目的 了解口腔扁平苔藓(OLP)患者外周血中辅助性T细胞17(Th17)与调节性T细胞(Treg)的平衡变化,探讨它们在OLP发病机制中的作用及意义。方法 选取17例正常组和33例OLP患者(网纹型15例,糜烂型18例)的外周血,应用流式细胞术(FCM)检测Th17、Treg细胞的表达水平,实时荧光定量聚合酶链反应(qPCR)检测它们的转录因子维甲酸相关孤核受体γt(RORγt)和叉头状转录因子3(Foxp3)mRNA的表达。结果 OLP外周血中Th17、Treg细胞及RORγt、Foxp3 mRNA表达均升高(P<0.05),但Treg细胞和Foxp3 mRNA表达在OLP两型间差异无统计学意义。Th17/Treg比值在OLP中升高(P<0.05),其中糜烂型OLP显著高于正常组及网纹型OLP(P<0.01),但网纹型OLP与正常组相比差异无统计学意义。Spearman相关分析显示Th17细胞和Th17/Treg比值与体征计分、RAE计分存在正相关关系(r=0.66,P=0.00;r=0.66,P=0.00;r=0.52,P=0.00;r=0.50,P=0.00);同时Th17细胞与Treg细胞也存在正相关关系(r=0.39,P=0.03)。结论 OLP外周血中Th17和Treg细胞以及它们的比例均增高,Th17/Treg失衡在糜烂型OLP的发病过程中起了一定作用。  相似文献

目的探讨外源性白细胞介素(interleukin,IL)35对口腔扁平苔藓(oral lichen planus,OLP)患者外周血中辅助性T细胞17(helper T cell 17,Th17)与调节性T细胞(regulatory T cell,Treg)平衡的影响。方法选取2016年10至12月就诊于贵州医科大学附属医院口腔内科黏膜专科门诊的12例OLP患者外周血(OLP组)(男性1例,女性11例,26~68岁;其中非糜烂型OLP4例,糜烂型OLP 8例),同期收集贵州医科大学附属医院体检中心的13名健康人外周血(健康对照组)(男性1名,女性12名,20~68岁),无菌提取两组外周血单个核细胞,流式细胞术(flow cytometry,FCM)分选外周血CD4+T细胞,采用实时荧光定量PCR(quantitative real-time PCR,qPCR)检测两组外周血CD4+T细胞中Th17、Treg细胞特异转录因子维甲酸相关孤核受体γt(retinoic acid receptor-related orphan receptorγt,RORγt)、叉头状转录因子(forkhead box3,Foxp3)mRNA表达水平;将OLP患者外周血分选出的CD4+T细胞分为实验组与对照组,实验组加入重组人IL-35蛋白(recombinant human IL-35,rhIL-35),对照组加入等体积磷酸盐缓冲液,分别进行细胞体外培养,收集培养结束后的细胞,qPCR检测上述因子的表达水平。结果OLP组CD4+T细胞中Foxp3、RORγt mRNA的相对表达量[M(Q25,Q75)分别为0.15(0.09,0.30)和1.04(0.45,2.15)]均显著大于健康对照组[分别为0.04(0.02,0.06)和0.10(0.05,0.11)](Z=-4.134,P<0.01;Z=-3.699,P<0.01)。OLP组RORγt/Foxp3 mRNA比值[6.22(3.67,15.34)]显著大于健康对照组[2.50(1.24,5.23)](Z=-2.665,P=0.007)。OLP患者外周血实验组CD4+T细胞Foxp3 mRNA相对表达量[0.40(0.21,1.22)]显著大于对照组[0.15(0.11,0.26)](Z=-2.510,P=0.012),两组RORγt mRNA表达差异无统计学意义(P>0.05),实验组RORγt/Foxp3 mRNA比值[3.44(1.55,8.16)]显著小于对照组[6.22(4.43,12.21)](Z=-2.746,P=0.006)。结论OLP患者外周血中存在Th17细胞占优势的Th17/Treg平衡异常,外源性IL-35可通过促进Treg细胞扩增,实现对OLP患者外周血中Th17/Treg平衡的调节。  相似文献

本文应用免疫组织化学技术对口腔扁平苔藓病损中的郎格罕氏细胞数量,分布情况及与T细胞的关系进行了研究,结果显示口腔扁平病损中郎格罕氏细胞周围有HLA-DR阳性的T细胞及角朊细胞,并且发现郎格罕氏细胞与T细胞有接触关系。作者认为郎格罕氏细胞与T细胞之间的密切接触在递呈抗原及启动局部免疫反应过程中起重要作用。  相似文献

