Findings in primary hepatic carcinoid tumor: US, CT, MRI, and angiography.   总被引:6,自引:0,他引:6  
Two asymptomatic patients with surgically proven solitary primary hepatic carcinoid tumors are reported. Ultrasonography showed hyperechoic masses containing multiple small cystic areas in both cases. On unenhanced CT, one tumor was of low density and one was isodense with multiple low density foci. One tumor showed marked retention of contrast medium on post-angiographic CT. Magnetic resonance imaging revealed low intensity masses on T1-weighted images and high intensity tumors with multiple areas of higher intensity on T2-weighted imaging. The small low density areas in these masses corresponded histopathologically to multiple vascular lakes. Late enhancement of the mass was presumed to correspond with proliferative fibrous tissue within the mass.  相似文献

The authors present the radiologic findings of a neurilemoma arising from the kidney. Ultrasound, CT, and angiography could not exclude the diagnosis of renal cell carcinoma (RCC). Magnetic resonance (MR) imaging at 1.5 T showed an isointense tumor on T1-weighted images and a high signal intensity on T2-weighted images compared with normal renal parenchyma. The MR findings were helpful in the differentiation from RCC.  相似文献

PURPOSE: Evaluating the MR findings of renal cortical necrosis was the purpose of this study. METHOD: Eight series of T1-/T2-weighted (n = 8) and contrast-enhanced T1-weighted (n = 4) MR images in six patients with renal cortical necrosis diagnosed by renal biopsy (n = 4) or on clinical grounds (n = 2) were reviewed. In those who had follow-up MRI (n = 2) or comparable CT (n = 3), interval changes of MR findings and comparison with CT images were done. RESULTS: Swollen kidney with dark signal intensity rim involving the inner cortex and column of Bertin was noted on T2-and T1-weighted images. It was more conspicuous on T2-weighted images. The lesion did not enhance and was differentiated from uninvolved renal parenchyma. In the follow-up MRI, thickened dark signal intensity was more prominent and proved to be calcification or fibrosis. CONCLUSION: MRI, especially T2-weighted and contrast-enhanced T1-weighted imaging, was helpful in evaluating renal cortical necrosis.  相似文献

Inflammatory pseudotumor (IPT) is a quasineoplastic lesion that most commonly involves the lung and the orbit; kidney involvement is rare. We report a case of inflammatory pseudotumor of the kidney. The patient was a 61-year-old man who presented with no symptoms. Nonenhanced computed tomography (CT) demonstrated an ill-defined, isodensity mass measuring 3.5 cm in the lower portion of the left kidney. Contrast-enhanced CT showed that branches of the renal artery without encasement penetrated the tumor; there was a little enhancement in the mass on the arterial phase and homogeneous enhancement on the venous phase. On magnetic resonance imaging the mass showed intermediate signal intensity on T1-weighted images (T1WIs) and low signal intensity on T2WIs. Most IPTs of the kidney appear as an ill-defined, hypovascular, homogeneous tumor on CT images, with variable signal intensity on MRI T1WIs and low signal intensity on T2WIs. Our case had the same imaging findings, with branches of the renal artery penetrating the tumor. If the renal tumor has these radiological findings, the tumor may be IPT.  相似文献

PURPOSE: To clarify the findings of nondiffuse fatty change of the liver on ferumoxides-enhanced magnetic resonance (MR) images. MATERIALS AND METHODS: Of 202 patients who underwent ferumoxides-enhanced MR imaging, eight who had nondiffuse fatty change of the liver at computed tomography (CT) were examined as study subjects. MR imaging findings before and 1 hour after ferumoxides administration were compared with CT findings. RESULTS: Focal fatty areas of the liver showing low attenuation on CT images were depicted as areas of relatively high intensity on the ferumoxides-enhanced T1-weighted images in all patients. On enhanced T2-weighted images, focal fatty change showed relatively high intensity in three and isointensity in one of the four patients. Focal spared areas appearing as areas of relatively high attenuation on CT images were depicted as areas of relatively low intensity on the ferumoxides-enhanced T1- and T2-weighted images in all patients. CONCLUSION: Although prior reports of hepatic MR imaging with ferumoxides indicated that there is accumulation of ferumoxides within focal fatty areas that are no longer seen after the administration of contrast medium, this study revealed that focal fatty change and focal spared areas of fatty liver may be pseudotumors because of the relatively high intensity of fatty areas of the liver. Radiologists can distinguish these conditions from hepatic tumors by using the opposed-phase gradient-echo sequence or the fat-saturation technique.  相似文献

