The science of sports massage is of interest to many populations including athletes, athletic trainers, coaches, as well as sports physiologists. While evidence to support or refute the effects of massage on sports performance is insufficient to make definitive statements, new reports and trends within data help formulate an understanding of sports massage. This article will review sports massage research on topics including lactate clearance, delayed onset of muscle soreness (DOMS), muscle fatigue, the psychological effect of massage, and injury prevention and treatment. Articles referenced in Medline, Cochrane Database, the authors library, and references from articles are included in this review. Most studies contain methodological limitations including inadequate therapist training, insufficient duration of treatment, few subjects, or over or under working of muscles that limit a practical conclusion. Muscle soreness associated with DOMS is reduced with massage, although whether force recovers more quickly is still unclear. The research literature to date is insufficient to conclude whether massage facilitates recovery from a fatiguing effort. Both tissue healing and a psychological effect of massage are areas that may prove promising with further research. Results from published literature support a positive trend for massage to benefit athletic recovery and performance; a need for further research into sports massage, especially well-designed studies utilizing therapists specifically trained to administer this type of therapy, is warranted.  相似文献

IntroductionPatellar tendinopathy (PT) has a high prevalence in jumping athletes and presents a significant burden on athletes and clinicians due to its long-lasting duration and persistent symptoms. This scoping review aimed to map existing evidence on prevention and in-season management interventions for PT in athletes, evaluating intervention parameters and outcomes.MethodsThis scoping review was reported in accordance with the PRISMA-ScR. Databases searched included MEDLINE, CINAHL, AMED, EMBase, SPORTDiscus, and the Cochrane library (Controlled trials, Systematic reviews). All primary study designs investigating prevention or in-season management interventions for PT, while maintaining athletes in sport were considered for inclusion.Results5987 articles were identified with 29 included in the review. Five studies investigated exercise-based prevention interventions on athletes at risk for PT, including two randomized controlled trials (RCTs), two cohort studies and one case-control study. 24 studies investigated in-season management or rehabilitation in athletes with PT, including 18 RCTs, three case reports, one cohort study, one case series, and one retrospective review. Of these 24 studies, 22 used various resistance training interventions, one used ESWT and one used patellar strapping and taping. The types of resistance training included eccentric, heavy-slow, isometric, inertial flywheel, blood-flow restriction, and isotonic training. Eccentric training was used in 9 studies, with single leg decline squats the most common exercise used in 7 studies. Outcome measures and intervention parameters were heterogenous throughout studies.ConclusionDespite a dearth of studies on preventative interventions for athletes with PT, resistance training may be a useful prophylactic method. Eccentric, heavy slow and isometric resistance training have been found to be feasible and clinically beneficial in-season. There are a lack of studies showing that ESWT offers any additional benefit over resistance training in competing athletes. Patellar strapping and taping may offer short-term pain relief during training and competition. Systematic reviews are required to make definitive recommendations for PT.  相似文献

The health benefits of leisure-time physical activity are well known, however the effects of engaging in competitive sports on health are uncertain. This literature review examines mortality and longevity of elite athletes and attempts to understand the association between long-term vigorous exercise training and survival rates. Fourteen articles of epidemiological studies were identified and classified by type of sport. Life expectancy, standardised mortality ratio, standardised proportionate mortality ratio, mortality rate, and mortality odds ratio for all causes of death were used to analyse mortality and longevity of elite athletes. It appears that elite endurance (aerobic) athletes and mixed-sports (aerobic and anaerobic) athletes survive longer than the general population, as indicated by lower mortality and higher longevity. Lower cardiovascular disease mortality is likely the primary reason for their better survival rates. On the other hand, there are inconsistent results among studies of power (anaerobic) athletes. When elite athletes engaging in various sports are analysed together, their mortality is lower than that of the general population. In conclusion, long-term vigorous exercise training is associated with increased survival rates of specific groups of athletes.  相似文献

