Purpose:To assess the change in Near Induced Transient Myopia (NITM) and other accommodation parameters, before and after undergoing LASIK surgery for myopia correction.Methods:Twenty-nine myopic subjects were recruited from a tertiary eye hospital in India. Age range was 21 to 35 years with an average age of 26.1 ± 3.5 years. Mean spherical equivalent was –3.86 D ± 1.50 D presurgery. NITM, lag of accommodation, near point of convergence (NPC), accommodative amplitude (AA), and binocular near accommodative facility (AF) were measured. All data were collected 21 days prior to and 30 days after LASIK surgery.Results:NITM, lag of accommodation and amplitude of accommodation were significantly lower (NITM -0.05 ± 0.15, Lag 0.38 ± 0.38, AA 10.27 ± 2.24) after surgery when compared to before (NITM 0.26 ± 0.12, Lag 0.77 ± 0.51, AA 12.18 ± 2.02; P < 0.001). Accommodative facility increased and near point of convergence was significantly more distal following surgery (AF 10.70 ± 2.29, NPC 7.96 ± 1.63) when compared to prior (AF 8.65 ± 2.74, NPC 5.62 ± 1.71; P < 0.001).Conclusion:Significant changes in NITM and accommodation function should be expected in the short term following LASIK surgery. This study supports the importance of evaluating accommodative parameters and patient counselling prior to and following refractive surgery.  相似文献

PurposeTo examine the interactions between accommodation and overstimulation of the retinal ON- and OFF-pathways, and their association with changes in choroidal thickness (ChT) and vascularity.MethodsOptical coherence tomography imaging of the choroid of twenty young adults (ages 25 ± 5 years) was performed before and after a series of 30-minute-long viewing tasks, including reading a bright text on dark background (ON-pathway overstimulation) and dark text on bright background (OFF-pathway overstimulation), and a control task of viewing a movie with unbiased ON-/OFF-pathway activation. The viewing tasks were performed with relaxed, and 5 diopter (D) accommodation (induced by soft contact lenses) demands. Both reading texts were matched for the mean luminance (35 cd/m2), luminance contrast (87%), and letter size (approximately 11.8 arc minutes). The change in ChT from baseline associated with contrast polarity and accommodation was examined using linear mixed model analysis.ResultsThe subfoveal ChT decreased significantly by −7 ± 1 µm with 5 D accommodation compared with relaxed accommodation (−3 ± 1 µm; P < 0.001), and by −9 ± 1 µm with OFF-pathway compared with ON-pathway overstimulation (−4 ± 1 µm; P = 0.002) and the control condition (−2 ± 1 µm; P < 0.001). Overstimulation of the OFF-pathway, but not the ON-pathway, resulted in a significantly greater choroidal thinning compared with the control condition, both at relaxed (−7 ± 1 µm; P = 0.003) and 5 D (−11 ± 1 µm; P = 0.005) accommodation levels. Similar changes were also observed for macular total, stromal, and luminal ChT.ConclusionsRetinal OFF-pathway stimulation enhanced the choroidal thinning associated with accommodation, thereby providing a potential mechanism that involves accommodation and the retinal OFF-signaling pathway, linking near work and myopia.  相似文献

PurposeThis study investigated whether refractive correction improved accommodative function of hyperopic children while engaged in two sustained near activities.MethodsSustained accommodative function of 63 participants (aged 5–10 years) with varying levels of uncorrected hyperopia (>/= +1.00 D and < + 5.00 D spherical equivalent in the least hyperopic eye) was measured using eccentric infrared photorefraction (PowerRef 3; PlusOptix, Germany). Binocular accommodation measures were recorded while participants engaged in 2 tasks at 25 cm for 15 minutes each: an “active” task (reading small print on an Amazon Kindle), and a “passive” task (watching an animated movie on liquid crystal display [LCD] screen). Participants also underwent a comprehensive visual assessment, including measurement of presenting visual acuity, prism cover test, and stereoacuity. Reading speed was assessed with and without hyperopic correction. Refractive error was determined by cycloplegic retinoscopy.ResultsHyperopic refractive correction significantly improved accuracy of accommodative responses in both task (pairwise comparisons: t = −3.70, P = 0.001, and t = −4.93, P < 0.001 for reading and movie tasks, respectively). Accommodative microfluctuations increased with refractive correction in the reading task (F(1,61) = 25.77, P < 0.001) but decreased in the movie task (F(1,59) = 4.44, P = 0.04). Reading speed also significantly increased with refractive correction (F(1,48) = 66.32, P < 0.001).ConclusionsCorrecting low-moderate levels of hyperopia has a positive impact on accommodative performance during sustained near activity in some schoolchildren. For these children, prescribing hyperopic correction may benefit performance in near vision tasks.  相似文献

