目的:探讨平阳霉素碘油乳剂(PLE)经肝动脉灌注对兔正常肝脏组织的影响。方法:14只4~5个月龄日本大耳白兔,体重(2.5±0.2)kg,按注入生理盐水或PLE的量分为假手术组、实验A组(低剂量组)和实验B组(高剂量组)。各组兔均开腹穿刺肝动脉,按分组剂量注入PLE。术后1,2,4,6周取病理切片,HE染色,光镜下观察肝脏组织学改变;免疫组织化学(免疫组化)染色标记血小板衍化生长因子B链(PDGF-B),并行图象分析。结果:A组HE染色肝细胞呈一过性水样变,变性在2周时最重,至6周已明显减轻。B组HE染色2周时肝细胞明显水样变,4周时可见汇管区纤维组织增生,6周时部分肝组织出现明显假小叶结构。免疫组化染色显示,PDGF-B在肝细胞胞膜及纤维间隔中有明显表达。结论:经肝动脉灌注PLE可导致正常肝脏组织产生不同程度的肝纤维化;PDGF-B参与了肝纤维化的病理过程。  相似文献

目的:探讨肺泡巨噬细胞Toll样受体2(TLR2)的激活机制及其在肝脏缺血再灌注(HIR)中肺损伤的意义。方法:用野生型小鼠C3h/Heouj和TLR4缺失小鼠C3h/Hej建立HIR动物模型。于再灌注1,6,12h后经支气管肺泡灌洗液获取肺泡巨噬细胞,采用荧光定量PCR方法检测TLR2/4mRNA的表达。同时检测支气管肺泡灌洗液中内毒素及肿瘤坏死因子(TNF)的水平,肺组织湿干重比值,肺组织髓过氧化物酶的浓度,并进行肺组织学评分。结果:C3h/Heouj组HIR缺血再灌后各时点肺泡巨噬细胞TLR2/4mRNA表达升高,TLR2mRNA表达持续升高,TLR4mRNA6h达到最高值。同时C3h/Heouj组HIR后支气管肺泡灌洗液中TNF水平明显升高,肺损伤加重,肺组织湿干重比值持续升高,肺组织髓过氧化物酶持续增加(P<0.05)。C3h/Hej组HIR后TLR2mRNA表达仅轻度升高,且支气管肺泡灌洗液中TNF水平低于C3h/Heouj组(P<0.05),肺损伤轻于C3h/Heouj组(P<0.05)。结论:HIR可致肺泡巨噬细胞表面TLR4的激活,可上调TLR2的表达,从而可加重HIR时的肺损伤。  相似文献

摘要:目的 探讨原位肝移植中动脉异位重建的方法及效果。 方法 回顾性分析我院10年来的440例肝移植中36例因受体血管异常而行异位重建的方法及术后处理措施等。 结果 36例中行供肝动脉与受体肾下腹主动脉吻合20例,与肾上腹主动脉吻合10例,与胃左动脉吻合4例,与脾动脉吻合2例。5例围手术期死亡,但吻合口通畅,31例存活3个月至4年无血管相关并发症,仅1例术后2个月因胆道缺血坏死行再次肝移植。 结论 肝移植时受体肝动脉有病变或异常改变时,应将受体肾下或肾上腹主动脉、脾动脉、胃左动脉与供肝动脉进行异位重建,可取得满意效果。  相似文献

目的: 探讨经顺铂(DDP)处理胆囊癌细胞后survivin表达及其与肿瘤细胞耐药之间的关系。 方法:采用MTT比色法测定胆囊癌细胞对4种化疗药物的敏感性。RT-PCR检测survivin mRNA的表达。Western blot检测survivin蛋白表达的变化。结果:GBC-SD细胞对化疗药物的敏感性从高到低依次为DDP>ADM>5-FU>MMC。化学药物处理后的第1天,3组胆囊癌细胞的survivin mRNA表达水平均降低;其中0.5μg/mL DDP+GBC-SD组下降了10%,3μg /mL DDP+GBC-SD组下降36%,6μg /mL DDP+GBC-SD组下降了28%。第3天,0.5μg/mL DDP组和3μg/mL DDP组GBC-SD细胞的survivin mRNA表达与第1天比较,分别上升22%和64%,但6μg/mL DDP组仍持续降低,仅为第1天的66%。0.5μg/mL DDP组和3μg/mL DDP组作用3d后的GBC-SD细胞中survivin蛋白含量分别升高了15%和12%,而6μg/mL DDP组则下降了80%。 结论:低浓度的DDP即能诱导胆囊癌细胞内survivin的表达增加,这可能是胆囊癌细胞对化疗药物产生耐药性的因素之一。  相似文献

