In patients with subluxation of the patella, injury of the patellar articular cartilage is frequently observed and correct evaluation is important to manage these patients. We examined 11 patients with subluxation of the patella and five normal volunteers. In 12 patellofemoral joints of seven patients with subluxation of the patella, the abnormalities observed on MRI were compared with those on arthroscopy and/or at operation. MRI was performed with a Magnetom 1.5 T (Siemens) using the round surface coil. Pulse sequences were SE (TR 400 ms/TE 19 ms), FLASH (TR 320 ms/TE 15 ms FA 90 degrees and 40 degrees), and SE (TR 2000 ms/TE 26, 70 ms). We analysed MR findings of the 12 abnormal joints and 10 normal joints according to the following classification of abnormalities observed on arthroscopy. (1) normal appearance (n = 3 joints), (2) softening and fibrillation (n = 6), (3) fragmentation (n = 3), and (4) erosion to bone (n = 0). In only one of the six cases with softening and fibrillation observed on arthroscopy, MRI could visualize the thickening of patellar articular cartilage, but in all three cases with fragmentation observed on arthroscopy, MRI could visualize the thin inhomogenous cartilage with irregular surface. The combination of SE (TR 400 ms/TE 19 ms) and FLASH (TR 320 ms/TE 15 ms FA 90 degrees) are extremely effective pulse sequence to detect the abnormalities of patellar articular cartilage. We conclude that MRI is a useful noninvasive method of detecting advanced changes in patellar articular cartilage.  相似文献

Acute injury of the ligaments of the knee: magnetic resonance evaluation   总被引:5,自引:0,他引:5  
Eleven acutely injured knees and 13 normal knees were examined by magnetic resonance imaging (MRI) to assess the value of this modality in detecting acute ligamentous injury of the knee. The presence of torn ligaments in the injured knees was determined by arthroscopy and/or arthrotomy in ten cases and clinical follow-up in one case. The anterior and posterior cruciate ligaments (ACL and PCL) were demonstrated by sagittal spin echo (SE) images through the intercondylar notch (TE = 30 ms; TR = 2,000 ms). The tibial and fibular collateral ligaments (TCL and FCL) were evaluated on coronal SE images (TE = 30 ms, TR = 200 or 530 ms; TE = 120 ms, TR = 2,000 or 2,120 ms). The ACL and PCL were considered torn on MR if they appeared disrupted or were not seen in their normal anatomical positions. The collateral ligaments were considered torn if abnormal high-intensity signal was noted in adjacent soft tissues on TE = 120 ms images or if disruption of a ligament was apparent. Eleven of 15 torn ligaments and 80 of 81 normal ligaments were correctly identified by these criteria. It is concluded that MR imaging may be useful in detecting acute injury of ligaments of the knee.  相似文献

This paper deals with a study to obtain the optimal sequence of gradient echo (GE) for T1- and T2*-weighted images similar to T1- and T2-weighted images of spin echo (SE). Two GE sequences, fast low angle shot (FLASH) and fast imaging with steady-state precession (FISP), were performed in 15 cases of liver metastasis in various combination of flip angle (FA), repetition time (TR), and echo time (TE). The optimal combinations were summarized as follows: 1) T1-weighted FLASH image with FA of 40 degrees, TR of 22 msec and TE of 10 msec, 2) T1-weighted FISP image with FA of 70 degrees, TR of 100 msec, TE of 10 msec, 3) both T2*-weighted FLASH and FISP images with FA of 10 degrees, TR of 100 msec and TE of 30 msec. Not only to provide the adequate T1- and T2*-weighted images but also to enable breath-holding MR imaging, GE sequences can optionally take place SE in cases of deteriorated images caused by moving artifacts. Other applications support the re-examination and further detailing when required, conveniently rather in short time.  相似文献

Owing to the intrinsically complex behavior of the signal intensity of fast gradient-refocusing MR sequences, agreement as to the clinically most useful sequence parameters has not yet been reached. This study evaluates the FLASH (fast low-angle shot) sequence for gray-white matter differentiation on normal volunteers at 1.5 T. The FLASH gradient-echo sequence is essentially T1-dependent. For very fast imaging and T1 weighting, the following parameters yield the best results: a flip angle of 30-50 degrees with TR = 20 and TE = 10. To replace T1-weighted SE by the faster FLASH sequence, the best results are achieved by a flip angle of 70-120 degrees with TR = 150-300 and TE = 10 (or shorter, if possible). The most valuable proton-density aspect is achieved by a flip angle of 30 degrees with TR = 300 and TE = 16.  相似文献

Purpose: To evaluate fat-suppressed (FS) proton-density-weighted (PDw) turbo spin-echo (TSE) magnetic resonance imaging (MRI) compared to arthroscopy in the detection of meniscal lesions.