PurposeTo identify the association between the choroidal thickness (ChT) with age and axial length (AL) under different refractive errors (REs) in Chinese adults.MethodsSwept-source optical coherence tomography was used to measure ChT in 2126 right eyes of 2126 participants. The participants were classified as having pathologic myopia (PM), high myopia without PM (HM), low myopia (LM), and nonmyopia (non-M) according to their REs and META-PM (the Meta-Analysis of Pathologic Myopia) classification criteria.ResultsThe mean age was 52.49 ± 20.39 years (range, 18−93 years), and the mean RE was −5.27 ± 5.37 diopters (D; range, −25.5 to +7.75 D). The mean average ChT was 159.25 ± 80.75 µm and decreased in a linear relationship from non-M to PM (190.04 ± 72.64 µm to 60.99 ± 37.58 µm, P < 0.001). A significant decline in ChT was noted between 50 and 70 years (r = −0.302, P < 0.001) and less rapidly after the age of 70 years (r = −0.105, P = 0.024). No correlation was noted between age and ChT under 50 years (P = 0.260). A significantly higher association with AL was noted in the central fovea (βHM = −23.92, βLM = −23.88, βNon-M = −18.80, all P < 0.001) and parafoveal ChT (βHM = −22.87, βLM = −22.31, βNon-M = −18.61, all P < 0.001) when compared with the perifoveal region (βHM = −19.80, βLM = −18.29, βNon-M = −13.95, all P < 0.001). Within each group of PM, HM, LM, and non-M, regression analysis showed that the coefficients of age and AL with different macular regions of ChT varied significantly.ConclusionsChT was negatively correlated with age after 50 years. The thinning of the choroid was more prominent in the center and parafoveal regions as AL increased. Varied distributions of ChT decrease associated with AL and age were noted among different refractive groups.  相似文献

PurposeTo investigate translatory movement during the lateral gaze in patients with horizontal strabismus using magnetic resonance imaging.MethodsPatients with esotropia or exotropia and normal controls underwent orbital magnetic resonance imaging during the central gaze and lateral gaze at 40°. The position of the static tissues was superimposed three-dimensionally for all gazes using a self-developed software, allowing the analysis of the net eyeball movement. Then, the eyeball centroid coordinates were extracted for each gaze, and the distance and direction of centroid movement from the central to lateral gaze were calculated.ResultsThe mean distance ± standard deviation of the centroid movement was 1.0 ± 0.5 mm during abduction in the exotropia group, which was significantly longer than that in the esotropia (0.6 ± 0.3 mm; P = 0.003) and control (0.7 ± 0.2 mm; P = 0.002) groups. Conversely, the centroid moved farther in the esotropia group (0.9 ± 0.3 mm) than the exotropia (0.6 ± 0.3 mm; P = 0.005) and control (0.7 ± 0.2 mm; P = 0.023) groups during adduction. Posterior translation during abduction was longer in the exotropia group (−0.8 ± 0.3 mm) compared with the esotropia (−0.5 ± 0.3 mm; P = 0.017) and control (−0.4 ± 0.3 mm; P = 0.001) groups, whereas that during adduction was longer in the esotropia group (−0.4 ± 0.4 mm) than the exotropia (−0.1 ± 0.2 mm; P = 0.033) and control (−0.1 ± 0.2 mm; P = 0.026) groups.ConclusionsDuring abduction, more translatory movement occurred in the exotropia group, whereas the centroid moved farther in the esotropia group during adduction. The translatory movement difference between both strabismus groups implies that there is a difference in biomechanics among the types of strabismus.  相似文献

