RATIONALE AND OBJECTIVES: To evaluate the magnetic resonance imaging and electron microscopic findings of the hyperacute stage of cerebral fat embolism in cats and the time needed for the development of vasogenic edema. METHODS: Magnetic resonance imaging was performed at 30 minutes (group 1, n = 9) and at 30 minutes and 1, 2, 4, and 6 hours after embolization with triolein (group 2, n = 10). As a control for group 2, the same acquisition was obtained after embolization with polyvinyl alcohol particles (group 3, n = 5). Magnetic resonance images were analyzed qualitatively and quantitatively. Electron microscopic examination was done in all cats. RESULTS: In group 1, the lesions were iso- or slightly hyperintense on T2-weighted (T2W) and diffusion-weighted (DWIs) images, hypointense on the apparent diffusion coefficient (ADC) map image, and markedly enhanced on the gadolinium-enhanced T1-weighted images (Gd-T1WIs). In group 2 at 30 minutes, the lesions were similar to those in group 1. Thereafter, the lesions became more hyperintense on T2WIs and DWIs and more hypointense on the ADC map image. The lesions were enhanced on Gd-T1WIs at all acquisition times. In group 3, the lesions showed mild hyperintensity on T2WIs at 6 hours but hypointensity on the ADC map image from 30 minutes, with a tendency toward a greater decrease over time. The lesions were not enhanced on Gd-T1WIs at any time point. Electron microscopic findings revealed discontinuity of the capillary endothelial wall, perivascular and interstitial edema, and swelling of glial and neuronal cells in groups 1 and 2. Cellular swelling and interstitial edema were more prominent in group 2. In group 3, interstitial edema was seen; however, discontinuity of the endothelial wall was absent. CONCLUSIONS: The lesions were hyperintense on T2WIs and DWIs, hypointense on the ADC map image, and enhanced on Gd-T1WIs. On electron microscopy, the lesions showed cytotoxic and vasogenic edema with disruption of the blood-brain barrier. Vasogenic edema seems to develop within 30 minutes in cerebral fat embolism in cats.  相似文献

PURPOSE: The purpose of this study was to evaluate the cerebral hemodynamic change in the hyperacute stage of cerebral fat embolism induced by triolein emulsion, by using MR perfusion imaging in cat brains. METHODS: By using the femoral arterial approach, the internal carotid arteries of 14 cats were infused with an emulsion of triolein 0.05 mL. T2-weighted (T2WI), diffusion-weighted (DWI), apparent diffusion coefficient (ADC) map, perfusion-weighted (PWI), and gadolinium-enhanced T1-weighted (Gd-T1WI) images were obtained serially at 30 minutes and 2, 4, and 6 hours after infusion. The MR images were evaluated qualitatively and quantitatively. Qualitative evaluation was performed by assessing the signal intensity of the serial MR images. Quantitative assessment was performed by comparing the signal-intensity ratio (SIR) of the lesions to the contralateral normal side calculated on T2WIs, Gd-T1WIs, DWIs, and ADC maps at each acquisition time and by comparing the relative cerebral blood volume (rCBV), cerebral blood flow (CBF), and mean transit times (MTT) of the lesions to the contralateral normal side calculated on PWI. RESULTS: In the qualitative evaluation of the MR images, the lesions showed hyperintensity on T2WIs, enhancement on the Gd-T1WIs, and isointensity on DWIs and the ADC maps. In the quantitative studies, SIRs on the Gd-T1WIs, DWIs, and ADC maps peaked at 2 hours after infusion. The SIRs on the T2WIs peaked at 4 hours after infusion and decreased thereafter. On PWIs, the rCBV, rCBF, and MTT of the lesion showed no significant difference from the contralateral normal side (P = .09, .30, and .13, respectively) and showed no significant change of time course (P = .17, .31, and .66, respectively). CONCLUSION: The embolized lesions induced by triolein emulsion showed no significant difference in cerebral hemodynamic parameters from those on the contralateral normal side. The result may suggest that consideration of the hemodynamic factor of embolized lesions is not necessary in further studies of the blood-brain barrier with triolein emulsion.  相似文献

