Abdominal aortic aneurysm (AAA) is a dilatation of the infra-renal abdominal aorta to greater than 3 cm. Population screening is offered to men in the year of their 65th birthday in the UK. Patients with small, asymptomatic AAAs (<5.5 cm) are entered into surveillance programmes and have their cardiovascular risk factors managed aggressively. An AAA ≥5.5 cm diameter, or one which is symptomatic, should be considered for surgical repair to prevent rupture. Aneurysm repair can be undertaken using either an open surgical or endovascular approach; the decision should be tailored to the individual patient and made by the surgeon and patient, with input from a multi-disciplinary team.  相似文献

OBJECTIVE: To determine whether the presence of an iliac aneurysm compromises outcome of endovascular exclusion of AAA and to ascertain the fate of the iliac aneurysmal sac. PATIENTS AND METHODS: Between April 1997 and March 2001, data on 336 consecutive patients undergoing endovascular repair for AAA were entered in a prospective database. Suitability for endovascular repair was assessed by preoperative contrast-enhanced computed tomography. A maximum common iliac artery (CIA) diameter > or = 20 mm was defined as iliac aneurysm. Patients with and without iliac aneurysms were compared to early (immediate conversion or perioperative death) and late failure (increase in aneurysm diameter or persisting graft-related endoleak, or late AAA rupture or conversion). RESULTS: Fifty-nine patients (18%) had iliac aneurysms, 19 were bilateral, for a total of 78 aneurysmal iliac arteries (median diameter 23 mm; range 20-50 mm). A distal seal was achieved by landing in 33 external iliac arteries, in 20 ectatic CIAs, and in 25 normal CIAs. Operating time differed significantly between patients with and without CIA aneurysms (153 +/- 71 vs 123 +/- 55 min, p = 0.0001), whereas no statistically significant differences were found with respect to early and late failure (2% vs 3%, p = 0.5 and 14% vs 8%, p = 0.11, respectively). There were no cases of buttock or colon necrosis. At a median follow-up of 14 months (range 0-46; i.q.r. 7-27 months) common iliac diameter decreased > or = 2 mm in 49 cases, remained stable in 25, and increased > or = 2 mm in 3. CONCLUSION: The presence of iliac aneurysm rendered endoluminal AAA repair more complex but did not affect feasibility and long-term outcome of the procedure. In our experience internal iliac exclusion was never associated with significant morbidity. These data may be useful when considering endovascular repair in high-risk patients with challenging anatomy.  相似文献

INTRODUCTION: abdominal aortic dilatation can occur above the graft following repair of infra-renal abdominal aortic aneurysm (AAA). This study aimed to determine the incidence and possible aetiological associations of recurrent juxta-anastomotic aneurysms following open repair of AAA. METHODS: the diameter of the infra-renal aorta above the graft of 135 patients who had previously undergone open AAA repair was determined using ultrasound. In those where the diameter was greater than 40 mm a CT scan was undertaken. Co-morbid and operative details were determined from the patients and their clinical notes. RESULTS: seven patients had true juxta-anastomotic aneurysms (>40 mm) in the residual infra-renal abdominal aorta, the occurrence of which was associated with tobacco smoking and hypertension. There was no association with other co-morbid factors, surgical operative details or the development of iliac aneurysms (which occurred in 3% of patients). CONCLUSIONS: true juxta-anastomotic aneurysms develop in the residual infra-renal neck of patients following open repair of abdominal aortic aneurysm. Tobacco smoking and hypertension are significant factors associated with the development of these aneurysms. This group of patients may warrant surveillance to prevent aneurysm rupture.  相似文献

