PURPOSE: To evaluate the safety and efficacy of laser in situ keratomileusis (LASIK) in selected post-radial-keratotomy (RK) eyes with residual myopia and astigmatism. SETTING: TLC-The Brea Laser Eye Center, Brea, California, USA. METHODS: Nine eyes of 6 patients who had had RK but had residual myopia and/or astigmatism had LASIK. All RK eyes had 8 radial incisions, were more than 1 year post-RK, had no epithelial inclusion cysts or corneal disease, and had had no subsequent ocular surgery. Follow-up was a minimum of 13 months, at which time uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), manifest refraction, cycloplegic refraction, keratometry, central and peripheral pachymetries, intraocular pressure, and a subjective assessment of visual function were obtained. RESULTS: At the last follow-up, the mean spherical equivalent (SE) was -0.156 diopter (D) +/- 0.174 (SD). All eyes treated for distance vision had a UCVA of 20/25 or better. No patient lost BCVA. No intraoperative or postoperative complications occurred. Seven eyes had morning and evening measurements. The mean change in manifest SE from morning to evening was -0.143 D. Six of the 7 eyes (86%) had 0 to 1 Snellen line change in UCVA from morning to evening. The subjective questionnaire revealed a high degree of satisfaction with overall vision, minimal glare, and less fluctuation in daily vision than before LASIK. CONCLUSION: Laser in situ keratomileusis is safe and efficacious for reducing residual myopia and astigmatism in properly selected RK patients.  相似文献

Laser in situ keratomileusis after penetrating keratoplasty   总被引:2,自引:0,他引:2  
PURPOSE: To assess the outcomes of laser in situ keratomileusis (LASIK) after penetrating keratoplasty (PKP). SETTING: Hospital de Clínicas de Porto Alegre, Department of Ophthalmology, Federal University of Rio Grande do Sul, Porto Alegre, RS, Brazil. METHODS: Fourteen eyes of 13 patients who had LASIK after PKP were retrospectively reviewed. The interval between LASIK and PKP was at least 1 year, and the follow-up after LASIK was also at least 1 year. All patients had a stable refractive error for a minimum of 6 months after all sutures were removed, regular and symmetric topographic astigmatism, and a minimal ultrasonic central corneal pachymetry of 500 microm. The Chiron Automatic Corneal Shaper and the Meditec Aesculap MEL 60 excimer laser were used. RESULTS: At 12 months, mean myopia decreased from -5.33 diopters (D) +/- 4.22 (SD) to 0.19 +/- 1.71 D, mean hyperopia decreased from +5.04 +/- 3.32 D to + 0.42 +/- 0.46 D, and mean astigmatism decreased from 5.37 +/- 2.12 D to 2.82 +/- 2.42 D (47.5% of mean percentage reduction). Retreatment was necessary in 42.9% of eyes because of cylindrical undercorrection. Uncorrected visual acuity improved in 11 eyes (78.6%). Best spectacle-corrected visual acuity improved in 6 eyes (42.8%) and was maintained in 4 eyes (28.6%); 5 eyes (35.7%) lost 1 Snellen line. Intraoperative complications included 1 buttonhole flap. Postoperative complications included interface epithelial ingrowth at the periphery (2 eyes) and pseudophakic retinal detachment 2 years after LASIK (1 eye). CONCLUSION: Laser in situ keratomileusis after PKP safely and predictably corrected the spherical component of the refraction. However, the predictability of LASIK in correcting post-PKP astigmatism was poor.  相似文献