PurposeTo explore the effect of gaze direction and eyelid closure on intraocular pressure (IOP).MethodsEleven patients with primary open-angle glaucoma previously implanted with a telemetric IOP sensor were instructed to view eight equally-spaced fixation targets each at three eccentricities (10°, 20°, and 25°). Nine patients also performed eyelid closure. IOP was recorded via an external antenna placed around the study eye. Differences of mean IOP between consecutive gaze positions were calculated. Furthermore, the effect of eyelid closure on gaze-dependent IOP was assessed.ResultsThe maximum IOP increase was observed at 25° superior gaze (mean ± SD: 4.4 ± 4.9 mm Hg) and maximum decrease at 25° inferonasal gaze (−1.6 ± 0.8 mm Hg). There was a significant interaction between gaze direction and eccentricity (P = 0.003). Post-hoc tests confirmed significant decreases inferonasally for all eccentricities (mean ± SEM: 10°: −0.7 ± 0.2, P = 0.007; 20°: −1.1 ± 0.2, P = 0.006; and 25°: −1.6 ± 0.2, P = 0.006). Eight of 11 eyes showed significant IOP differences between superior and inferonasal gaze at 25°. IOP decreased during eyelid closure, which was significantly lower than downgaze at 25° (mean ± SEM: −2.1 ± 0.3 mm Hg vs. −0.7 ± 0.2 mm Hg, P = 0.014).ConclusionsOur data suggest that IOP varies reproducibly with gaze direction, albeit with patient variability. IOP generally increased in upgaze but decreased in inferonasal gaze and on eyelid closure. Future studies should investigate the patient variability and IOP dynamics.  相似文献

PurposeDefocus blur imposed by positive lenses can induce hyperopia, whereas blur imposed by diffusers induces deprivation myopia. It is unclear whether the retina can distinguish between both conditions when the magnitude of blur is matched.MethodsTen emmetropic (average 0.0 ± 0.3 diopters [D]) and 10 subjects with myopia (−2.7 ± 0.9 D; 24 ± 4 years) watched a movie on a large screen (65 inches at 2 meters (m) distance. The movie was presented either unfiltered (“control”), with calculated low-pass filtering equivalent to a defocus of 2.5 D, or with binocular real optical defocus of +2.5 D. Spatial filtering was done in real-time by software written in Visual C++. Axial length was followed with the Lenstar LS-900 with autopositioning system.ResultsWatching unfiltered movies (“control”) caused no changes in axial length. In emmetropes, watching movies with calculated defocus caused axial eye elongation (+9.8 ± 7.6 µm) while watching movies with real positive defocus caused shorter eyes (−8.8 ± 9.2 µm; difference between both P < 0.0001). In addition, in myopes, calculated defocus caused longer eyes (+8.4 ± 9.0 µm, P = 0.001). Strikingly, myopic eyes became also longer with positive defocus (+9.1 ± 11.2 µm, P = 0.02). The difference between emmetropic and myopic eyes was highly significant (−8.8 ± 9.2 µm vs. +9.1 ± 11.2 µm, respectively, P = 0.001).Conclusions(1) In emmetropic human subjects, the retina is able to distinguish between real positive defocus and calculated defocus even when the modulation transfer function was matched, (2) in myopic eyes, the retina no longer distinguishes between both conditions because the eyes became longer in both cases. Results suggest that the retina in a myopic eye has reduced ability to detect positive defocus.  相似文献