OBJECTIVE: Endoluminal repair of abdominal aortic aneurysm (AAA) is predicated on stability of the proximal neck of the aneurysm. Reports on morphologic changes in the proximal neck after endoluminal repair of AAA have thus far been limited in duration to 3 years or less. The aim of this study was to document changes in diameter of the proximal neck of AAA in a group of patients who had undergone endoluminal repair between 5 and 9 years previously. METHODS: Between May 1992 and December 1996, 61 patients with AAA were treated with endoluminal repair by the senior author. The following patients were excluded from the study group: those requiring primary conversion to open repair at the original operation (n = 8), those with false aneurysm (n = 1), and those with dissection in the proximal neck (n = 1). Fifty-one patients (48 men and three women) with a mean age of 71 years remained in the study group. The endoprostheses used were modified Parodi (n = 4), Endovascular Technologies (n = 14), White-Yu (n = 10), Stentor/Vanguard (n = 21), and Bard 1996 prototype (n = 2). Morphologic changes in the proximal aortic neck were studied with contrast computed tomographic scan with the methodology recommended by the Ad Hoc Committee for Standardized Reporting Practices for Endovascular AAA Repair (revised version). The maximum transverse diameter of the proximal neck was measured 1 cm below the most inferior renal artery. A Kaplan-Meier analysis was performed showing the proportion of patients at risk with a demonstrated enlargement of the neck at each interval of time compared with the predischarge computed tomographic scan. A longitudinal study of morphologic changes in the proximal aortic neck was also undertaken in 28 patients with successful endoluminal repair who survived 5 years. RESULTS: The Kaplan-Meier curve showed a probability of no dilatation of the proximal neck of 0.943 at 7 years after endoluminal AAA repair. Of 28 patients with 5 years of follow-up after discharge, only two had increases in the diameter of the proximal neck greater than 2 mm. The endograft in both patients had undergone migration before any proximal neck dilation. A paired t test showed that the overall average increase of 0.4 mm (standard error, 0.3 mm) in these 28 patients was not statistically significant (P =.23). CONCLUSION: A high probability (0.943 at 7 years) exists of no enlargement of the proximal neck of AAA after endoluminal repair. We hypothesize that endografts positioned correctly immediately below the renal arteries protect the proximal neck from dilatation in a manner that does not occur after open repair of AAA.  相似文献

Background: The Asian population is believed to have lower incidence of abdominal aortic aneurysm (AAA), and hence, the benefit of screening is uncertain. The size of native aorta in Asians, which shall affect the definition of AAA, has also never been reported. Our study investigated the prevalence of AAA and the infra‐renal aortic diameter (AD) in Chinese patients with severe coronary artery disease. Methods: This is a prospective observational study of infra‐renal aortic size for patients who had coronary artery bypass surgery by ultrasound. The patients' demographics, important co‐morbidities and maximum AD were recorded. Results: The study included 624 consecutive Chinese patients (mean age = 63.2 years). The mean maximum infra‐renal AD was 17.5 mm for men and 14.8 mm for women. The presence of AAA was defined as maximum AD greater than 30 mm. The result was also compared with an alternate definition that defines AAA as maximum AD of greater than 1.5 times of the group's mean. Eleven patients had an AD greater than 30 mm, and the prevalence of AAA was only 1.8%. With AAA defined as maximum AD of 1.5 times greater than the group's mean, 19 patients had AAA. The prevalence of AAA in this high‐risk group would become 3% overall. Conclusion: The prevalence of AAA in Chinese patients was low, and the result did not support routine screening. The smaller mean infra‐renal AD in Chinese merits validation by large‐scale study and consideration when deciding threshold for small AAA repair in our locality.  相似文献