PURPOSE: We studied the efficacy, predictability, and safety of laser in situ keratomileusis (LASIK) for moderate to high simple and compound myopic astigmatism. METHODS: Ninety-two eyes of 46 consecutive patients who had LASIK for myopic astigmatism (64 eyes, astigmatism 3.00 to 9.00 D; myopia 0 to -20.00 D), or simple myopia (28 eyes, myopia -4.00 to -20.00 D; astigmatism 0 to 0.50 D) were retrospectively studied. Mean baseline spherical equivalent refraction (SE) in the myopia group was -8.11 +/- 3.94 D and in the astigmatism group, -8.55 +/- 4.49 D. All eyes underwent LASIK using the Nidek EC-5000 laser by the same surgeon. RESULTS: At 6 months after LASIK in the myopia group versus the astigmatism group, 24 eyes (85%) vs. 54 eyes (84%) were available for follow-up, 12 eyes (50%) vs. 13 eyes (24%) had uncorrected visual acuity (UCVA) of 20/20, 19 eyes (79%) vs. 44 eyes (81%) had UCVA of 20/40, 8 eyes (33%) vs. 18 eyes (33%) had SE within +/- 0.50 D, 15 eyes (62%) vs. 39 eyes (72%) had SE within +/- 1.00 D, and mean SE was -1.22 +/- 1.17 D vs. -0.74 +/- 1.46 D. Mean astigmatism (vertexed to the corneal plane) in the astigmatism group was 2.77 D at 0 degrees before surgery and 0.32 D at 7 degrees at 6 months. None of the myopic eyes and three of the astigmatic eyes (5%) lost > or = 2 lines of best spectacle-corrected visual acuity. CONCLUSION: LASIK with the Nidek EC-5000 laser for myopic astigmatism was reasonably effective, predictable, and safe. Astigmatism was under-corrected with the nomogram implemented in this study.  相似文献

PURPOSE: To evaluate the visual and refractive results of laser in situ keratomileusis (LASIK) retreatment on eyes with residual myopia with or without astigmatism. METHODS: LASIK retreatment was performed on 35 eyes of 23 patients for correction of residual myopia, with or without astigmatism, with a mean manifest spherical equivalent refraction of -2.17+/-0.82 D (range, -1.00 to -3.87 D) and mean refractive astigmatism of -0.55+/-0.61 D (range, 0 to -1.75 D). Retreatment was performed 3 to 18 months after primary LASIK (mean, 5.1+/-2.6 mo). The corneal flap of the previous LASIK was lifted and laser ablation was performed using the Chiron-Technolas Keracor 116 excimer laser. Follow-up was 12 months for all eyes. RESULTS: At 1 year after retreatment, manifest spherical equivalent refraction was reduced to a mean -0.23+/-0.28 D (range, 0 to -0.87 D), and refractive astigmatism was reduced to a mean -0.16+/-0.25 D (range, 0 to -0.75 D). Thirty-two eyes (91.5%) had a manifest spherical equivalent refraction within +/-0.50 D of emmetropia, and 33 eyes (94.3%) had 0 to 0.50 D of refractive astigmatism. Uncorrected visual acuity was 20/20 or better in 11 eyes (31.4%). Spectacle-corrected visual acuity was not reduced in any eye after retreatment. There were no significant complications. CONCLUSION: LASIK retreatment was effective for correction of residual myopia or astigmatism after primary LASIK. Refractive results were predictable with good stability after 3 months. Lifting the flap during LASIK retreatment was relatively easy to perform and did not result in visual morbidity in eyes treated from 3 up to 18 months after primary LASIK.  相似文献

PURPOSE: To evaluate the visual and refractive results of conventional (non-wavefront) laser in situ keratomileusis (LASIK) for treatment of myopia and myopic astigmatism using the Alcon LADARVision 4000 excimer laser system and nomogram adjustment techniques. METHODS: A retrospective analysis of 499 eyes that had LASIK for myopia and myopic astigmatism was performed. Preoperative manifest spherical equivalent refraction ranged from -0.43 to -6.00 D and preoperative astigmatism ranged from 0 to -4.75 D. Patients were evaluated during 3 months following surgery. RESULTS: One month after surgery, 72% of eyes examined (298/415 eyes) had uncorrected visual acuity (UCVA) of 20/20 or better. Three months after surgery, 83% of eyes examined (216/261 eyes) had UCVA of 20/20 or better. One and three months after surgery, 82% and 83% of eyes, respectively, were within +/-0.50 D of attempted correction; 97% of eyes were within +/-1.00 D at both 1 and 3 months. No eye lost more than 1 line of best spectacle-corrected visual acuity (BSCVA) at 3 months after surgery. At the 3-month examination, 83% of eyes had UCVA better than or equal to preoperative BSCVA. CONCLUSIONS: Conventional LASIK to correct myopia and myopic astigmatism was safe and effective using the Alcon LADARVision 4000 excimer laser system. Outcomes were substantially improved throughout development of an accurate nomogram, derived from continually updated regression analysis of previous refractive results.  相似文献