ObjectivesTo investigate whether intensive supervised rehabilitation following ACL reconstruction leads to superior self-reported function and sports participation compared to less supervised rehabilitation.DesignSystematic review and meta-analysis.MethodsWe included randomised controlled trials (RCTs) comparing supervised rehabilitation to rehabilitation with a similar protocol that used less supervised sessions for athletes following ACL reconstruction. Two reviewers independently screened studies and extracted data. The Physiotherapy Evidence Database (PEDro) scale was used to evaluate methodological quality and GRADE to evaluate overall quality of evidence. Self-reported function and sports participation were the primary outcomes. Data were pooled using random effects meta-analyses.ResultsOur search retrieved 4075 articles. Seven articles reporting on six RCTs were included (n = 353). Very-low to low-certainty evidence suggests intensive supervised rehabilitation is not superior to less supervised rehabilitation following ACL reconstruction for improving self-reported function, sports participation, knee flexor and extensor strength, range of motion, sagittal plane knee laxity, single leg hop performance, or quality of life.ConclusionBased on uncertain evidence, intensive supervised rehabilitation is not superior to less supervised rehabilitation for athletes following ACL reconstruction. Although high-quality RCTs are needed to provide more certain evidence, clinicians should engage athletes in shared decision making to ensure athletes’ rehabilitation decisions align with current evidence on supervised rehabilitation as well as their preferences and values.  相似文献

ObjectivesResistance training in untrained adolescents can positively effect health-related fitness as well as improve muscular power and sports performance. The impact of resistance training on adolescent athletes is less clear. The purpose of this review is to determine the effectiveness of resistance training programs on muscular power and sports performance in adolescent athletes.DesignSystematic review and meta-analysis of previously published studies investigating resistance training in adolescent athlete populations.MethodsA systematic search of Medline, Embase, and SPORTDiscus databases was conducted on 21st March 2011 to identify studies evaluating resistance training programs on power and sports performance in adolescent athletes.ResultsThirty-four studies were identified. All but two of the studies reported at least one statistically significant improvement in an alactic muscular power outcome. The most common indicators of alactic power were vertical jump (25 studies) and sprint running (13 studies) performance. Fourteen studies provided data to allow for pooling of results in a meta-analysis. A positive effect was detected for resistance training programs on vertical jump performance (mean difference 3.08 [95% CI 1.65, 4.51], Z = 4.23 [P < 0.0001]).ConclusionsThere is sufficient evidence to conclude that resistance-training interventions can improve muscular power in adolescent athletes. A positive effect on sports performance attributable to participation in resistance training was reported by almost half the included studies, however limited objective evidence to support these claims was found. Improvements in motor performance skills, such as jumping, are widely stated as indicators of improvements in sporting performance.  相似文献

Sudden cardiac death (SCD ) is a devastating event in athletes. Screening efforts that were first directed at athletes younger than 35 years are now focusing on the rapidly growing group of older sportspersons. Athletes aged ≥35 years have a 10‐fold increased risk of exercise‐related cardiac arrest, mostly due to coronary artery disease (CAD ). Although cardiac imaging is pivotal in identifying CAD , the role of imaging modalities in screening asymptomatic older sportspersons remains unclear. We performed a scoping review to identify the role of cardiac imaging to detect CAD in older sportspersons and to identify gaps in the existing literature. We searched MEDLINE , EMBASE and the Cochrane library for studies reporting data on cardiac imaging of CAD in sportspersons ≥35 years. The systematic search yielded 1737 articles, and 14 were included in this scoping review. Imaging modalities included two echocardiography, one unenhanced computed tomography (CT ) for coronary artery calcium scoring (CACS ), three CACS and contrast‐enhanced CT angiography (CCTA ), two CACS and cardiac magnetic resonance (CMR ), one CCTA with CMR and echocardiography, two CCTA , two CMR , and one myocardial perfusion imaging article. The low number of relevant articles and the selection bias introduced by studying specific groups, like veteran marathon runners, indicate the need for future research. Cardiac CT (CACS and CCTA ) probably has the highest potential for pre‐participation screening, with high diagnostic value to detect CAD and low radiation dose. However, currently there is insufficient evidence for incorporating routine cardiac imaging in the pre‐participation screening of asymptomatic sportspersons over 35 years.  相似文献

