Sixteen patients underwent coronary revascularization with bilateral internal thoracic artery (ITA) grafts between 1988 and 1989 at the Okayama University Hospital. A total 39 coronary grafts were performed, being an average of 2.4 grafts per patient. Each patient received bilateral ITA grafts, and in 5 patients an additional 7 grafts were constructed with 5 autologous veins and 2 gastroepiloic arteries. The right ITA was grafted as a free graft in 4 patients. The ITA graft patency rate was 96.8 per cent (31/32) at the time of hospital discharge. The postoperative morbidity included one reoperation for bleeding and one myocardial infarction. Coronary artery bypass grafting with bilateral ITA grafts can be safely performed and its application facilitates complete revascularization with arterial grafts. 相似文献

From April 1996 to July 1999, 241 consecutive patients underwent complete arterial revascularization with composite T-graft, including right coronary artery grafting with free right internal thoracic artery (ITA) (ITA group). They were compared with 127 bilateral ITA patients in whom saphenous vein grafts (SVG) was used for grafting the right coronary system (SVG group). The SVG group included more diabetics (40 vs. 29%), more emergency cases (21 vs. 12.4%), and the number of anastomoses per patient was higher (3.8 vs. 3.35, P=0.025). Thirty-day mortality was 3.9 and 4.1% in the SVG and the ITA groups, respectively (P=NS). Occurrence of perioperative complications (sternal infection, myocardial infarction, cerebrovascular accident, and bleeding) was not statistically significant. However, in sum, the complications rate was higher in the ITA group (8.3 vs. 2.4%, P=0.032). Midterm followup (2-56 months) showed increased return of angina in the ITA group (9.1 vs. 1.6%, P=0.00). However, 4-year survival (Kaplan-Meier) was comparable (91.7% in the SVG and 87% in the ITA group). In conclusion, early results of complete arterial revascularization with composite T-graft are similar to those of bilateral ITA grafting of the left and right system revascularization with SVG. However, lower return of angina in the SVG group makes SVG grafting preferable for the right coronary system. 相似文献

BACKGROUND: The internal thoracic artery is widely recognized as the ideal graft for coronary artery bypass procedures. However, because of the inadequate length of the conduit, use of bilateral internal thoracic artery grafting was not suitable for complete revascularization. To overcome this limitation, the T graft was introduced in the 1990s. We decided to prospectively assess the safety of this technique. METHODS: One hundred six patients with a mean age of 51.5 years underwent complete revascularization with an internal thoracic artery T graft. Mean left ventricular ejection fraction was 0.60 (range, 0.22 to 0.85). RESULTS: No patient required reexploration for bleeding, and no patient died within 30 days after operation. On the basis of electrocardiographic changes, 3 patients sustained a perioperative myocardial infarction. One patient had a sternal wound infection. Mean follow-up was 35 months (range, 15 to 61 months). The actuarial survival rate was 99% +/- 1% at 5 years. No myocardial infarctions were reported during the follow-up. Seven patients had recurrent angina. Eighty patients (76%) underwent postoperative stress tests, and 90% had negative results. CONCLUSIONS: Complete myocardial revascularization with the T graft is a safe and reliable technique with excellent midterm results. 相似文献

Coronary revascularization that is neurologically uneventful in patients with bilateral totally occluded internal carotid arteries has not been previously reported. We performed saphenous vein coronary artery bypass grafting on three such patients and observed them for 6 to 23 months. Preoperatively two of our patients had chronic stable symptoms of cerebrovascular insufficiency, and one had received cerebral revascularization via a superficial temporal-to-middle cerebral artery bypass. Controversy exists regarding proper cerebral protective maneuvers during coronary revascularization for patients with advanced cerebrovascular disease. Cerebral protection for our patients during cardiopulmonary bypass included hypothermia and high perfusion flows and pressures. Two patients also received prophylactic sodium thiopental. None of these three patients had a stroke perioperatively or during the follow-up period. We believe that these case histories strongly suggest that the functional state of the cerebral collateral circulation, as judged by preoperative neurological symptoms, predicts neurological outcome after coronary revascularization better than the specific occlusive anatomy of the extracranial carotid arteries. 相似文献