Material and Methods: In a prospective study, 31 knee joints were imaged on a 1.5T MR scanner before arthroscopy using the following sequences: (a) coronal and sagittal FS-PDw TSE (TR/TE: 4009/15 ms); (b) coronal T1w SE (TR/TE: 722/20 ms), and sagittal PDw TSE (TR/TE: 3800/15 ms). Other imaging parameters were: slice thickness 3 mm, FOV 160 mm, matrix 256×256. A total of 186 meniscal regions (62 menisci; anterior horn, body, posterior horn) were evaluated. Standard of reference was arthroscopy. Sensitivity, specificity, negative predictive value (npv), positive predictive value (ppv), and accuracy were calculated.

Results: Arthroscopically, meniscal lesions were detected in 55/186 segments (35 medial and 20 lateral meniscal lesions). Sensitivity, specificity, npv, ppv, and accuracy for combination of coronal and sagittal FS PDw TSE were 91.4%, 98.3%, 95%, 97%, and 93.5% for the medial meniscus, and 90%, 98.6%, 97.3%, 94.7%, and 96.8% for the lateral. The results were comparable to the combination of coronal T1w SE and sagittal PDw TSE for the medial (88.6%, 98.3%, 93.4%, 96.9%, 91.4%) and the lateral (90%, 95.9%, 97.2%, 85.7%, 92.5%) meniscus.

Conclusion: FS PDw TSE-MR sequences are an excellent alternative for the detection of meniscal lesions in comparison with diagnostic arthroscopy.  相似文献

Our objective was to compare a water-excitation (WE) 3D fast low-angle shot (FLASH) MR sequence for faster imaging of articular cartilage defects of the knee to a conventional fat-saturated (FS) 3D FLASH MR sequence. This prospective study included 16 knees of 16 patients with suspected cartilage lesions. The MR imaging in transverse and sagittal planes included (a) FS 3D FLASH (TR/TE: 45 ms/11 ms, scan time 8 min, flip angle 50°), and (b) WE 3D FLASH (TR/TE: 28 ms/11 ms, scan time 4 min 58 s, flip angle 40°). For each sequence signal-to-noise ratios (SNR) and contrast-to-noise ratios (CNR) were quantified. The detected cartilage lesions were evaluated using a semi-quantitative four-scale scoring system (grades 0–III). The data were compared between the sequences using the paired Student's t-test. No statistically significant differences between the sequences were found for SNR, CNR, and cartilage defect grading (p=0.14–0.8). The WE 3D FLASH MR imaging seems to be promising for fast imaging of articular cartilage lesions of the knee. Electronic Publication  相似文献

PURPOSE: To describe the magnetic resonance imaging (MRI) signs of technically successful osteochondral plug transfer and to correlate the findings with histology using the Mankin score. MATERIAL AND METHODS: The study was done in a prospective animal experiment: 11 adult black-head sheep underwent surgical treatment with osteochondral plug transfer of a knee joint. The animals were killed 6 months later and MRI of the joints was done immediately. MRI was applied with a 1.5T MR scanner using a spin-echo (SE) T1-weighted, turbo spin-echo (TSE) T2-weighted with spectral fat suppression and a fat-suppressed 3D-spoiled gradient echo (GRE) sequence (manufacturer's acronym: FLASH) (TR 50.0 ms, TE 11.0 ms, flip 35 degrees). After MRI, all knee joints were dissected and a biopsy of the plug and the adjacent cartilage was taken. Classification of the cartilage biopsies was carried out in accordance with a modified Mankin score. RESULTS: Cartilage repairs with a hypointense cartilage signal in the FLASH 3D sequence were correlated with poor histological results (lower Mankin score). Histologically, the regions of cartilage with a hypointense signal showed a fibrocartilage-like repair tissue. Hyaline cartilage with well-defined layers had the same signal intensity in the FLASH sequence relative to adjacent hyaline cartilage. There were two plugs with a surface defect, graded as Outerbridge grade 1 in MRI and histology. Both had a poor outcome in the histologic Mankin score. Grade 2-4 lesions were not observed in the MRI study nor in the histologic study. CONCLUSION: MRI is a useful non-invasive tool for evaluating the morphologic status of osteochondral plug transfers. A good postoperative result of the cartilage repair was found histologically if an isointense cartilage signal of the graft was documented in the FLASH 3D sequence, and the graft had good congruity with the articular surface without defects.  相似文献

