While percutaneous transluminal coronary angioplasty (PTCA) as a primary modality for treating acute myocardial infarction (MI) has been shown to have important advantages over thrombolysis, a survival benefit has not been demonstrated because of the small size of the individual trials. To increase the statistical power to detect a survival benefit, we performed a meta-analysis of trials of PTCA and thrombolysis. We pooled the data for all randomized, controlled trials; randomized, controlled trials stratified according to thrombolytic agent [streptokinase vs. tissue plasminogen activator (TPA)]; and all trials. Pooling was performed by calculating the Mantel-Haenszel odds ratio with the Robins, Greenland, and Breslow estimate of variance. Calculation of the Q statistic was performed to assess heterogeneity. For all four analyses, the odds ratio indicated a significant survival advantage of PTCA over thrombolysis: all randomized controlled trials [0.57,95% confidence index (CI): 0.48,0.68)]; streptokinase trials [0.61,95% CI: 0.43,0.87); TPA trials (0.52,95% CI: 0.36,0.76); all trials (0.51,95% CI: 0.43,0.61). The Q statistic was not significant for any of the analyses. The results of our meta-analysis support the hypothesis that PTCA is associated with a significant reduction in mortality compared with thrombolysis.  相似文献

OBJECTIVES: This study compares the efficacy of primary angioplasty and systemic thrombolysis with t-PA in reducing the in-hospital mortality of patients with anterior AMI. BACKGROUND: Controversy still exists about the relative benefit of primary angioplasty over thrombolysis as treatment for AMI. METHODS: Two-hundred and twenty patients with anterior AMI were randomly assigned in our institution to primary angioplasty (109 patients) or systemic thrombolysis with accelerated t-PA (111 patients) within the first five hours from the onset of symptoms. RESULTS: Baseline characteristics were similar in both groups. Primary angioplasty was independently associated with a lower in-hospital mortality (2.8% vs. 10.8%, p = 0.02, adjusted odds ratio 0.23, 95% confidence interval 0.06 to 0.85). During hospitalization, patients treated by angioplasty had a lower frequency of postinfarction angina or positive stress test (11.9% vs. 25.2%, p = 0.01) and less frequently underwent percutaneous or surgical revascularization after the initial treatment (22.0% vs. 47.7%, p < 0.001) than did patients treated by t-PA. At six month follow-up, patients treated by angioplasty had a lower cumulative rate of death (4.6% vs. 11.7%, p = 0.05) and revascularization (31.2% vs. 55.9%, p < 0.001) than those treated by t-PA. CONCLUSIONS: In centers with an experienced and readily available interventional team, primary angioplasty is superior to t-PA for the treatment of anterior AMI.  相似文献

