既然牙种植体的成功率是可靠的 ,学术界的注意力正从描述种植体的成功转向对种植体失败因素的细致分析。该研究的目的 1Bicom种植体 1-5年的存留率 ;2从客观的统计学角度明确牙种植体失败的相关危险因素。根据研究目的 ,我们采用了回顾性队列研究设计 ,选择的研究对象为一个或多个种植体植入的患者 ,预测对象包含 ;人口统计、健康状况调查、解剖因素、种植体相关固位钉、修复相关因素、围手术期因素和其它相关因素。主要考查的结果变量是种植体取出 ,定义为种植失败。总的种植体成功率 1采用 kaplan-Meier分析法评估种植体总的成功率。采用…  相似文献

背景:膜引导骨再生技术是利用膜的机械屏障原理,阻挡软组织长入种植体周骨缺损,使成骨细胞在膜下空间中优势生长,形成新骨充填骨缺损。 目的:于上前牙种植中应用引导骨再生技术,观察其对修复效果的影响。 方法:74例单颗前牙种植成功患者按照随机数字表法分组为对照组与观察组,各37例。对照组采用上前牙二期种植,但未行骨增量;观察组采用引导骨再生技术实施上前牙种植,待其种植体初期稳定后植入人工骨粉。于术后当天、种植牙负重6个月、12个月比较2组患者软组织红色美学指数及种植体周围骨水平。 结果与结论:2组患者种植牙负重6,12个月红色美学指数值较基线水平比较均明显提高(P < 0.05),但2组间比较差异无显著性意义(P > 0.05)。种植牙负重12个月时,除唇侧龈缘水平和牙槽突外形外,2组患者红色美学指数其余各项指数值较基线水平均明显升高(P < 0.05);但2组间红色美学指数各项指数值比较(P > 0.05)。种植牙负重12个月时,2组患者种植体边缘骨水平值比较差异无显著性意义(P > 0.05)。结果说明相比正常骨组织种植修复来说,临床应用引导骨再生技术于上前牙种植中修复可取得较好效果,且两者修复效果无明显差异性。  中国组织工程研究杂志出版内容重点:组织构建;骨细胞;软骨细胞;细胞培养;成纤维细胞;血管内皮细胞;骨质疏松;组织工程全文链接:  相似文献

目的:探讨口腔种植修复牙列缺损的临床分析。方法回顾性分析我院2009年1月~2011年12月入院治疗的60例牙列缺损患者的临床资料,实施手术前对患者进行详细资料收集和口腔检查,局麻后对患者进行口腔种植修复。根据种植体的舒适程度和美观程度。结果舒适程度满意的有45例(75%),较满意的有12例(20%),不满意的有3例(5%);美观程度满意的有41例(68.33%),较满意的16例(26.67%),不满意的有3例(5%)。结论口腔种植修复牙列缺损的效果较好。  相似文献

背景:目前国内广泛使用的种植模板是在石膏模型上制作完成,由于无法明确植入区颌骨的三维结构,因此不能准确地指导种植体植入,容易导致植体植入并发症。 目的:利用CT技术、计算机辅助设计/计算机辅助制作技术设计制作口腔种植模板,以准确定位种植体植入的位置和方向。 方法:CT扫描带有定位标记的上颌游离缺失石膏模型。在软件MIMICS中进行三维重建和种植模拟。根据受植区颌骨的解剖结构兼顾修复学和美学的要求设计种植方案,并将虚拟种植体放置在理想的位置上。设计模板形态同时将虚拟种植体位置角度信息转移到模板上,导入快速成型设备制作树脂模板。在模型上完成种植然后评价模板的精度。 结果与结论:种植体植入后顶端偏差为(?0.87±0.21)mm,底端偏差为(0.82±0.16)mm。种植体在垂直方向上的偏差较颊舌向和近远中向略大。采用此方法可以将种植前设计准确地转移到种植模板中,从而更有效地指导种植,有助于提高种植的安全性、准确性和成功率。  相似文献

背景:影像学检查可及时了解牙种植位置信息,但目前常用的数字化颌骨曲面断层对判定骨的高度和宽度存在误差。 目的:比较数字化曲面断层与锥体束CT在牙种植中的应用价值。 方法:选取要求行种植治疗修复缺失牙,牙周状况良好,符合种植术适应证的患者53例(65颗),种植前以一直径为3 mm的钢球为标记物置于患者待种植区,锥体束CT及普通数字化曲面断层分别测量三维重建图像及曲面断层片中标记物的直径,并与真实值进行比较,分析二者骨整合检出率差异。 结果与结论:相比于数字化曲面断层,口腔种植前锥体束CT测量的小钢球直径与真实值差异较小(P < 0.01)。种植后3个月,锥体束CT和普通数字化曲面断层得出的未发生骨结合的检出率接近(P > 0.05)。说明与普通数字化曲面断层相比,锥体束CT可以更准确的评估种植前牙槽骨的骨量,并可在种植后检查中更清晰的反映种植体周围骨质情况。  相似文献

