目的总结21例内镜下环形肌切开术(POEM)的护理体会。方法对2011年6月-2012年6月21例确诊为贲门失弛缓症(AC)并接受POEM治疗护理进行回顾性分析。结果21例患者均成功行POEM治疗。其中3例术中少许渗血,经内镜下电凝止血,出血停止;2例出现纵隔和皮下气肿,2~3d后气肿消退。结论术前术中术后精心的护理,可减少并发症,提高手术成功率。 相似文献

目的探讨经口内镜下肌切开术(POEM)治疗贲门失弛缓症(AC)的临床疗效。方法选取2012年6月-2016年5月浙江大学医学院附属第一医院确诊为AC并接受POEM治疗的44例患者,观察患者手术前后AC临床症状评分系统(Eckardt评分)及胸部CT、食管X线造影结果,评价手术疗效。结果 44例患者均成功完成POEM,黏膜下隧道切开长度(12.9±1.1)cm,肌切开长度(9.6±2.4)cm。术后44例患者经1~39个月随访,术后Eckardt评分(1.1±1.1)分,较术前(6.6±2.0)分明显降低(P0.01)。术前、术后行胸部CT 10例,术后食管最大径均值(25.7±8.5)mm,较术前(41.5±14.9)mm明显减小(P0.01)。术前、术后行X线食管造影10例,术后食管最大径较术前缩小(29.3±16.8)%。44例患者体重由术前的(53.2±10.9)kg增加至术后的(58.3±11.7)kg,差异有统计学意义(P0.01)。结论 POEM作为治疗AC的一种新兴的微创手术,其短中期疗效确切,可以有效缓解患者吞咽困难等症状,改善患者的营养状况。 相似文献

目的:探讨心理护理联合经口内镜下环形肌切开术( POEM)治疗贲门失弛缓症( AC)的效果。方法选择2011年6月—2012年6月确诊AC行POEM手术患者80例,围术期给予心理干预。手术前后采用Eckardt评分比较患者AC临床症状改善情况,采用汉密尔顿焦虑量表( HAMA)、汉密尔顿抑郁量表( HAMD)比较患者心理改善情况。结果80例AC患者术前Eckardt评分平均(8±3)分,术后3个月平均(2±2)分,术后6个月平均(2± 1)分,手术前后比较差异有统计学意义(H=155.613,P<0.01)。术前HAMA>14分患者19例,术后3个月降至5例,手术前后患者焦虑情况比较差异有统计学意义(χ2=19.030,P<0.01);术前HAMD>20分患者12例,术后3个月降至3例,手术前后患者抑郁情况比较差异有统计学意义(χ2=11.522,P<0.01)。结论 POEM手术联合心理护理治疗AC对焦虑抑郁情绪的改善及临床症状的缓解具有良好的效果,并发症少,安全可行。 相似文献

目的探讨经口内镜下肌切开术(POEM)治疗贲门失弛缓症(AC)的临床方法和护理。方法研究确诊为AC并接受POEM治疗的6例患者的临床资料。POEM主要步骤:食管黏膜层切开;分离黏膜下层,建立黏膜下"隧道";胃镜直视下切开环形肌;金属夹关闭黏膜层切口。护士做好术前、术中配合和术后护理工作。结果 6例患者均成功完成POEM手术,手术时间60~128 min,平均76.5 min,黏膜下隧道长度9~12 cm,平均10.3 cm,环形肌切开长度7~10 cm,平均8.2 cm,1例出现皮下气肿,1例左侧气胸。术后随访l~10个月,6例吞咽困难均明显得到解除。结论 POEM治疗AC短期疗效肯定,可以迅速缓解患者吞咽困难,但其长期疗效及远期并发症仍有待随访观察。要求术者有较高的内镜操作水平,良好的术中医护配合及严密的病情观察和护理是手术成功的关键。 相似文献

目的探讨经口内镜下肌切开术(POEM)治疗贲门失弛缓症(AC)护理配合技巧与安全性。方法研究2011年12月至2012年4月确诊AC并接受POEM治疗护理的8例患者的临床资料。POEM主要手术配合步骤;协助食管黏膜下注射;食管黏膜层切开;分离黏膜下层建立黏膜下“隧道”;内镜直视下纵行切开环形肌;金属钛夹关闭黏膜层切口。结果所有8例患者均成功接受POEM手术,无1例因为护理配合出现与POEM相关的严重并发症。8例患者吞咽困难等症状得到明显改善,均顺利出院。结论经口内镜下肌切开术治疗贲门失弛缓症短期疗效肯定。护理配合中的快、稳、准可有效地减少手术的并发症,提高手术治疗的安全性。 相似文献