PURPOSE: To determine the relative rates of common iliac artery (CIA) expansion after elective straight aortic tube-graft replacement of infrarenal abdominal aortic aneurysms (AAA). METHODS: Five participating centers in this 2004 study entered patients they had managed by an aortoaortic tube graft for elective AAA repair. The procedures took place between January 1995 and December 2003. Postoperative computed tomography (CT) scans were obtained for all patients in 2004 to assess changes in CIA diameter. Measurements on preoperative and postoperative CT scans were all made at the same level using the same technique. RESULTS: Entered in the study were 147 patients (138 men, 9 women) with a mean age of 68 years. Mean follow-up from aortic surgery to verification of CIA diameter on the postoperative CT scan was 4.8 years. Mean preoperative CIA diameter was 13.6 mm vs 15.2 mm postoperatively. No patient developed occlusive iliac artery disease during follow-up. Three patients (2%) required repeat surgery during follow-up for a CIA aneurysm. The 147 patients were divided into three groups based on preoperative CIA diameter shown in CT scan: group A (n = 59, 40.1%), both CIA were of normal diameter; group B (n = 53, 36.1%), ectasia (diameter between 12 and 18 mm) of at least one CIA; group C (n = 35, 23.8%), an aneurysm (diameter >18 mm) of at least one CIA. CIA diameter increased by a mean of 1 mm (9.4%) over 5.5 years in group A vs 1.7 mm (12.1%) over 4.3 years in group B and 2.3 mm (12.7%) over 4.2 years in group C. The three patients who required repeat surgery for a CIA aneurysm during follow-up were all in group C. Four variables were associated with aneurysmal change in CIA: initial CIA diameter, celiac aorta diameter on the preoperative CT scan, a coexisting aneurysm site, and the follow-up duration. CONCLUSIONS: Tube-graft placement during AAA surgery is justified even for moderate CIA dilatation (<18 mm). CIA aneurysms with a preoperative diameter > or =25 mm enlarge more rapidly and warrant insertion of a bifurcated graft during the same surgical session as AAA repair. The evolutive potential of CIA between 18 mm and 25 mm in diameter justifies a bifurcated graft when the celiac aorta diameter is >25 mm or the patient's life expectancy is > or =8 years.  相似文献

BACKGROUND: Screening for abdominal aortic aneurysm (AAA) has been carried out in Gloucestershire since 1990. All men in the county are offered aortic ultrasonography in their 65th year. Men with an aortic diameter of less than 26 mm are considered 'normal' and no follow-up is arranged. The aim of this study was to ascertain if men with 'normal' aortic diameters at age 65 years ever develop a clinically significant aneurysm. METHODS: A cohort study was performed on 223 65-year-old men who had an aorta of less than 26 mm in diameter in 1988. These men had repeat ultrasonography in 1993 and 2000. The causes of death in men who died during this interval were investigated. RESULTS: Eight men were lost to follow-up. As far as it was possible to ascertain, none of the 86 men who died over the 12-year interval did so from ruptured AAA. There was no clinically significant increase in mean aortic diameter in the remaining 129 men who had three serial ultrasonographic scans over the 12-year interval. CONCLUSION: A single, 'normal' ultrasound scan at age 65 years effectively rules out the risk of clinically significant aneurysm disease for life in men.  相似文献

Around two-thirds of abdominal aortic aneurysms (AAA) are incidentaldiscoveries during the investigation of backache, hip pain orurinary tract complaints. They are much more common in men thanwomen (5:1) and account for 2% of all deaths in men aged >60yr. Open surgical repair of the aneurysm is considered as thestandard, traditional method of treatment. Surgery is recommendedwhen the AAA exceeds 55 mm in anteroposterior diameter as measuredby ultrasound scan. The risk of spontaneous rupture dependson aneurysm size, ranging from <1% per annum for AAA <55mm diameter to >17% per annum for aneurysms >60 mm diameter.Ninety per cent of AAAs are located distal to the renal arteries. Endovascular repair of an aortic aneurysm using an in-situ prostheticgraft was suggested as a technique in 1969 by Dotter, but wasonly first performed successfully by Parodi and colleagues in1990. Over the last 10 yr, the availability of endovascularstent grafts has provided an alternative treatment for patientswith AAA, especially the elderly with significant co-existingmedical conditions. Endovascular repair is much less invasive.However, it is challenging technically and requires a multidisciplinaryapproach. During endovascular surgery, an aortic stent graft is passedvia the femoral arteries through the aortic lumen to fit tightlyabove and below the AAA. The aim is to exclude the aneurysmsac from the systemic circulation, thereby decreasing or eliminatingthe risk of future rupture. The procedure is performed throughincisions in one or both groins; no laparotomy is required.However, certain anatomical considerations apply.  相似文献