PurposeTo investigate the effect of short-wavelength light (SL) on guinea pigs with lens-induced myopia (LIM) and the possible retinoic acid (RA)–related mechanisms.MethodsTwo-week-old guinea pigs (n = 60) with monocular −5D lenses were reared under white light (WL, 580 lux) or SL (440 nm, 500 lux). The left eyes were uncovered as control. Refractive error (RE) and axial length (AL) were measured at baseline, one week, two weeks, and four weeks after intervention. Retinal RA was measured from four guinea pigs after two and four weeks of treatment with HPLC. Two-week-old guinea pigs (n = 52) with monocular −5D lens were fed with either RA or its synthesis inhibitor citral every third day in the morning, and half from each group were reared under WL or SL conditions. RE and AL were recorded at baseline and two and four weeks after intervention. Retinal RA was measured after four weeks of intervention.ResultsAt the end of treatment, guinea pigs exposed to SL were less myopic than to WL (2.06 ± 1.69D vs. −1.00 ± 1.88D), accompanied with shorter AL (P = 0.01) and less retinal RA (P = 0.02). SL reduced retinal RA even after exogenous RA supplementation (P = 0.02) and decelerated LIM compared to WL (1.66 ± 1.03D vs. −3.53 ± 0.90D). Citral slowed ocular growth, leading to similar RE in W+CI and S+CI groups (3.39 ± 1.65D vs. 5.25 ± 0.80D).ConclusionsOverall, SL reduced LIM in guinea pigs, even in those supplemented with oral RA, accompanied by reduced retinal RA levels. Oral RA accelerated eye elongation, but citral equally decelerated eye elongation under SL and WL with no significant retinal RA reduction.  相似文献

PurposeThis study aimed to evaluate differences in objective and subjective accommodation dynamically and simultaneously.MethodsThirty-four pre-presbyopic healthy volunteers (mean age ± SD, 41.0 ± 3.2 years) participated in this study. Initially, the reaction time for detecting a change in the target was measured at near. Dynamic accommodation was then monocularly recorded using an open-view Shack–Hartmann aberrometer and compared with the amplitude and velocity of subjective accommodation.ResultsThe objective amplitude of accommodation (0.97 ± 0.32 diopter [D]) was significantly greater than the subjective amplitude of accommodation (0.62 ± 0.43 D; P < 0.001). The accommodative velocity was significantly faster for the “before the accommodation” response time (0.47 ± 0.38 D/s) than the “after the accommodation” response time (0.21 ± 0.22 D/s; P = 0.007).ConclusionsThe human eye under the monocular condition quickly adjusts to the focal plane to clearly archive the nearby object, and the focal plane thereafter is slowly and accurately adjusted to the visual target after visual recognition.  相似文献

Purpose:To evaluate peripapillary-RNFL thickness in myopia by Cirrus OCT among north Indian population by spherical equivalent (SE), age, gender, and axial length (AL).Methods:This was a cross-sectional study held during 2019–2020. Patients aged 18–60 years underwent ophthalmic examination including retinoscopy, AL, and OCT RNFL thickness. Persons with previous ocular surgery or ocular ailment other than refractive error were excluded. The peripapillary-RNFL thickness was noted and compared by demographic determinants.Results:We examined 300 eyes of 300 persons (mean age: 30.75 ± 8.57 years; 144 males/156 females). Among them, 224 were myopes and 76 were emmetropes (EM). The mean SE was − 3.3 ± 0.4D (range: −11.0D to + 0.37D). The mean AL was 24.61 ± 1.92 mm (22.1–29.5). Overall temporal, nasal, superior, inferior, and mean peripapillary-RNFL thickness was 66.31 ± 7.58, 78.57 ± 16.00, 120.63 ± 11.69, 116.60 ± 15.80, and 95.50 ± 10.84 mm, respectively. Temporal, nasal, superior, inferior, and mean peripapillary-RNFL thickness was 73.97 ± 8.36, 94.84 ± 7.63, 127.96 ± 8.96, 136.89 ± 6.53, and 108.34 ± 6.28 mm, respectively, in EM eyes as compared to 63.71 ± 6.18, 73.05 ± 14.24, 118.21 ± 11.53, 109.71 ± 11.50, and 91.14 ± 8.31 mm, respectively, in myopic eyes (P < 0.001). Association of peripapillary-RNFL thickness with myopia and its different grades was P < 0.001. Association of mean peripapillary-RNFL thickness with age was P > 0.005 and gender was P = 0.168. Correlation between SE and RNFL thickness was positive and significant. Correlation between AL and RNFL thickness was negative but statistically significant. Association of AL with SE was P < 0.001.Conclusion:We provide normative peripapillary-RNFL thickness in the north Indian population in order to help in screening for myopia with comorbidity such as glaucoma based on RNFL thickness.  相似文献

