The detection and characterization of pancreatic tumors as well as the reliable staging of pancreatic cancer are important tasks for radiologic evaluation. Contrast-enhanced helical CT has been the standard modality for pancreatic imaging in many institutions, but MR imaging has gained a considerable role in the evaluation of patients with equivocal CT findings. Recently, the first organ-specific MR contrast agent targeted to the liver and pancreas, mangafodipir trisodium, has been registered in the European Union (EU) for use in MR imaging of the pancreas. This paper reviews technical considerations and characteristic imaging findings of mangafodipir-enhanced MR imaging in the assessment of focal pancreatic lesions. Contrast-enhanced MRI has proven to be very helpful in the detection of small tumors or the identification of tumor-simulating lesions in patients with equivocal CT findings. Mangafodopir may improve the staging of pancreatic cancer by increasing the sensitivity of MRI in the detection of liver metastases. The liver is more often involved with metastatic disease than primary liver tumors. The accurate detection and characterization of liver metastases are crucial since patient management depends on it. The imaging options, mainly consisting of contrast-enhanced ultrasound (CEUS), multidetector computed tomography (CT), magnetic resonance imaging (MRI) with diffusion-weighted imaging (DWI), extra-cellular contrast media and liver-specific contrast media as well as positron emission tomography/computed tomography (PET/CT), are constantly evolving. PET/MRI is a more recent hybrid method and a topic of major interest concerning liver metastases detection and characterization. This review gives a brief overview about the spectrum of imaging findings and focus on an update about the performance, advantages and potential limitations of each modality as well as current developments and innovations.  相似文献

The early detection of focal liver lesions, particularly those which are malignant, is of utmost importance. The resection of liver metastases of some malignancies (including colorectal cancer) has been shown to improve the survival of patients. Exact knowledge of the number, size, and regional distribution of liver metastases is essential to determine their resectability. Almost all focal liver lesions larger than 10 mm are demonstrated with current imaging techniques but the detection of smaller focal liver lesions is still relatively poor. One of the advantages of magnetic resonance imaging (MRI) of the liver is better soft tissue contrast (compared to other radiologic modalities), which allows better detection and characterization of the focal liver lesions in question. Developments in MRI hardware and software and the availability of novel MRI contrast agents have further improved the diagnostic yield of MRI in lesion detection and characterization. Although the primary modalities for liver imaging are ultrasound and computed tomography, recent studies have suggested that MRI is the most sensitive method for detecting small liver metastatic lesions, and MRI is now considered the pre-operative standard method for diagnosis. Two recent developments in MRI sequences for the upper abdomen comprise unenhanced diffusion-weighted imaging (DWI), and keyhole-based dynamic contrast-enhanced (DCE) MRI (4D THRIVE). DWI allows improved detection (b = 10 s/mm(2)) of small (< 10 mm) focal liver lesions in particular, and is useful as a road map sequence. Also, using higher b-values, the calculation of the apparent diffusion coefficient value, true diffusion coefficient, D, and the perfusion fraction, f, has been used for the characterization of focal liver lesions. DCE 4D THRIVE enables MRI of the liver with high temporal and spatial resolution and full liver coverage. 4D THRIVE improves evaluation of focal liver lesions, providing multiple arterial and venous phases, and allows the calculation of perfusion parameters using pharmacokinetic models. 4D THRIVE has potential benefits in terms of detection, characterization and staging of focal liver lesions and in monitoring therapy.  相似文献

OBJECTIVE: CT is the established imaging technique for evaluation of pancreatic adenocarcinoma. MRI, however, can play a major role in this disease. The objective of this study is to illustrate the strengths of MRI for evaluating pancreatic adenocarcinoma. CONCLUSION: The superior soft-tissue contrast of MRI compared with CT is useful in the detection and characterization of non-contour-deforming pancreatic masses. MRI compared with CT may be more sensitive in the detection of distant disease, better for defining appropriate surgical candidates, and better for characterizing small liver metastases and peritoneal and omental metastases.  相似文献

Magnetic resonance imaging (MRI) of the liver has made considerable progress due to improvements in the examination technique. Sensitivity for the detection of focal liver lesions is higher for MRI than for CT. In the differential diagnosis of liver tumors MRI is remarkably accurate. This is particularly true for hemangiomas, liver cell carcinomas and focal nodular hyperplasias. From a clinical view point differentiation between hemangiomas and metastases is of utmost importance. Future improvements in MR diagnosis of liver diseases are expected due to fast imaging techniques and liver-specific contrast agents.  相似文献

