噬血细胞综合征是一类由原发性或继发性免疫异常导致过度炎症反应的急危重症疾病,儿童较成人常见。本文报告1例癫痫性精神病患者在使用拉莫三嗪等抗癫痫药物治疗过程中出现反复发热、皮疹,外周血两系减少(白细胞、血小板),肝功能受损,脾大,铁蛋白增加,骨髓穿刺发现嗜血细胞,明确诊断为噬血细胞综合征,使用糖皮质激素治疗效果显著。本病例提出了拉莫三嗪可能导致严重免疫系统反应-嗜血细胞性淋巴组织增多症,早期快速鉴别、诊断、治疗,可显著改善患者结局。  相似文献

目的 探讨硫辛酸(lipoic acid,LA)对MPTP诱导的C57BL/6帕金森病(Parkinson's disease,PD)小鼠模型黑质及纹状体TH、parkin、PINK1、DJ-1自噬相关蛋白表达水平的影响。方法 将60只C57BL/6小鼠随机分为PD模型组、正常组、治疗组、预保护组,采用背部皮下注射MPTP制作PD模型(每组各15只),行为学评价造模,Western Blotting法检测黑质、纹状体TH、parkin、PINK1、DJ-1蛋白的表达水平,免疫组化法观察黑质及纹状体阳性神经元数,免疫荧光检测PINK1的表达水平。结果(1)与正常组比较,PD模型组小鼠黑质、纹状体内TH、Parkin、PINK1、DJ-1的表达水平均明显降低(P<0.01);(2)与PD模型组比较,预保护组黑质、纹状体内TH、PINK1、Parkin、DJ-1表达水平均明显增高(P<0.05);(3)与治疗组比较,预保护组小鼠黑质、纹状体内TH、PINK1、Parkin表达水平略增高,预保护组DJ-1表达水平略降低,但两组比较无统计学差异(P>0.05)。结论 线粒体自噬参与PD小鼠的发病过程; 硫辛酸可能通过上调PD小鼠自噬相关蛋白的表达水平而发挥对多巴胺神经元的保护作用,从而为PD的防治提供新的思路。  相似文献

自噬是细胞在各种应激条件下诱导的高度保守的分解代谢过程,可防止细胞损伤并在能量或营养缺乏的情况下促进细胞生存,并对各种细胞毒性损伤做出保护反应。因此该过程的功能障碍会导致许多人类疾病发生,已有研究表明其与神经退行性疾病、炎症性疾病和癌症之间存在明确的病因学联系。自噬通过多条信号通路调节髓母细胞瘤(MB)的发生发展。因此,调控自噬通路可能成为MB的新型治疗方案,该文将对近年来MB与自噬相关调控机制的研究进展做一综述。<title/>国际神经病学神经外科学杂志, <div>自噬(Autophagy)是一种细胞自我消化的过程,其可以将损伤的、错误折叠的以及无功能的蛋白质吞噬并通过溶酶体发生降解。自噬发生于肿瘤进展的不同阶段,近来越来越多体内体外试验证据表明自噬在促进胶质瘤(Glioma)增殖以及抑瘤方面均发挥了重要作用。在抑瘤方面,胶质瘤应激状态下可发生致死性自噬,促进胶质瘤细胞的死亡。在促瘤方面,胶质瘤术后给予化疗,胶质瘤细胞也可发生一种保护性自噬,最终会导致替莫唑胺(TMZ)耐药,大幅减弱治疗效果。本文将对自噬在胶质瘤中的双重作用,基于自噬调节通路基础上研究的胶质瘤治疗方案以及近年来的新进展作一阐述。  <a <div>目的评价库欣病患者经鼻蝶垂体腺瘤切除术(TSS)术前术后的血细胞、血电解质及血糖血压水平与其手术缓解效果的相关性,探讨血细胞、血电解质等常规检查对库欣病患者的治疗价值,评估这些实验室一般检查对库欣病手术疗效及诊断的指导意义。方法以2003年1月至2009年12月经病理证实为垂体促肾上腺皮质激素(ACTH)腺瘤的101例库欣病患者为研究对象,均行经鼻蝶窦垂体腺瘤切除术。按库欣病缓解标准分为手术缓解组(A组)和未缓解组(B组),收集所有患者术前、术后及术后跟踪随访(术后1个月、3个月、6个月及1年之后)的血细胞(白细胞、中性粒细胞、嗜酸性粒细胞、淋巴细胞和红细胞)、电解质(钠离子、钾离子和钙离子)水平及术前术后的血压、空腹血糖的水平。结果 A组与B组的术前术后1个月、3个月、6个月及1年随访中在血细胞、电解质方面具有显著性差异。术前术后的血压及空腹血糖也有显著性的变化。结论 TSS治疗可改善库欣病患者术前血细胞、电解质的异常水平;缓解组与未缓解组的血细胞和电解质水平有显著差异,术前术后短期内的血压、空腹血糖也有显著变化。相比耗费大、条件苛刻的内分泌学检查项目,库欣病患者的血细胞、电解质水平的动态变化以及术后血压及血糖的改变也可作为评估垂体瘤手术疗效的重要指标,这在临床上有着不可小视的作用。  <a 61例垂体腺瘤手术标本按侵袭性垂体腺瘤综合判定方法分为侵袭组和非侵袭组,采用免疫荧光检测Beclin-1和MAPLC3蛋白的表达,RT-PCR法检测Beclin-1和MAPLC3mRNA的表达,分析侵袭组和非侵袭组Beclin-1和MAP1LC3表达水平的差异。结果 61例垂体瘤患者中,32例为侵袭性垂体腺瘤,29例为非侵袭性垂体腺瘤。侵袭性垂体腺瘤组Beclin-1的蛋白和mRNA表达均低于非侵袭性腺瘤组(P0.05),MAPLC3的蛋白和mRNA表达也均低于非侵袭性腺瘤组(P0.05),差别具有统计学意义。结论自噬相关基因Beclin-1和MAPLC3在侵袭性垂体腺瘤中的表达下调,提示其可能与侵袭性垂体腺瘤的侵袭性有关。  <a