The aim of this study was to work out the cross-sectional imaging characteristics of desmoplastic fibroma (DF). In 3 patients with histologically proven DF, the imaging characteristics obtained with cross-sectional techniques were reviewed retrospectively. Radiographs and CT scans were available in all patients, and plain and contrast-enhanced MR examinations in 2 patients. Compared with conventional radiographs, CT allowed more accurate assessment of the extent of bone destruction including cortical breakthrough and articular invasion. Intramedullary tumor growth and soft tissue extension was best detected with MRI. Apart from heterogeneity on MR images, DF displayed nonspecific low signal intensity on unenhanced T1-weighted images and an intermediate to high signal intensity including areas of low intensity on T2-weighted images. Desmoplastic fibroma showed a distinct, inhomogeneous gadolinium enhancement. Although cross-sectional imaging features of DF are nonspecific, some MR characteristics, such as inhomogeneous contrast enhancement and the presence of low-intensity regions on T2-weighted images, are helpful in determining the differential diagnosis. Cross-sectional imaging of DF is useful for local staging of the tumor because it provides valuable information about the extent of bone destruction as well as medullary and extraosseous spread.  相似文献

目的:探讨阔韧带平滑肌瘤的MR表现及诊断价值.方法:回顾性分析13例经手术和病理证实为阔韧带平滑肌瘤的MR表现.结果:13例中肿块位于子宫右侧4例,左侧8例,两侧1例.肿块呈圆形、类圆形10例,不规则形或分叶状3例.肿块大于5 cm占84.6%(11/13).T1WI肿块均呈等或略低信号;T2WI为中等信号,其中11例肿块中见不均匀斑点状、梭条状高信号影,2例见大片状液化坏死灶.注射钆喷替酸葡甲胺(Gd-DTPA)后扫描肿块实质部分明显强化,显示包膜完整,与子宫等周围脏器分界清晰10例,包膜不清,与子宫有粘连且分界不清3例;包膜不强化且显示更清晰.结论:阔韧带平滑肌瘤的MR表现具有一定的特征性:盆腔内、子宫外大而有完整包膜的实性或实性为主的肿块,T1WI呈等或略低信号,T2WI呈中等信号,其内常可见不均匀斑点状、梭条状高信号影,增强扫描肿瘤强化明显.MR具有良好的软组织分辨率、多方位成像的特点,有利于肿瘤结构、成份与包膜显示,对明确诊断有肯定的价值.  相似文献

We report a case of seminal vesicle cyst associated with ipsilateral renal agenesis in a 37-year-old patient with papillary carcinoma of the bladder. Ultrasonography showed absence of the right kidney, a bladder tumour and a round retrovesical hypoechogenic mass with posterior acoustic enhancement. It showed low attenuation on CT, low signal intensity on T1-weighted MR images and high signal intensity on T2-weighted MR images. Intravenous urography (IVU) and cystoscopy were also performed. After surgery, it proved to be a seminal vesicle cyst. The embryology, imaging characteristics and differential diagnosis of seminal vesicle cysts are discussed. The CT and magnetic resonance (MR) findings in 13 patients with advanced Zollinger-Ellison syndrome are described. In eight patients (62%) one or more primary tumors were found with both methods. All patients with proven liver metastases (n = 7) were identified by MR. Computed tomography was positive in six of these patients. Three patients with lymph node metastases were identified on CT and MR and one patient had bone metastases. Computed tomography and MR were inferior to selective arteriography in the detection of multiple lesions of the pancreas in a patient with multiple endocrine neoplasia-I syndrome. On the T1-weighted MR images, the primary tumors demonstrated no consistency with regard to their signal intensity relative to the adjacent pancreatic parenchyma. All gastrinomas had an increased relative signal intensity on the T2-weighted images with the exception of a calcified tumor. Liver and lymph node metastases had a low signal intensity on the T1-weighted images and an increased signal intensity on the T2-weighted images. The signal intensity of primary tumors and metastases was independent of size. In conclusion, MR was able to detect abnormalities based on its outstanding lesion/normal tissue contrast, whereas CT diagnosis was based mostly on contour distortion. For the current technique, MR is considered at least equal to CT.  相似文献

