Duplex Doppler ultrasonography for monitoring liver transplants   总被引:4,自引:0,他引:4
The utility of duplex Doppler ultrasonography (US) for monitoring was evaluated after 19 liver transplantations. The assessment of the resistive index (RI) of the hepatic arteries showed no significant difference in the RI measured in grafts with stable function compared with grafts with ischemic damage, acute rejection, chronic rejection, or cytomegalovirus hepatitis. Nor was there any correlation between the levels of RI and the severity of acute rejection. In 2 out of 5 liver transplants in which hepatic artery pulsations were not identified with US, hepatic artery thrombosis was found at angiography. Although high echogenicity of the parenchyma was often observed during acute rejection episodes, it was not diagnostic for rejection. Fluid collections observed around the grafts resolved spontaneously. Thus, duplex Doppler US for monitoring liver transplants is especially useful for the diagnosis of hepatic artery thrombosis. It was not helpful for the diagnosis of acute or chronic rejection.  相似文献

The value of duplex ultrasonography (US) in the assessment of pancreatic transplants was studied in 22 patients over a 1 1/2-year period. Ninety-eight duplex US examinations were performed, and the Doppler arterial resistive indexes (RIs) correlated with clinical events after transplantation. The RI was 0.70 or less in the parenchymal vessels in all instances of normal transplant function and greater than 0.70 in seven of eight clinical episodes of rejection (87.5%). In all studies performed during these eight cases of rejection, the positive predictive value of an RI exceeding 0.70 was 100%. The negative predictive value of an RI of less than 0.70 in excluding rejection was 90%. High RI values were not found in isolated episodes of cyclosporine toxicity, pancreatitis, peripancreatic hemorrhage, or infection. Duplex US may prove to be more accurate in the diagnosis of rejection of pancreatic transplants versus renal transplants because the former cases have fewer causes of increased vascular impedance and diminished perfusion.  相似文献

Duplex Doppler sonography recently has been used to evaluate renal transplants. Some authors have stated that high resistive indexes (RIs) occur in the presence of acute renal transplant rejection. RIs less than 0.7 are considered as probably excluding acute transplant rejection. We performed a prospective study of duplex sonographic examinations of pediatric patients (mean age, 8 years; 13 boys, two girls) with renal allografts and clinically suspected transplant disease. The results of 22 duplex studies were correlated with histopathologic data obtained between July 1987 and June 1988. RIs of the arcuate arteries in patients with acute rejection (n = 14) averaged 0.62 (range, 0.50-0.80). The RI in patients with chronic rejection (n = 1) was 0.59. RIs in patients with acute tubular necrosis (n = 3) averaged 0.66 (range, 0.59-0.72). RIs in patients with cyclosporine A toxicity (n = 4) averaged 0.66 (range, 0.58-0.79). Tubulointerstitial rejection was predominant, with only two patients showing minimal acute vascular rejection. Thirteen of 14 pediatric patients with histologically proved renal transplant rejection had a resistive index of less than 0.70. This study refutes the concept that resistive indexes of less than 0.7 exclude acute rejection.  相似文献

Eighty hepatic artery Doppler ultrasound (US) examinations performed in 49 patients after liver transplantation were retrospectively analyzed to determine if loss of diastolic flow correlated with pathologic evidence of acute allograft rejection. All 80 Doppler examinations were performed within 7 days of hepatic needle biopsy. Forty-three Doppler waveforms from 27 patients showed normal diastolic flow. Seventeen Doppler studies in 17 patients showed complete absence of diastolic flow. Review of biopsy results for each group showed no significant difference in the proportion of acute allograft rejection present (42% for the normal group and 46% for the group lacking diastolic flow). The data from 53 US and biopsy examinations performed 2 days apart in 37 patients confirmed the lack of correlation between absent hepatic artery diastolic blood flow and rejection. The authors conclude that the loss of hepatic artery diastolic flow has no apparent clinical application for the diagnosis of acute hepatic allograft rejection.  相似文献