PURPOSE: To evaluate the safety and predictability of laser in situ keratomileusis (LASIK) retreatment following primary procedures for high myopia and astigmatism. SETTING: Corneal Diseases and Excimer Laser Research Unit, Department of Ophthalmology, University of Dundee, Dundee, United Kingdom. METHODS: This prospective observational study of retreatment comprised a cohort of 109 eyes having primary LASIK for high myopia and astigmatism with a spherical equivalent (SE) of -9.70 diopters (D) +/- 4.06 (SD). Twenty-four eyes (22%) with an initial myopic SE of -9.83 +/- 3.50 D, a comparable subset of the entire group (P < .05), had retreatment for residual myopia (-3.02 +/- 2.17 D) to improve uncorrected visual acuity (UCVA) by reelevating the corneal flap and ablating the stromal bed. RESULTS: The mean follow-up after retreatment was 12.8 +/- 5.1 months (range 1.5 to 24 months; 19 eyes >/=6 months, 13 eyes > or = 12 months). The mean myopic SE was reduced to +0.53 +/- 0.62 D at 1 week, +0.05 +/- 0.50 D at 1 month, +0.30 +/- 0.50 D at 6 months, and +0.18 +/- 0.42 D at the latest follow-up, 12.8 months. At the latest review, 62% of eyes were within +/-0.50 D of emmetropia and 100% were within +/-1.00 D. The mean refraction did not alter statistically between 1 week and subsequent times. The mean UCVA improved from 6/30 prior to retreatment to 6/9 at the latest follow-up. Uncorrected visual acuity of 6/6 or better, 6/9 or better, and 6/12 or better was achieved by 33.0%, 75.0%, and 95.8% of eyes, respectively. No significant complications that led to a loss of best corrected visual acuity were encountered, although retreatment procedures were more uncomfortable than primary procedures and self-limiting; epithelial ingrowth that did not threaten vision was common, and 2 patients complained of nighttime visual symptoms. CONCLUSIONS: Retreatment of residual myopia by reelevating the flap was relatively safe and predictable, with a low risk of sight-threatening complications. However, longer term studies may be required to detect late complications.  相似文献

OBJECTIVE: To describe the safety, effectiveness, and predictability of laser in situ keratomileusis (LASIK) for correcting residual myopia after primary photorefractive keratectomy (PRK). DESIGN: A retrospective, noncomparative case series. PARTICIPANTS AND INTERVENTION: Thirty-six consecutive eyes of 30 patients underwent LASIK after primary PRK. A Multiscan Schwind excimer laser was used for LASIK enhancement. MAIN OUTCOME MEASURES: Uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity, refraction, videokeratography, and complications were determined before and after LASIK retreatment. Follow-up was at least 12 months. RESULTS: Before LASIK, 11.11% of eyes showed a UCVA of 20/40 or better. This increased to 94.44% 12 months after LASIK. A UCVA of 20/25 or better was achieved in 0% before and in 72.22% after retreatment. Refraction +/-0.5 diopters (spherical equivalent) represented 0% of eyes before and 77.78% of eyes after enhancement. Before LASIK, two eyes had significant haze. Haze remained in these two eyes and appeared in another eye. CONCLUSIONS: Laser in situ keratomileusis proved to be safe and effective for treating residual myopia after PRK. Care must be taken when considering LASIK retreatment in patients with significant haze after primary PRK.  相似文献

PURPOSE: To evaluate the safety, efficacy, predictability, and visual results of laser in situ keratomileusis (LASIK) using the Bausch & Lomb Technolas(R) 217 scanning-spot laser for the treatment of myopia and compound myopic astigmatism. SETTING: Stanford Eye Laser Center, Stanford, California, USA. METHODS: In a prospective study, 110 eyes of 58 patients with a mean spherical equivalent (SE) of -4.87 diopters (D) +/- 2.5 (SD) (range -1.0 to -11.4 D) had LASIK for myopia and compound myopic astigmatism using the Technolas 217 scanning-spot laser. The primary outcome variables included preoperative and postoperative best spectacle-corrected visual acuity (BSCVA), uncorrected visual acuity (UCVA), achieved versus attempted correction, vector analysis of astigmatism, and complications. RESULTS: One hundred ten eyes were available for analysis at 3 months. The mean SE refraction was reduced 95% (mean -0.23 +/- 0.4 D), and astigmatism was reduced 70% (mean 0.28 +/- 0.3 D). One day postoperatively, 100% of eyes had a UCVA of 20/40 or better, 96% had 20/25 or better, 77% had 20/20 or better, and 39% had 20/15 or better. At the 3-month examination, 100% of eyes had a UCVA of 20/40 or better, 95% had 20/25 or better, 83% had 20/20 or better, and 48% had 20/15 or better. Ninety percent of eyes were within +/-0.5 D of emmetropia. No eye lost 2 or more lines of BSCVA. CONCLUSIONS: The Technolas 217 scanning-spot laser produced excellent postoperative UCVA and BSCVA. It was predictable, safe, and effective for the treatment of myopia and compound myopic astigmatism. Long-term follow-up is needed to assess the stability of the procedure.  相似文献