Hepatic magnetic resonance (MR) imaging was performed in 12 patients with 13 amebic liver abscesses. While no specific image or intensity pattern was noted, most lesions were round or oval with smooth, well-defined margins; had decreased signal intensity compared with that of liver parenchyma on T1-weighted images and increased signal intensity on T2-weighted images; and had prominent, often multiple rims of variable signal intensity. Signal homogeneity within the abscess was present more often on T1- than on T2-weighted images. Diaphragmatic disruption was seen in two cases on coronal MR images. An amebic empyema was differentiable from sympathetic pleural effusions by its hyperintensity on both T1- and T2-weighted images. In patients who also underwent computed tomography (CT) or ultrasonography (US), no lesion was missed with any modality, and except for shape, no consistent features were found among images obtained with the different modalities. The data suggest that CT, US, and MR imaging are comparably effective in the detection of amebic abscess.  相似文献

Eight patients with intracranial tuberculomas were studied with CT and magnetic resonance (MR) imaging. Large, ring enhancing, solid lesions on CT showed low intensity on T2-weighted images and intermediate intensity on T1-weighted images. Small lesions, with ring enhancement on CT, showed central bright signal on T2-weighted images with a peripheral low intensity rim surrounded by high intensity edema. The MR imaging features of the tuberculomas were found to be distinct from those of abscesses, metastases, and gliomas.  相似文献

PURPOSE: To describe imaging findings of desmoplastic small round cell tumor (DSCRT) and to clarify the relation between radiologic appearances and clinicopathologic features. MATERIALS AND METHODS: CT scans and MRI examinations of four male patients with histologically confirmed DSCRT (mean age = 20 years) were retrospectively analyzed. RESULTS: The common imaging finding was multiple peritoneal masses with regular contour situated within mesentery. Tumors showed central low attenuation in 75% of patients on nonenhanced CT. All tumors showed inhomogeneous enhancement on CT. Small foci of punctate calcification were identified in a part of the tumor in all patients. Bone metastases were identified on enhanced CT in two patients 2 and 8 months after diagnosis, respectively. Pleural dissemination was identified in all patients, and one patient had double-sided dissemination. T2-weighted MRI showed inhomogeneous high signal intensity, and small cysts were identified in two patients. Fluid-fluid levels were identified in six tumors on T2-weighted images, which suggested the presence of hemorrhage. Tumors had inhomogeneous signal intensity low or isointense relative to skeletal muscle on T1-weighted images. Two lesions exhibited inhomogeneous enhancement with central low intensity consistent with necrosis. CONCLUSION: Imaging studies can depict disseminated characteristics and disease extent during the clinical course of DSRCT.  相似文献

The authors retrospectively reviewed the clinical, computed tomography (CT), and magnetic resonance (MR) imaging findings in seven patients with pathologically proved Rathke cleft cysts. All the cysts were located in the anterior sella turcica or the anterior suprasellar cistern. Five cysts had both intra- and suprasellar components, one was entirely intrasellar, and the other was predominantly suprasellar in location. The size of the cysts ranged from 8 to 20 mm. CT scans demonstrated low-density homogeneous lesions in four cases. On MR images of three of these four cases, the cysts had the same intensity as cerebrospinal fluid on T1- and T2-weighted images, while in the fourth case, the cyst was hyperintense on the T1-weighted images. In the remaining three cases, CT showed slight hyperdensity relative to brain parenchyma, suggestive of contrast enhancement. MR showed signal heterogeneity of these lesions with focal components of diminished signal intensity of T2-weighted images. These same foci appeared iso- to slightly hyperintense on T1-weighted images.  相似文献

Two cases of hepatic infarction in toxemia of pregnancy are described. Both patients had associated laboratory findings of the HELLP syndrome (hemolytic anemia, elevated liver enzyme levels, and low platelet levels). Computed tomography (CT) showed nonenhancing low-attenuation areas varying in configuration from peripheral, well-defined, wedge-shaped lesions to larger abnormalities. Magnetic resonance imaging showed a corresponding area of low signal intensity on T1-weighted images with high signal intensity on T2-weighted images. Ultrasound showed several well-defined peripheral hypoechoic areas. The low attenuation and lack of enhancement on CT scans and the absence of mass effect are the key distinguishing features.  相似文献

