目的 评价生物陶瓷材料iRoot BP Plus作为根管倒充填材料在根尖手术中的临床疗效.方法 选择临床符合纳入标准的慢性根尖周炎患牙,进行显微根尖手术治疗,对患牙进行根尖切除、超声器械根尖倒预备和iRoot BP Plus根管倒充填.术后1年复查,通过临床检查和X线片评定结果,分析临床疗效.结果 2013年2月至2014年2月期间完成显微根尖外科手术59例,共80颗患牙.术后1年有效回访53例,72颗患牙,术后1年治疗成功率达94.4%(68/72).患者性别、年龄、牙位、根尖周病变范围大小、根尖周病变类型、有无窦道等因素对疗效的影响均无统计学差异(P>0.05).结论 iRoot BP Plus作为根尖手术倒充填材料,有良好的临床效果.  相似文献

目的:研究在显微根尖手术治疗难治性慢性根尖周炎时应用MTA的可行性、特点、成功率及相关影响因素。方法:分析2013年1月~2015年12月间本院牙体牙髓科完成的显微根尖外科手术106例计146颗难治性慢性根尖周炎患牙的临床资料,其中上颌前牙75例、下颌前牙13例、上颌前磨牙6例、下颌前磨牙4例、上颌磨牙4例、下颌磨牙4例。手术包括根尖切除、根尖倒预备、根尖倒充填主要步骤,术后1年回访,通过临床症状和放射学检查评估疗效,并对预后影响因素进行卡方检验分析。结果:显微根尖外科手术联合MTA应用的1年成功率达95.9%(140/146),根据患者性别、年龄、牙位、根尖病灶范围分别统计手术疗效,组间差异均无统计学意义。失败5例6颗患牙,主要原因为根折及合并重度牙周炎。结论:在治疗难治性慢性根尖周炎时采用显微根尖手术联合MTA应用可取得满意效果;显微根尖手术是治疗慢性根尖周炎的有效手段。  相似文献

目的 观察三氧化矿物凝聚体(mineral trioxide aggregate,MTA)应用于显微根尖手术治疗慢性根尖周炎的效果.方法 无法行常规根管再治疗的慢性根尖周炎患者64例91颗患牙,随机分为观察组和对照组.观察组患者32例48颗,行显微根尖手术,应用MTA进行倒充填治疗.对照组患者32例43颗,行传统外科根尖手术联合银汞合金充填治疗.术后每隔3个月定期复诊,随访12个月,通过临床和X线检查评估根尖周病损愈合情况,进行疗效判定.结果 观察组痊愈27颗,改善17颗,成功率91.67%(44/48);对照组痊愈19颗,改善12颗,成功率72.09%(31/43).观察组治疗成功率明显高于对照组(x2=5.997,P=0.014).结论 MTA应用于显微根尖手术治疗慢性根尖周炎效果满意.  相似文献

目的探讨根尖刮治术对难治性根尖周炎的治疗效果。方法难治性根尖周炎患者40例患牙47颗,完成根管治疗后,即刻行根尖刮治术,术后3、6、12个月复查,摄根尖片,进行疗效评价。结果随访1年,39颗患牙临床症状消失,无任何不适,根尖片显示患牙根尖周有新骨形成或原有根尖周透射影缩小;5颗患牙临床症状明显改善,咀嚼时偶感不适,根尖片显示患牙根尖周有新骨形成或原有根尖周透射影缩小;总有效率为93.6%。2颗患牙术后症状无减轻,经患者同意后拔除。1颗患牙术后6个月复发,患者出现咀嚼痛,颊侧牙龈出现瘘管,根尖片示根尖周透射影与术前比较没有变化。结论完善根管治疗后即刻行根尖刮治术,可有效治疗难治性根尖周炎。  相似文献