Over a two-year period, 275 duplex Doppler ultrasound (US) examinations were performed on 75 renal allograft recipients. Retrospective visual analysis of the Doppler tracings was compared to concurrent clinical findings and to biopsy results. One hundred eight of the 176 Doppler examinations (61%) that showed acute rejection clinically or histologically were interpreted as rejection, while 80 of 99 examinations (81%) in clinically normal patients were interpreted as normal. Two hundred thirty-four examinations had resistive index (RI) calculations. Seventy-two of 141 examinations (51%) with RI less than 0.70 had clinical or biopsy evidence of rejection. Studies compared with only concurrent biopsies revealed that 35 of 39 US examinations interpreted as rejection were confirmed histologically, but only one of 32 examinations that appeared normal sonographically was histologically normal. The low sensitivity of Doppler US, whether by waveform analysis or RI calculation, makes it a poor screening test for acute rejection. The findings support the conclusion that Doppler sonography cannot replace biopsy in the evaluation of renal transplant dysfunction, particularly when the waveform analysis is normal and the RI less than 0.70.  相似文献

PURPOSE: To determine whether criteria for screening patients with sickle cell anemia for stroke established with a nonimaging transcranial Doppler ultrasonographic (US) technique are applicable to studies performed with a transcranial Doppler US imaging technique. MATERIALS AND METHODS: One hundred sixty-eight examinations in 66 children were performed for sickle cell stroke screening. Children were examined with nonimaging and imaging transcranial Doppler US techniques on the same day, for a total of 84 paired examinations. The time-averaged maximum mean velocity (V(mean)) and resistive index (RI) were calculated in the middle cerebral arteries, bifurcations of the distal internal carotid arteries, distal internal carotid arteries, anterior cerebral arteries, posterior cerebral arteries, and basilar arteries. The maximum systolic velocity (V(max)) was evaluated in the distal internal carotid arteries and middle cerebral arteries. V(mean), V(max), and RI measurements were subjected to repeated-measures multivariate analysis of covariance, and the Pearson product moment correlation was used for middle cerebral artery velocity, age, and hemoglobin. RESULTS: V(mean) measurements obtained with nonimaging and imaging techniques varied substantially for the bifurcation of the distal internal carotid artery, the posterior cerebral artery, and the basilar artery. Substantial differences were found in RIs for every vessel. Examination time was shorter with the nonimaging technique. CONCLUSION: V(mean) measurements in the middle cerebral artery, distal internal carotid artery, and anterior cerebral artery did not vary substantially between nonimaging and imaging transcranial Doppler US. RI data did not yield comparable measurements.  相似文献

Role of color Doppler US in the evaluation of renal transplant   总被引:4,自引:0,他引:4
AIM: To assess the role of Ultrasound (US), US Color Doppler (CD) and Power Doppler (PD) in the diagnosis and in the follow-up of renal graft pathology by evaluating morphological and functional features of the vasculature and comparing these to other clinical parameters. MATERIAL AND METHODS: From January 1990 to June 2000, four hundred and thirty-six renal allograft recipients (mean age 45 years) underwent periodical US, CD and PD (mean follow-up 48 months) to evaluate morphology and perfusion of the graft. Resistive index (RI) and pulsatility index (PI) were measured in order to monitor flow variations from the renal to the arcuate arteries. PD was used mainly to study the morphology of the cortical vessels. The examinations were performed on an Esaote Biomedica AU-4/5 (Genoa, Italy) using a convex 3.5 MHz probe and a linear 7.5 MHz probe. On the basis of clinical data the patients were divided into 3 groups: A) Normal, B) Acute graft dysfunction, C) Chronic allograft nephropathy. In 87 patients (20%) percutaneous biopsy or FNAB was performed. RI and PI mean values +/-SD were calculated and compared to the other diagnostic parameters considered: serum creatinine level, US morphology, CD and PD vascularization. Finally RI and PI for each group were compared using the t -test in order to determine the statistical significance of the correlation between these indices and the patients clinical conditions. RESULTS: The 436 patients were divided as follows: Group A) 170 patients (39%); Group B) 105 patients (24%); Group C) 161 patients (37%). Urological and surgical complications were ruled out in all patients. RI and PI showed a similar trend exceeding cut-off values in Group B) and C) with highest peaks in Group B. Statistical analysis demonstrated the efficacy of this method in the differentiation between normal and pathological grafts, but there was a reduced statistical difference between the two pathological groups. Histological analysis performed on 87 patients (20%) showed good correlation with RI. DISCUSSION AND CONCLUSIONS: CD is a non-invasive diagnostic method which provides flow-metric quantitative parameters for the hemodynamic assessment of the renal transplant. These values present a certain sensitivity but are not specific of renal graft dysfunction, as there is no reliable differentiation between acute rejection and other parenchymal pathologies. During the follow-up, RI and PI have no predictive value. RI variations from renal artery to cortical vessels (hylum-cortical ratio) show a good correlation with the clinical evolution of the transplant. The evaluation of RI and PI can generally be limited to renal and interlobar arteries as arcuate sampling is necessary only when the hylum-cortical ratio shows reduction or inversion. Integration of clinical and instrumental diagnoses can reduce the number of biopsies.  相似文献

