Although, emotions play a crucial role in schizophrenia, the changes in emotional dimension still remain controversial. The aim of our work was: 1) to compare the disorders of emotional control between the examined groups: S--non-chronic schizophrenic patients (n = 50), CS--chronic schizophrenic patients (n = 50), N--healthy controls (n = 50), R--right brain-damaged patients (n = 30), and L--left brain-damaged patients (n = 30), 2) to assess a level of impairment of emotional control, its relation to lateralised hemisphere damage and chronicity of schizophrenic process. All psychiatric subjects were diagnosed as paranoid schizophrenics according to DSM-IV criteria and were scored on the PANSS scale after four weeks of neuroleptic treatment. Brain-damaged patients were included if they experienced single-episode cerebrovascular accidents causing right or left hemisphere damage (confirmed in CT scan reports). The neurological patients were examined at least 3 weeks after the onset of cerebrovascular episode. Emotional control was assessed using Brzeziński Questionnaire of Emotional Control aimed at the evaluation of: 1) control in perception and interpretation of emotive situation, 2) emotional arousal, 3) emotional-rational motivation, and 4) acting caused by emotions. Our results revealed significantly greater impairment of emotional control in schizophrenics (chronic schizophrenics, in particular) compared to healthy volunteers. Chronicity of the schizophrenic process seemed to intensify emotional control impairment. Interestingly, no significant qualitative and quantitative differences in emotional control mechanism between unilateral brain-damaged patients and the control group were found.  相似文献

The aim of this study was the assessment of cerebral specialisation in perception of emotional chimeric drawings in 50 non-chronic schizophrenics (S), 50 chronic schizophrenics (CS), 30 right brain-damaged inpatients (P), and 50 normal controls. All were marked right handers. The assessment was performed after a four-week treatment. Structure and intensity of schizophrenia symptomatology were scored on the PANSS scale. Happy-sad chimeric face drawings (David 1989) were viewed twice in free vision. A perceiver bias towards left hemiface of chimeric drawings (LHF bias) and sad bias were scored in all subjects. Subjects rated their mood at the time of testing on a visual analogue scale. The schizophrenics and right brain-damaged inpatients showed significantly weaker LHF bias compared with healthy subjects, which may suggest right cerebral hemisphere dysfunction in perception of emotional chimeric drawings. Moreover, chronic schizophrenic patients revealed significantly weaker LHF bias and sad bias compared to non-chronic subjects. There was no correlation of left perceptual bias with clinical ratings: PANSS scale, MMSE, number of hospitalisations, neuroleptic dose, and current mood, which suggests stable properties of the perceptual deficit.  相似文献

Aspects of emotional facial expression (responsivity, appropriateness, intensity) were examined in brain-damaged adults with right or left hemisphere cerebrovascular lesions and in normal controls. Subjects were videotaped during experimental procedures designed to elicit emotional facial expression and non-emotional facial movement (paralysis, mobility, praxis). On tasks of emotional facial expression, patients with right hemisphere pathology were less responsive and less appropriate than patients with left hemisphere pathology or normal controls. These results corroborate other research findings that the right cerebral hemisphere is dominant for the expression of facial emotion. Both brain-damaged groups had substantial facial paralysis and impairment in muscular mobility on the hemiface contralateral to site of lesion, and the left brain-damaged group had bucco-facial apraxia. Performance measures of emotional expression and non-emotional movement were uncorrelated, suggesting a dissociation between these two systems of facial behaviour.  相似文献

Emotional behaviour in non-chronic and chronic schizophrenic patients, right and left hemisphere-damaged patients and healthy people was observed. Similar type of emotional reactions have been recorded in schizophrenia and right hemisphere damage. This might suggest the similarity of localisation of the cerebral dysfunction in the two patient groups. The difficulties with catching verbal expressions and verbal humour might be associated with the frontal cortex dysfunction.  相似文献

This study examines the contribution of the lexical/verbal channel to emotional processing in 16 right brain-damaged (RBD), 16 left brain-damaged (LBD) and 16 normal control (NC) right-handed adults. Emotional lexical perception tasks were developed; analogous nonemotional tasks were created to control for cognitive and linguistic factors. The three subject groups were matched for gender, age and education. The brain-damaged groups were similar with respect to cerebrovascular etiology, months post-onset, sensory-motor status and lesion location. Parallel emotional and nonemotional tasks included word identification, sentence identification and word discrimination. For both word tasks, RBDs were significantly more impaired than LBDs and NCs in the emotional condition. For all three tasks, RBDs showed a significantly greater performance discrepancy between emotional and nonemotional conditions than did LBDs or NCs. Results were not affected by the valence (i.e. positive/negative) of the stimuli. These findings suggest a dominant role for the right hemisphere in the perception of lexically-based emotional stimuli.  相似文献

