AIM:To evaluate the effectiveness of a short-type single-balloon-enteroscope(SBE) for endoscopic retrograde cholangiopancreatography(ERCP) in patients with a reconstructed intestine.METHODS:Short-type SBE was developed to perform ERCP in postoperative patients with a reconstructed intestine.Short-type SBE is a direct-viewing endoscope with the following specifications:working length,1520 mm;total length,1840 mm;channel diameter,3.2 mm.In addition,short-type SBE has a water-jet channel.The study group comprised 22 patients who underwent 31 sessions of short-type SBE-assisted ERCP from June 2011 through May 2012.Reconstruction was performed by Billroth-Ⅱ(B-Ⅱ) gastrectomy in 6 patients(8 sessions),Roux-en-Y(R-Y) gastrectomy in 14 patients(21 sessions),and R-Y hepaticojejunostomy in 2 patients(2 sessions).We retrospectively studied the rate of reaching the blind end(papilla of Vater or choledochojejunal anastomosis),mean time required to reach the blind end,diagnostic success rate(defined as the rate of successfully imaging the bile and pancreatic ducts),therapeutic success rate(defined as the rate of successfully completing endoscopic treatment),mean procedure time,and complications.RESULTS:Among the 31 sessions of ERCP,the rate of reaching the blind end was 88% in B-Ⅱ gastrectomy,91% in R-Y gastrectomy,and 100% in R-Y hepaticojejunostomy.The mean time required to reach the papilla was 18.3 min in B-Ⅱ gastrectomy,21.1 min in R-Y gastrectomy,and 32.5 min in R-Y hepaticojejunostomy.The diagnostic success rates in all patients and those with an intact papilla were respectively 86% and 86% in B-Ⅱ gastrectomy,90% and 87% in R-Y gastrectomy,and 100% in R-Y hepaticojejunostomy.The therapeutic success rates in all patients and those with an intact papilla were respectively 100% and 100% in B-Ⅱ gastrectomy,94% and 92% in R-Y gastrectomy,and 100% in R-Y hepaticojejunostomy.Because the channel diameter was 3.2 mm,stone extraction could be performed with a wire-guided basket in 12 sessions,and wireguided intraducta 相似文献

目的分析消化道改道患者通过气囊辅助内镜行经内镜逆行胰胆管造影(ERCP)的疗效和安全性。方法收集2016年4月-2017年10月陆军总医院收治的消化道改道术后行气囊辅助内镜ERCP患者12例(4例胆肠吻合术后,8例胰十二指肠切除术后),4例患者采用双气囊小肠镜,8例患者采用单气囊小肠镜。回顾性分析治疗结果及并发症和术后转归情况。结果 10例患者完成气囊辅助内镜ERCP,总体成功率为83.3%,2例患者系单纯性吻合口狭窄,8例在狭窄的基础上合并肝内胆管结石。2例未完成的患者中1例因胆肠吻合口极度狭窄,导丝未能进入胆管内;1例因R-Y肠肠吻合口角度太小,镜身和套管无法进入输入袢。术后发生黄疸1例,消化道穿孔1例,并发症发生率为16.7%,均经内科保守治疗后痊愈。结论气囊辅助内镜ERCP安全性良好,疗效确切,与传统ERCP相比,成功率较低,有独特的并发症。 相似文献

