A 20-year-old female with a left-sided concealed atrioventricular accessory pathway was studied during programmed stimulation to investigate the mechanisms of prevention of tachycardia by timed stimulation of the ventricle after a tachycardia initiating atrial extrastimulus. After the atrial tachycardia initiation zone was determined, the prvention zone of the ventricular stimulus was determined for each initiating interval. Two different prevention mechanisms were found, one whereby the ventricular depolarization was blocked retrogradely in the accessory pathway and another whereby the ventricular depolarization was conducted over the accessory pathway to the atrium but subsequently blocked anterogradely in the AV node. The preventive zone timed from the last ventricular complex to the ventricular stimulus was always greater than 30 msec whereby the minimum preventive internal was determined by the ventricular refractory period. This could be of value when such a preventive algorithm is incorporated into a dual-chamber antitachycardia pacemaker. Knowing the exact mechanisms by which the prevention occurs allows one to predict how tachycardia can be prevented when the atrial extra beat originates from a different site and when the parameters of the heart are changed due to changes in antonomic tone or catecholamine level.  相似文献

Among a consecutive series of 600 patients who underwent radiofrequency catheter ablation for AV node reentrant tachycardia, 14 patients (age 29-76 years) had a prolonged AH interval during sinus rhythm (172±18 ms, range 140 to 200). Seven of them had unsuccessful ablation during the previous ablation sessions. Eight patients with anterograde dual AV node pathway physiology received anterograde slow pathway ablation, and the other 6 patients without dual-pathway physiology received retrograde fast pathway ablation. All patients had successful elimination of AV nodal reentrant tachycardia after a mean of 4±4 radiofrequency applications, power level 36±6 watts and a pulse duration of 42±4 seconds. The postablation AH interval remained unchanged. During a follow-up period of 25±13 months, one patient who received slow pathway ablation developed 2:1 AV block with syncope. As compared with the other 586 patients without a prolonged AH interval, these 14 patients had significantly poorer anterograde AV nodal function and lower incidence of anterograde dual AV node physiology (P<0.01). We concluded that slow pathway ablation in patients with dual pathway physiology, and retrograde fast pathway ablation in patients without dual pathway physiology were effective and safe in patients with a prolonged AH interval. However, delayed onset of symptomatic AV block is possible and careful follow-up is necessary.  相似文献

Ablation of Concealed Accessory Pathways. Introduction: Feasibility of radiofrequency (RF) ablation using a two-catheter technique without coronary sinus catheterization was studied in 100 consecutive patients with a single concealed left free-wall accessory path-way.
Methods and Results: Tachycardia was induced by electrical stimulation in the right atrium/right ventricle, and the presence of a concealed left free-wall accessory pathway was suggested electrocardiographically (negative P wave in leads I and/or a VL during orthodromic tachycardia) or by earlier atrial activation in the pulmonary artery compared to the high right atrium. Mapping of the mitral annulus was performed during right ventricular pacing or orthodromic tachycardia, and RF energy was applied at the site with the earliest retrograde atrial activation. Ablation was considered effective if tachycardia could not be induced, and if VA dissociation or exclusive retrograde nodal conduction was observed. Ablation was initially successful in 98 of 100 patients. Mean number of radiofrequency pulses were 3.2 ± 2. Mean fluoroscopy time and total procedure time was 14 ± 9 and 107 ± 32 minutes, respectively. There were no complications related to the procedure. At a mean follow-up of 22 ± 13 months, two patients experienced tachycardia recurrence and required a second procedure, which was successful.
Conclusions: Our results suggest that RF catheter ablation of concealed left free-wall accessory pathways can be safely, effectively, and rapidly performed using a simplified two-catheter technique with no need for coronary sinus catheterization.  相似文献

VA Interval Via Accessory Pathway During Bundle Branch Reentry. Bundle branch reentrant (BBR) complex is commonly induced during programmed ventricular stimulation with single ex-trastimulus. In patients with atrioventricular accessory pathway, BBR beat frequently triggers orthodromic tachycardia. This study was designed to determine whether evaluation of the ventriculoatrial conduction time during BBR (VABBR) induced with right ventricular extrastimulation (i.e., left bundle branch block morphology) can separate left free-wall (LFW) accessory pathways from left posteroseptal (LPS) or right-sided pathways. Thirty-eight patients with single atrioventricular accessory pathways were included. There were 28 men and 10 women with a mean age of 26 years. The accessory pathway was localized in LFW in 23 patients (group I) and LPS in seven (group ID. Eight patients (group III) had pathways located in the right side. In each patient, VABBR was determined and compared with the following: (1) V2A2 interval exclusively via accessory pathway; and (2) ventriculoatrial conduction time during orthodromic tachycardia with narrow QRS complex (VANQ), left bundle branch block plus normal axis (VALB-NA) or left axis (VALB-LA). In group I, VABBR values (170–245 msec, mean 196.1 ± 20.5 msec) were 0–25 msec longer than V2A2 (170–245 msec, mean 191.3 ± 19.1 msec) and 45–125 msec greater than VANQ (100–155 msec, mean 125.6 ± 14.1 msec). VABBR was identical to VALB-LA but 25–55 msec greater than VA,LB-NA (140–205 msec, mean 160.9 ± 20.8 msec). In group II, VABBR values (100–140 msec, mean 118.6 ± 14.3 msec) were 15–30 msec shorter than V2A2 (125–165 msec, mean 140.7 ± 14.3 msec) and 15–25 msec longer than VANQ (85–120 msec, mean 100.7 ± 12.0 msec). Comparing VABBR with VALB-NA or VALB-LA did not show any statistically significant difference. In group III, VABBR values were consistently shorter than V2A2 and identical to VANQ. Thus, assessment of VABBR is a simple and useful method that can be reliably utilized to differentiate LFW pathways from LPS or right-sided pathways. BACKGROUND: Para-Hisian pacing is an effective method of differentiating between pathways for retrograde conduction over the accessory pathway (AP) and over the atrioventricular node (AVN). When performing para-Hisian pacing, the pacing spike sometimes captures only the His bundle, which we named "pure" Hisian pacing (Hc). OBJECTIVE: We evaluated the significance of pure Hisian pacing for predicting the pathways of ventriculoatrial conduction. METHODS: In 62 patients with supraventricular tachycardia, both para-Hisian and pure Hisian pacing were carried out during the sinus rhythm, resulting in three different types of electrocardiographic complexes with wide (local ventricular myocardial capture), slightly narrow (both local myocardial and His bundle capture), and very narrow QRS widths (Hc). A change of atrial activation sequence as demonstrated by these pacing modes indicated the presence of multiple retrograde pathways. The diagnosis of retrograde pathways by para-Hisian pacing with or without Hc was evaluated. RESULTS: In 22 patients with AVN reentrant tachycardia, para-Hisian pacing alone was able to correctly predict ventriculo-atrial conduction exclusively through the AVN without requiring findings from Hc. In 40 AP patients, para-Hisian pacing showed a pattern of retrograde conduction through the AVN in six, through both the AVN and AP in 10, and through an AP in 24 patients. Four of these 24 patients were diagnosed as having multiple pathways (AP+AVN or dual APs) by the addition of Hc. CONCLUSION: Pure Hisian pacing can help disclose another pathway for retrograde conduction in AP patients, which is unpredicted by ordinary para-Hisian pacing.  相似文献