Purpose

To investigate the changes of intraocular pressure (IOP) and anterior eye segment biometric parameters under different accommodative statuses in progressing myopes and emmetropes.

Methods

Forty-six progressing myopes and 40 emmetropes participated in this study. All the subjects had their IOP and anterior eye segment biometric parameters (including corneal thickness, anterior chamber depth, anterior chamber angle width, and lens thickness) measured using iCare rebound tonometer and VisanteTM anterior segment-optical coherence tomography while accommodative stimuli of 0, 3, and 6D were presented.

Results

There was no significant difference in IOP between progressing myopes and emmetropes when no accommodation was induced (16.22±4.11 vs 17.01±3.72, respectively, t=−0.93, P>0.05). However, IOP significantly increased with accommodation in progressing myopes (mean change +1.02±2.07 mm Hg from 0D to 6D, F=5.35, P<0.01), but remained unchanged (mean change −0.76±3.22 mm Hg from 0D to 6D, F=1.46, P>0.05) in emmetropes. Meanwhile, we found that their anterior chamber depth decreased (P<0.01), anterior chamber angle narrowed (P<0.01), and lens thickened (P<0.01) significantly with accommodation, both in progressing myopes and emmetropes.

Conclusions

Although no difference was detected between the IOPs of progressing myopes and emmetropes without accommodation, accommodation could induce transient IOP elevation in progressing myopes. Simultaneously, we found that their anterior chamber depth decreased, anterior chamber angle narrowed, and lens thickened with accommodation. Although emmetropes showed the similar anterior eye segment structure changes, their IOPs did not increase with accommodation. Our study indicated that IOP elevation with accommodation in progressing myopes might be related to myopia progression.  相似文献

PurposeTo evaluate the potential effect of age and refractive error on visual acuity (VA) performance and quick contrast sensitivity function (qCSF) in normal Chinese adults.MethodNinety-two subjects with normal best corrected distance VA (BCDVA) were enrolled in this pilot study. Measurements included BCDVA, best corrected near VA (BCNVA), unaided VA (UNVA), habitual spectacle-corrected near VA (SCNVA) and qCSF. For analyses, subjects were categorized into three age groups (20~40 years, 41~60 year and >60 years) and four refractive groups (hyperopia, emmetropia, myopia and high myopia). Relationships between age, refractive error, types of VA and qCSF were tested using simple and multiple linear regressions analyses.ResultMean age and refractive error of the study participants were 44.04 ± 12.68 years and −1.86 ± 2.91D, respectively. Among the stratified age groups, a hyperopic shift of refraction (−3.24 ± 2.88D vs. −1.24 ± 2.64D vs. 0.39 ± 1.42D, respectively; P < 0.001) and a reduction in BCNVA (P = 0.014), SCNVA (P < 0.001) and cut-off spatial frequency (SF) (P = 0.032) were found with increasing age. Among the four refractive groups, the SCNVA and cut-off SF of hyperopia were worse compared to the other refractive statuses (all P < 0.05). Age was significantly associated with cut-off SF (standardized β = −0.29, P = 0.005) after adjustment for SER, gender and all types of VA.ConclusionFor normal Chinese adults with normal BCDVA, age was the main factor associated with CSF, which may be independent of refractive error.Subject terms: Outcomes research, Public health  相似文献