目的 探讨和比较生物陶瓷材料无机三氧化矿物凝聚体(mineral trioxide aggregate, MTA)和iRoot BP plus在成年根尖孔未闭合患牙形成根尖屏障中的临床应用效果。方法 将200颗根尖孔未闭合恒牙根尖周炎患牙随机分为两组,分别采用MTA和iRoot BP plus制备根尖屏障。比较两种生物陶瓷材料在制备根尖屏障时的操作时间,分析不同根尖孔破坏类型、根尖区病变范围、根尖区充填情况以及患者年龄段对治疗成功率的影响。结果 iRoot BP plus组制备根尖屏障的操作时间比MTA组短,差异有统计学意义(P<0.05);不同根尖孔破坏类型、根尖区病变范围、根尖区充填情况以及患者年龄段对治疗成功率并无显著影响(P>0.05)。结论 MTA和iRoot BP plus在治疗根尖孔未闭合恒牙根尖周炎的临床效果相似,但iRoot BP plus的操作性更好。  相似文献

目的:评价“根尖诱导成型术联合根尖屏障术”治疗成年患者根尖开敞慢性根尖周炎的临床疗效。方法:根据临床随机对照研究原则,将29例成人根尖开敞慢性根尖周炎患者随机纳入研究组与对照组。研究组15颗患牙行根尖诱导成型术联合根尖屏障术,即采用Vitapex连续封药3月,待根尖形成钙化屏障后,再经根管iRoot BP Plus封闭根尖。对照组15颗患牙直接实施根尖屏障术,采用iRoot BP Plus封闭根尖。观察两组术后根管充填情况、根尖周指数(old-periapical index, O-PAI)、根尖透射区面积、根管下段1/3操作时间和临床疗效。结果:研究组15颗患牙均恰填,对照组15颗患牙10颗恰填,5颗超填。研究组术后12个月的O-PAI、根尖透射区面积、根管下段1/3操作时间明显低于对照组(P<0.05)。而12个月随访成功率,研究组显著高于对照组(P<0.05)。结论:采用“根尖诱导成型术联合根尖屏障术”治疗成年根尖开敞慢性根尖周炎的患牙,降低了O-PAI指数和根尖透射区面积,提高了成功率,且操作相对简单,可用于临床推广。  相似文献

目的 用倾向性分析的统计学方法比较和评价硅酸钙基生物陶瓷材料iRoot BP Plus或MTA作为根尖倒充填材料的临床疗效,并确定影响显微根尖手术成功率的预后因素。方法 检索2016—2018年在本院行显微根尖手术的病例。对使用iRoot BP Plus或MTA作为根管倒充填材料进行手术患者的年龄、性别、病变牙位、根管治疗类型、是否合并牙周病变5个变量采用最近邻匹配法进行1∶1倾向性评分匹配,分析其临床效果。采用多因素二元Logistic回归分析评价显微根尖手术的影响因素,并绘制森林图。结果 纳入患牙共179颗。所有协变量的绝对标准化差值均<0.1。iRoot BP Plus组和MTA组显微根尖外科手术的1年成功率分别为91.2%和89.7%(P=0.771)。PSM后多因素二元Logistic回归分析结果发现年龄、病变牙位、是否合并牙周病变对显微根尖手术预后具有显著影响。结论 MTA或者iRoot BP Plus作为根尖切除后的倒充填材料均可取得良好的临床效果。年龄、病变牙位、是否合并牙周病变是影响显微根尖手术临床疗效的显著因素。  相似文献

