Increased symmetrical fluorine-18 fluorodeoxyglucose (FDG) uptake in the cervical and thoracic spine region is well known and has been attributed to muscular uptake. The purpose of this study was to re-evaluate this FDG uptake pattern by means of co-registered positron emission tomography (PET) and computed tomography (CT) imaging, which allowed exact localisation of this uptake. Between April and November 2001, 638 consecutive patients referred for PET/CT were imaged on an in-line PET/CT system (GEMS). This system combines an advanced GE PET scanner and a multirow-detector computer tomograph (Lightspeed, GEMS). The examination included PET with FDG and one CT acquisition with 80 mA. For CT, the following parameters were used: 140 kV, 80 mA, reconstructed slice thickness 5 mm, scan length 867 mm, AT 22.5 s. CT data were used for attenuation correction as well as image co-registration. Image analysis was performed on an Entegra work-station (ELGEMS). All patients with symmetrical uptake within the neck, thorax and shoulder regions were selected and the exact localisation of uptake determined (muscle, bone, fatty tissue or articulation). In 17 of the 638 patients (2.5%), increased, symmetrical FDG uptake in the shoulder region in a typical pattern was found. If extensive, this pattern included FDG activity comparable to brain activity in the lower cervical spine, the shoulder region and the upper thoracic spine in the costovertebral region. A less extensive pattern only involved intermediate FDG uptake in the lower cervical spine and shoulder region or in the shoulder region alone. In seven female patients (average 32.3 years), the extensive uptake pattern was seen. The average body mass index (BMI) was 19.0 (range 16.8-23.4). In the other ten patients (two male, eight female, average age 37.1 years), the average BMI was 22.7 (18.7-27.7). In all patients, the soft tissue uptake was clearly localised within the fatty tissue of the shoulders as demonstrated by PET/CT co-registration. The uptake in the region of the thoracic spine was localised in the region of the costovertebral joints. Symmetrical FDG uptake in the shoulder, neck and thoracic spine region is probably related to uptake in adipose tissue, especially in underweight patients. Hypothetically, this FDG uptake could represent activated brown adipose tissue during increased sympathetic nerve system (SNS) activity due to cold stress.  相似文献

Objective To evaluate the efficacy of F-18-fluoro-2-deoxy-D-glucose positron emission tomography/computed tomography (FDG PET/CT) in differentiating malignant from benign pathologic fractures. Materials and methods F-18 FDG PET/CT was performed on 34 patients with pathologic fractures between May 2004 and June 2007. Fractures were located in tubular bones (26), in the pelvis (six), in the spine (one) and in a rib (one). The FDG uptake pattern at the fracture site was described, whether FDG uptake occurred in the marrow or cortex and soft tissue. Maximum standardized uptake values (SUVmax, the largest value at the region of interest) were measured at the fracture site, including cortical bone, bone marrow and soft tissue. As a reference standard, biopsy was used for 12 patients and clinical follow-up for 22 patients. Sensitivity, specificity and diagnostic accuracy of PET/CT were calculated. Results There were 19 malignant and 15 benign fractures. In the malignant fractures, PET/CT demonstrated high (mean SUVmax 12.0, range 4.3 to 45.7) F-18 FDG uptake in bone marrow in most cases (17 of 19). In benign fractures, there was low FDG uptake (mean SUVmax 2.9, range 0.6 to 5.5) within cortical bone or adjacent soft tissue around the fracture, rarely in the marrow. There were significant differences in the pattern of intramedullary FDG uptake (P < 0.001) and in the mean SUVmax (P < 0.01) between malignant and benign fractures. The sensitivity, specificity and diagnostic accuracy of F-18 FDG PET/CT were 89.5%, 86.7% and 88.2%, respectively, with a cut-off SUVmax set at 4.7. The time interval between fracture and PET/CT did not significantly influence FDG uptake at the fracture site. Conclusion F-18 FDG PET/CT reliably differentiated between malignant and benign fractures based on the SUVmax and based on medullary uptake, which was characteristic for malignant fractures. Purpose It has recently been suggested that FDG accumulation in the brown adipose tissue varies as a function of age, sex and outdoor temperature. The aim of this study was to assess changes in FDG uptake in brown fat in patients based on serial PET/CT scans and to compare our results with previous findings. Methods Early response to neoadjuvant chemotherapy in 33 female breast cancer patients was assessed by FDG PET. Five PET/CT scans were performed for each patient. PET/CT images were analysed retrospectively. PET scans were considered positive when diffuse, symmetrical, abnormal “USA” (uptake in supraclavicular area) fat was detected. Results A total of 163 PET images were analysed. Seventy-four PET scans (45%) revealed abnormal FDG uptake in the supraclavicular area. These foci were present on uncorrected and attenuation-corrected images. FDG uptake was identical on all five scans in only five patients. No significant relationship was found between abnormal FDG uptake and outdoor temperature, age or time interval between chemotherapy and PET. Abnormal FDG uptake in the neck seemed to predominantly occur in patients with a low body mass index (p<0.05). Most significant changes in the PET/CT scan results were observed during chemotherapy with docetaxel (p<0.05). When observed, bilateral uptake in the neck was more intense than background uptake (p<0.00001). Conclusion This study shows that FDG uptake in the neck varies as a function of time, that it is unrelated to age or outdoor temperature, and that bilateral uptake is generally intense.  相似文献