Emotional discrimination was studied in patients with schizophrenia (n = 20) and matched controls. Performance of the emotion-discrimination tasks in the schizophrenic patients was impaired, relative to their performance of an age-discrimination task. Performance patterns in the patient group could also be reliably distinguished from those of normal controls. The impairment was associated with the severity of both emotional and nonemotional symptoms specific to schizophrenia, but not with the severity of nonspecific symptoms. The deficit associated with schizophrenia is more marked than that reported for depression (Gur et al., 1992), particularly for the emotion-discrimination tasks, and showed no difference between "happy" discrimination and "sad" discrimination. The main difficulty in patients with schizophrenia is the assignment of emotional valence to neutral faces. The magnitude of the deficit underscores the salience of emotional impairment in schizophrenia, and its relation to cognitive dysfunction in this disorder merits further scrutiny.  相似文献

The present study examined both quantitative and qualitative hypnotizability differences among four psychiatric patient groups (dissociative disorder (n = 17), schizophrenic (n = 13), mood disorder (n = 14), and anxiety disorder (n = 14) patients), and normals (college students (n = 63). Dissociative disorder patients earned significantly higher corrected total scores on the Stanford Hypnotic Susceptibility Scale, Form C (mean = 7.94), than all other groups. Likewise, dissociative disorder patients initially recalled significantly fewer items when the posthypnotic amnesia suggestion was in effect (mean = .41) and reversed significantly more items when the suggestion was canceled (mean = 3.82) than all other groups. In contrast, schizophrenic patients recalled significantly fewer items when the amnesia suggestion was in effect (mean = 1.85) and reversed significantly fewer items when it was canceled (mean = .77) than the remaining groups. This qualitative difference between schizophrenic patients and the other groups on the suggested posthypnotic amnesia item was observed even though there were no significant quantitative differences between groups in overall hypnotic responsivity.  相似文献

BACKGROUND: Abnormalities in the patterns and frequency of cortical folding might help identify temporal and regionally specific disturbances in normal neurodevelopmental processes that occur in association with schizophrenia. Few studies, however, have investigated gyral complexity in schizophrenia. METHODS: High-resolution magnetic resonance images were used to examine differences in gyral complexity, measured three-dimensionally in five separate hemispheric regions covering the entire cortical surface, in patients with first-episode schizophrenia (n = 50) compared with demographically matched healthy comparison subjects (n = 50). The modulating effects of gender and hemisphere were also examined. RESULTS: Significant increases in cortical folding were observed in the right superior frontal cortex in male schizophrenic patients compared with male control subjects, but not between female diagnostic groups. Significant main effects of hemisphere were found in frontal, parietal, and occipital regions in directions complementary to cerebral torques. CONCLUSIONS: Results support previous findings of right frontal hypergyria in male schizophrenic patients and suggest that these abnormalities predate illness onset and are of neurodevelopmental origin. Therefore, in schizophrenia, sexually dimorphic developmental processes and differences in hemispheric connectivity, which have been shown to influence the organization and/or frequency of cortical folding, seem to be disturbed during gyral formation in utero.  相似文献

The present study investigates the relationship between affective mood state as assessed by self- and hetero-rating scales and information processing of negative versus positive emotional stimuli in unilateral brain-damaged patients. Results show a high positive correlation between depressive mood scores and rating of self-awareness. Left brain-damaged (LBD) patients do not differ significantly from right brain-damaged (RBD) patients on both rating scales. RBD patients mismatch negative emotional stimuli in the nonverbal matching task. Results are discussed with reference to interaction between expression of emotion and cognition. The selective neglect of negative information is interpreted as a more basic emotional impairment.  相似文献

OBJECTIVE: In this cross-sectional study, we tested whether the coping styles of emotional suppression and fighting spirit were associated with mood disturbance in cancer patients participating in professionally led community-based support groups even when demographic, medical, and group support variables were taken into account. METHODS: A heterogeneous sample of 121 cancer patients (71% female, 29% male) completed the Courtauld Emotional Control Scale (CECS), the Mini-Mental Adjustment to Cancer Scale (Mini-MAC), a measure of perceived group support, and the Profile of Mood States (POMS). RESULTS: Consistent with hypotheses, lower emotional suppression and greater adoption of a fighting spirit, in addition to older age and higher income, were associated with lower mood disturbance. Gender, time since diagnosis, presence of metastatic disease, time in the support group, perceived group support, cognitive avoidance, and fatalism were unrelated to mood disturbance. CONCLUSION: Expression of negative affect and an attitude of realistic optimism may enhance adjustment and reduce distress for cancer patients in support groups.  相似文献

