以某焦化厂焦炉车间153名生产工人和无职业性多环芳烃接触者50人为研究对象。记录其离开工作岗位的时间,收集晨尿。用高效液相色谱法测定尿中1-羟基芘水平。采用方差分析和协方差分析的统计学方法研究不同焦炉位置作业及脱离职业暴露后不同时间对尿1-羟基芘水平的影响。焦炉工尿1-羟基芘水平呈现炉顶炉侧炉底对照趋势;脱离职业暴露后,焦炉工尿1-羟基芘水平呈8 h以内组8~16 h组16 h以上组趋势。说明焦炉的作业位置以及脱离职业暴露后的时间均可影响焦炉工尿1-羟基芘水平。  相似文献

炼焦职业暴露工人与对照人群尿中1-羟基芘水平比较   总被引:5,自引:0,他引:5  
目的比较焦炉工和非职业暴露人群尿中1-羟基芘的不同水平,并探讨可能的各种影响因素.方法 2002年上半年,以某焦化厂265名炼焦作业工人和226名非职业接触对照人群为调查对象,职业接触组收集工作班末尿,对照组收集晨尿.根据环境中多环芳烃监测和调查表评价外暴露因素和可能的混杂因素,采用高效液相色谱法测定尿中1-羟基芘含量,比较两组人群尿中1-羟基芘的不同水平,并分析外暴露等级、吸烟、饮酒、年龄和体重指数的可能影响.结果尿中1-羟基芘浓度呈炉顶工〉炉侧工〉炉底工〉对照的趋势,几何均数依次为13.49、6.56、1.38和0.35 μmol/mol肌酐,差异有统计学意义;各组人群尿中1-羟基芘浓度超标率分别依次为94.81%、84.73%、35.09%和0.88%,差异有统计学意义.对照人群中≥20支/d吸烟组的尿中1-羟基芘水平明显高于不吸烟组,修正几何均数分别为0.47和0.31 μmol/mol肌酐,差异有统计学意义;对照人群中饮酒组的尿中1-羟基芘水平明显低于不饮酒组,修正几何均数分别为0.33和0.47 μmol/mol肌酐,差异有统计学意义.结论焦炉工尿中1-羟基芘水平较高,以炉侧工和炉顶工最为明显,非职业暴露人群尿中1-羟基芘本底水平与吸烟和饮酒因素有关.  相似文献

目的 研究焦炉工人外周血淋巴细胞热休克蛋白72(HSP72)表达特征及其影响因素.方法 选取某焦化厂焦炉工人267人和对照30人,蛋白印迹(Western blot)法检测外周血淋巴细胞HSP 72的表达,高效液相色谱法测定作业环境空气中苯并(a)芘[B(a)P]浓度.收集研究对象个人信息.结果 炉顶工、炉侧工外周血淋巴细胞HSP 72表达水平(G±SG)(1.20±0.47,1.30±0.37)显著高于对照组(0.85±0.34),炉侧工显著高于炉底工(1.01±0.35),差异有统计学意义(P<0.05).不同作业环境[B(a)P]浓度与相应作业区工人外周血淋巴细胞HSP72表达水平呈二项式分布(R2=0.9999).Logistic分析显示,与对照组相比,焦炉逸散物暴露引起淋巴细胞HSP 72表达阳性的调整OR为1.31,其中炉顶工、炉侧工调整OR分别为1.49和2.69.多元线性回归分析显示,在低暴露水平下(炉底工、对照组),吸烟指数和饮酒是HSP 72表达水平升高的抑制因素(β分别为-0.0001和-0.05,P<0.05).结论 焦炉逸散物可诱导外周血淋巴细胞HSP72表达升高,但高暴露剂量为焦炉逸散物对HSP 72表达有抑制作用.低暴露水平下吸烟和饮酒对HSP 72表达有抑制作用.  相似文献