ABSTRACT

Omega-3 (n-3) polyunsaturated fatty acid (PUFA) supplementation has recently been proposed as an ergogenic aid for athletes. This claim is primarily based on mechanistic evidence that n-3PUFA’s exert anti-inflammatory properties and act to change the functional capacity of the muscle cell by modifying the membrane fluidity of proteins and lipids within the cell membrane. In this review, we critically evaluate the scientific literature that examines the efficacy of n-3PUFA supplementation to improve athlete performance within the context of promoting muscle adaptation, energy metabolism, muscle recovery and injury prevention (e.g. muscle loss during immobilisation, or concussion). These findings have applications to athletes competing in strength/power-, endurance- and team-, based sports. Based on available information, there is some scientific evidence that n-3PUFA supplementation may improve endurance capacity by reducing the oxygen cost of exercise. Moreover, several studies report a benefit of n-3PUFA supplementation in promoting recovery from eccentric-based muscle damaging exercise. In contrast, there is insufficient evidence from studies in athletic populations to support the claim that n-3PUFA supplementation facilitates muscle growth during resistance training or preserves muscle mass during catabolic scenarios such as energy restriction or immobilisation. Moving forward, there remains ample scope to investigate context-specific applications of n-3PUFA supplementation for sport performance.  相似文献

The prevalence of spondylolysis in the Spanish elite athlete   总被引:7,自引:0,他引:7  
The diagnosis of spondylolysis is a major cause of concern for patients and their families, especially when the patients are young athletes with promising futures in their sports. In this study, 3152 case histories of high-level athletes were evaluated to determine which sports had a higher prevalence of spondylolysis. The overall percentage of spondylolysis among athletes in this study (8.02%) was not very much higher than that among the general population, which varies between 3% and 7%. However, when each sport was considered separately we found much higher values for some sports, with the highest percentages occurring in throwing sports (26.67%), artistic gymnastics (16.96%), and rowing (16.88%). The analysis of the biomechanical movements involved in the sports with greater prevalence of spondylolysis has led us to include the element of torsion against resistance as another possible causative factor for spondylolysis that should be added to the already known causative mechanisms, lumbar hyperextension and rotation. We have divided the sports into three risk groups according to the prevalence of spondylolysis shown and the characteristics of the sample, and we recommend systematic radiological examination of the lumbar spine in athletes considered to be at greater risk of developing spondylolysis.  相似文献

Studies regarding ankle and foot overuse injuries are quite diverse in research methodology, data reporting, and outcomes. The aims of this systematic review were to analyze the methodology of published studies regarding ankle and foot overuse injuries in different sports disciplines and to summarize epidemiological data of ankle and foot overuse injuries. Four electronic databases, PubMed (MEDLINE), EMBASE, CINAHL, and SPORTDiscus® were systematically searched up to June 2011. A total of 89 articles on 23 sports disciplines were included in this review. Soccer, running, and gymnastics were the most frequently studied sports. Achilles tendinopathy, plantar fasciitis, and stress fracture were the most frequently studied injuries. Study design and reporting methods were heterogeneous. Most studies suffered from a weak methodology and poor reporting. The most common weaknesses were lack of a clear case definition, describing assessment procedures and reporting sample characteristics. Due to methodological heterogeneity of studies, inter‐sports and intra‐sports comparisons and meta‐analysis were not possible. Methodology of most studies on incidence and prevalence of ankle and foot overuse injuries is insufficient. Based on the results, we recommend authors to clearly define cases, describe assessment procedures and report sample characteristics adequately.  相似文献