PURPOSE: To evaluate the results of laser in situ keratomileusis (LASIK) for uniocular high myopia in pediatric eyes. SETTING: Dr. Agarwal's Eye Hospital, Chennai (Madras), India. METHODS: Sixteen eyes that were treated by LASIK for uniocular high myopia were retrospectively analyzed. The mean patient age was 8.4 years +/- 1.83 (SD) (range 5 to 11 years). Laser in situ keratomileusis was performed using the Technolas Keracor 217 excimer laser and the Automated Corneal Shaper microkeratome, which created a 160 microm corneal lamellar flap. Postoperatively, patients were reviewed at 1 day, 1 week, and 1, 6, and 12 months. The examination included uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), refraction, anterior segment evaluation, grading of haze based on a 5-point scale, intraocular pressure, corneal topography, and fundus evaluation. The Student t test was used for statistical analysis. RESULTS: The charts of all patients were analyzed at the 12 month visit. The mean preoperative spherical equivalent (SE) was -14.88 +/- 3.69 diopters (D) (range -9.00 to -23.00 D) and the mean postoperative SE, -1.44 +/- 1.14 D (range 0 to -2.50 D) (P < .05). The safety index was 1.01 (mean postoperative BCVA 0.54 and mean preoperative BCVA 0.53; P = .77). The efficacy index was 0.53 (mean postoperative UCVA 0.28 and mean preoperative BCVA 0.53). None of the eyes had an induced astigmatism of more than 0.5 D. Twelve eyes regained their BCVA, 2 lost 1 line of BCVA, and 2 gained 1 line. Three eyes had grade 2 haze. No retinal complications were observed. CONCLUSION: In this study, LASIK for uniocular high myopia in pediatric eyes provided encouraging results in the management of select cases of anisometropic amblyopia when other measures failed. A larger study with a longer follow-up is necessary to determine the long-term effects.  相似文献

PURPOSE: To evaluate the efficacy, safety, predictability, and surgically induced astigmatism (SIA) of laser in situ keratomileusis (LASIK) for the correction of myopia and myopic astigmatism. SETTING: Department of Ophthalmology, National Taiwan University Hospital, Taipei, Taiwan. METHODS: This retrospective study comprised 69 eyes that had LASIK to correct myopia and 74 eyes that had LASIK to correct myopic astigmatism. The excimer laser keratectomy was performed using a Summit Apex Plus machine. Refraction, visual acuity, and computerized corneal videokeratography data from the preoperative and postoperative examinations were collected. The astigmatic change was calculated by the Alpins vector analysis method. RESULTS: The preoperative spherical equivalent at the glasses plane in the myopia and myopic astigmatism groups was -8.08 diopters (D) and -9.73 D, respectively. At 6 months, the spherical equivalent and residual corneal astigmatism were -0.25 D and 0.85 D, respectively, in the myopia group and -0.71 D and 0.82 D, respectively, in the myopic astigmatism group. In the myopia group, 88% of eyes were within +/-1.0 D of the intended myopia correction and in the myopic astigmatism group, 85% were within +/-1.0 D of the targeted spherical equivalent and 90% were within +/-1.0 D of the intended astigmatism correction. The uncorrected visual acuity was 20/40 or better in 94.1% of eyes in the myopia group and 92.5% of eyes in the myopic astigmatism group. The SIA magnitude was 0.66 D with the axis randomly distributed in the myopia group. The mean astigmatism correction index was 0.97, the mean magnitude of error was 0.13 D +/- 0.62 (SD), and the mean angle of error was -3.70 +/- 13.73 degrees in the myopic astigmatism group. CONCLUSION: Laser in situ keratomileusis had similar predictability, safety, and efficacy in the treatment of myopia and myopic astigmatism. The astigmatism correction was effective, but the results suggest that subjective astigmatism of less than 1.0 D need not be treated with the Summit Apex Plus laser.  相似文献