Coronary angioplasty is being increasingly used as the primary treatment for patients with acute myocardial infarction, but controversy remains over its potential adoption in preference to thrombolysis as standard care. This report summarizes the published evidence on health outcomes after primary angioplasty compared with thrombolysis or no intervention for patients with acute myocardial infarction. The usefulness of percutaneous transluminal coronary angioplasty (PTCA) in patients with evolving myocardial infarction remains controversial. We retrospectively assessed the efficacy of PTCA on myocardial salvage in acute myocardial infarction in comparison with the efficacy of intracoronary thrombolysis (ICT). Sixty-two patients with initial anteroseptal myocardial infarction who had been treated within 6 hrs after the onset of chest pain were categorized into 4 groups: 1) spontaneous recanalization: n = 14, 2) successful PTCA: n = 25 (this group was further subdivided into 2 groups: direct PTCA group, primary PTCA without prior ICT: n = 19; and rescue PTCA group, PTCA after unsuccessful ICT: n = 6), 3) successful ICT group (n = 12), and 4) unsuccessful recanalization group (n = 11). Left ventricular function in the chronic phase was assessed by contrast ventriculography using the global ejection fraction (EF) and regional wall motion (RWM) was assessed by the centerline method. Patients with recanalization had a significantly higher EF than did those without (62 +/- 12 vs 50 +/- 13%, p < 0.01). The mean EFs for groups with successful reperfusion were as follows: 65 +/- 8% for the spontaneous recanalization group, 61 +/- 14% for PTCA group (64 +/- 13% for direct PTCA group, 51 +/- 13% for rescue PTCA group) and 60 +/- 12% for the ICT group. The EFs for the spontaneous recanalization group and the direct PTCA group were significantly greater than that for the rescue PTCA group. The time to reperfusion and the thrombolysis in myocardial infarction (TIMI) flow grade before reperfusion did not affect the preservation of global left ventricular function. RWM of the infarcted area in patients with recanalization were less hypokinetic than that in patients without (p < 0.01). The mean RWM (SD/chord) in the successfully reperfused groups were -2.3 +/- 1.2 for the spontaneous recanalization group, -2.6 +/- 1.2 for the PTCA group (-2.3 +/- 1.1 for the direct PTCA group, -3.3 +/- 1.0 for rescue PTCA group) and -3.0 +/- 0.5 for the ICT group. Hypokinesis of the infarcted area was more severe in the rescue PTCA group than in the spontaneous recanalization group and the direct PTCA group (multiple comparison test p < 0.01, respectively), and hypokinesis was more severe in the ICT group than in the direct PTCA group (Student's t-test, p < 0.05).(ABSTRACT TRUNCATED AT 400 WORDS)  相似文献

OBJECTIVES: The Air Primary Angioplasty in Myocardial Infarction (PAMI) study was designed to determine the best reperfusion strategy for patients with high-risk acute myocardial infarction (AMI) at hospitals without percutaneous transluminal coronary angioplasty (PTCA) capability. BACKGROUND: Previous studies have suggested that high-risk patients have better outcomes with primary PTCA than with thrombolytic therapy. It is unknown whether this advantage would be lost if the patient had to be transferred for PTCA, and reperfusion was delayed. METHODS: Patients with high-risk AMI (age >70 years, anterior MI, Killip class II/III, heart rate >100 beats/min or systolic BP <100 mm Hg) who were eligible for thrombolytic therapy were randomized to either transfer for primary PTCA or on-site thrombolysis. RESULTS: One hundred thirty-eight patients were randomized before the study ended (71 to transfer for PTCA and 67 to thrombolysis). The time from arrival to treatment was delayed in the transfer group (155 vs. 51 min, p < 0.0001), largely due to the initiation of transfer (43 min) and transport time (26 min). Patients randomized to transfer had a reduced hospital stay (6.1 +/- 4.3 vs. 7.5 +/- 4.3 days, p = 0.015) and less ischemia (12.7% vs. 31.8%, p = 0.007). At 30 days, a 38% reduction in major adverse cardiac events was observed for the transfer group; however, because of the inability to recruit the necessary sample size, this did not achieve statistical significance (8.4% vs. 13.6%, p = 0.331). CONCLUSIONS: Patients with high-risk AMI at hospitals without a catheterization laboratory may have an improved outcome when transferred for primary PTCA versus on-site thrombolysis; however, this will require further study. The marked delay in the transfer process suggests a role for triaging patients directly to specialized heart-attack centers.  相似文献

Primary angioplasty is increasingly being advocated as the preferred approach for treating acute ST-segment elevation myocardial infarction regardless of whether interinstitutional transfer is required. This review critically analyzes the evidence comparing primary angioplasty with thrombolytic therapy and concludes that reasonable health care professionals may still find considerable uncertainty about the superiority of primary angioplasty for all situations. The magnitude of benefit for primary angioplasty over thrombolysis is probably less than 1 to 2 lives saved/100 patients treated and largely depends on the choice of thrombolytic agent, time to treatment, place of treatment, and adjunctive therapy. There is little evidence that systematically transferring patients for primary angioplasty in routine practice will provide any health benefits over thrombolysis. Consequently, it may be most useful to view these treatments as complementary rather than competitive. Thrombolysis remains a clinically and economically attractive option for the treatment of acute myocardial infarction that does not require the radical restructuring of our health care systems.  相似文献