To evaluate the prevalence of abdominal aortic aneurysm (AAA) and occlusive peripheral vascular disease (PVD) in Japanese residents, and to examine the correlations between these diseases and the risk factors of atherosclerosis, 348 residents of a village in central Japan aged between 60 and 79 years were screened. The screening for AAA was performed using ultrasonography (US) and that for PVD was performed by palpation and Doppler US. No AAA was found, and a right common iliac arterial aneurysm was detected in a 79-year-old man (0.3%). The mean diameter of the infrarenal abdominal aorta was 18.7 mm and an abdominal aorta of 25 mm or greater in diameter was seen in 16 participants (4.6%), all of whom need to be followed up. PVD was suspected in two patients (0.6%) with a low ankle brachial pressure index. Of a total of five patients diagnosed or suspected of having a common iliac arterial aneurysm or PVD, four (80%) had at least one risk factor for atherosclerosis. Thus, we conclude that Japanese residents with risk factors predisposing them to atherosclerosis such as hypertension, obesity, abnormal serum lipid levels, and a history of smoking should be selectively screened for AAA and PVD due to the low prevalence of these diseases and from the viewpoint of cost-effectiveness.  相似文献

OBJECTIVE: Accurate data regarding the prevalence and associated risk factors for aneurysmal disease is essential when determining the appropriateness of screening for abdominal aortic aneurysms (AAA). Although women are poorly represented in most large studies of AAA prevalence, the US Preventative Services Task Force recently recommended against primary screening for AAA in women. The purpose of this analysis was to define the prevalence and risk factors associated with the development of AAA in women. METHODS: A free duplex ultrasound screening was offered to men and women with cardiovascular risk factors or a family history of AAA. Patients were recruited through advertising at local screening centers and screenings were performed between 2004 and 2006. Demographic information and cardiovascular and aneurysmal disease risk factors were obtained for each patient through a questionnaire. A total of 17,540 subjects were screened for AAA, including 10,012 women (mean age 69.6 years) and 7528 men (mean age 70.0 years). Univariate and multivariable logistic regression analysis was performed on the subset of women that were screened to determine risk factors for and prevalence of AAA. RESULTS: Seventy-four aneurysms were detected in women (including four aneurysms >5 cm diameter and 70 aneurysms 3 to 5 cm diameter) while 291 were detected in men, resulting in prevalence rates of 0.7% and 3.9%, respectively. Increasing age (odds ratio [OR]= 4.57, 95% confidence interval [CI] 1.98 to 10.54, P < .0001), history of tobacco use (OR = 3.29, 95% CI 1.86 to 5.80, P < .0001), and cardiovascular disease (OR= 3.57, 95% CI 2.19 to 5.84, P < .0001) were independently associated with AAA in women on univariate and multivariable analysis. Women with multiple atherosclerotic risk factors were more commonly found to have AAAs and had a prevalence rate of AAA as high as 6.4%. CONCLUSION: Although the medical literature suggests a low prevalence rate of AAA in women in the general population, specific risk factors are associated with the development of AAA, and subgroups of women can be identified that are at a substantially increased risk of aneurysmal disease. In particular, elevated rates of AAA were found among women of advanced age (> or =65 years) with a history of smoking or heart disease. These data support the notion that women with such risk factors should be considered for AAA screening.  相似文献

PURPOSE: A method of achieving successful completion of endoluminal repair of an abdominal aortic aneurysm (AAA) in the presence of intraoperative iatrogenic rupture of the aneurysm is reported. METHODS: An 83-year-old woman with an AAA that was 7 cm in diameter was treated electively by means of endoluminal repair with a Vanguard bifurcated prosthesis (Boston Scientific, Natick, Mass). No difficulty was experienced with the introduction of the delivery catheter, despite extreme angulation in the aneurysm. An acute episode of hypotension prompted an aortogram to be performed. Extravasation of contrast outside the aneurysm sac was demonstrated. The balloon on the delivery catheter was immediately advanced to the suprarenal aorta and inflated. Hypotension was reversed, and hemodynamic stability was restored, thus enabling deployment of the prosthesis to proceed and the repair to be completed by means of the endoluminal method. RESULTS: The patient's blood pressure remained stable after deflation of the balloon, allowing a postprocedure aortogram to be performed. Exclusion of the aneurysm sac was demonstrated. Exclusion of the aneurysm sac from the circulation and a large retroperitoneal hematoma were confirmed by means of a postoperative contrast computed tomography scan. Convalescence was complicated by acute renal failure, pneumonia, and prolonged ileus. The patient remained well and active at the follow-up examination 6 months after operation. CONCLUSION: Iatrogenic perforation of an AAA during endoluminal repair may be treated by endovascular means and does not necessarily require conversion to open repair, although this may be the safest option.  相似文献

