涎腺腺样囊性癌细胞转移潜能与细胞外基质的关系

目的 了解不同转移能力的涎腺腺样囊性癌（ｓａｌｉｖａｒｙ ａｄｅｎｏｉｄ ｃｙｓｔｉｃ ｃａｒｃｉｎｏｍａ，ＳＡＣＣ）细胞与细胞外基质粘附、对细胞外基质侵袭及趋化运动的能力。方法 ＭＴＴ法测定ＳＡＣＣ－ＬＭ，ＳＡＣＣ－８３，ＡＣＣ－Ｍ，ＡＣＣ－２与纤维粘连蛋白、层粘连蛋白、Ⅳ型胶原的粘附，用改良Ｂｏｙｅｄｎ ｃｈａｍｂｅｒ方法观察了ＳＡＣＣ细胞对人工重组基底膜的侵袭以及趋化运动。结果 高转移的ＳＡ     

涎腺腺样囊性癌过继免疫治疗的实验研究

为研究涎腺腺样囊性癌过继免疫治疗的特点，通过体外试验观察了肿瘤坏死因子α（ｔｕ－ｍｏｒｎｅｃｒｏｓｉｓｆａｃｔｏｒα，ＴＮＦ－α）与γ干扰素（ｉｎｔｅｒｆｅｒｏｎ－γ，ＩＦＮ－γ）分别作用于白细胞介素２（ｉｎｔｅｒｌｅｕｋｉｎ－２，ＩＬ－２）诱导的淋巴因子激活杀伤（ｌｙｍｐｈｏｋｉｎｅ－ａｃｔｉｖａｔｅｄｋｉｌｅｒ，ＬＡＫ）细胞杀伤靶细胞不同阶段时对ＬＡＫ细胞杀伤活性的影响。结果表明腺样囊性癌（ａｄｅｎｏｉｄｃｙｓｔｉｃｃａｒｃｉｎｏｍａ，ＡＣＣ）－２为ＬＡＫ杀伤敏感细胞，ＴＮＦ－α与ＩＦＮ－γ均适合作为增强剂增强ＬＡＫ细胞杀伤ＡＣＣ－２的活性。ＴＮＦ－α的增强方式是与ＩＬ－２共同诱导ＬＡＫ细胞，而ＩＦＮ－γ则通过预保温ＡＣＣ－２细胞提高其被ＬＡＫ细胞杀伤的敏感性。     

细胞粘附蛋白在涎腺腺样囊性癌细胞中的表达

目的　探讨具有不同转移能力的涎腺腺样囊性癌（ａｄｅｎｏｉｄ ｃｙｓｔｉｃ ｃａｒｃｉｎｏｍ ａ; Ａ Ｃ Ｃ）细胞系中粘附分子表达的差异。方法　采用免疫组化方法对两株人腺样囊性癌细胞系（ Ａ Ｃ Ｃ２ 和 Ａ Ｃ Ｃ Ｍ）培养细胞及其 Ｓ Ｃ Ｉ Ｄ 小鼠移植瘤模型进行粘附分子 Ｃ Ｄ４４ｓ（标准型）、 Ｃ Ｄ４４ｖ６（变异型）、 Ｅｃａｄｈｅｒｉｎ（ Ｅｃａｄ）、αｃａｔｅｎｉｎ（αｃａｔ）、βｃａｔｅｎｉｎ（βｃａｔ）的检测。结果　（１） Ａ Ｃ Ｃ肺转移灶的肿瘤细胞中粘附分子的表达普遍高于其皮下移植瘤细胞;（２） Ａ Ｃ Ｃ Ｍ 肺转移灶肿瘤生长缓慢,细胞分化相对较好,而其皮下移植瘤和腹腔种植瘤肿块生长迅速,细胞分化差;（３） Ｃ Ｄ４４ｖ６ 在 Ａ Ｃ Ｃ Ｍ 和 Ａ Ｃ Ｃ２ 体外培养细胞中均有弱阳性表达,而在异体移植瘤中包括皮下移植瘤和肺部转移灶均无表达。结论　 Ａ Ｃ Ｃ细胞中 Ｃ Ｄ４４ｓ 的表达受细胞分化程度影响,并影响了肿瘤细胞本身的生长状况; Ｅｃａｄ／ｃａｔ复合体的表达与否或表达强度与肿瘤细胞的分化成正比; Ｃ Ｄ４４ｖ６ 在涎腺 Ａ Ｃ Ｃ中的表达和转移能力可能无关。

为探讨肿瘤转移抑制基因ｎｍ２３与涎腺腺样囊性癌（ａｄｅｎｏｉｄｃｙｓｔｉｃｃａｒｃｉｎｏｍａ，ＡＣＣ）的关系，应用ＬＳＡＢ免疫组化染色方法，研究ｎｍ２３表达产物二磷酸核苷激酶（ｎｕｃｌｅｏｓｉｄｅｄｉｐｈｏｓｐｈａｔｅｋｉ－ｎａｓｅ，ＮＤＰＫ）在ＡＣＣ中的表达。结果：ＮＤＰＫ／ｎｍ２３在ＡＣＣ中有较高表达，阳性率为６４．０％（１６／２５）；其中，有肺转移者阳性率为１２．５％（１／８），无肺转移者阳性率为８８．２％（１５／１７），差异有极显著性（Ｐ＜０．０１）；ＮＤＰＫ表达与临床分期有关，分期越高，阳性率越低，差异有显著性（Ｐ＜０．０５），与病理分型差异无显著性（Ｐ＞０．０５）。结论：ｎｍ２３基因在人ＡＣＣ肺转移过程中起到抑制转移的作用，可作为临床颈侧ＡＣＣ患者转移和预后的指标。