摘要:为探讨左侧结肠癌并急性肠梗阻理想的处理原则和方法,回顾分析58例左侧结肠癌并发急性肠梗阻行一期切除吻合术患者的临床资料。本组均成功手术,无手术死亡,术后除7例有切口不同程度液化感染外,无吻合口漏、腹腔感染等并发症,均痊愈出院。提示:对能耐受手术切除的左侧结肠癌并发梗阻,在必要的围手术期处理前提下,一期切除吻合是可行的。避免了横结肠造口、二期手术、癌肿扩散及并发症的发生。  相似文献

笔者采用明胶海绵或不锈钢圈栓塞双侧髂内动脉治疗骨盆骨折合并盆腔血肿7例,其中6例患者术前处于休克前期或休克期,血压低,出血明显,栓塞成功后,出血停止,血压回升,栓塞后2d血压恢复正常基础水平。提示:双侧髂内动脉栓塞对治疗骨盆骨折合并盆腔血肿效果明显,是一种有效的治疗手段。  相似文献

目的:探讨经外膜缓释雷公藤内酯醇(triptolide)对自体移植静脉内膜增生的抑制作用。方法:健康雄性新西兰大白兔24只,建立颈外静脉-颈总动脉移植模型。随机将动物等分为3组。空白组移植血管不给任何处理, F-127多聚凝胶对照组在移植血管外膜喷洒20 %F-127多聚凝胶0.5 mL,实验组在移植血管外膜喷洒携带雷公藤内酯醇300μg的F-127多聚凝胶0.5 mL。术后2周取标本。用组织形态学方法检测血管内膜增生程度,免疫组化检测标本中bcl-2和Fas的表达,TUNEL法检测标本中血管平滑肌细胞(VSMC)凋亡的水平。结果:静脉移植2周后,与空白组和F-127对照组比较,实验组血管内膜增生明显受抑制(P<0.05),bcl-2的表达[(18.2±8.4) %]显著减少,而Fas的表达[(21.4±8.9) %]显著增加,凋亡细胞[(28.4±7.6) %]也显著增加(P<0.05)。结论:经外膜缓释雷公藤内酯醇可有效抑制移植静脉内膜增生,这一作用可能系通过促进VSMC凋亡而实现的。  相似文献

回顾性分析股动脉假性动脉瘤18例的临床资料。1例因介入穿刺引起的股动脉假性动脉瘤行局部压迫治疗,15例行假性动脉瘤切除术,2例行股动脉结扎术。结果示1例股动脉结扎术患者术后出现肢体坏死,行膝上截肢后康复出院,另17例痊愈出院。提示对股动脉假性动脉瘤行动脉瘤切除、股动脉端端吻合可作为首选的手术方式。  相似文献

目的:探索一种治疗彻底、创伤少、瘢癍痕小、费用低的乳腺纤维瘤治疗方法。方法:回顾近3年来465例采用手法张力美容切口治疗乳腺纤维瘤患者的临床资料。结果:465例手术均最大限度争取行乳晕或腋窝皱褶或乳腺下方皱褶切口。切口均甲级愈合,无明显瘢痕,双乳对称,外形功能无影响,站立时切口不明显。结论:手法张力美容切口治疗乳腺纤维瘤是一种适合大部分乳腺纤维瘤患者的手术方法,具有治疗彻底、创伤少、瘢痕小、费用低。  相似文献