Metastatic disease to the liver is an important disease from the diagnostic, prognostic and therapeutic points of view. Different imaging modalities, such as US, CT, scintigraphy, and MRI, have been used for detection, characterization, therapy planning, and follow-up of this disease with variable degrees of success and failure. This review handles the problems which face the different imaging modalities in diagnosis of liver metastases in view of the pathological background of the disease. It also discusses the indications, strong points, and shortcomings of each of the imaging modalities in diagnosis of metastases, and surveys the recent efforts done to improve their performance through the optimization of quality control and in the innovations in the field of contrast agents. Finally, a protocol is suggested for the clinical management of patients with liver metastases to optimize cost-effectiveness of the imaging modalities in this era of multimodality approach in diagnostic imaging. Hepatic hemangioma presenting atypical radiologic findings: a case report   总被引:3,自引:0,他引:3  
A 69-year-old woman was referred to our hospital due to a liver tumor that was incidentally noted on ultrasound (US). US revealed a pedunculated mass of 5 cm in diameter, with a heterogeneous echo pattern. On arterial phase dynamic contrast-enhanced computed tomography (CT), a tiny enhancing dot in the upper aspect of the mass was seen; whereas, the main portion of the lesion appeared as hypoattenuating. The tumor was of low intensity on T1-weighted magnetic resonance (MR) images, and showed slightly heterogeneous high intensity on T2-weighted MR images. The most characteristic feature of the tumor was its exophytic appearance. On post-gadolinium hepatic arterial dominant-phase MR images, the tumor showed nodular enhancement centrally, with progressive spread of enhancement on later images. Angiography showed dilatation of the right posterior inferior branch of the hepatic artery and C-shaped opacification. Since we could not rule out malignancy for these nonspecific radiologic findings, a partial resection of the liver was carried out, resulting in a pathological diagnosis of hepatic hemangioma. This hemangioma had marked hyalinization and fibrosis, causing a heterogeneous appearance on MR images. The tumor presented an exophytic appearance, which caused some diagnostic confusion.  相似文献

Detection and characterization of focal lesions in the cirrhotic liver may pose a diagnostic dilemma. Several benign and malignant lesions may be found in a cirrhotic liver along with hepatocellular carcinoma (HCC), and may exhibit typical or atypical imaging features. In this pictorial essay, we illustrate computed tomography and magnetic resonance imaging findings of lesions such as simple bile duct cysts, hemangioma, focal nodular hyperplasia-like nodules, peribiliary cysts, intrahepatic cholangiocarcinoma, lymphoma, and metastases, all of which occur in cirrhotic livers with varying prevalences. Pseudolesions, such as perfusion anomalies, focal confluent fibrosis, and segmental hyperplasia, will also be discussed. Imaging characterization of non-HCC lesions in cirrhosis is important in formulating an accurate diagnosis and triaging the patient towards the most appropriate management.The detection and characterization of focal lesions in a cirrhotic liver on computed tomography (CT) and magnetic resonance imaging (MRI) is a challenging task due to the marked changes in the organ architecture. Although hepatocellular carcinoma (HCC) is the most frequent primary tumor arising in a cirrhotic liver, several other benign and malignant lesions may be encountered in this setting (1, 2). It is thus not surprising that CT and MRI have limited specificity for the diagnosis of HCC in cirrhosis.Imaging characterization of focal lesions in cirrhosis is of the utmost importance for appropriate patient management. The radiologist’s primary task is to maximize tumor detection (i.e., minimize false negatives), because missing the diagnosis of HCC may preclude potentially curative therapies, such as hepatic resection, percutaneous ablation procedures, and, in selected patients, liver transplantation. However, it is equally important to avoid the misdiagnosis of benign liver lesions as HCC (i.e., minimize false positives) because this diagnostic interpretation may incorrectly increase the tumor burden. This may also result in the ineligibility of the patient for potentially curative treatments or the inappropriate assignment of increased priority scores for patients on the waiting list for liver transplantation. In this paper, we discuss and illustrate CT and MRI features of both common and uncommon non-HCC liver lesions occurring in cirrhotic patients.  相似文献

Since the advent of second generation ultrasound (US) contrast agents, ultrasound has caught up with other imaging modalities for the detection and characterization of liver metastases and as a result of its high temporal and spatial resolution it can in some cases even be superior to computed tomography (CT) and magnetic resonance imaging (MRI). Many studies have demonstrated a sensitivity and specificity of over 90%. Due to its high temporal resolution contrast-enhanced US (CEUS) is capable of detecting even a very short duration of hyper-enhancement during the arterial phase. Radiation protection and lack of adverse effects on renal or thyroid function are additional arguments why CEUS should be recommended as the first imaging modality in the evaluation of hepatic metastases in cases of favorable scanning conditions.  相似文献