BACKGROUND AND PURPOSE: Carbon monoxide intoxication has delayed effects on the cerebral white matter characterized by bilateral, confluent lesions that reflect diffuse demyelination. To increase our understanding of this process, we assessed the diffusion characteristics of these lesions. METHODS: Five consecutive patients with delayed encephalopathy of CO intoxication were examined with diffusion MR imaging. Diffusion-weighted images (DWIs) were obtained 25-95 days after their exposure to CO and during a relapse of neuropsychiatric symptoms, which occurred after an initial recovery. Imaging was performed at 1.5 T by using a spin-echo echo-planar sequence with diffusion gradients of 0, 500, and 1000 s/mm(2). DWIs and apparent diffusion coefficient (ADC) maps were visually evaluated, and mean ADCs were calculated from the periventricular white matter and the centrum semiovale, where confluent hyperintensity was seen on T2-weighted images. Findings were compared with those of normal-looking white matter. RESULTS: In all five patients, both T2-weighted images and DWIs showed the white matter lesions as bilateral, diffuse, confluent areas of hyperintensity in the periventricular white matter and centrum semiovale. On ADC maps, these lesions were isointense, with focal areas of hypointensity (n = 4) or diffuse hypointensity (n = 1). Mean ADC values of the white matter lesions were significantly lower than those of normal-looking white matter, regardless of their isointensity or hypointensity on ADC maps (P <.05). CONCLUSION: Bilateral, confluent, white matter lesions in patients with delayed encephalopathy of CO intoxication show decreased diffusivity.  相似文献

PURPOSE: The authors investigated whether fatty acid emulsion affects the blood-brain barrier (BBB), whether disrupted BBB is reversible, and whether the fatty acid emulsion technique may be a model for BBB research. MATERIALS AND METHODS: The fat emulsion was made with 0.05 mL of oleic acid or linoleic acid and 20 mL of normal saline. The internal carotid artery in 14 cats was infused with oleic acid emulsion (group 1) and with linoleic acid emulsion in 12 cats (group 2). Gd-enhanced T1-weighted (Gd-T1WI), diffusion-weighted (DWI), and additional apparent diffusion coefficient (ADC) map magnetic resonance imaging (MRI) was obtained at 1 hour, 1 and 4 days, and 1 week after infusion. MRI findings were evaluated qualitatively. Quantitatively, the signal intensity ratio (SIR) of the lesion to the contralateral hemisphere was measured on Gd-T1WIs. The SIRs were statistically analyzed using the student t test. The brain tissue was removed immediately for light and electron microscopy examination if the lesion showed no contrast enhancement and was isointense on DWIs and the ADC maps. RESULTS: The lesions appeared at 1 hour in both groups as contrast enhancement on Gd-T1WIs, as isointensity or mild hyperintensity on DWIs, and as isointensity on the ADC maps. On day 1, these MRI findings were decreased in group 1 and were not seen in group 2. At 1 hour, the SIRs of group 1 were significantly higher than those of group 2 (P = 0.016). On day 1, the SIRs of both groups approximated 1.0. Light microscopy findings revealed minor necrosis and demyelination in one cat from group 1 and in 3 cats from group 2. Electron microscopy examinations showed minimal findings in the cortical lesions in groups 1 and 2. CONCLUSIONS: Infusion of unsaturated fatty acid emulsion into the carotid artery of cats revealed vasogenic edema of the brain and reversible changes as depicted on MRI. This unsaturated fat emulsion technique may be used as a model for research on BBB disruption.  相似文献

