Intake of high–glycemic index (GI) food has been postulated to reduce satiety, resulting in an increased total energy intake and reduced access to body fat as fuel. Thus, we hypothesize that high dietary GI and glycemic load (GL) are associated with an increased prevalence of obesity in the Korean population. Dietary GI and GL were calculated for 933 Korean men and women based on dietary intake assessed by food frequency questionnaires and using a GI table developed from published GI databases in a cross-sectional design. Mean differences in dietary GL and carbohydrate intake between obese and nonobese men were statistically significant after adjusting for covariates (P = .027 and .021, respectively). High dietary GL and carbohydrate intake were negatively associated with the prevalence of obesity among men in a multivariate-adjusted logistic regression model (P for trend = .026 and .036, respectively). Statistically significant effects of dietary GI and GL on the prevalence of obesity among women were observed in a generalized linear model (P = .002 and .042, respectively) and a logistic regression model (P for trend < .001 and = .007, respectively), after adjusting for covariates. Women with higher dietary GI and GL were more likely to be obese, a result consistent with our hypothesis. However, an inverse association for dietary GL and carbohydrate and prevalence of obesity was found in men, suggesting that mechanisms contributing to the prevalence of obesity between sexes may be different.  相似文献

目的了解目前中国居民碳水化合物的消费情况与成人膳食生糖负荷的现状。方法使用2002年中国居民营养与健康状况调查数据,根据食物血糖生成指数(glycemic index,GI)和碳水化合物摄入量计算膳食生糖负荷(glycemic load,GL)。分析中国成人膳食GL的性别、年龄和城乡分布,分析不同膳食GL/4184kJ水平居民的膳食结构,探索中国成人膳食GL/4184kJ的合理范围。结果我国成人膳食GL92%来源于粮谷类。每人每日膳食GL平均为269.5,农村287.3,城市233.0,农村高于城市。青年平均为279.0,中年272.2,老年238.8,青年最高,其次是中年,最低是老年。男性平均为292.8,女性249.0,男性高于女性。低膳食GL/44184kJ的人群膳食碳水化合物供能比低,脂肪供能比高,粮谷类消费低,脂肪消费高,超重和肥胖发生率高。结论粮谷类是我国成人膳食GL的最主要来源。我国居民低膳食GL/4184kJ的人群粮谷类消费过低,脂肪消费过高,膳食结构不合理。城市居民较农村居民更为严重。以人群粮谷类食物摄入量、碳水化合物供能比、脂肪供能比在推荐的合理范围为依据,初步认为中国成人的膳食GL/4184kJ适宜范围为80~120。  相似文献

Age-related hearing loss is a frequent disability in older adults and nutrition could play a role in the development of this condition. Carbohydrate nutrition [including dietary glycemic index (GI) and load (GL)] may be linked to hearing loss. We aimed to determine the association between carbohydrate nutrition (including mean dietary GI and GL, and the dietary intakes of carbohydrate and sugar), starch, cereal and total fiber, and age-related hearing loss. The Blue Mountains Hearing Study is a population-based survey of age-related hearing loss (1997-1999 to 2002-2004). Hearing loss was measured in 2956 participants (aged ≥50 y) and was defined as the pure-tone average of frequencies 0.5, 1.0, 2.0, and 4.0 kHz > 25 dB hearing level. Dietary data were collected in a semiquantitative FFQ. A purpose-built database based on Australian GI values was used to calculate the mean GI. A higher mean dietary GI was associated with an increased prevalence of any hearing loss, comparing quintiles 1 (lowest) and 5 (highest), [multivariable-adjusted odds ratio = 1.41 (95% CI = 1.01-1.97)]. Participants in the highest quartile of mean dietary GL intake compared with those in the lowest quartile had a 76% greater risk of developing incident hearing loss (P-trend = 0.04). Higher carbohydrate and sugar intakes were associated with incident hearing loss (P-trend = 0.03 and P-trend = 0.05, respectively). In summary, a high-GL diet was a predictor of incident hearing loss, as was higher intake of total carbohydrate. Hence, high postprandial glycemia might be a potential underlying biological mechanism in the development of age-related hearing loss.  相似文献