目的通过回顾性分析,研究iRoot SP辅助单尖充填法及多种预后因素对根管治疗临床效果的影响。 方法从2014—2018年于中山大学附属口腔医院接受根管治疗且能追踪复查的患者中,选择使用iRoot SP辅助单尖充填后且复诊时间为1年以上患者的110颗患牙纳入分析。根据每颗患牙复诊时临床和影像学检查结果,将患牙的预后分为成功和失败两组。使用连续性修正卡方检验和Fisher精确检验,对术前是否存在根尖阴影、术前根尖阴影的直径大小和有无封闭剂超充等8项可能影响iRoot SP单尖充填临床效果的预后因素进行统计分析。 结果110颗接受iRoot SP单尖充填治疗的患牙总体成功率为95.5%。其中,83颗术前有根尖阴影患牙的治疗成功率为96.4%,68.7%为已愈合;27颗术前无根尖阴影患牙的治疗成功率为92.6%。术前有根尖阴影和无根尖阴影患牙的治疗成功率差异无统计学意义(χ2 = 0.084,P = 0.772)。术前根尖阴影直径小于或等于5 mm的患牙共53颗,治疗成功率为98.1%;而阴影直径大于5 mm的患牙共30颗,治疗成功率为93.3%。两组患牙治疗成功率的差异无统计学意义(χ2 = 0.259,P = 0.611)。44颗封闭剂超充患牙的治疗成功率为97.7%,患牙有无iRoot SP超充的治疗成功率差异也无统计学意义(χ2 = 0.218,P = 0.640)。 结论iRoot SP单尖充填法是可行的根管充填技术。  相似文献

目的:分析生物根管封闭剂iRoot SP在扁形根管根尖封闭中的性能,为临床工作提供指导.方法:取48颗根管的近远中径与颊舌径之比小于0.5的单根管离体前磨牙,根管预备后,随机分为A、B、C3组,每组各16颗.分别选用Vitapex (A组)、AH Plus(B组)、iRoot SP(C组)作为根管封闭剂,结合连续波热牙胶垂直充填技术进行根管充填.所有样本经根尖染料渗透实验,采用透明牙技术,比较3组离体牙微渗漏情况,评价iRoot SP在扁形根管根尖封闭中的性能.采用SPSS19.0软件包对数据进行统计学分析.结果:A组染料渗入深度显著大于B、C组,差异具有显著性(P<0.05);B、C组染料渗入深度相比,无显著差异(P>0.05).结论:iRoot SP和AH Plus在扁形形根管根尖封闭中的性能接近,效果优于Vitapex根管充填糊剂.  相似文献

目的:评价根尖切除加直视下根管充填一次完成术治疗前牙慢性根尖周炎的临床疗效.方法:选取因根管治疗失败或根尖病变范围较大而无法单纯用根管治疗治愈的患者34例(患牙40颗),随机分为2组,每组20颗;观察比较根尖切除加直视下根管充填一次性完成术(A组)与根管充填完成后再行根尖切除术(B组)在治疗时间、术后疼痛率、充填质量和近期疗效之间的差异;采用SPSS12.0软件包分别进行t检验、χ2检验和Mann-Whitney U检验.结果:根管充填时间A组显著少于B组(P<0.05):术后疼痛率、充填质量和近期疗效评价,2组差异无统计学意义.结论:根尖切除加直视下根管充填一次性完成术治疗前牙慢性根尖周炎操作简便,临床近期疗效与根管充填完成后再行根尖切除术相似.  相似文献

目的 研究iRoot BP Plus用于外伤露髓年轻恒牙活髓切断术对牙齿变色率及根管钙化的影响。方法 选取2018年1月至2020年6月于本院行活髓切断术的82例患儿,随机分为对照组(使用MTA盖髓材料)和观察组(使用iRoot BP Plus盖髓材料)各41例,随访1年,比较两组不同时间的手术疗效、根管钙化和牙齿变色情况,以及手术时间、肿胀时间和疼痛持续时间、牙齿功能和美观度,并记录两组患儿术后并发症。结果 两组术后3个月、末次随访的疗效差异无统计学意义(P>0.05);观察组肿胀时间、术后疼痛持续时间短于对照组(P<0.05);观察组的舒适、固定和美观评分高于对照组(P<0.05);术后6个月及末次随访观察组的根管钙化和牙齿变色率低于对照组(P <0.05);观察组的并发症发生率为2.44%,低于对照组的14.63%(P<0.05)。结论 iRoot BP Plus与MTA均能较好地用于外伤露髓年轻恒牙的牙髓切断术,其中iRoot BP Plus能减少根管钙化、牙齿变色情况,应用前景良好。  相似文献