We report 4 cases of congenital and acquired coronary sinus ostial atresia incidentally found during electrophysiologic assessment for supraventricular arrhythmias. Congenital variants consisted of coronary sinus drainage predominantly via persistent left superior vena cavae and partial coronary sinus unroofing into the left atrium. The acquired variant was inadvertently produced during surgery for cor triatriatum. A variety of electrophysiologic substrates including right and left-sided accessory pathways and both typical and atypical AV nodal reciprocating tachycardia were identified. Approaches to imaging and accessing the coronary sinus when the os cannot be cannulated are discussed, including a search for right atrial accessory venous collaterals, venography to rule-out a persistent left superior vena cava, and coronary angiography.  相似文献

ABSTRACT We have observed two patients with long QT interval, ventricular tachycardias of “torsade de pointe” type and repeated ventricular fibrillation episodes, who also turned out to have significant hypothyroidism. This was suspected from the clinical picture in one patient and after haematological test in the other. In addition to hypothyroidism, both patients had associated factors which may have contributed to the development of the arrhythmia. After having reached an euthyroid state, both patients normalized their QT intervals, were relieved from earlier symptoms of cardiac arrhythmias and exhibited no longer any documented arrhythmia. Before thyroid substitution, both patients had marked signs of delayed ventricular repolarization even by invasive electrophysiological methods. Our observations indicate that hypothyroidism should be considered a possible primary cause in cases with long QT syndrome (LQTS). Furthermore, the possibility of LQTS in patients with hypothyroidism should be considered.  相似文献

Background

Supraventricular tachycardias (SVT) are a common arrhythmia therefore an accurate diagnosis is of clinical importance. Although an ECG performed during tachycardia greatly aids diagnosis, patient history and predisposing factors also improve diagnostic accuracy.

Methods

This prospective study included 100 consecutive patients undergoing electrophysiological study for SVT with the aim to reassess their clinical characteristics and describe frequent predisposing factors, such as the “sign of lace‐tying” that to our knowledge has not previously been reported. Each patient completed an extensive questionnaire (70 questions) during their hospital stay.

Results

Our series comprised: 67% of patients with atrioventricular nodal reentrant tachycardia (AVNRT); 24% with an accessory pathway; and 9% presented atrial tachycardia. Half of the population were male and 29% of the cohort presented hypertension. Syncope during tachycardia appeared in 15% of patients, dizziness in 52% and thoracic pain in 59%. We encountered a predisposing risk factor for SVT in 53% of cases; with 32% exhibiting an anteflexion of the trunk termed the “sign of lace‐tying.” Data also showed that younger patients tended to present AVRT and regular pounding in the neck appeared only in patients with AVNRT.

Conclusions

Overall, our study has highlighted the importance of considering clinical signs and patient characteristics both before and during SVT for the precise diagnosis of paroxysmal SVT. Furthermore, 32% of patients presented the “sign of lace‐tying” or body position change before SVT, implying a diagnosis of SVT.

We report on a case of coronary embolization of an elastic membrane fixing the ACS RX Multilink stent over its balloon, after a successful stent delivery. The membrane was dislodged from the balloon in the ostium of the right coronary artery as the delivery balloon was being pulled back into the guiding catheter. All attempts to retrieve the membrane, to cover it with another stent, or to push it into a terminal segment of the artery to limit the jeopardized myocardial mass were unsuccessful. No CABG was performed because the left coronary system was normal and the procedure occurred 2 months after an incomplete inferior myocardial infarction. There was no increase in cardiac enzymes nor electrocardiographic signs of a new myocardial infarction at discharge or at 1-mo follow-up. It seems preferable to avoid further use of stents tied to their balloons with an intermediary, and possibly detachable, element. Cathet. Cardiovasc.