腺样囊性癌在纤维连接蛋白中趋化功能变化及其浸润机制

目的 通过腺样囊性癌（ａｄｅｎｏｉｄ ｃｙｓｔｉｃ ｃａｒｃｉｎｏｍａ,ＡＣＣ）在纤维连接蛋白（ｆｉｂｒｏｎｅｃｔｉｎ,ＦＮ）中的固相趋化功能变化探讨其浸润机制。方法 用Ｂｏｙｄｅｎ小室法和流式细胞术等测定了３种ＡＣＣ细胞对ＦＮ的趋化反应和细胞表面的受体变化。结果 ３种ＡＣＣ均比鳞状细胞癌（ｓｑｕａｍｏｕｓ ｃｅｌｌ ｃａｒｃｉｎｏｍａ,ＳＣＣ）趋化功能强;ＡＣＣ和ＳＣＣ细胞表面α２、α３、α６和β１受体亚单位水平高,且ＡＣＣ３α５也高;受体阻断可见：ＦＮ中阻断ＡＣＣ的α５、β１受体亚单位趋化功能呈高度抑制,阻断α４呈中度抑制,ＳＣＣ则阻断α１、α４和β１呈强抑制。结论 ＦＮ中ＡＣＣ比ＳＣＣ侵袭力强;各种细胞不同受体在ＦＮ中趋化反应的作用有一定规律。     

涎腺腺样囊性癌c—erbB—2癌基因蛋白的研究

采用ｃ－ｅｒｂＢ－２癌基因蛋白抗体，对３４例涎腺腺样囊性癌（包括筛孔型，管状型，实性型）进行免疫组化染色，结果显示，涎腺腺样囊性癌３种组织学类型均过度表达ｃ－ｅｒｂＢ－２癌基因，经统计学分析表明，筛孔型与管状型涎腺腺样囊性癌的ｃ－ｅｒｂＢ－２癌基因表达差异有显著性（Ｐ〈０．０５），提示ｃ－ｅｒｂＢ－２癌基因的过度表达与肿瘤的分化及恶性程度关系密切。     

涎腺多形性腺瘤的纤维粘连素和层粘蛋白的表达和分布

纤维粘连素（ＦｉｂｒｏｎｅｃｔｉｎＦＮ）是细胞外基质蛋白；层粘蛋白（ＬａｍｉｎｉｎＬＭ）是基底膜成分，两者均对细胞的生长、分化、极向化和移动等行为起重要介导作用，本文采用免疫组织化学方法观察ＦＮ和ＬＭ在正常涎腺及涎腺多形性腺瘤的分布和表达，正常腺体中ＦＮ的表达定位于闰管细胞胞浆；ＬＭ定位于各级导管细胞胞浆，导管及腺泡的基底膜，多形性腺瘤中ＦＮ阳性细胞分散在肿瘤上皮结构区，在玻璃样、粘液样和软骨样间质中的变异肌上皮细胞ＦＮ呈强阳性，ＬＭ阳性见于肿瘤上皮细胞的邻接面和玻璃样间质中的变异肌上皮细胞，软骨样间质ＬＭ阴性

Immediate loading of hydroxyapatite-coated implants in the maxillary premolar area: three-year results of a pilot study

STATEMENT OF THE PROBLEM: Although immediate loading of implants in the edentulous mandible has been described in the literature, there is limited information regarding immediate loading of single implants. PURPOSE: This prospective clinical study evaluated the clinical parameters of immediately loaded single-threaded hydroxyapatite-coated (HA) root form implants. MATERIAL AND METHODS: Ten human subjects were included in this report. In all situations, a screw-retained provisional acrylic resin crown was placed in the maxillary premolar area immediately after implant surgery. Definitive screw-retained metal-ceramic prostheses were placed 6 months after surgery. Standardized periapical radiographs were made before implant surgery, immediately after surgery, and 1, 3, 6, 12, and 36 months after implant surgery. Mobility (measured with the Perio-Test), distance from implant platform to the gingival crevice, distance from the implant platform to the depth of the sulcus, peri-implant probing depth, and bleeding on probing index were recorded at 3, 6, 12, and 36 months after implant placement. For clinical measurements, a 5-mm healing abutment was placed. RESULTS: All implants appeared clinically osseointegrated. Standardized radiographs demonstrated mean marginal bone loss of 0.6, 0.7, 0.8, 0.9, and 1.0 mm at 1, 3, 6, 12, and 36 months after implant surgery, respectively. Mean implant mobility was -3.3 at the day of surgery and -3.8, -3.4, -3.6, and -4.2 at 3, 6, 12, and 36 months, respectively. The distance from implant platform to the gingival crevice was 2.8, 2.4, 2.4, and 3.1 mm at 3, 6, 12, and 36 months, respectively. The distance from the implant platform to the depth of the sulcus was 0.8, 0.9, 0.9, and 1.1 mm at 3, 6, 12, and 36 months, respectively. The peri-implant probing depth was 3.6, 3.3, 3.2, and 4.3 mm at 3, 6, 12, and 36 months, respectively. The bleeding on probing index was 0.4, 0.4, 0.4, and 0.1 at 3, 6, 12, and 36 months, respectively. CONCLUSION: The results of this prospective pilot study provide initial evidence that single root form implants may be immediately loaded when placed at the maxillary premolar area.