Objectives: The purpose of this study was to evaluate the MRI characteristics of venous thrombus over set time thresholds with histopathological correlation in a porcine model. Methods: Inferior vena cava thrombi were induced in 12 pigs. MRI was performed in three pigs 2 h, 1 day, 3 days and 2 weeks after thrombus induction. Results: The MRI characteristics were analysed in correlation with histopathological findings. The thrombi after 2 hours, which consisted of red blood cells (RBCs), showed isointensity on T(1 )weighted images (T(1)WIs) and hyperintensity on both T(2 )weighted images (T(2)WIs) and diffusion-weighted images (DWIs). The mean apparent diffusion coefficient (ADC) value was 1.93 × 10(-3) mm(2) s(-1). The thrombi after Day 1, which consisted of RBCs and migrating neutrophils at the periphery, showed isointensity on T(1)WIs, slight hyperintensity on T(2)WIs and hypointensity on DWIs. The mean ADC value was 1.62 × 10(-3) mm(2) s(-1) [corrected]. The thrombi after Day 3, which consisted of RBCs and peripheral inflammatory cells including macrophages, showed isointensity with peripheral hyperintense regions on T(1)WIs and hypointensity on both T(2)WIs and DWIs. The mean ADC value was 1.67 × 10(-3) mm(2) s(-1). After 2 weeks, the thrombi, which revealed RBC lysis surrounded by granulation tissues, showed isointensity on T(1)WIs and hyperintensity on T(2)WIs and DWIs. The mean ADC value was 2.48 × 10(-3) mm(2) s(-1). Conclusion: The temporal MRI characteristics seemed to be related to chemical and physical changes in RBC and organisation of granulation tissues. Free radicals generated by macrophages might also be related to some extent.  相似文献

目的探讨脑内型表皮样囊肿的MRI特点,以加强对该病的认识,提高正确诊断率。资料与方法分析经手术病理证实的8例脑内型表皮样囊肿的MRI表现。全部患者均进行了T1WI、T2WI平扫及增强检查,5例患者行液体衰减反转恢复(FLAIR)序列检查。结果8例表皮样囊肿中,位于颞叶5例,额叶2例,额、颞叶交界区1例。所有病灶均发生在皮质区或皮质下区。病变轮廓不光整者5例,较光整者3例。病变与周围组织结构均分界清晰。在T1WI上病灶呈均匀低信号3例,以低信号为主其内间以斑片状、点状及条状中等信号或略高信号者4例,中心呈低、等混杂信号外围见同心圆形等信号者1例;在T2WI上呈均匀高信号,信号强度与脑脊液信号相似者4例,以高信号为主其内间以斑片状、点状及条状略高信号者3例,以高信号为主其内间以略高信号及略低信号者1例;5例行FLAIR序列检查,病灶均呈略高、等、低混杂信号,病灶周边环以线样或斑片样高信号。除1例病灶周围脑组织有轻度水肿外,均未见灶周水肿征象。病灶占位效应轻,周围组织受压征象不明显。MR增强扫描,2例见斑片状不均匀强化,其余病例均未见明显强化。结论脑内型表皮样囊肿的MRI表现具特征性,MRI表现有助于对该病的诊断。  相似文献

The MRI features of small renal cell carcinomas (RCCs) were retrospectively reviewed and correlated with histology in 24 patients. MRI features on both T1- and T2-weighted images were classified into hypointensity, isointensity, and hyperintensity. Each tumor was pathologically classified into four types: alveolar, papillary, tubular, and cystic. These findings were correlated with MR signal intensities. Alveolar tumors showed hypointensity to isointensity on T1-weighted image and isointensity to hyperintensity on T2-weighted image. In contrast, all papillary tumors showed hypointensity on T2-weighted image. Four of six tumors with hypointensity on T2-weighted image were caused by hemosiderin deposition, hemorrhage, and necrosis. However, there were two papillary RCCs that showed hypointensity on T2-weighted image despite no hemosiderin deposition and no hemorrhage. We conclude that papillary RCC is associated with T2-hypointense appearance as well as hemosiderin deposition, hemorrhage, and necrosis.  相似文献

目的:探讨软骨肉瘤的影像学表现,提高对本病诊断的准确性。方法:回顾性分析25例经手术病理证实的软骨肉瘤的X线、CT和MRI资料。结果:25例软骨肉瘤中原发23例,其中普通型20例,去分化型、黏液型及透明细胞型各1例;继发2例。X线及CT表现:原发的23例中溶骨性骨破坏5例,混合性骨破坏18例,形成软组织肿块20例,其中表现为低密度19例,等密度1例。20例普通型及1例黏液型见散在钙化。MRI表现:普通型软骨肉瘤T1WI呈等或稍低信号,T2WI呈等或稍高信号;黏液型T1WI呈等、低混杂信号,T2WI呈稍高、低的混杂信号,钙化T2WI呈低信号;透明细胞型病例T1WI及T2WI呈近乎等信号;去分化型T1WI呈等低混杂信号,T2WI呈不均匀高信号。6例普通型及1例继发型出现环形、间隔样中等-明显强化,黏液型及去分化型呈不均匀中等-明显强化。结论:普通X线及CT是软骨肉瘤影像诊断及鉴别诊断的主要手段,MRI显示软骨信号为重要提示。MRI显示病变范围更为清楚、准确,为临床治疗提供可靠依据。  相似文献