IntroductionCalcium hydroxide has been used as a traditional pulpotomy agent for a long time but has some disadvantages. iRoot BP Plus (Innovative Bioceramix Inc, Vancouver, Canada) is a newly developed, ready-to-use calcium silicate–based bioactive ceramic with excellent bioactivity and sealing ability. However, whether iRoot BP Plus shows superiority over calcium hydroxide as a pulpotomy material on permanent incisors with complicated crown fractures remains unknown.MethodsThis research included 205 permanent incisors with complicated crown fractures. These teeth were treated with pulpotomy and divided into 2 groups according to the pulpotomy material (105 treated with iRoot BP Plus and 100 with calcium hydroxide). Clinical and radiographic information was collected during the 12- to 24-month follow-up period. The formation of reparative dentin bridges and pulp canal obliteration were analyzed using radiographs in both groups.ResultsThe success rates for recall in the average follow-up period of 17.5 ± 4.4 months (12–24 months) after pulpotomy treatment were significantly different between the 2 groups, with 99% for the iRoot BP Plus group and 93% for the calcium hydroxide group. Reparative dentin bridges were observed in 92.4% of the iRoot BP Plus group and 90% of the calcium hydroxide group, but the difference was not significant. Pulp canal obliteration was observed in 2 teeth (2%) in each group.ConclusionsThe success rates obtained in our study indicate that iRoot BP Plus as a pulpotomy agent can be a suitable alternative to calcium hydroxide to manage complicated crown fractures.  相似文献

Objectives: This study evaluated the intensity and duration of postoperative pain after the removal of root canal filling material in retreatment procedures of upper incisor teeth with chronic apical periodontitis, using different techniques.

Materials and methods: One hundred and thirty-five patients requiring retreatment of upper incisor teeth with chronic apical periodontitis were included in the study. The patients were assigned to three groups of 45 patients, according to the method used to remove old canal filling material. In group 1, canal filling material was removed using hand files. In group 2, the canal filling material was removed with ProTaper universal retreatment (PTUR) instruments. In group 3, Reciproc instruments were used to remove canal filling material. Teeth were then medicated with calcium hydroxide and sealed using temporary filling material. The presence of postoperative pain was assessed after 6, 12, 24, 48 and 72?h, 7 days, and finally after 10 days.

Results: In all time intervals, except for 72?h, 7 days and 10 days, group 1 participants reported more intense postoperative pain than those in groups 2 and 3 (p?=?0.02). In all time intervals, there was no difference in the pain scores between groups 2 and 3 (p?=?0.08). In all groups, the intensity of postoperative pain decreased over time. The required time to remove canal filling material was less for the Reciproc group compared to the hand and ProTaper retreatment groups (p?=?0.032).

Conclusions: Hand files caused greater postoperative pain after non-surgical endodontic retreatment (NSER) of upper incisor teeth with chronic apical periodontitis compared to the ProTaper retreatment and Reciproc files.  相似文献

目的:观察iRoot SP一次性根管治疗前牙窦道型慢性根尖周炎的临床疗效.方法:纳入240 例前牙窦道型慢性根尖周炎患者,随机分成2 组(n=120),在Nd:YAG激光消毒下,进行一次性根管充填:观察组采用iRoot SP以热牙胶连续波垂直加压技术充填根管,对照组采用AH-plus糊剂以热牙胶连续波垂直加压技术充填根管,观察术后48 h疼痛发生情况、术后10 d窦道愈合情况及1 年后的疗效.结果:术后48 h观察组和对照组疼痛发生率分别为7.14%和15.0%(P=0.038);术后10 d,观察组和对照组窦道愈合情况有效率分别为95.8%和88.3%(P=0.031);术后1 年复诊,2 组疗效成功率分别为98.3%和94.8%(P>0.05).结论:iRoot SP和AH-plus糊剂在一次性根管治疗前牙窦道型慢性根尖周炎中均取得良好疗效,但iRoot SP术后反应发生率低,窦道愈合快.  相似文献