PURPOSE: To compare the visual and refractive outcomes of laser in situ keratomileusis (LASIK) and laser-assisted subepithelial keratectomy (LASEK) in the treatment of high myopia. SETTING: Institute of Vision Research, Department of Ophthalmology, College of Medicine, Yonsei University, and Balgeunsesang Ophthalmology Clinic, Seoul, South Korea. METHODS: Four hundred seventy eyes of 240 patients with manifest refraction spherical components greater than -6.00 diopters (D) were assigned to 2 groups: 324 eyes (167 patients) were treated with LASIK and 146 eyes (73 patients), with LASEK. Uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), remaining refractive error, corneal haze, and complications were followed in both groups for 12 months. RESULTS: At 12 months, the mean spherical equivalent (SE) was within +/-0.50 D of emmetropia in 205 eyes (63.3%) in the LASIK group and 81 eyes (55.5%) in the LASEK group and within +/-1.00 D in 261 eyes (80.6%) and 104 eyes (71.2%), respectively. The UCVA was 20/25 or better in 269 LASIK eyes (83.0%) and 111 LASEK eyes (76.0%). There was more than a 1-line loss of BSCVA in 4 LASIK eyes (1.2%) and 21 LASEK eyes (14.3%). The between-group differences in SE, magnitude of cylinder, UCVA, and haze were statistically significant (P<.05). CONCLUSIONS: Both LASIK and LASEK were safe and effectively treated eyes with high myopia. Laser in situ keratomileusis provided superior results in visual predictability and corneal opacity.  相似文献

PURPOSE: To evaluate the efficacy, predictability, and safety of custom laser in situ keratomileusis (LASIK) using the Visx wavefront platform CustomVue for the treatment of myopia and myopic astigmatism. SETTING: Stanford University Eye Laser Center, Stanford, California, USA. METHODS: This retrospective analysis was of the initial 140 eyes of 78 patients treated consecutively with LASIK for myopia using the Visx Star S4 excimer laser. Primary outcome variables, including uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), manifest refraction, and complications, were evaluated at 1 week and 1 and 3 months. Vector analysis was performed on eyes that received astigmatic correction. RESULTS: Mean preoperative spherical equivalent (SE) was reduced from -3.89 diopters +/- 1.48 (SD) to an SE of -0.21 +/- 0.36 D at 1 month and -0.28 +/- 0.36 D at 3 months (P < .001). At 1 and 3 months, UCVA was 20/20 or better in 84.3% and 87.9%, respectively. Eighty-six percent of eyes at 1 month and 81.4% of eyes at 3 months were within +/-0.5 D of emmetropia. No eyes lost more than 2 lines of BSCVA. Vector analysis yielded an index of success of 0.39, indicating a 61% success rate in achieving the astigmatic surgical correction at 3 months. The preoperative root-mean-square value of 0.28 +/- 0.08 microm increased slightly to 0.33 +/- 0.11 microm at 1 month and 0.34 +/- 0.11 microm at 3 months. CONCLUSION: Wavefront-guided LASIK using the Visx CustomVue system was effective, predictable, and safe for correction of low to moderate myopic refractive error.  相似文献

PURPOSE: To evaluate the efficacy and safety of laser in situ keratomileusis (LASIK) for myopic regression and undercorrection after photorefractive keratectomy (PRK). SETTING: The Eye Institute, Sydney, Australia. METHODS: Fifty eyes of 32 patients were treated by LASIK for residual myopia following primary PRK. The mean spherical equivalent refraction (SEQ) was -2.92 diopters (D) +/- 1.57 (SD) (range -0.75 to -7.88 D). The mean refractive cylinder was 0.96 +/- 0.74 D (range 0 to 3.50 D). For analysis, the eyes were divided into 2 groups: those with 0 or low corneal haze (Group 1) and those with severe corneal haze (Group 2). In Group 1, the SEQ was -1.99 +/- 0.79 D (range -0.75 to -3.75 D) and in Group 2, -3.77 +/- 1.62 D (range -0.75 to -7.90 D). The procedure was performed using the Chiron Automated Corneal Shaper and the Summit Apex Plus laser. The mean interval between PRK and LASIK was 25 months (range 9 to 59 months). The following parameters were studied before and after LASIK retreatment: SEQ, mean refractive cylinder, uncorrected visual acuity (UCVA), and best corrected visual acuity (BCVA). Complications after LASIK retreatment were evaluated. RESULTS: Six months after LASIK, the mean SEQ in all eyes was -0.65 +/- 0.86 D (range +1.50 to -3.35 D); 70.0% of eyes were within +/-1.00 D of emmetropia and the UCVA was 6/12 or better in 72.5%. The mean SEQ in Group 1 was -0.22 +/- 0.55 D (range -0.88 to -1.50 D) and in Group 2, -0.97+/- 0.92 D (range 0.12 to -3.25 D); the UCVA was 6/12 or better in 94.0% of eyes in Group 1 and in 56.0% in Group 2. No statistically significant between-group difference was found in lines of Snellen acuity lost or gained at 6 months. No eye lost more than 1 line of BCVA. CONCLUSIONS: Laser in situ keratomileusis appears to be a safe, effective, and predictable procedure for treating eyes with 0 or low haze with residual myopia after PRK. It is less predictable in eyes with severe haze.  相似文献

