目的:研究ERCC1 Asn118Asn(C→T,rs11615)多态性与中国汉族晚期大肠癌患者对奥沙利铂(Oxaliplatin)一线化疗效果的关系。方法:99例晚期大肠癌患者化疗前取静脉血并提取DNA,以实时荧光定量PCR法对ERCC1 Asn118Asn进行SNP分型。患者接受奥沙利铂化疗,观察至疾病进展时间(1TrP)及疾病控制率(DCR),比较不同基因型与化疗效果的关系。结果:ERCC1 Asn118Asn基因位点在本研究人群中的突变频率C/C为50.51%,C/T为41.41%,T/T为8.08%。其中62例Ⅳ期大肠癌患者疾病控制率为54.84%,C/C与C/T+T/T基因型在疾病控制组(CR+PR+SD)和未控组(PD)之间分布差异有统计学意义(OR:3.764,95%CI:1.310~10.813)。37例Ⅲ期大肠癌患者中位DFS为7个月(95%CI:5.483~9.653),C/C型患者中位DFS为10个月(95%CI:8.215~11.642),而C/T+T/T型患者为5个月(95%CI:4.663~7.267),两者差异显著(Log—rank X^2=12.864,OR=2.237,P〈0.01)。结论:ERCC1 Asn118Asn基因多态性与中国汉族晚期大肠癌患者接受奥沙利铂一线化疗后的临床效果有关。  相似文献

Aim: Platinum agents have shown to be effective in the treatment of colorectal cancer. We assessed whethersingle nucleotide polymorphisms (SNPs) in GSTP1, ERCC1 Asn118Asn and ERCC2 Lys751Gln might predictthe overall survival in patients receiving oxaliplatin-based chemotherapy in a Chinese population. Methods:SNPs of GSTP1, ERCC1 Asn118Asn and ERCC2 Lys751Gln in 335 colorectal cancer patients were assessedusing TaqMan nuclease assays. Results: At the time of final analysis on Nov. 2011, the median follow-up periodwas 37.7 months (range from 1 to 60 months). A total of 229 patients died during follow-up. Our study showedGSTP1 Val/Val (HR=0.44, 95% CI=0.18-0.98), ERCC1 C/C (HR=0.20, 95% CI=0.10-0.79) and ERCC2 G/G(HR=0.48, 95% CI=0.19-0.97) to be significantly associated with better survival of colorectal cancer. GSTP1Val/Val, ERCC1 C/C and ERCC2 G/G were also related to longer survival among patients with colon cancer,with HRs (95% CIs) of 0.41 (0.16-0.91), 0.16 (0.09-0.74) and 0.34 (0.16-0.91), respectively. Conclusion: GSTP1,GSTP1, ERCC1 Asn118Asn and ERCC2 Lys751Gln genotyping might facilitate tailored oxaliplatin-basedchemotherapy for colorectal cancer patients.  相似文献

目的:探讨切除修复交叉互补基因1(ERCC1)Asn118Asn(C→T,rs11615)多态性与中国汉族晚期胃癌患者对奥沙利铂一线化疗效果的关系。方法:71例晚期胃癌患者化疗前抽静脉血提取DNA,以real-timePCR法对ERCC1基因进行SNP分型。患者接受奥沙利铂化疗,观察疾病控制率(CR+PR+SD)及至肿瘤进展时间(TTP),并比较不同基因型与疗效的关系。结果:47例Ⅳ期胃癌患者可以评价化疗疗效,疾病控制率为55.3%,C/C与C/T+T/T基因型在疾病控制组和无效组之间分布无统计学意义(χ2=2.755,P=0.097)。71例晚期胃癌患者中位TTP为6个月,C/T+T/T基因型患者其中位TTP为5个月,与纯合基因型C/C中位TTP6个月相比,差异无统计学意义(OR=0.615,P=0.071)。其中24例Ⅲ期胃癌患者中位TTP为8个月,C/T+T/T型患者中位TTP为7个月,与纯合基因型C/C中位TTP8个月相比,差异无统计学意义(OR=0.397,P=0.111)。结论:ERCC1Asn118Asn基因多态性与中国汉族晚期胃癌患者接受奥沙利铂一线化疗后的临床效果有相关趋势,需要进一步观察论证。  相似文献

