Typical ocular and CNS melanomas are hyperintense on T1-weighted MR images and hypointense on T2-weighted MR images. We performed MR imaging in 48 patients with melanoma metastatic to visceral organs. Images were reviewed retrospectively in order to determine whether there were predominant MR features specific for visceral melanoma and to see if visceral metastases have MR characteristics similar to metastases in the CNS. Eleven patients also were examined after injection of gadopentetate dimeglumine to evaluate the enhancement characteristics of these tumors. Two hundred sixty-one lesions were found. Lesions were classified according to their signal intensities relative to uninvolved liver on T1-weighted, T2-weighted, and short TI inversion recovery (STIR) pulse sequences. Most commonly, lesions were either hypointense or isointense on T1-weighted sequences and hyperintense on T2-weighted and STIR sequences (185 lesions). Less frequently, lesions were hyperintense on T1-weighted sequences and hypointense or isointense on T2-weighted and STIR sequences (59 lesions). A mixed pattern was seen on T1- and T2-weighted sequences in 17 lesions. The patterns did not correlate with lesion size. Of the three sequences studied by subjective comparison, the STIR sequence in our series had the highest sensitivity for lesion detection and yielded the highest lesion conspicuity. Injection of gadopentetate dimeglumine in 11 patients did not increase either the number or the conspicuity of lesions seen. Our results show that visceral metastases from melanoma have a wide variety of appearances on MR images. The STIR sequence appears to be optimal, and the metastases do not enhance with gadopentetate dimeglumine.  相似文献

Purpose: To compare the diagnostic values of fluid-attenuated inversion recovery (FLAIR) and gradient spin-echo (GRASE) with those of conventional spin-echo (SE) and fast SE T2-weighted sequences in the evaluation of acute cerebrovascular lesions at 0.5 T.Material and Methods: Twenty-two consecutive patients with the clinical diagnosis of acute cerebrovascular accident were examined by MR imaging within the first 48 h of ictus. MR examination included 5-mm axial conventional SE and turbo SE (TSE) T2-weighted, dual-echo GRASE and FLAIR sequences. The patients also had pre- and postcontrast T1-weighted axial images. Two examiners evaluated the images and scored the conspicuity of the acute lesions.Results: Regardless of location, FLAIR provided the best lesion conspicuity in the detection of acute infarcts, followed by the GRASE sequence. In the posterior fossa, TSE and SE demonstrated the lesions better than GRASE and FLAIR techniques. In the detection of hemorrhagic elements within the ischemic region, TSE demonstrated statistically significant superiority over other sequences.Conclusion: In the detection of acute ischemic lesions in locations other than the posterior fossa, FLAIR provided the best lesion conspicuity among four T2-weighted sequences, including SE, TSE, GRASE and FLAIR. However, for the posterior fossa examination, preference of SE or TSE T2-weighted sequences is suggested.  相似文献

BACKGROUND AND PURPOSE: The T1-weighted fast spin-echo (T1-FSE) MR imaging sequence is not used routinely, since the speed advantage is not as dramatic as it is in T2-weighted imaging. We evaluated the T1-FSE sequence to determine whether this technique can replace the conventional T1-weighted spin-echo (T1-SE) sequence for routine contrast-enhanced imaging. METHODS: Sixty-nine patients with intracranial enhancing lesions underwent both T1-SE and T1-FSE sequences in a random order after administration of contrast agent. Acquisition time was 55 seconds for the T1-FSE sequence and 2 minutes 38 seconds for the SE sequence. The conspicuity of enhancing lesions, peritumoral edema, and gray-to-white matter contrast as well as motion and flow artifacts were analyzed. Signal-to-noise ratios of enhancing lesions, gray matter, and white matter as well as contrast-to-noise ratios (CNRs) of enhancing lesions, with gray matter with white matter as the standard, were calculated. RESULTS: The conspicuity of enhancing lesions was better on T1-FSE sequences than on T1-SE sequences, although the difference in the CNRs of enhancing lesions did not reach significance. Images obtained with the T1-FSE sequence showed less flow and motion artifacts than did those obtained with the T1-SE sequence. The conspicuity of peritumoral edema and gray-to-white matter contrast was lower on the T1-FSE images than on the T1-SE images. CONCLUSION: The T1-FSE sequence reduces imaging time and has the potential to replace the conventional T1-SE sequence for the evaluation of enhancing lesions in the brain when time is a consideration.  相似文献