The development in oncologic liver surgery as well as modified interventional therapy strategies of the liver have resulted in improved diagnostic imaging. The evolution of contrast agents for MR imaging of the liver has proceeded along several different paths with the common goal of improving liver-lesion contrast. In MRI contrast agents act indirectly by their effects on relaxation times. Contrast agents used for hepatic MR imaging can be categorized in those that target the extracellular space, the hepatobiliary system, and the reticuloendothelial system. The first two result in a positive enhancement, the last one in a negative enhancement. Positive enhancers allow a better characterization of liver metastases using dynamic sequence protocols. Detection rate of liver metastases is increased using hepatobiliary contrast-enhanced MRI compared to unenhanced MRI. Negative enhancers, iron oxide particles, significantly increase tumor-to-liver contrast and allow detection of more lesions than other diagnostic methods. Iron-oxide enhanced MRI enables differential diagnosis of liver metastases comparing morphologic features using T2 and T1-weighted sequences.  相似文献

目的:探讨磁共振扩散加权成像对结直肠癌肝转移的诊断价值。方法:27例结直肠癌患者行肝脏磁共振扩散加权成像、非增强磁共振和多层CT检查。阅片前告知阅片者患者为结直肠癌术后,但不提供临床病史和既往影像学资料。结果:多层CT、非增强磁共振和扩散加权成像对肝转移灶的敏感度分别为72%(45/63)、76%(48/63)和93%(58/63),扩散加权成像对肝转移灶具有更高的敏感度且与多层CT和非增强MRI相比,差异有显著性意义。扩散加权成像对肝转移患者敏感度最高(82%),而多层CT和非增强MRI分别为77%和66%。结论:磁共振扩散加权成像对结直肠癌肝转移灶的检出率高于多层CT和非增强MRI。  相似文献

OBJECTIVE: The purpose of this study was to compare mangafodipir trisodium-enhanced MR imaging performed with a phased array coil and contrast-enhanced single-detector helical CT for accuracy in the detection and local staging of pancreatic adenocarcinoma and in the differentiation between cancer and focal pancreatitis. SUBJECTS AND METHODS: Forty-two patients with suspected pancreatic masses underwent contrast-enhanced helical CT and mangafodipir trisodium-enhanced MR imaging at 1.5 T. The images were assessed for the presence or absence of tumors; characterization of masses; and presence of vascular invasion, lymph node metastases, or liver metastases. Imaging findings were correlated with findings at laparotomy, laparoscopy, biopsy, or follow-up. RESULTS: Focal masses were present in 36 patients (cancer, n = 26; focal pancreatitis, n = 7; other, n = 3). The sensitivity for lesion detection of MR imaging was 100% and of CT, 94%. Two small malignant lesions were missed on CT. For the diagnosis of tumor nonresectability, the sensitivity of MR imaging and CT was 90% and 80%, respectively. Liver metastases were missed on MR imaging in one of the eight patients and on CT in four. For differentiation between adenocarcinoma and nonadenocarcinoma, the sensitivity of MR imaging was 100% (positive predictive value, 90%; negative predictive value, 100%), and the sensitivity of CT was 92% (positive predictive value, 80%; negative predictive value, 67%). Receiver operating characteristic analysis revealed that the mean area under the curve for MR imaging was 0.920 and for CT, 0.832 (not significant). CONCLUSION: Mangafodipir trisodium-enhanced MR imaging is as accurate as contrast-enhanced helical CT for the detection and staging of pancreatic cancer but offers improved detection of small pancreatic metastases and of liver metastases compared with CT.  相似文献

Radiologic assessment of metastases to the thyroid gland   总被引:2,自引:0,他引:2  
PURPOSE: We reviewed the radiologic and clinical data in patients with metastatic disease to the thyroid gland and assessed the role of radiologic techniques in this disorder. METHOD: The findings on US (n = 11), CT (n = 7), MRI (n = 6), palpation or US-guided fine needle aspiration (FNA) biopsy, and clinical records were reviewed in 11 cases of pathologically verified metastatic tumors of the thyroid gland. RESULTS: Five patients had palpable thyroid nodules and six had nonpalpable nodules discovered incidentally with imaging procedures. Three patients had no known malignancies at the time of diagnosis of thyroid tumors. Correct diagnosis was obtained in 10 of the 11 cases with FNA biopsy. Thyroid metastases were detected in all of the cases with US and MRI and six of the seven cases with CT. Thyroid metastases were solitary (n = 5) or multiple (n = 6), and about half of them measured <2 cm in diameter. These tumors typically had well defined margins and no calcification and sometimes had cystic portions. Multiple nodules within the same patient were radiologically quite similar to each other. On US, metastases appeared as hypoechoic or markedly hypoechoic areas without halo, on CT as low density areas, and on MRI as areas of varying signal intensities. Half of the metastases showed hypointensity on either T2-weighted images or gadolinium-enhanced T1-weighted images. The tumors involved lymph nodes in 10 cases and other remote organs in 5. Level I or II or parotid nodes were involved in six cases. CONCLUSION: These radiologic features may alert clinicians to a possibility of metastatic thyroid cancer. US combined with US-guided FNA biopsy is suitable for early diagnosis of metastases to the thyroid gland.  相似文献