The published data on the predictive value of polymorphism of ERCC1 and XPD in patients with advanced non-small cell lung cancer receiving platinum-based chemotherapy are inconclusive. To derive a more precise estimation of the relationship, a meta-analysis was performed. Relevant studies were identified by searching the Medline, Embase, CNKI and American Society of Clinical Oncology abstract databases. Inclusion criteria were patients with advanced NSCLC, received platinum-based chemotherapy, evaluation of polymorphism of ERCC1 and XPD and overall response rate (ORR). A total of 12 studies were included in this meta-analysis. For studies evaluating ERCC1 polymorphism at codon 118, the ORR for the wild-type C/C genotype versus the heterozygous C/T and T/T genotype was 2.17 (95% confidence interval (CI), 1.43–3.33; P = 0.000). For studies evaluating XPD Asp312Asn and XPD Lys751Gln, the pooled OR was 1.33 (95% CI, 0.92–1.91; P = 0.13) and 1.02 (95% CI, 0.72–1.45; P = 0.915), respectively. The results indicated that platinum-based chemotherapy sensitivity was significantly associated with polymorphism of ERCC1 C118T. However, XPD Asp312Asn and XPD Lys751Gln were not predictive makers for platinum-based chemotherapy in patients with advanced NSCLC.  相似文献

ERCC1 (excision repair cross-complementation group 1) and XPD (ERCC2, excision repair cross-complementation group 2) as genes have been known to be belonged to the nucleotide excision repair pathway and therefore related to DNA repair. Polymorphisms in these genes have been rarely evaluated in terms of predicting cancer patient survival. We investigated whether these polymorphisms have an effect on response to chemotherapy and survival in 109 patients with non-small-cell lung cancer treated with cisplatin combination chemotherapy. Polymorphisms of ERCC1 Asn118Asn (C --> T), XPD Lys751Gln (A --> C) and Asp312Asn (G --> A) were evaluated using a SNaPshot kit. As for chemotherapy response, treatment response did not show statistically significant differences between the wild genotypes and the variant genotypes for the ERCC1 and XPD gene. The median survival time of all patients was 376 days (95% CI, 291-488). As for survival rate according to the polymorphism of codon 118 in ERCC1, median survival time in patients showing C/C genotype was 486 days (95% CI, 333-x), which was significantly different from the 281 days (95% CI, 214-376) of patients with the variant genotype (T/T or C/T) (P = 0.0058). Using the Cox-proportional hazards model, the polymorphism of codon 118 in ERCC1, response to chemotherapy, weight loss and performance status effected overall survival significantly (P = 0.0001, 0.0001, 0.0028 and 0.0184, respectively). However, polymorphisms of codons 751 and 312 in the XPD gene did not affect patient survival (P = 0.4711 and 0.4542, respectively). Therefore, we suggest that the C/C genotype in codon 118 of ERCC1 is a surrogate marker for predicting better survival in non-small-cell lung cancer patients treated with cisplatin combination chemotherapy.  相似文献

BACKGROUND: Genetic polymorphisms involved in DNA repair and apoptosis are suspected to influence patient response to cancer treatment. To evaluate the effect of genetic variations on chemotherapy and/or radiotherapy, we genotyped four single nucleotide polymorphisms (SNPs) in ATM (A60G), ERCC1 (Asn118Asn), APE1 (Asn148Glu), and iASPP (A67T), and examined their associations with treatment response among patients with advanced non-small cell lung cancer (NSCLC). METHODS: Included in the study were 230 patients diagnosed with inoperable advanced NSCLC. Of these patients, 76 received platinum-based chemotherapy, 125 received chemotherapy plus radiation, and 29 received radiotherapy only. The SNPs were genotyped using the TaqMan methods. RESULTS: Among the patients who received chemotherapy only, ERCC1 (Asn118Asn) genotype was significantly associated with treatment response. Patients with either one or two T alleles (T/T+C/T) at Asn118Asn were more likely not to respond to platinum-based chemotherapy compared to those without the T allele (OR=4.10, 95% CI: 1.31-12.85). For patients who were treated with both chemotherapy and radiotherapy, treatment response seemed to differ substantially between patients with different genotypes of iASPP (A67T). Patients carrying an A allele (A/T+A/A) at A67T were more likely to respond to combined chemotherapy and radiotherapy compared to those not carrying the A allele (OR=0.25, 95% CI: 0.08-0.74). An association with treatment response was also suggested for the selected polymorphism in APE1, but no association was found for the ATM polymorphism. CONCLUSION: We found that SNPs in ERCC1 and iASPP were associated with response to chemotherapy or combined chemotherapy and radiotherapy in NSCLC patients. These findings support the notion that genetic variations related to DNA repair or apoptosis may affect the effect of chemotherapy or radiation on NSCLC.  相似文献