The pelvis: T2-weighted fast spin-echo MR imaging.   总被引:4,自引:0,他引:4  
Fast spin-echo (SE) T2-weighted magnetic resonance (MR) imaging provides images with highly T2-weighted contrast in substantially reduced imaging times. In a prospective evaluation, fast SE T2-weighted imaging of the pelvis was compared with conventional SE T2-weighted imaging in 30 consecutive patients in whom pelvic pathologic conditions were suspected. Three reviewers independently analyzed the images for (a) overall image quality, (b) pelvic organ definition, (c) conspicuity of pelvic fluid, and (d) conspicuity of pelvic pathologic conditions. Fast SE images were rated superior to conventional SE T2-weighted images in 60% (54 of 90) of the case reviews for overall image quality, in 69% (62 of 90) for pelvic organ definition, in 63% (57 of 90) for conspicuity of pelvic fluid, and in 68% (43 of 63) for conspicuity of pelvic pathologic conditions. The fast SE examinations were typically three to four times faster than conventional SE T2-weighted examinations. No pathologic findings seen on conventional SE T2-weighted images were undetected on fast SE images. Fast SE images may replace conventional SE T2-weighted images in MR imaging of the pelvis.  相似文献

PURPOSETo compare an ultrafast T2-weighted (half-Fourier acquisition single-shot turbo spin-echo [HASTE]) pulse sequence with fast spin-echo T2-weighted sequences in MR imaging of brain lesions.METHODSFast spin-echo and HASTE images of 34 consecutive patients over the age of 50 years or with suspected demyelinating disease were reviewed independently by two neuroradiologists for the number of lesions less than 5 mm and greater than or equal to 5 mm, and for lesion conspicuity, gray-white matter differentiation, and extent of periventricular confluent signal abnormality. The reviewers also assessed for the presence of hemosiderin and extent of motion artifacts.RESULTSPer patient, the mean number of 5-mm or larger lesions detected on fast spin-echo images (1.4) relative to the number detected on HASTE images (0.8) was not statistically significant. For lesions less than 5 mm, fast spin-echo images showed more lesions (7.5) than HASTE images did (2.4). The fast spin-echo images were better at depicting gray-white matter differentiation, conspicuity of lesions, and periventricular signal abnormality. Of four T2 hypointense lesions seen on fast spin-echo images, none was detected on HASTE images.CONCLUSIONAlthough the HASTE technique might be useful for rapid imaging of the brain, our study shows a diminished sensitivity for the detection of lesions less than 5 mm in diameter and for T2 hypointense lesions.  相似文献

Pyogenic liver abscess: contrast-enhanced MR imaging in rats   总被引:1,自引:0,他引:1  
MR imaging was used to evaluate experimentally induced pyogenic liver abscesses in an animal model. Rats were examined before and after IV administration of either gadolinium-diethylenetriaminepentaacetic acid (Gd-DTPA), ferrite particles, or both contrast agents together. Pyogenic liver abscesses appeared hypointense on T1-weighted images and hyperintense on T2-weighted images. Bolus administration of Gd-DTPA using a fast spin-echo sequence with repetition time of 250 msec and echo time of 20 msec (SE 250/20) showed transient selective enhancement of normal hepatic tissue and increased lesion conspicuity, quantitatively assessed by the contrast-to-noise ratio, which increased from -35.7 to -59.0. Delayed leakage of Gd-DTPA into the abscess center partially obscured small lesions at 30-60 min. Ferrite particles reduced the signal intensity of normal liver, and the abscess then appeared homogenously hyperintense. Applying the SE 500/32 sequence, the contrast-to-noise ratio increased from -1.2 to +74.0. Coordinated administration of both contrast agents showed a further increase in contrast to +94.0, with a hyperintense abscess rim surrounded by hypointense liver. Gd-DTPA increases abscess-liver contrast by rim enhancement of the abscess wall, and ferrite increases the abscess-liver contrast by selectively decreasing the signal intensity of surrounding normal liver. As a result of increased contrast-to-noise ratio, both contrast agents, alone or in combination, increase the conspicuity of hepatic abscesses.  相似文献

