目的探讨交感神经皮肤反应(SSR)对抑郁症患者的临床价值。方法对45例抑郁症患者治疗前后和43例正常健康者分别进行SSR测定和汉密尔顿抑郁量表(HAMD)评定,并加以比较。结果抑郁症组治疗前SSR测定的异常率为84.4%(38/45),而治疗后异常率为13.3%(6/45)。治疗前SSR测定波潜伏期及波幅值较治疗后及对照组分别延长和降低,差异有统计学意义(P<0.01);治疗后抑郁症组HAMD评定分值较治疗前降低,差异有统计学意义(P<0.01)。相关分析结果表明,抑郁症患者SSR波潜伏期与HAMD分值呈显著正相关(P<0.01),其波幅与HAMD分值呈显著负相关(P<0.01)。结论交感神经皮肤反应测定可作为判定抑郁症患者自主神经功能的参考指标应用于临床。  相似文献

目的 应用心率变异性(Heart rate variability,HRV)的短时频域分析,探讨以躯体症状为主的抑郁症患者心理应激与自主神经功能的变化特点.方法 对51例以躯体症状表现为主的抑郁症患者组(躯体症状组)和57例以情绪核心症状为主的抑郁症患者组(情绪障碍组)、42例躯体形式障碍组(躯体形式组)及48例正常对照组分别在静息、应激及放松过程记录LF值、HF值、LF/HF值,应激变化值及放松变化值.结果 (1)躯体症状组LF应激变化值较情绪障碍组、躯体形式组及正常对照组高(P=0.006,0.031,0.000);躯体症状组LF放松变化值较情绪障碍组、躯体形式组及正常对照组低(P=0.040,0.011,0.004).(2)躯体症状组HF应激变化值较情绪障碍组、正常对照组高(P=0.000,0.000);躯体症状组HF放松变化值较躯体形式组、正常对照组低(P=0.014,0.000).(3)躯体症状组LF/HF应激变化值较情绪障碍组、躯体形式组及正常对照组高(P=0.000,0.025,0.000);躯体症状组LF/HF放松变化值较情绪障碍组、躯体形式组及正常对照组低(P=0.018,0.027,0.000).结论 以躯体症状表现为主的抑郁症患者应激后自主神经表现为高反应性,低习惯化和弱恢复性.  相似文献

躯体化症状为主的抑郁症心率变异性对照研究   总被引:2,自引:0,他引:2  
目的:探讨躯体化症状为主的抑郁症患者自主神经系统的特点及抑郁症躯体化表现的发生机制。方法:对30例躯体化症状为主的患者(A组)和30例情绪症状为主的患者(B组)及30例健康正常者(C组)分别进行汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA)评定及短时(10min)心率变异性(HRV)分析。结果:A、B、C3组HRV分析指标RR间期标准差(SDNN)分别为(50.84±19.92)ms、(94.93±28.80)ms、(105.77±22.05)ms,A组显著低于C组(t=5.68,P〈0.01),较B组低(t=3.73,P〈0.05);3组LF/HF分别为(6.26±2.11)ms、(3.51±2.44)ms、(1.80±1.07)ms,A组显著高于C组(t=6.35,P〈0.01),较B组高(t=2.50,P〈0.05);A组HAMD评分(28.30±6.26)分高于B组(24.35±6.69)分,二者差异有显著性(t=3.14,P〈0.05);A组HAMA评分(22.70±4.92)分显著高于B组(13.05±4.71)分,二者差异有显著性(t=6.17,P〈0.01);HAMD焦虑/躯体化因子分与HRV指标SDNN、LF、HF、VLF和LF/HF均呈中度相关(r分别为0.49、0.61、0.58、0.50、0.63)(P〈0.05或P〈0.01)。结论:躯体化症状表现为主的抑郁症患者多伴有焦虑,自主神经功能紊乱。  相似文献