PurposeGiven the potential role of light and its wavelength on ocular growth, we investigated the effect of short-term exposure to the red, green, and blue light on ocular biometry in the presence and absence of lens-induced defocus in humans.MethodsTwenty-five young adults were exposed to blue (460 nm), green (521 nm), red (623 nm), and white light conditions for 1-hour each on 4 separate experimental sessions conducted on 4 different days. In each light condition, hyperopic defocus (3D) was induced to the right eye with the fellow eye experiencing no defocus. Axial length and choroidal thickness were measured before and immediately after the light exposure with a non-contact biometer.ResultsAxial length increased from baseline after red light (mean difference ± standard error in the defocussed eye and non-defocussed eye = 11.2 ± 2 µm and 6.4 ± 2.3 µm, P < 0.001 and P < 0.01, respectively) and green light exposure (9.2 ± 3 µm and 7.0 ± 2.5 µm, P < 0.001 and P < 0.001) with a significant decrease in choroidal thickness (P < 0.05, both red and green light) after 1-hour of exposure. Blue light exposure resulted in a reduction in axial length in both the eyes (−8.0 ± 3 µm, P < 0.001 in the defocussed eye and −6.0 ± 3 µm, P = 0.11 in the non-defocused eye) with no significant changes in the choroidal thickness.ConclusionsExposure to red and green light resulted in axial elongation, and blue light resulted in inhibition of axial elongation in human eyes. Impact of such specific wavelength exposure on children and its application in myopia control need to be explored.  相似文献

PurposeTo identify the impact of hypertension (HTN) on macular microvasculature in type 2 diabetes (T2DM) patients without clinical diabetic retinopathy.MethodsIn this retrospective cross-sectional study, subjects were divided into three groups: controls (control group), patients with T2DM (DM group), and patients with both T2DM and HTN (DM + HTN group). The vessel length density (VD) was compared among the groups. Linear regression analyses were performed to identify factors associated with VD.ResultsThe VD in the control, DM, and DM + HTN groups was 20.43 ± 1.16, 19.50 ± 1.45, and 18.19 ± 2.06 mm−1, respectively (P < 0.001). The best-corrected visual acuity (B = −9.30; P = 0.002), duration of T2DM (B = −0.04; P = 0.020), HTN (B = −0.51; P = 0.016), signal strength (B = 1.12; P < 0.001), and ganglion cell–inner plexiform layer thickness (B = 0.06; P < 0.001) were significant factors affecting VD in patients with T2DM. Additionally, the hemoglobin A1c (HbA1c) (B = −0.49; P = 0.016) was significantly associated with VD in patients with both T2DM and HTN.ConclusionsPatients with T2DM had impaired macular microvasculature, and patients with T2DM with HTN exhibited greater impairment of the microvasculature than did patients with T2DM only. Additionally, physicians should be aware that the macular microvasculature would be more vulnerable to hyperglycemic damage under ischemic conditions by HTN.  相似文献

PurposeThe prevalence of myopia increases with both age and grade for children attending school. The current study aimed to distinguish the effects of aging and grade on myopia.MethodsGrade 1 students (706 at baseline in 2009, mean age 6.56 ± 0.29 years, range 6.00 to 6.99 years old, 55.5% boys) were followed up until 2012. Cycloplegic spherical equivalent (SE) was measured annually.ResultsThe sample in a grade was divided into three 4-month age blocks according to their birth month. Within each grade, there were no significant differences in SE between age blocks (all P > 0.05), despite an age range of one year. More myopic SE was observed in the youngest block of grade 2 compared to the oldest block of grade 1 (difference, −0.36 ± 0.08 D; P < 0.001), although age of the two blocks only differed by four months. Similarly, more myopic SE were found in the youngest block in grade 3 than the oldest block in grade 2 (differences, −0.50 ± 0.10 D; P < 0.001) and in the youngest block in grade 4 than the oldest block in grade 3 (differences, −0.82 ± 0.14 D; P < 0.001).ConclusionsExposure to schooling, rather than age, appears to be the major driver of refractive development, at least in the early years of schooling. Interventions during this period, involving reductions in educational pressure and increased time outdoors may have major effects on the subsequent development of myopia.  相似文献