目的 探讨不同牙槽窝形态上颌中切牙即刻种植即刻负重时,植入位点及轴向对种植体周围骨界面应力分布的影响。方法 参照1名健康成年人口腔锥体束计算机断层(cone beam computed tomography, CBCT)影像资料,建立偏唇型、中间型、偏腭型3种牙槽窝形态的上颌中切牙即刻种植即刻负重三维有限元模型;模拟不同植入位点(根尖位点、偏腭/唇侧位点)及轴向(牙长轴、牙槽骨长轴);对已建模型以100 N力进行不同角度(0°、30°、45°、60°、90°)的应力加载;应用ANSYS软件分析种植体周围牙槽骨的应力情况。结果 成功建立12个不同牙槽窝形态上颌中切牙即刻种植即刻负重三维有限元模型。偏唇型及中间型牙槽窝形态行即刻种植即刻负重时,沿牙槽骨长轴方向偏腭位点植入种植体更易获得良好的种植体骨界面生物力学特性;偏腭型牙槽窝形态行即刻种植即刻负重时,在偏唇位点植入,不论是沿牙长轴方向还是沿牙槽骨长轴方向植入种植体,种植体周牙槽骨所受等效应力远小于根尖位点植入。结论 不同牙槽窝形态、植入位点及轴向都会对上颌中切牙即刻种植即刻负重种植体骨界面生物力学特点产生影响。临床中,应针对不同牙槽窝形...  相似文献

背景:计算机辅助设计制造的手术导板辅助的不翻瓣种植导航能准确地将种植体植入到术前设计位置,在计算机制造的牙颌模型上手工制作的导板同样能够用在牙槽骨萎缩患者的种植修复中。 目的:观察利用自制导板对骨宽度萎缩的上颌前磨牙缺失患者实施不翻瓣种植导航手术临床效果,评价此种方法的临床应用可行性。 方法:8例可用骨宽度不充足的上颌前磨牙缺失并需要进行种植义齿修复患者共计10颗患牙,平均前磨牙区牙槽骨高度为(13.45±1.67) mm。通过颅颌面CT扫描、三维重建分析以及快速成型模型模拟手术分析,膜片热压成形方法制作手术导板,精确测量种植区域可用牙槽骨长度、宽度、高度及角度。 结果与结论:种植导板固位力合适,取戴方便。与模型模拟手术比较,10颗缺失牙术后种植体植入位置准确,无上颌窦底黏膜穿通、骨壁侧穿、邻牙损伤等并发症,植入手术平均用时为(17.0±5.8) min。除1例患者因上颌窦黏膜下存有断根植入10 mm长种植体外,其余患者种植体植入长度均>13 mm。患者术后仅有轻度肿痛。植入后3~6个月行烤瓷冠修复。提示在种植手术导板的引导下,应用不翻瓣种植导航手术可将种植体准确植入到可用骨宽度不足的上颌前磨牙缺失患者术前预设计位置,表明其在牙槽骨萎缩、可用骨宽度不足患者治疗中具有较为明显的技术优势与广阔的应用前景。  相似文献

BACKGROUND: Implant-abutment connection has become a key factor in the success of implant restoration, but  the effects of different implant-abutment connections on peri-implant tissues is unclear. OBJECTIVE: To compare the effects of three different implant-abutment connections on the alveolar bone and peri-implant soft tissues at the maxillary esthetic zone. METHODS: Sixty patients undergoing aesthetic restoration of single maxillary anterior tooth were enrolled randomly, including 20 cases of Bego Replace system, 20 of Osstem Replace system, and 20 cases of ITI Replace system. All the patients had a follow-up of 3 to 6 months after the surgery, and then received the two-stage surgery. Radiographic and clinical examinations were made at the same time. Peri-implant bone height measured was used as the reference line. The final crowns were made after 2 weeks, and the peri-implant bone resorption and pink esthetic scores were evaluated after 12 months shown through X-ray films. RESULTS AND CONCLUSION: There were significant differences in peri-implant marginal bone loss and pink esthetic scores among different groups after implant restoration. The amount of marginal bone loss around implants using platform switching was significantly less than that using platform butt-joint (P < 0.0.5), and the pink esthetic scores of the former group was higher than the later one (P < 0.05). These findings indicate that aesthetic restoration of a single maxillary tooth implant using platform switching is able to preserve better marginal bone and acquire better aesthetic effects.    相似文献