PURPOSETo compare a rapid fluid-attenuated inversion-recovery (FLAIR) sequence with T1-weighted, fast spin-echo proton density-weighted, and T2-weighted images in the evaluation of cerebrovascular disease.METHODSAll patients underwent standard T1-, proton density-, and T2-weighted fast spin-echo and fast FLAIR MR imaging at 1.5 T. Images were compared for lesion size, location, and conspicuity.RESULTSForty-five infarctions were identified on T2-weighted and fast FLAIR sequences. Lesion size was comparable on the proton density-weighted, fast T2-weighted, and fast FLAIR sequences, although lesion conspicuity was superior on the fast FLAIR images in 43 (96%) of the lesions. Associated periventricular and pontine hyperintensities were more extensive on the fast FLAIR images.CONCLUSIONOur modified fast FLAIR technique provided improved conspicuity of infarctions and white matter disease as compared with T1-, proton density-, and T2-weighted spin-echo images, and a reduced scan time compared with conventional FLAIR sequences in patients with cerebrovascular disease.  相似文献

This study was undertaken to determine whether phased array breath-hold T1- and T2-weighted sequences can replace non-breath-hold spin echo (SE) sequences in the imaging of focal liver lesions by comparing overall image quality, liver-lesion contrast, and artifact. Both breath-hold and non-breath-hold T1-weighted and T2-weighted imagings of focal liver lesions were prospectively compared in 120 patients with suspected focal liver lesions imaged at 1.5 T with use of a body phased array multicoil. Breath-hold images were acquired with T1-weighted fast low-angle shot (FLASH) and T2-weighted turbo spin echo (TSE) sequences, and non-breath-hold images were made with conventional T1- and T2-weighted SE sequences. Qualitative image analysis was done by three blinded readers, and quantitative analysis was done. The highest signal-to-noise ratios were obtained with breath-hold T1-weighted FLASH sequence. The signal-to-noise ratios of breath-hold T2-weighted TSE sequence were slightly inferior to those of non-breath-hold SE sequence. Both T1-weighted and T2-weighted breath-hold sequences had less image artifact. Overall image quality of breath-hold sequences was better than that of non-breath-hold sequences for both T1- and T2-weighted sequences (P < .01). The tissue contrast of T1-weighted FLASH sequence was superior to that of SE sequence (P < .01). On T2-weighted imaging, tissue contrast of solid lesions was better on conventional SE sequence than that on breath-hold TSE sequence (P < .01). Respiratory ghost artifact was less prominent on T1-weighted FLASH sequence, although this artifact was occasionally seen on breath-hold T2-weighted TSE sequence. In a state-of-art MR unit with use of a phased array multicoil, conventional T1-weighted can be replaced by breath-hold sequences. On T2-weighted imaging, because solid tumor-liver contrast on breath-hold TSE imaging is inferior to that on non-breath-hold SE image, breath-hold imaging may not replace conventional non-breath-hold T2-weighted SE sequence.  相似文献

At liver magnetic resonance (MR) imaging in 38 patients, a breath-hold T2-weighted fast spin-echo (SE) pulse sequence optimized with fast recovery was compared with a conventional respiratory-triggered fast SE sequence and a breath-hold single-shot fast SE sequence. Mean signal-to-noise ratios for liver and contrast-to-noise ratios for hepatic lesions were higher with the breath-hold fast-recovery fast SE sequence than with the respiratory-triggered fast SE sequence (P <.05). Breath-hold fast-recovery images displayed better lesion clarity than did single-shot fast SE images (P <.05) and fewer image artifacts than did respiratory-triggered fast SE images (P <.05). The ability to determine lesion size and the overall image quality was best with the breath-hold fast-recovery sequence (P <.05). These results may justify use of the breath-hold fast-recovery fast SE pulse sequence for first-line T2-weighted MR imaging of the liver.  相似文献

The contribution of proton spectroscopic (PS) imaging to magnetic resonance (MR) imaging of the liver was assessed at 0.5 T in 55 patients with known or suspected hepatic malignancy. PS images were compared subjectively with T1- and T2-weighted spin-echo (SE) images for hepatic lesion detection and conspicuity. For hepatic metastases (n = 27), PS images were equal to T1-weighted images in lesion detection in 17 patients but showed fewer lesions in five patients and false-negative results in two. When compared with T2-weighted images, PS images depicted more lesions in six patients, an equal number of lesions in 18, and fewer lesions in two. Hepatomas (n = 8) were detected with each sequence in all patients. Hepatomas were often more conspicuous on PS images than on T2-weighted images; they were of equal conspicuity on PS and T1-weighted images in most cases. Whereas fatty infiltration (n = 16) appeared on PS images as areas of low signal intensity similar to that of paraspinal muscle, it produced no detectable abnormality on either T1- or T2-weighted images. PS imaging is inferior to T1-weighted SE imaging in the detection of hepatic metastases. The major role of PS imaging at intermediate field strength is to differentiate focal fatty infiltration from hepatic metastases.  相似文献