OBJECTIVES: Prevalence of abdominal aortic aneurysms (AAA) is not exactly known among patients with coronary artery disease (CAD) who are considered for surgical revascularisation. We evaluated the value of screening AAA among coronary patients admitted in our cardiovascular surgery unit. METHODS: Over a 24-month period, an abdominal echography was proposed to male patients aged 60 or more while hospitalised for surgical coronary revascularisation. Patients with previous investigation of the aorta were excluded. The aorta was considered aneurysmal when the anterior-posterior diameter was of 30 mm or more. RESULTS: Three hundred and ninety-five consecutive patients all accepted a proposed abdominal echographic screening for AAA. Forty unsuspected AAA were detected (10.1%). The mean diameter was 38.9 +/- 1.3 mm. Four AAA were larger than 50 mm and considered for surgery after the CABG procedure. Surveillance was proposed to the other 36, especially the 10 patients with an AAA larger than 40 mm. Patients with AAA were significantly older than those without AAA (71.3 +/- 0.8 vs. 69.4 +/- 0.3 years, P<0.05). Smoking history (P<0.05) and hypertension (P<0.05) were also associated more frequently with AAA. More than 16% of the patients being smokers and suffering hypertension presented with unsuspected AAA. CONCLUSIONS: In-hospital screening of AAA is very efficient among patients with coronary artery disease. Therefore, patients with CAD may be considered for routine AAA screening.  相似文献

目的:探讨对于复杂型肾下腹主动脉瘤(AAA)行腔内修复(EVAR)治疗的操作要点和治疗效果。方法:回顾行EVAR治疗的15例复杂型肾下AAA患者的临床资料,分析术中操作要点和临床结局。结果:手术技术成功率为100%,无中转开腹病例,1例(6.67%)术后5 d死于急性心衰。瘤颈成角过大2例患者均使用肱-股双导丝技术完成手术;髂动脉狭窄患者7例,4例利用肱-股双导丝技术及球囊扩张后置入支架,其余经球囊扩张完成操作;1例左髂动脉闭塞的患者采用对侧髂动脉进入导丝通过闭塞段完成手术;8例重度扭曲患者通过超硬导丝将扭曲段纠正后释放支架。术中无瘤体破裂、血管穿孔及医源性血管夹层等严重并发症出现。随访期间,1例患者术后2年出现腰椎结核,死于多脏器功能衰竭;内漏3例,二次手术干预1例。结论:随着经验的积累,技术的进步及支架的不断完善,EVAR治疗复杂型肾下AAA是可行、有效的。  相似文献

OBJECTIVES: The aim of this study was to estimate the agreement between ultrasonography (US) and computed tomography (CT) in the measurement of aortic diameter in subjects with and without abdominal aortic aneurysms (AAA). MATERIAL AND METHODS: Sixty-one subjects were investigated, 33 with a diameter >30mm and 28 with a diameter <30mm. Difference and variabily anteroposterior (AP) and transverse (TR) diameters were analysed. RESULTS: In non-aneurysmal aortas US gave larger AP: 2.8mm (95% CI 1.7-4.0) and TR: 3.8mm (2.3-5.3) diameter. In AAAs the mean diameter did not differ significantly. In non-aneurysmal aortas we expect 95% of differences to be <5.7mm in AP and <7.6mm in TR measurements. In aneurysmal aortas we expect 95% of differences to be <8.0mm in AP and <10.6mm in TR measurements. CONCLUSION: The difference and variability between US and CT varies depends on the diameter of the aorta and how it is measured. These differences have importance for clinical practice and for research.  相似文献