PURPOSE: To evaluate the results of combining phakic posterior chamber intraocular lens (IOL) implantation and excimer corneal surgery to treat high myopia or myopia with astigmatism. SETTING: Service d'Ophtalmologie, H?pital Purpan, University of Toulouse, Toulouse, France. METHODS: Thirty-two eyes of 28 patients with extreme myopia or myopia combined with astigmatism were treated by implantation of a phakic posterior chamber IOL. Residual refractive errors were treated no earlier than 6 weeks after IOL implantation by photorefractive keratectomy (PRK) in eyes with low refractive errors or by laser in situ keratomileusis (LASIK) in eyes with higher residual refractive errors. RESULTS: The mean preoperative spherical equivalent (SE) refraction was -18.7 diopters (D). The refractive astigmatism ranged from 0 to 3.5 D. After excimer laser treatment, the SE refraction ranged from -0.5 to -2.5 D and the refractive astigmatism, from 0 to 1.5 D in the PRK group. In the LASIK group, spherical ametropia ranged from -1.5 to +1.5 D and astigmatism, from 0 to 1.0 D. After excimer laser treatment, the uncorrected visual acuity improved in all eyes but a loss of 1 line of the corrected vision after IOL implantation occurred in 22.2% of PRK-treated eyes and in 13.6% of LASIK-treated eyes. CONCLUSIONS: Bioptic treatment of extreme myopia and myopia associated with astigmatism appears to be safer and more predictable than other methods of treatment.  相似文献

PURPOSE: To evaluate the effect of preoperative keratometry on the refractive outcome after laser in situ keratomileusis (LASIK) for myopia. SETTING: University Eye Clinic, Prince of Wales Hospital, Hong Kong, China. METHODS: In this retrospective study, the records of patients who had LASIK for myopia greater than -6.0 diopters (D) using the Chiron Automated Corneal Shaper and the Schwind Keratome-F excimer laser were reviewed. RESULTS: Laser in situ keratomileusis was performed in 167 eyes of 103 patients (mean age 34.7 years +/- 7.5 [SD]). Preoperative myopic spherical equivalent (SE) refraction was -9.0 +/- 2.0 D (range -6.0 to -13.9 D). Three months after surgery, SE refraction was -0.04 +/- 1.1 D (range +2.3 to -3.3 D); uncorrected visual acuity > or = 20/40 was present in 91.8% of 110 eyes in which emmetropia was the postoperative goal. Mean preoperative keratometry was 43.9 +/- 1.5 D (range 40.3 to 48.1 D). When eyes were stratified by the degree of preoperative myopia in 1.0 D steps, a trend toward greater undercorrection was noted in eyes with preoperative keratometry < 43.5 D than in those with steeper keratometry (> 44.5 D) in all myopia groups except the -7.0 to -7.9 D group. This difference was statistically significant in eyes with a preoperative SE of -10.0 to -10.9 D and -11.0 to -11.9 D. CONCLUSIONS: Preoperative keratometry appeared to influence the refractive outcome after myopic LASIK. Eyes with flatter corneas tended to have greater undercorrection than eyes with similar myopia and steeper corneas. Validation of these findings in larger data sets using the methodology described may improve the predictability of current LASIK nomograms, particularly in eyes with high myopia.  相似文献