目的:探讨ERCC5基因启动子区单核苷酸多态性与中国汉族晚期大肠癌奥沙利铂疗效的相关性。方法:收集以奥沙利铂为主化疗的105例晚期大肠癌患者化疗开始前静脉血,提取基因组DNA,应用PCR-LDR方法检测ERCC5基因3个SNP位点-1415C〉T(rs2094258)-7、63A〉G(rs2016073)及-413C〉T(rs943245)多态性,分析不同基因型与疾病控制率、无进展生存期(PFS)的相关性。结果:携带ERCC5-763GG-、763AG和-763AA基因型的患者化疗疾病控制率分别为86.7%、69.2%和52.6%,其中携带-763GG基因型的患者疾病控制率显著高于携带-763AA基因型的患者,P=0.028。携带-763AA基因型的患者中位PFS(36/95例,6个月)低于携带-763AG基因型(48/95例,8个月)及携带-763GG基因型(11/95例,10个月)的患者,差异有统计学意义,χ^2=9.205,P=0.002。-1415C〉T多态性与化疗疾病控制率和PFS之间均无显著相关性。-413C〉T位点未发现遗传多态。结论:ERCC5启动子区-763A〉G单核苷酸多态性与晚期大肠癌奥沙利铂临床疗效相关。  相似文献

[目的]探讨DNA修复基因ERCC1 118和XRCC3 241多态性对于接受一线含铂化疗方案的非小细胞肺癌(NSCLC)患者的疗效预测作用。[方法]130例晚期接受含铂化疗的NSCLC患者进入研究.应用Taqman探针结合实时荧光PCR方法分析其外周血基因多态性.分析ERCC1 118和XRCC3 241多态性与疗效、疾病进展时间和总体生存期之间的关系。[结果]130例患者的总体有效率为20%.中位生存时间为15个月.ERCC1 118和XRCC3 241多态性与疗效无显著相关性。ERCC1 118基因型C/T或T/T患者的生存时间显著延长(P=0.003)。Cox多因素分析显示,ERCC1 118基因型C/T或T/T以及化疗有效患者的生存期显著延长。[结论]DNA修复基因ERCC1 118基因型C/T或T/T多态性可以延长NSCLC患者铂类治疗后的生存时间.  相似文献

To examine the association between genetic polymorphisms of XRCC1 Arg399Gln(G→A) and response to oxaliplatin-based chemotherapy in advanced colorectal cancer. XRCC1 genotypes of totally 99 patients(37 stage III, 62 stage IV)with advanced colorectal cancer treated with oxaliplatin-based chemotherapy were detected by TaqMan-MGB probe allelic discrimination method. And clinical response of 62 patients in stage IVafter 2 to 3 cycles of chemotherapy were evaluated. Also time to progress (TTP) of all patients were evaluated. Of the genotype frequencies in all patients, up to 52.53?% were G/G genotype, 9.09?% were A/A genotype, and 38.38?% were G/A genotype. The response rate (CR+PR) of 62 patients in stage IV was 61.29?% (19/31). Patients with G/G genotype showed enhanced respond to chemotherapy compared to those with G/A+A/A (x (2)?=?5.6, P?=?0.029; OR?=?3.845, 95?%CI?=?1.231?~?12.01, P?=?0.018). 目的 探讨切除修复交叉互补基因1(ERCC1)、X线修复交叉互补基因1(XRCC1)和谷胱甘肽巯基转移酶π1(GSTP1)多态性与中国晚期胃癌患者接受含奥沙利铂方案一线化疗后生存期的关系.方法 85例晚期胃癌患者接受奥沙利铂+5-氟尿嘧啶为基础的联合化疗方案化疗,并在化疗前抽取患者静脉血,提取基因组DNA,以实时荧光定量PCR法行多态性检测,比较不同基因型与患者生存期的关系.结果 85例患者中位至疾病进展时间为5.3个月,中位生存期为8.0个月.ERCC1-118 C/C、XRCC1-399 G/G和GSTP1-105 A/G+G/G基因型为优势基因型,携带3个、2个、1个、0个优势基因型患者的中位生存期分别为12.5、10.0,6.5和4.5个月,组间差异有统计学意义(χ2=35.54,P<0.01).结论 ERCC1-118、XRCC1-399和GSTP1-105基因多态性与中国晚期胃癌患者接受含奥沙利铂化疗方案一线化疗后的生存期相关,可预测患者的预后.  相似文献