We aimed to determine the mean hepatic artery resistance index (RI) in presinusoidal portal hypertension and to compare the values with those in sinusoidal portal hypertension. The hepatic artery RIs of 11 patients with presinusoidal portal hypertension, 12 patients with sinusoidal portal hypertension and 16 healthy subjects were examined with duplex Doppler ultrasound. Mean hepatic artery RIs of three groups were compared. In patients with presinusoidal portal hypertension, mean RI in the hepatic artery (0.63 ± 0.06) was significantly lower (p < 0.05) than that in the patients with sinusoidal portal hypertension (0.73 ± 0.03). There was no significant difference (p > 0.05) in the mean RIs of the hepatic artery between the patients with presinusoidal portal hypertension (0.63 ± 0.06) and the controls (0.67 ± 0.05). Mean RI value in patients with sinusoidal portal hypertension (0.73 ± 0.03) was significantly higher (p < 0.05) than the mean values in the control group (0.67 ± 0.05). Hepatic arterial resistance does not change in presinusoidal portal hypertension, whereas it increases in sinusoidal portal hypertension. However, there are some overlaps in the RI values which raise difficulties in the differentiation of these two forms of portal hypertension. Rejection, the leading cause of liver allograft dysfunction, is usually detected by liver biopsy. The purpose of this study was to determine if there are angiographic findings that correlate with this posttransplantation complication. In a retrospective study, the angiograms of 35 patients with histologically proven allograft rejection were reviewed. The examinations were done because of suspected posttransplantation vascular complications. Abnormal hepatic arteriograms were observed in 30 (86%). Eleven (37%) of the 30 had hepatic artery thrombosis (all had acute rejection). Nineteen (63%) of the 30 had varying degrees of intrahepatic arterial narrowing (14 had acute and five had chronic rejection). Additional findings in patients with acute rejection included stretching of the intrahepatic arterial tree (five cases) and slow flow, poor peripheral arterial filling, and a decrease in the number of intrahepatic arteries (10 cases total). Intrahepatic branch vessel stenoses and occlusions were seen in four patients with chronic rejection. We conclude that there is good correlations between the angiographic findings and histologic evidence of rejection. Although angiography is not advocated as a test for transplant rejection, detection of certain findings raises the possibility of rejection.  相似文献

Renal transplantation patients undergoing biopsy were examined with duplex Doppler sonography to establish the sensitivity and specificity of this technique in the detection of rejection. The study population consisted of 89 consecutive patients, who received renal transplants 1 week to 17 years before our examination and underwent 96 core needle biopsies within 24 hr of sonography. The resistive index (RI), defined as the difference between the peak systolic and end diastolic flow velocities divided by the peak systolic velocity (expressed as a percentage), was measured in the main, segmental, interlobar, and arcuate renal arteries. Furthermore, the prominence of the renal pyramids, as determined by their size and echogenicity, was prospectively evaluated. Biopsy was used to establish diagnosis, but in cases of equivocal results, hospital course was the final arbiter. The most frequent diagnoses in the patients were acute rejection (41 patients) and chronic rejection (19 patients). Receiver-operating-characteristic curve analysis established that, regardless of the vessel in which it was measured, the use of RI to assign a diagnosis of acute rejection was no better than establishing this in a random manner. We did note, however, that patients with chronic rejection or cyclosporine toxicity were unlikely to have RIs greater than 80%. We further found that prominent pyramids were neither sensitive nor specific in the detection of acute rejection and that prominent pyramids were not correlated with elevated RI.  相似文献