PURPOSE: To determine the safety and efficacy of arcuate keratotomy (AK) to reduce or eliminate steep asymmetrical islands after laser in situ keratomileusis (LASIK) and automated lamellar keratoplasty (ALK). SETTING: Rancho Bernardo Laser and Vision Center, San Diego, California, USA. METHODS: This retrospective and noncomparative interventional study comprised 8 eyes of 5 patients who received arcuate incisions to reduce topographically defined asymmetrical steep islands after LASIK (n = 7) and ALK for hyperopia (n = 1). The islands occurred unexpectedly after standard excimer laser treatment (VISX Star and S2) for myopic astigmatism that induced irregular astigmatism and unwanted optical effects. Cases occurred consecutively over 16 months, an incidence of less than 1%. Arcuate incisions extended 30 or 45 degrees in LASIK eyes based on the amount of residual refractive astigmatism and were placed at 7.0 mm on the steep axis. Radial incisions were limbal sparing to a 5.0 mm optical zone following the Casebeer nomogram. Outcome measures were uniformity of postenhancement topography, uncorrected visual acuity (UCVA), postenhancement refraction, and reduction in unwanted optical effects. RESULTS: The preenhancement mean spherical equivalent (SE) in the LASIK eyes was -0.99 diopters (D) +/- 0.48 (SD); the astigmatism ranged from 0.50 to 1.75 D. After incisional keratotomy, the mean SE was -0.21 +/- 0.12 D and the net residual astigmatism was reduced to a range of 0 to 0.75 D. All 8 eyes had AK to reduce topographically defined steep islands that were causing visual blurring and unwanted optical effects including ghosting, halos, and polyplopia. After enhancement, there was a marked reduction in the asymmetrical steep islands by topography, fewer unwanted optical effects, less refractive astigmatism, and improved UCVA. Two patients retained small inferior steep areas that may indicate forme fruste keratoconus, although this condition was not detected preoperatively by topographical indices. CONCLUSION: Uncorrected visual acuity improved and unwanted optical effects were reduced after AK on the peripheral axis of asymmetrical steep islands that occurred after uneventful LASIK and hyperopic ALK. This relatively simple incisional approach may be used until more advanced topography-linked ablations with rapid and precise eye tracking are perfected and widely available.  相似文献

PURPOSE: To compare the visual outcomes and complications of laser in situ keratomileusis (LASIK) with those of surface treatment by laser-assisted subepithelial keratectomy (LASEK), photorefractive keratectomy with mechanical epithelial removal (M-PRK), and transepithelial photorefractive keratectomy (T-PRK). SETTING: Tertiary care eye center. METHODS: This retrospective review comprised all cases of LASIK, LASEK, M-PRK, and T-PRK performed at King Khaled Eye Specialist Hospital between July 1, 2004, and June 30, 2005. Separate statistical analyses were performed for eyes with low to moderate myopia (spherical equivalent [SE] less than -6.00 diopters [D]) and high myopia (SE -6.00 to -11.25 D). RESULTS: Of 696 eyes that met the inclusion criteria, 464 had LASIK, 104 had LASEK, 69 had M-PRK, and 59 had T-PRK. Eyes with low to moderate myopia had a statistically significantly smaller mean difference between logMAR final postoperative uncorrected visual acuity (UCVA) and preoperative best spectacle-corrected visual acuity (BSCVA) after T-PRK and M-PRK than after LASIK or LASEK. A higher percentage of eyes with high myopia had a final UCVA within +/-2 lines of the preoperative BSCVA with T-PRK than with LASIK, LASEK, or M-PRK. There were more major non-flap-related complications after LASEK than after LASIK, M-PRK, or T-PRK. CONCLUSIONS: In eyes with low to moderate myopia, T-PRK and M-PRK provided slightly better visual outcomes than LASIK or LASEK. In eyes with high myopia, T-PRK provided better visual outcomes than LASIK, LASEK, and M-PRK. Laser in situ keratomileusis was associated with the most major postoperative complications.  相似文献