目的:研究口腔扁平苔藓(oral lichen planus,OLP)患者损害组织中浸润的T细胞受体(T cell receptor,TCR)BV基因的取用格局。方法:采用实时定量聚合酶链反应,分析OLP患者损害组织中浸润的T细胞和外周血中T细胞TCR BV基因的取用格局。采用GraphPad Prism version 5.00软件对数据进行Mann-Whitney U检验。结果:43例OLP患者的损害组织对BV4、BV14和BV18基因的取用显著高于39例OLP患者外周血对这些BV基因的取用。结论:OLP患者损害组织浸润的T细胞优势取用BV4、BV14和BV18基因,提示带有BV4、BV14和BV18基因的T细胞参与了OLP的发病。  相似文献

目的:研究口腔扁平苔藓(OLP)患者外周血调节性T细胞(Treg)水平的改变.方法:收集口腔扁平苔藓(普通型)患者和健康人外周血.采用流式细胞术分析其中CD4+ CD25+T细胞、CD4+ CD25highT细胞、CD4+ Foxp3+ T细胞、CD4+CD25+ Foxp3+ T细胞比例的改变.结果:OLP患者外周血中CD4+ CD25high T 细胞、CD4+ Foxp3+ T细胞水平较正常对照组明显升高(P<0.05);CD4+ CD25hight细胞与CD4+ Foxp3+ T细胞水平存在正相关性(r=0.349,P<0.05),CD4+ CD25+ Foxp3+ T细胞与CD4+ Foxp3+ T细胞水平存在明显正相关性(r:0.802,P<0.01).结论:OLP患者外周血调节性T细胞水平发生了改变.  相似文献

目的:通过观察调节性T细胞(regulatory Tcells,Treg)和Thl7细胞(helper Tcells17,Th17)在口腔扁平苔藓(OLP)组织中的表达情况,探讨其在OLP发生发展过程中的变化。方法:应用免疫组织化学双标记技术检测35例OLP患者和19例对照组口腔黏膜组织中CD25+Foxp3+、CD4+IL-17+细胞的表达。结果:OLP组病损组织中有大量CD25+Foxp3+细胞浸润,较对照组显著增多,两者差异非常显著(P〈0.01)。CD4+IL-17+细胞在OLP组织中的表达较正常口腔黏膜组织有增加趋势,但差异无统计学意义(P〉0.05)。经进一步统计学分析,CD25+Foxp3+细胞和CD4+IL-17+细胞数量改变在OLP组织中表达呈正相关(P〈0.05)。结论:Treg细胞的数量在OLP组织中增多,Thl7细胞可能协同Treg细胞,在OLP发生发展过程中发挥了一定的作用。  相似文献

Objective: Oral lichen planus (OLP) and lichenoid lesions (OLL) are regarded as precursor lesions of oral squamous cell carcinoma (OSCC) with potential for malignant transformation. This potential is not clear due to difficulties in diagnosis of OLP and OLL. Our aim was therefore to evaluate previously identified OLP and OLL as precursor lesions in OSCC and to identify cancer related etiological factors such as smoking and alcohol consumption.

Material and methods: We retrospectively reviewed all cases (total 323, comprising 164 females and 159 males) with OSCC treated at the Department of Oral and Maxillofacial Diseases and Surgery, Helsinki University Hospital during 2015. Confirmed by histopathological biopsy, 58 (17.9%) had OLP and 13 had OLL (4.0%) as precursor lesion.

Results: Patients with OLP were slightly older than those without it. OLP was more common in females than in males (p?p?=?.006) and cancer relapses less common (p?=?.005). Smoking was less frequent in patients with OLP and OLL (p?p?Conclusion: Our findings confirm the importance of active follow-up of all patients with OLP and OLL even in patients who do not fit a traditional high-risk category for OSCC.  相似文献

Oral lichen planus (OLP) is one of the most common chronic inflammatory oral mucosal diseases with T‐cell‐mediated immune pathogenesis. In subepithelial and lamina propria of OLP local lesions, the presence of CD4+ T helper (CD4+ Th) cells appeared as the major lymphocytes. These CD4+ T lymphocytes can differentiate into distinct Th cell types such as Th1, Th2, Treg, Th17, Th22, Th9, and Tfh within the context of certain cytokines environment. Growing evidence indicated that Th1/Th2 imbalance may greatly participate into the cytokine network of OLP immunopathology. In addition, Th1/Th2 imbalance can be regulated by the Treg subset and also greatly influenced by the emerging novel CD4+ Th subset Th17. Furthermore, the presence of novel subsets Th22, Th9 and Tfh in OLP patients is yet to be clarified. All these Th subsets and their specific cytokines may play a critical role in determining the character, extent and duration of immune responses in OLP pathogenesis. Therefore, we review the roles of distinct CD4+ Th subsets and their signature cytokines in determining disease severity and susceptibility of OLP and also reveal the novel therapeutic strategies based on T lymphocytes subsets in OLP treatment.  相似文献