We prospectively evaluated the role of microbubble ultrasound contrast for detection of hepatic artery thrombosis following liver transplantation. The hepatic artery of adult liver transplant recipients with suspected thrombosis on surveillance Doppler ultrasound (US) were re-examined by a second observer. In patients with no hepatic spectral Doppler signal the microbubble contrast agent Levovist was used. The presence or absence of flow following microbubble contrast was evaluated against arteriography or repeated Doppler US findings. A total of 794 surveillance Doppler US examinations were performed in 231 patients. Hepatic artery flow was demonstrated in 759 of 794 (95.6%) examinations. Microbubble ultrasound contrast was administered in 31 patients (35 studies) with suspected hepatic artery thrombosis. Following microbubble US contrast the hepatic artery could not be demonstrated in 13 of 35 (37.1%) studies (12 patients). Eight patients had arteriography: there was hepatic artery thrombosis in 7 patients and 1 patient had a patent, highly attenuated artery. Detection of a patent hepatic artery increased from 759 of 794 (95.6%) to 781 of 794 (98.4%) with the addition of microbubble contrast. Upon independent reading of the data, the degree of operator confidence in the assessment of the hepatic artery patency prior to microbubble contrast was 4.7 (CI 1.92–7.5) but rose to 8.45 (CI 7.06–9.84) following microbubble contrast (p<0.0001). In 22 of 35 (62.9%) of studies arteriography could potentially have been avoided. Ultrasound microbubble contrast media may reduce the need for invasive arteriography in the assessment of suspected hepatic artery thrombosis.  相似文献

PURPOSE: To determine whether a change in hepatic venous flow pattern can be seen during hepatic graft rejection and if it is reversible with treatment. MATERIALS AND METHODS: Thirty-nine children received 42 liver allografts during a 10-year span. Doppler ultrasonographic (US) recordings of hepatic venous wave patterns were reviewed. Nine children (ten grafts) with continuously monophasic flow were not included in the study. Changes from triphasic to monophasic flow were recorded and correlated with clinical findings in all 30 patients and biopsy findings in 25. Biopsy results were compared with US data recorded within 24 hours of biopsy. Standard statistical tests were conducted to assess value of Doppler US in diagnosis of graft rejection. Significance was assessed with chi(2) statistics. RESULTS: Of 113 Doppler US recordings in 30 children, 74 showed an episode of change in flow from triphasic to monophasic in 27 patients; biopsy correlation existed for 39 episodes. Thirty-five episodes were due to acute graft rejection (true-positive results). Thirty-nine episodes were due to a variety of pathologic causes (determined with biopsy results for 12 and by clinical means for 27) (false-positive results). Thirty-six assessments were true-negative (US and biopsy results negative for rejection); three were false-negative. When US results were evaluated against clinical and biopsy data, analysis revealed that change to monophasic flow predicted rejection with sensitivity of 92% (35 of 38) and specificity of 48% (36 of 75). Negative predictive value of evidence of persistent triphasic flow was 92% (36 of 39). In the subgroup of US findings with biopsy correlation, specificity increased from 48% (36 of 75) to 75% (36 of 48). It was zero (0 of 27) for the group with clinical correlation only. CONCLUSION: Change of hepatic venous flow pattern from triphasic to monophasic is sensitive but nonspecific for detection of graft rejection. Evidence of persistent triphasic flow helps eliminate the possibility of graft rejection with a high negative predictive value.  相似文献

Ultrasonography (US) of the native kidneys is commonly requested for acute renal failure (ARF), although in most cases the examination results are negative. To detect changes in the Doppler waveform associated with ARF and determine whether Doppler US can provide significant diagnostic information not available with standard US, 91 patients with ARF were studied to determine a mean resistive index (RI) for each patient. Forty-six patients had acute tubular necrosis (ATN) with a mean RI +/- 1 standard deviation of .85 +/- .06, which was significantly higher than the mean RI of .67 +/- .09 in 30 patients with prerenal ARF (P less than .01). Fifteen patients had ARF due to non-ATN intrinsic renal disease (mean RI, .74 +/- .13). An elevated RI (greater than or equal to .75) occurred in 91% of patients with ATN versus only 20% of patients with prerenal azotemia. Patients with severe liver disease (hepatorenal syndrome) are a subset of those with prerenal ARF that accounted for most of the elevated RIs in this group. The study demonstrates that intrarenal Doppler US allows detection of changes associated with ARF far more often than standard US. More important, Doppler US may be helpful in distinguishing ATN from prerenal azotemia.  相似文献