OBJECTIVE: To review available evidence establishing the validity of the preparticipation evaluation (PPE) as a method for screening health risk prior to participation in exercise and sport. Specific emphasis was placed on reviewing original research evaluating methods to screen participants for risk of sudden cardiovascular death. Literature on the current state of the PPE as a screening tool for athletic participation was examined. DATA SOURCES: Electronic databases were searched for articles relating to mass screening for sports participation and sudden cardiac death in athletes published up to January 2004. Databases searched included Medline (OVID Web, 1966-2004), PubMed (1966-2004), Sport Discuss (1975-2004), Current Contents, CISTI Source (1993-2004), Cochrane Database of Systematic Reviews, and EBM Reviews. Additional references from the bibliographies of retrieved articles were also reviewed. SELECTION CRITERIA: All study designs were retrieved, but only those studying athletes and/or student-athletes under age 36 years were reviewed. Of the original research retrieved, the majority of the articles sought to establish incidence or prevalence of cardiovascular causes of sudden death in athletes or the validity of various screening tools. Original research articles seeking to establish the current use of the PPE in all its various forms were also reviewed. All of the articles selected for review consisted of type II, population-based data. DATA EXTRACTION AND SYNTHESIS: The initial literature search identified 639 papers. Of these, 310 articles that met the selection criteria were reviewed, and 25 articles were identified as original research directly relating to the PPE. All of these contained type II evidence-population-based clinical studies. The majority of the literature on the PPE consists of type III evidence-case-based opinion papers and position papers from respected authors and sports medicine societies and reports of expert committees. This literature was also reviewed, but only original research relevant to the PPE is reported in this article. The majority of these studies examined cardiovascular diseases and screening procedures. RESULTS: The 5 studies that assessed the format or effectiveness of the PPE concluded that it was inadequate. The format of the PPE is not standardized and does not consistently address the American Heart Association recommendations for cardiovascular screening history and physical exams. A variety of health care professionals, some without proper training, administer the PPE. The 12 original studies that looked at specific cardiovascular screening techniques were divided on the effectiveness of history, physical examination, electrocardiogram, and echocardiography for detecting cardiovascular risks for sudden death in athletes. CONCLUSIONS: A PPE is required by most sport organizations in America, but research as to its effectiveness is very limited. PPEs have been mandatory in Italy for many years, and we can draw on some the data recorded over this time. Otherwise, very few studies in America or elsewhere have been performed on the PPE process. The research available indicates that the PPE is not implemented adequately or uniformly. An opportunity exists to create a standardized, validated PPE that meets medical standards for quality and provides sensitive, specific screening of potential participants in sport and exercise.  相似文献

Female athletes have a four- to sevenfold increased risk of anterior cruciate ligament (ACL) injury compared with their male counterparts playing at similar levels in the same sports. The elevated risk of ACL injury in females coupled with the geometric increase in female sports participation in the last 30 years has led to a rapid rise in these injuries. This large increase in ACL injury incidence has fueled studies into both mechanisms of injury and interventions to prevent injury. A review of published multidisciplinary approaches demonstrates that several training protocols have utilized multiple components targeted toward injury prevention training and were able to reduce injury incidence in female athletes. Similar training techniques may also be used to gain improvements in measures of performance. The purpose of this review is to highlight training components that may reduce ACL injury risk and assess their potential for combined use in performance-oriented protocols.  相似文献

