1854年,Killam发现并报告了4例发生于产后的脊椎滑脱患者并提出了脊柱滑脱的定义。Robert于1年后证明本病的病因为椎弓崩裂。腰椎椎弓峡部是指上、下关节突之间的狭窄部分。单纯峡部裂多无明显临床症状,多在X线检查中无意发现。峡部发生断裂后,受累脊柱运动节段骨性结构发生破坏,最终导致腰椎滑脱。腰椎滑脱的临床症状主要是下腰痛。  相似文献

目的 探讨成人腰椎峡部裂型滑脱症的微创外科治疗方法与临床效果.方法 成人峡部裂型腰椎滑脱症患者21例,男12例,女9例;年龄29~73岁,平均51.7岁;L4.5滑脱7例,L5S1滑脱14例;双侧峡部裂19例,单侧2例.根据Meyerding分级:Ⅰ度滑脱13例,Ⅱ度滑脱7例,Ⅲ度滑脱1例.患者全麻,经后路双侧旁中央26 mm切口,在内镜辅助下行单侧或双侧椎管减压、椎间植骨cage融合、新型Sextant-R经皮椎弓根螺钉系统复位与内固定.结果 平均手术时间170 min,平均出血量160 ml,平均卧床时间7.5 d,平均住院时间19 d.20例患者获得随访,随访时间4~32个月,平均12.5个月.术后1年随访17例,患者VAS腰痛评分从术前平均(6.0±2.6)分降至(2.9±2.5)分;VAS腿痛评分从术前平均(6.7±3.3)分降至(2.8±1.6)分;Oswestry功能指数从术前平均44.3%降至27.1%;Nakai分级优良率90%.矢状面X线片腰椎滑脱率从术前35.5%±2.5%降至8.3%±7.5%,滑脱椎的腰前凸角从术前平均11.5°±1.7°增至16.8°±9.5°,椎间隙平均高度从术前(5.4±2.5)mm增至(9.1±3.0)mm.根据Lenke动态X线片评估标准:13例(76%)椎间骨性融合、2例(12%)椎间部分骨性融合、2例(12%)椎间无明显骨性融合.3例发生并发症.结论 内镜辅助下后路腰椎管减压、椎间植骨cage融合及经皮椎弓根螺钉系统复位与内固定治疗成人峡部裂型腰椎滑脱症手术创伤小,近期临床疗效好.  相似文献

Methods Twenty-one patients with adult isthmic spondylolisthesis underwent minimally invasive surgery. There were 12 men and 9 women, with the mean age of 51.7 years. Isthmic spondylolisthe-sis occurred at the L4.5 in 7 patients, and at the L5S1 in 14 patients. According to Meyerding classification, 13 cases were of Grade Ⅰ, 7 of Grade Ⅱ, 1 of Grade Ⅲ. Under general anesthesia, guiding by fluoroscopy, the surgeries performed minimally invasive microendoscopic(METRx) techniques for posterior decompression, interbody cage fusion and novel Sextant-R percutaneous pedicle screw techniques for deformity reduction and fixation. Results Seventeen cases were followed up 1 year postoperatively. The average low back pain VAS reduced from preoperative 6.0±2.6 to postoperative 2.9±2.5. The average leg pain VAS decreased from preoperative 6.7±3.3 to postoperative 2.8±1.6. The average ODI decreased from preoperative 44.3% to post-operative 27.1%. The Nakai good and excellent rate was 90%. The mean operative time 170 min, blood loss 160 ml, and postoperative stay in bed 7.5 d. The sagittal spondylolisthesis rate significantly decreased from preoperative 35.5%±2.5% to postoperative 8.3%±7.5%. The lordotic angle from preoperative 11.5°±1.7° in-creased to postoperative 16.8°±9.5°; the intervertebral disc height from preoperative (5.4±2.5) mm increased to postoperative (9.1±3.0) mm. According to Lenke judgement for fusion, complete fusion rate was 76%, in-complete fusion rate was 12%, nonfusion rate was 12%. Conclusion The minimally invasive microendo-scopic (METRx) assisted with a novel Sextant-R percutaneous pedicle screw systems for deformity reduction and fixation to treat adult isthmic spondylolisthesis, is not only a minimally invasive and safe surgical tech-nique, also an effective treatment for deformity reduction and fixation.  相似文献