PURPOSETo compare the MR signal intensity patterns and enhancement pattern of intracranial tuberculomas with their histopathologic features.METHODSMR images of six patients with surgically proved intracranial tuberculoma were reviewed retrospectively and were compared with histologic findings of the resected specimen. Detailed histologic examination was performed to look for the extent and characteristics of caseation necrosis, fibrosis, and inflammatory cellular infiltrates at each area of different signal intensities and at the enhancing areas on MR. Signal intensities for T1- and T2-weighted images were compared with normal gray matter.RESULTSOn T1-weighted images, the granulomas showed a slightly hyperintense rim surrounded by a complete or partial rim of slight hypointensity and central isointensity or mixed isointensity and hyperintensity in five patients and homogeneous isointensity in one patient. Histologically, the zone of central isointensity or mixed intensity corresponded to caseation necrosis plus adjacent cellular infiltrates. The hyperintense and hypointense rims corresponded to the layers of collagenous fiber and the layers of the inflammatory cellular infiltrates, respectively. On T2-weighted images, the entire portion of the granuloma showed slightly heterogeneous isointensity or hypointensity with small markedly hypointense foci in five patients, and a hyperintense center surrounded by a hypointense rim in one patient. Histologic layers were not discriminated on T2-weighted images. On postcontrast T1-weighted images, there were single or multiple conglomerate ring enhancements within a tuberculoma in all six patients, corresponding to the layers of both collagenous and inflammatory cells.CONCLUSIONCombination of the described signal intensity patterns and conglomerate ringlike enhancing appearance of the lesion is characteristic of tuberculoma, and may play an important role in differentiating intracranial tuberculomas from other ring-enhancing brain lesions.  相似文献

BACKGROUND AND PURPOSE: In fat embolism, free fatty acid is more toxic than neutral fat in terms of tissue damage. We evaluated the hyperacute embolic effects of triolein and oleic acid in cat brains by using MR imaging and electron microscopy. METHODS: T2-weighted imaging, diffusion-weighted imaging, and contrast-enhanced T1-weighted imaging were performed in cat brains after the injection of triolein (group 1, n = 8) or oleic acid (group 2, n = 10) into the internal carotid artery. MR images were quantitatively assessed by comparing the signal intensity ratios of the lesions with their counterparts on T2-weighted images, apparent diffusion coefficient (ADC) maps, and contrast-enhanced T1-weighted images. Electron microscopic findings in group 1 were compared with those in group 2. RESULTS: Qualitatively, MR images revealed two types of lesions. Type 1 lesions were hyperintense on diffusion-weighted images and hypointense on ADC maps. Type 2 lesions were isointense or mildly hyperintense on diffusion-weighted images and isointense on ADC maps. Quantitatively, the signal intensity ratios of type 1 lesions in group 2 specimens were significantly higher on T2-weighted images (P =.013)/(P =.027) and lower on ADC maps compared with those of group 1. Electron microscopy of type 1 lesions in both groups revealed more prominent widening of the perivascular space and swelling of the neural cells in group 2, in contrast to notable endothelial defects in group 1. CONCLUSION: MR and electron microscopic data on cerebral fat embolism induced by either triolein or oleic acid revealed characteristics suggestive of both vasogenic and cytotoxic edema in the hyperacute stage. Tissue damage appeared more severe in the oleic acid group than in the triolein group.  相似文献

目的 探讨MRI对软组织脉管畸形的诊断价值.资料与方法 回顾性分析575例脉管畸形患者的MRI表现.结果 490例血管畸形,T1WI等信号88例,混杂信号402例;T2WI不均匀高信号475例,较均匀高信号15例,圆形血管样高信号210例,在病变的边缘可见尖角样改变216例.43例淋巴管畸形,T1WI等信号2例,混杂信号36例;T2WI不均匀高信号38例,5例T1WI、T2WI表现为脂肪增厚,另见囊状灶26例.42例血管、淋巴管混合畸形,T1WI等信号2例,混杂信号40例;T2WI不均匀高信号42例.结论 MRI能反映脉管畸形的特征,对其诊断与鉴别诊断有重要价值.  相似文献