PURPOSE: To evaluate the results of laser in situ keratomileusis after photorefractive keratectomy. METHODS: Eighty eyes of 80 patients with residual myopia after photorefractive keratectomy were reoperated with laser in situ keratomileusis. The study was retrospective. Laser in situ keratomileusis was performed using the automated corneal shaper microkeratome and Chiron Technolas 217-C d1 excimer laser. Data measured after laser in situ keratomileusis included uncorrected visual acuity, best-corrected visual acuity, refraction, haze, pachymetry, and keratometry. The follow-up was at least 12 +/- 1.6 months (range, 12 to 15 months). RESULTS: After laser in situ keratomileusis the mean spherical equivalent was -0.24 diopters +/- 0.78. (range, -3 to +1.5) at 12 months, and the mean uncorrected visual acuity was 0.76 diopters +/- 0.24 (range, 0.1 to 1). Sixty-five eyes (81.3%) had various degrees of haze after laser in situ keratomileusis. One eye (1.2%) lost 2 lines of best-corrected visual acuity. CONCLUSIONS: Laser in situ keratomileusis enhancement may be a good alternative to correct residual myopia and astigmatism after primary photorefractive keratectomy. Corneal haze is a common problem in these eyes, and the treatment after laser in situ keratomileusis enhancement should be the same as the treatment after primary photorefractive keratectomy.  相似文献

PURPOSE: To prospectively evaluate the safety, efficacy, and visual performance of excimer laser enhancement following photoastigmatic refractive keratectomy (PARK) with the Summit Apex Plus. SETTING: Stanford University School of Medicine Eye Laser Clinic, Stanford, California, USA. METHODS: As part of a Food and Drug Administration clinical trial, 93 eyes of 56 patients with a mean spherical equivalent (SE) of -4.98 diopters (D) +/- 1.80 (SD) (range -1.75 to -8.50 D) had PARK for compound myopic astigmatism using the Summit Apex Plus excimer laser and a poly(methyl methacrylate) erodible mask. Seventeen eyes with a mean SE of -2.08 +/- 0.76 D required excimer laser refractive keratectomy for residual spherical myopia or compound myopic astigmatism. Patients were prospectively followed 1, 3, 6, 9, and 12+ months after the enhancement procedure. Primary outcome variables included uncorrected visual acuity (UCVA), refraction, vector analysis, best spectacle-corrected visual acuity (BSCVA) under standard ambient conditions (photopic, scotopic, and glare), corneal clarity, and contrast sensitivity function curve under photopic and scotopic conditions. RESULTS: At the last postoperative visit, the mean sphere had been corrected 82% to a residual of -0.29 +/- 1.23 D and mean SE had been corrected 65% to a residual of -0.74 +/- 1.27 D. Eighty-two percent of eyes were within +/-1.0 D of attempted correction. Eighty-eight percent had a UCVA of 20/40 or better. Vector analysis demonstrated a difference vector of within +/-1.0 D in 75% of eyes that had PARK retreatment. There was no significant loss in the contrast sensitivity curve. Late regression associated with corneal haze and loss of BSCVA occurred in 2 eyes (11.7%). CONCLUSIONS: Retreatment following PARK for compound myopic astigmatism results in effective reduction in residual spherical myopia and compound myopic astigmatism. An improvement in UCVA without loss of contrast sensitivity can be expected in most eyes. However, regression, corneal haze, and loss of BSCVA may occur. Further studies are indicated to predict risk factors for these complications.  相似文献

PURPOSE: To evaluate the efficacy, predictability, and safety of bitoric laser in situ keratomileusis (LASIK) for the correction of mixed astigmatism. SETTING: Instituto Oftalmológico de Alicante, Alicante, Spain. METHODS: This prospective study included 28 eyes of 21 patients with mixed astigmatism who had bitoric LASIK using the Hansatome microkeratome (Bausch & Lomb Surgical) and the Chiron Technolas 217 excimer laser (Bausch & Lomb Surgical). The main outcome measures were uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), defocus equivalent, blur strength, and refraction. RESULTS: Six months after bitoric LASIK, the mean UCVA was 0.70 +/- 0.23 (SD). The percentage of eyes with a UCVA of 20/40 or better was 78.6% and of 20/20, 21.4%. There was a statistically significant increase in the mean BCVA from 0.71 +/- 0.19 before surgery to 0.83 +/- 0.15 at 6 months (P =.0004). Three eyes (10.7%) lost 1 line of BCVA; 19 eyes (67.9%) gained 1 or more lines. The mean preoperative astigmatism of -4.04 +/- 1.13 diopters (D) was reduced to -0.67 +/- 0.79 D after surgery. The defocus equivalent was less than 1.00 D in 75.0% of eyes and less than 0.50 D in 64.3%. Vector analysis showed that the mean achieved correction was 97.4% of the intended correction. CONCLUSIONS: Bitoric LASIK was a safe, effective, and predictable procedure in the treatment of mixed astigmatism. It is a means to improving BCVA in a significant percentage of patients.  相似文献