A patient with a history of colon carcinoma and computed tomography (CT) evidence suggestive of a liver metastasis underwent an F-18 FDG PET scan. The PET scan confirmed a metastasis in the left lobe of the liver. The study also demonstrated bilateral symmetric uptake involving the inferior subscapular regions. Correlation with the previous CT showed this to correspond to bilateral elastofibroma dorsi, which had been prospectively reported on the CT. Elastofibroma dorsi is a rare benign mass consisting of fibroelastic tissue and fat. It has been previously described on PET, CT, ultrasound, and magnetic resonance imaging (MRI). On CT, the masses are of soft tissue density, similar to adjacent musculature, and may contain linear areas of low density secondary to fat. On MRI, elastofibroma dorsi has been shown to have variable enhancement, which may reflect increased vascularity of the lesion. In this case, the masses demonstrated low-grade diffuse F-18 FDG uptake.  相似文献

OBJECTIVE: Hibernoma is a benign tumor of brown fat that has imaging features similar to those of malignant fat-containing soft-tissue tumors. Hibernoma is metabolically active on (18)F-FDG PET/CT, and its presence can lead to false-positive interpretations. We present three cases in which fatty lesions with increased radiotracer uptake identified on FDG PET/CT turned out to be hibernomas. The standardized uptake values of the lesions were similar to those reported in the literature for liposarcoma. However, all three patients had variable standardized uptake values over time. CONCLUSION: Variation in standardized uptake values over time is an imaging characteristic that may be helpful for differentiating hibernoma and malignant fatty tumor.  相似文献

We present a unique case of a patient with a clinical history of distal esophageal carcinoma and CT appearance of lipomatous hypertrophy of the interatrial septum who displayed fluorine-18-fluorodeoxyglucose (FDG) uptake in the interatrial septal fat on only the second of 4 serial PET/CT studies. As PET/CT scans are used for the initial staging of esophageal carcinoma, one should not mistake more benign causes of mediastinal uptake for metastasis when observing an intermittent pattern of uptake on a PET/CT scan.  相似文献

Purpose

There is evidence that the link between obesity and cardiovascular disease might relate to inflammation in both fat tissue and the arterial wall. 18F-FDG uptake on PET is a surrogate marker of vessel wall inflammation. The aim of the study was to measure FDG uptake in both regions using PET and identify links between adipose and arterial inflammation.