焦炉作业工人神经行为功能改变的特征分析   总被引:5,自引:0,他引:5  
目的 通过苯并(a)芘[B(a)P]职业接触人群调查,探讨多环芳烃所致神经行为功能改变.方法 选择某钢铁公司焦化厂健康成年男性工人200名作为接触组,以该公司的原材料处和自动化处一般情况相近的88名工人作为对照组.高效液相色谱法测定作业环境空气中B(a)P,收集调查对象的班后尿,用高效液相色谱法检测尿中1-羟基芘的水平;用WHO推荐的神经行为核心测试组合(NCTB)检测研究对象的神经行为功能.结果 作业环境空气中炉底、炉侧、炉顶的B(a)P浓度分别为0.0195、0.186、1.624 μg/m3,炉侧、炉顶的B(a)P浓度超过职业卫生标准(0.15 μg/m3)0.24倍和9.83倍.与对照组相比,焦炉工尿1-羟基芘的水平[(3.42±0.98)μmol/mol肌酐]高于对照组[(2.75±1.09)μmol/mol肌酐](t=-5.162,P=0.000),接触组与对照组在年龄、工龄、吸烟、饮酒及吃烧烤食物情况间均衡,文化水平不均衡,以文化水平为协变量进行协方差分析,焦炉工在总数字跨度、数字跨度顺序、2次打点正确数得分低于对照组,2次打点总数得分高于对照组,差异有统计学意义,按照尿1-羟基芘浓度对数值的30和70百分位数将研究对象分为3组(<3.10 μmol/mol肌酐,3.10 μmoL/mol肌酐~,>3.87 μmoL/mol肌酐),总数字跨度、数字跨度顺序、数字跨度倒序、习惯用手、数字译码、视觉记忆得分显示出随尿1-羟基芘浓度增高而降低的趋势;多元线性回归和偏相关分析表明,尿1-羟基芘浓度影响总数字跨度、数字跨度顺序、数字跨度倒序、数字译码和视觉记忆得分,而且与上述指标存在负相关,偏相关系数分别为-0.309、-0.284、-0.209、-0.158、-0.144.结论 多环芳烃主要引起接触人群感知、记忆功能改变.  相似文献

目的 探讨焦炉工外暴露等级与尿中 1-羟基芘浓度之间的关系。方法 以某焦化厂 30 5名生产工人和 30名非接触者为调查对象 ,统一收集工作周末班后 6h尿 ,并收集个人信息。根据工人的工作岗位将调查对象分为炉顶、炉侧、炉底和对照 4个外暴露等级。采用碱水解 -高效液相色谱法测定尿中1-羟基芘浓度。使用协方差分析和多元线性回归分析暴露等级、吸烟和饮酒等情况对尿中 1-羟基芘浓度的影响。结果 尿中 1-羟基芘的浓度呈炉顶 >炉侧 >炉底 >对照的趋势 ,与外暴露等级之间呈显著相关(Spearman相关系数为 0 5 35 ,P <0 0 1) ;在同样外暴露条件下 ,吸烟量显著影响尿中 1-羟基芘的水平(P <0 0 5 ) ;未发现饮酒对尿中 1-羟基芘水平的显著影响。结论 焦炉工尿中 1-羟基芘水平与外暴露等级之间存在良好的剂量 -效应关系 ,可反映多环芳烃暴露个体的内剂量水平  相似文献

目的 研究CYPIAl多态性与焦炉工尿中1-羟基芘水平的关系.方法 按照苯并[a]芘(B[a]P)接触水平将223名焦炉工分为炉顶组(76名)、炉侧组(82名)和炉底组(65名),以119名无职业性接触多环芳烃的工人为对照组,采用聚合酶链反应-限制性片段长度多态性分析(PCR-RFLP)CYPIAl基因Mspl位点多态性、等位基凶特异性扩增方法 检测CYPlAl基因1462V位点的多态性.结果 炉顶组、炉侧组和炉底组焦炉工尿中1-羟基芘水平分别为(3.77+0.64)、(3.57+0.49)、(3.26+0.80)μmol/mol Cr,明显高于对照组,差异有统计学意义(PO.05).炉侧组和炉顶组CYPIAl 1462V位点的各基因型尿中1-羟基芘水平均为Ⅱe/Ⅱe<Ⅱe/Val相似文献