Chronic injuries of the spinal cord: assessment with MR imaging   总被引:6,自引:0,他引:6  
Seventy-six patients with persistent myelopathy secondary to chronic spinal cord injuries underwent examination with magnetic resonance (MR) imaging, the results of which were correlated with neurologic findings. Twenty-one patients received follow-up study during and after the acute stage; 55 patients were examined only at a chronic stage. Spinal cord abnormalities were seen in 48 patients according to five patterns: (a) normal signal intensity on T1- and T2-weighted images (pattern N/N, n = 28), (b) normal signal intensity on T1-weighted and hyperintensity on T2-weighted images (pattern N/Hi, n = 18), (c) hypointensity on T1-weighted and hyperintensity on T2-weighted images (pattern Lo/Hi, n = 17), (d) cord atrophy (n = 5), and (e) longitudinal syrinx formation with hypointensity on T1- and hyperintensity or isointensity on T2-weighted images (n = 8). Patients with pattern N/N had only slight neurologic damage and an excellent prognosis. Patients with pattern N/Hi had mild neurologic impairment, frequently associated with cord compression. Patients with pattern Lo/Hi had the worst prognosis. Atrophy was observed in patients with a long history of myelopathy.  相似文献

BACKGROUND AND PURPOSE: Clinical cerebral-fat embolism shows both reversible and irreversible changes. We used MR imaging to investigate the reversibility of embolized lesions induced with a fat-emulsion technique and to evaluate the histologic findings. METHODS: A fat emulsion was made with 0.05 mL of triolein and 20 mL of normal saline and vigorous to-and-fro movement through a three-way stopcock. In 50 cats, the internal carotid artery was infused with the fat emulsion. Cats were divided into six groups on the basis of time delay after embolization: 1 hour; 1 and 4 days; and 1, 2, and 3 weeks. MR imaging and histologic examination were performed at these times. RESULTS: Embolized lesions were hyperintense on T2-weighted images, isointense or mildly hyperintense on diffusion-weighted images, isointense on apparent diffusion coefficient maps, and enhancing on gadolinium-enhanced T1-weighted images at 1 hour. These MR imaging findings were less evident at day 1 and reverted to normal after day 4 (isointense on all images). Electron microscopy showed minimal findings in the cortical lesion in groups 1 and 2 (group 1 at 1 hour and group 2 at 1 hour and 1 day). Light microscopic findings revealed evidence of necrosis-small focal gliosis and demyelination in the periventricular white matter-in only one cat. The number of intravascular fat globules was not significantly different between groups, as visualized by oil red O staining. CONCLUSION: Cerebral-fat embolism induced by a triolein emulsion revealed reversible MR findings and minimal histologic findings.  相似文献

BACKGROUND AND PURPOSE: Intracranial lipomas are uncommon congenital malformations that most often are asymptomatic. A rare subset of intracranial lipomas arises within the vestibule of the inner ear, which can cause sensorineural hearing loss. We present the CT and MR imaging characteristics of lipomas within the vestibule of the inner ear and propose a pathogenetic mechanism for this malformation. METHODS: We retrospectively reviewed five cases of vestibular lipoma. T1-weighted axial conventional spin-echo and T2-weighted axial fast spin-echo sequences were performed in all five cases. Four patients underwent T1-weighted fat-saturated imaging, and one underwent CT scanning with Hounsfield units measured. RESULTS: Four female patients ranging in age from 10 to 40 years and one 26-year-old male patient had sensorineural hearing loss. Two patients had progressive high-frequency loss; one, sudden-onset high-frequency loss; and two, congenital high-frequency loss. All had lesions within the vestibule that displayed hyperintensity on T1-weighted images, isointensity to slight hypointensity on T2-weighted fast spin-echo images, hypointensity with fat saturation, and fat attenuation on CT scans. Three of the cases were associated with lipoma of the cerebellopontine angle. In none of these cases were the cerebellopontine angle and labyrinthine lipomas contiguous. CONCLUSION: Intravestibular lipoma is considered when a focal hyperintense lesion is identified within the vestibule of the inner ear on T1-weighted MR images. Lesion hypointensity on fat-saturated T1-weighted MR images verifies its lipomatous nature and separates it from intralabyrinthine hemorrhage or highly proteinaceous fluid.  相似文献