The efficacy of contrast-enhanced magnetic resonance imaging (MRI) for detecting and characterizing, or excluding, hepatic masses was assessed in 404 patients, following the intravenous administration of mangafodipir trisodium (MnDPDP) injection, a hepatic MRI contrast agent. An initial contrast-enhanced computed tomography (CT) examination was followed by unenhanced MRI, injection of MnDPDP (5 micromol/kg IV), and enhanced MRI at 15 minutes post injection. Agreement of the radiologic diagnoses with the patients' final diagnoses was higher for enhanced MRI and for the combined unenhanced and enhanced MRI evaluations than for unenhanced MRI alone or enhanced CT using the clinical diagnosis as the gold standard. Mangafodipir-enhanced MRI uniquely provided additional diagnostic information in 48% of the patients, and patient management was consequently altered in 6% of the patients. MnDPDP-enhanced MRI was comparable or superior to unenhanced MRI and enhanced CT for the detection, classification, and diagnosis of focal liver lesions in patients with known or suspected focal liver disease.  相似文献

PURPOSE: To describe the imaging features of inflammatory pseudotumors of the liver. INTRODUCTION: Inflammatory pseudotumors of the liver are rare benign lesions that may simulate malignancy on imaging studies. Diagnosis is most frequently confirmed after surgical resection of the lesion. MATERIALS AND METHODS: Retrospective study from 1998 to 2006 of histologically proven cases of inflammatory pseudotumors of the liver. A combination of the following imaging modalities were utilized: US, contrast enhanced US, helical CT and MRI. RESULTS: A total of seven lesions (mean diameter of 61.4 mm) were detected in 6 patients (mean age of 66 years). Clinical and laboratory results were non-specific. The following imaging studies were available: US in 5 cases, including one with contrast material, CT in 5 cases and MRI in 3 cases. All tumors were hypoechoic on US, with no enhancement after injection of Levovist. The tumors were generally hypodense on noncontrast CT and enhancement, when present, was delayed and moderate. On MRI, the tumors were iso- or slightly hyperintense on T2W images and iso- or slightly hypointense on T1W images with subtle peripheral enhancement on delayed imaging. CONCLUSION: The differential diagnosis of inflammatory pseudotumor of the liver should be known to radiologists and could be suggested in a clinical context of chronic inflammatory process in patients with non-specific liver mass showing imaging features of partial fibrosis with delayed enhancement.  相似文献

目的:探讨高场MRI在肝转移瘤(HMs)中的检出、诊断与鉴别诊断价值。方法:收集经临床、影像证实的HMs 43例,所有病例均进行高场MRI平扫和动脉期、门脉期、延迟期扫描,其中23例尚做螺旋CT平扫、动脉期、门脉期扫描,分析记录HMs螺旋CT、高场MRI平扫及多期增强扫描特征。结果:共计174枚瘤灶在MRI平扫上呈长T2长T1信号,动脉期122枚瘤灶不强化,门脉期174枚瘤灶均呈环状强化,延迟期108枚瘤灶呈向心性充填;23例79枚瘤灶螺旋CT平扫均呈低密度,动脉期49枚瘤灶不强化,30枚呈环状强化,门脉期73枚呈环状强化,4枚呈向心性充填;43例HMs高场MRI均准确诊断,螺旋CT漏检2例,误诊7例。结论:高场MRI对HMs的检出、诊断与鉴别诊断具重要价值,可作为HMs的首选检查方法。  相似文献