Methods

Included in the study were 173 cardiovascular patients who were prospectively imaged with FDG PET/CT. Arterial FDG uptake was measured in the carotid arteries and ascending aorta. The same was done in fat tissue in the neck, the presternal region (both subcutaneous) and the pericardium. FDG uptake was quantified as average maximal target-to-background ratio (meanTBRmax). Multivariate regression analyses were performed to identify significant associations between arterial and adipose tissue FDG uptake and clinical variables as given by the standardized correlation coefficient (β).

Results

FDG uptake values in all fat tissue regions were highly predictive of vascular FDG uptake in both the carotids (β 0.262, p?<?0.0001, in the neck subcutaneous region) and aorta (β 0.22, p?=?0.008, in the chest pericardial region; β 0.193, p?=?0.019, in the chest subcutaneous region). Obesity was significantly associated with elevated FDG uptake in adipose tissue (β 0.470, p?<?0.0001, in the neck subcutaneous region; β 0.619, p?=?0.028, in the chest subcutaneous region; β 0.978, p?=?0.035, in the chest pericardial region).

Conclusion

FDG uptake in diverse fat tissue regions was significantly associated with arterial FDG uptake, a reasonable surrogate of inflammation. Increasing body weight significantly predicted the level of fatty inflammation. FDG PET therefore provides imaging evidence of an inflammatory link between fat tissue and the vasculature in patients with cardiovascular disease.  相似文献

An 83-year-old asymptomatic woman with a history of breast cancer treated with resection, radiation, and chemotherapy was studied with F-18 FDG positron emission tomography/computed tomography (PET/CT) for staging purposes. The whole-body F-18 FDG PET/CT images revealed poorly circumscribed, bilateral soft tissue masses between the inferior tips of the scapulae and chest wall with a moderate degree of F-18 FDG accumulation. The maximum standardized uptake values were 2.8 for both the left and right lesions. A biopsy or further imaging was not recommended because the lesions were asymptomatic, bilateral, and their imaging features were diagnostic for elastofibroma dorsi. Comparison to a CT scan performed 2 years earlier demonstrated stability of the lesions, further supporting our diagnosis. Recognition of this moderately metabolic benign entity in oncologic patients will avoid unnecessary medical, radiologic, or surgical interventions.  相似文献

PURPOSE: We report benign axillary tracer uptake on F-18 FDG PET scans in patients with rheumatoid arthritis. PATIENTS AND METHODS: Axillary tracer uptake was observed in 9 patients who were undergoing PET for staging or restaging of malignancy. The uptake was not in a pattern that would be expected for the patients' known malignancies. Additional clinical information was obtained and quantitation of tracer uptake in the upper extremities and axillae was performed. RESULTS: All 9 patients were found to have rheumatoid arthritis. Increased axillary tracer uptake was bilateral in 7 patients and unilateral in 2 patients. In 8 of the patients the wrists were included in the field of view and showed elevated FDG uptake. On follow-up there was no evidence of malignancy in any of the axillary foci. CONCLUSION: Increased axillary tracer uptake on F-18 FDG PET scans can be seen in conjunction with increased uptake in the wrists in patients with rheumatoid arthritis, and is not necessarily an indication of malignancy.  相似文献

Of 21 cases of cardiophrenic angle adenopathy (CAA) detected on computed tomography (CT) examination, 12 were due to malignant lymphomas, seven to carcinomas, and two to mesotheliomas. Of the nine lesions that were not malignant lymphomas, four were of supradiaphragmatic and five of infradiaphragmatic origin. On average, CAA was detected 4.6 years after the primary neoplasm was diagnosed. Only 29% of the patients were alive 14 months after CAA was detected on CT scans. Radiography was inferior to CT in the detection of CAA, with only 35% of radiography results clearly positive. Malignant lymphoma is a major but not exclusive cause of CAA, and it must be differentiated from lymphatic seeding of supradiaphragmatic and infradiaphragmatic malignancies. Unilateral CAA may point to supradiaphragmatic neoplasms or, if right-sided, to ovarian carcinoma, and bilateral CAA may indicate another neoplasm of infradiaphragmatic origin.  相似文献