目的 探讨短发夹RNA(shRNA)靶向沉默α2, 8-唾液酸转移酶4(ST8SIA4)表达对乳腺癌BT549细胞侵袭、迁移的影响。方法 采用实时荧光定量PCR(QPCR)和Western blotting检测乳腺癌BT549细胞和乳腺正常上皮MCF-10A细胞中ST8SIA4 mRNA和蛋白水平。采用LipofectamineTM2000向BT549细胞随机转染成功构建的靶向沉默ST8SIA4表达的shRNA载体片段(沉默组)或无义shRNA片段(对照组),转染48 h后采用QPCR检测ST8SIA4 mRNA水平,Western blotting检测ST8SIA4、磷酸化丝/苏氨酸蛋白激酶(p-Akt)、神经纤毛蛋白2(NRP2)和肿瘤坏死因子-α(TNF-α)的表达情况,Transwell实验和划痕实验分别检测细胞的侵袭和迁移能力。结果 与MCF-10A细胞相比,BT549细胞的ST8SIA4 mRNA和蛋白水平均升高(P<0.05)。转染48 h后,沉默组的ST8SIA4 mRNA和蛋白水平分别为0.19±0.02和0.13±0.02,均低于对照组的0.98±0.11和0.52±0.05(P<0.05);沉默组的侵袭细胞数、迁移率及p-Akt、NRP2和TNF-α蛋白相对表达量分别为(39.5±4.2)个、(16.9±1.3)%、0.22±0.03、0.31±0.04和0.27±0.06,均低于对照组的(91.3±8.5)个、(38.4±3.9)%、0.58±0.07、0.55±0.05和0.46±0.05,差异有统计学意义(P<0.05)。结论 ST8SIA4基因在乳腺癌BT549细胞中高表达,沉默其表达可抑制侵袭和迁移过程,可能与上调p-Akt、NRP2和TNF-α蛋白表达有关。  相似文献

Purpose

To evaluate the effect of excision repair cross-complementing group 1 (ERCC1) and X-ray cross-complementing group 1 (XRCC1) gene polymorphisms on treatment outcome in patients receiving oxaliplatin-based regimens for metastatic colorectal cancer.

Methods

Hundred and thirteen patients with a diagnosis of metastatic colorectal cancer were treated with oxaliplatin-based chemotherapy. ERCC1 codon 118C/T and XRCC1 codon 399A/G polymorphisms were tested by real-time polymerase chain reaction (RT-PCR) method in peripheral blood lymphocytes of these patients. Disease control rates and survivals were compared by types of genotypes.

Results

Analyses of the patterns of the polymorphism located at ERCC1 codon 118 showed that 55 (48.67%) patients were homozygous for C/C genotype, 15 (13.27%) were homozygous for the T/T genotype, and 43 (38.06%) were heterozygous for C/T genotype. Analyses of the polymorphism located at XRCC1 codon 399 showed that 61 (53.98%) patients were homozygous for A/A genotype, 13 (11.50%) were homozygous for the G/G genotype, and 39 (34.52%) were heterozygous for A/G genotype. After two cycles of chemotherapy, there was complete response (CR) in 1 patient, partial response (PR) in 24 patients, and stable disease (SD) in 56 patients. Altogether in 81 (71.68%) patients the disease was controlled after chemotherapy. Thirty-two (28.32%) patients showed disease progression. After adjusting for some clinical factors, both the ERCC1 polymorphism and the XRCC1 polymorphism lost their roles in predicting DCR (P = 0.662, P = 0.631) and MST (P = 0.692, P = 0.572). But the combination of ERCC1 and XRCC1 polymorphisms was significantly associated with DCR (P = 0.01) and MST (P = 0.000) independently.

