In the United States, more than one million children of working mothers are cared for in the homes of nonrelatives. An informal social system exists through which mothers and day care givers find each other, make, maintain, and dissolve their private arrangements, almost entirely without the intervention of any social agency. This paper describes the development of a new kind of day care service using consultation methods for preventive intervention at the neighborhood level. This Day Care Neighbor Service provides consultation to a network of women who perform an informal neighboring role in relation to family day care arrangements. The primary, secondary, and tertiary preventive potentials of this approach are discussed.2680 SW Ravensview Drive, Portland, Ore., 97210, is director of the Day Care Neighbor Service which is a continuing part of the Field Study of the Neighborhood Family Day Care System. Psychiatric day care has a long tradition. However, psychotherapeutic day care institutions specialising in particular disorders or certain therapeutic approaches are still the exception. A day care unit for behaviour therapy was established at the Clinic for Psychiatry and Psychotherapy, University Hospital Hamburg-Eppendorf in 1998 as part of a complex inpatient, day care, and outpatient behaviour therapy unit. The immediate and high acceptance by patients and their doctors indicates a strong need of such a treatment setting. We present how this day care unit works and how it differs from the traditional psychiatric day care.  相似文献

Objectives: The purpose of this study was to understand the degree to which mental health services targeting anxiety and depression disorders are offered by adult day services (ADS) centers in the US. In addition, researchers wanted to determine whether staffing and organizational characteristics are associated with the provision of medication management, individual counseling, and group counseling for participants with anxiety or depression.

Method: Data were drawn from the MetLife National Study of ADS. Hierarchical logistic regression analyses were conducted to determine which staffing and organizational factors were associated with the provision of services to treat anxiety and depression.

Results: Approximately, three in four adult day programs provided medication management for the treatment of anxiety and depression while 38% provided individual counseling and almost 30% group counseling. Programs offering medication management were more likely to have more registered nurse (RN) service hours available per shift and higher costs. Programs that provided individual and group counseling for participants with anxiety or depression were more likely to have more hours of RN and social work services available and a lower percentage of participants who pay privately for services.

Conclusion: The results suggest that ADS are well positioned to act as a platform for delivering mental health care to older persons with anxiety or depression.  相似文献

This study compared Spanish speaking monolingual patients at a Hispanic Clinic, with Hispanics and non-Hispanics at a conventional mental health center. A questionnaire in English and Spanish surveyed sociodemographic characteristics and subjectively reported health needs. The monolingual Hispanic Clinic patients perceived a significantly greater need for help with health and benefits, and better access to physical health care providers. Hispanics seen at the Hispanic Clinic report more difficulties managing medications than the other groups suggesting that this problem is not adequately addressed by receiving services in a culturally sensitive clinic, possibly due to the persistent effect of language barriers and low level of education.