PURPOSE: (18)F-fluorodeoxyglucose ((18)F-FDG) positron emission tomography (PET) imaging has been used in the assessment of paediatric malignancies. PET/CT increases the diagnostic accuracy in adult cancer patients. The present study assesses the incremental value of FDG PET/CT in paediatric malignancies. METHODS: A total of 118 (18)FDG PET/CT studies of 46 paediatric patients were reviewed retrospectively. PET and PET/CT results were classified as malignant, equivocal or benign, compared on a site- and study-based analysis, and also compared with the clinical outcome. RESULTS: Three hundred and twenty-four sites of increased FDG uptake were detected. Discordant PET and PET/CT interpretations were found in 97 sites (30%) in 27 studies (22%). PET yielded a statistically significant higher proportion of equivocal and a lower proportion of benign lesion and study results (p<0.001) than PET/CT. With PET there were 153 benign (47%), 84 (26%) equivocal and 87 (27%) malignant sites, while PET/CT detected 226 benign (70%), 10 (3%) equivocal and 88 (27%) malignant lesions. PET/CT mainly improved the characterisation of uptake in brown fat (39%), bowel (17%), muscle (8%) and thymus (7%). The study-based analysis showed that 17 equivocal and seven positive PET studies (20%) were interpreted as benign on PET/CT, while three equivocal studies were interpreted as malignant. The study-based sensitivity and specificity of PET/CT were 92% and 78% respectively. CONCLUSION: PET/CT significantly improved the characterisation of abnormal (18)FDG foci in children with cancer, mainly by excluding the presence of active malignancy in sites of increased tracer activity.  相似文献

Background

Acute complicated pyelonephritis (ACP) is an upper urinary tract infection associated with coexisting urinary tract abnormalities or medical conditions that could predispose to serious outcomes or treatment failures. Although CT and magnetic resonance imaging (MRI) are frequently used in patients with ACP, the clinical value of 18F–fluorodeoxyglucose positron emission tomography and computed tomography (FDG PET/CT) has not been systematically investigated. This single-center retrospective study was designed to evaluate the potential usefulness of FDG PET/CT in patients with ACP.

Methods

Thirty-one adult patients with ACP who underwent FDG PET/CT were examined. FDG PET/CT imaging characteristics, including tracer uptake patterns, kidney volumes, and extrarenal imaging findings, were reviewed in combination with clinical data and conventional imaging results.

Results

Of the 31 patients, 19 (61%) showed focal FDG uptake. The remaining 12 study participants showed a diffuse FDG uptake pattern. After volumetric approximation, the affected kidneys were found to be significantly enlarged. Patients who showed a focal uptake pattern had a higher frequency of abscess formation requiring drainage. ACP patients showing diffuse tracer uptake patterns had a more benign clinical course. Seven patients had suspected extrarenal coinfections, and FDG PET/CT successfully confirmed the clinical suspicion in five cases. FDG PET/CT was as sensitive as CT in identifying the six patients (19%) who developed abscesses. Notably, FDG PET/CT findings caused a modification to the initial antibiotic regimen in nine patients (29%).

Conclusions

FDG PET/CT may be clinically useful in the assessment of patients with ACP who have a progressive disease course.