目的 探讨腹主动脉瘤大小对腔内隔绝术及隔绝后治疗结果的影响.方法 回顾性分析1997年3月至2007年6月共429例腹主动脉瘤的临床资料,依腹主动脉瘤直径将患者分为<55 mm组(A组,n=274)及≥55 mm组(B组,n=155).根据术前影像学资料研究两组动脉瘤大小、瘤颈长度、瘤颈直径、瘤颈扭曲程度及髂动脉是否受累等,并探讨动脉瘤大小对腔内隔绝术及治疗结果的影响.结果 A组平均年龄71.1岁,B组73.7岁(P<0.05).B组有冠心病史者(36.1%)明显多于A组(18.6%)(P<0.05),B组伴高血压病、糖尿病、慢性阻塞性肺疾病者多于A组,但两组间差异无统计学意义.A组腹主动脉瘤平均直径为(46.6±6.8)mm,B组为(66.8±11.2)mm(P<0.05);B组腹主动脉瘤较A组近端瘤颈短、瘤颈直径大、瘤颈扭曲、易累及髂动脉(P<0.05);B组患者应用腹膜外径路、髂内动脉重建或髂内动脉栓塞等附加手术、术中牵张导丝均多于A组,术中发生内漏数及使用移植物个数均高于A组(P<0.05).围手术期并发症发生率B组高于A组,病死率无明显差异.术后B组内漏率及二次干预率均高于A组.结论 腔内隔绝术治疗腹主动脉瘤获得了较好的临床效果,而腹主动脉瘤的大小对腔内隔绝术存在一定影响,较小腹主动脉瘤无论在术中操作、围手术期并发症、术后随访等方面均优于较大腹主动脉瘤.  相似文献

目的:探讨腹主动脉瘤合并髂动脉瘤的腔内修复术(EVAR)方法。方法:回顾性分析2007年8月—2014年3月35例腹主动脉瘤合并髂动脉瘤行EVAR术患者资料,其中9例合并单侧髂内动脉瘤,1例合并双侧髂内动脉瘤,14例合并单侧髂总动脉瘤(直径18 mm),11例合并双侧髂总动脉瘤,所用腔内技术包括栓塞髂内动脉瘤后覆盖,髂内动脉瘤单纯覆盖,"喇叭口"支架,以及"三明治"技术重建一侧髂内动脉等。结果:所有腔内技术均获得成功,手术时间(125±40)min,出血量(173±65)m L。术中发现内漏8例(22.9%),其中I型内漏4例(近端2例,远端2例)均经球囊扩张后内漏消失,III型内漏1例,经扩张及部分加弹簧圈栓塞后内漏消失,II型内漏2例及IV型内漏1例,均未予处理。35例术后随访6~60个月,无动脉瘤破裂,2例术后6个月发现腹主动脉瘤体增大,造影确诊远端I型内漏,经弹簧圈栓塞后内漏消失,其余33例瘤体直径无增大。结论:对于合并髂动脉瘤的腹主动脉瘤患者,有效处理髂内动脉,然后根据髂总动脉直径选择合适的治疗方法可以达到理想的近期效果。  相似文献

目的:探讨胸主动脉夹层动脉瘤合并腹主动脉夹层动脉瘤病人一期腔内隔绝术治疗的可行性、手术操作技巧及并发症防治原则。方法和结果:1例Stanford B型胸主动脉夹层动脉瘤合并腹主动脉夹层动脉瘤及双侧髂动脉瘤的病人于2006年3月在本中心接受了腔内隔绝术。MRA检查提示.主动脉弓降交界处开始出现夹层.真腔受压变窄,以胸腹交界处及腹主动脉中段最明显,最扁窄处为0.5cm;假腔在腹主动脉中段明显,最大径约5.0cm,假腔再人口位于左髂总动脉近端。双侧髂总动脉迂曲并呈瘤样扩张。腹腔干、肠系膜上动脉及双侧肾动脉均发自真腔。手术在全麻下进行:降主动脉植入规格为34-34-100mm的直管型Talent移植物,封闭夹层裂口:腹主动脉植入规格为AOI26-12-170mm Talent移植物,远端连接12.12.68mmTalent移植物至一侧髂外动脉,行双侧股-股转流。瘤体隔绝完全,手术约耗时300min,失血1000ml,透视6min,使用威视派克450ml。术后21d出院。术后随访半年,病人生活质量良好,复查CTA显示:移植物通畅,瘤腔内均完全形成血栓。结论:腔内隔绝术的微创特点使一期治疗Stanford B型主动脉夹层动脉瘤合并腹主动脉瘤成为一种比较安全的手术。主动脉长段隔绝也有利于降低截瘫的发生率。  相似文献