PurposeTo investigate the effect of home quarantine during the COVID-19 pandemic on myopia progression in children and its associated factors.MethodsMyopic children aged 7 to 12 years with regular follow-up visits every half a year from April 2019 to May 2020 were included. Cycloplegic refraction was measured at baseline and at two follow-up visits. The first follow-up visit (visit 1) was conducted before the COVID-19 home quarantine, whereas the second (visit 2) was four months after the home quarantine. Myopia progression at visits 1 and 2 were compared. Factors associated with changes in myopia progression were tested with a multiple regression analysis.ResultsIn total, 201 myopic children were enrolled. There was a significantly greater change in spherical equivalent at visit 2 (−0.98 ± 0.52 D) than at visit 1 (−0.39 ± 0.58 D; P < 0.001). Students were reported to have spent more time on digital devices for online learning (P < 0.001) and less time on outdoor activities (P < 0.001) at visit 2 than at visit 1. Children using television and projectors had significantly less myopic shift than those using tablets and mobile phones (P < 0.001). More time spent on digital screens (β = 0.211, P < 0.001), but not less time on outdoor activities (β = −0.106, P = 0.110), was associated with greater myopia progression at visit 2.ConclusionsChanges in behavior and myopic progression were found during the COVID-19 home quarantine. Myopic progression was associated with digital screen use for online learning, but not time spent on outdoor activities. Purpose:To analyze the ocular biometric parameters of eyes with acute primary angle closure (APAC) as compared to fellow eyes.Methods:A cross-sectional study was conducted on 27 patients presenting with recent onset APAC to a tertiary eye institute in India. Anterior and posterior ocular biometric parameters were measured simultaneously by anterior segment optical coherence tomography (AS-OCT), A-scan, ultrasound biomicroscopy (UBM), and B-mode ultrasonogram (USG). The parameters measured were anterior chamber depth (ACD), anterior chamber angle (ACA), angle opening distance (AOD500, AOD750), lens vault (LV), axial length (AL), ciliary body thickness maximum (CBTmax) and at the point of scleral spur (CBT0), anterior placement of the ciliary body (APCB), and retinochoroidal thickness (RCS).Results:Mean age ± SD of patients with APAC was 55.66 ± 7.2 years with female preponderance (21:6 patients). Mean presenting IOP ± SD of the affected eye and fellow eye were 54.74 ± 11.67 mm Hg and 18.7 ± 11.67 mm Hg, respectively. Eyes with APAC had statistically significant narrower anterior ocular biometric parameters, higher LV, decreased ciliary body thickness, more APCB, and longer AL than the fellow eyes. CBTmax is the only variable that had significance (b = −0.421,95% CI: −0.806 to − 0.035, P = 0.034) in the univariate analysis with RCS thickness in APAC eyes. Further, there was a correlation between CBT0 and APCB with CBTmax both in univariate (b = 0.894, P < 0.0001 and b = −0.351, P = 0.039) and multivariable analysis (b = 0.911, P < 0.0001 and b = −0.416, P = 0.016)Conclusion:Compared to the fellow eyes, APAC eyes had different ocular biometric parameters. In addition to known biometric parameters associated with pupillary block (narrower anterior biometric parameters-ACA, ACD, and AOD), our study found multiple nonpupillary block factors such as higher lens vault and thinner and more anteriorly placed ciliary body to be associated with APAC.  相似文献