Purpose Peritoneal carcinomatosis can be difficult to diagnose using computed tomography (CT). The purpose of this study was to evaluate the role of 2-(fluorine 18) fluoro-2-deoxy-d-glucose (FDG) positron emission tomography (PET) in the detection of peritoneal carcinomatosis.Methods We reviewed the CT and FDG PET radiological reports and clinical charts of 18 patients with peritoneal carcinomatosis and 17 cancer patients without peritoneal carcinomatosis. We also assessed FDG PET scans from 20 healthy volunteers as a baseline study. The maximum standardised uptake values (SUVmax) over peritoneal lesions in cancer patients and over the area of most intense intestinal uptake in healthy volunteers and cancer patients without peritoneal carcinomatosis were measured.Results The sensitivity and positive predictive value (PPV) of combined FDG PET and CT were superior to those of CT alone for the detection of peritoneal lesions (sensitivity: 66.7% vs 22.2%, p<0.025; PPV: 92.3% vs 50.0%, p<0.05). The most frequent pattern of FDG uptake in patients with peritoneal carcinomatosis was abnormally intense focal uptake near the abdominal wall. An SUVmax threshold of 5.1 produced a diagnostic accuracy of combined FDG PET and CT of 78%. The additional information provided by FDG PET allowed a more accurate diagnosis in 14 patients (40.0%), and led to alteration of the therapeutic strategy in five (14.3%) of the enrolled cancer patients.Conclusion We found that use of an intra-abdominal FDG uptake cut-off value for SUVmax of >5.1 assists in the diagnosis of peritoneal carcinomatosis. FDG PET may play an important role in the clinical management of patients with suspected peritoneal carcinomatosis.  相似文献

Purpose

The aim of this study was to evaluate the physiologic and benign F-18 fluorodeoxyglucose (FDG) avid foci in patients with breast cancer.

Methods

On 309 F-18 FDG PET/CT scans of 241 women with breast cancer, the hypermetabolic lesions compared with the surrounding normal region were evaluated retrospectively. Available reports of other relevant radiological imaging, medical records, and follow-up PET/CT were reviewed for explanations of the abnormal uptake.

Results

Among the 70 physiologic foci, muscular uptake of the lower neck following the surgical and/or radiation therapy of ipsilateral breast (29%), hypermetabolic ovaries (16%) and uterine (10%) uptake during the ovulatory and menstrual phases during the normal menstrual cycle were identified, and also hypermetabolic brown fat in cold-induced thermogenesis (7%), non-specific bowel uptake (35%) were observed. Among the 147 benign lesions, sequelae of the chest wall and breasts following surgical and/or radiation therapy, were often observed (27%). Hypermetabolic thyroid glands were noted as adenomas and chronic thyroiditis (18%). Reactive hyperplasia of cervical or mediastinal lymph nodes (32%), degenerative osteoarthritis and healed fractures (15%), hypermetabolic benign lung lesions (6%) were observed.

Conclusion

Altered physiologic and benign F-18 FDG uptake in the lower cervical muscle and chest wall following ipsilateral breast surgery or radiotherapy were common, and also normal physiologic uptake in ovary and uterus, brown fat, thyroid were considered as predominant findings in women patients with breast cancer. Knowledge of these findings might aid in the interpretation of FDG PET/CT in patients with breast cancer.  相似文献

Increased 18F-fluorodeoxyglucose (FDG) uptake of brown fat on 18F-FDG positron emission tomography (PET) originating from physiological activation is a common incidental finding and is usually located in the neck, shoulder, and supraclavicular areas. We present a case of an incidental pheochromocytoma showing diffusely increased 18F-FDG uptake in bilateral perirenal fat tissue as well as supraclavicular and paravertebral fat tissue on 18F-FDG PET/CT. The patient had no clinical symptoms except hypertension, and a pheochromocytoma was confirmed in a postsurgical specimen. A pheochromocytoma should be considered a cause in cases of increased 18F-FDG uptake of perirenal brown fat.  相似文献

F-18 fluoro-2-deoxyglucose positron emission tomography combined with computed tomography (FDG and PET/CT) is increasingly becoming the standard in staging and restaging patients with a range of malignancies including B-cell lymphoma. However, there are well-known pitfalls in PET/CT with FDG imaging, which comprise infection, inflammation, physiological variants, and benign pathologic conditions. Fat necrosis is the result of death of adipose tissue from disease, injury, or pathologic conditions. We describe a case of false positive PET/CT and FDG scan in a patient with fat necrosis mimicking B-cell lymphoma after 6 cycles of rituximab with cyclophosphamide, doxorubicin, vincristine, and prednisone (R-CHOP) treatment. In interpreting PET/CT and FDG images with inconsistency in lesion response, fat necrosis should be considered in the differential diagnosis.  相似文献