Endoscopic retrograde cholangiopancreatography (ERCP) remains challenging in patients who have undergone surgical reconstruction of the intestine. Recently, many studies have reported that balloon-enteroscope-assisted ERCP (BEA-ERCP) is a safe and effective procedure. However, further improvements in outcomes and the development of simplified procedures are required. Percutaneous treatment, Laparoscopy-assisted ERCP, endoscopic ultrasound-guided anterograde intervention, and open surgery are effective treatments. However, treatment should be noninvasive, effective, and safe. We believe that these procedures should be performed only in difficult-to-treat patients because of many potential complications. BEA-ERCP still requires high expertise-level techniques and is far from a routinely performed procedure. Various techniques have been proposed to facilitate scope insertion (insertion with percutaneous transhepatic biliary drainage (PTBD) rendezvous technique, Short type single-balloon enteroscopes with passive bending section, Intraluminal injection of indigo carmine, CO2 inflation guidance), cannulation (PTBD or percutaneous transgallbladder drainage rendezvous technique, Dilation using screw drill, Rendezvous technique combining DBE with a cholangioscope, endoscopic ultrasound-guided rendezvous technique), and treatment (overtube-assisted technique, Short type balloon enteroscopes) during BEA-ERCP. The use of these techniques may allow treatment to be performed by BEA-ERCP in many patients. A standard procedure for ERCP yet to be established for patients with a reconstructed intestine. At present, BEA-ERCP is considered the safest and most effective procedure and is therefore likely to be recommended as first-line treatment. Background Prior Billroth II gastrectomy is an important factor presenting difficulties in endoscopic retrograde cholangiopancreatography
(ERCP) administration. We retrospectively evaluated the usefulness and safety of therapeutic ERCP using an anterior oblique-viewing
endoscope for bile duct stones in patients with prior Billroth II gastrectomy.
Methods Forty-three patients with bile duct stones after Billroth II gastrectomy who underwent ERCP from January 1998 to February
2008 were enrolled in this study. We used anterior oblique-viewing endoscopes for all procedures. Endoscopic sphincterotomy
was performed using a needle knife guided by a biliary stent. A total of 808 patients without gastrectomy who had undergone
ERCP for bile duct stones in the same period were reviewed as controls.
Results The success rate of access to the papilla of Vater was 88.4%, and the average time required for such access was 13 min. In
cases of successful access, selective cannulation of the bile duct and complete stone removal were achieved in 94.7% and 94.6%
of patients, respectively. The incidence of complications was 4.7%. As for the success rate of selective cannulation, complete
stone removal ratio, and the incidence of complications, there were no significant differences compared with the control group.
Conclusions Use of an anterior oblique-viewing endoscope enables good success rates in selective cannulation and complete stone removal
to be achieved in patients with prior Billroth II gastrectomy. The safety of therapeutic ERCP for removal of bile duct stones
in those patients is comparable to that in patients with normal anatomy. 相似文献

Although endoscopic retrograde cholangiopancreatography (ERCP) is technically difficult in patients with altered gastrointestinal tract, double‐balloon endoscopy (DBE) allows endoscopic access to pancreato‐biliary system in such patients. Balloon dilation of biliary stricture and extraction of bile duct stones, placement of biliary stent in patients with Roux‐en‐Y or Billroth‐II reconstruction, using DBE have been reported. However, two major technical parts are required for double‐balloon ERCP (DB‐ERCP). One is insertion of DBE and the other is an ERCP‐related procedure. The important point of DBE insertion is a sure approach to the afferent limb with Roux‐en‐Y reconstruction or Braun anastomosis. Short type DBE with working length 152 cm is beneficial for DB‐ERCP because it is short enough for most biliary accessory devices. Background: Endoscopic papillary balloon dilation(EPBD) for common bile duct(CBD) stones removal in Billroth Ⅱ gastrectomy patients is feasible. However, the long-term outcomes of this technique are not clear. The aim of this study was to evaluate the procedural and long-term outcomes of EPBD for removal of CBD stones in Billroth Ⅱ gastrectomy patients.Methods: The records of patients with previous Billroth Ⅱ gastrectomy referred for CBD stones removal with endoscopic retrograde cholangiopancreatography(ERCP) between July 1, 2008 and September 1,2016 were retrospectively reviewed. The main outcomes of stone clearance, ERCP-related adverse events,and stone recurrence were analyzed.Results: A total of 83 patients with previous Billroth Ⅱ gastrectomy underwent ERCP in our center were reviewed. Forty-nine consecutive patients with previous Billroth Ⅱ gastrectomy referred to EPBD for removal of CBD stones underwent 59 ERCP procedures were enrolled in the end. The overall successful CBD stones clearance was achieved in 42 patients(85.7%). ERCP-related adverse events was in 3 ERCP procedures(5.1%). Severe complications, including perforation and bleeding, were not observed. Six of 49 patients(12.2%) had stone recurrence after a median period of22.5 months(range 6–71 months) from the end of stone removal treatment. Female [odds ratio(OR) = 11.352; 95% confidence interval(95% CI): 1.040–123.912; P = 0.046] and previous mechanical lithotripsy(OR = 13.423; 95% CI: 1.070–168.434; P = 0.044) were significantly associated with stone recurrence.Conclusions: At long-term follow-up, EPBD for removal of CBD stones appeared to be safe and effective in Billroth Ⅱ gastrectomy patients. Female and previous mechanical lithotripsy may be risk factors for stone recurrence. 相似文献