目的探讨经口内镜下肌切开术(POEM)治疗儿童贲门失弛缓症(AC)的临床疗效及并发症发生情况。方法回顾性分析该院2012年11月-2016年11月确诊AC并接受POEM治疗的17例患儿的临床表现、POEM手术过程及术后随访情况。结果 17例患儿均成功完成POEM,无严重并发症发生,手术时间(62.65±35.54)min;17例患儿术后随访12~36个月,手术前后AC临床症状评分(Eckardt评分)分别为5(4.0-6.0)和0(0.0-1.0)分,差异有统计学意义(P=0.000),临床症状明显缓解,体重减轻情况明显改善。结论 POEM治疗儿童AC安全性高,短期疗效确切,长期疗效及远期并发症有待大样本量的随访监测。 相似文献

目的探讨经口内镜下肌切开术(POEM)治疗贲门失弛缓症(AC)的疗效。方法收集2014年5月-2016年3月遵义医学院附属医院消化内科收住入院的27例确诊为AC的患者,并实施POEM术,总结手术完成情况、手术前后临床症状、食管测压、术后并发症及随访过程等资料。结果 27例患者行POEM治疗的成功率为100.0%,1例患者发现皮下气肿,1例患者发现少量气胸,行POEM患者中随访症状较前明显改善,食管测压较前亦有显著变化(P0.05)。结论 POEM治疗AC的短期疗效肯定,术后能够明显缓解吞咽困难等症,但其长期疗效及远期并发症仍有待进一步随访和观察。 相似文献

Achalasia is a type of esophageal motility disorder, consisting of relaxation dysfunction of the lower esophagus and disturbed esophageal peristalsis. Related clinical symptoms include dysphagia, regurgitation, chest pain, and weight loss. Traditional treatment options include endoscopic botulinum toxin injection, endoscopic pneumatic dilation, and laparoscopic Heller’s myotomy. These therapies mainly relieve symptoms by reducing the pressure on the lower esophageal sphincter and reducing blood flow resistance at the esophagogastric junction. Based on endoscopic submucosal dissection and natural orifice transluminal endoscopic surgery, peroral endoscopic myotomy (POEM) is a purely endoscopic method of myotomy with minimal invasiveness and a low rate of adverse events when performed by experienced operators. Since then, numerous studies have shown the significant clinical efficacy and safety of POEM. Background and aimsPeroral endoscopic myotomy (POEM) is an established therapy for achalasia, but outcome evaluation has often been limited to Eckardt score (ES). The present study was aimed to improve knowledge about outcome evaluation and predictive outcome factors by performing a comprehensive objective evaluation of achalasia patients treated by POEM.MethodsThis single centre prospective study reports outcome data 12 months after POEM in treatment-naive achalasia patients. A predefined follow-up protocol included ES, high resolution manometry, 24-h pH measurement, upper endoscopy and timed barium esophagogram (TBE). Univariate and multivariate regression analyses were performed to analyze association between post-POEM variables and identify predictive factors for objective outcome.ResultsFifty patients were included with a drop-out rate of <5%. ES, lower oesophageal sphincter pressures, TBE heights and maximal oesophageal diameter were all reduced after POEM (p < .001), and 28% (13/47) of the patients had a positive 24-h pH registration. An oesophageal diameter >3 cm after POEM was associated with treatment failure assessed by ES (p = .04) and TBE (p = .03). Advanced achalasia stage (p = .02) and long symptom duration (p = .04) were identified as independent predictive factors for poor outcome assessed by TBE.ConclusionsThe present study confirms that POEM is an efficient therapy for achalasia. The comprehensive objective evaluation after POEM demonstrates that long symptom duration and major changes in oesophageal anatomy at diagnosis imply poor treatment outcome, and a post-POEM dilated oesophagus is associated with treatment failure.

Achalasia is a primary motility disorder with incomplete lower esophageal sphincter relaxation; it has an annual incidence of 0.11 cases per 100 000 children. Peroral endoscopic myotomy (POEM) is a new endoscopic treatment method for achalasia. Reports about POEM in pediatric patients are rare. We herein report the case of a 9‐year‐old female patient with achalasia who underwent POEM. The patient underwent endoscopic balloon dilatation because medication was not effective at a previous hospital; however, endoscopic balloon dilatation was not effective either. She then underwent successful POEM upon admission at our hospital. The patient was symptom‐free at 2 years postoperatively with no signs of esophagitis in the absence of proton‐pump inhibitor therapy. 相似文献