A 48-year-old woman presented with a 50-day history of irregular vaginal bleeding and lower abdominal pain. Ultrasound indicated an extremely large occupying lesion in the pelvic cavity that was highly suggestive of malignancy. Fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) was performed to further assess the nature of pelvic abnormality. PET/CT images demonstrated a diffusely lobulated mass ranging from cervix up to the inferior pole of kidneys with mild FDG uptake. Simultaneously, multiple nodules in bilateral lungs and a hypodense lesion in the right ventricle were shown without FDG-avidity. Based on the imaging results, the presumptive diagnosis was uterine intravenous leiomyomatosis with intracardiac extension and pulmonary benign metastases, which was subsequently confirmed by MRI and the lesion biopsy.  相似文献

OBJECTIVE: Incidental (18)F-FDG-avid breast lesions are commonly encountered in patients with cancer who undergo staging PET/CT. This pictorial essay discusses breast lesions that show increased FDG activity, mimicking breast cancer, with biopsy-confirmed benign diagnosis. CONCLUSION: Acute and chronic inflammation, physiologic lactation, and benign breast masses, including silicone granuloma, fat necrosis, fibroadenoma, and postsurgical changes, may show increased FDG uptake on PET/CT. These conditions can often be differentiated from malignancy by correlative imaging, including mammography, sonography, or MRI.  相似文献

Hepatic adenomatosis is an uncommon benign neoplasm, with the presence of multiple adenomas (generally more than 4) within the liver. A 52-year-old woman presented with multiple (>10) solid liver lesions detected with abdominal ultrasonography and verified with magnetic resonance imaging (MRI). Subsequently, F-18 FDG PET/CT demonstrated increased uptake in these lesions. Histology revealed hepatic adenomatosis. F-18 FDG PET/CT cannot reliably differentiate hepatic adenomas from malignant processes on the basis of uptake.  相似文献

ObjectiveIncidental 18-Fluoro-2-deoxyglucose positron emission tomography (18F-FDG) uptake in the ribs is a relatively common finding on positron emission tomography/computed tomography (PET/CT) images of cancer patients. This study examined the role of 18F-FDG PET/CT in differentiating between benign lesions and metastases on the ribs.MethodsThis study included 264 lesions in 172 PET/CT cases with underlying malignancy showing newly developed indeterminate 18F-FDG rib uptake between June 2009 and May 2010. Patients with more than five FDG rib uptakes or hematologic malignancy were excluded. Malignancy was confirmed either histologically or by imaging studies, and clinical follow-up with serial images was at least 6 months. The maximum standardized uptake value (SUVmax) of the rib lesion was recorded. The FDG uptake patterns (focal or segmental; discrete or non-discrete) and CT findings (evidence of fracture, soft tissue lesions, osteoblastic and/or osteolytic lesions) were recorded.ResultsThere were 206 benign lesions and 58 metastases. The SUVmax was significantly higher in the metastatic group (3.0±1.8) than in the benign group (2.5±1.1), (P= .014). For the differential diagnosis between benign and metastatic lesions, the best SUVmax cut-off was determined to be 2.4. Significant indicators for metastasis were a segmental FDG uptake pattern (OR=10.262, 95% CI 4.151–25.371), presence of an osteoblastic/-lytic lesion (OR=22.903, 95% CI 10.468 to 50.108) and the absence of fractures on CT (OR=291.629, 95% CI 39.09–2175.666).ConclusionSUVmax alone is not sufficient to differentiate benign and metastatic rib lesions in cancer patients. The diagnostic accuracy can be further increased when findings of the CT part of PET/CT are considered.  相似文献