Focal nodular hyperplasia of the liver: MR findings in 35 proved cases   总被引:6,自引:0,他引:6  
MR images of 28 patients with 35 lesions of hepatic focal nodular hyperplasia were reviewed to determine the frequency of findings considered typical of this condition (isointensity on T1- and T2-weighted pulse sequences, a central hyperintense scar on T2-weighted images, and homogeneous signal intensity). Fifteen lesions were imaged at 0.6 T with T1- and T2-weighted spin-echo (SE) pulse sequences; 20 lesions were imaged at 1.5 T with T1-weighted SE and gradient-echo pulse sequences and T2-weighted SE pulse sequences. Diagnosis of focal nodular hyperplasia was made pathologically in 25 patients, with nuclear scintigraphy in four, and with follow-up imaging in six. Only seven lesions (20%) were isointense relative to normal liver on both T1- and T2-weighted images. On T1-weighted SE images, 21 lesions (60%) were isointense relative to normal liver, 12 (34%) were hypointense, and two (6%) were hyperintense. On T2-weighted SE images, 12 lesions (34%) were isointense and 23 (66%) were hyperintense relative to normal liver. A central scar was present in 17 lesions (49%) and was hypointense relative to the lesion on T1-weighted images and hyperintense on T2-weighted images. Twenty lesions (57%) were of homogeneous signal intensity throughout the lesion, except for the presence of a central scar. All three MR imaging characteristics were present in three cases (9%). We conclude that hepatic focal nodular hyperplasia has a wide range of signal intensity on MR imaging.  相似文献

BACKGROUND AND PURPOSEThe aim of this study was to establish the MR imaging characteristics of choroidal hemangioma and to compare them with those of uveal melanoma.METHODSAmong 41 patients examined at 1.5 T (4-cm surface coil, T1-weighted and fast spin-echo T2-weighted sequences), 25 had uveal melanoma and 16 had circumscribed choroidal hemangioma. After i.v. bolus injection of gadopentetate dimeglumine, dynamic and T1-weighted sequences were acquired.RESULTSIn patients with choroidal hemangioma, uniform signal characteristics were detected on fast T2-weighted images. In 15 of 16 patients with choroidal hemangioma, lesions were isointense with vitreous on fast spin-echo T2-weighted images, whereas lesions in 24 of 25 patients with uveal melanoma were hypointense. Signal characteristics of uveal melanoma and hemangioma did not differ significantly on plain T1-weighted images. Enhancement was earlier and much stronger for circumscribed choroidal hemangioma than for uveal melanoma. After i.v. bolus application of gadopentetate dimeglumine, the increase of signal intensity was higher for circumscribed choroidal hemangioma (signal intensity ratio, 5.8) than for uveal melanoma (signal intensity ratio, 2.2).CONCLUSIONCircumscribed choroidal hemangioma may be difficult to differentiate from melanoma by ophthalmologic examination. Differentiation may not be possible if direct viewing of uveal space-occupying lesions is hampered by opaque vitreous media. The characteristic findings on fast spin-echo T2-weighted MR images and early enhanced images aid in differentiating choroidal hemangioma from uveal melanoma.  相似文献

AIM: To compare T1-weighted magnetization transfer (MT) with fluid attenuated inversion recovery (FLAIR) imaging for evaluating conspicuity and number of lesions in individuals with brain tuberculoma. MATERIALS AND METHODS: In all 28 patients with brain tuberculoma underwent MR examination using fast spin-echo (FSE) T2, spin-echo (SE) T1, T1-weighted MT and FLAIR imaging. Post-contrast T1-weighted MT imaging was taken as the gold standard for assessing the number of lesions. Tuberculomas detected both on T1-weighted MT and FLAIR imaging were examined for the wall to be defined, and were divided into two groups on the basis of presence (group 1) or absence (group 2) of perilesional oedema visible on FLAIR imaging. The mean signal intensity of the wall of the lesions and adjacent oedema or brain parenchyma was analyzed qualitatively and quantitatively. RESULTS: The number of lesions detected on T1-weighted MT was higher than on FLAIR imaging (209 versus 163). Conspicuity in both groups was better on T1-weighted MT images qualitatively as well as quantitatively. The difference in the signal intensity of the wall of the lesion and perilesional oedema was statistically significant only on T1-weighted MT images in group 1 (p=0.0003 versus 0.3), whereas in group 2 it was statistically significant both on T1-weighted MT and FLAIR imaging (p=0.009 versus 0.05). CONCLUSION: FLAIR imaging is not helpful in the examination of brain tuberculomas compared with T1-weighted MT imaging, as it neither contributes to the characterization of lesion nor assesses the true disease load.  相似文献