PURPOSE: To compare the accuracy of five T2-weighted sequences in the detection of liver lesion at magnetic resonance (MR) imaging after superparamagnetic iron oxide (SPIO) enhancement. MATERIALS AND METHODS: Forty-nine candidates for hepatic resection with known coloretal metastases were examined. Before SPIO enhancement, fast spin-echo (SE) images were obtained. After enhancement, the same fast SE sequence and long; TR/short TE, short TE, long TR/TE, and T2-weighted fast low-angle shot (FLASH) sequences were used. All images were viewed independently by four observers who were blinded to the results of the other imaging sequences, the results of the other observers, and the findings at surgery and histopathologic examination. Four weeks after the initial reading, the combined long TR/short TE and long TR/TE dual-echo images were also viewed as an additional set. The alternative free response receiver operating characteristic (ROC) method was used to analyze the results, which were correlated with findings at surgery, intraoperative ultrasonography, and histopathologic examination. RESULTS: Irrespective of lesion size, the accuracy of all sequences after enhancement was significantly greater than that of the nonenhanced fast SE sequence (P < .01). Dual-echo and FLASH sequences were significantly more accurate than the enhanced fast SE sequence (P < .03 or P < .02, respectively). For all lesions, lesions smaller than 1 cm, and lesions 1 cm or larger, mean accuracies were as follows: dual-echo, 0.75, 0.54, and 0.93; FLASH, 0.75, 0.54, and 0.95; and enhanced fast SE, 0.72, 0.49, and 0.92. CONCLUSION: At 1.0 T, dual-echo and FLASH sequences are the most accurate pulse sequences after SPIO enhancement.  相似文献

RATIONALE AND OBJECTIVES: The purpose of this study was to evaluate the utility of magnetic resonance (MR) imaging for indicating the extent of true tissue necrosis of the liver after radiofrequency (RF) ablation in comparison with histopathologic findings in dog models and an autopsy case. MATERIALS AND METHODS: RF ablation of the liver parenchyma was performed on three dogs under general anesthesia. MR appearances of the RF-ablated regions on T1-weighted fast-low angle shot (FLASH; repetition time/echo time [TR/TE]/flip angle: 120/3.8/70),T2-weighted turbo spin echo (3000/80/echo train = 25) and contrast-enhanced T1-weighted images were compared with histopathologic findings. An autopsy case with hepatocellular carcinoma was also enrolled in this study. RESULTS: All ablated regions showed three zones on T1-weighted FLASH images: a central zone with low intensity, a broad hyperintense middle zone, and a surrounding hypointense band. The central and middle zones corresponded to the degrees of coagulation necrosis observed during histopathologic examination, whereas no viable cells were seen in these zones during the microscopic examination using nicotinamide adenine dinucleotide diaphorase stain. The surrounding hypointense band corresponded to sinusoidal congestion in the acute phase and to fibrotic change in the subacute phase. CONCLUSION: MR imaging using the FLASH sequence can accurately determine the extent of the necrotic area after RF ablation.  相似文献

Forty surgically proved gliomas have been studied by MR: 21 were low-grade gliomas and 19 were anaplastic astrocytomas and glioblastomas. The examination involved the use of T1-weighted (IR and/or SE, TR 500 ms, TE 50 ms) and T2-weighted scans (SE, TR 1000 ms, TE 50 with multiple echos). All patients underwent CT and surgical removal or stereotactic biopsy whose findings were retrospectively compared with those of MR. MR findings were similar in low-grade and anaplastic astrocytomas, but quite different from imaging of glioblastomas. Tumoral cyst and areas of necrosis were recognized on MR studies and confirmed by surgical findings. Differentiation between tumor and oedema was difficult. The MR images did not allow a more specific diagnosis of nature or of malignancy than available with CT up to now, but offer major advantages, such as a superior depiction of tumor extent and anatomic relationships.  相似文献

The value of T2-weighted fast spin-echo imaging of the musculoskeletal system was assessed in 22 patients with various neoplastic, inflammatory, and traumatic disorders. Images were acquired with high echo number (i.e., echo train length) fast spin-echo (FSE; TR 2000 ms, effective TE 100 ms, echo number 13, lineark-space ordering), conventional spin-echo (SE; TR 2000 ms, TE 100 ms) and gradient-echo (GRE) sequences (TR 600 ms, TE 34 ms, flip angle 25°). Signal intensities, signal-to-noise ratios, contrast, contrast-to-noise ratios, lesion conspicuousness, detail perceptibility, and sensitivity towards image artifacts were compared. The high signal intensity of fat on FSE images resulted in a slightly inferior lesion-to-fat contrast on FSE images. However, on the basis of lesion conspicuity, FSE is able to replace time-consuming conventional T2-weighted SE imaging in musculoskeletal MRI. In contrast, GRE images frequently showed superior lesion conspicuity. One minor disadvantage of FSE in our study was the frequent deterioration of image quality by blurring, black band, and rippling artifacts. Some of these artifacts, however, can be prevented using short echo trains and/or short echo spacings.  相似文献