Objective: The application effect of xenograft materials in patients with implant restoration after minimally invasive tooth extraction is explored. Methods: From February 2016 to December 2019, 80 patients with minimally invasive dental implants were treated in our hospital and selected as the research object. They were divided into the observation group and the control group based on to the random number table methods, and 40 patients in each group. The patients in the control group were sutured directly after minimally invasive tooth extraction. After the minimally invasive tooth extraction in the observation group, Bio-Oss Collagen was implanted into the alveolar fossa, which was covered with Bio-Gide absorbable biofilm. Changes in the height of the mastoid, the width and height of the crest crest, and the width of the lip between the two groups of patients immediately and 6 months after the tooth extraction were compared.Results: Changes of the alveolar ridge height and width in the observation group were less than those in the control group, and the difference was statistically significant(t=5.816, 3.997,and P=0.000); the width of lip and tongue and the height of the interdental mastoid in the observation group. The amount of change was less than that in the control group, and the difference was statistically significant(t=7.384, 10.686,and P=0.000). Conclusion: Bio-Oss Collagen and Bio-Gide absorbable biofilms have significant clinical effects on patients with minimally invasive tooth extraction and implant restoration, helping to maintain the stability of the alveolar ridge height and width, which is conducive to the creation of later dental implants in good conditions.  相似文献

背景:IPS e.max Press热压铸全瓷以其较好的美观效果和生物学性能已被越来越多地应用于临床修复。 目的:分析IPS e.max Press全瓷冠用于前牙美学修复的边缘适合度及颜色匹配等情况。 方法:纳入前牙美容修复患者52例(138颗牙),其中男20例,女32例,年龄18-45岁,前牙缺损62颗,牙外伤46颗,畸形小牙30颗,分为2组修复,治疗组(n=26)采用IPS e. max Press全瓷冠和传统钴铬合金烤瓷冠修复,对照组(n=26)采用传统钴铬合金烤瓷冠修复,修复后1周复诊,调查患者满意度;修复后随访6个月、2年,评估修复体完整性、边缘适合度、牙龈健康状况及颜色匹配等情况。 结果与结论:修复后1周,治疗组患者对修复体颜色、外形及舒适度方面的满意度显著高于对照组(P < 0.05);治疗后6个月,治疗组边缘适合度、牙龈健康状况及颜色匹配显著优于对照组(P < 0.05);治疗后2年,治疗组边缘适合度、牙龈健康状况及颜色匹配显著优于对照组(P < 0.05)。表明IPS e.max Press全瓷冠修复前牙具有良好的边缘适合度、牙龈健康状况及颜色匹配等生物学特性,患者满意度高。 中国组织工程研究杂志出版内容重点:生物材料;骨生物材料; 口腔生物材料; 纳米材料; 缓释材料; 材料相容性;组织工程  相似文献

【摘 要】 目的:观察不同修复材料用于前牙美容修复的效果及远期随访结果。 方法:选择80例行前牙美容修复患者进行前瞻性研究,按随机数表法分为A组(40例,58颗患牙)与B组(40例,62颗患牙)。A组采取3M-Z350纳米复合树脂进行修复,B组采用3M-Z100复合树脂进行修复。比较两组术后1 d、术后1周、术后1个月及术后3个月的敏感发生率、疼痛[采用视觉模拟评分法(VAS)评估]及随访3年的边缘密合度、颜色匹配度、修复体完整度。 结果:A组术后1 d、术后1周、术后1个月的患牙敏感发生率分别为8.62%、5.17%、3.45%,显著低于B组的22.58%、17.74%、14.52%(P<0.05)。两组术后3个月的患牙敏感发生率分别为6.91%与16.13%,差异无统计学意义(P>0.05)。A组术后1 d、术后1周、术后1个月的VAS评分分别为2.51±0.54、2.03±0.47、1.58±0.38,均显著低于B组的2.72±0.59、2.35±0.50、1.81±0.42(P<0.05)。两组术后3个月的疼痛VAS评分未见统计学意义(P>0.05)。两组随访3年的修复体完整度差异无统计学意义(P>0.05)。A组随访3年的边缘密合度与颜色匹配度显著优于B组(P<0.05)。 结论:3M-Z350纳米复合树脂用于前牙美容修复的术后敏感发生率低于3M-Z100复合树脂,疼痛更轻,且远期随访的良好边缘密合度与颜色匹配度均更佳,值得推广。  相似文献