Left main occlusive disease (LMD) is a potentially fatal lesion which is optimally treated with surgical revascularization. Although the internal thoracic artery (ITA) is recognized as having superior long term patency, there has been concern regarding possible flow limitation. Because of this concern, there may be reluctance to use only this conduit in patients with LMD in whom high graft flows are desirable. From 1985 to 1990, 45 patients (38 males, 7 females) with LMD ranging in age from 37 to 75 years (mean 55.9 +/- 8.7) underwent revascularization using bilateral ITA grafts placed to the left anterior descending and circumflex arteries. The right ITA was used as a free graft in 19 of 45 (42%) patients and the left ITA was used as a free graft in 3 of 35 (7%). No saphenous vein grafts were placed to the left coronary system in any patient. Over half of these patients (24 patients, 53%) also had occlusive disease in the right coronary artery. A saphenous vein graft was placed to the right coronary artery in 22 of 45 (49%) patients. Ventricular function in this patient subset was good (mean LV score 7.1 +/- 2.1). Intra-operative ITA graft flows were 49.7 +/- 29.1 ml/min for grafts to the left anterior descending and 45.5 +/- 31.7 ml/min for circumflex grafts. There were no perioperative deaths. Morbidity included myocardial infarction, stroke and reoperation for bleeding in 1 patient each (2.2%). Low cardiac output occurred in 2 patients (4.4%). In an effort to develop a noninvasive method to evaluate flow characteristics of the internal thoracic artery grafts (ITAG) after coronary artery bypass grafting, we performed duplex scanning of ITAGs of 51 patients who underwent bypass grafting. The ITAG was visualized with a duplex scanner of 7.5 MHz through the first or second left intercostal space. The visualization of the ITAG was adequate to make reliable measurements in 47 patients (92.2%). The diameter of the vessel, systolic peak velocity, and diastolic peak velocity were recorded, and systolic flow volume, diastolic flow volume, velocity ratio, flow volume ratio, and diastolic flow volume fraction were calculated. The velocity ratio, flow volume ratio, and diastolic flow volume fraction were markedly higher in the unstenotic subjects than in the stenotic subjects. In the group in which severe LAD stenosis were recognized preoperatively, both systolic and diastolic flow volumes were increased compared with moderately stenotic group. No differences in flow characteristics could be demonstrated between the subjects with old anterior myocardial infarction and without it. In 10 patients in whom flow pattern was abnormal or not identified, angiography revealed graft stenosis or predominant native coronary arterial flow. A 12 1/2-year experience with 227 patients who underwent coronary revascularization with bilateral internal mammary artery (IMA) and supplemental saphenous vein grafts is presented. There were 725 coronary grafts performed (3.2 per patient). Forty-four IMA grafts were sequential. In 42 patients, the right IMA was placed through the transverse sinus to revascularize the circumflex artery. Operative mortality was 4% (9 of 227 patients). Postoperative complications included sternal infection (4 patients), reexploration for bleeding (5), and diaphragmatic dysfunction (10). Postoperative arteriography in 30 patients (mean interval, 4 1/4 years) revealed that 92% (34/37) of the left IMA and 87% (26/30) of the right IMA grafts were patent. Late follow-up was complete in 207 surviving patients. Eighteen late deaths (9 cardiac and 9 noncardiac) occurred. Actuarial survival was 83% at 10 years after operation. Of 68 patients followed from 8 to 12 1/2 years (mean interval, 10 1/2 years), 69% were asymptomatic, and 28% were in New York Heart Association Functional Class II. We conclude that bilateral IMA grafting can be accomplished with low operative risk and provides excellent long-term results. 相似文献

The internal mammary artery (IMA) is being recognized as the conduit of choice for myocardial revascularization. From January 1972 through June 1988, 1,087 patients received bilateral IMA and supplemental vein grafts. There were 917 men (84.4%) and 170 women (15.6%) with a mean age of 62.4 years (range, 29 to 84 years). Three hundred ninety-four patients (36.2%) had unstable angina, and 194 (17.8%) had left main coronary artery stenosis greater than 50%. In all, 3,741 coronary grafts were performed, with a mean of 3.4 per patient. Hospital mortality was 2.7% (29 patients). Hospital complications included reoperation for bleeding, 19 patients (1.7%); sternal infection, 16 patients (1.5%); respiratory failure, 35 patients (3.2%); perioperative myocardial infarction, 22 patients (2.0%); and stroke, 20 patients (1.8%). Post-operative arteriography in 53 patients (mean postoperative time, 53.0 months) showed that 92.1% (58/63) of the left IMA and 84.9% (45/53) of the right IMA grafts were patent. Follow-up was completed on 1,058 hospital survivors. There were 82 late deaths (7.8%). The actuarial survival for patients discharged from the hospital was 80.0 +/- 3.2% (plus or minus standard error of the mean) at 10 years and 60.0% +/- 5.0% at 15 years. At follow-up, 866 patients (90.3%) were asymptomatic and in New York Heart Association class I and 68 (7.1%) were in class II. This longitudinal analysis demonstrates that bilateral IMA grafting has a low operative risk and provides excellent long-term functional improvement and survival. 相似文献