Background:This study evaluated the visual function after implantation of a multifocal toric intraocular lenses (IOLs).Results:At 6 months postoperatively, uncorrected distance visual acuity (logarithm of the minimum angle of resolution) was 0.09 ± 0.04, corrected distance visual acuity was 0.02 ± 0.11, and uncorrected near visual acuity was 0.12 ± 0.07. The mean SE was −0.095 ± 0.394 D (±0.50 D in 90%). Refractive astigmatism at the 6-month follow-up visit was significantly reduced to 0.35 ± 0.32 D from 1.50 ± 0.41 D presurgery (P < 0.05). The mean IOL axis rotation was 3.20 ± 1.55°. Postoperative CS levels were high. Postoperative total order aberrations (TOAs), lower-order aberrations (LOAs), higher-order aberrations (HOAs), and spherical aberrations were decreased compared with preoperative values (P < 0.05). At 3 months postoperatively, TOAs, LOAs, and HOAs with a 3 mm pupil diameter as well as TOAs, LOAs, and astigmatism aberrations with a 5 mm pupil diameter were statistically lower than those at 1-month post surgery, but without subsequent significant changes (P > 0.05). There was an increase in MTF results between preoperative and postoperative evaluations at all spatial frequencies.Conclusions:The diffractive multifocal toric IOL is able to provide a predictable astigmatic correction with apparently outstanding levels of optical quality after implantation.  相似文献

目的:比较间歇性外斜视和正常眼位人群之间,间歇性外斜视注视眼和非注视眼之间的调节灵活度和调节反应。方法:病例对照研究。选取2016年10月至2017年1月在温州医科大学附属眼视光医院门诊就诊的35例间歇性外斜视患者作为间歇性外斜视组,另选取24例正位眼或外隐斜斜视度≤6 △ 的门诊患者作为对照组。采用±2.00 D的反转拍和开放视野式自动验光仪分别测量受试者在单眼和双眼注视近距视标时的调节灵活度和调节反应。采用t检验对数据进行分析。结果:间歇性外斜视组的双眼调节灵活度为(8.8±4.1)cpm,显著低于对照组[(10.9±3.1)cpm],两者差异有统计学意义(t=-2.165,P=0.035)。间歇性外斜视组的非注视眼调节灵活度显著低于注视眼(t=4.657,P<0.001)。双眼注视40 cm处视标时,间歇性外斜视组的注视眼调节反应高于对照组的主导眼(t=-2.163,P=0.035)。双眼注视状态下,间歇性外斜视组注视眼的调节反应为(1.89±0.30)D,高于其在单眼注视状态下的调节反应[ (1.64±0.34)D],两者差异有统计学意义(t=3.801,P=0.001)。结论:间歇性外斜视患者的双眼调节灵活度低于对照人群,其注视眼和非注视眼的调节灵活度和调节反应均不一致,且双眼注视时调节反应高于单眼注视状态。  相似文献

PurposeTo determine the effects of optically imposed astigmatism on myopia development in chickens.MethodsChicks were randomly assigned to wear either spherical (−10D, “LIM”, n = 14) or sphero-cylindrical lenses (n ≥ 19 in each group) monocularly for a week from 5 days of age. All lenses imposed the same magnitude of spherical-equivalent hyperopic defocus (−10D), with the two astigmatic magnitudes (−8D or −4D) and four axes (45°, 90°, 135°, or 180°) altered to simulate four subtypes of clinical astigmatism. At the end of the treatment, refractive state was measured for all birds, whereas ocular axial dimensions and corneal curvature were measured for subsets of birds.ResultsSphero-cylindrical lens wear produced significant impacts on nearly all refractive parameters (P < 0.001), resulting in myopic-astigmatic errors in the treated eyes. Compared to LIM, the presence of astigmatic blur induced lower myopic error (all except L180 group, P < 0.001) but with higher refractive astigmatism (all P < 0.001) in birds treated with sphero-cylindrical lenses. Distributions of the refractive, axial, and corneal shape parameters in the sphero-cylindrical lens-wear groups indicated that the astigmatic blur had directed the eye growth toward the least hyperopic image plane, with against-the-rule (ATR) and with-the-rule (WTR) astigmatisms typically inducing differential biometric changes.ConclusionsThe presence of early astigmatism predictably altered myopia development in chicks. Furthermore, the differential effects of WTR and ATR astigmatisms on anterior and posterior segment changes suggest that the eye growth mechanism is sensitive to the optical properties of astigmatism.  相似文献