Purpose: To determine whether the difference of resistive indexes (RIs) in spleen and kidney (DI-RISK) is a more specific ultrasonographic (US) marker of intrarenal parenchymal damage than intrarenal RI alone. Materials and Methods: The study was approved by the local ethics committee. All study participants provided informed consent. The authors defined standard values for renal RI, splenic RI, and DI-RISK in 152 healthy subjects; carotid intima media thickness (IMT) was assessed as a marker of systemic vascular disease. Next, the authors measured these US parameters and collected echocardiographic data in 290 patients with chronic kidney disease (stage 2-4) recruited between September 2008 and February 2011 to evaluate the DI-RISK across the spectrum of stages of kidney function. Correlation coefficients were calculated with the Spearman test, and multivariate linear regression was used to analyze independent predictors of renal RI, splenic RI, and DI-RISK. Results: Healthy subjects had a mean age of 34.3 years ± 8.7, and patients with chronic kidney disease had a mean age of 65.0 years ± 12.3 (P < .001). In healthy subjects, both renal and splenic RIs were associated with IMT (renal RI: r = 0.19, P = .022; splenic RI: r = 0.23, P = .005); there was no correlation between DI-RISK and IMT (r = -0.10, P = .215). Similarly, in patients with chronic kidney disease, renal and splenic RIs correlated with IMT (renal RI: r = 0.33, P < .001; splenic RI: r = 0.30, P = .001). DI-RISK was associated with the estimated glomerular filtration rate (eGFR; r = -0.19, P = .001) but not with IMT (r = 0.08, P = .174). At multivariate regression analysis, DI-RISK was independently associated with eGFR but not with extrarenal factors. Conclusion: In patients with chronic kidney disease, renal RIs do not selectively indicate organ damage, but also mirror systemic vascular disease. The authors introduced DI-RISK as a potential US marker that may more specifically reflect kidney damage. ? RSNA, 2012.  相似文献

Colour Doppler US is well established for imaging of hepatic vessels in the assessment of pre- and post-liver transplant patients. Unfortunately, a full colour Doppler US examination of the portal or hepatic venous and hepatic arterial systems is frequently precluded by technical factors. Ultrasound contrast agents are useful in enhancing vascular Doppler signal and play an important role in liver transplantation assessment. A series of patients with vascular problems illustrates the role of US contrast in the pre-transplant candidate, where portal vein patency and direction of flow is assessed, presence of portal vein thrombus is confirmed and cavernous transformation demonstrated. Occlusion of hepatic veins in Budd-Chiari syndrome is confidently confirmed. Following liver transplantation, US contrast allows a comprehensive assessment of hepatic artery thrombosis, hepatic artery stenosis and pseudoaneurysm formation. The need for further imaging is reduced or confidently deferred in many instances. Ultrasound contrast agents play an important role in the liver transplant candidate. PURPOSE: To evaluate the usefulness of routine ultrasonographic (US) evaluation of the hepatic arterial resistive and pulsatility indexes and of the direction of portal venous blood flow for the diagnosis of intrahepatic arterioportal fistulas (APFs) in patients with liver cirrhosis. MATERIALS AND METHODS: In all patients with cirrhosis examined at one center over 4 years, the resistive (RI) and the pulsatility (PI) indexes in the right and left branches of the hepatic artery were evaluated with Doppler US. An APF was suspected when an RI decrease of at least 20% and a PI decrease of at least 30% were present in one hepatic lobe relative to values in the other lobe and portal blood flow in the lobe with the decreased values was reversed. The RI and PI in patients with an APF were compared with those in 75 patients with cirrhosis and without APFs at angiography. RESULTS: Seven patients with an APF were identified. APFs suspected at Doppler US were always confirmed with angiography. The percent differences +/- SD in the RI and the PI between the two intrahepatic branches of the hepatic artery in patients with versus in patients without an APF were as follows: RI, 35% +/- 6 (range, 27%-42%) versus 5% +/- 4 (range, 0%-15%) (P: <.001); PI, 50% +/- 5 (range, 41%-58%) versus 11% +/- 7 (range, 0%-26%) (P: <.001). CONCLUSION: The intrahepatic arterial resistive and pulsatility indexes and the direction of portal blood flow should be evaluated in routine screening for APFs in patients with liver cirrhosis.  相似文献

