目的:探讨能维持机械通气下的早产儿最佳中性体温的护理方法。方法:将在我院NICU住院的给予机械通气的早产儿100例随机等分为试验组和对照组,试验组对在辐射式抢救台上行机械通气下的早产儿,使用保鲜膜覆盖及水床式鸟巢的护理方法;对照组给予常规护理方法。观察两组患儿入院后2,12,48 h的体温情况。结果:使用保鲜膜覆盖及水床式鸟巢组的早产儿,在入院后2,12,48 h的体温均明显高于对照组,且中性温度的稳定性优于对照组。结论:保鲜膜覆盖结合水床式鸟巢为早产儿提供了一个温暖而恒定贴身的外界环境,使早产儿感到温暖而舒适,维持了稳定的中性体温。  相似文献

AIM: This paper reports an audit of the effect on admission temperatures of using occlusive polyethylene wrap applied immediately after the birth of extremely premature infants. BACKGROUND: Use of occlusive polyethylene wrap during the early postnatal management of the premature infant reduces evaporative and convective heat loss. METHOD: Retrospective pre-intervention audit was carried out, followed by the introduction of occlusive polyethylene wrap for thermal management during resuscitation and early stabilization. Prospective post-intervention audit was then performed. The pre-intervention (control) group infants were immediately dried with prewarmed towels and resuscitated under radiant heat. Infants in the intervention group were managed under radiant heat, were not dried but were immediately enclosed in an occlusive polyethylene wrap. RESULTS:The demographic characteristics of the two groups were comparable. Use of occlusive polyethylene wrap resulted in higher admission temperatures for infants less than 27 weeks gestation (z=108.50, P<0.01). There was no statistically significant improvement in admission temperatures for 27-29 week infants. The rate of hypothermia on admission (<35.6 degrees C per axilla) was lower in the intervention group (chi(2)=5.12, d.f.=1, P=0.02), but more infants recorded temperatures exceeding 37.2 degrees C during the first 12 hours (chi(2)=23.45, d.f.=1, P<0.01). There were no other adverse effects noted. CONCLUSION: Use of occlusive polyethylene wrap improved admission temperatures for infants less than 27 weeks gestation. This intervention is easy to implement and does not interfere with resuscitation. However, removal of the wrap should be considered following admission to a closed care system in the neonatal intensive care unit because, in the intervention group, hyperthermia in the first 12 hours was a potential side effect.  相似文献

There has been no objective means for imaging the three-dimensional (3D) morphology of the clitoris-a poorly understood, complex structure. A Live 3D ultrasound system with a matrix-array transducer was used for data acquisition from eight women. The transducer was positioned in front of and about 3 cm away from the clitoris, with a gel pad or water pad being placed in between. The pads allowed the delicate structures to be imaged without noticeable deformation. Quality images could be obtained with use of a water pad in all patients. The imaging volume was big enough to cover the clitoral glans and body simultaneously, allowing real-time 3D visualisation. To cover the entire clitoris, the probe was moved from one side of the crus to the other, or a four subvolume scan was performed. 3D clitoral anatomy was depicted from 71% of 51 water pad data-sets. The study demonstrates the feasibility of obtaining 3D clitoral ultrasound images. This will improve scientific and clinical understanding of the clitoral role in sexual activity. The minimally-compressive scanning offers an opportunity to visualise dynamic 3D (4D) morphology of other deformable body parts.  相似文献

IntroductionPlastic wrap is an example of an intervention developed to prevent or minimise heat loss in the newborn. Plastic wrap enables clinical staff easy of access and visualisation of the newborn at all times.Case presentationA 24-week infant was placed in a plastic wrap as part of a clinical trial. A cord bleed was noticed soon after birth. The rapidness of the problem indicated an additional advantage of the application of plastic wrap.  相似文献

Contrast-enhanced CT is currently the gold standard for the diagnosis of kidney infarction. Particularly in patients with renal insufficiency, however, the use of iodine contrast media is limited. Sonographic examination using ultrasound contrast media may be an alternative for these patients. CASE REPORTS: We examined three patients with suspected ischaemic kidney infarction with ultrasound contrast media. Scanning was performed with low mechanical index, and SonoVue was used as by contrast-enhancing agent. In all three patients, kidney infarctions were clearly shown. In two patients, the lesions were confirmed by contrast-enhanced CT, in one no CT was performed because of impaired renal function. CONCLUSION: Kidney infarctions can reliably be detected by contrast-enhanced ultrasound using SonoVue as contrast agent. This is particularly valuable in patients with renal impaired renal function and other contraindications against iodine contrast media.  相似文献

