目的综述经阴道路径自然腔道内镜外科的临床应用进展。方法经自然腔道内镜手术是治疗肾脏疾病的新型外科手术方式,理论上可以体表无创达到腹壁无疤痕,减少患者术后痛苦等并发症,可使患者术后尽早恢复。检索收集国内外关于经阴道路径自然腔道内镜外科的相关文献进行综述。结果 PubMed检索结果获得文献68篇,CNKI检索获得文献40篇,文献发表时间主要集中在2007年以后,研究主要关注于经阴道路径自然腔道内镜手术在泌尿外科临床运用中的结果,多数文献的结论有利经自然腔道内镜手术。结论目前经自然腔道内镜手术的理论优势已经得到动物试验和体外模型试验的证实,随着设备及技术的逐步完善,观念的更新及手术操作等问题的解决,经自然腔道内镜手术将创造微创外科的全新时代。

Force sensing in natural orifice transluminal endoscopic surgery

Background  

Natural orifice transluminal endoscopic surgery (NOTES) may represent the next frontier for therapeutic minimally invasive surgery; however, its feasibility is currently limited by the lack of suitable instruments. Identifying the forces required to manipulate tissue during NOTES is a necessary first step in the development of better instrumentation.     

经阴道自然腔道内镜全子宫切除手术的临床对比分析

目的探讨经阴道自然腔道内镜手术(vaginal natural orifice transluminal endoscopic surgery,vNOTES)在全子宫切除中应用的可行性和安全性。 方法回顾性分析2019年4月至12月在中国医学科学院肿瘤医院深圳医院接受vNOTES全子宫切除术(vNOTES组)35例患者情况,结合文献报道的分析经脐单孔腹腔镜手术(经脐单孔腹腔镜组)、传统腹腔镜手术(传统腹腔镜组)患者数据,分析不同途径全子宫切除术的患者基线信息和手术情况。 结果vNOTES组、经脐单孔腹腔镜组、传统腹腔镜组患者基线情况基本均衡具有可比性。vNOTES组手术时间45～172 min,平均(92.9±29.8)min;术中出血量20～400 ml,平均(119.4±83.9)ml;留置尿管时间10～41 h,平均(22.0±7.4)h;术后下床时间5～24 h,平均(17.3±4.2)h;肛门排气时间5～32 h,平均(16.8±6.8)h;术前术后血红蛋白差值0～45 g/L,平均(13.6±11.1)g/L;术后24 h的术后视觉模拟评分法疼痛评分(visual analogue scale,VAS)0～3分,平均(1.0±1.1)分。术后患者满意度评分8～10分,平均(9.6±0.6)分。无邻近器官损伤、无输血、无术后感染等并发症的发生。三组手术时间比较,差异无统计学意义(P>0.05)。与经脐单孔腹腔镜组、传统腹腔镜组相比,vNOTES组术中出血量较多、肛门排气时较短,差异均有统计学意义(P<0.01)。vNOTES组术后24 h VAS较传统腹腔镜组低,差异有统计学意义(P<0.01)。亚组比较结果显示,vNOTES大子宫组和传统腹腔镜组在手术时间方面提示较长,差异存在统计学意义(P<0.05)。与经脐单孔腹腔镜组和传统腹腔镜组相比,vNOTES大子宫组术中出血较多、肛门排气时间较短(P<0.01)。 结论vNOTES全子宫切除是一种安全、可行的手术方式,在术后恢复及患者满意度方面优于经脐单孔腹腔镜及传统腹腔镜。vNOTES大子宫切除可能具有优势。

经自然孔道内镜外科技术(natural orifice transluminal endoscopic surgery,NOTES)是一项颇具魅力、令人颇感新奇的技术.其核心技术是通过自然存在的孔道建立的手术入路将病灶切除(或相关处理),在表面无可见的创口,因而使其成为近期国内外学术会议上的热点话题.     

Hybrid natural orifice transluminal endoscopic surgery for ileocecal resection

Although laparoscopic colectomy is commonly performed around the world,an operative wound formed during the surgery is large but not sufficient enough to convert for the majority of open surgery.Thus,a certain sized skin incision is required to remove the resected colon.Here we report the case of a pure laparoscopic ileocecal resection which involves transanal specimen extraction.We present a case characterized by a laterally spreading type of tumor of the cecum.We performed a pure laparoscopic ileocecal resection and the resected specimen was removed transanally using colonoscopy.Intracorporeal functional anastomosis was then performed using a flexible linear stapling device under supporting barbed suture traction.The patient was discharged without complications on postoperative day 4.Laparoscopic colectomy performed with minimal incision could essentially increase the usage of this surgical technique.Although our method is restricted to flat or small lesions,we think it is a feasible and realistic solution for minimization of operative invasion because it involves specimen extraction through a natural orifice.

Recent advances of natural orifice transluminal endoscopic surgery in urological surgery

Natural orifice transluminal endoscopic surgery designates a surgical procedure that utilizes one or more patent natural orifices of the body with the intention to puncture a hollow viscera in order to enter the abdominal cavity. First carried out at the beginning of the 2000s in experimental models, it can be considered the natural evolution of laparoscopy towards the ideal of scarless surgery, as the avoidance of a large abdominal scar is associated with better cosmetic results and better recovery. However, the technology currently available does not allow the performance of complete pure natural orifice transluminal endoscopic surgery procedures. The surgical tools used are not specially designed for this approach, so difficulties in retracting organs, bleeding control and clashing of instruments are the main obstacle surgeons face. For this reason, the current available technique is the so‐called hybrid approach, where a natural orifice approach is combined with some abdominal trocars, using the natural orifice as the exit door for the specimen removal. As not many comparative studies have been published evaluating the advantages of natural orifice transluminal endoscopic surgery in front of traditional laparoscopic surgery, a review of the history of natural orifice transluminal endoscopic surgery, and an assessment of the available evidence of this technique regarding renal and urological pelvic surgery are performed in this article.