目的:分析导致再次胆道手术的原因,以期减少胆道再手术率。方法:总结1990—1999年间收治的再次胆道手术患者828例的临床资料,对胆道疾病再次手术的原因进行归类分析。结果:再手术的主要原因是结石复发或残留,占65.10%;结石合并Oddi括约肌狭窄占33.82%;单纯Oddi括约肌狭窄占9.54%;胆管损伤性狭窄和胆肠吻合口狭窄占10.39%;胆道系统肿瘤占6.52%。结论:胆道再手术的主要原因仍以结石复发或残留为主,其次为Oddi括约肌狭窄;损伤性胆管狭窄等与手术有关的因素不容忽视。减少胆道再次手术的关键在于初次手术的彻底性和手术方法的合理性。  相似文献

目的 观察急性坏死性胰腺炎(ANP)时肠粘膜屏障的改变和肠源性细菌和内毒素移位。方法 将Wistar大鼠随机分为正常对照组(n=6)、假手术组(n=30)和ANP组(n=39)。采用人工胆汁胰管逆行灌注法制作ANP模型。观察胰腺、小肠病理改变和小肠粘膜上皮细胞间紧密连接(冷冻蚀刻电镜)变化。动态测定血浆D-乳酸、内毒素水平,以及腹腔脏器细菌移位率。结果 ANP后小肠粘膜损伤,皮皮细胞间紧密连接破坏甚至消失,血浆D-乳酸水平上升,发病早期即出现内毒素血症;ANP发生后72h脏器细菌移位率达到59.5%。结论 ANP早期肠粘膜屏障功能受损。导致肠道细菌和内毒素移位,成为全身炎症反应和胰腺继发感染的根源。  相似文献

Candida albicans translocation across the gut mucosa following burn injury   总被引:3,自引:0,他引:3  
Normal guinea pigs were challenged intragastrically with Candida albicans 1 hr prior to a 30 or 50% flame burn to determine if burn injury increased translocation of the yeasts across gut mucosa. Tissues were harvested between 3 and 24 hr postburn and cultured on Sabouraud dextrose agar. Control animals (no yeast challenge) showed no yeast in intestinal homogenates or in the mesenteric lymph nodes (MLN). At a dose of 1 X 10(9) yeasts, they did not escape from the gut lumen, with either a 30 or 50% burn. At a dose of 2 to 4 X 10(10) organisms, they translocated to the MLN in 92% of the 50%-burned animals (P less than 0.001), 75% of the 30%-burned animals (P less than 0.05), and 12.5% of unburned animals. The ileal mucosa appeared to be the most susceptible site for yeast invasion. To observe the penetration through the gut mucosa and/or translocation to other tissues, yeasts were labeled with biotin before administration, and tissues were stained with avidin-peroxidase diaminobenzidine sequence. With biotinylated yeasts, phagocytized organisms were observed in large numbers in the lamina propria and mesenteric lymph nodes but they were not viable upon culture. Toluidine blue staining of semithin sections revealed that translocated yeasts were located selectively in the lymphoid follicles of the MLN, entrapped by macrophages.  相似文献

目的 探讨磷脂酶A2(PLA2)抑制剂喹吖因对大鼠肠道缺血再灌注(gut ischemia reperfusion,GIR)损伤后肠源性细菌/内毒素移位的影响。方法 42只wistar大鼠随机分为正常组(6只)和GIR损伤组(36只)。GIR损伤组又分为GIR损伤对照组、再灌注后3h喹吖用药组和12h喹吖用药组,每组12只。取正常对照组、GIR损伤对照组及用药组48、72h门、腔静脉血测定内毒素含量、腔静脉血浆TNFα含量和肠系膜淋巴结、肝、肺、肾等肠道外器官组织进行细菌培养。结果 早期应用喹吖因,无论是GIR损伤后3h用药组还是12h用药组,均可明显降低GIR损伤后血浆内毒素和TNFα水平(P<0.01);显降低GIR损伤后肠系膜淋巴结和肺、肝、肾组织器官中的细菌移位发生率(P<0.05-0.01)。结论 GIR损伤后早期应用喹吖因可显地降低肠源性细菌/内毒素移位,减轻促炎性介质和细菌因子的释放,减轻肠道外脏器损伤。  相似文献