This article is aimed to aid the sonographer and medical care provider in better interpretation and clinical application of first-trimester ultrasound findings in early pregnancy. We review current uses, applications, and interpretations of first-trimester ultrasound. A discussion of basic embryology and normal as well as abnormal first-trimester sonographic findings are discussed. We also cover the utility of the first-trimester ultrasound in screening for fetal aneuploidy. Attention is also given to the role of the first-trimester ultrasound in the prediction of adverse perinatal and maternal outcomes.  相似文献

Optimization of the skeletal muscle contraction during cardiac assist is important to achieve maximal cardiac assist and yet avoid overstimulation that may injure skeletal muscle. Dynamic Cardiomyoplasty suffers from lack of an objective, reproducible, and accurate technique to measure skeletal muscle shortening fraction after wrap and training of the muscle. A recruitment curve is considered the best way to select the proper stimulation level to achieve supramaximal contraction without overstimulating the muscle. A fluoroscopic technique of determining latissimus dorsi recruitment cuive was evaluated in five goats undergoing dynamic Cardiomyoplasty with an anterior cardio–subcutaneous wrap. Two pairs of stainless steel targets (0.5 and 1 cm of diameter) were implanted on each side of the muscle wrap. One pair of sonomicrometer crystals was also implanted. Displacement of the targets was measured under fluoroscopy at five different stimulation levels. Correlation coefficients between targets on the inside surface of the wrap and the sonomicrometer crystals, and targets on the outside surface of the wrap and the sonomicrometer crystals were 0.71 (P ± 0.05) and 0.60 (P ± 0.05), respectively. Targets on the inside surface of the wrap were more accurate than targets on the outside surface of the wrap for measurement of skeletal muscle shortening fraction and establishment of a recruitment curve. Adverse effects from the targets were not observed.  相似文献

Hand-held sonomammography was used intraoperatively to localize 52 masses in 45 women in an out patient operating suite. All but five women had a positive X-ray mammogram. The ultrasound mammogram was able to identify all lesions. In 42 women studied by a preoperative ultrasound examination before the operating room procedure, all the masses were successfully identified. Precise localization was successfully performed in the operating room before sterile preparation in less than 10 minutes. Relocalization after incision in the sterile operating field was necessary in two cases. This technique requires no ionizing radiation and expedites outpatient surgical removal of the lesion. Of the 52 masses, there were 32 fibroadenomas, three carcinomas, three cysts, six cases of focal fibrous mastitis, and eight patients with focal fibrocystic disease. In patients with positive sonograms for nonpalpable masses, ultrasound localization is a fast, accurate alternative to X-ray needle placement.  相似文献

Super-resolution land cover mapping by deep learning   总被引:1,自引:0,他引:1  
Super-resolution mapping (SRM) is a technique to estimate a fine spatial resolution land cover map from coarse spatial resolution fractional proportion images. SRM is often based explicitly on the use of a spatial pattern model that represents the land cover mosaic at the fine spatial resolution. Recently developed deep learning methods have considerable potential as an alternative approach for SRM, based on learning the spatial pattern of land cover from existing fine resolution data such as land cover maps. This letter proposes a deep learning-based SRM algorithm (DeepSRM). A deep convolutional neural network was first trained to estimate a fine resolution indicator image for each class from the coarse resolution fractional image, and all indicator maps were then combined to create the final fine resolution land cover map based on the maximal value strategy. The results of an experiment undertaken with simulated images show that DeepSRM was superior to conventional hard classification and a suite of popular SRM algorithms, yielding the most accurate land cover representation. Consequently, methods such as DeepSRM may help exploit the potential of remote sensing as a source of accurate land cover information.  相似文献