钛(Ti)及其合金作为20世纪末兴起的种植体材料,具有高强度、低重量、生物相容性好等优势,随着种植体技术的快速发展以及临床上需求的不断增加,Ti种植体迅速且广泛的应用于骨科治疗之中,特别是针对牙缺损的修复。然而,由于骨整合速率缓慢、细菌感染以及过量活性氧累积等多种因素导致种植体早期植入稳定性差,致使植入人体后服役寿命短,成为限制牙种植体植入成功率的重要因素。因此,种植体的临床早期植入稳定性有待进一步提高。从影响临床早期植入稳定性的材料因素、生物因素以及其它因素等3个方面,综述国内外Ti基牙种植体的相关研究,以及提高临床早期植入稳定性的研究现状和发展动态,为牙种植体植入成功率的提高提供参考。  相似文献

Dental implants have been considered to be a successful treatment modality. Recently, achieving a good osseointegration is not the ultimate goal for the restorative dentist. Successful implant treatment demands the best gingival esthetic success along with stability and function of the implant. This study was performed to obtain answers to some controversial points pertaining to esthetics and function of implants in maxilla. Immediate flapless implantation into the extraction sockets in maxillary anterior zone is an emerging treatment option in dentistry--the esthetic success of which was in debate for long. The proposed study compared the esthetic success of immediate flapless implants (ILA), to immediate implants with the need for flap (ILB) and, delayed implants (DSL) in single tooth restorations, in the anterior region of the maxilla. The other aim of the study was to find out if any relation exists between the interproximal crestal bone height and papilla height. Analysis was done irrespective of treatment procedure in the same study group using periodontal sounding and radiographs to find out the relation. From the study involving 106 participants, including 21 ILA, 22 ILB and 63 DSL cases, we received highest papillary index score of 2.6 average from group ILA, followed by ILB and DSL, after 3 months of prosthetic loading. From the periodontal sounding and radiographic study it was evident that, when the distance between the base of the contact point of crowns and height of interproximal bone was less than 5, the papilla was present 100% of the time, but when the distance increased to 6 and more than 7 mm, the papilla was present only 46.5 and 24 percentage of the time respectively.  相似文献

To provide an anatomical basis for clinical implant esthetics,we evaluated the morphology of the nasopalatine canal(NPC) and analyzed labial and interproximal bone anatomy at the maxillary anterior region.We sought to investigate the effect of maxillary protrusion and tooth labiolingual inclination on labial bone anatomy in Chinese adults.Three dimensional(3D) images were reconstructed using cone-beam computed tomography(CBCT) images from 80 Chinese subjects and by SimPlant 11.04.The dimensions of the NPC,the thickness and profile of the labial bone,the width and height of the interproximal bone,angle sella-nasion-subspinale(SNA) and angle upper central incisor-nasion,subspinale(U1-NA) were measured.The incisive foramen of the NPC was markedly wider than its nasal foramen.The dimension of its labial bone wall demonstrated an increasing width from the crestal to apical measurements.The labial bone at the maxillary anterior region was rather thin,especially at 3 mm below the cemento-enamel junction(CEJ) and the mid-root level;the profile of the labial bone was more curved at the central incisor,and the interproximal bone became wider and shorter posteriorly.There were significant relationships between maxillary protrusion and labial bone profile,tooth labiolingual inclination and labial bone thickness(P 〈 0.02).To achieve optimal esthetic outcome of implant,bone augmentation is necessary at the maxillary anterior region.For immediate or early placement at the maxillary anterior region,the implant should be located palatally to reduce labial bone resorption and marginal recession;its apex should be angulated palatally to avoid labial perforation at the apical region.To protect the NPC,implants at the central incisor region should be placed away from NPC.  相似文献

目的 建立兔下颌骨前牙即刻种植种植体周围骨缺损的动物模型,考察牙髓干细胞(DPSCs)在种植体周围骨缺损中的骨再生能力.方法 将8只新西兰白兔随机分为两组,分别拔除兔双侧下颌前牙,在拔牙窝颊侧建立2 mm×3 mm骨缺损区,即刻植入种植体,对照组植入Bio-oss骨粉和磷酸盐缓冲液,实验组植入Bio-oss骨粉和DPSCs.通过大体观察和组织学观察(苏木精-伊红染色、Goldner's三色染色和扫描电镜观察)评价植入后4周骨缺损区的骨再生能力.结果 所有实验兔种植体植入顺利,且种植体植入后均稳固,无明显差异.苏木精-伊红和Goldner's三色染色观察均可见实验组有新生骨组织形成,且部分新生骨组织为编织骨,骨细胞体积大、数量多,呈编织状排列,骨细胞分化成熟;而对照组可见少量新生骨样组织形成,骨细胞分化较成熟.扫描电镜结果显示,实验组新生骨与种植体间的骨结合较对照组多.结论 与单独使用Bio-oss骨粉比较,DPSCs与Bio-oss骨粉联用的成骨能力相对较强,新生骨组织与种植体的结合能力更好.  相似文献