The present investigation was performed to study the kinetics of tissue distribution and deposition of Escherichia coli and endotoxin translocating from the intestine after thermal injury. Escherichia coli was grown in the presence of 14C glucose and both labeled bacteria and endotoxin prepared from the labeled bacteria were used as translocation probes. Escherichia coli (10(8) to 10(10) bacteria) and E. coli endotoxin (100 micrograms per animal) were gavaged into the stomach immediately before a 30% burn injury was inflicted in mice. Animals were killed 1, 4 and 24 hours after burn injury. Translocation occurred extensively within 1 hour after burn injury. Expressed as amount of radioactivity per gram of tissue, translocation was greatest in the mesenteric lymph node (MLN) followed by spleen, lung, and liver. Translocation of endotoxin was similar to translocation of intact bacteria, with the exception that less radioactivity could be found in the peritoneal cavity and more in the liver. Both intact E. coli and endotoxin translocated directly through the intact bowel wall. Killing of bacteria was greatest in the MLN and spleen, approximating 95% to more than 99% of translocating bacteria. Killing efficiency was lowest in the lungs. It is concluded that estimation of translocation by viable bacterial counts in tissues grossly underestimates the extent of translocation of bacteria and ignores the extent of translocation of endotoxin. Translocation of endotoxin may have biologic significance that is independent of and in addition to translocation of intact bacteria.  相似文献

Previously we have shown that under certain conditions, bacteria can pass through the intact epithelial mucosa to the mesenteric lymph nodes (MLN), liver, spleen, and bloodstream to cause infection, a process termed bacterial translocation. To extend these studies, we determined the influence of protein malnutrition and endotoxemia on bacterial translocation in burned (25% TBSA) and unburned mice. The results of these experiments documented that protein malnutrition did not promote bacterial translocation from the gut in either burned or unburned animals, although it did disrupt the normal indigenous gut flora. In contrast, a nonlethal dose of endotoxin (IP) promoted bacterial translocation to the mesenteric lymph nodes in burned and unburned mice, but only in burned mice did the bacteria translocate from the gut to other systemic organs (p less than 0.01). Furthermore, the mortality rate of mice receiving only endotoxin or burn was less than 10%, while the combination of endotoxin plus a thermal injury increased the mortality rate to 100% (p less than 0.01). These studies support the concept that bacteria may translocate from the gut to other organs and be a potential source of lethal infections after thermal injury.  相似文献

Bacterial infections are frequent complications after liver resection. Of 138 patients who underwent major hepatectomy, 11 patients (8%) developed intra-abdominal sepsis in the postoperative period. Seven bacterial strains of gut origin were isolated from the abdominal cavity. Eight patients had multiple bacteria cultured. In the experimental studies on rat models, positive mesenteric lymph node cultures were seen 2 hours after removal of 70% and 90% of the total weight of the rat liver, and 12 hours after 50% hepatectomy, persisting for 3 and 4 days after 50% and 70% hepatectomy, respectively. The incidences of bacteremia 2 and 4 hours after 90% hepatectomy were 80% and 100%, respectively; 6 hours after 70% liver resection, the incidence of bacteremia was 33%. Blood cultures were positive in only 6% of the rats following 50% hepatectomy, and in none of the controls. Thus, bacterial translocation occurs in the early course after hepatectomy, the incidence being proportional to the amount of liver tissue removed.  相似文献

We investigated the role of mesenteric lymph nodes in postburn systemic spread of intestinal bacteria. Group 1 minipigs (n=8) had a 40% third-degree burn. Group 2 minipigs (n=7) had the same burn injury, but their mesenteric lymph nodes were removed immediately after burn. Group 3 minipigs (n=8) had sham burn, and group 4 minipigs (n=6) had mesenteric lymph node removal under anesthesia. All minipigs were killed at 48 hours, and tissues were harvested for bacteriological culture. Group 1 showed a large number of positive cultures from several of the systemic organs. Group 2 demonstrated no positive cultures in any of the tissues except the peritoneal fluid. These data suggest that bacterial translocation occurs mainly via mesenteric lymphatics to mesenteric lymph nodes and, thence, into other systemic tissue. After major burns, mesenteric lymph nodes may become an additional focus of infection.  相似文献