OBJECTIVE: To evaluate the effects of continuous low-level heat wrap therapy for the prevention and early phase treatment (ie, 0-48 h postexercise) of delayed-onset muscle soreness (DOMS) of the low back. DESIGN: Two prospective randomized controlled trials. SETTING: Outpatient medical facility. PARTICIPANTS: Sixty-seven subjects asymptomatic of back pain and in good general health (mean age, 23.5+/-6.6 y). INTERVENTIONS: Participants performed vigorous eccentric exercise to experimentally induce low back DOMS. Participants were assigned to 1 of 2 substudies (prevention and treatment) and randomized to 1 of 2 treatment groups within each substudy: prevention study (heat wrap, n=17; control [nontarget muscle stretch], n=18) and treatment study (heat wrap, n=16; cold pack, n=16). Interventions were administered 4 hours before and 4 hours after exercise in the prevention study and between hours 18 to 42 postexercise in the treatment study. MAIN OUTCOME MEASURES: To coincide with the expected occurrence of peak symptoms related to exercise-induced low back DOMS, hour 24 postexercise was considered primary. Pain intensity (prevention) and pain relief (treatment) were primary measures, and self-reported physical function and disability were secondary measures. RESULTS: In the prevention study, at hour 24 postexercise, pain intensity, disability, and deficits in self-reported physical function in subjects with the heat wrap were reduced by 47% (P<.001), 52.3% (P=.029), and 45% (P=.013), respectively, compared with the control group. At hour 24 in the treatment study, postexercise, pain relief with the heat wrap was 138% greater (P=.026) than with the cold pack; there were no differences between the groups in changes in self-reported physical function and disability. CONCLUSIONS: In this small study, continuous low-level heat wrap therapy was of significant benefit in the prevention and early phase treatment of low back DOMS.  相似文献

Microbubble contrast media are used to enhance ultrasound images. Because ultrasound is a real-time investigation, contrast-enhanced ultrasound offers possibilities for perfusion imaging. This review is conducted to evaluate the safety of contrast-enhanced ultrasound and its possible role in medical imaging. The safety of diagnostic ultrasound is still an important field of research. The wanted and unwanted effects of ultrasound and microbubble contrast media as well as the effects of ultrasound on these microbubbles are described. Furthermore, some of the possible applications and indications of contrast-enhanced ultrasound will be discussed. The shared advantages of MRI and ultrasound are the use of non-ionizing radiation and non-nephrotoxic contrast media. From this review it can be concluded that, for certain indications, contrast enhanced ultrasound could be a safe alternative to MRI and a valuable addition to medical imaging.  相似文献

Background

Ultrasound is an ideal imaging modality for low-resource settings given its portability, ease of use, and wide range of applications. Commercially available ultrasound gels may be unavailable or cost prohibitive in low-resource settings.

Objective

Our aim was to investigate the quality of images obtained with an alternative ultrasound gel made from cornstarch and water in comparison with commercially available gel.

Methods

The cornstarch gel was made by heating water and cornstarch. The gel was used to obtain standard ultrasound images of the gallbladder, kidneys, bladder, heart, and neck vessels by the study investigators. Commercial gel was used as the control. Twenty-four ultrasound clips were shown to four blinded radiologists and two ultrasound-fellowship−trained emergency department physicians. They rated the images on adequacy (yes/no) and graded the images detail, resolution, and quality using a continuous 0−10 scale.

Results

A total of 144 video clips were shown to reviewers and 129 data-collection sheets were returned. There was no statistical difference in the proportion of images deemed to be of adequate quality: cornstarch-based gel = 0.97 (95% confidence interval [CI] 0.92−1.00) vs. commercially available gel = 0.85 (95% CI 0.75−0.94) (p = 0.053). The cornstarch gel was superior to commercial gel on all three image parameters: detail (p = 0.002), resolution (p = 0.018), and quality (p = 0.013).

Conclusions

In this study, a gel made from cornstarch and water was an acceptable coupling medium and provided equally adequate images as compared with commercial ultrasound gel. This inexpensive gel made from ubiquitous materials can be an acceptable alternative to commercial gel in low-resource settings.  相似文献

目的探讨使用国产止血带时,通过改进使用方法检测其止血效果及止血带损伤情况。方法以成人下肢手术为例制作弧形气压止血带保护套。选择施行择期下肢膝关节以下部位手术的成年患者100例,随机分为传统方法组、弧形气压止血带保护套组(改良方法组)各50例。传统方法组先用1张纱布垫缠绕肢体皮肤,缠上止血带,再用绷带缠在止血带上加以固定;改良方法组将保护套衬垫面缠在肢体上,在衬垫面上缠止血带,将反折面向上反折固定保护止血带,反折面两端的布带绑在保护套上。比较两组气囊止血带压力、止血效果及止血带损伤情况。结果两组气囊止血带压力比较,差异有统计学意义(P〈0.05),改良方法组气囊止血带压力低于传统方法组;两组充气时间比较,差异无统计学意义(P〉0.05);两组止血效果及止血带损伤情况比较,差异有统计学意义(P〈0.01),改良方法组手术时止血效果及减压后出现的止血带损伤进行综合判断,效果明显好于传统方法组。结论使用弧形气压止血带保护套能有效降低气囊止血带压力而不延长充气时间,达到止血效果及减少止血带损伤情况发生。  相似文献