目的 探讨焦炉作业工人尿中1-羟基芘水平及其临床意义.方法 将煤化工厂147名焦炉作业工人分为焦油组(接触焦油作业工人)60人和焦炉烟尘组(接触焦炉烟尘作业工人)87人,另选择79名非焦炉作业工人为对照组,采用高效液相色谱紫外检测方法对作业工人尿中1-羟基芘含量进行分析.结果 焦油组和焦炉烟尘组尿中1-羟基芘水平分别为(2.20±0.49)和(1.92±0.59)μg/L,明显高于对照组[(0.59±0.40)μg/L],差异有统计学意义(P<0.01);焦油组尿中1-羟基芘水平明显高于焦炉烟尘组,差异有统计学意义(P<0.01).焦油组与焦油烟尘组作业工人尿中1-羟基芘水平不受工龄、性别和吸烟等因素的影响.结论 焦炉作业工人尿中1-羟基芘水平明显增高,以接触者尿中1-羟基芘水平作为评价人体接触多环芳烃指标,可以较好地反映近期接触水平.  相似文献

目的探讨焦炉逸散物接触对煤焦作业工人免疫调节因子的影响。方法采用职业卫生现场调查与实验室研究相结合的方法,测定作业环境中苯并(a)芘、苯溶物、焦炉逸散物等有害物质的质量浓度;选择91名焦炉作业工人作为接触组,80名无任何职业有害物质接触史的体检者作为对照组,采集空腹静脉血,用试剂盒检测血清中重要细胞免疫调节因子白细胞介素(IL)-2、IL-10、肿瘤坏死因子-α(TNF-α)的表达水平。结果焦炉作业工人工作场所存在职业有害因素,且炉顶有害物质质量浓度高于炉侧,存在职业有害因素;与对照组相比,焦炉作业工人血清IL-10、TNF-α水平均升高,IL-2水平则降低,差异有统计学意义(P<0.05)。在接触组中,炉顶工组、炉底工组IL-10、TNF-α高于炉侧工组,差异有统计学意义(P<0.05),IL-2水平差异无统计学意义(P>0.05)。焦炉作业工人血清IL-10与TNF-α水平及工作区域有害物质质量浓度之间呈正相关,IL-2水平及工作区域有害物质质量浓度之间呈负相关。结论本研究提示焦炉逸散物的接触可以引起体内免疫系统分泌的相应免疫调节因子IL-10、TNF-α水平升高,IL-2水平降低。IL-10、TNF-α...  相似文献

目的 通过对比研究同一批焦炉工人焦炉逸散物暴露5年前后肺通气功能的损伤情况,进一步探讨焦炉逸散物与焦炉工人肺通气功能损伤的关系.方法 选取山西某焦化厂焦炉作业工人165人(暴露组)和辅助工人52人(对照组)为研究对象,使用统一调查表收集工人个人信息.定点定期采集工作场所中空气样本,高压液相色谱仪测定空气样本中苯并(a)芘浓度.分别于2009和2013年使用便携式肺功能仪测定研究对象肺通气功能,并进行对比分析.结果 2009至2013年同一区域B(a)P浓度的差异无计学意意义(P>0.05).与2009年比较,2013年对照组工人各肺功能测定值及异常率差异无统计学意义(P>0.05),而暴露组工人用力肺活量(FVC%)、第1秒用力呼气容积(FEV10%)、最大通气量(MVV%)、肺活量(VC%)、25%肺活量时呼气流速(FEF.%)均显著降低,FVC%、FEV10%和VC%异常率显著升高,差异有统计学意义(P<0.05).新出现肺功能异常的工人主要分布在炉顶和炉侧,炉顶组FVC%、FEV1.0%、VC%新增异常人数(率)明显高于对照组和炉底组,炉侧组明显高于对照组,差异有统计学意义(P<0.05).多因素logistic回归分析结果显示,继续暴露5年后,炉顶作业工人FVC%、FEV1.0%和VC%新出现肺功能异常的调整OR分别为7.939、5.966和4.956;接焦工龄为暴露组工人FVC%、FEV1.0%、MVV%、VC%异常的危险因素.接焦工龄与炉顶作业存在正向交互作用,差异有统计学意义(P<0.05).结论 焦炉工人肺功能损伤与焦炉逸散物暴露有关,焦炉逸散物暴露水平和暴露时间是焦炉工人肺功能损伤的主要影响因素,二者存在正交互作崩.  相似文献