Nutrient intakes of 2149 black and white, 9- and 10-year-old girls varied by race, household income, and parental education. Of the three variables, higher education was most consistently associated with more desirable levels of nutrient intakes, that is, lower percentage of dietary fat and higher levels of vitamin C, calcium, and potassium. Higher income was related to higher intakes of vitamin C, but lower intakes of calcium and iron. Higher income was associated with lower percentage of dietary fat. After adjustment for income and education, race was associated with intakes of calcium, vitamin C, and to a lesser extent, percentages of kilocalories from total fat and polyunsaturated fat, and potassium. Black girls had a significantly lower intake of calcium (720 versus 889 mg) and a higher intake of vitamin C (91 versus 83 mg).Proportions of the cohort with inadequate or excessive intakes of micronutrients and macronutrients were also estimated. A high proportion of girls exceeded the recommended intake level of 30% of kilocalories from total fat (90% of black girls; 84% of white girls) and 10% of kilocalories from saturated fat (92 and 93%, respectively). Low intakes of calcium (40% of black girls and 20% of white girls) and zinc (36 and 38%, respectively) commonly were found for girls of both races.  相似文献

目的探讨膳食碳水化合物和生糖负荷与中国成人血脂水平及血脂异常患病危险的关系,为膳食防治提供科学依据。方法使用2002年中国居民营养与健康状况调查数据,根据食物血糖生成指数(glycemic index,GI)和碳水化合物摄入量计算膳食生糖负荷(glycemic load,GL)。以膳食GL作为指标,采用单相关分析和多元回归分析探讨膳食碳水化合物与中国成人血脂水平的关系;采用趋势分析和Logistic回归分析探讨膳食碳水化合物与中国成人血脂异常患病危险的关系。结果膳食GL与人群血浆总胆固醇(TC)水平和低密度脂蛋白胆固醇(LDL-C)水平呈负相关,高GL可降低高TC血症和高LDL-C血症患病危险;膳食GL与人群高密度脂蛋白胆固醇(HDL-C)水平呈负相关,高GL可增加低HDL-C血症患病危险;膳食GL与血浆甘油三酯(TG)水平呈负相关,但未见高GL对高TG血症患病风险有显著影响。结论以粮谷类食物为主,碳水化合物为主要能量来源的中国传统膳食模式有利于预防血脂异常的发生。  相似文献

To assess the association between dietary glycemic index (GI), glycemic load (GL) and dietary quality indices in Iranian adults. This cross section was conducted among 1571 Iranian adults aged ≥19 years. GI, GL and diet quality indices were estimated by 24-h recall and DDS was calculated using a validated 48-item food frequency questionnaire. Dietary transition in the Arctic is associated with decreased quality of diet, which is of particular concern for women of childbearing age due to the potential impact of maternal nutrition status on the next generation. The study assessed dietary intake and adequacy among Inuit women of childbearing age living in three communities in Nunavut, Canada. A culturally-appropriate quantitative food-frequency questionnaire was administered to 106 Inuit women aged 19-44 years. Sources of key foods, energy and nutrient intakes were determined; dietary adequacy was determined by comparing nutrient intakes with recommendations. The prevalence of overweight/obesity was >70%, and many consumed inadequate dietary fibre, folate, calcium, potassium, magnesium, and vitamin A, D, E, and K. Non-nutrient-dense foods were primary sources of fat, carbohydrate and sugar intakes and contributed >30% of energy. Traditional foods accounted for 21% of energy and >50% of protein and iron intakes. Strategies to improve weight status and nutrient intake are needed among Inuit women in this important life stage.  相似文献