IntroductionThe aim of this retrospective, cohort case series was 2-fold: (1) to evaluate the outcomes of teeth with necrotic pulps and apical periodontitis using long-term calcium hydroxide (Ca[OH]2) (healing was assessed via the periapical index [PAI] system) and (2) to explore the possible association of fractures in relation to long term Ca(OH)2 exposure.MethodsA total of 242 cases, diagnosed with pulpal necrosis and apical periodontitis, were treated with long-term Ca(OH)2 using a standardized protocol. Injectable and powdered Ca(OH)2 were placed sequentially in the root canal system. All cases were re-evaluated within a 3-month period until radiographic healing was observed. Clinical and radiographic evaluations were performed annually. Pre- and postoperative periapical radiographs were evaluated using the PAI system.ResultsOf the 242 cases, 219 participants completed their treatment with annual follow-up. The average Ca(OH)2 time was 5.4 months with a range of 1–12 months. Overall, by the latest follow-up visit, 90.0% (197/219) were classified as "healed." The overall mean preoperative and postoperative PAI scores were 4.07 (±0.80) and 1.76 (±0.70), respectively. Kappa statistics showed an almost perfect agreement for inter-rater (κ = 0.91) and intrarater (κ = 0.95) reproducibility for both examiners.ConclusionsWithin the limitations of this study, the use of long-term Ca(OH)2 in the treatment of teeth with necrotic pulps and apical periodontitis resulted in a predictable high outcome. There was no association observed between long-term Ca(OH)2 use and the incidence of fractures during this study. Ca(OH)2 is a suitable material of choice as an interappointment dressing for teeth diagnosed with pulpal necrosis and apical periodontitis.  相似文献

The aim of this study was to evaluate radiographic periapical repair after endodontic treatment using QMix or NaOCl irrigation in single-rooted teeth with apical periodontitis. In this randomised clinical trial, 60 patients were selected and randomised into two groups (n = 30). After instrumentation of root canals, QMix or NaOCl was used as a final irrigation and the canals obturated with gutta-percha and AH Plus sealer. All the participants were recalled after 12 months and 15 patients were lost to the study so 45 patients were evaluated radiographically according to periapical index (PAI) scores. A significant decrease was seen in PAI scores after 1 year in all patients (p < 0.001). In the QMix and NaOCl group, absence or reduction of radiolucency was observed in 23 of 25 teeth (92%) and 20 of 20 teeth (100%), respectively. There was no significant difference between two groups according to the decrease in PAI score (p = 0.508). Root canal treatments with or without QMix irrigation contributed equally to periapical healing.  相似文献

目的 观察联合应用不同根管充填糊剂治疗慢性根尖周炎的临床疗效.方法 选取2007-2009年山东大学口腔医学院综合科门诊诊治的慢性根尖周炎患者298例(422颗牙),按患者就诊先后顺序随机分为4组.A组(AH Plus糊剂+Vitapex糊剂+牙胶尖)75例患者108颗牙,B组(AH Plus糊剂+牙胶尖)75例患者105颗牙,C组(Vitapex糊剂+牙胶尖)74例患者105颗牙,D组[氧化锌丁香油糊剂(ZOE)糊剂+牙胶尖]74例患者104颗牙.对4组术后7 d、6个月、1年的临床疗效进行比较、分析.结果 298例患者根管充填7 d后复查,A、C组术后反应轻,与B组比较,差异具有统计学差异(P<0.05);D组术后反应最重,与其他3组比较,差异均有统计学意义(均P<0.05).根管充填6个月、1年后复查的结果没有差异(P>0.05),A组有效率最高,与其他3组比较,差异有统计学意义(P<0.05);D组有效率最低,与其他3组比较,差异有统计学意义(P<0.05).结论 AH Plus糊剂+Vitapex糊剂+牙胶尖治疗慢性根尖周炎,既诱导了根尖周阴影区的骨质增生修复,又能使根尖孔形成完善的封闭,达到治愈根尖周炎、防止继发感染或复发的目的 .  相似文献