Conclusions

This is the first study which showed that polymorphisms of ERCC1 and XRCC1, in combination not individually, were independent predictors for DCR and OS. This may contribute to the selection of patients who would benefit from oxaliplatin-based chemotherapy for metastatic colorectal cancer.  相似文献

Background: Although the predictive value of the excision repair cross-complementing group 1 (ERCC1)C118T polymorphism in clinical outcomes of patients with colorectal cancer (CRC) receiving oxaliplatin-basedchemotherapy has been evaluated in numerous published studies, the conclusions are conflicting. Therefore, weperformed the present meta-analysis to determine the precise role of the ERCC1 C118T polymorphism in thisclinical situation and help optimize individual chemotherapy. Materials and Methods: A multiple search strategywas used to identify eligible studies. Pooled odds ratios (ORs) and their 95% confidence intervals (CIs) wereused to estimate objective response and oxaliplatin-induced toxicity, with hazard ratios (HRs) with 95%CIs forprogression-free survival (PFS) and overall survival (OS). Results: A total of 22 studies including 2,846 CRCpatients were eligible in the analysis. Overall, no significant correlation was found between the ERCC1 C118Tpolymorphism and objective response to oxaliplatin-based chemotherapy, in all patients or in the Asian andCaucasian subgroups. However, the pooled analysis showed that the PFS and OS were significantly shorter inpatients who carried T/T or T/C genotypes of ERCC1 C118T as compared to the C/C genotype. On stratifiedanalysis by ethnicity, the ERCC1 118T allele was associated with a favorable prognosis in Caucasians (PFS,HR=0.58, 95%CI: 0.24-1.44; OS, HR=0.38, 95%CI: 0.22-0.64) but an unfavorable prognosis in Asians (PFS,HR=2.49, 95%CI: 1.87-3.33; OS, HR=2.63, 95%CI: 1.87-3.69) based on a dominant model. In addition, wefailed to find a statistically significant impact of ERCC1 C118T polymorphism on oxaliplatin-induced toxicity.Conclusions: The ERCC1 C118T polymorphism may have prognostic value in patients with CRC undergoingoxaliplatin-based chemotherapy.  相似文献

The excision repair cross-complementation group 1 (ERCC1) plays an essential role in DNA repair and has been linked to resistance to platinum-based anticancer drugs among advanced non-small cell lung cancer (NSCLC) patients. We systematically evaluate whether ERCC1 Asn118Asn and C8092A genetic variants are associated with treatment response of platinum chemotherapy. We preformed a meta-analysis using ten eligible cohort studies (including 11 datasets) with a total of 1,252 NSCLC patients to summarize the existing data on the association between the ERCC1 Asn118Asn and C8092A polymorphisms and response to platinum regiments. Odds ratio or hazard ratio with 95% confidence interval were calculated to estimate the correlation. We found that neither ERCC1 C8092A polymorphism nor Asn118Asn variant is associated with different response of platinum-based treatment among advanced NSCLC patients. Additionally, these two genetic variants are not related to treatment response in either Caucasian patients or Asian patients. Our meta-analysis indicates that the ERCC1 Asn118Asn and C8092A polymorphisms may not be good prognostic biomarkers for platinum-based chemotherapy in patients with stage III-IV NSCLC.  相似文献

Aim: SNPs of ERCC1 and ERCC2 genes have been found to be associated with response to platinum therapyin different clinical settings. In the current study, we investigated the relationship of SNPs in ERCC1 and ERCC2to cisplain response and survival in osteosarcoma patients. Methods: 267 consecutive patients diagnosed withosteosarcoma between January 2003 to January 2005 were followed up until the end of January 2010. ERCC1Asn118Asn, ERCC1 Gln504Lys, ERCC2 Asp312Asn and ERCC2 Lys751Gln polymorphisms were detectedbased upon the Sequenom MassARRAY platform. Results: For ERCC1 Asn118Asn, the variant genotype T/Twas strongly significantly associated with a higher event free survival when compared with the wild-type C/C,with an adjusted OR (95% CI) of 0.39 (0.14-0.95). ERCC2 751 A/A genotype showed increased event free survivalof osteosarcoma (HR=0.44; 95%CI=0.10-0.87). However, we did not find significant association of ERCC1Gln504Lys and ERCC2 Asp312Asn polymorphisms with prognosis of osteosarcoma. Conclusion: We first reportassociations of four SNPs, ERCC1 Asn118Asn, ERCC1 Gln504Lys, ERCC2 Asp312Asn and ERCC2 Lys751Gln,with risk of death from osteosarcoma in a Chinese population, indicating ERCC1 118T/T and ERCC2 A/A maybe used as surrogate markers for clinical outcome of osteosarcoma treatmetn with cisplain.  相似文献