PURPOSE: To determine whether measurements with transcranial Doppler ultrasonography (US) of resistive indexes (RIs) of basal cerebral arteries with pressure provocation can be used to identify infants and children with craniosynostosis who have abnormal intracranial compliance and to study the effects of surgery on compliance. MATERIALS AND METHODS: Transcranial Doppler US was performed through the temporal squama, fontanels, and existing skull defects prior to and immediately following cranioplasty. Twenty-four studies were performed in six patients with multisuture synostosis, and 61 studies were performed in 26 patients with single-suture synostosis. Study findings were compared with those of 23 control subjects and were characterized as normal or abnormal on the basis of age-specific normal criteria for RI. RESULTS: In multisuture synostosis, results of six of the nine preoperative transcranial Doppler US studies were abnormal. During postoperative follow-up, three recurrences requiring reoperation occurred, one of which was detected with abnormal transcranial Doppler US findings. In single-suture synostosis, results of seven of the 26 preoperative transcranial Doppler US studies were abnormal, and all occurred in young infants with sagittal and unicoronal synostosis. Immediate effects of surgery were variable. All patients with sagittal synostosis had a significant immediate postoperative increase in RI, which normalized during postoperative follow-up. There was no significant difference in RI between patients with successfully treated craniosynostosis and control subjects. CONCLUSION: Transcranial Doppler US can be used to identify patients with craniosynostosis with decreased intracranial compliance, and it is a suitable noninvasive test to monitor the effects of surgery on compliance.  相似文献

Background

There is no doubt that the role of Different diagnostic imaging is well established in the evaluation of patients who are being evaluated for potential liver transplantation but it plays a huge role in the success of transplanted liver operations. Technical advances in imaging equipment and techniques allow more accurate assessment of postoperative living donor transplantation complications.

Objective

To assess the role and importance of different radiological imaging modalities in evaluating and diagnosing recipient complications after living donor liver transplantation.

Materials and methods

50 patients who underwent living donor liver transplantation (LDLT) were followed for at least 6?months and submitted for routine investigation including laboratory tests and imaging. The biliary complications were diagnosed with ultrasound (US) but MRCP was more diagnostic over ultrasound in case of location of strictures. Endoscopic retrograde cholangiopancreatography (ERCP) or percutaneous cholangiography (PTC) was used for therapeutic reasons. Stenting was tried in cases of biliary leakage. Doppler US was used in cases of graft rejection. CTA was used in cases whose HA (hepatic artery) couldn’t be detected by Doppler. Conventional angiography was used as a therapeutic tool for restoration of HA patency. This study was held between November 2014 until December 2016.

Results

Over 50 patients who underwent living donor liver transplantation. The morbidity rate was 66% (33 patients), where 17 patients passed an uncomplicated course, Biliary tract complications occurred in 13 patients (26%). Vascular complications were found in 8 patients (16%), one had portal vein thrombus and another patient had hepatic artery stenosis and underwent stent. Mild pleural effusion was seen in 30 patients (60%). Moderate to marked pleural effusion was seen in 13 patients (26%), Ascites was considered significant in case of moderate to marked or that persist after two weeks. Six patients complained significant collections, two of them improved by single tapping, whereas two patients required pig tail drainage (7 to14 days). The incidence of rejection was 24% in our study (12 patients) 75% of rejection (nine patients) occurred during the first two months postoperative. One case showed multiple hypodense hepatic focal lesions and the diagnosis was made by histopathology (biopsy) and was lymphoma.

Conclusion

The different radiological modalities are a cornerstone in the success of the liver transplantation operation together with curious postoperative follow up are the key for diagnosis of most of the complications including vascular, biliary and collections and even rejection cases.  相似文献
