Ocular trauma-related glaucoma

Glaucoma following ocular injury is a multifactorial condition. Management of posttraumatic ocular hypertension is especially critical in that many of the complications that may develop are preventable. Management is aimed at preventing further damage to the injured eye.

Rhegmatogenous retinal detachments with cytomegalovirus retinitis

The most important ocular opportunistic infection in patients with AIDS is cytomegalovirus (CMV) retinitis. Management of CMV retinitis has become complex, as the life expectancy of many patients has increased exponentially. Most retinal detachments in CMV retinitis are rhegmatogenous in nature. Because of atrophic changes in the retina and alterations in the vitreous, surgical management of these detachments leads to a stepladder approach. Options include laser demarcation, scleral buckle, and vitrectomy with silicone oil. In some patients, a combination of all three procedures may be necessary. Management in all cases should be individualized, with the realization that failure of retinal reattachment may occur.

Risk adapted management of central retinal vein occlusion

Für das Management von Patienten mit Zentralvenenthrombosen existiert keine Standardtherapie. Nur für wenige der bisher angewandten therapeutischen Strategien wurde eine Wirksamkeit in gro?en Patientenkollektiven nachgewiesen, z. T. wird über ein ungünstiges Nebenwirkungsprofil berichtet. Ziel der Akuttherapie mu? die Verbesserung der Restperfusion sein, wobei prognostisch relevante Faktoren wie die Aktivit?t von Komponenten des Gerinnungssystems, Blutflu?parameter und pr?disponierende Systemerkrankungen berücksichtigt werden sollten. Ein risikoadaptiertes Management schlie?t eine frühzeitige Heparinisierung ein. Weitere therapeutische M?glichkeiten sind die H?modilution, Fibrinolyse und die hyperbare Sauerstofftherapie. In prospektiven Studien mu? die Wirksamkeit neuer effizient erscheinender Therapieoptionen an stratifizierten Patientenkollektiven untersucht werden.

Dematiaceous fungal keratitis. Clinical isolates and management.

Clinical and laboratory features of 16 cases of keratitis that were caused by dematiaceous pigmented fungi are reported. Management, including the treatment of nine cases with Natamycin (Pimaricin), resulted in corneal healing in 14 cases, and therapeutic surgery in two cases.     

PLATEAU IRIS

Acute glaucoma can recur following peripheral iridectomy for angle closure glaucoma. One type is caused by iris folding into and occluding the angle following pupil dilatation. Affected eyes have a characteristic type of angle recess. Management involves awareness, prevention and miosis.     

An unusual intracerebral foreign body asSociated with eye trauma

A case report is presented of penetrating orbital trauma in the driver of a motor vehicle which collided with a kangaroo. A tooth fragment from the kangaroo traversed the orbit, lodging intracranially. Management of the case is described and appropriate choice of antibiotic regime discussed.     

The treatment of animal bite injuries of the eye and ocular adnexa

Animal bites to the eye and ocular adnexa may result in significant morbidity. Management includes wound care and surgical repair of traumatized tissue, treatment of infection (most commonly Pasteurella multocida), appropriate tetanus and rabies prophylaxis, and notification of state public health officials.