Emergency percutaneous transluminal coronary angioplasty (PTCA) is accepted as an important reperfusion intervention for acute myocardial infarction (AMI). Although its primary success rate is well documented, the frequency of restenosis after this procedure is unclear. The frequency of restenosis was determined in patients undergoing emergency PTCA at least 6 months after PTCA was performed during AMI. Of 66 consecutive patients undergoing emergency PTCA, 25 had a second, elective catheterization at an average of 22 months after AMI and 6 underwent repeat catheterization because of recurrent chest pain. Restenosis of the PTCA site was found in 10 of the 31 patients (32%) restudied. Also, 14 (45%) of these 31 patients showed progression of narrowing in the non-infarct-related coronary arteries. In summary, patients in whom AMI is treated by PTCA are at risk for restenosis and for progressive narrowing of the non-infarct artery.  相似文献

A total of 533 patients with acute myocardial infarction of less than 4-h duration were enrolled in the multicenter randomized trial of intracoronary thrombolysis compared to conventional treatment. In two of the five participating centers, an additional coronary angioplasty immediately after thrombolysis was attempted in 46 patients. According to the treatment allocation and early and late patency of the infarct related vessel, patients were subdivided into three groups: conventionally treated (group A); successful coronary angioplasty following thrombolysis with persistent patent infarct related vessel (group B); and late patency of the infarct related vessel postthrombolytic therapy without angioplasty (group C). The highest global ejection fractions were observed in group B (54% +/- 10%) and group C (55% +/- 13%), while the lowest ejection fraction was found in group A (47% +/- 14%). The sequential changes in global ejection fraction from the acute to the chronic stage was + 4% (p = 0.05) in group B, while no significant changes could be demonstrated in group C. Furthermore, in the group successfully treated by angioplasty, the improvement in global ejection fraction was more pronounced and persisted up to three months after the intervention. This was supported by analysis of regional myocardial function of the infarct zone (+ 16% improvement, p = 0.01). The long-term clinical follow-up (median 24 months) of the patients successfully treated by combined procedure of thrombolysis and angioplasty (group B) was most favourable with a lower incidence of re-infarction (6%), and late coronary bypass surgery (13%) and/or (re)-percutaneous transluminal coronary angioplasty (3%) was performed less frequently. These results suggest that reperfusion may need to be supplemented by additional revascularization procedures in order to optimize the changes of obtaining full functional recovery and so to improve the prognosis.  相似文献