We report the imaging findings in two patients with proliferating trichilemmal tumors. In the first patient, the tumor arose on the lower lip, a very unusual location for this type of tumor, and showed malignant transformation with metastasis to a regional lymph node. It was seen as a poorly marginated soft-tissue mass with isointense signal on T1-weighted MR images and hyperintense signal on T2-weighted images. Large areas of high signal intensity caused by necrosis were also found within the tumor on T2-weighted images. After i.v. administration of contrast material, the mass showed significant enhancement, with considerable portions remaining unenhanced. In the second patient, the tumor originated from a preexisting trichilemmal cyst and occurred in the hair-bearing area of the posterior part of the neck. CT scans showed a well-encapsulated cystic mass that contained multiple speckled calcifications in a wall of variable thickness. There were several foci of smooth soft-tissue elevations from the inner wall of the mass, which corresponded histologically to proliferating portions of trichilemmal cyst.  相似文献

Four patients with traumatic hemorrhage in the right adrenal gland were examined with computed tomography (CT) and ultrasound (US) (initial US studies in three patients; follow-up study at least 1 month after the trauma in all patients). CT showed an enlarged inhomogeneous gland of normal to increased attenuation. This finding was associated with stranding of the periadrenal fat and stranding of the subcutaneous fat of the right flank, which corresponded to the site of trauma. US showed an enlarged, hyperechoic mass with a bright central echo that became cystic on follow-up examinations. Magnetic resonance imaging, performed in one patient, showed an enlarged gland that was isointense with liver on T1-weighted images and had increased signal intensity on T2-weighted images. Stranding of the periadrenal and subcutaneous fat of the right flank was present on T2-weighted images and appeared as streaky areas of increased signal intensity.  相似文献

We report two fatal cases of methotrexate (MTX)-induced disseminated necrotising leukoencephalopathy (DNL) in which MRI was repeated from the onset. Initial T2-weighted images showed multiple areas of high signal, mainly in deep cerebral white matter, which on follow-up, spread and coalesced to involve the entire white matter. Small irregular low-signal foci on T2-weighted images were seen within the high-signal lesions. Multiple areas of contrast enhancement corresponded to these low-signal foci. The condition of both patients deteriorated and they died. We compared their MRI findings with those of seven patients with mild MTX-related leukoencephalopathy, six of whom were asymptomatic; one had transient neurological symptoms. They showed no contrast enhancement, but rather mild-to-moderate diffuse high signal in deep white matter, which later disappeared. These findings suggest that multiple low-signal foci on T2-weighted images with contrast enhancement may be characteristic of DNL, and that contrast-enhanced imaging is useful to differentiate this condition from mild leukoencephalopathy.  相似文献

The magnetic resonance (MR) findings in three patients with malignant pleural mesothelioma are described. All patients had a circumferential pleural mass surrounding the lung on the affected side. These tumors had a signal of intermediate intensity on T1-weighted images. The T2-weighted images showed a slight increase in signal intensity of the mass, with focal areas of very high signal intensity due to pleural fluid. Adenopathy was demonstrated by CT and MR in two patients. The extent of the tumor and its effects on adjacent structures were well appreciated on the coronal MR images.  相似文献

Magnetic resonance (MR) images obtained in 18 patients with pathologically confirmed mycetoma in the body (n = 4) or lower extremity (n = 14) were retrospectively reviewed and compared with computed tomographic (CT) scans in 15 patients and surgical findings in 10. T1-weighted images showed an infiltrating mass (same signal intensity as muscle) involving skin, subcutaneous fat, muscles, tendons, and other tissues. On T2-weighted images, the mass and affected structures showed moderately increased signal intensity. Bone marrow involvement was detected in seven patients and was best visualized on T1-weighted images. CT showed moderate enhancement of the infiltrative process in all patients. Bone changes, seen in nine, included coarse trabeculation, periosteal reaction, endosteal proliferation, and patchy destruction. MR imaging and CT were comparable and correlated well with surgery in showing the extent of soft-tissue involvement. Early bone changes (important for therapy planning for pedal mycetoma) were seen only at CT. The study showed that MR imaging is sensitive for assessing the extent of mycetoma in the soft tissues. CT should be the method of choice for staging pedal lesions because it can be used to detect early bone involvement.  相似文献

Magnetic resonance imaging was performed on nine kidneys in six patients with renal infarction caused by renal arterial embolism secondary to cardiac valvular disease. The time interval between the onset of the symptoms attributable to renal infarction and the date of MRI was 2-23 days. On both T1- and T2-weighted images the signal intensity of the infarcted area was lower than that of the noninfarcted area in six kidneys and higher in three kidneys. Postcontrast T1-weighted images demonstrated the extent of the infarction except in the kidney in which the infarcted areas had high signal intensity on precontrast T1-weighted images.  相似文献