The liver has a unique dual blood supply, which makes helical computed tomography (CT) a highly suitable technique for hepatic imaging. Helical CT allows single breath-hold scanning without motion artifacts. Because of rapid image acquisition, two-phase (hepatic arterial phase and portal venous phase) evaluation of the hepatic parenchyma is possible, improving tumor detection and tumor characterization in a single CT study. The arterial and portal venous supplies to the liver are not independent systems. There are several communications between the vessels, including transsinusoidal, transvasal, and transplexal routes. When vascular compromise occurs, there are often changes in the volume of blood flow in individual vessels and even in the direction of blood flow. These perfusion disorders can be detected with helical CT and are generally seen as an area of high attenuation on hepatic arterial phase images that returns to normal on portal venous phase images; this finding reflects increased arterial blood flow and arterioportal shunting in most cases. Familiarity with the helical CT appearances of these perfusion disorders will result in more accurate diagnosis. By recognizing these perfusion disorders, false-positive diagnosis (hypervascular tumors) or overestimation of the size of liver tumors (eg, hepatocellular carcinoma) can be avoided.  相似文献

Sensitivity of PET/MR images in liver metastases from colorectal carcinoma   总被引:1,自引:0,他引:1  
Our aim was to evaluate the sensitivity of positron emission tomography/magnetic resonance image (PET/MRI) in the detection of liver metastases in patients from colorectal cancer as compared with computed tomography (CT), magnetic resonance imaging (MRI), PET and PET/CT images. From April 2008 to April 2010, twenty-four patients (mean age 56.5±10.5 years) with liver metastases from colorectal cancer diagnosed by pathology were retrospectively studied as above. All image data were respectively collected and fused. PET/CT and PET/MRI fusion images were successfully performed with a PET-MR-CT robot transmission-fusion imaging system. Pathologic findings and clinical follows-up were performed as referenced standards. Images were reviewed independently by at least three experts. We found a total number of 121 metastatic lesions and 35 of them, with a maximum diameter less than 1cm. According to a per-lesion analysis, the sensitivity on liver metastases was 64.5%, 80.2% and 54.5% on CT, MRI and PET, respectively. Based on reconstruction imaging analysis, PET/CT and PET/MRI showed sensitivities of 84.2% and 98.3%. Sensitivity comparison of PET/MRI had superior sensitivity of 98.08%. Paired data analysis (McNemar) resulted a type I error which equated to 0.05. There was a statistically significant difference between CT and MRI or PET for the detection of patients with liver metastatic lesions (P<0.05). However, PET/MRI can efficiently detect more metastatic lesions than PET/CT (P<0.05) among those with diameter <1cm. In conclusion, PET/MRI was a quite efficient diagnostic modality compared to conventional imaging modalities and should be considered the procedure of choice in the detection of liver metastatic lesions from colorectal cancer.  相似文献

OBJECTIVE: To compare MRI of the liver with mangafodipir trisodium (MnDPDP) and computed tomography with intravenous contrast media in the follow-up of liver metastases in patient on chemotherapy for colorectal carcinoma. MATERIALS AND METHODS: This was a prospective study with patients on chemotherapy for liver metastases from colorectal cancer. Patients underwent both contrast-enhanced helical CT using 2 cc/kg contrast at 3 cc/sec and mangafodipir trisodium-enhanced MR imaging at 1.5 T using 2-3 cc/min contrast at 5 micro mol/kg within a two week interval. Two experienced radiologists independently reviewed all scans in a blinded fashion and recorded image quality as well as presence and number of liver lesions. Statistical analysis was performed using the wilcoxon signed rank test. RESULTS: All examinations were of good quality. A total of 71 lesions were detected at CT, with 69 lesions consistent with metastases and 2 lesions consistent with cysts. A total of 98 lesions were detected at MRI, with 97 consistent with metastases and 1 lesion consistent with a cyst. T1 weighted images with MnDPDP significantly detected two additional lesions compared to CT (p<0.05). No significant difference was demonstrated between T1 weighted images without MnDPDP and CT or between T2 weighted images and CT. CONCLUSION: Magnetic resonance imaging with MnDPDP is significantly more sensitive than unenhanced MRI and helical CT for the follow-up of liver lesions.  相似文献

The liver is a common site for various benign and malignant focal lesions. The initial modality for assessing liver lesions is ultrasound or CT. MRI with its superior soft tissue contrast offers multiple advantages over other imaging modalities. Contrast agents have been developed that increase the detection rate and provide more specific information in comparison to unenhanced techniques. In the mean time three classes are available for MR imaging of the liver: extracellular gadolinium chelates, hepatobiliary and reticulo-endothelia, superparamagnetic agents. We describe in this review the most common focal lesions, their diagnostic possibilities, and the imaging protocols. Clinical use of these contrast agents facilitates detection and differential diagnosis of focal liver lesions that may help to avoid invasive procedures such as biopsy for lesion characterization.  相似文献