Studies have suggested differential associations of specific indicators of socioeconomic position (SEP) with nutrient intake and a cumulative effect of these indicators on diet. We investigated the independent association of SEP indicators (education, income, occupation) with nutrient intake and their effect modification. This cross-sectional analysis included 91,900 French adults from the NutriNet-Santé cohort. Nutrient intake was estimated using three 24-h records. We investigated associations between the three SEP factors and nutrient intake using sex-stratified analysis of covariance, adjusted for age and energy intake, and associations between income and nutrient intake stratified by education and occupation. Low educated participants had higher protein and cholesterol intakes and lower fibre, vitamin C and beta-carotene intakes. Low income individuals had higher complex carbohydrate intakes, and lower magnesium, potassium, folate and vitamin C intakes. Intakes of vitamin D and alcohol were lower in low occupation individuals. Higher income was associated with higher intakes of fibre, protein, magnesium, potassium, beta-carotene, and folate among low educated persons only, highlighting effect modification. Lower SEP, particularly low education, was associated with lower intakes of nutrients required for a healthy diet. Each SEP indicator was associated with specific differences in nutrient intake suggesting that they underpin different social processes.  相似文献

Dietary factors are not consistently associated with metabolic syndrome abnormalities. In this cross-sectional study, we hypothesized that distinct clustering patterns exist in metabolic syndrome abnormalities and that those patterns are differentially associated with dietary factors. To test this hypothesis, we examined distinct clustering patterns of metabolic syndrome abnormalities and their association with dietary factors in Korean adults. A total of 141 subjects were recruited through the Family Medicine Division of the General Hospital in Seoul. Subjects who had complete data on waist circumference, blood pressure, blood glucose and lipid indicators, and no medication usage were included in this study. Dietary intake data were obtained by multiple 24-hour recalls (2-4 days) through on-site or telephone interviews. To identify clustering patterns of metabolic syndrome abnormalities, factor analysis was used for waist circumference, systolic and diastolic blood pressure, blood glucose, triglycerides, and high-density lipoprotein cholesterol. Three distinct clustering patterns were identified: (1) high blood pressure, (2) dyslipidemia, and (3) high blood glucose. The high blood pressure pattern was significantly associated with higher alcohol intake and lower carbohydrate intake. The dyslipidemia pattern was significantly associated with a diet of high glycemic index and glycemic load. The high blood glucose pattern was associated with lower carbohydrate intake. Metabolic syndrome abnormalities had 3 distinct clustering patterns independently associated with dietary factors. Diets with high glycemic index and glycemic load were strongly linked with the dyslipidemia pattern, and high alcohol intake was linked with the high blood pressure pattern in Korean adults.  相似文献

摘要:目的 了解厦门市城区居民碳水化合物消费情况及膳食血糖负荷及膳食胰岛素负荷的现状,调查膳食血糖负荷及膳食胰岛素负荷与生化指标。方法 根据全国营养调查方案,2010年10-12月对厦门市城区的6个社区居委会成年居民进行调查,采用连续3 d 24 h回顾法对其中的291人进行膳食调查、身体测量及生化指标测量;根据膳食血糖生成指数(GI)和碳水化合物摄入量计算膳食血糖负荷(GL);根据膳食胰岛素指数(II)和能量及食用频率计算膳食胰岛素负荷(IL)。结果 被调查的厦门市城区成年居民的平均膳食血糖指数为75.2;平均膳食血糖负荷为179.1;平均膳食胰岛素指数为30.4,平均膳食胰岛素负荷为247.2。粮谷类对膳食GL、IL的贡献达92.0%、83.1%。不同GL、IL水平的被调查人群的身体测量及生化指标间的差异无统计学意义;通过多元线性回归分析并未发现膳食GI、GL、II、IL与糖尿病的患病风险有明显的相关性。结论 粮谷类食物是厦门市城区居民膳食GL、IL的主要来源,低膳食GL的人群粮谷类消费低,脂肪消费高,膳食结构不合理;并未发现膳食GL、IL与身体测量及生化指标之间的联系。  相似文献