Coronary angioplasty has been applied in patients with recent myocardial infarction, but results of angioplasty of multiple vessels early after myocardial infarction in patients with severe multivessel disease have not been reported. Coronary angioplasty of multiple vessels was performed in 105 patients 0 to 15 days (mean 5 +/- 4) after recent myocardial infarction. There were 77 men (73%) and 28 women (27%), with a mean age of 57 years. All patients had severe multivessel disease, 68% with two vessel and 32% with three vessel disease. Twenty-eight patients (27%) had successful thrombolysis before angioplasty and 70 (67%) had postinfarction angina. Mean left ventricular ejection fraction was 58 +/- 10% and was less than 45% in 13 patients (12%). Angioplasty was attempted in 319 lesions (mean 3 lesions per patient, range 2 to 9) and 252 vessels (mean 2.4 vessels per patient, range 2 to 4), with success in 302 lesions (95%) and 237 vessels (94%); angioplasty was done in two stages in 59 patients (56%). Clinical success was achieved in 102 patients (97%). Complications included myocardial infarction in six patients (5.7%) (one Q wave, five non-Q wave), urgent bypass surgery in two (1.9%) and death in one (0.9%); overall, seven patients (7%) had a major complication. All patients had a follow-up duration greater than 1 year (mean 31 months, range 12 to 73). Clinical recurrence developed in 24 patients (23%), of whom 21 had repeat angioplasty, 1 had bypass surgery and 2 were managed medically. The efficacy of combined thrombolysis and angioplasty for the purpose of coronary reperfusion after acute myocardial infarction has been controversial. The present study was conducted, therefore, to evaluate the effects of angioplasty following administration of conventional thrombolytic agents on the long-term prognosis of acute myocardial infarction patients. A total of 409 patients admitted to the hospital within 12 hours of the onset of infarction between January 1990 and May 2001 were studied retrospectively. These included 151 patients treated with thrombolysis alone (group T), 73 patients treated with angioplasty alone (group A), and 35 patients treated with angioplasty after thrombolysis (group T&A). Group T&A had shorter intervals from onset to initial treatment than group A (3.0 hours vs 6.3 hours, p < 0.01), a higher reperfusion success rate than group T (91.4% vs 74.8%, p < 0.01), and more improved left ventricular wall motion than group A. One-year cardiac mortality rates tended to be higher in group T, which had a higher rate of unsuccessful reperfusion than groups T&A or A (8.1% vs 3.4% vs 3.5%). The frequencies of hemorrhagic complications were similar among the 3 groups. From these findings, we conclude that thrombolytic therapy with subsequent angioplasty is an effective strategy for achieving cardiac reperfusion following acute myocardial infarction.  相似文献

Transluminal coronary angioplasty (TCA) has become the treatment of choice of residual stenosis after thrombolysis for myocardial infarction, but the long-term results of TCA are imperfectly evaluated. Seventy patients underwent TCA after thrombolysis on account of a significant (greater than 50 p. 100) residual stenosis of the artery responsible for the infarction. TCA was performed less than 6 hours after the onset of symptoms in 15 patients who had neither clinical nor electrocardiographic evidence of reperfusion; 4 of these patients were in a state of cardiogenic shock. In the remaining patients TCA was performed 1 to 10 days (mean 3.2 days) after thrombolysis. A primary success was obtained in 64 patients (91 p. 100). Two patients had emergency aorto-coronary bypass. During their stay in hospital, 5 patients presented with symptoms of reocclusion which in 4 of them occurred less than 24 hours after TAC, and 2 of these 4 patients had to be reoperated upon; 2 patients died suddenly. During a 6 to 18 months' follow-up period (mean 10.5 months), the infarction recurred in 3 patients; the recurrence took place during the 3rd month in 2 of them (1 had another thrombolysis and later TAC) and during the 6th month in the third one. At 6 months, 4 patients were suffering from exertion angina and 2 asymptomatic patients had a positive exercise test. Fifty-two control coronary arteriographies were performed at 6 months. Autopsy findings are reported for 6 patients who died early (8, 9, 12, 13 and 14 days) or late (52 days) after combined thrombolysis and percutaneous transluminal coronary angioplasty (PTCA) for acute myocardial infarction. Morphologic changes in the coronary arteries at the site of revascularization included injury to the inner portion of the arterial wall (intimal splitting, subintimal dissection, medial tears and submedial dissection) and necrosis of medial smooth muscle cells. Residual mural thrombi and thrombotic reocclusion were noted within the arterial lumen. There was a beginning neointima formation in all patients who died early and a reobstructing neointima proliferation in the patient who died late after PTCA. The results of this study support the suggestion that both rupture and dissection of the inner arterial wall and necrosis of the tunica media resulting from irreversible dilatation of the grossly intact outer layers are the most important mechanisms of PTCA. Response to arterial wall injury after PTCA is a neointima formation leading to covering of mural thrombi and thrombogenic intimal, medial and adventitial substances and smoothing of the luminal surface. Large residual mural thrombi and excessive neointimal proliferation may cause restenosis within a few weeks.  相似文献