Female athletes have a 4 to 6 times higher incidence of anterior cruciate ligament injury than do male athletes participating in the same landing and pivoting sports. This greater risk of anterior cruciate ligament injury, coupled with a geometric increase in participation (doubling each decade), has led to a significant rise in anterior cruciate ligament injuries in female athletes. The gender gap in anterior cruciate ligament injury, combined with evidence that the underpinnings of this serious health problem are neuromuscular in nature, leads to the development of neuromuscular interventions designed to prevent injury. A systematic review of the published literature yielded 6 published interventions targeted toward anterior cruciate ligament injury prevention in female athletes. Four of 6 significantly reduced knee injury incidence, and 3 of 6 significantly reduced anterior cruciate ligament injury incidence in female athletes. A meta-analysis of these 6 studies demonstrates a significant effect of neuromuscular training programs on anterior cruciate ligament injury incidence in female athletes (test for overall effect, Z = 4.31, P < .0001). Examination of the similarities and differences between the training regimens gives insight into the development of more effective and efficient interventions. The purpose of this "Current Concepts" review is to highlight the relative effectiveness of these interventions in reducing anterior cruciate ligament injury rates and to evaluate the common training components between the training studies. In addition, the level of rigor of these interventions, the costs and the difficulty of implementation, the compliance with these interventions, and the performance benefits are discussed. This review summarizes conclusions based on evidence from the common components of the various interventions to discuss their potential to reduce anterior cruciate ligament injury risk and assess their potential for combined use in more effective and efficient intervention protocols.  相似文献

Abstract

Objective: Patellofemoral pain syndrome (PFPS) is one of the most frequently diagnosed knee conditions in the primary care, orthopedic, and sports medicine settings. Although strength training and stretching programs have traditionally been the mainstay of patient treatment, there are no consensus recovery protocols for runners experiencing PFPS. The purpose of our review is to examine recent literature regarding the efficacy of various treatment modalities in the management of patients with PFPS. Methods: Our review included 33 articles from a PubMed literature search using the search term PFPS treatment. The search was limited to randomized controlled trials, crossover case-controlled studies, and cohort studies with ? 10 participants, with trial data that were published within the last 5 years. Results: Strength training and stretching exercises continue to be strongly supported by research as effective treatment options for runners with PFPS. Recent studies have confirmed that quadriceps and hip strengthening combined with stretching in a structured physiotherapy program comprise the most effective treatment for reducing knee pain symptoms and improving functionality in patients with PFPS. As previous studies have shown, therapies such as proprioceptive training, orthotics, and taping may offer benefits as adjunctive therapies but do not show a significant benefit when they are used alone in patients with PFPS. Additionally, recent research has confirmed that surgical and pharmacologic therapies are not effective for the management of patients with PFPS. Conclusion: A large number of athletes are impacted by PFPS every year, particularly young runners. Sports medicine researchers have investigated many possible therapies for patients with PFPS; however, no clear guidelines have emerged regarding the management of the syndrome. Our review analyzes recent literature on PFPS and identifies specific treatment recommendations. The most effective and strongly supported treatment modality for patients with PFPS is a combined physiotherapy program, including strength training of the quadriceps and hip abductors and stretching of the quadriceps muscle group. Adjunctive therapies, including taping, biofeedback devices, and prefabricated orthotic inserts, may provide limited additive benefits in select populations. Surgery should be avoided in all patients with PFPS.  相似文献

Heavy training in combination with inadequate recovery actions can result in the overtraining/staleness syndrome and burnout. Even young and aspiring elite athletes develop staleness. The aim was therefore to determine the incidence and nature of staleness, and its association with training behavior and psychosocial stressors in young elite athletes. A sample of 272 individuals from 16 sports completed questionnaires on training, staleness, and psychosocial stress and 37% reported being stale at least once. The incidence rate was higher for individual sports (48%) compared with team (30%) and less physically demanding sports (18%). Stale athletes reported greater perceptual changes and negatively elevated mood scores in comparison to healthy athletes. Staleness was distinguished from burnout on the basis of motivational consequences; 41 % of the athletes lost their motivation for training, which in turn indicates a state of burnout. Further, 35 % of the athletes reported low satisfaction with time spent on important relationships, 29% rated the relationship with their coach as ranging from very, very bad to only moderately good. The results indicate that staleness is a widespread problem among young athletes in a variety of sports, and is not solely related to physical training, but also to non-training stressors.  相似文献