Evaluation of aortic endografts is an exciting new application for contrast enhanced ultrasound. Detection of endoleaks is critical as they may lead to progressive enlargement of a treated aneurysm with a risk of aneurysm rupture. In this article, contrast enhanced ultrasound is presented as a potentially accurate, safe and economical alternative to CTA, the current practice standard, for endoleak detection.  相似文献

OBJECTIVES: The present investigation focuses on the following questions: 1. Are complementary medical services paid for by a health insurer used in addition to orthodox medical services, or as substitute for them?; 2. If health insurers include complementary medical services in the basic cover, what will be the effect on costs?; 3. If complementary medical services as included in the basic cover, what will be the effect on the policyholders' subjective state of health? STUDY DESIGN: A randomized experiment was set up in which 7500 members of Switzerland's biggest health insurance fund, Helvetia, were offered free supplementary insurance for alternative medicine for 3 years. This simulated a situation in which the experimental group had access to the full range of complementary medical treatments under their health insurance policies. The remaining members in the scheme (670,000) people) formed the control group. To evaluate the effect on costs, we analysed the health insurer's cost and benefits data. In addition, a survey was carried out among random samples of subjects from the experimental group and from the control group using the 36-Item Short-Form Health Survey (SF-36) to examine the effects of including complementary medicine on subjective state of health. RESULTS: The analysis of the cost data shown that subjects used alternative in addition to orthodox medical services. It is also clear that alternative medical treatments are given in combination with orthodox medicine; less than 1% of the experimental group used exclusively alternative medical services. However, as only a very small percentage of experimental subjects (6.6%) took advantage of complementary medicine, no significant impact on overall health costs can be inferred. On the other hand, multiple regressions show that use of complementary medicine has a greater effect on treatment costs than sex, age or language region. Neither at the beginning nor the end of the experiment were any significant differences noted in the scales of the SF-36 between the experimental and the control group. Nor did multiple regressions reveal any effects on subjects' state of health due to the inclusion of complementary medicine in the basic insurance cover.  相似文献

The aim of the study was to evaluate the effectiveness of Angio PLanewave UltraSensitive imaging (Angio PL.U.S.) as an alternative to contrast-enhanced ultrasound (CEUS) and computed tomography angiography (CTA) for endoleak detection and classification in patients after endovascular aneurysm repair. A total of 28 patients underwent a post-endovascular aneurysm repair follow-up with color Doppler ultrasound, power Doppler ultrasound, CEUS, Angio PL.U.S and CTA examinations. CTA revealed 17 endoleaks in 14 patients (50%): 3 type Ia, 13 type II and 1 type III. There were no differences between Angio PL.U.S. and CEUS in terms of sensitivity, specificity or accuracy (93%, 100% and 97%). We did not observe any statistically significant differences between CTA, CEUS and Angio PL.U.S. in terms of the endoleak identification ability. Angio PL.U.S. may be considered as a potential tool to follow-up patients after endovascular aneurysm repair implantation, especially in patients who cannot be examined with CTA or CEUS.  相似文献

Monitoring spinal curvature in adolescent kyphoscoliosis requires regular radiographic examinations; however, the applied ionizing radiation increases the risk of cancer. Ultrasound imaging is favored over radiography because it does not emit ionizing radiation. Therefore, we tested an ultrasound system for spinal curvature measurement, with the help of spatial tracking of the ultrasound transducer. Tracked ultrasound was used to localize vertebral transverse processes as landmarks along the spine to measure curvature angles. The method was tested in two scoliotic spine models by localizing the same landmarks using both ultrasound and radiographic imaging and comparing the angles obtained. A close correlation was found between tracked ultrasound and radiographic curvature measurements. Differences between results of the two methods were 1.27 ± 0.84° (average ± SD) in an adult model and 0.96 ± 0.87° in a pediatric model. Our results suggest that tracked ultrasound may become a more tolerable and more accessible alternative to radiographic spine monitoring in adolescent kyphoscoliosis.  相似文献

The availability, portability, safety, and other features of ultrasound have ushered this relatively new imaging modality into the everyday clinical practice of multiple disciplines. Features unique to ultrasound lend this imaging modality the opportunity for extensive use in the ICU. A review of its uses in this capacity includes bedside diagnosis of common disorders seen in the ICU setting, such as DVT, cholecystitis, and abscess. Bedside sonography also can aid in the treatment of such disorders, including DGC of pseudoaneurysms, fluid aspirations, and abscess drainages. This article is a review and could not possibly cover all bedside uses of ultrasound or provide in-depth information of specific uses described in this article. Hopefully, this article will spark an interest and prove as a starting point on a rewarding learning adventure.  相似文献