[目的]探讨焦炉逸散物对焦炉作业工人脂质过氧化损伤效应指标血清MDA和酶性抗氧化指标血清SOD活性的影响,为进一步探讨焦炉逸散物对作业工人的危害机制以及为焦炉作业工人的健康监护提供基础资料。[方法]以焦炉作业工人106名为接触组,按工种分为3组,以不接触有害物质的某医院体检者72名作为对照组,对血清MDA含量及血清SOD活性进行测定,并测定作业环境空气中的苯溶物浓度。[结果]炉顶工、炉侧工和炉底工的血清MDA含量分别为:(10.68±5.25)mmol.L-1、(6.61±4.87)mmol.L-1和(6.79±4.45)mmol.L-1,三者均高于对照组(5.61±3.33)mmol.L-1,其中仅炉顶工与对照组比较,差异有统计学意义(P﹤0.05)。炉顶工、炉侧工和炉底工的血清SOD活性分别为:(60.48±15.35)U/ml、(59.67±16.35)U/ml和(76.44±13.46)U/ml,三者均低于对照组(77.78±13.12)U/ml,炉顶工和炉侧工分别与对照组和炉底工比较,差异均具有统计学意义(P﹤0.01)。[结论]接触焦炉逸散物的焦炉工体内脂质过氧化和抗氧化代谢水平会受到影响,测定血清中MDA含量和SOD活性对焦炉工的职业健康监护具有重要意义。  相似文献

目的分析职业暴露和吸烟对水泥作业工人肺通气功能和胸部X线影像的影响。方法对某市参加职业病体检的工人进行不同层次分组作回顾性研究。结果水泥厂工人不接尘组与接尘组比较,接尘组FVC%、FEV1%、FEV1%(%)低于标准值例数及胸片0^+例数明显高于不接尘组,差异有非常显著性(P〈0.01);而工期尘肺两者无统计学意义(P〉0.05)。水泥厂接尘工人吸烟组与不吸烟组肺通气功能与胸片的比较,吸烟组FVC%低于标准值例数明显高于不吸烟组,差异有非常显著性(P〈0.01);吸烟组FEV1%低于标准值例数高于不吸烟组,差异有显著性(P〈0.05),FEV1%(%)低于标准值例数、胸片0^+、Ⅰ期尘肺例数无统计学意义(P〉0.05)。结论水泥企业的职业暴露和吸烟对工人的肺通气功能影响是明显的,对X线胸部影像改变也是肯定的,肺通气功能变化比X线影像变化敏感。  相似文献