This study was designed to quantify the daily distribution of training intensity in a group of well-trained junior cross-country skiers and compare the results of three different methods of training intensity quantification. Eleven male athletes performed treadmill tests to exhaustion to determine heart rate and VO2 corresponding to ventilatory thresholds (VT1, VT2), maximal oxygen consumption (VO2max), and maximal heart rate. VT1 and VT2 were used to delineate three intensity zones. During the same time period, all training sessions (N=384, 37 strength training, 347 endurance) performed over 32 consecutive days were quantified using continuous heart rate registration and session Rating of Perceived Exertion (RPE). In addition, a subset of 60 consecutive training sessions was quantified using blood lactate measurements. Intensity distribution across endurance training sessions (n=318) was similar when based on heart rate analysis (75+/-3%, zone 1; 8+/-3%, zone 2; 17+/-4%, zone 3) or session RPE (76+/-4%, zone 1; 6+/-5%, zone 2; 18+/-7%, zone 3). The number of athletes with disabilities participating in organized sports and the popularity of the Paralympic Games is steadily increasing around the world. Despite this growing interest and the fact that participation in sports places the athlete at risk for injury, there are few studies concerning injury patterns, risk factors, and prevention strategies of injuries in disabled athletes. In this systematic literature search and critical review, we summarize current knowledge of the epidemiology of sports‐related injuries in disabled athletes and describe their characteristics, incidence, prevalence, and prevention strategies. The outcomes of interest were any injury, either an acute trauma or an overuse event. PubMed, EMBASE, CINAHL, and Google Scholar were systematically searched and 25 of 605 identified studies met the inclusion criteria. Lower extremity injuries were more common in walking athletes, whereas upper extremity injuries were more prevalent in wheelchair athletes. The methodologies and populations varied widely between the studies. Few studies were sports or disability specific, which makes it difficult to determine specific risk factors, and few studies reported injury severity and prevention of injuries. Further longitudinal, systematic sports and disability specific studies are needed in order to identify and prevent injuries in athletes with disabilities.  相似文献

The relationship between balance ability and sport injury risk has been established in many cases, but the relationship between balance ability and athletic performance is less clear. This review compares the balance ability of athletes from different sports, determines if there is a difference in balance ability of athletes at different levels of competition within the same sport, determines the relationship of balance ability with performance measures and examines the influence of balance training on sport performance or motor skills. Based on the available data from cross-sectional studies, gymnasts tended to have the best balance ability, followed by soccer players, swimmers, active control subjects and then basketball players. Surprisingly, no studies were found that compared the balance ability of rifle shooters with other athletes. There were some sports, such as rifle shooting, soccer and golf, where elite athletes were found to have superior balance ability compared with their less proficient counterparts, but this was not found to be the case for alpine skiing, surfing and judo. Balance ability was shown to be significantly related to rifle shooting accuracy, archery shooting accuracy, ice hockey maximum skating speed and simulated luge start speed, but not for baseball pitching accuracy or snowboarding ranking points. Prospective studies have shown that the addition of a balance training component to the activities of recreationally active subjects or physical education students has resulted in improvements in vertical jump, agility, shuttle run and downhill slalom skiing. A proposed mechanism for the enhancement in motor skills from balance training is an increase in the rate of force development. There are limited data on the influence of balance training on motor skills of elite athletes. When the effectiveness of balance training was compared with resistance training, it was found that resistance training produced superior performance results for jump height and sprint time. Balance ability was related to competition level for some sports, with the more proficient athletes displaying greater balance ability. There were significant relationships between balance ability and a number of performance measures. Evidence from prospective studies supports the notion that balance training can be a worthwhile adjunct to the usual training of non-elite athletes to enhance certain motor skills, but not in place of other conditioning such as resistance training. More research is required to determine the influence of balance training on the motor skills of elite athletes.  相似文献