We analyzed the influence of codon 118 C→T polymorphism of ERCC1 on its protein expression levels, clinicopathological features, and outcome of 168 Chinese patients with metastatic colorectal carcinoma that had been treated with first-line FOLFOX-4 chemotherapy. A high prevalence of C/C genotype was noted (47.6%, n  = 80; 168 patients in total). A marked increase of ERCC1 protein expression levels was also noted in patients with C/T or T/T genotypes (70% vs 20%; P  < 0.01), which was associated with significantly lower response to FOLFOX-4 (36.4% vs 57.5%; p = 0.01), and shorter progression-free (7 months vs 13 months; P  < 0.01) and overall (16 months vs 25 months; P  < 0.01) survival times. By multivariate analysis, this polymorphism was also identified as an independent prognostic factor ( P =  0.02). These data suggest that Asian populations have a significantly higher prevalence of the C/C genotype in ERCC1 codon 118, which could be a key determinant for good responses to oxaliplatin-based treatment and favorable outcomes. BackgroundGenetic polymorphisms of genes involved in DNA repair and glutathione metabolic pathways may affect patients' response to platinum-based chemotherapy. We retrospectively assessed whether single nucleotide polymorphisms (SNP) of DNA-repair genes ERCC1, XPD, XRCC1 and glutathione S-transferase genes GSTP1, GSTT1 and GSTM1 predict overall survival (OS), response and toxicity in 119 non–small-cell lung cancer (NSCLC) patients treated with platinum-based regimens as first- or second-line chemotherapy.Patients and MethodsPatients' genotypes were determined by PCR-RFLP and sequencing approaches.ResultsERCC1 (Asn118Asn) genotype was significantly associated with response to treatment. Patients with either one or two C alleles (C/C, C/T) at Asn118Asn were more likely to respond to platinum-based chemotherapy compared with those without the C allele (Odds ratio, 0.10; 95% CI, 0.013-0.828; P = .033, by binary logistic regression). There was a significant association between the ERCC1 C8092A polymorphism and OS (P = .009, by log-rank test), with median survival times of 9.8 (C/C) and 14.1 (C/A or A/A) months, respectively, suggesting that any copies of the A allele were associated with an improved outcome. Cox's multivariate analysis suggested that the joint effect of ERCC1 polymorphic variants (C8092A and N118N) (0 vs. 2, hazard ratio 2.5; 95% CI, 1.26–4.96; P = .009) as well as the XRCC1 N399Q polymorphism (AA vs. GA/GG, hazard ratio 3.1; 95% CI, 1.4–6.8; P = .005) were independent prognostic factors for OS in advanced NSCLC patients treated with platinum-based chemotherapy.ConclusionThese findings support the notion that assessment of genetic variations of ERCC1 and XRCC1 could facilitate therapeutic decisions for individualized therapy in advanced NSCLC.  相似文献