To compare the clinical usefulness of T2-weighted breath-hold sequences for imaging the liver, 33 patients with 97 focal hepatic lesions were studied with a 1.0-T scanner by using T2-weighted breath-hold turbo spin-echo (SE) sequences and T2-weighted breath-hold half-Fourier single-shot turbo SE (HASTE) sequences with and without fat suppression. Images were quantitatively analyzed for liver signal-to-noise ratio (SNR) and lesion-to-liver contrast-to-noise ratios (CNR). Qualitative analysis was performed for lesion conspicuity, motion artifacts, and anatomic sharpness of extrahepatic structures. Breath-hold turbo SE imaging with fat suppression showed the highest CNR for cystic lesions and the best lesion conspicuity for cystic and solid lesions among the four sequences. For solid lesions, there was no significant difference of lesion-to-liver CNR between them. HASTE sequence was superior to turbo SE sequences in terms of motion artifacts; however, the usefulness for evaluating focal hepatic lesions was limited compared with turbo SE sequence with fat suppression. Addition of fat suppression was not helpful for HASTE imaging because of decreased lesion conspicuity and extrahepatic details without the advantage of reducing motion artifacts. This study suggests that turbo SE sequence with fat suppression is most useful for breath-hold T2-weighted liver imaging at 1.0 T. Addition of imaging without fat suppression can be considered for evaluating extrahepatic structures. HASTE sequence may have a role for imaging uncooperative patients due to absence of motion artifacts.  相似文献

PURPOSE: To determine if hypointense lesions clearly outline on T2-weighted fast spin-echo (SE) magnetic resonance (MR) images obtained during coagulative interstitial laser-induced thermotherapy (LITT) of a prostate with benign hyperplasia. MATERIALS AND METHODS: In six patients with benign prostatic hyperplasia (BPH), 12 LITT treatments were followed online with repetitive axial T2-weighted fast SE imaging (repetition time, 3,700 msec; echo time, 138 msec; acquisition time, 19 seconds). Development, time course, correlation with interstitial tissue temperature, and diameters of hypointense lesions around the laser diffusor tip were investigated. Lesion diameters on T2-weighted images acquired during LITT were compared with diameters of final lesions on T2-weighted images and unperfused lesions on enhanced T1-weighted SE images obtained at the end of therapy. RESULTS: Hypointense lesions developed within 20-40 seconds of LITT. Average correlation coefficients between interstitial temperature development and signal intensity development were 0.92 during LITT and 0.90 after LITT. Regression slopes were significantly steeper during LITT (0.67% signal intensity change per degree Celsius) than after LITT (0.47% per degree Celsius; P = .038). Lesions remained visible after LITT for all procedures. Average maximum diameters of lesions were 1-3 mm larger during LITT than after LITT (P = .0006-.019). CONCLUSION: Repetitive T2-weighted fast SE MR imaging during interstitial coagulative LITT of BPH demonstrates the development of permanent hypointense prostate lesions. However, posttherapeutic lesion diameters tend to be overestimated during LITT.  相似文献

Respiratory and cardiac motion correction may result in better turbo spin-echo (SE) imaging of the lung. To compare breath-hold cardiac-gated black-blood T2-weighted turbo SE and turbo short-inversion-time inversion-recovery (STIR) magnetic resonance (MR) imaging pulse sequences with conventional breath-hold turbo SE and half-Fourier acquisition single-shot turbo spin-echo (HASTE) sequences for lesion conspicuity of focal lung lesions, 42 patients with focal lung lesions were prospectively studied with MR imaging at 1.5 T. Helical computed tomography was used as a reference. In comparison with the conventional breath-hold turbo SE sequence, all black-blood sequences had fewer image artifacts arising from the heart and blood flow. The overall image quality for the black-blood turbo SE and turbo STIR sequences was superior to that for the breath-hold turbo SE and HASTE sequence (P < 0.01). Not only focal lung lesions but also surrounding inflammatory changes were clearly visualized with these two sequences. With the HASTE sequence, although several slices could be obtained in one breath-hold, both the tumor and vessels appeared blurred. We conclude that T2-weighted turbo SE and turbo STIR imaging of the lung with effective suppression of flow and motion artifacts provide high-quality images in patients with focal lung lesions.  相似文献