Patients following brain surgery for tumour were assessed using the Emotional and Social Dysfunction questionnaire on a self-rating and partner version of the questionnaire. Analyses were performed on those patients who had self-ratings following surgery for astrocytoma (n=13), meningioma (n=26), neuroma (n=13) and pituitary adenoma (n=17). Patients with astrocytoma were rated highest when compared to the other tumour groups, although all groups of patients performed more poorly on some of the individual scales compared to a matched control group of extra-cerebral neurosurgery patients and terminally ill cancer patients. A malignant (n=48) and benign (n=33) classification similarly showed a higher partner and self-rating of malignant tumour patients. Both diagnosis and location of lesion determined outcome independently. Some differences in profile and severity between patient self-ratings and partner ratings indicate the need to survey both perspectives. This study shows a broader based emotional dysfunction in these patients which includes such prominent features such as anger, helplessness, fatigue, emotional dyscontrol, indifference, and maladaptive behaviour. These results are discussed in terms of follow-up therapeutic care and the need to further explore the relationship between lesion location and emotional profile.  相似文献

In the last 10 years a very large number of clinical and experimental investigations have been devoted to the study of the relationships between emotions and cerebral dominance. The aim of the present survey is to summarize the most important results obtained along the following lines of research: --analysis of emotional behavior associated with unilateral brain lesions; --experimental research conducted both in brain-damaged patients and in normal subjects to study the processing of emotional informations and the expression of emotions; --investigations conducted in patients with right and left temporal-lobe epilepsy; --studies conducted on psychiatric patients. Two main interpretations of the relationships between emotions and cerebral dominance are considered: a) the first assumes the existence at the level of the right and left hemisphere of two specific mechanisms underlying opposite (positive vs negative) aspects of mood; b) the second assumes, on the contrary, an overall dominance of the right hemisphere for various kinds of emotions and affects. Data in favour of either interpretation are reported and some implications for psychopathology are shortly discussed.  相似文献

Spontaneous facial expression of emotion was studied in two groups of right (N = 23) and left (N = 39) brain-damaged patients and in a control group of normal subjects (N = 28). To elicit emotions four short movies, constructed to produce positive, negative or neutral emotional responses, were used. The method used to assess the facial expression of emotions was the Facial Action Coding System. Brain-damaged patients showed less facial responses to emotional stimuli than normal controls, but no difference was observed between subjects with right and left-sided lesions either with global or disaggregated data analyses, inconsistent with the hypothesis of a specialization of the right hemisphere for facial emotional expressions. An unexpected difference was observed in response to the unpleasant movie. Both normal controls and left brain-damaged patients often averted their gaze from the screen when unpleasant material was displayed, whereas right brain-damaged patients rarely showed gaze aversion. This finding suggests that the degree of emotional involvement or manner of coping with stressful input may be reduced as a result of right brain damage.  相似文献

In an effort to discern whether cerebral vascular injuries provoke specific emotional disturbances, 20 consecutively admitted stroke patients were compared with 10 orthopaedic patients. Both groups were examined for functional disabilities (Activities of Daily Living) and for psychiatric symptoms. Reliable and valid instruments, the Hamilton Rating Scale, the Visual Analogue Mood Scale, the Present State Exam, and the Mini-Mental State Exam were employed to display the psychopathology. More of stroke patients than orthopaedic patients were depressed (45% versus 10%) even though the level of functional disability in both groups were the same. Patients with right hemisphere stroke seemed particularly vulnerable and and displayed a syndrome of irritability, loss of interest, and difficulty in concentration, in addition to depression of mood (70% of right hemisphere stroke patients versus 0% left hemisphere stroke patients and 0% orthopaedic patients). We conclude that mood disorder is a more specific complication of stroke than simply a response to the motor disability. We suggest that a controlled trial of antidepressant medication is indicated for patients with this complication.  相似文献

Cardiovascular phase, especially diastole, influences attention and the event-related potential (ERP) of the right hemisphere of the brain. Depression and schizophrenia are characterized by attentional deficits, unique lateralization of brain function, and deviant phase relationships of biological oscillators. In the present study, the ERP was recorded during stimulation triggered by diastole and systole in control (n = 16), depressed (n = 16), and schizophrenic (n = 9) subjects. Fifty tones were presented and subjects were instructed to count them silently. Previous findings were supported of delayed latencies and increased amplitude in depressed patients and decreased amplitudes and delayed latencies in schizophrenics. An exaggerated effect of diastole on the ERP in the right hemisphere was observed in depressed patients, however, no cardiovascular effect on the ERP was apparent in schizophrenic patients. Results suggested that heart/brain networks are tightly coupled in normal controls, perhaps "overdriven" in depressed patients, and uncoupled in schizophrenics.  相似文献