Several studies have indicated the association between intake of foods high in dietary glycemic index (GI) and glycemic load (GL) with an increased risk of digestive tract cancers. We hypothesized that GI and GL may be associated with risk of esophageal squamous cell carcinoma (ESCC) in a high-risk population in Iran. In total, we interviewed 47 cases with incident of ESCC and 96 frequency-matched hospital controls, then calculated the average dietary GI and GL via a validated food frequency questionnaire. Dietary GL was calculated as a function of GI, carbohydrate content, and frequency of intake of certain foods. Dietary GI and GL levels were significantly higher among the ESCC cases compared with the controls (P < .05). After adjustment for potential confounders, those in the highest tertile of dietary GI had 2.95 times higher risk of ESCC compared with those in the lowest (95% confidence interval, 1.68-3.35; P for trend = .002). In addition, being in the highest tertile of dietary GL was positively associated with an ESCC risk (odds ratio, 3.49; 95% confidence interval, 2.98-4.41; P for trend = .001). Findings of the present study indicate that diets with high GI and GL might have potentially unfavorable effects on ESCC risk and suggest a possible role for excess circulating insulin and related insulin-like growth factor 1 in esophageal cancer development.  相似文献

OBJECTIVE: To determine if a relationship exists between blood glucose control and variability in nutrient intake from day-to-day in subjects with type 1 diabetes. METHODS: Two three-day diet records and one measurement of glycated hemoglobin (HbA1c) were obtained from 272 subjects with type 1 diabetes treated with a mixture of regular and NPH insulins before breakfast and supper and using a standardized algorithm to adjust insulin dose according to the results of self-monitoring of blood glucose two to four times daily. Day-to-day variation in nutrient intake was expressed as the coefficient of variation (CV = SDx100/mean). RESULTS: Nutrient intakes in the study population (mean +/- SD) were energy 8.35+/-2.43 MJ, fat 81+/-30 g, protein 94+/-28 g, carbohydrate 227+/-68 g, starch 126+/-38 g and dietary fiber 20+/-6 g with diet glycemic index being 84.2+/-7.4. Neither energy, nutrient intakes nor insulin dose was significantly related to HbA1c. Day-to-day variation of carbohydrate (p = 0.0097) and starch (p = 0.0016) intakes and diet glycemic index (p = 0.033) was positively related to HbA1c, and the associations remained significant when adjusted for age, sex, duration of diabetes and BMI. Day-to-day variation in energy, protein or fat intakes was not related to HbA1c. CONCLUSIONS: Consistency in the amount and source of carbohydrate intake from day-to-day is associated with improved blood glucose control in people with type 1 diabetes, a result which supports continued educational efforts to achieve adherence to a diabetes diet plan. This conclusion may not apply to people on intensified insulin therapy who adjust their insulin dose based on their actual carbohydrate intake at each meal.  相似文献

STUDY OBJECTIVE--As there is a social status gradient in chronic disease mortality in Australia, this study aimed to establish whether there were substantial differences among socioeconomically defined groups with respect to food choice and nutrient intake, in the context of risk of nutrition related chronic diseases. DESIGN AND PARTICIPANTS--Cross sectional data were collected from a randomly selected population sample of 1500 urban Australian adults. Data were collected by postal questionnaire, which included an assessment of dietary intake and questions on sociodemographic details. Three measures of social position were collected: occupation, educational status, and income status. Occupation was interpreted both on a continuous, prestige scale, and also as categorical occupational groupings. MAIN RESULTS--The study achieved a 70% response rate. Higher social status was generally associated with healthier dietary intakes, with lower fat and refined sugar densities, and higher fibre densities, but also with higher alcohol density. No differences were found in salt, polyunsaturated fat, protein, or complex carbohydrate densities across groups. Food intake differences were also found between occupational status groups, with the upper social groups tending to consume more wholegrain cereal foods, low fat milk, and fruit, and less refined cereal foods, full cream milk, fried meat, meat products, and discretionary sugar; but also more cheese and meat dishes. CONCLUSIONS--Although this study did show statistically significant differences across social status groups in relation to nutrient and food intakes, these differences were small compared to the disparity between intakes of all groups and the recommended patterns of intake, and did not appear to be great enough to be a major explanatory variable in differences in disease risk across groups.  相似文献