The purpose of the study was to follow up an earlier observation of pulmonary function among workers employed in firebrick-manufacturing factories. A 2-year follow-up study of pulmonary function among 442 workers in 30 firebrick-manufacturing factories was designed. Excluding 79 workers with a history of other occupational dust exposure, changes in pulmonary function of 291 firebrick workers were compared with pulmonary function in 72 control subjects over a period of 2 years. Baseline pulmonary function values (i.e., forced expiratory volume in 1 second [FEV1]/forced vital capacity [FVC] and forced expiratory flow after 50% of vital capacity has been expelled [FEF50%] in smoking firebrick workers, and FEV1/FVC and FEF75% in nonsmoking firebrick workers) were significantly lower than those in the comparison group. The statistical method for repeated measurements was used for comparison of the difference between follow-up and baseline lung function. There was no significant difference in FVC and FEV1 changes between firebrick workers and those in the comparison group during the 2-year follow-up period. The decreases in FEV1/FVC, peak expiratory flow rate, maximal midexpiratory flow, and FEF50% in the firebrick workers were significantly greater than in the comparison group, after adjustment for smoking status. The FEV1, maximal midexpiratory flow, FEF50%, and FEF75% also showed a dose-response relationship with job titles. The decrement of pulmonary function in the 2-year follow-up period was the worst in burning work, followed by crushing and molding. The results show that workers in firebrick-manufacturing factories with exposure to silica-containing dusts may contract obstructive pulmonary function defects.  相似文献

PURPOSE: This retrospective longitudinal study investigated the association between the Q192R polymorphism of the high-density lipoprotein-associated multifunctional antioxidant enzyme, paraoxonase-1 (PON1), and lung function decline, while taking into account smoking history. METHODS: The demographic, occupational, and respiratory symptom information and lung function variables were obtained from 216 male Saskatchewan grain workers. RESULTS: An interaction between the PON1 genotypes and smoking status was observed. Current smokers with the 192R allele had a lower forced expiratory volume in the first second (FEV(1)) and FEV(1) per forced vital capacity (FVC). The annual decline rate of FEV(1)/FVC in current smokers was greater among 192R allele carriers than noncarriers (0.58+/-0.05 vs. 0.35+/-0.04 %/yr, p<0.0001). A similar result was observed with FEV(1) (40.9+/-6.4 vs. -33.0+/-7.0 mL/yr, p=0.10). The annual decline rate of FVC was not influenced by the genotypes. CONCLUSIONS: These results strengthened the previous findings of our cross-sectional study, suggesting that the 192R allele may be a novel genetic risk factor for airway injury among current smokers.  相似文献

OBJECTIVES: In workers, sensitivity and occupational asthma are known to develop in reaction to the proteolytic enzymes used in the manufacture of detergents. This study was conducted to find out how this sensitivity to the proteolytic enzymes affects respiratory function, excluding occupational asthma. METHODS: Workers in the detergent industry ( n = 65) were divided into four groups according to their prick-test results, as follows: those positive for at least one enzyme (Lipolase, Savinase) (group I, n = 15); enzyme-negative cases (group II, n = 50); and from among these, enzyme-positive non-smoking cases (group Ia, n = 11); enzyme-negative non-smoking cases (group IIa, n = 32). Respiratory function tests for the last 10 years from the archives were assessed. For statistic analysis, average values were determined and the standard deviation calculated. For comparison of the groups, the Mann-Whitney U and Fischer Exact chi-square tests were used. RESULTS: The ratio of smokers, the cigarette burden, average age and the period of work were found to be similar between workers of groups I and II, and group Ia and group IIa ( P > 0.05). The average annual fall in the forced vital capacity (FVC) and the forced expiratory volume in the first second (FEV(1)) for the 10-year period was, respectively, found to be: group I, 64.1 +/- 7.8 ml and 58.7 +/- 9.6 ml; group II, 38.7 +/- 6.4 ml and 43.7 +/- 8.2 ml; group Ia, 60.7 +/- 8.1 ml and 56.1 +/- 10.2 ml; group IIa, 37.4 +/- 6.8 ml and 42.9 +/- 8.1 ml. The decrease in FVC and FEV(1) in group I compared with group II, and in group Ia compared with group IIa was statistically significant ( P < 0.001). CONCLUSIONS: In this study, the observation that the FVC and FEV(1) of the workers were much lower than expected, especially among those sensitive to the enzymes, independently of occupational asthma and cigarette smoking, led to the conclusion that it could be due to sensitivity to the enzymes.  相似文献