HYPOTHESIS: The use of albumin in the early resuscitation formula after major burn has been forbidden because of its damaging effect on the gut barrier function. We hypothesize that inhibition of the inducible isoform of nitric oxide synthase to stabilize endothelial permeability and to retain albumin in the vascular space will ameliorate the major trauma-induced gut barrier dysfunction. DESIGN, INTERVENTIONS, AND MAIN OUTCOME MEASURES: In experiment 1, specific pathogen-free rats undergoing 35% total body surface area burn or sham burn were given equal volumes (7.5 mL/kg) of isotonic sodium chloride solution or albumin from femoral veins for fluid resuscitation at 0, 4, or 8 hours after burn. In experiment 2, intraperitoneal S-methylisothiourea sulfate (7.5 mg/kg) was given immediately after burn to rats from different groups, as in experiment 1 (SMT groups). At 24 hours after burn, the intestinal mucosa was assayed for myeloperoxidase activity as an index for neutrophil sequestration, the distribution of fluorescein isothiocyanate-dextran across the lumen of small intestine was determined to evaluate the intestinal permeability, and bacterial translocation (BT) to the mesenteric lymph nodes (MLNs) and histological findings in the ileum were also examined. RESULTS: Compared with sham burn, burn induced significant increases in intestinal mucosa myeloperoxidase activity, intestinal permeability, BT to the MLNs, and villi sloughing in rats. Albumin administration at 0 or 4 hours after burn enhanced the increases in neutrophil sequestration, permeability, and villi sloughing compared with saline injection at the same times. In contrast, injection of albumin in the burn-SMT group did not aggravate these changes in intestinal myeloperoxidase activity, intestinal permeability, BT to the MLNs, and villi edema. Burn-SMT rats with albumin injections at 4 or 8 hours after burn showed significant 35% and 52% decreases, respectively, in intestinal permeability compared with burn-SMT-saline rats. Use of albumin at 8 hours after burn in combination with S-methylisothiourea significantly attenuated BT to the MLNs and reduced villi edema. CONCLUSIONS: Early albumin resuscitation aggravated the burn-induced gut damage. Albumin administration and inhibition of the inducible isoform of nitric oxide synthase in combination decreased burn-induced gut barrier dysfunction and reversed the damaging effect of albumin on gut barrier function and decreased BT.  相似文献

目的比较大鼠肠道缺血再灌注损伤时肠淋巴干结扎与不结扎对循环中炎性介质和细菌内毒素的影响。方法采用肠道缺血再灌注模型进行肠系膜淋巴管结扎。大鼠随机分4组,每组10只:空白组(A组);假手术组(B组);肠道缺血再灌注组(C组);肠道缺血再灌注加淋巴干结扎组(D组)。缺血再灌注后,作肠系膜淋巴结培养计算细菌易位率;检测循环中内毒素、D-乳酸、二胺氧化酶、TNF—α、IL-1β、IL-6和sICAM-1水平。结果细菌易位率A、B两组为0;C组40%,D组20%。与A、B组比较,C、D两组内毒素、D-乳酸和二胺氧化酶水平显著增高(P〈0.05),且D组显著低于C组;血循环中各细胞因子除sICAM-1在D组与C组之间没有显著差异外,C组的IL-1β、IL-6和TNF-α浓度均明显高于D组(P〈0.05)。结论肠道缺血再灌注损伤时,肠淋巴干结扎可减少细菌在肠系膜淋巴结的定植,并减轻肠道缺血再灌注的损伤及增加通透性,从而减轻全身炎性反应。  相似文献

Previous investigations of a treated model of hemorrhagic shock in the rat indicated the frequent occurrence of bacteremia that appeared to derive from the gut. This paper determines the incidence of bacteremia and endotoxemia during the acute shock period and compares this with similar observations in humans in varying degrees of shock. Studies in 26 rats indicated that bacteremia and endotoxemia was present in 50% and 87%, respectively, by the end of 2 hours at a mean arterial pressure of 30 mmHg. Observations in 50 patients admitted to the trauma unit showed that positive bacterial blood cultures were present in 56% when the admission systolic blood pressure was 80 mmHg or less (p less than 0.01 compared with either of the other groups). Endotoxemia was noticed in two such patients. Direct access of bacteria and endotoxin to the blood stream may occur during hemorrhagic or traumatic shock and is the probable cause of subsequent sepsis in traumatized patients when no other source is apparent.  相似文献