Background. Bilateral internal thoracic artery (ITA) harvesting is significantly underused, whereas the radial artery is being used with increasing frequency. We have retrospectively analyzed perioperative and short-term outcomes of patients receiving a radial artery versus those receiving a right ITA as a second arterial graft. Methods. Between February 1999 and May 2000, 250 patients underwent coronary artery bypass grafting using the radial artery (156 patients) or the right ITA (94 patients) in combination with the left ITA and, when required, the saphenous vein. Results. There was a higher prevalence of risk factors in the radial artery group. More coronary artery bypass graftings (p < 0.001) were performed with the radial artery. Operative mortality was not different (p = not significant). In the right ITA group there was more bleeding (p < 0.001) and a longer hospital stay (p < 0.001). Mean follow-up was 8.1 ± 3.9 months. The probability of survival was similar (p = not significant). Conclusions. The radial artery can extend the benefits of multiple arterial grafting to those patients who are usually excluded from bilateral ITA harvesting because of multiple risk factors. Bypass grafts from the internal mammary artery (IMA) to the coronary artery, performed with small suture and optical magnification, have been found to have an extremely high late patency rate when compared with saphenous vein grafts. This is thought to be due to the use of autogenous artery instead of vein, the closer approximation in size between graft and artery, the fact that one anastomosis instead of two is necessary, and because microsuture with magnification produces a smoother anastomosis. This has led us to explore the use of the splenic artery for coronary bypass to the distal right coronary artery on the diaphragmatic surface of the heart.Three patients have had a right coronary bypass using splenic artery brought through the membranous portion of the diaphragm. Two of these patients also had an IMA-to-left anterior descending coronary artery bypass. Thus, autogenous artery bypass to the two most frequently obstructed coronary branches has been shown to be practical. 相似文献

OBJECTIVE: By using a T-graft configuration, the myocardium may be completely revascularized with bilateral internal thoracic arteries. This study aimed to evaluate the perioperative morbidity and mortality in a single surgeon's early experience with a modified T-graft using bilateral internal thoracic arteries. METHODS: Between October 1994 to April 1997, 200 consecutive patients mostly selected per protocol, received a T-graft with bilateral internal thoracic arteries for stable angina pectoris (n = 157) or unstable angina pectoris (n = 43). The mean age of patients was 56 years (range of 36 to 78 years). There were 171 males and 29 females. Forty-three patients had diabetes. Concomitant procedures were performed in 8 patients. RESULTS: In 190 patients (95%), total arterial revascularization of the myocardium was achieved solely by the use of bilateral internal thoracic arteries in a T-graft configuration and the number of anastomoses per patient averaged 4.2. Ten patients (5%) received supplemental saphenous veins in addition to T-grafts for low cardiac output (n = 3), intraoperative regional ischaemia (n = 2), postoperative myocardial ischaemia (n = 2) and inadequate conduits (n = 3). The 30-day mortality was 0.5%. Perioperative myocardial infarct occurred in 2 patients (1.0%). Reasons encountered for early re-operation included bleeding (n = 7), sternal dehiscence (n = 5), suppurative sternitis (n = 3) and myocardial ischaemia (n = 2). Twelve patients received inotropes and intraaortic balloon counterpulsation was employed in 3 patients. CONCLUSION: When bilateral internal thoracic arteries were used in a T-graft configuration, total arterial revascularization of the myocardium was achieved with an acceptably low morbidity and mortality. 相似文献

BackgroundA new technique has been developed that permits complete arterial revascularization of the lateral and/or inferior wall of the heart using in situ bilateral internal thoracic artery grafts in awake patients. This technique, without cardiopulmonary bypass and mechanical ventilation, creates the least invasive revascularization method for the lateral and/or posterior wall of the heart yet described. MethodsIn 7 patients double or triple vessel coronary artery bypass grafting was performed without general anesthesia. A high thoracic epidural anesthesia was started one hour before surgery. Bilateral internal thoracic arteries were harvested and all anastomoses were performed with the off-pump technique by standard median sternotomy. Circumflex, or the right coronary artery, were anastomosed with bilateral internal thoracic arteries using a heart positioner. Six patients received double bypass grafting and one patient received triple bypass grafts (bilateral internal thoracic arteries and one radial artery). ResultsAll patients remained awake throughout the whole procedure. There was no perioperative myocardial infarction or mortality. Pneumothorax was observed in three patients, but it was repaired in two. Only one patient completed the procedure with unilateral pneumothorax. There were no hemodynamic and pulmonary problems during lateral or posterior wall revascularization. Two patients required unexpected coronary endarterectomy during circumflex and right coronary artery anastomoses. ConclusionsComplete arterial revascularization by median sternotomy using in situ bilateral internal thoracic artery grafts without general anesthesia is a feasible and safe procedure for multivessel disease. Although increasing use is being made of arterial grafts (internal thoracic arteries and right gastroepiploic artery) for coronary revascularization, application to left main coronary artery (LMT) patients is frequently not possible. During the period from December 1989 to July 1991, coronary revascularization was conducted on 9 LMT patients using only arterial grafts and no venous grafts. The bypass grafts were 6 left internal thoracic artery grafts, 9 right internal thoracic artery grafts and 9 right gastroepiploic artery grafts, a total of 24 grafts and an average of 2.7 bypasses per patient. There were no operated deaths, but five patients required IABP support after cardiopulmonary bypass. They had more than 90% stenotic lesions of left main coronary artery. In contrast, four patients with less than 90% stenotic lesion were uneventful. The cause of these catastrophic hemodynamics was considered reduced blood flow by graft spasm. All patients could be functionally placed in New York Heart Association Class I or II. Postoperative stress tests were made on eight patients and the results were normal in seven. Eight patients have had postoperative angiograms. Twenty-one of 22 grafts were patent. The present results demonstrate that an arterial bypass is possible even on LMT patients by IABP support. 相似文献