A system of emotional control of behavior is believed to be lateralized to the right hemisphere. Given that dopaminergic pathways are involved in affective behavior, depression, which is recognized as an integral part of Parkinson's disease, may be associated with a dopamine imbalance. The present study examined this hypothesis in patients with unilateral symptomatology indicating either left hemisphere parkinsonism (LHP) or right hemisphere parkinsonism (RHP). Sixteen patients were tested on a battery of neuropsychological tests and several scales for evaluating mood. The two groups did not differ significantly on either cognitive or emotional measures. However, RHP patients rated themselves higher on the Present Scale of Cantril, and showed some neglect of the left visual field, as compared to LHP patients.  相似文献

Disorders in nonverbal communication of emotion have been documented in patients with right hemisphere pathology; lexical expression of emotion is virtually unstudied. In this preliminary investigation, emotionally laden slides were used to elicit discourse from right brain-damaged (RBD), left brain-damaged (LBD), and normal control (NC) subjects. New techniques were developed to examine the ability of these subjects to express emotion in words; formalistic and pragmatic analyses of the discourse were conducted. RBDs, relative to NCs and LBDs, were less successful in using words to convey emotion and produced words of lower emotional intensity. LBD aphasics, despite their linguistic deficits, were comparable to NCs in conveying emotional valence. The data tend to support the speculation that the right hemisphere is dominant for lexical expression of emotion. This study has implications for the neuropsychological investigation of language, emotion, and the brain.  相似文献

Nonmotor symptoms in Parkinson's disease (PD) involving cognition and emotionality have progressively received attention. The objective of the present study was to investigate recognition of emotional prosody in patients with PD (n = 14) in comparison to healthy control subjects (HC, n = 14). Event-related brain potentials (ERP) were recorded in a modified oddball paradigm under passive listening and active target detection instructions. Results showed a poorer performance of PD patients in classifying emotional prosody. ERP generated by emotional deviants (happy/sad) during passive listening revealed diminished amplitudes of the mismatch-related negativity for sad deviants, indicating an impairment of early preattentive processing of emotional prosody in PD.  相似文献

OBJECTIVE: To study emotional behaviors in an acute stroke population. BACKGROUND: Alterations in emotional behavior after stroke have been recently recognized, but little attention has been paid to these changes in the very acute phase of stroke. METHODS: Adult patients presenting with acute stroke were prospectively recruited and studied. We validated the Emotional Behavior Index (EBI), a 38-item scale designed to evaluate behavioral aspects of sadness, aggressiveness, disinhibition, adaptation, passivity, indifference, and denial. Clinical, historical, and imaging (computed tomography/magnetic resonance imaging) data were obtained on each subject through our Stroke Registry. Statistical analysis was performed with both univariate and multivariate tests. RESULTS: Of the 254 patients, 40% showed sadness, 49% passivity, 17% aggressiveness, 53% indifference, 76% disinhibition, 18% lack of adaptation, and 44% denial reactions. Several significant correlations were identified. Sadness was correlated with a personal history of alcohol abuse (r = P < 0.037), female gender (r = P < 0.028), and hemorrhagic nature of the stroke (r = P < 0.063). Aggressiveness was correlated with a personal history of depression (r = P < 0.046) and hemorrhage (r = P < 0.06). Denial was correlated with male gender (r = P < 0.035) and hemorrhagic lesions (r = P < 0.05). Emotional behavior did not correlate with either neurologic impairment or lesion localization, but there was an association between hemorrhage and aggressive behavior (P < 0.001), lack of adaptation (r = P < 0.015), indifference (r = P < 0.018), and denial (r = P < 0.045). CONCLUSIONS: Systematic observations of acute emotional behaviors after stroke suggest that emotional alterations are independent of mood and physical status and should be considered as a separate consequence of stroke.  相似文献

Levels of aggressive behaviour on continuing care psychogeriatric wards are high. Various staffing factors, potentially amenable to improvement by training, are thought to be associated with aggressive behaviour. The relationship between aggressive behaviour in patients, measured by the Rating Scale for Aggressive Behaviour in the Elderly (RAGE), and the attitude of the nursing staff towards the patient and their perception of the patient, measured by the Modified Alienation Scale (MAS), was examined in a prospective study over 4 months on two continuing care psychogeriatric wards. There was a moderately strong correlation between the RAGE total score and the MAS total score (r = + 0.67) and its sub-scales of feeling distant from the patient (r = + 0.44), deliberate use of symptoms by the patients (r = + 0.36), alienation of the patient (r = + 0.85) and variable mood in the patient (r = + 0.77). Intervention studies designed to either directly reduce aggressive behaviour or reduce aggressive behaviour by improving staff attitude and perception, or both, in a range of settings, are suggested to identify clinically effective and cost-effective strategies for reducing aggressive behaviour.  相似文献