Three cases of lytic, calcified epiphyseal lesions with plain film and computed tomography features suggestive of chondroblastoma were imaged by magnetic resonance imaging. Histopathologic correlation was obtained in each case. Two cases of chondroblastoma showed low signal intensity on both short (TR600/TE20ms) and long (TR2500/TE80ms) spin echo (SE) images. The third case, a clear cell chondrosarcoma, demonstrated increased signal intensity on moderately T2 weighted (TR2500/TE40ms) images. These findings suggest that magnetic resonance imaging may be helpful in distinguishing these lesions.  相似文献

In order to optimize the parameters for the best visualization of the internal architecture of the hyaline articular cartilage a study both ex vivo and in vivo was performed. Accurate T1 and T2 relaxation times of articular cartilage were obtained with a particular mixed sequence and then used for the creation of isocontrast intensity graphs. These graphs subsequently allowed in all pulse sequences (spin echo, SE and gradient time (TR), echo time (TE) and flip angle (FA) for optimization of signal differences between MR cartilage zones. For SE sequences maximum contrast between cartilage zones can be obtained by using a long TR (> 1,500 ms) with a short TE (< 30 ms), whereas for GRE sequences maximum contrast is obtained with th shortest TE (< 15 ms) combined with a relatively long TR (> 400 ms) and an FA greater than 40°. A trilaminar appearance was demonstrated with a superficial and deep hypointense ozne in all sequences and an intermediate zone that was moderately hyperintense on SET1-weighted images, slightly more hyperintense on proton density Rho and SE T2-weighted images and even more hyperintense on GRE images.  相似文献

The magnetic resonance (MR) imaging characteristics of five primary intradural epidermoid tumors are described. At 0.35 T, the most consistent finding on spin echo imaging was a tumor signal intensity that differed from brain and CSF. On T1-weighted images [repetition time (TR) 0.5 s, echo time (TE) 30-40 ms] all tumors exhibited a signal intensity intermediate between brain and CSF. On moderately T2-weighted images (TR 2.0 ms, TE 60-80 ms) the tumor signal intensity was greater than brain and CSF in all cases. The tumor signal consistency was mixed in four of five lesions and homogeneous in one. The tumor margins were well defined in all cases; in three cases the tumor margins were irregular, in one case smooth, and in the last case, one margin was irregular and the remaining margins were smooth. These findings are contrasted with the MR appearance of arachnoid cysts. Using identical imaging factors, four arachnoid cysts were studied and exhibited a homogeneous signal intensity that was identical to CSF on all pulse sequences; their margins were smooth and well defined in each case. These MR findings contribute to the characterization of extraaxial lesions.  相似文献

目的对冰冻膝关节前交叉韧带(ACL)斜冠状面薄层解剖断面与正常人膝关节MRI特点进行对照研究,为ACL损伤分级诊断建立基础。方法1例(1只)膝关节标本行斜冠状面MRI确定角度,冰冻后沿斜冠状面1mm层厚铣切,观察ACL薄层解剖特点。选择50名正常人对其ACL进行MR斜冠状面扫描,观察ACL的MRI特点。结果1只膝关节标本的ACL在斜冠状面薄层断面清晰显示其全程走行,清晰地显示ACL前内束与后外束,前内束从股骨髁附着点后上方区自后向外侧及前内侧走行,止于髁间棘附着区前内侧方,后外侧束从股骨髁附着点前下方区,自后外侧及外下走行,止于髁间棘附着区后外侧方。斜冠状面MRI能够显示50名正常人ACL全程,显示率为100%。MRI能够区分ACL前内束与后外束结构,显示前内束自后外侧向前内走行,止于髁间棘附着区前内侧方,后外束自后外侧向外下走行,止于髁间棘附着区后外侧方。MRI显示ACL走行与薄层断面显示的走行一致。结论斜冠状面是观察ACL的最佳方位,临床对怀疑ACL损伤的患者必要时行MR斜冠状面扫描。  相似文献