Sixty-seven ocular tumors were studied with magnetic resonance (MR) imaging and computed tomography (CT). These tumors included primary uveal melanoma (n = 55), circumscribed choroidal hemangioma (n = 3), diffuse choroidal hemangioma (n = 1), retinal capillary hemangioma (n = 1), medulloepithelioma (n = 1), choroidal nevus (n = 1), retinoblastoma (n = 1), and choroidal metastases (n = 4). MR imaging demonstrated all these lesions, while CT demonstrated 88%. Associated retinal detachment was more easily distinguished from the neoplasms with MR imaging. Extrascleral extension of melanoma and hemorrhagic cystic necrosis within the melanoma were clearly demonstrated with MR imaging, but not with CT. Ninety-three percent of melanomas were markedly hyperintense, compared with the intensity of the vitreous body, on T1-weighted images and hypointense on T2-weighted images. All metastatic lesions were isointense on T1-weighted images and hypointense on T2-weighted images. The circumscribed choroidal hemangiomas were hyperintense on T1-weighted images and isointense on T2-weighted images. MR imaging is superior to CT in detection of intraocular tumors and may be more specific in diagnosis.  相似文献

Malignant uveal melanoma and simulating lesions: MR imaging evaluation   总被引:7,自引:0,他引:7  
Twenty-one patients with intraocular disease were studied by magnetic resonance (MR) imaging and computed tomography (CT). In 13 cases, malignant uveal melanoma was considered the likely diagnosis. Both imaging methods were accurate in determining the location and size of uveal melanomas. MR imaging was superior for the assessment of possible associated retinal detachment, for assessment of vitreous change, and for differentiating uveal melanoma from choroidal hemangioma and choroidal detachment. A case of retinal gliosis could not be differentiated from uveal melanoma by either technique. Uveal melanomas appeared as hyperintense lesions on T1-weighted images and as hypointense lesions on T2-weighted images. High signal intensity of the vitreous was observed in patients with vitritis and in those who were thought to have protein leaking into the vitreous as a result of impairment of the retinal-blood barrier.  相似文献

目的比较常规SE序列与动态Gd-U.----x增强扫描对肝局灶病变的诊断价值。方法对34例肝局灶病变做了常规SE平扫和动态Gd-U.---A增强及延迟万WI增强扫描;就各序列对肝局灶病变的检出率、病变的信噪比(C/N)值和图像质展进行定员和定性分析。结果36例共142个病灶,动态Gd-lyl?l?A检出率(138/142,958%)明st高于IFZWI和延迟TW[增强(128/14,叨%;119/142,838%)(P<005):动态Gd-IJ:1713A增强的C/N值高于TZWI和TIWI延迟增强(P<0.05);动态增强的伪影较L们少(P<0.01),而病灶清晰度各序列之间无明显差别(P>0.历)。结论动态u-ly:1717A增强扫描在病灶检出率、图像质显反C/N值方面均优于IWI,对肝局灶病变的诊断是一种有价值的方法,  相似文献

PURPOSE: To evaluate fat-suppressed T2-weighted magnetic resonance (MR) imaging with conventional spin-echo (SE), breath-hold fast SE, respiratory-triggered fast SE, and breath-hold multishot SE echo-planar sequences for the detection of focal hepatic lesions. MATERIALS AND METHODS: Fat-suppressed T2-weighted MR images obtained with the four sequences in 55 patients with 81 solid and 129 nonsolid lesions were retrospectively analyzed. Image review was conducted on a segment-by-segment basis; a total of 440 liver segments were reviewed separately for solid and nonsolid lesions by three independent radiologists. Diagnostic accuracy was evaluated with receiver operating characteristic analysis. RESULTS: The mean lesion-to-liver contrast-to-noise ratio was highest on the multishot SE echo-planar images of both solid and nonsolid lesions. Fat-suppressed respiratory-triggered fast SE images had significantly better (P < .05) or comparative detectability of both solid and nonsolid lesions compared with the other types of images. Image quality was best on the respiratory-triggered fast SE images. CONCLUSION: Fat-suppressed respiratory-triggered fast SE imaging should replace fat-suppressed conventional SE imaging as a standard T2-weighted imaging examination in the detection of focal hepatic lesions.  相似文献