BACKGROUND: It is unclear whether immediate dietary effects on blood glucose influence the risk of developing type 2 diabetes. OBJECTIVE: The objective of this study was to examine whether the dietary glycemic index (GI) and glycemic load (GL) were associated with the risk of type 2 diabetes in older adults. DESIGN: The Health, Aging, and Body Composition Study is a prospective cohort study of 3075 adults who were 70-79 y old at baseline (n=1898 for this analysis). The intakes of specific nutrients and food groups and the risk of type 2 diabetes over a 4-y period were examined according to dietary GI and GL. RESULTS: Dietary GI was positively associated with dietary carbohydrate and negatively associated with the intakes of protein, total fat, saturated fat, alcohol, vegetables, and fruit. Dietary GL was positively associated with dietary carbohydrate, fruit, and fiber and negatively associated with the intakes of protein, total fat, saturated fat, and alcohol. Persons in the higher quintiles of dietary GI or GL did not have a significantly greater incidence of type 2 diabetes. CONCLUSIONS: These findings do not support a relation between dietary GI or GL and the risk of type 2 diabetes in older adults. Because dietary GI and GL show strong nutritional correlates, the overall dietary pattern should be considered.  相似文献

目的了解高等职业技术学院学生膳食营养状况,为开展营养指导工作提供依据。方法对青海高等职业技术学院385名学生采用24 h膳食回顾法连续3 d跟踪调查,计算学生每人每日营养素摄入量,并与推荐摄入量比较。结果与《中国居民膳食营养素参考摄入量》比较,学生的能量、蛋白质、脂肪、维生素、矿物盐摄入量均有不同程度的不足;钙摄入量明显不足,仅占推荐摄入量的33.3%;维生素C摄入量不足,占推荐量的64.0%;蛋白质主要来源于粮谷类食物,占蛋白总量的52.1%;脂肪供能比例偏低,占总能量15.8%。结论该学院学生膳食结构不合理。应全面增加各种营养素摄入量,加强营养知识教育,提高学生的营养水平。  相似文献

We aimed to investigate associations between dietary macronutrient proportions and prospective visceral adiposity index changes (ΔVAI). The study included 1254 adults (18–74 years), from the Tehran Lipid and Glucose Study (TLGS), who were followed for three years. Dietary intakes were assessed twice using food frequency questionnaires. Associations of dietary macronutrient with ΔVAI and risk of visceral adiposity dysfunction (VAD) after three years were investigated. The percentage of energy intake from protein in the total population, and from fat in women, were associated with higher increases in VAI. A 5% higher energy intake from protein substituted for carbohydrate, monounsaturated fatty acids (MUFAs), and polyunsaturated fatty acids (PUFAs) was associated with higher ΔVAI. Higher energy intake from animal protein substituted for PUFAs was positively associated with ΔVAI. Substituting protein and PUFAs with MUFAs were related to higher ΔVAI. The associations were similar in men and women, but reached significance mostly among women. Risk of VAD was increased when 1% of energy from protein was replaced with MUFAs. Substituting protein for carbohydrate and fat, and fat for carbohydrate, resulted in increased risk of VAD in women. Higher dietary proportions of protein and animal-derived MUFA may be positively associated with ΔVAI and risk of VAD.  相似文献

  目的  调查分析天津市孤独症谱系障碍(ASD)儿童的膳食营养素摄入量和营养状况,为ASD儿童制定科学有效的营养干预措施提供依据。  方法  采用3 d 24h膳食回顾法调查天津市某特殊教育学校90名3~9岁ASD儿童24种膳食营养素的摄入情况,并对儿童进行体格测量。  结果  ASD儿童中超重肥胖儿童共31名,占34.4%;3名消瘦者占3.3%;1名生长迟缓者占1.1%。各年龄组ASD儿童维生素A、维生素B1、维生素D、维生素B6、叶酸、钙和碘的平均摄入量均低于推荐摄入量标准,铜、磷和锌的摄入量均高于推荐摄入量标准,其中有超过10%的ASD儿童铜和镁的摄入量超过可耐受最高摄入量。能量、脂肪、维生素A摄入情况构成在正常、超重、肥胖组差异有统计学意义(χ2值分别为9.24,10.03,P值均 <0.05)。  结论  孤独症谱系障碍儿童超重、肥胖率较高,营养素缺乏与过量的现象同时存在,建议采取个性化的饮食营养干预方案。  相似文献