股骨粗隆下区骨折是髋关节周围骨折中最难处理的一种骨折类型,多由高能量创伤所致的粉碎性骨折。非手术治疗有较高的畸形愈合、不愈合以及其他并发症发生率,因而,粗隆下骨折多主张手术治疗。Russell-Taylor分类方法临床较常用,有利于指导内固定方法的选择。对于小粗隆以下平面的骨折,采用标准的交锁髓内钉较理想。骨折波及到小粗隆但梨状窝未累及时,选用头髓钉或95°角钢板可取得满意效果。对于大粗隆骨折并累及梨状窝时,采用滑行钉器械固定较好。DHS器械不适宜用于粗隆下骨折的治疗。  相似文献

目的探讨采用后路腰椎椎管减压、钉棒内固定并椎间植骨融合术和后路腰椎减压并Dynesys系统内固定手术治疗退变性腰椎滑脱的效果。方法应用后路腰椎椎间融合(posterior lumbar interbody fusion,PLIF)术进行椎管减压、钉棒系统固定并椎体间植骨融合手术治疗退变性腰椎滑脱37例;应用后路腰椎管减压并Dynesys内固定手术治疗退变性腰椎滑脱5例。结果随访9~39个月,平均26个月,腰痛疼痛视觉模拟量表(visual analogue scale,VAS)评分术前为8.7分,随访时为2.1分;腿痛VAS评分术前为7.6分,随访时为2.3分。术前Oswestry功能障碍指数(Oswestry disability index,ODI)为58.2%,随访时为21.2%。无严重手术并发症发生。术后X线片复查显示椎间高度均得到不同程度的恢复,滑脱椎体完全复位或者基本复位,椎间植骨融合。无融合器移位或螺钉松动、断裂。结论后路腰椎椎管减压、钉棒内固定并椎间植骨融合术和后路腰椎减压并Dynesys内固定手术治疗退变性腰椎滑脱效果满意,安全彻底的神经根管减压是取得满意临床效果的关键。  相似文献

目的:探讨微创通道辅助下治疗伴邻近节段退变腰椎滑脱症的治疗方法及临床疗效。方法:2014年4月至2016年1月,36例单节段腰椎滑脱症伴邻近节段退变患者接受微创通道下简化手术治疗(目标神经靶点减压、邻近节段椎间或椎板间小关节融合、单边内固定),其中退变性腰椎滑脱23例,峡部裂性腰椎滑脱13例,MeyerdingⅠ度16例,Ⅱ度17例,Ⅲ度3例。采用腰痛视觉模拟(VAS)评分、功能障碍指数(ODI)、JOA评分评估临床效果。结果:36例患者术中出血230~480 ml,平均340 ml;术后出血15~80 ml,平均43 ml;手术时间176~240 min,平均193 min;术中X线暴露时间2~6 s,平均3.6 s。2例术中硬膜囊撕裂,没有出现神经损伤表现。30例获得随访,时间12~17个月,平均15.2个月。术前、术后5 d及术后3个月随访时腰背痛VAS评分分别是7.6±1.7、1.9±0.4、0.8±0.4,术后5 d、术后3个月与术前比较差异有统计学意义(P0.05)。术前及术后3个月ODI评分分别为35.9±1.2和3.7±0.7,差异有统计学意义(P0.05)。术前、术后5 d及术后1、3个月JOA评分分别为13.2±0.4、24.4±0.4、27.4±0.1、27.9±0.5,术前与术后5 d及术后1、3个月比较差异有统计学意义。结论:微创通道下治疗伴邻近退变的腰椎滑脱症是一种安全有效,创伤小,操作简化,康复快的微创治疗方法。  相似文献
