The purpose of this study was to evaluate the effect of splinting teeth on the results of periodontal reconstructive surgery using a specific carbonate bone replacement graft (BRG) material. Forty-five patients were randomly treated with a periodontal surgery approach. Natural coral calcium BRG was utilised in 33 patients. This 33-patient group was divided into three equal groups. In the presplint group, teeth were splinted to at least two rigid teeth before surgery, in the postsplint group, teeth were splinted at suture removal, and in the nonsplint group, the treated teeth were not splinted at all. In 12 patients, teeth were treated with surgical debridement (DEBR) alone and not splinted. Periodontal probing depth (PPD), clinical probing attachment level (CPAL), and tooth mobility were measured using desmodontometry (DDM) and periotest (PTV) with reproducible methods before surgery and at various periods up to 1 year afterwards . A decrease in PPD (5.4 mm, SD 1.4 mm) and tooth mobility (DDM-horizontal 257 μm, SD 60 μm) and a gain of CPAL (5.1 mm, SD 1.4 mm) were seen following the use of BRG in presplint teeth. In the same group, PPD and tooth mobility were significantly reduced compared to nonsplint teeth. DEBR alone showed reductions in tooth mobility and PPD and a significantly smaller gain in CPAL than in presplint teeth treated with BRG. The less favourable improvement in periodontal function of postsplint or nonsplint teeth seemed to be due to the loss of BRG material caused by tooth mobility. These results indicate that an undisturbed wound healing process using BRG together with tooth stability is beneficial to overall clinical success. Objective To investigate the vertical bone height and the bone density of the palate for implants placement using cone beam CT(CBCT)and to provide references to the safe and stable placement of palatal implants.Methods Three-dimensional reformatting images were reconstructed with the selected CBCT scanning data of 34 patients aged 18 to 35 yeras,by means of EZ implant software.The vertical bone height was measured at 20 interesting sites of palate.Bone density was measured at 10 sites that could support 3.0mm long implants.The data of the vertical bone height and bone density were analyzed by Kmeans cluster analysis.Results According to the cluster analysis results,the 10 sites were classified into 3 clusters.There were statistical differences among these three clusters in bone height and bone density(P<0.05).The ISD result showed that the greatest mean value of vertical bone height was obtained in cluster 2,followed by cluster 1 and cluster 3;the highest bone density was founded in cluster 3,followed by cluster 1 and cluster 2.Conclusions Evaluation of the sites for palatal implant placement with cone beam CT would be helpful in safe and stable implantation.  相似文献

磨牙纵裂是临床常见病,通常是由于死髓牙脆性增加,咀嚼压力过大,牙面有高陡牙尖等原因造成。以往磨牙纵裂被认为是拔牙的适应证,随着牙齿治疗手段的提高,对纵裂磨牙的保存治疗逐渐受到重视。为了探讨磨牙纵裂的原因、治疗和预防的方法,笔者对126颗纵裂磨牙采用外结扎以及双鸠尾内固定,并作根管治疗和全冠修复治疗。经6个月至2年的临床随访观察,治疗效果满意。现报道  相似文献

目的探讨前牙牙冠延长术后采用氧化锆全瓷和金属烤瓷(porcelainfusedtometal,PFM)冠修复对牙周的影响。方法选择2009--2011年惠州市口腔医院修复科收治的拟行牙冠延长术+冠修复患者18例(共108颗上颌前牙),分成两组(全瓷组和PFM组),每组54颗,均行牙冠延长术;术后6个月,全瓷组行氧化锆全瓷冠修复,PFM组行钴铬合金烤瓷冠修复。随访8~32个月,评价两组冠修复后的牙周状况。结果PFM组有34颗(62.96%)修复后出现牙周病损,其中轻度病损25颗,中度病损5颗,重度病损4颗;而全瓷组仅2颗(3.70%)发生轻度牙周病损。经Ridit分析,两组间差异有统计学意义(P〈0.05)。结论前牙牙冠延长术后采用氧化锆全瓷冠修复的牙周状况明显优于PFM冠修复。  相似文献

目的研究Vitapex糊剂用于深龋露髓年轻恒磨牙部分活髓切断术的临床效果。方法选择2004年7月至2007年9月于大连市口腔医院儿童口腔病科就诊的年轻恒磨牙深龋露髓患儿58例(61颗患牙),行部分活髓切断术,牙髓断面分别覆盖Vitapex(试验组)或氢氧化钙(对照组)糊剂,定期复查并比较疗效。结果试验组成功率为96.9%,对照组成功率为89.7%,组间差异无统计学意义(P>0.05)。结论部分活髓切断术是治疗深龋露髓年轻恒磨牙的有效方法,Vitapex是可供选用的较为理想的活髓保存剂。  相似文献

目的    探讨烤瓷冠和可摘局部义齿联合修复在牙列缺损伴重度磨耗咬合重建中的应用。方法    选择2006—2009年中共辽宁省委机关门诊部口腔科就诊的牙列缺损伴重度磨耗患者24例,对其进行咬合重建后采用烤瓷冠和可摘局部义齿联合修复,随访6~36个月,观察疗效。结果    修复后临床效果的主观评价为美观24例、咀嚼有力24例、感觉舒适22例。修复后基牙均无根尖病变、叩痛及松动等现象。1例患者出现义齿塑料部分损坏,其余均未见异常。修复前牙周膜增宽的基牙,修复后牙周膜宽度基本恢复正常,牙槽嵴高度无明显改变。结论    牙列缺损伴重度磨耗患者进行咬合重建后,应用烤瓷冠和可摘局部义齿联合修复方法可得到满意的修复效果。  相似文献

目的评价Vitapex糊剂与氢氧化钙根管充填糊剂用于根尖诱导成形术的临床疗效。方法选择2007年1月至2008年10月在沈阳市沈河区牙病防治所儿科门诊就诊的60例患者共82颗根尖孔发育不全的患牙为研究对象,按照就诊次序分为两组。根管预备及消毒后,分别用Vitapex糊剂和氢氧化钙根管充填糊剂行根尖诱导成形术,术后定期复查,待根尖形成或根端闭合后行常规根管充填。结果治疗2年后,Vitapex糊剂组40颗患牙成功31例,改善7例,失败2例,总有效率为95%;氢氧化钙根充糊剂组42颗患牙成功17例,改善13例,失败12例,总有效率为71.4%,两组治疗有效率比较,差异有统计学意义(P<0.05)。结论 Vitapex糊剂用于根尖诱导成形术具有良好的治疗效果,值得临床推广。  相似文献