Lung function was studied in 354 coke oven plant workers in the Lorraine collieries (Houillères du Bassin de Lorraine, France) who retired between 1963 and 1982 and were still alive on 1 January 1988. A spirometric examination was performed on 68.4% of them in the occupational health service. Occupational exposure to respiratory hazards throughout their career was retraced for each subject. No adverse effect of occupational exposure on ventilatory function was found. Ventilatory function was, however negatively linked with smoking and with the presence of a respiratory symptom or discrete abnormalities visible on pulmonary x ray films. The functional values were mostly slightly lower than predicted values and the most reduced index was the mean expiratory flow, FEF25-75%. The decrease in forced expiratory volume in one second (FEV1) was often parallel to that in forced vital capacity (FVC), but it was more pronounced for subjects who had worked underground, for smokers of more than 30 pack-years, and for subjects having a respiratory symptom. Pulmonary function indices were probably overestimated because of the exclusion of deceased subjects and the bias of the participants.  相似文献

The objectives of this study were to determine the relationship between exposure levels and respiratory abnormalities, to measure FVC and FEV1(1) changes per year based on work duties and to investigate the prevalence of and factors related to pneumoconiosis. A total of 583 male workers from 50 iron foundries in central Taiwan were investigated. First, workers' respiratory symptoms were categorized using a modified American Thoracic Society (ATS) questionnaire and then were verified by physician's examination. Next, pulmonary function tests were performed including: forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and forced expiratory flow rate. A chest radiograph was used to diagnose pneumoconiosis according to ILO criteria. Furnace workers were found to have the highest prevalence of chronic phlegm, thoracic disorders and chronic bronchitis. In general, smokers had a higher prevalence of respiratory symptoms as compared with non-smokers. Pulmonary function abnormalities and pneumoconiosis were closely linked to smoking and work duration. After adjusting for age, height and smoking there was a significant decrease based on work duration in FVC and FEV1 for furnace and moulding workers compared with after-processing and administrative workers. The overall prevalence of pneumoconiosis was 8.8%, highest among furnace (16.3%) and after-processing workers (11.4%) and lowest among administrative workers (2.5%). Using multiple logistic regression, the risk of developing pneumoconiosis (as compared with the administrative workers) for furnace workers was highest (8.98 times greater risk), followed by after-processing workers (6.77 times greater risk) and moulding workers (5.41 times greater risk). Prolonged exposure to free silica, and smoking habits, can result in respiratory abnormalities among foundry workers.  相似文献

目的了解陶瓷作业工人肺功能损害的情况,为预防尘肺病发生提供依据。方法选择某陶瓷厂200名接尘工人,平均年龄为(32.31±8.25)岁,另选取非接尘工人50名为对照组,平均年龄为(34.88±9.75)岁,采用意大利的spirolabII型的肺功能仪,测试的项目包括肺活量(VC)、用力呼吸肺活量(FVC)、第1s用力呼气量(FEV1)、一秒率(FEV1%)、最大呼气中段流速(FEF25-75)、最大呼气25%、50%、75%肺活量时流量(FEF25%、FEF50%、FEF75%);应用SPSS13.0统计软件统计分析。结果接尘组的肺功能指标FEV1、FEV1%、FEF25-75、FEF50%、FEF75%明显低于对照组,差异有统计学意义(P〈0.05);接尘工龄在10年~、15年~、20年~组的工人肺功能指标中VC、FVC、FEF25-75、FEF50%、FEF75%的测试均值明显低于接尘工龄〈5年组,差异有统计学意义(P〈0.05)。结论陶瓷粉尘对作业工人的肺功能有损害作用,且随着接尘工龄的增加肺功能受损害程度增加。  相似文献