目的 比较以躯体症状与以精神症状为主的两种抑郁症患者出院后社会功能损害状况及其影响因素。方法 收集符合CCMD-2-R诊断标准的抑郁症患者67例,按不同的主诉分成两组,用病史问卷及汉密尔顿抑郁量表(HAMD)进行调查与评定;出院后1年随访时,再次进行HAMD评分,同时采用SDSS评定患者的社会功能损害,以及对其影响因素进行相关分析。结果 1年后随访发现,两组的社会功能存在极显著差异;患者的社会功能与随访时的HAMD评分、复发次数、迟滞因子分和阳性家族史等因素相关。结论 以躯体症状为主的抑郁症患者治疗后能保持较好的社会功能,关键在于早期识别与治疗。  相似文献

目的:比较在综合医院就诊,分别以躯体症状或情感症状为主诉的抑郁症患者的区别.方法:将在内科普通门诊及心理咨询门诊确诊的抑郁症患者87例,采用汉密尔顿抑郁量表(HAMD),汉密尔顿焦虑量表(HAMA)和自编的深度调查问卷,对患者进行测评,分成情感症状组及躯体症状组.结果:两组在年龄、性别、婚姻、职业及HAMD和HAMA评分等无显著差异.两组在文化程度、性格特征、躯体不适出现频度、首诊求医方式、对自身疾病认识,以及对抑郁症诊断的接受程度有显著差异.在有关抑郁症知识、治疗及婚姻、工作影响等方面无显著差异.结论:以躯体症状或情感症状为主诉的抑郁症患者在某些方面有所不同.  相似文献

目的 探讨交感神经皮肤反应(SSR)对帕金森病自主神经功能障碍的诊断价值.方法 对解放军第一○二医院神经内科自2006年7月至2008年8月门诊或住院的47例帕金森病患者及与之相匹配的20例健康人进行SSR检测,并对其中有自主神经功能障碍患者与无自主神经功能障碍患者的结果进行对比分析. 结果帕金森病患者与健康人相比,上肢潜伏期明显延长(1.55±0.18 vs 1.42±0.29),波幅明显降低(1.87±0.26 vs 2.56±1.47);下肢潜伏期明显延长(2.13±0.16vs 2.04±0.27),波幅明显降低(0.49±0.21 vs 0.76±0.39),差异均有统计学意义(P<0.05).有自主神经功能障碍患者与无自主神经功能障碍患者相比,上肢潜伏期明显延长(1.56 ± 0.17 vs 1.53 ± 0.15),波幅明显降低(1.75±0.21 vs 1.89±0.33);下肢潜伏期明显延长(2.17 ± 0.18 vs 2.08±0.24),波幅明显降低(0.46±0.20 vs 0.51±0.17),差异均有统计学意义(P<0.05). 结论 SSR检测结果与患者临床表现相一致,其对帕金森病患者的自主神经功能障碍有诊断价值.  相似文献

对38例多发性硬化(MS)患者和41例正常人的交感神经皮肤反应(SSR)与多方式诱发电位进行研究。结果:38例确诊和近似确诊的MS患者SSR异常率(81.6%)比异常的诱发电位更为常见。在伴有自主神经症状的21例MS患者中19例SSR异常(90.5%),17例无自主神经症状的MS患者中12例SSR异常(70.6%)。提示:SSR能客观有效地反映自主神经功能状态,检出MS的亚临床异常。将SSR与多方式诱发电位联合评估MS中枢神经系统损害,具有更高的敏感性,有助于MS的早期诊断。  相似文献

目的:了解抑郁症患者的躯体症状特征。方法:采用自制调查表对符合中国精神障碍分类与诊断标准第3版抑郁发作的119例门诊及住院患者进行调查。根据汉密尔顿抑郁量表(HAMD)评分,将119例患者分为轻度、重度抑郁症两组,比较两组的一般资料及躯体症状。结果:两组的一般资料差异无显著性,而重度抑郁组躯体症状与轻度抑郁组差异显著。结论:抑郁症的躯体症状非常突出,应引起足够重视。  相似文献