Abdominal Aortic Aneurysm (AAA) is a dilatation of the infra-renal abdominal aorta to greater than 3 cm. Population screening is offered to men in their 65th year in the UK. Patients with small AAAs (<5.5 cm) are entered into surveillance programs and should have cardiovascular risk factors managed aggressively. An AAA with ≥5.5 cm diameter should be considered for surgical repair to prevent rupture. Open surgical repair has proven to be a durable treatment for AAA and while less often performed than endovascular aneurysm repair (EVAR) it remains a common approach in the surgical management of AAA. While associated with higher short-term risks than EVAR, the long-term outcomes are similar and many younger patients have a preference for open repair as routine follow-up is not required.  相似文献

OBJECTIVE: There is evidence of a negative association between diabetes and abdominal aortic aneurysm (AAA). The aim of this study was to assess whether there is a similar relationship between both diabetes and glucose level, and infra-renal aortic diameter throughout its range. DESIGN AND METHODS: Infra-renal aortic diameter was measured using ultrasound in 12,203 men aged 65-83 years as part of a trial of screening for AAA. A range of cardiovascular risk factors were also assessed. In a follow-up study, fasting serum glucose was measured in 2,859 non-diabetic men. Aortic diameter was logarithmically transformed and treated as both a continuous and categorical variable in stepwise multivariate linear and logistic models. RESULTS: The median aortic diameter was slightly smaller in the diabetic men (21.3+/-3.9 vs 21.6+/-3.8, P<0.0001). There was an independent negative association between diabetes and AAA (OR 0.79, 95% CI: 0.63,0.98), and an inverse correlation (Coefficient: -0.0064, p=0.0024) between fasting glucose and aortic diameter in non-diabetic men. CONCLUSIONS: Diabetes is inversely associated with both AAA and aortic diameter in men over 65 years. This association is independent of other risk factors for AAA. Aortic diameter also has an inverse relationship with fasting glucose concentrations in men without diabetes.  相似文献

OBJECTIVES: This study aimed to determine the incidence of abdominal aortic aneurysm (AAA) in a large group of siblings of Australian AAA patients to determine if screening in this group is justified. METHODS: 1254 siblings of 400 index AAA patients were identified and offered aortic ultrasound screening. An age and sex matched control group was recruited from patients having abdominal CT scans for non-vascular indications. AAA was defined by an infrarenal aortic diameter of > or =3 cm or a ratio of the infrarenal to suprarenal aortic diameter of > or =2.0. A ratio of 1.0-1.5 was considered normal, and a ratio of >1.5 to <2.0 was considered ectatic. Aortic enlargement was defined as ectasia or aneurysm. RESULTS: 276 (22%) siblings could be contacted and agreed to screening or had previously been diagnosed with AAA. All 118 controls had normal diameter aortas. 55/276 siblings had previously been diagnosed with AAA. The remaining 221 siblings underwent ultrasound screening. Overall, 30% (84/276) had enlarged aortas (5% ectasia, 25% aneurysmal); 43% of male siblings (64/150) and 16% of females siblings (20/126). The incidence was 45% in brothers of female index patients, 42% in brothers of male patients, 23% in sisters of female patients, and 14% in sisters of male index patients. CONCLUSIONS: The overall incidence of aortic enlargement of 30% found in this study warrants a targeted screening approach with ultrasound for all siblings of patients with AAA. A similar targeted approach for screening of the children of AAA patients would also seem advisable.  相似文献