Background and Aims: Patients with Billroth II (B‐II) gastrectomy present technical difficulties during endoscopic stone removal due to altered anatomy. Although endoscopic sphincterotomy alone or endoscopic balloon dilation alone has been used for removal of bile duct stones in patients with B‐II gastrectomy, the results are not satisfactory. The aim of this study was to evaluate the efficacy and safety of endoscopic papillary large balloon dilation (EPLBD) for removal of bile duct stones in patients with B‐II gastrectomy. Methods: Twenty‐six patients (20 men and six women; median age 72 years) with bile duct stones and a history of B‐II gastrectomy were enrolled. After cannulation, limited endoscopic sphincterotomy was performed. Then, balloon dilation (balloon size, 10–15 mm) was performed and stones were removed conventionally or via mechanical lithotripsy. Successful stone removal and complications were evaluated. Results: In all cases, stones were successfully removed. The median number of sessions for complete stone removal was one (range 1–3). Stone removal by mechanical lithotripsy was achieved in three patients (11.5%). There were no significant complications, such as bleeding, pancreatitis, or perforation. Conclusions: Endoscopic papillary large balloon dilation is an effective and safe method for removal of bile duct stones. We suggest consideration of this technique for removal of bile duct stones in patients with B‐II gastrectomy. 相似文献

OBJECTIVE : To investigate the risk factors for postoperative pancreatitis following endoscopic retrograde cholangiography (ERC), endoscopic retrograde pancreatography (ERP), endoscopic retrograde cholangiopancreatography (ERCP), endoscopic sphincterotomy (EST) and endoscopic biliary stenting. METHODS : Four hundred and twelve patients referred to the endoscopy unit were divided into seven groups: (i) double ducts (pancreatic duct and biliary duct) contrast media filling group (ERCP group); (ii) biliary duct contrast media filling group (ERC group); (iii) pancreatic duct contrast media filling group (ERP group); (iv) ERCP plus biliary stenting group (ERCP + stent group); (v) ERC plus stenting group (ERC + stent group); (vi) ERCP plus EST and stone extraction (SE) group (ERCP + EST + SE group); and (vii) ERC plus EST and SE group (ERC + EST + SE group). Differences in postoperative serum amylase at 4 and 24 h, as well as clinical symptoms, were compared among the different groups. RESULTS : The incidence of postoperative hyperamylasemia at 4 and 24 h was 17.7 and 4.4%, respectively. The overall incidence of postoperative acute pancreatitis was 3.9% and the ERP group had the highest incidence of postoperative acute pancreatitis among the seven groups. CONCLUSIONS : Repeated pancreatic duct contrast filling during ERCP manipulation is the main risk factor for postoperative pancreatitis and therapeutic ERCP, such as EST, stenting and SE, does not increase the incidence of postoperative pancreatitis. 相似文献

Background. Perforation related to endoscopic retrograde cholangiopancreatography (ERCP) is a rare complication associated with significant morbidity and mortality. This study evaluated the management and outcomes of these perforations. Patients and methods. Between July 1996 and December 2002, a total of 6620 ERCPs were performed at our regional endoscopy unit serving the 1.5 million population of Southern Alberta. Thirty perforations (0.45%) were identified and retrospectively reviewed. Results. Seven of these 30 patients were found to have guidewire perforations of the bile duct, 11 perforations were peri-ampullary, 3 duodenal, 1 esophageal, and 1 patient had a perforation of an afferent limb of a Billroth II anastomosis. In seven patients the location of the perforation could not be determined (unknown). All patients with guidewire perforations were recognized during ERCP, and all were managed medically. Of the 11 peri-ampullary perforations, 7 of these patients had a pre-cut sphincterotomy, 5 underwent surgery and 4 patients died. Delay in diagnosis occurred in all patients that died. Of the three duodenal perforations, all required operation and one patient died. Of the seven ‘unknown’ retroperitoneal perforations, two patients required surgery and there was no mortality. The patients with esophageal and afferent limb perforations both recovered uneventfully after surgery. Most patients who required surgery had retroperitoneal fluid seen on CT scanning. Conclusions. We found that most guidewire perforations can be managed medically with little morbidity. Pre-cut sphincterotomy is a risk factor for perforation. Peri-ampullary and duodenal perforations have a high morbidity and mortality rate. In particular, retroperitoneal fluid collections on CT scans, delay in diagnosis and failure of medical therapy requiring salvage surgery are associated with poor outcomes. Background and Aim: Endoscopic papillary balloon dilatation (EPBD) and endoscopic sphincterotomy (EST) are two common nonsurgical treatments endoscopic retrograde cholangiopancreatography (ERCP) for choledocholithiasis. The aim of this study was to compare the efficacy and safety of EPBD and EST in the treatment for choledocholithiasis, confining the analysis to work reported in the last decade. Methods: The rate of overall postoperative complications was chosen as the primary outcome, and 10 other outcomes were secondary outcomes. Relative risk (RR) or Peto odds ratio (OR) were computed as the measures of pooled effects. We planned sensitivity analyses a priori for examining the change in robustness of the sensitivity to excluding studies with some inappropriate objects, technique defects or without full‐text acquisition. Results: For complete stone removal, EPBD was similar to EST (95% vs. 96%, P = 0.36) and overall postoperative complications (14.0% vs. 11.7%, P = 0.53). The incidence of post‐ERCP cholangitis (2.5% vs. 1.8%, P = 0.40), basket impaction (0.9% vs. 0%, P = 0.16) and perforation (0.0% vs. 0.4%, P = 0.17) were equivalent between EPBD and EST. On the other hand, EPBD caused more post‐ERCP pancreatitis (PEP) (9.4% vs. 3.3%, P < 0.00001), but less hemorrhage (0.1% vs. 4.2%, P < 0.00001). People undergoing EPBD required more use of endoscopic mechanical lithotripsy (35.0% vs. 26.2%, P = 0.0004). The results of sensitivity analyses showed no substantial change. Conclusion: EPBD is comparable to EST for stone extraction, though it requires more endoscopic mechanical lithotripsy (EML). EPBD may outweigh EST for patients with coagulopathy; however, it may cause more PEP. 相似文献

