远端粉碎性骨折在手法复位不良佳时,应积极手术;采用在手法整复基本恢复桡骨远端长度及各种移位的基础上,外固定架结合有限内固定,必要时有限切开直视下撬拨复位内固定。     

掌心石膏模握式外固定治疗近节指骨骨折

指骨骨折是手部常见骨折,2004年3月至2006年6月,作者采用掌心石膏模型垫加绷带外固定治疗近节指骨骨折68例,获得满意的效果,现总结如下。

经皮撬拨复位结合外固定支架治疗跟骨粉碎性骨折

目的 探讨经皮撬拨复位结合外固定支架治疗粉碎性跟骨骨折的疗效.方法 21例跟骨粉碎性骨折采用经皮撬拨复位联合外固定支架治疗,测量并比较术前术后B(o)hler角,根据美国足踝骨科协会(AOFAS)踝-后足评分系统进行评分.结果 21例均获随访,时间5～42(23.2±2.7)个月.跟骨在侧、轴位上的形态较术前明显恢复:术前B(o)hler角-21～40(11.2±3.7)°,术后拆除外固定后B(o)hler角18～40(32.4±6.8)°,术前术后比较差异有统计学意义(P＜0.05).踝-后足功能进行评分为67～95分,优8足,良10足,可2足,差1足,优良率为85.7%.结论 经皮撬拨结合外固定支架固定治疗粉碎性跟骨骨折具有损伤小、并发症少等优点,疗效确切.     

外固定架结合微型钢板治疗难复性桡骨远端骨折

2003年6月～2007年9月,我院应用外固定架结合微型钢板治疗难复性桡骨远端骨折48例,疗效良好。 1材料与方法1．1病例资料本组48例,男15例,女33例,年龄22～73（40．5±10．5）岁。左侧20例,右侧28例。均为新鲜闭合性骨折。     

External fixation of distal radial fractures: results and complications

External fixation of unstable fractures of the distal radius yields satisfactory results but has a high complication rate. We studied thirty-five fractures in thirty-four patients to determine whether the results obtained with external fixation warranted it use. At a mean follow-up period of 31 months, the results of treatment were assessed by interviews and clinical and radiographic examination of both wrists. Twelve fractures had an excellent result, twelve had a good result, ten had a fair result, and one had a poor result. Radiographic results were graded excellent in ten fractures, good in thirteen, fair in five, and poor in seven. No correlation was found between the anatomical results and the clinical results or the patients' subjective ratings. Complications that were related directly to the fixation pins occurred in fourteen of the fractures. There were forty-five additional complications. The frequency of complications and the limitations of external fixation demand caution on the part of the surgeon to prevent iatrogenic morbidity, which would limit the benefits of the technique.     

超关节外固定架治疗桡骨远端粉碎性骨折

我院自2000年以来采用超关节单臂外固定架治疗桡骨远端粉碎性骨折36例，术后骨折对位对线佳，愈合快，腕关节功能恢复好，取得了良好的疗效。