Objective: To evaluate the efficacy of direct MR arthrography for diagnosing plica synovialis mediopatellaris (PSM).Material and Methods: Twenty-two knees of 19 patients were examined by direct MR arthrography. In 19 of those knees, field-echo T2*-weighted transaxial images were obtained with intra-articular administration of 40 ml of saline ("direct MR arthrography"). In the other 3 knees, T1-weighted transaxial images were obtained with intra-articular administration of 40 ml of gadopentetate dimeglumine. Conventional MR images were evaluated prior to direct MR arthrography in 12 of the 22 knees. The results of direct MR arthrography and conventional MR imaging were compared with the arthroscopic findings.Results: Direct MR arthrography demonstrated 17 of 19 PSMs proved on arthroscopy. The features of 16 PSMs on direct MR arthrography corresponded accurately with the arthroscopic classification. Conventional MR demonstrated only 3 plicae.Conclusion: Direct MR arthrography enabled accurate diagnosis of PSMs and could replace diagnostic arthroscopy.  相似文献

The purpose of this investigation was to compare gadopentetate and saline as contrast media in MR arthrograms of the glenohumeral joint. In 60 consecutive patients MR arthrograms with either gadopentetate (n = 26) or saline (n = 34) were performed. After injection of gadopentate, 3D gradient-echo (GE) images were obtained (TR 32 ms, TE 10 ms, flip angle 40 °). With saline, double-echo steady-state images (heavily T2-weighted 3D GE images) were obtained (TR 40 ms, TE 9/45 ms, flip angle 40 °). In the last 14 of these patients T2-weighted turbo spin-echo (SE) images were added (TR 2900 ms, TE 96 ms). Contrast-to-noise ratios standardized for imaging times proved to be superior for the gadolinium arthrograms compared with GE and SE saline arthrograms (intra-articular fluid vs subacromial fat: p = 0.0001 and 0.0008; intra-articular fluid vs supraspinatus tendon: p = 0.0001 and 0.046). Using a qualitative scoring system gadolinium arthrograms were superior to saline arthrograms (p < 0.0001 and p < 0.0001). 目的探讨眼眶磁共振扫描成像技术。方法采用GE1.5TsignaEchospeed或GE1.5TsignaTwinspeed,对30例患者做眼眶磁共振成像(MagneticResonanceImaging)。脉冲序列:SET1加权(weightedImagingWI),FSET2加权(weightedImagingWI),T2WI采用脂肪抑制技术或SET1flair,FRFSET2WIT2WI采用脂肪抑制技术。扫描方位:横轴位(Axial)、冠状位(Coronal)、矢状斜位(ObliqueSagittal)和横轴俯卧位(prone)等。扫描参数:Fov18cm×18cm,SET1WI:TR440msTE11ms,FSET2WI:TR3000msTE98ms或T1flair:TR2143ms,TE11ms,TI750ms,FRFSET2WI:TR3500msTE80ms,矩阵256×256,接收带宽15.63MHz,平均激励次数4次,层厚3~4mm,无间隔。增强对比剂用GdDTPA0.1mmolkg。结果横轴位可清晰显示视神经全长及眼内外直肌及与病变的关系。冠状位在同一层面可清晰显示视神经、眼内外直肌、眼上下直肌的断面及与病变的关系。矢状斜位可清晰显示视神经全长、眼上下直肌及与病变的关系。结论磁共振扫描参数的优化组合和选择正确的扫描方位,及T2WI脂肪抑制技术等的应用更有助于显示正常眼眶解剖结构及眼眶病变的定位和诊断。  相似文献

Recent experience has shown that parathyroid adenomas vary in their MR signal intensity, which raises the question of whether the signal intensity is related to different histologic characteristics. In order to address this question, 10 patients who had MR imaging studies (four at 0.35 T, six at 1.5 T) showing large- to medium-sized parathyroid adenomas and who subsequently underwent surgery with histologic proof of the lesion were evaluated. The MR appearance was compared with histologic characteristics. The adenomas were classified into three groups according to the MR appearance: group I, low signal intensity on short TR/TE images, high signal intensity on long TR/TE images (n = 5); group II, low signal intensity on short and long TR/TE images (n = 3); group III, high signal intensity on short and long TR/TE images (n = 2). Histologic analysis revealed that the major features of each group were different. High cellularity without degeneration or fibrosis was observed for all five adenomas from group I. In group II, all three adenomas showed cellular degenerative changes, old hemorrhage with hemosiderin-loaded macrophages, and/or fibrosis. In group III, both adenomas showed evidence of acute hemorrhage without significant degenerative or fibrotic changes. These data suggest that the signal intensity of parathyroid adenomas on T1- and T2-weighted images corresponds at least in part to differences in histologic composition.  相似文献