目的探讨DNA修复基因ERCC1 118C/T和XRCC1 Arg194Trp多态性与进展期非小细胞肺癌(non-small cell lung cancer,NSCLC)患者铂类药物化疗敏感性的关系。方法采用PCR-RFLP技术检测149例经病理确诊的接受含铂两药方案化疗的NSCLC患者外周血ERCC1 118和XRCC1 194位点的基因型,并分析其与化疗疗效的关系。结果经2个周期化疗后,149例进展期NSCLC患者化疗有效率为32.9%。携带至少1个ERCC1 118T突变基因患者的化疗有效率至少是C/C野生型基因携带者的3倍(49.1%vs 23.4%,OR=3.156,95%CI:1.548~6.334,P=0.001)。携带至少1个XRCC1 194Trp突变基因患者的化疗有效率显著高于Arg/Arg基因型携带者(41.3%vs 23.2%,OR=2.326,95%CI:1.138~4.753,P=0.019)。ERCC1 118C/T和XRCC1 Arg194Trp 2个基因多态之间存在一定的联合作用,携带至少1个ERCC1 118 T突变基因同时又携带至少1个XRCC1 194Trp突变基因型者的化疗有效率明显高于同时携带ERCC1 118C/C和XRCC1 194Arg/Arg野生型基因者(66.7%vs 17.1%,OR=9.714,95%CI:3.104~30.406,P<0.001)。结论与单基因检测比较,2个基因的联合检测在预测铂类药物化疗敏感性中的价值更大。ERCC1 118和XRCC1 194基因多态联合与NSCLC患者对铂类药物化疗敏感性相关,ERCC1和XRCC1基因型的联合检测有可能成为预测铂类药物化疗敏感性的指标。  相似文献

目的:探讨DNA损伤修复基因ERCC1 Asn118Asn和XRCC1 Arg399Gln多态性与接受奥沙利铂一线化疗的中国晚期大肠癌患者生存期的关系.方法:99例晚期大肠癌患者化疗前抽取静脉血并提取DNA,以real-time PCR法对ERCC1、XRCC1基因进行SNP分型.患者接受奥沙利铂为主的化疗方案化疗,比较不同基因型与患者生存期的关系.结果:ERCC1 Asn118Asn基因位点在所研究的中国大肠癌患者中的突变频率为:C/C 50.51%、C/T 41.41%、T/T8.08%;XRCC1 Arg399Gln基因突变频率为:G/G 52.53%、G/A 38.38%、A/A 9.09%.99例晚期大肠癌患者中位TTP为7个月.ERCC1C/C基因型患者中位TTP 10个月,C/T T/T型患者中位TTP 5个月,二者有显著统计学差异(P<0.01);XRCC1G/G基因型中位TTP 10个月,G/A A/A基因型中位TTP 5个月,二者比较差异有显著性(P<0.01).两个基因联合多态性分析发现,同时携带ERCC1 C/C和XRCC1 G/G基因型、ERCC1 C/C和XRCC1 G/A A/A基因型、ERCC1 C/T T/T和XRCC1 G/G基因型、以及ERCC1 C/T T/T和XRCC1 G/A A/A基因型的患者中位TTP分别为11个月、6个月、5个月和5个月,4组比较差异有显著性(P<0.01).结论:ERCC1 Asn118Asn、XRCC1 Arg399Gln基因多态性与中国晚期大肠癌患者接受奥沙利铂一线化疗后的生存期有关.  相似文献

目的:探讨DNA修复基因ERCC1 C118T和XPD Lys751Gln单核苷酸多态性与非小细胞肺癌(non-small-cell lung carcinoma,NSCLC)患者对含铂方案化疗敏感性的关系。方法:选择经病理确诊为NSCLC的患者73例,在实施化疗前采取静脉血,提取DNA,行DNA测序、用PCR-RFLP方法检测ERCC1 C118T和XPD Lys751Gln基因型。所有患者均经含铂方案化疗,观察疗效,统计临床获益率,分析NSCLC患者ERCC1和XPD单核苷酸多态性与含铂方案化疗敏感性的关系。结果:ERCC1 C118TC/C、C/T和T/T基因型临床获益率分别为94.9%、71.4%和83.8%。基因型C/C临床获益率明显高于C/T、T/T(P〈0.05)。XPD Lys751Gln基因型Lys/Lys、Lys/Gln临床获益率分别为80.3%和75.0%。基因型Lys/Lys与Lys/Gln临床获益率间的差异无统计学意义(P=0.702)。未检测到XPD Gln/Gln基因型。ERCC1 C118T、XPD Lys751Gln多态之间在对含铂方案的化疗敏感性方面无协同作用(P=0.134和P=0.236)。结论:DNA修复基因ERCC1 C118T单核苷酸多态性与NSCLC含铂方案化疗的敏感性有关,可作为预测NSCLC患者铂类药物化疗敏感性的参考指标之一。  相似文献