目的探讨肝硬化患者接受经内镜逆行胰胆管造影(ERCP)治疗的安全性。方法回顾性分析2010年1月至2013年11月在北京佑安医院接受ERCP治疗的347例患者的临床资料,将所有患者分为无肝病组(258例)、慢性肝炎组(40例)和肝硬化组(49例)。采用单因素方差分析和Kruskal-Wallis H分析3组患者ERCP术后ALT、AST、白蛋白(Alb)、TBil、白细胞(WBC)、中性粒细胞百分数、血淀粉酶的变化,χ2检验分析ERCP术后高淀粉酶血症、急性胰腺炎、感染、出血和穿孔等并发症发生率和疾病诊断的分布。结果肝硬化组患者术前血清Alb、ALT、AST水平低于无肝病组和慢性肝炎组,差异有统计学意义(F=3.68,P=0.028;H=14.03,P=0.001和H=8.00,P=0.018)。肝硬化组患者术后TBil水平高于其他2组,差异有统计学意义(H=6.69,P=0.035)。44.9%(22/49)的肝硬化组患者术后3 d的AST和TBil高于术前,在3组中的比例最高。肝硬化组患者术后1 d中性粒细胞百分数为(73.9±12.7)%,与无肝病组(74.8±11.0)%相近,高于慢性肝炎组,3组比较差异有统计学意义(H=7.31,P=0.026)。虽然术后3 d中性粒细胞百分数80%的患者的比例在3组中差异无统计学意义,但是肝硬化组患者的比例最高,为18.4%(9/258)。肝硬化组、无肝病组和慢性肝炎组ERCP术后24 h高淀粉酶血症的发生率是分别是53.1%、31.8%和40.0%,差异有统计学意义(χ2=8.48,P=0.014);3组患者ERCP术后总并发症的发生率分别为18.4%、8.1%和7.5%,差异有统计学意义(χ2=26.73,P0.00 1)。结论 ERCP诊治肝硬化患者具有良好的耐受性、安全性和成功率,术后需要警惕高淀粉酶血症。 相似文献

[目的]探讨治疗性内镜逆行胰胆管造影(endoscopic retrograde cholangiopancreatography,ERCP)在高龄患者中应用的有效性和安全性。[方法]将行治疗性ERCP操作的患者169例,按年龄分组,其中≥70岁83例(高龄组),70岁86例(非高龄组);记录2组患者ASA分级、基础疾病、ERCP操作以及并发症情况,对结果进行统计分析、比较2组患者治疗性ERCP的应用情况,[结果]169例患者进行了181次治疗性ERCP操作。2组间例数、性别比例均差异无统计学意义(P0.05),高龄组患者ASA分级、胆道系统结石及胰腺或胆道恶性肿瘤发生率均显著高于非高龄组(P0.05);治疗性ERCP后2组操作成功率、并发症发生率均差异无统计学意义(P0.05)。[结论]高龄组进行治疗性ERCP操作的疗效与非高龄组一致,治疗性ERCP在高龄患者中的应用是安全、有效的。 相似文献