目的:探讨膳食结构、尤其是脂肪类食物在妊娠期糖代谢异常发病过程中的作用。方法:选择孕24~28周检查诊断为妊娠期糖尿病(GDM)和妊娠期糖耐量减低(GIGT)孕妇62例作为GDM组,选择同期糖试验正常(GNGT)孕妇65例作为对照组。对GDM组孕妇饮食干预治疗前进行膳食和活动状况的调查。比较两组孕妇体重指数、活动水平、代谢率和膳食结构中三大营养素供能比例,特别是膳食脂肪、脂肪酸类型等。结果:①GDM组孕妇每日需要量为(2419±41)kcal,摄入能量为(2188±53)kcal;对照组每日需要量和摄入量分别为(2421±39)kcal和(2141±32)kcal,两组比较差异无统计学意义(P>0.05)。GDM组孕妇体重指数(BMI)和活动水平(PAL)为(28.2±0.8)kg/m2和1.35±0.02,对照组为(25.1±0.4)kg/m2和1.42±0.01,GDM组体重指数高于对照组,活动水平低于对照组,两组比较均具明显统计学差异(P<0.01)。②GDM组孕妇碳水化合物、蛋白质和脂肪供能百分比分别为(53.1±3.7)%、(17.0±0.4)%和(29.3±1.3)%,对照组分别为(53.1±1.7)%、(16.9±0.6)%和(26.0±1.0)%。GDM组脂肪供能百分比高于对照组,两组比较差异具有统计学意义(P<0.05);两组碳水化合物和蛋白质供能百分比比较,差异无统计学意义(P>0.05)。③问卷调查显示,两组孕妇膳食种类摄入相对量"适量"占80.5%~91.9%。孕妇饱和脂肪酸(SFA)主要来源于猪、牛、羊、鸡等动物,绝对值比较GDM组摄入频率高于对照组;多不饱和脂肪酸(PUFA)主要来源于鱼、植物油、坚果类,GDM组低于对照组;单不饱和脂肪酸(MUFA)主要来源于植物油、鸡蛋和菌类,两组略有差异。结论:①孕妇高脂肪、高SFA和低PUFA的饮食结构可能与妊娠期糖代谢异常的发病有关。②孕期应控制孕妇BMI的增加,根据BMI指导营养摄入及活动水平。  相似文献

Adequate and balanced nutrition during gestation is essential for achieving healthy pregnancy outcomes. This retrospective survey tested the hypothesis that maternal nutrition (macronutrients intake, micronutrients intake, and weight gain) during gestation will impact gestational outcomes (gestational age, birth weight, and labor spontaneity). The objectives of this study were to test the impact of macronutrients and micronutrients consumed during gestation, pregestational weight and weight gain during gestation on gestational age, birth weight, and labor spontaneity among Jordanian women. Study participants (n = 700) were recruited from a women's hospital in northern Jordan. Data were collected using structured interviews. Dietary determinants examined in the study included daily intake of macronutrients (energy, carbohydrate, protein, fat, and n-3 and n-6 fatty acids) and micronutrients (zinc, calcium, folate, vitamin C, vitamin A, and vitamin B6), pregestational body mass index, total weight gain, and weight gain pattern during pregnancy. Dietary data were obtained from a semiquantitative food frequency questionnaire. Mean (SEM) daily intakes of energy, carbohydrate, and protein were 10894 (138) Kj, 320 (3.8) g, and 90 (0.95) g, respectively; with fat contributing 36% of total energy. Average daily energy and carbohydrate intake during pregnancy were related negatively to gestational age (P < .05). Pregestational body mass index was associated positively with gestational age and birth weight (P < .05). Intakes of vitamin A and calcium had significant relationships with birth weight. Daily intakes of zinc, calcium, vitamin B6, and n-3 and n-6 fatty acids were associated with labor spontaneity. Dietary factors consumed during pregnancy are associated with pregnancy outcomes.  相似文献