BACKGROUND AND PURPOSE: Subdural empyema (SDE), an infection of the subdural space, occurs most often in pediatric patients as a complication of meningitis, sinusitis, or otitis media. Diffusion-weighted imaging (DWI) has been used in the past to investigate intracerebral infections. The purpose of this study was to determine the signal intensity characteristics of SDE on DWIs as well as the corresponding apparent diffusion coefficient (ADC) maps. METHODS: MR studies of 10 patients with SDEs were retrospectively reviewed. Included were routine sequences and DWI, which consisted of an axial single-shot echo-planar spin-echo sequence (TR/TE, 4000/110) with b values of 0, 500, and 1000 s/mm(2). Signal-intensity characteristics on routine MR images and DWIs were evaluated. In seven patients, ADC values of the lesions were calculated by using two b values. Follow-up imaging study was performed in seven patients. RESULTS: In nine patients, the empyema was hyperintense on DWIs. In the remaining patient, the empyema showed mixed hyperintensity and hypointensity. ADC values were lower than those of normal cortical gray matter and much lower than those of reactive subdural effusions. In all seven patients with persistent clinical signs of infection, the empyemas were hyperintense on follow-up DWIs. CONCLUSION: SDE had high signal intensity on DWIs and low signal intensity on ADC maps, with an ADC value lower than that of the normal cortical gray matter. Diffusion MR imaging can be valuable in distinguishing SDE from effusion and in the follow-up of subdural collections.  相似文献

The magnetic resonance signal intensity and cyst content were analyzed in 2 new and 17 previously reported cases of Rathke cleft cyst. Lesions displaying hypointensity on T1-weighted images (T1WI) and hyperintensity on T2-weighted images (T2WI) contained CSF-like fluid. Cases showing hyperintensity on T1WI and isointensity on T2WI frequently contained mucoid material. Hyperintensity on both T1WI and T2WI suggests the presence of blood. A small area of hypo- or isointensity indicates thickening of the cyst wall or collection of cellular debris.  相似文献

目的 以DSA为参照评价子宫肌瘤的MRI表现。资料与方法 对DSA上有明显血供的子宫肌瘤行MRI检查,所有病例采用SE T1、turbo—SE T2WI和SET1WI增强扫描,对比剂为Gd—DTPA,剂量0.1mmol/kg体重,对不同成像序列的MRI信号特点和相关征象进行分析。结果 37个肌瘤T1WI均表现为等、低信号T2WI多为等、高信号(28个肌瘤),增强扫描肌瘤均出现明显强化,信号多数高于或等于子宫肌层(35个肌瘤)。多数肌瘤内部为均匀结节状或团块状改变,其间可见分隔结构,肌瘤包膜在T1WI、T1WI和增强扫描时能明确显示。结论 DSA表现明显血供的子宫肌瘤MRI有一定的特征性表现,增强扫描能够较为准确地反映肌瘤血供。  相似文献

目的:评价MRI平扫及Gd-DTPA动态增强对射频治疗肝细胞癌疗效随访的价值。材料和方法:回顾性分析了55例RF术前及术后的MRI表现,并与活检及长期随访结果进行对照。结果:RF术前病灶在T1加权上多呈相对低信号、T2加权像上呈相对高信号,RF术后病灶转变成T1加权较高信号、T2加权均匀一致的等低信号,残存的癌结节表现为病灶周围包膜不规整或周边小结节并在动态增强时呈单峰型“快进快出”强化表现,活检提示癌灶残存。结论:MR平扫评价RF的疗效基本可靠,动态增强能进一步提高评价信心及准确率。RF术后定期MR随访具有重要意义。  相似文献

Cavernous hemangiomas occur very rarely in the cavernous sinus and are difficult to diagnose preoperatively. MR images obtained in five patients with surgically verified cavernous hemangiomas in the cavernous sinus were reviewed. MR images showed hypointensity on T1-weighted images and well-defined hyperintensity on T2-weighted images with marked homogeneous enhancement after contrast material administration. We report the characteristic MR imaging observations for these lesions.  相似文献
